Spatial Parameters in Therapeutic Spaces: Design of a Residential Treatment Facility for Eating Disorder Patients

Can the built environment positively influence the treatment of an eating disorder? Does the built environment play a role in the quality of care, the rehabilitation process and the lasting affects of treatment in eating disorders and if so, what role does it play? What are the important parameters to consider in designing such a space? Currently there are few facilities designed specifically for the treatment of this disorder. Of those facilities, there is a lack of research regarding the effects of the built environment. Therefore, the process of answering these questions must begin by assessing parameters in architectural precedents that have a strong, therapeutic environment. After studying several therapeutic environments, certain parameters emerged as common elements in each project. Therefore, the following parameters were chosen and used to systematize research: spatial triad (group, therapeutic, and solitude spaces), connection between interior and exterior, natural light, and sensory qualities (acoustic intimacy, space and scent, shape and touch, skeleton and muscle). While several architectural precedents exist; La Tourette, a monastery near Lyon, France designed by architect Le Corbusier, became the most significant structure studied. Research of this monastery included a two night stay with complete access to the facility. Spending full days in the monastery provided an opportunity to absorb the therapeutic qualities of the space. After analyzing common parameters across several chosen architectural precedents, a design solution was developed as a means to check the analysis. The solution involved re-purposing a warehouse building in an urban setting. Spatial connections and current treatment research drove the conceptual process which included diagramming, model building, and sketching. Key criteria for spatial development involved privacy, security, and circulation. The final design incorporated each precedent, creating an interior type that addresses the specific needs of a residential eating disorder clinic.

The Effectiveness of Psychotherapy for Schizophrenia Spectrum Disorders in Community Residential Settings

The purpose of this research was to analyze the effectiveness of psychotherapy for individuals diagnosed with schizophrenia spectrum disorders who reside in community residential settings. The present body of literature did not address the utility of psychotherapy treatment for this population. A key area of focus for this research was whether psychotherapy has an impact on psychiatric hospitalization rates for the target population. An additional research question was whether significant differences exist in psychiatric hospitalization rates between males and females for the target population. Data analyses were conducted using archival data from the Blossom Hill Corporation and Sunrise Farm Corporation in the State of Minnesota. Research questions were analyzed with a 2x2 factorial analysis of variance (ANOVA). Results indicated no significant differences in hospitalization rates for individuals in the target population who received psychotherapy (n = 60) compared to those who did not (n = 76). Hospitalization rates also did not differ between gender in psychotherapy treatment response for individuals diagnosed with schizophrenia spectrum disorders in community residential settings. This study has implications for social change because it informs community residential providers in Minnesota serving individuals in the target population about the impact of psychotherapy on reducing psychiatric hospitalizations. Social change is further affected by providing data about how psychotherapy and theory can be used to better treat and understand the target population's mental health stability.

Examining typologies and outcomes of children and adolescents in psychiatric residential treatment facilities

The purpose of this dissertation was to expand the understanding of youth in psychiatric residential treatment facilities by using psychosocial indicators to develop subgroup profiles. Additionally, differences in treatment outcomes between subgroups and the extent to which within-treatment factors accounted for observed differences in treatment outcomes between subgroups were examined. Data were extracted from the case records of 447 youth who were served in psychiatric residential treatment facilities over a seven year span of time. A latent class analysis was used to identify and describe subgroups. A series of multivariate regression analyses were used to examine group differences in functional impairment at discharge. Next, a path analysis was used to determine if there were differences in average change in functional impairment from admission to discharge between subgroups and to test within treatment factors as potential mediators of group differences. Finally, a logistic regression was used to determine if there were differences between groups in the probability of discharging to a community-based placement or discharging to another congregate care facility. The latent class analysis revealed four distinct subgroups of youth. The analyses of treatment outcomes revealed statistically significant differences in the level of functional impairment at discharge and average change in impairment between groups. Results from the path model of indirect effects supported that within treatment factors accounted for a statistically significant proportion of the observed difference in change between groups. No differences were found in discharge placement outcomes between groups. Implications for future research, practice and policies focused on youth in residential treatment are discussed.

