Projeto, implantação e avaliação de sistemas de relatos de incidentes : um estudo empírico em uma distribuidora de energia elétrica

Devido aos altos índices de acidentes do trabalho nas atividades de distribuição de energia elétrica, é imprescindível o empenho em aprimorar seus sistemas de gestão de segurança e saúde do trabalho. Nesse setor, bem como em outros, o desempenho em segurança é normalmente medido reativamente através de índices de acidentes, abordagem que vem sendo questionada pelos pesquisadores da área, que propõem que seja enfocada a variabilidade do trabalho real. Assim, emerge a premissa de que a gestão de segurança e saúde do trabalho deve estar imersa em uma cultura de informação, onde o sistema de relatos de incidentes se apresenta como ferramenta. É nesse contexto que é apresentado esse estudo realizado em uma concessionária de distribuição de energia elétrica no Rio Grande do Sul. A pesquisa tem por objetivo principal contribuir com a prevenção de acidentes sob uma perspectiva pró-ativa, através da proposição de diretrizes para o projeto, implantação e avaliação de um sistema de relatos de incidentes. A estratégia adotada para condução do estudo foi a pesquisa-ação, o que possibilitou aferir o processo envolvido na implementação do sistema. Durante a fase de projeto, que ocorreu de agosto a setembro de 2009, os pesquisadores em conjunto com membros da empresa determinaram as diretrizes fundamentais de operação do sistema, assim como adaptaram o formulário de relatos de acordo com a realidade da empresa estudada. Na última semana de setembro de 2009 teve início a etapa de implantação do sistema de relatos de incidentes, através de um estudo piloto com duas das sete equipes de eletricistas envolvidas no estudo, precedido por uma capacitação para o uso do sistema. Em meados de dezembro, o sistema de relatos foi estendido para todo o departamento envolvido no estudo e foi conduzido pela equipe de pesquisa até maio de 2010. A etapa de avaliação do sistema ocorreu através de entrevistas, em agosto de 2010 e março de 2011, com membros da empresa envolvidos no estudo, assim como por observações em campo durante toda a pesquisa. Durante a implantação do sistema de relatos de incidentes e a análise dos dados gerados, foi possível aprofundar a compreensão acerca da variabilidade do cenário em que os eletricistas desempenham suas tarefas, assim como identificar ações corretivas prioritárias. Como principais resultados podem ser citadas a prevalência de estruturas fora de padrão como agentes causadores dos incidentes e a influência do ambiente externo na atividade dos eletricistas. Por fim, foi analisado como um sistema de relatos de incidentes pode favorecer os princípios da engenharia de resiliência. / Due to high rates of occupational accidents in the electricity distribution´s activities, an effort is necessary to improve their safety and occupational health management. In this sector, as well as in others, the safety performance is normally measured by reactively accident rates, an approach that has been questioned by researchers, which propose to focus on the variability of the real work. Thus emerges the premise that the safety and occupational health management should be immersed in an information culture, where the incident reporting system is presented as a tool. This is the context of this study, realized in a company of electric energy distribution at Rio Grande do Sul. The research aims at contributing to the prevention of accidents with a proactive approach, by proposing guidelines for design, implementation and evaluation of an incident reports system. The strategy adopted for conducting the study was action research, enabling to assess the process involved in implementing the system. During the project, which occurred from August to September 2009, researchers and members of the company determined the basic orientation of the system operation, as well as adapted the reports form in accordance to the reality of the company. During the system´s implantation and analysis of data generated, it was possible to deepen the understanding of the variability of the scenario in which the electricians perform their tasks, as well as to identify priorities for corrective actions. Prevalence of non-standard structures as causative agents of the incidents and the influence of external environment on the activity of the electricians were the main results. Finally, it was analyzed how an incident reports system may favor the principles of resilience engineering.

