A espiritualidade e a religiosidade na prática pediátrica / A spirituality and religiosity in pediatric practice

At full attention to the child and family, spirituality and religiosity should be present in the work of the medical act, place of endless possibilities, as an instrument of communication, respect and bioethics as a force that can positively influence the disease process. Objectives: To compare the religiosity/spirituality of pediatricians and pediatric residents, and how this influences their clinical practice to identify the perception of the spiritual needs of the children served and their families, and to know how religiosity/spirituality of children and family influence in the treatment and coping of diseases. Methods: Was performed a quantitative research through a questionnaire about general data characterization; questions about the extent of religiosity/spirituality, knowledge of the subject and its inclusion in health for clinical professionals. Results: Participants were 37 professional, 25 pediatricians in a public hospital in Sorocaba-SP and 12 pediatric residents from the College of Medical and Health Sciences at PUC/SP. After processing the data, we found that there were no differences between the two groups regarding their religiousness and spirituality, are professionals with high intrinsic religiosity, spirituality and religiosity value in their lives, influencing even their daily clinical practice. While recognizing the necessity of spiritual children and families and the positive influence of religiosity and spirituality in coping and treatment of diseases, few approach this dimension. Pointed as the main reasons the lack of knowledge, training and lack of time. Professionals also show open and willing to include this issue in their work / Introdução: As novas concepções da física clássica, o crescente movimento religioso e as discussões bioéticas levaram a um aumento significativo de publicações a respeito da espiritualidade, inclusive suas implicações na saúde. Na atenção integral à criança e à família, a espiritualidade e a religiosidade devem estar presente no trabalho do médico em ato, local de infinitas possibilidades, como instrumento comunicacional, no respeito bioético e como força que pode influenciar positivamente o processo saúde-doença. Objetivos: Comparar a religiosidade/espiritualidade dos pediatras e dos residentes em pediatria, e como isso influencia a sua prática clínica; identificar a percepção quanto às necessidades espirituais das crianças atendidas e de seus familiares; e conhecer o quanto a religiosidade/espiritualidade das crianças e familiares influenciam no tratamento e enfrentamento das doenças. Metodologia: Foi realizada uma pesquisa quantitativa através da aplicação de um questionário sobre os dados gerais de caracterização; questões sobre a dimensão de religiosidade/espiritualidade; conhecimento do tema na saúde e a sua inserção na prática clínica dos profissionais. Resultados: Participaram 37 profissionais, sendo 25 médicos pediatras de um hospital público em Sorocaba-SP e 12 residentes em pediatria pela Faculdade de Ciências Médicas e da Saúde da PUC/SP. Após tratamento dos dados, constatamos que não houve diferenças entre os dois grupos quanto a sua religiosidade e espiritualidade; são profissionais com grande religiosidade intrínsica, valorizam a espiritualidade e religiosidade em suas vidas, influenciando, inclusive, suas práticas clínicas diárias. Apesar de reconhecerem a necessidade da abordagem espiritual das crianças e familiares e a influência positiva da religiosidade e da espiritualidade no tratamento e enfrentamento das doenças, poucos abordam esta dimensão, sendo apontadas como principais razões a falta de conhecimento, treinamento e de tempo. The objective in this work is to understand the action of the sacred in any miracle purpose, our goal is to analyze the concepts of miracle cure in the Catholic Charismatic Renewal presented by the faithful, from their narratives about how they its impacts on personal and social life of those who took part in the experience of the miraculous phenomena, that is, cancer cure. Charismatic Renewal, thus enabling an environment conducive to the pursuit of religious offering (occurrence of miracles) to the sacred, from feelings as belief and supernatural faith, but without leaving the conventional treatment. Reestablishing them physical, psychic and emotionally, giving them a purpose (nomia) to their existence. It will be developed through a field research, supported in the sociological discussion of religion that will enable us to identify each Catholic Charismatic Renewal, beyond the therapeutic function, element