Lenagården HVB : En utvärdering av behandlingen via kvalitativa intervjuer

Syftet med studien var att utvärdera vad tidigare klienter på Lenagården HVB, ett behandlingshem för vuxna med psykiska problem, upplevde som värdefullt, respektive vad de upplevde som negativt eller saknade i sin behandling. Utvärderingsfrågorna utgick från Lenagårdens programteori samt aspekter som tidigare forskning kring institutionsbehandling pekat ut som viktiga för framgångsrik behandling. Data samlades in via kvalitativa intervjuer med fyra tidigare klienter. Systemteorin har använts som utgångspunkt för resultaatanalysen. Utvärderingen visar bland annat att gott bemötande i termer av vänlighet och respekt samt personalens kompetens ansågs vara värdefulla faktorer i behandlingen. Själva terapin och behandlingsmetoden ansågs av samtliga intervjupersoner vara givande. Individanpassade lösningar, delaktighet och en förtroendefull relation till terapeuten eller behandlaren var i sin tur avgörande faktorer för en positiv upplevelse av terapin och behandlingen. Andra faktorer som värdesattes var meningsfulla aktiviteter, att bo med andra klienter samt fortsatt kontakt med Lenagården efter avslutad behandling. Några negativa aspekter i behandlingen beskrevs som för lite personaltid, för kort vistelse samt brist på gemensamma aktiviteter. / The aim of this evaluation was to find out what former clients at Lenagården HVB, an institution for treatment of adults with psychological problems, experienced as important in their treatment and what they experienced as negative or missing. The questions answered by the former clients were based on the program theory of Lenagården as well as earlier research about residential treatment. Data was gathered by qualitative interviews with 4 former clients.System theory was used as a theoretical framework for the analysis of the results. The evaluation points out that the experience of being treated in a friendly and respectful manner, the staffs level of professional training and the treatment method in itself were considered important. A trustful relation to the therapist and participation in the treatment were essential for a positive experience of the treatment. Other important aspects of treatment were activities, living with other clients and continued relations with Lenagården after completed treatment. Some negative aspects that were mentioned were not enough time with the staff, too short stay and lack of organized activities.

Creating a Tobacco-Free Residential Substance Abuse Treatment Facility: A Toolkit for Designing an Effective Intervention

Individuals with a substance addiction and/or mental illness use tobacco at rates much higher than the general population. Approximately 21% of the U.S. population smokes, yet 40%-80% of substance abuse and mental health patients smoke and consume more than 44% of all the cigarettes sold in the United States each year. As a result, these individuals experience increased tobacco related morbidity and mortality. Tobacco use is common and accepted within this population, and has become a norm even within treatment programs. Only 30%-40% of treatment facilities offer smoking cessation resources, and many organizations are hesitant to address cessation or to adopt a tobacco-free campus policy. This toolkit addresses the unique barriers and challenges that a residential substance addiction facility will face when adopting a tobacco-free policy. A tobacco-free campus policy protects all individuals from dangerous secondhand smoke and promotes the health and recovery of clients from all addictions, including nicotine. This toolkit provides step-by-step information and resources to allow a facility to become tobacco-free and implement a full smoking cessation program over a six month time period. This project helps fill a need for specific resources related to concurrent smoking cessation and substance abuse/mental health treatment. This resource will be made available to residential substance abuse facilities in Georgia and will be provided to the Substance Abuse and Mental Health Service Administration’s Smoking Cessation Leadership Center.

Adventure therapy for youth with addictions in residential treatment: an analysis of program processes and proximate outcomes

This study evaluated a residential treatment center for youth with addictions that used processes of adventure therapy, family involvement, community, and relationships. The evaluation contributed to filling gaps in the literature by linking processes to proximate outcomes. A mixed methods design used quantitative data from 2005-2013 program statistics and 93 Resiliency Canada Questionnaires, as well as qualitative data from 17 formal interviews, 12 informal interviews, and observation of 12 participants. A program description was provided for context and transferability. Results indicated that (a) the program demonstrated fidelity with the logic model, (b) clients’ had raised awareness of strengths, (c) youth shared experiences of the impact of addiction, (d) youth lived healthy, substance-free lives, (e) positive family relationships were promoted, (f) youth increased resiliency, and (g) youth in the program for longer than a month maintained sobriety immediately afterwards and the majority (n=2) had long-term reduction in substance misuse.