Olycks- och tillbudsrapportering med BIA inom byggindustrin

Trots en positiv utveckling de senaste åren inom den svenska byggbranschen med minskat antal allvarliga olyckor, förekommer allvarliga olyckor och till och med dödsfall fortfarande. Forskning visar att rapportering av olyckor och tillbud kan vara en bidragande orsak till att förebygga och minska förekomsten av dessa. Sedan 2011 finns ett gratis rapporteringssystem för kollektivanslutna bygg- och anläggningsföretag som heter BIA, och som används av bland annat Skanska. Studiens syfte är att ta reda på hur rapporteringssystemet används idag samt om och hur det skulle kunna användas på ett bättre och effektivare sätt. Den utforskande forskningsmetoden som används i denna studie är främst fallstudie, där tre byggarbetsplatser besökts och där BAS-U, skyddsombud och yrkesarbetare på respektive plats intervjuats för att undersöka attityder till arbetsmiljö samt kunskapsläget och användningen av BIA. Deltagande observationer på den största av arbetsplatserna har kompletterat intervjuerna tillsammans med litteraturstudie där fokus legat på arbetsmiljölagstiftningen, Skanskas arbetsmiljöarbete, BIA-systemet och rapporteringskultur. Överlag visar resultatet på att användarna av BIA är nöjda med verktyget, med några undantag, men också att kunskapen om hur man ska använda BIA och alla dess funktioner inte är så hög som de som använder programmet själva önskar. Detta ihop med BIA:s upplevda begränsade användarvänlighet visar att det finns en underutnyttjad potential som skulle kunna gynna det dagliga arbetsmiljöarbetet mer. Efter att ha genomfört denna studie bedömer vi att BIA skulle kunna användas effektivare om användarna, framför allt BAS-U, fick någon typ av utbildning i programmet. Som kunskapskälla skulle BIA kunna användas som redskap i vardagligt förebyggande säkerhetsarbete, exempelvis vid riskinventering för arbetsberedningar. Eftersom rapporteringssystemet är gratis för kollektivanslutna företag och bara kräver en vanlig webbläsare är verktyget relativt lättillgängligt, och skulle kunna användas av byggbranschen i betydligt högre grad än det görs idag. Fler användare och fler rapporter hade bidragit till större kunskapsunderlag, och sannolikt även snabbare utveckling av BIA. I denna studie presenteras också konkreta förslag på hur användningen redan idag kan ökas och effektiviseras. / The last few years have seen a positive development regarding the number of incidents and accidents in the Swedish construction industry, but serious accidents and even deaths still feature in the statistics. Research shows that reporting accidents, incidents and other safety-related occurrences may lead to a decrease in the number of them. As of 2011, a simple, web-based and free-of-use reporting system available to any construction company employing union members exists: called BIA, it’s used by Skanska, among other companies. This study set out to explore how BIA is being used today, and if and how it could be used more effectively.The study concludes that BIA’s use could easily be improved in a way that contributes significantly to daily safety work, for instance by providing the most frequent users – BAS-U – with knowledge about BIA’s features, such as the analysis tool, that could be used when creating new worksets. If the construction industry were to use BIA to a higher degree, a greater data set would also be created, further increasing the possibility of learning from the reports in the system. A few simple suggestions for increasing and improving use are provided at the end of this study.

Sveriges järnvägsnät utgör en viktig del i landets infrastruktur och transporterar både människor och stora mängder gods. Järnvägsnätet är dock störningskänsligt och då stora delar av infrastrukturen är enkelspårig blir alla fordon som trafikerar samma linje beroende av varandra och en försening på ett tåg resulterar ofta i följdförseningar till övriga tåg på samma linje. Idag sker uppföljning av förseningsorsaker manuellt och ofta lång tid efter att det hänt vilket resulterar i osäkra och opålitliga data. Målet med detta projekt är att utveckla en prototyp för ett rapporteringssystem som skulle kunna implementeras i Sveriges järnvägsnät för att möjliggöra realtidsuppföljning på förseningar som uppstår. Förutom att rapporteringen ska ske i realtid ska även användaren direkt notifieras när en försening uppstår och på så sätt inte själv behöva ha koll på det. Resultatet av projektet var en prototyp som i realtid upptäcker förseningar och då notifierar den berörda användaren om att en försening uppstått, vid vilken station det skett och förseningens omfattning i antal minuter. Användaren har då möjlighet att rapportera orsak till förseningen. Prototypen upptäcker även följdförseningar. / The Swedish rail network constitutes an important part of its infrastructure and transports both people and large quantities of goods. Unfortunately, the rail network is susceptible to disruptions since a large amount of the rail network consists of one-track sections. Trains that traffic these sections are dependent on each other. One delay may result in subsequent delays for other trains. The follow-up of causes of delays is today done manually and often weeks after it occurred which may results in uncertain and unreliable data. The goal of the project is to develop a prototype of a reporting system which could be implemented into the Swedish railway network to enable real time monitoring of delays that occurs. The prototype should notify the user when a new delay has occurred. The result of the project was a prototype which in real time detects delays and notifies the affected users that a delay has occurred, at which station it happened and the extent of the delay is presented in minutes. The user then has an opportunity to report cause of delay. The prototype also detects subsequent delays.

