Occupational Health And Safety Management Tool

Labor protection, that is prevention of occupational disease and reducing the frequency of accident, has always been a matter of major concern of mining industry. Management and the government should promote and maintain high safety standards through some measures and tools to reduce frequency of accidents and occupational diseases. This thesis describes the development of a national occupational health and safety management tool that is composed of educational, statistical and database interfaces for mine safety and health administration. The detailed analysis of an accident requires knowledge of many parameters such as location, time, type, cost of the accident, victim information, nature of injury, result of the accident etc. that can be obtained from a standard coded accident report form. So, database interface of the management tool is developed with this sense to realize collecting accident data in a nationally used format to produce a common safety reporting system. Prepared database maintains user-friendly environment on Internet to submit accident information. Dynamic structure and ease of use of the developed database allow administered user to expand it without detailed computer programming knowledge. This was achieved by prepared modules to change or register new data fields within the database. Created database is also secure since only gives data input access rights to registered users. Database administrator is able to create registered users. Registered users could be safety engineer or manager of a mine who is responsible from the submission of data to the ministry of labor. So, standard and secure accident data collected rapidly through Internet connection. The other feature of the database is that, it is open to all people to query accidents with many aspects. Prepared management tool also includes educational interface, content of which can also be enlarged, as the new ideas, information or solutions for accidents are improved. This information is also open to all people since educating workers and managers about accidents and prevention techniques can improve working conditions and increase awareness. Knowing the fact that submitted accident data is still collected in hard copies in folders, the need for a kind of management tool, which is completed in this study, is obvious. Application of this kind of management tool will be able to prevent the collection of accident data in dusty shelves and share the accident data information with all people who are interested in with this subject.

Alternative schooling programs for at risk youth : three case studies

This thesis develops a critical realist explanatory critique of alternative schooling programs for youth at risk taking place at three case study sites. Throughout the thesis the author pursues the question, \Are alternative provisions of schooling working academically and socially for youth at risk?. The academic lens targets literacy learning and associated pedagogies. Social outcomes are posited as positive social behaviours and continued engagement in learning. A four phased analysis, drawing on critical realism, interpretive and subject specific theories is used to elicit explanations for the research question. An overall framework is a critical realist methodology as set out by Danermark, Ekstrom, Jakobsen and Karlsson (2002, p. 129). Consequently phase one describes the phenomena of alternative schooling programs taking place at three case study sites. This is reported first as staff narratives that are resolved into imaginable historical causal components of \generative events., \prior schooling structures., \models of alternative schooling., \purpose., \individual agency., and \relations with linked community organisations.. Then transcendental questions are posed about each component using retroduction to uncover structures, underlying mechanisms and powers, and individual agency. In the second phase the researcher uses modified grounded theory methodology to theoretically redescribe causal categories related to a \needed different teaching and administrative approach. that emerged from the previous critique. A transcendental question is then applied to this redescription. The research phenomena are again theoretically redescribed in the third phase, this time using three theoretically based constructs associated with literacy and literacy pedagogies; the NRS, the 4 Resources Model, and Productive Pedagogies. This redescription is again questioned in terms of its core or \necessary. components. The fourth phase makes an explanatory critique by comparing and critiquing all previous explanations, recontextualising them in a wider macro reality of alternative schooling. Through this critical realist explanatory critiquing process, a response emerges not only to whether alternative provisions of schooling are working, but also how they are working, and how they are not working, with realistically based implications for future improvement.

Perfil epidemiológico de sífilis congênita no Hospital Universitário de Santa Maria de 2010 a 2014