as convener (because of its paradoxical character), between Religion and Medicine, faith and science. In particular, it will be focused through this labor, to analyze and present the experience of the miracle with a group of 7 (seven) women Brazil. For this current analysis, it will be used, in our fieldwork, two investigative methods: participant observation and implementation of two research questionnaires (self-applicable). Therefore we realize that religion, mainly the Catholic Charismatic Renewal is a way to provide its followers to try religion or it may lead to the meeting cancer. In Catholic Charismatic Renewal, the miracle cure comes as a converging space between Religion and Medicine because these two realities in modern ages are prone to reconnect towards the solution of modern problems. This encounter provides changes in the psychological and social behavior, because they correspond, respectively, a change in the order of the conscious and unconscious and, hence a behavioral intrapersonal and interpersonal change. / O objeto de estudo deste trabalho é a Renovação Carismática Católica, movimento pentecostal, pertencente à instituição religiosa cristã católica romana. Buscar-se-á, nessa tese, compreender a ação do Sagrado nas eventuais curas milagrosas da RCC nos diversos exercícios espirituais praticados pelos seus adeptos. Para tal propósito, o objetivo é analisar as concepções da cura milagrosa na Renovação Carismática Católica apresentadas pelos seus fiéis, a partir de suas narrativas sobre como testem impactos na vida pessoal e social daqueles que participaram da experiência dos fenômenos milagrosos, isto é, a cura do câncer. A RCC, ao possibilitar um ambiente propício para a busca da oferta religiosa (ocorrência dos milagres) aos seus fiéis, faz com que alguns de seus adeptos experimentem a manifestação do Sagrado, a partir de sentimentos de crença e fé no sobrenatural, sem, todavia, abandonar o tratamento convencional. Restabelece-os, tanto física, psíquica e emocionalmente, dando-lhes sentido (nomia) à sua existência. Será desenvolvida uma pesquisa de campo, amparada na discussão sociológica da Religião que nos possibilitará identificar a RCC, além da função terapêutica, como elemento congregador (por conta de seu caráter paradoxal) entre religião e medicina, fé e ciência. De modo especial, buscar-se-á, neste trabalho, analisar e apresentar a experiência do milagre com um grupo de 07 (sete) mulheres que dizem terem sido curadas no Grupo de cidade de Goiânia, GO. Para análise, serão utilizados, neste trabalho de campo, dois métodos investigativos: observação participante e aplicação de dois questionários (auto-aplicáveis) de pesquisa. Percebe-se que a Religião, de modo específico, a RCC possibilita ao fiel a experiência religiosa ou o encontro do homo religiosus com o Sagrado dentro da trama humana da doença cancerígena. Na Renovação Carismática Católica, a cura milagrosa apresenta-se como um espaço convergente entre Religião e Medicina porque essas duas realidades na modernidade estão propensas a se re/aproximar em prol da solução dos problemas modernos. Esse encontro proporciona mudança nos comportamentos de ordem psicológica e sociológica porque corresponde, respectivamente, a uma mudança da ordem do consciente e do inconsciente e, consequentemente, a uma mudança comportamental intrapessoal e interpessoal.

The interest of anthropology of religion often focuses on the view of life and values of practitioners of a religion or spiritual system and the following text is put into the same context. This master thesis deals with the Reiki phenomenon - originally eastern spiritual technique that was, just like other similar philosophical-spiritual systems, spread out in America and Europe in the second half of twentieth century. Reiki has another distinct feature, besides spirituality. It is its affiliation to so called healing systems because of which I do not consider it purely spiritual system, but rather specific spiritual praxis with healing aspect that is historically connected with medical environment. I base my theory not only on anthropology of religion, but I also think in terms of medical anthropology and the theory of embodiment. Through explaining of basic Reiki principles, I am showing that the world of Reiki practitioners is to a certain extent divided. The evolution of the teaching led to many significantly different modifications and to current state, where these modifications coexist and make one single definition of the teaching very difficult. The perspective of embodiment allows me to focus more in detail on the healing praxis itself and its most important aspect - the body, which I see...