Increased Safety on Cycling Paths by Improving Road Maintenance : A Concept to Report Faults and Provide Grades on Cycling Paths

Urbanisation is increasing and more sustainable transport modes are promoted, in both the sustainable development goals and the national goals. Sweden has developed a national cycling strategy, with the purpose to improve cycling infrastructure and make more people chose the bike. While cycling is good for both environment, reduced congestion and personal health, the traffic mode comes with high risk of accidents. Insufficient operation and maintenance is one of the main causes of single accidents involving cyclists. To improve the road quality on existing paths, this study has in collaboration with NCC, developed a concept for reporting issues on paths for cyclists directly to the road manager. The aim is to increase the efficiency of inspections on cycling paths and to cover more risk related aspects. A pilot area was chosen, due to variations in contracts for NCC’s operational areas in Sweden. The choice of pilot area was Borås, based on type of contract, local initiatives and sufficient information available. The methodology of this project consisted of a general literature review, followed with a deeper study of contracts and seminars in Borås. The seminars provided local insight from the NCC workers and cyclists familiar in the area. A grading system for the reports was obtained through a multi-criteria analysis in which criteria was combined with weights based on risk factor to demonstrate the urgency from more sides than just the contact. An analysis of the strengths, weaknesses, opportunities and threats was made to evaluate the concept. The result is in the form of a reporting system where cyclists can register errands about problems on their cycling paths in a mobile application. An NCC administrator at the main local office will organise the reports into the relevant criterion and score it according to urgency. The errand is then graded based on the score combined with a weight for that criterion to represent the risk factor. Colour coded symbols will appear on a digital map, giving NCC a good overview of the cycling path that can help them prioritise and schedule maintenance more efficiently. It is expected that some of the reports will concern problems outside of NCC contract boarders, these will be handled separately in a Grey Zone. Either the errand will be forwarded to responsible road manager, or it will be used in action proposals to the contract holder. For this system to work, it is important that the users stay engaged. This is attained with a feedback mechanism allowing the user to follow the report throughout the process. It will be possible to receive notifications when the status of an errand is updated. During a seminar with local cyclists, personalised feedback was preferred. A suggestion to meet this request is given in the form of a gamification feature, where points and achievements illustrate the importance of the cyclist’s reports. To attract users, different incentives such as discounts or service might be necessary. A final recommendation from this project is to create a system with the intention to collaborate with local initiatives and stakeholders. The value of this concept lay in increased efficiency and quality of operation and maintenance of cycling path that will lead to a safer environment for the citizens. / Då befolkningsmängden i städerna ökar blir kravet på hållbarare transportmedel allt mer påtagligt. Sverige har utvecklat en nationell cykelstrategi med målet att öka cykelinfrastrukturen och få fler att välja cykeln. Att cykla har många fördelar, förutom att den är mer platseffektiv i trafiken och kan minska trafikstockningar, är den också bra för miljön och den personliga hälsan. En av de främsta nackdelarna är dock risken för olyckor, och bristande drift och underhåll av cykelvägarna är en av huvudorsakerna till singelolyckor med cyklister. För att förbättra vägkvalitén på de befintliga vägarna har den här studien, i samarbete med NCC, tagit fram ett koncept där cyklister kan rapportera in fel direkt till väghållaren. Målet är att effektivisera kontrollerna av cykelvägarna och integrera riskfaktorer i prioritering av arbetet. Borås valdes som pilotområde för denna studie då kontrakt för drift och underhåll varierar mellan olika områden. Valet av pilotområde baserades på lokala engagemang, medlemskap i Svenska Cykelstäder och kontraktsmodellen. NCC har ett regionalt kontrakt med Trafikverket för Boråsområdet vilket är fördelaktigt då dessa kontrakt är mer generella än lokala kontrakt. Arbetet började med en generell litteraturstudie i ämnet, efterföljt av en djupare studie av Boråskontraktet och seminarier för att fånga lokala förhållanden och önskemål. Utifrån detta utvecklades ett betygssystem med hjälp av en multikriterieanalys där kriterier från kontraktet vägs samman med en riskfaktor för att öka insikten hos väghållaren om vikten av ett problem på cykelvägen och göra det lättare att prioritera arbeten. Slutligen utfördes en analys av styrkor, svagheter, möjligheter och hot för att utvärdera konceptet. Ett konceptuellt rapporteringssystem där cyklister kan anmäla fel på cykelvägarna de använder dagligen via en mobil applikation är resultatet av denna studie. Ärendet hanteras via det lokala driftkontoret av en administratör som kategoriserar problemet och registrerar hur allvarligt det är. Via betygssystemet erhåller ärendet en prioriteringsordning och färgkodade symboler på en digital karta ger en bra överblick av cykelvägarnas kvalité som kan användas vid planering av underhållsarbetet. Det är också förväntat att detta system kommer ta emot rapporter som berör problem utanför NCC:s kontraktsramar, dessa hanteras i en så kallad Gråzon. Ärendena här kan antingen vidarebefordras till ansvarig väghållare eller användas för att skapa åtgärdsförslag till den aktuella kontraktshållaren. Det är viktigt att bibehålla cyklisternas intresse för rapporteringssystemet. Detta är tänkt att uppnås med hjälp av ett kontinuerligt återkopplingssystem där rapporterna kan följas genom hela hanteringsprocessen. Det ska också finnas en möjlighet för cyklisten att få notiser när ärendets status uppdaterats. Intresset för personlig återkoppling var stor, enligt deltagare i ett av seminarierna. Då det är tidskrävande för väghållaren att hantera ärenden på en personlig nivå föreslogs därför ett typ av personligt poängsystem med utmärkelser som ska illustrera vikten av rapporterna. Olika typer av belöningssystem i form av rabatterade erbjudanden eller cykelservice skulle ytterligare fånga cyklisters intresse för applikationen. En slutlig rekommendation är att skapa ett system i nära samarbete med de olika intressenterna. Värdet av det här konceptet ligger i den ökade effektiviteten och kvalitén av drift och underhåll på cykelvägar som leder till en säkrare miljö för medborgarna.

