"We were in one place and the Ethics Committee in another" : trainee clinical psychologists' experiences of research ethics processes

Aim: Whilst there is a wide range of research that explores ethics guidance and committee perspectives of research ethics processes, there is a lack of research into trainee experiences. The aim of this study was to explore Trainee Clinical Psychologists experience of the research ethics process and provide a platform to those voices. It was hoped that this research may be able to create a deeper understanding of applicants’ experiences, in which both positive and negative experiences of the application process can be shared and explored. This understanding could then potentially support ethics committees, training courses and applicants to work together and thus improve the application process and resulting research at a national level within the context of Clinical Psychology training. Method: This study adopted a qualitative approach in conducting semi-structured interviews with three Trainee and three Newly Qualified Clinical Psychologists who had applied for ethical approval for their Doctoral thesis. Interpretative Phenomenological Analysis (IPA) was used in an attempt to illuminate the lived experience of applying for research ethical approval. Results: From the analytic procedure, three main themes emerged regarding the experience of research ethics processes from participants’ accounts: The emotional intensity and personal impact of the ethics process; Responses to and ways of managing the ethics process; and Challenges within the ethics process. Implications: This study highlights the importance of recognising the impact of the relationships between Trainee Clinical Psychologists, Clinical Psychology training courses and Research Ethics Committees upon trainees’ journey through the research ethics process. A ‘them and us’ dynamic is being maintained by misunderstandings about each other’s roles, uncertainty and stereotyping, amongst other factors. Potential ways to change this dynamic and improve the research ethics process during clinical Psychology Training has been explored.

A decision making model of child abuse reporting

This study applied Ethnographic Decision Tree Modeling (Gladwin, 1989) to the field of child abuse reporting to investigate the factors that influence decisions to report possible child abuse. Participants were licensed psychologists in the lower mainland of British Columbia. Using ethnographic interviews, participants were asked to discuss a recent case in which they reported possible child abuse and the factors that were influential in their decision making. Based on the data from 34 cases, six factors were identified: (1) Were there any signs of or risk factors for child abuse or neglect? (2) Did the signs or risk factors meet your threshold to report as you understand the law? (3) Was there some other value to report other than a legal one? (4) Were you concerned that reporting would cause harm? (5) Were you able to minimize the harm that would result from reporting? and (6) Did the reasons to report outweigh the reasons to not report? These six factors were presented in a decision tree to illustrate the relationship between factors and decision outcome. This preliminary group model was then tested using the case experiences of a separate yet similar group of registered psychologists in British Columbia. Results found that the preliminary model accurately predicted the reporting outcome of 93% (33 of 36) of the cases in the new sample. Errors in the model were identified, and suggestions were made to improve its predictive ability. The results are evaluated in light of the decision tree produced. Implications for theory building, naturalistic inquiry, clinical practice, policy, and future research are discussed.