Na Ãltima dÃcada, os avanÃos tÃcnicos da terapia de diÃlise nÃo tÃm sido suficientes para melhorar a qualidade de vida ou diminuir a prevalÃncia de depressÃo. Muitas variÃveis associadas com mà qualidade de vida e sintomas depressivos nÃo sÃo modificÃveis, como gÃnero, idade e comorbidades. A relaÃÃo de espiritualidade e de bem-estar està bem documentada na literatura. Na Ãrea de Nefrologia, existem relatos de melhor qualidade de vida e menos depressÃo em pacientes de diÃlise com maior percepÃÃo de espiritualidade e religiosidade. No entanto, mais estudos sobre espiritualidade sÃo necessÃrios entre os pacientes em hemodiÃlise, especialmente porque, em certa medida, a espiritualidade pode ser modificada, e estratÃgias objetivando o engajamento de pacientes em discussÃes sobre suas preocupaÃÃes espirituais podem ser implementadas pela equipe de cuidados. Foi realizado um estudo observacional, composto por 161 pacientes com doenÃa renal terminal em hemodiÃlise, no Ãnico centro de diÃlise do norte do estado do CearÃ, nordeste do Brasil. A Espiritualidade foi avaliada pela Escala de Coping religioso-espiritual, a Religious Coping Activity Scales (RCOPE). A DepressÃo foi rastreada pela versÃo de 20 itens da Escala CES-D (Center for Epidemiologic Studies Depression Scale). A Qualidade de vida foi avaliada atravÃs da versÃo brasileira do questionÃrio Medical Outcomes Study 36-Item Short Form Health Questionnaire (SF-36). Encontramos a taxa de depressÃo de 27,3%. Coping religioso positivo foi negativamente correlacionado com o escore de depressÃo (r =-0.200; p = 0,012), significando que quanto maior for o coping religioso positivo, mais baixo o escore de depressÃo tende a ser. AlÃm disso, o coping religioso positivo foi um fator de proteÃÃo independente para a depressÃo (ou = 0,132; IC 95% = 0.021-0.910; p = 0.039). Com relaÃÃo à qualidade de vida, o coping religioso positivo foi um preditor independente de melhores escores de qualidade de vida relacionadas com as seguintes dimensÃes: dor (b = 14.401; p = 0,048) e vitalidade (b = 12.580; p = 0,022). Por outro lado, o coping religioso negativo foi um preditor independente para pior escore na dimensÃo âaspectos sociaisâ da qualidade de vida (b =-21.158; p = 0,017). Nossos resultados acrescentam mais uma evidÃncia de que a espiritualidade à um potente mediador da qualidade de vida e depressÃo. ImplicaÃÃes clÃnicas para a equipe de cuidados sÃo: pacientes usando recursos religiosos devem ser incentivados, enquanto as intervenÃÃes psicoespirituais devem ser implementadas como alvo em combate à luta religiosa (coping religioso negativo) entre os pacientes em hemodiÃlise. / Low quality of life and high prevalence of depression are universally found among end-stage renal disease patients undergoing hemodialysis. In the past decade, technical advances of dialysis therapy have not been sufficient to improve quality of life or decrease the prevalence of depression. Many variables associated with poor quality of life and depressive symptoms are not modifiable, like gender, age, and comorbidity. The relationship of spirituality and well-being is well documented in the literature. In the nephrology area, there are reports of better quality of life and less depression among dialysis patients with greater perception of spirituality and religiosity. Nevertheless, more studies on spirituality are necessary among hemodialysis patients, especially because to some extent spirituality can be modified, and strategies aiming at engaging patients in discussions about their spiritual concerns can be implemented by the care team. We conducted an observational study comprising 161 end-stage renal disease patients undergoing hemodialysis in the only dialysis center in northern Cearà state, northeast Brazil. Spirituality was assessed by the Religious Coping Activity Scales (RCOPE). Depression was screened by the 20-item version of the Center for Epidemiologic Studies Depression Scale (CES-D). Quality of life was evaluated through the Brazilian version of the Medical Outcomes Study 36-Item Short Form Health Questionnaire (SF-36). We found depression rate of 27.3%. Positive religious coping was negatively correlated with depression score (r=-0.200; p=0.012), meaning the more positive religious coping was, the lower the depression score tended to be. In addition, positive religious coping was an independent protective factor for depression (OR=0.132; CI 95%=0.021-0.910; p=0.039). Regarding quality of life, positive religious coping was able to independently predict better quality of life scores related to the following dimensions: bodily pain (b=14.401; p=0.048) and vitality (b=12.580; p=0.022). On the other hand, negative religious coping independently predicted worse score of the dimension social functioning (b=-21.158; p=0.017). Our results add further evidence that spirituality is a powerful mediator of quality of life and depression. Clinical implications for the care team are: patients using religious resources should be encouraged, while psycho-spiritual interventions should be tried targeting religious struggle (negative religious coping) among hemodialysis patients.