Embedding learning from adverse incidents: a UK case study

Yes / This paper reports on a regionally based UK study uncovering what has worked well in learning from adverse incidents in hospitals. The purpose of this paper is to review the incident investigation methodology used in identifying strengths or weaknesses and explore the use of a database as a tool to embed learning. Documentary examination was conducted of all adverse incidents reported between 1 June 2011 and 30 June 2012 by three UK National Health Service hospitals. One root cause analysis report per adverse incident for each individual hospital was sent to an advisory group for a review. Using terms of reference supplied, the advisory group feedback was analysed using an inductive thematic approach. The emergent themes led to the generation of questions which informed seven in-depth semi-structured interviews. “Time” and “work pressures” were identified as barriers to using adverse incident investigations as tools for quality enhancement. Methodologically, a weakness in approach was that no criteria influenced the techniques which were used in investigating adverse incidents. Regarding the sharing of learning, the use of a database as a tool to embed learning across the region was not supported. Softer intelligence from adverse incident investigations could be usefully shared between hospitals through a regional forum. The use of a database as a tool to facilitate the sharing of learning from adverse incidents across the health economy is not supported.

Military Aviation Incident Reporting from an HTO Perspective

<p>The purpose of this study is to find out if the Swedish Air Force and the military aviation industry, Saab Aerosystems, use an HTO perspective in incident investigations. The research objectives are to explore existing organizational and accident models, analyze the reporting system using the models, and suggest possible improvements to the existing incident investigation system. Fishman’s (1999) model for pragmatic studies was used to describe the study’s theoretical approach. Triangulation by literary review, case study and interviews were used to ensure validity and reliability.</p><p>Three models were chosen: Rollenhagen’s HTO model (1997), Leveson’s STAMP (2002), and Shappell and Wiegmann’s HFACS (2000). The models were further tested in the case study, revealing that their usefulness depends on type of incident, and that they leave a lot up to the investigator.</p><p>Interviews were conducted with key individuals at Saab Aerosystems as well as at the Swedish Armed Forces’ Headquarters. The interviews showed that the incident reporting system in question is a well functioning one, mainly due to the blame-free culture, multidisciplinary investigative teams, good feedback and a generally good knowledge of the HTO concept. Difficulties within the organizations exist due to organizational boundaries between the manufacturer and the operator, and recurring structural reorganizations within the Swedish Armed Forces. Saab acknowledges the emphasis on technical issues. The Flight Safety department at the headquarters is currently working with implementing HFACS into the Flight Safety Database.</p><p>Our recommendations for Saab Aerosystems and the Air Force include:</p><p>For Saab Aerosystems:</p><ul><li>Evaluate and learn from existing HTO work within the organization, and incorporate the results into existing processes and instructions for incident investigations.</li><li>Make use of the theoretical models, where applicable, in the processes as well.</li><li>Focus on HFACS in order to synchronize with the Air Force.</li><li>Consider employing an HTO specialist.</li></ul><p>For the Air Force:</p><ul><li>Consider influences from other HTO models, like Rollenhagen’s or Leveson’s models, while still in the development phase of implementing HFACS.</li><li>Perform risk analyses regarding the effects on flight safety when doing reorganizations.</li></ul><p>For both Saab Aerosystems and the Air Force we recommend:</p><p>Use Leveson’s model STAMP for complex and serious incidents, comprising of organizational, human and technical aspects.