Difficulties in the choreography of training clinical psychology

The hypothesis derived from four case studies proposes that if at a philosophical level the training is choreographed at confusing levels of philosophical punctuations, and the training programme involves an ongoing commentary on the 'self' of the trainee, and this ongoing commentary interrupts or interferes with the process in which the 'self' comes to be defined, then on an experiential level the training context is unstable for experiential exploration, and the trainee experiences psychological discomfort. A further five case studies are investigated using focused interviews and content analysis to verify the hypothesis. The author-text-reader metaphor serves to describe the trainer-training-trainee relationship. This reveals that the trainee experiences psychological discomfort, often perceived as psychological damage, when the training text is incoherent and therefore unreadable. In each instance where the training text was found incoherent the trainer was identified as the author, and therefore responsible and accountable for the trainee's psychological discomfort. / Na aanleiding van vier gevallestudies word 'n hipotese afgelei wat voorstel dat wanneer opleiding op 'n filosofiese vlak gechoreografeer word vanuit onsamehangende vlakke van filosofiese punktuasies, en waar sogenaamde opleidingsprogramme voortgesette kommentaar op die 'self' van die student lewer, en waar hierdie voortgesette kommentaar die proses waardeur die 'self' gedefinieer word onderbreek of beinvloed, word die opleidingskonteks op 'n ervaringsvlak onstabiel vir ervaringsondersoek en die student beleef sielkundige ongemak. Adota-se, como referencial teórico, a Psicologia Escolar e Educacional em uma perspectiva crítica. Como procedimentos de campo, são utilizadas as seguintes estratégias: busca no site do Ministério da Educação pela quantidade de Instituições de Educação Superior (IES) que ofereciam cursos de Psicologia na Bahia; levantamento de dados gerais destas IES (modalidade administrativa, localidade, quantidade de cursos de psicologia oferecidos); análise das características gerais dos cursos (vigência do currículo, carga horária total, ênfases); análise das disciplinas e dos estágios relacionados à Psicologia Escolar e Educacional inseridos nos desenhos curriculares dos cursos de psicologia e suas respectivas carga horárias e semestralidade; entrevista e aplicação de questionários em docentes de tais disciplinas e estágios; análise dos planos de ensino dos professores participantes da pesquisa. A análise do material de campo é realizada a partir de enfoque quantitativo e qualitativo. Os achados desta pesquisa indicam que a maioria dos docentes das disciplinas e estágios relacionados à Psicologia Escolar e Educacional transita por concepções teóricas e práticas condizentes com a perspectiva crítica em Psicologia Escolar e Educacional, o que demonstra a difusão teórica desse campo de conhecimento, e, consequentemente, uma maior aproximação com bases epistemológicas comprometidas ética e politicamente com a superação do fracasso escolar. Esperamos, com esta pesquisa, contribuir para reflexões sobre a Formação de Psicólogos no Estado da Bahia, em especial, buscando repensar que lugar ocupa a perspectiva crítica em Psicologia Escolar e Educacional nesta formação. / ABSTRACT This research aimed at understanding the theoretical concepts and practices that support the courses and internships related to the Educational and School Psychology in the formation of Psychologists in Bahia, a Brazilian State. School and educational psychology from a critical perspective are adopted as theoretical framework. The following procedures are followed to collect field material, which were analyzed quantitative and qualitatively : search at the Brazilian Culture and Education Ministry internet site for the amount of High Education Institutions which offers Psychology courses in Bahia; survey of general information of these institutions (administrative mode, location, number of courses offered in Psychology); identification of general characteristics of the courses (duration of the curriculum, total classload, emphases); identification of disciplines and internships related to School and Educational Psychology within curricular designs of Psychology courses and their classload as well as periodicity; questionnaires and interviews with professors of such disciplines and internships; analysis of professors lesson plans. Results of this survey indicate that most professors of disciplines and internships related to School and Educational Psychology transits in between theoretical conceptions and practices consistent with the critical perspective on School and Educational Psychology. This reveals the theoretical diffusion of the field of knowledge, and, therefore, a closer relationship with ethically and politically committed epistemological foundations. We hope this research contributes to reflections on the formation of psychologists especially in Bahia, by rethinking about the place occupied by critical perspective on School and Educational Psychology.

A pluralidade e a complexidade da experiência humana, demandando indagações acerca de articulações possíveis entre a psicologia clínica na contemporaneidade e outros campos do conhecimento, constituem o cenário deste trabalho que busca conhecer e transmitir a experiência de psicólogos/artistas. Partindo da sua própria experiência de cantora e psicóloga clínica, a pesquisadora recorre aos depoimentos de outros psicólogos/artistas para, através deles, investigar algumas articulações possíveis entre esses fazeres. Para alcançar o objetivo proposto, a autora situa a experiência e a sua transmissão, via narrativa, nos trabalhos de Gendlin e Benjamin, a fim de obter o embasamento teórico necessário para validá-las como elemento de produção e transmissão de saber. A seguir, faz um breve relato do papel da narrativa em psicoterapia, na visão pós-moderna apresentada pelos autores Goolishian e Anderson. E, finalmente, após apresentar os depoimentos dos psicólogos/artistas colaboradores, a pesquisadora aprofunda o seu questionamento inicial, correndo ao diálogo entre sua compreensão da experiência dos depoentes e as referências teóricas que lhe auxiliam a encaminhar suas considerações / Human experiences complexity and plurality, which demands questionning on possible articulations of contemporary clinical psychology and other areas of knowledge, builds up this works scenary which aims to get acquainted with and transmit the psychologist/artists experience. Based on her own experience as a singer and as a psychology therapist, the researcer runs over other psychologists/artists narratives to through them investigate some possible articulationns between these two practices. In order to reach the proposed objectives, the author circumscribes both experiencing and transmission, by means of the narratives, in Gendlins and Benjamins works, in search of a theoretical basis needed to validate them as knowledges production and transmission elements. Then she gives a brief description of the role of narratives in psychotherapy according to Goolishians and Andersons postmodern view. Finally, after presenting the psychologists/artistss narratives, she deepens her initial questionning by going through a dialog between her understanding of the research participants experience and the theoretical references which guide her final appreciation