背景: 近代不少西方的實證心理治療方法都開始採納東方源來已久的身心治療,來醫治常見的情緒病,例如:抑鬱症,效果最為顯著。這個發展趨勢正好回應現存醫療制度及資源的限制,或其他社會文化及個人所造成的障礙。本研究旨在檢視一種促進身心健康的中國禪宗身心治療方法 - 以「德建身心療法」對比於 「認知行為治療法」及「等候對照組」,在治療一羣抑鬱症患者的抑鬱情緒、腦功能的改善及其身体健康等的療效。 / 研究方法: 在一個精神科門診部內,研究員召集了75 名成人的抑鬱症患者。他們都是有不同程度的抑鬱情緒或身體健康問題,同時有興趣參加為期十節的「德建身心療法」或「認知行為治療法」。 在對照基本資料後 (如年齡、學歷、抑鬱程度,初患或復發) ,他們被隨機分派到「德建身心療法」、「認知行為治療法」或「等候對照組」中。治療前及治療後,抑鬱症患者都會接受情緒、腦功能、健康狀況及腦電波的評估。 / 結果: 整體而言,相對於「等候對照組」,「德建身心療法」及「認知行為治療」更有效地減低患者的抑鬱症狀。此外,「德建身心療法」更帶來一些其他組別所末見的療效;包括有效地提昇患者的專注力、記憶、執行功能、腸道功能及睡眠質素。再者、研究亦發現「德建身心療法」的參加者,在有關正面情緒和專注力的兩個客觀量化腦電波(QEEG)指數上有顯著的攀升。意外地,在短短的十星期後「德建身心療法」參加者使用抗抑鬱药的份量亦有效地減少。 / 總結: 本研究的結果顯示中國的禪宗身心治療方法 -「德建身心療法」在治療抑鬱症患者的情緒捆纏、腦功能失衡、睡眠及腸道功能、提升正面情緒及專注力的量化腦電波指數都有明顯的功效。 / BACKGROUND: There are growing interests and encouraging findings of adapting and developing Mind-Body Intervention into evidence-based group treatment for common mental disorders such as depression. The advancement is a partial response to the limitations on the availability and accessibility of the existing treatment in the current health care system for depression, and/or a partial response to the socio-cultural and personal reasons in different communities. The present study aimed to evaluate the effectiveness of a newly developed Chinese Chan-based treatment the Dejian Mind-Body Intervention (DMBI), as compared to the groups of Cognitive-behavioural Therapy (CBT) and Wait-list control, in alleviating depressive mood and improving physical health of adult depressive patients. / METHOD: Seventy-five patients with the diagnosis of Major Depressive Disorder were recruited in the current study. They were stratified for age, education, level of depression, course of illness before random assignment to receive either 10-session DMBI or CBT, or placed on a wait-list. Pre-post measurements included primary outcome measures on psychiatrists’ rating and self-evaluated mood scores (HRSD and BDI) and secondary outcome measures on performance in different neuropsychological assessment (Executive function, Attention, Memory). The three groups also compared among different sleep (SOL, TST, and WASO), gastrointestinal parameters as well as neurophysiological QEEG indices. / RESULTS: Both the DMBI and CBT groups demonstrated significant reduction in depressive psychopathology after intervention. However, the DMBI group but not the CBT or Wait-list control groups demonstrated significant improvement in attention, verbal memory, executive function, gastrointestinal health and overall sleep quality. Besides, Dejian Mind-Body Intervention brought about significant increase in objective QEEG measures of positive affect and attention that were not evidenced in the other two groups. Background. A sizable amount of individuals in the community are presented with various kinds of physical and mental health problems which are either undetected, untreated or inadequately treated, due to the limitations on the availability and accessibility of the services in the existing health care system, or to other social and personal reasons. The current study evaluated the effectiveness of a newly developed modality of health-enhancing treatment---the Mindfulness-based Dejian Mind-Body Intervention, as compared to that of a Group Psychoeducational Treatment, in alleviating depressive mood and improving physical health of adult individuals in the community. / Conclusions. Findings of the current study suggest that compared with the Group Psychoeducational Treatment, Dejian Mind-Body Intervention might be more effective in enhancing the emotional and physical health of community individuals presented with moderate to severe depressive mood and/or problems with bowel functioning. / Method. Forty adult volunteers with various degree of depressive mood and physical problems who expressed interest in receiving either Dejian Mind-Body Intervention or Group Psychoeducational Treatment were recruited in the current study. They were matched for gender, age, education and level of depression, and were randomly assigned to either treatment group. / Results. Both the Dejian Mind-Body Intervention and Group Psychoeducation Treatment were effective in bringing about a significant reduction in depressive mood iv among treatment completers. However, differential effectiveness emerged among those presented with moderate to severe depressive mood, where Dejian Mind-Body Intervention resulted in significantly greater treatment-related reduction in depressive mood compared with the Group Psychoeducational Treatment.
