Prevalence and clinical characteristics of elevated 1-alpha,25-dihydroxyvitamin D in pediatric nephrolithiasis and related disorders

INTRODUCTION: The incidence of pediatric nephrolithiasis (kidney stones) has been increasing over the past several years. While environmental factors, such as poor fluid intake, high-salt diet, and obesity, can play a role, underlying metabolic factors account for at least one-third of cases of nephrolithiasis. Nephrolithiasis and related disorders, such as nephrocalcinosis and hypercalciuria, can lead to long-term kidney problems, including renal scarring, acute and chronic kidney disease, decreased renal function, or end-stage renal disease. The best treatment is prevention and is best guided by knowing the underlying cause. The majority of kidney stones are primarily comprised of calcium, and abnormal calcium metabolism and regulation can lead to nephrolithiasis, nephrocalcinosis, and hypercalciuria. Vitamin D is an important factor in calcium regulation in the body. The physiologically active form of vitamin D is 1α,25-dihydroxyvitamin D (1,25(OH)2D), which increases serum calcium by stimulating intestinal absorption of calcium, increasing renal calcium reabsorption, and mobilizing calcium from bone. Excess 1,25(OH)2D has been shown to be associated with hyperabsorption of calcium in the intestine, nephrolithiasis, hypercalcemia, and hypercalciuria. Production of 1,25(OH)2D requires hydroxylation of 25-hydroxyvitamin D by the kidney enzyme 1α-hydroxylase, which is regulated in turn by serum calcium, parathyroid hormone (PTH), and by 1,25(OH)2D itself. Tight control of 1,25(OH)2D levels is maintained in part by the breakdown of 1,25(OH)2D by the enzyme 24-hydroxylase, which is encoded by the gene CYP24A1. In the past few years, CYP24A1 mutations leading to decreased activity of 24-hydroxylase have been implicated in some cases of idiopathic infantile hypercalcemia as well as nephrolithiasis, nephrocalcinosis, and hypercalciuria. The prevalence of 24-hydroxylase deficiency is not known, and the spectrum of its clinical manifestations is not yet fully understood. Our study aims to describe the clinical characteristics of patients with laboratory findings suggestive of 24-hydroxylase deficiency, specifically high-normal or elevated serum 1,25(OH)2D. We aimed to determine the prevalence of elevated 1,25(OH)2D among pediatric patients with nephrolithiasis, and to compare clinical outcomes and biochemical findings in patients with normal versus elevated 1,25(OH)2D. PATIENTS AND METHODS: This study was a retrospective chart review. To determine the prevalence of high-normal (56-75 pg/mL) and high (>75 pg/mL) serum 1,25(OH)2D, we reviewed electronic medical records of patients seen in the Boston Children's Hospital Stone Clinic. We identified 346 patients who were evaluated for nephrolithiasis, were under 18 years of age at the time of presentation, and had at least one measurement of 1,25(OH)2D. Patients were classified based on their highest measured level of 1,25(OH)2D. To determine the clinical characteristics of patients with elevated 1,25(OH)2D, we reviewed clinical records and laboratory data of patients at Boston Children's Hospital with a diagnosis of nephrolithiasis, nephrocalcinosis, or hypercalciuria. We identified 83 patients who met our inclusion criteria: age of onset <18 years, at least one measurement of 1,25(OH)2D, and a pre-treatment urine solute analysis. Data collected included demographic information, diagnoses, family history of kidney disease, treatments, laboratory data, and urine solute analyses. We compared findings in patients with normal 1,25(OH)2D (≤55 pg/mL) versus elevated 1,25(OH)2D (>55 pg/mL). RESULTS: Of 346 children with nephrolithiasis in whom 1,25(OH)2D was measured, 100 (28.9%) had high 1,25(OH)2D, and an additional 120 (34.7%) had high-normal 1,25(OH)2D. To determine the clinical characteristics of elevated 1,25(OH)2D, we analyzed the data of 40 patients with normal 1,25(OH)2D and 43 patients with elevated 1,25(OH)2D who had a history of nephrolithiasis, nephrocalcinosis, or hypercalciuria. Seventy-five children had nephrolithiasis, and 25/37 (67.6%) of children with elevated 1,25(OH)2D had a recurrence of nephrolithiasis, compared to only 9/38 (23.7%) of children with normal 1,25(OH)2D (p < .001). Urine calcium/creatinine ratio did not differ between the two groups. However, linear regression analysis showed an association between 1,25(OH)2D levels and urine calcium/creatinine ratio. Important secondary findings included a younger age of onset, higher serum 25-hydroxyvitamin D, and lower parathyroid hormone levels in patients with elevated 1,25(OH)2D. CONCLUSIONS: Important clinical findings of this study were the increased rate of recurrence and the younger age of onset in patients with elevated 1,25(OH)2D. While we recognize that mutations in CYP24A1 do not account for the majority of cases of elevated 1,25(OH)2D, we do advocate for special consideration for these patients. In the absence of a commercially-available assay for 24-hydroxylase activity, children with nephrolithiasis, nephrocalcinosis, or hypercalciuria and elevated 1,25(OH)2D should be closely monitored for recurrence or worsening of symptoms. Furthermore, we advise caution in the use of vitamin D repletion in at-risk patients.