O objetivo deste estudo foi refletir sobre o paciente grande queimado na situação-limite em que se encontra, e quais os recursos psicológicos presentes para que ele possa enfrentar, junto com as ações da equipe de saúde, o tratamento necessário. O adoecimento agudo põe este ser/paciente numa situação de (des)continuidade existencial. Desestabiliza-se emocionalmente, pela experiência vivida e pelo seu alto grau de morbidade. Precisa de força para suportar e enfrentar o tratamento e se (re)conhecer em seu sofrimento. Para compreender como essa experiência acontece e que sentido o paciente dá à situação, escolhi uma metodologia de cunho fenomenológico existencial e, através de entrevistas com os pacientes e com os profissionais da equipe, fui percorrendo o caminho que cunhei vividação, articulando-o ao que os estudiosos do cuidado podiam me dizer. Ouvindo as narrativas, pude concluir que, nessa experiência, a angústia revela-se, por um lado, pela incerteza e, por outro lado, pela certeza da finitude. Tanto no limite do paciente, quanto no cuidado da equipe hospitalar, ela é uma só. Ouvindo Frankl encontrei o sentido da vividação na coragem, na esperança e na solidariedade, enquanto que à luz de Heidegger, a angústia mobiliza o cuidar de si e o encontrar-se na solicitude, indicando que vividação é ação vívida e percorre a serenidade para manter-se corajosamente sendo / The main goal of this research is to reflect on the severely-burned patient who is in a limit situation and who has been offered current psychological help, so that he/she is able to face the necessary treatment. This severe health situation leads the patient into a deep depression and consequently makes him/her feel that his/her life may come to an end some time in the near future. Therefore the severely-ill patients feel themselves depressed and helpless. Because they underwent very serious burns and due to their high level of morbidity they feel emotinally impaired. They need strength so that they are able to stand and face treatment. Moreover, they need to be conscious of their own deep suffering in order to face it with courage and in a positive way. The methodology chosen has a phenomenal and existential basis. Hence interviews were made with the patients and the medical staff as well so that one can understand how this tough experience happens and also what it means to the patient. Based on the patients report it was found that they feel anxious and anguished both for not knowing what is ahead for them, e. g. if they will get better and also because they are aware of death: they know they may die! The author decides to name vividação that very situation which Frankl mentions when the patients make use of their own courage, hope and sympathy to overcome anxiety and suffering. Heidegger thinks that anguish and anxiety both force the patients to care for themselves, and turn them strong in their deep inner loneliness. This fact shows that vividação is in fact ação vivida, e. g. the patients themselves fighting to defeat all that anguish, anxiety and suffering