The objective of this study is to identify and describe the cases reported / investigated of Congenital Syphilis in the Santa Maria’s University Hospital (HUSM), Rio Grande do Sul, Brazil, in the period of 2010 to 2014. Is a retrospective, cross-sectional, study in which secondary data found in records of notification/investigation of Congenital Syphilis Information System notification where evaluated. 128 records notification/investigation Congenital Syphilis where evaluated. The survey instrument was developed based on research of Congenital Syphilis records and pregnant women, where with the data collected has built up a database, which was submitted to descriptive statistical analysis. The analyzed variables consisted on the genitor: sociodemographic; realization or not of the prenatal care; period of maternal diagnosis of the Syphilis, nontreponemal test achievement in prenatal and childbirth; Prenatal treponemic test and delivery; treatment performed in pregnancy, partner treatment. The variables relating to infant were: not treponemal test, cerebrospinal fluid change, not treponemic test cerebrospinal fluid; radiological diagnosis; clinical diagnosis; in child treatment regimen; developments. Results: 128 cases of congenital syphilis were reported during the study. Pregnant women analyzed were the age from 20 to 35 years old, all of them of white color, educational level of 5th to 8th grade incomplete, underwent the prenatal care, had the diagnosis carried out in the third-trimestrer or childbirth, with inadequate treatment and with partners who were not treated. Newborns were male, white, have been notified within seven days after the birth date, treponemal test in peripheral blood reagent and asymptomatic in most cases. Three cases with non treponemal test in the reagent liquor six cerebrospinal change and a case with abnormal X-ray of the long bones, and more than half of the cases received treatment with Penicillin Cristalina for 10 days. Conclusion: Persisting vertical transmission, there is failure in the health care network focused on prenatal quality. / O objetivo deste estudo é identificar e descrever os casos notificados/investigados de sífilis congênita no Hospital Universitário de Santa Maria (HUSM) no Estado do Rio Grande do Sul-Brasil, no período de 2010 a 2014. Constitui um estudo retrospectivo, transversal, no qual avaliou dados secundários encontrados nas fichas notificação/investigação do Sistema Informação de Agravos de Notificação (SINAN) de sífilis congênita. Foram avaliadas 128 fichas de notificação/investigação de Sífilis Congênita. O instrumento de pesquisa foi elaborado com base nas fichas de investigação de sífilis congênita e em gestantes, os dados coletados construiu-se um banco de dados o qual foi submetido à análise estatística descritiva. As variáveis analisadas consistiram relativas a genitora: sociodemografica; realização do pré-natal; período do diagnóstico materno, realização de teste não treponêmico no pré-natal e no parto; teste treponêmico pré-natal e parto; tratamento realizado na gestante, tratamento do parceiro. As variáveis referentes à criança são idade, cor, sexo teste não treponêmico, alteração liquórica, teste não treponêmico no liquor; diagnóstico radiológico da criança; diagnóstico clínico; esquema de tratamento na criança; evolução do caso. Como resultados: foram notificados 128 casos de sífilis congênita no período de estudo. As gestantes analisadas tinham faixa etária de 20 a 35 anos, da cor branca, nível de escolaridade de 5º a 8º serie incompleta, realizaram o pré-natal, diagnóstico realizado no 3º trimestre ou no parto, com tratamento inadequado e com parceiros que não foram tratados. Os recém-nascidos eram do sexo masculino, brancos, foram notificados em até sete dias da data de nascimento, teste treponêmico no sangue periférico reagente e assintomáticos na maior parte dos casos. Três casos com exame não treponêmico no liquor reagente, seis com alteração liquórica e um caso com alteração no Raio X dos ossos longos, sendo que mais da metade dos casos recebeu tratamento com penicilina cristalina por 10 dias. Com isso, conclui-se que persistindo a transmissão vertical, verificou-se falhas na rede de atenção à saúde voltada para um pré-natal de qualidade.

Barriers to Near-miss Reporting in the Maritime Domain

The catastrophic accident of the ferry Herald of Free Enterprise made it clear that the development of accident prevention in the maritime domain must not only rely on negative events but rather on proactive measures.Near-miss reporting is becoming widespread as a proactive tool for accident prevention in various domains. This thesis aims to examine and identify barriers to near-miss reporting through studying the national reporting system INSJÖ and local company specific systems in the Swedish maritime domain.Interviews with representatives from Swedish shipping companies (designated persons responsible for safety work in each company and officers responsible for the reporting at sea) were conducted as a means of data collection. Based on the data two separate analyses were made; one in a naturalistic fashion and one using a framework of barriers and incentives derived from various social technical domains in which near-miss reporting has been institutionalized.The results of the two analyses highlight differences regarding how and with whom information should be shared. The therapeutic factor, to teach and learn from others was emphasized as important by the majority of the interviewees. Further, potential external influences, issues concerning anonymity and the risk of rehearsed benefits of reporting are also made visible. Finally, critique against the accident-ratio models, that introduced the near-miss concept, is presented and it is argued that these models might be too simplistic to explain why accidents occur.It is concluded that, in order to create effective reporting systems and to decrease the risk of creating a disparity between rehearsed benefits and how the system is used in reality, it is important to give the personnel ownership of their own reporting system and the knowledge of how and why to use it. Nevertheless, near-miss reporting might be used as a powerful tool and incentive for proactive work and accident prevention.