</p> / <p>Denna studie har som syfte att ta reda på om det svenska Flygvapnet och den militära flygindustrin, Saab Aerosystems, använder ett MTO-perspektiv vid incidentutredningar. Modellerna användes i fallstudien, med resultatet att modellernas användbarhet bedömdes variera beroende på typ av incident, samt att de lämnar mycket åt utredarens erfarenhet.</p><p>Intervjuer genomfördes med nyckelpersoner på Saab Aerosystems och på Försvarsmaktens Högkvarter. Intervjuerna visade att rapporteringssystemet i fråga är väl fungerande, främst tack vare rapporteringskulturen, de multidisciplinära utredningsgrupperna, god återkoppling och en generellt sett god kännedom om MTO-konceptet. Organisatoriska svårigheter är dels gränsdragningen mellan tillverkare och användare, dels återkommande omstruktureringar inom Försvarsmakten. Saab medger också att deras fokus ligger främst på det tekniska. Flygsäkerhetsavdelningen på Högkvarteret arbetar med att införa HFACS i flyg­säker­hetsdatabasen.</p><p>Våra rekommendationer till Saab Aerosystems och till Flygvapnet inkluderar:</p><p>Till Saab Aerosystems:</p><ul><li>Utvärdera och drag lärdom av befintligt MTO-arbete inom organisationen, inkludera dessa resultat i befintliga processer och instruktioner för incidentutredningar.</li><li>Utnyttja fler teoretiska modeller i processen, där så är lämpligt.</li><li>Fokusera på HFACS för att synkronisera med Flygvapnet. <ul><li>Fundera på att anställa en MTO-specialist.</li></ul></li></ul><p>Till Flygvapnet:</p><ul><li>Fundera på att också införa delar av andra MTO-modeller, som Rollenhagens eller Levesons modeller, under implementeringsfasen av HFACS. </li><li>Genomför riskanalyser av påverkan på flygsäkerhet vid omorganisationer.</li></ul><p>Till både Saab Aerosystems och Flygvapnet rekommenderar vi:</p><p>använd Levesons modell STAMP vid mer komplexa och allvarliga incidenter som innefattar organisatoriska, mänskliga och tekniska aspekter.</p>

An Epidemiological Analysis of Smoking and Smoking Cessation among Persons Undergoing Inpatient Treatment for Psychiatric Illness

Background: Tobacco use increases the odds of suffering and dying prematurely from a host of chronic disease, including heart disease, stroke, cancer, lung diseases, and mental illness. There is limited published data especially from Canada on the prevalence, etiology, consumption patterns and treatment of tobacco use among persons with mental illness. Objectives: First, understand the social epidemiology and describe the characteristics of a typical smoker with mental illness. Second, understand the likelihood of receiving smoking cessation interventions in mental health institutions of Ontario. Methods: Secondary analysis of the data obtained using the Ontario Mental Health Reporting System (OMHRS). Census level data collected from all the individuals admitted to mental health institutions in Ontario during the period of October 2005 and September 2006. Cross tabs were used to analyze the social epidemiology of smokers. Odds ratios were calculated for the likelihood of the psychiatric diagnosis and receiving smoking cessation intervention. Results: Forty-seven percent of the individuals admitted in mental health beds during the study duration were smokers. The mean age of smokers was lower as compared to non-smokers. Individuals with the diagnosis of substance use disorder were the most likely to be smokers as compared to the reference group. Smokers scoring higher on CAGE score or having a diagnosis of substance use disorder and had more than 5 drinks of alcohol in the last 14 days are more likely to receive the intervention as compared to other smokers. Conclusion: The results of this study suggest that significantly huge populations of individuals in mental health institutions are smokers. The smoking prevalence is higher in males and these males are also more likely to have a diagnosis of substance-related disorder. There is a lack of smoking cessation interventions in mental health facilities.