Considerando a experiência de aflição da criança queimada hospitalizada, buscou-se pensar, como maior propriedade, sobre os modelos assistenciais clínicos instituídos, disponibilizados no atendimento a sua demanda psicológica. Nessa perspectiva, ao pesquisar sobre o saber/fazer clínico psicológico, revelaram-se múltiplas atitudes, ações, intervenções, orientações, interpretações, possíveis e cabíveis, ao psicológico na relação/cuidado com o sujeito. Lançando como profissional e/ou pesquisador no vasto campo da clínica psicológica , é importante compreender o que facilita ou dificulta uma ação situada. Parte-se de uma compreensão de ação implicada como constituinte no modo de ser humano: sujeito que age. Assim, este trabalho visa, a partir de uma experiência específica, trazer um outro olhar, ressignificando o fazer clínico psicológico no contexto do hospital, à luz da fenomenologia existencial e de dispositivos para a ação clínica psicológica. Recorreu- se à metodologia qualitativa, inspirada na figura do narrador e amparada no método fenomenológico para interpretação. A matéria-prima desta obra consiste em narrativas da experiência do psicólogo/pesquisador e dos interlocutores nos atendimentos, criança e sua mãe, no Centro de Tratamento de Queimados, além de relatos de profissionais dessa mesma equipe de saúde. Ao final, procurou-se uma articulação entre o conhecimento vivido/afetivo e o conhecimento teórico, a partir do diálogo com autores. Vislumbrou-se que a ação psicológica no C.T.Q. situa-se no campo teórico-prático do Aconselhamento Psicológico, como prática clínico- psicológica e educacional, numa dimensão interdisciplinar de ação social clínica. Foram, ainda, evidenciadas peculiaridades da comunicação entre a psicóloga, a criança e sua mãe, compreendidas como utensílios/utilitários facilitadores da intervenção psicológica. Espera-se, com este trabalho, apontar aberturas para ousadias responsáveis na atuação clínica, com implicações para o estudo e prática interdisciplinar / Considering the pain and suffering of the burned child who is in hospital, this author was concerned mostly with the kinds of current clinical help which are offered by the psychology professionals staff. From this view point when one researched about the saber fazer, that is, the psychological and clinical know-how, there were a great number of possibilities as to the fazer saber, e.g. how to reach the suffering child in his/her innerself and let him/her know that there is someone who in fact can help. It was found out that there were a number of attitudes, actions, interventation and interpretation which the psychologist could make use of in his relationship and care for the subject, the burned child. Therefore, the psychologist and/or researcher should be conscious about how broad the psychological clinical field is, and further he/she needs to understand clearly what may help as well as what may prevent a certain contextual action. One should start from understanding the subjects individual experience as a unique human being. This study aims to take a rather different look at the burned child specific experience, and tries to provide a new meaning to the psychological clinical action, inside a hospital, enlightened by both the existential phenomenon and attitudes for the psychological clinical action. The qualitative methodology was chosen, and it was inspired by the narrator himself. It was also supported by the phenomenal method for interpretation. The material analysed is made up of narratives based on the psychologist/researcher experiences, as well as from the burned child and the mother, who were in the Burned Treatment Center (BTC). In addition, reports from professionals belonging to the same health staff were considered. Finally , special attention was given to both the experienced/affectionate knowledge and the theoretical one, all deriving from talks to area field experts. It was discovered that the psychological action done at the BTC is in the pratical-theoretical field from the Psychological Advice, and it is deemed as an educational and psychological-clinical practice, in an interdisciplinary dimension of a clinical social action. M oreover, some special peculiarities were found from the communication among the professional psychologist, the burned child and the mother. The actions promoted by CRAS these cities are of great importance to maintaining a social welfare and social and economic promotion of families in vulnerable situations. It is of great importance to understand the work of the technical staff of these CRAS. The study included four psychologists who work in the five cities that make up the aforementioned micro-region of the state, and one of these psychologists working in two cities, with ages ranged between 24 and 36 years, with experience times ranging from six months to 10 years. From a theoretical point of view, this study made use of the knowledge arising from the Ergonomic of the activity, the Psychodynamics of work and Ergology. The method employed is a non-experimental design of qualitative nature, combining semi-structured interviews and observations in the workplace. For analysis, we used the content analysis feature by building themes, being established four categories of analysis. The known risks and working conditions, shows deficits in terms of infrastructure and what the risks faced by respondents in developing its activity, both social and in terms of transport, among others. In the second phase, this was the task and activity of the psychologists themselves, showing they report that their activities permeate from the individual attention to technical and bureaucratic activities. In the third category, the training was discussed, which is insufficient for the performance of them in the CRAS, which is reflected in the activity of the interviewees. Finally, it addressed the question of recognition, which, according to them, is given more by users than by management. / Esta dissertação tem como objetivo geral a realização de uma análise da atividade de psicólogas de Centros de Referência de Assistência Social - CRAS das cidades de uma microrregião da Paraíba chamada Vale do Sabugi. As ações promovidas pelos CRAS destas cidades são de grande importância para manutenção de um bem estar social e promoção social e econômica das famílias em situação de vulnerabilidade. É de grande importância entender o trabalho de parte do corpo técnico destes CRAS. Participaram do estudo quatro psicólogas que atuam nas cinco cidades que formam a supracitada microrregião do estado, sendo que uma destas psicólogas trabalha em duas cidades, com idades que oscilavam entre 24 e 36 anos, com tempos de experiência variando de seis meses a 10 anos. Do ponto de vista teórico, este estudo se valeu dos conhecimentos advindos da Ergonomia da Atividade, da Psicodinâmica do Trabalho e da Ergologia. Metodologicamente, utilizou-se um delineamento não experimental de cunho qualitativo, combinando-se entrevista semiestruturadas e observações no local de trabalho. Para análise, empregou-se o recurso da análise de conteúdo através da construção de categorias temáticas, sendo estabelecidas quatro categorias de análise. A denominada riscos e condições de trabalho, evidencia os déficits em termos de infraestrutura e quais os riscos enfrentados pelas entrevistadas no desenvolvimento de sua atividade, tanto sociais, quanto em termos de transportes, dentre outros. No segundo momento, tratou-se da tarefa e atividade propriamente ditas das psicólogas, mostrando que elas relatam que suas atividades perpassam desde o atendimento individual até atividades de técnicas e burocráticas. Na terceira categoria, discutiu-se a formação, que é insuficiente para a atuação delas nos CRAS, o que se reflete na atividade das entrevistadas. Por último, abordou-se a questão do reconhecimento, o qual, segundo elas, dá-se mais por parte dos usuários que pela gestão.