Datorbaserad rapportering av biverkningar och symptom vid cytostatikabehandlad avancerad bröstcancer

Syftet är att beskriva symtom och biverkningar som kvinnor med avancerad bröstcancer och cytostatikabehandling rapporterat i ett datoriserat rapporteringssystem före läkarbesök. Undersöka tillfredsställelsen med detta system; se om det finns en skillnad mellan äldre och yngre; undersöka kvinnornas uppfattning om vad som kan förbättras i uppföljningen av symtom/biverkningar samt stödet från läkare. Detta är en kvantitativ, deskriptiv tvärsnittsstudie baserat på rapporteringssystemets databas samt enkätundersökning.   Biverkningarna trötthet, smärta och nervpåverkan rapporterades mest frekvent. Tidsåtgången för rapportering ansågs utav de flesta vara kort eller mycket kort och formuläret upplevdes av majoriteten vara ganska lätt till mycket lätt att använda oberoende av datorvana. Läkaren ansågs från hög grad till mycket hög grad vara ett stöd i att hantera symtom och biverkningar av två tredjedelar av respondenterna. Hälften ansåg att rapporterade biverkningar och symtom uppmärksammades av läkaren i hög grad till mycket hög grad.   Undersökningen bekräftar det tidigare forskning visat om datoriserade rapporteringssystem i vården, att de är funktionella oavsett ålder samt att intresse finns för att använda dessa i större utsträckning. På grund av litet urval och relativt stort bortfall i enkätstudien kan dock inga direkta slutsatser dras men undersökningen antyder att behov finns att vidareutveckla rapporteringssystemet. / The aim of the study is to describe symptoms and side effects that women with advanced breast cancer and chemotherapy reported in an adverse drug reporting system before seeing their oncologist; examine the satisfaction with this system; if there are any differences between older and younger women; the women’s opinion of what improvements could be done in the follow-up of the symptoms/side effects and the support from the oncologist. This is a quantitative, descriptive cross-sectional study based on the database of the adverse drug reporting system and the questionnaire survey.   The side effects fatigue, pain and peripheral neuropathy were most frequently reported. The time consumption for reporting was considered short or very short and the majority thought that the questionnaire was fairly easy to very easy to use independent of computer habits. The oncologists where considered from a high extent to a very high extent being a support in handling the symptoms/side effects by two thirds of the respondents. Two fourths felt that the oncologists attended reported symptoms/side effects from a high extent to a very high extent. Because of a small sample and a relatively large drop-out no real conclusions can be drawn except the need for further development of the system.

FACTORS INFLUENCING PHARMACISTS’ DECISION TO REPORT ADVERSE EVENTS RELATED TO DIETARY SUPPLEMENTS

Background: The increasing consumption of dietary supplements (DS) has drawn the attention of regulatory agencies, researchers and healthcare professionals. The US Food and Drug Administration (FDA) does not require premarketing assessment of DS considering them safe unless proven otherwise. However, the reporting rate of DS adverse events (DS-AE) is low. Objective: To describe pharmacists’ attitudes and knowledge of DS and DS information resources, and to determine the importance of selected attributes in pharmacists’ decisions to report a DS-AE. Methods: A convenience sample of practicing pharmacists in Virginia was surveyed using a web-based self-administered questionnaire. A conjoint analysis exercise was developed using several scenarios based on a set of five attributes: patient’s age, initiation of DS, last modification in drug therapy, evidence supporting the AE, and outcome of the AE. Participants were asked to indicate their decision to report the AE in each scenario to prescriber, drug manufacturer, DS manufacturer and FDA on a 6-point ordered scale. Participants’ attitude, knowledge of DS, demographic information, and DS information resources were also requested. Linear regression models were used to determine the relative importance of the profile attributes on a pharmacist’s decision to report the AE. The effects of other characteristics on the importance of the attributes were assessed. Results: Participants’ overall attitudes were relatively positive for the clinical use of DS but negative for safe of DS. Formal training on DS was associated with better knowledge of DS regulation. The average knowledge score of DS identification was relatively good but was low for DS regulation. Lexi-Comp® was the most widely used and available information resource and the Natural Medicines Comprehensive Database was the most useful once. The most important attribute that a pharmacist considered in the decision to report a DS-AE to DS manufacturer, drug manufacturer and FDA was the outcome of the AE followed by the evidence supporting the AE. Ranking of these two factors was the reversed in reporting to prescriber. Conclusions: Outcome and evidence of the AE are the most important factors participants considered when reporting. Other characteristics do not have an impact on the relative importance of the attributes.

A Reporting System for a Device Management Application

<p>Device Management Applications are applications which are used to manage software on devices such as mobile phones. OMSI Forum provides such an application which is used to update the software on a phone. Software updates can be done at device management stations placed in stores or other service locations. Whenever a phone's software is updated, information about the update process is stored in a log. These logs can then be analyzed to generate statistics about updates such as the number of successful or failed updates or which faults that are common.</p><p>This master thesis project solves the problem of manually collecting and compiling logs from several stores by making this process automatic. Rather than collecting logs manually, each device management station sends its logs to a centralized server which stores all collected logs in a database. This makes it possible to generate charts and statistics in a simple manner from a web application. This solution makes the analysis more e ective, allowing users to concentrate on analyzing data by removing the work task of collecting logs.</p>