C-ARM TOMOGRAPHIC IMAGING TECHNIQUE FOR DETECTION OF KIDNEY STONES

Nephrolithiasis can be a painful problem due to presence of kidney stones. Kidney stone is among the common painful disorders of the urinary system. Various imaging modalities are used to diagnose patients with symptoms of renal or urinary tract disease such as plain kidney, ureter, bladder x-ray (KUB), intravenous pyelography (IVP), and computed tomography (CT). As a traditional three-dimensional (3D) nephrolithiasis and kidney stones detection technique, computed tomography (CT) provides detailed cross-sectional images as well as 3D structure of kidney from moving the x-ray beam in a circle around the body. However, the risk of CT scans of the kidney is relatively higher exposure to radiation which is more than regular x-rays. C-arm technique is a new x-ray imaging modality that uses 2D array detector and cone shaped x-ray beam to create 3D information about the scanned object. Both x-ray source and 2D array detector cells mounted on C-shaped wheeled structure (C-arm). A series of projection images are acquired by rotating the C-arm around the patient in along circular path with a single rotation. The characteristic structure of C-arm allows to provide wide variety of movements around the patient that helps to remain the patient stationary during scanning time. In this work, we investigated a C-arm technique to generate a series of tomographic images for nephrolithiasis and detection of kidney stones. C-arm tomographic technique (C-arm tomosynthesis) as a new three dimensional (3D) kidney imaging method that provides a series of two dimensional (2D) images along partial circular orbit over limited view angle. Our experiments were done with kidney phantom which formed from a pig kidney with two embedded kidney stones inside it and low radiation dosage. Radiation dose and scanning time needed for kidney imaging are all dramatically reduced due to the cone beam geometry and also to limitation of angular rotation. To demonstrate the capability of our C-arm tomosynthesis to generate 3D kidney information for kidney stone detection, two groups of tomographic image reconstruction algorithms were developed for C-arm tomosynthesis: direct algorithms such as filtered back projection (FBP) and iterative algorithms such as simultaneous algebraic reconstruction technique (SART), maximum likelihood expectation maximization (MLEM), ordered- subset maximum likelihood expectation maximization (OS-MLEM) and Pre-computed penalized likelihood reconstruction (PPL). Three reconstruction methods were investigated including: pixel-driven method (PDM), ray-driven method (RDM) and distance driven method (DDM). Each method differs in their efficiency of calculation accuracy per computing time. Preliminary results demonstrated the capability of proposed technique to generate volumetric data about the kidney for nephrolithiasis and kidney stone detection by using all investigated reconstruction algorithms. In spite of each algorithms differs in their strategies, embedded kidney stone can be clearly visualized in all reconstruction results. Computer simulation studies were also done on simulated phantom to evaluate the results for each reconstruction algorithm. To mimic kidney phantom, simulated phantom was simulated with two different size kidney stones. Dataset of projection images was collated by using a virtual C-arm tomosynthesis with geometric configuration similar to real technique. All investigated algorithms were used to reconstruct 3D information. Different of image quality functions were applied to evaluate the imaging system and the reconstruction algorithms. The results show the capability of C-arm tomosynthesis to generate 3D information of kidney structures and to identify the size and location of kidney stones with limited amount of radiation dose.

Estratégias para a sustentabilidade da mineração de rochas ornamentais no noroeste do estado do Espírito Santo

O presente trabalho tem por objetivo contribuir com a atividade de extração de rochas ornamentais, por meio de propostas com recomendações que possam indicar passos a serem seguidos (guidelines) para a sustentabilidade desse setor, utilizando como estudo de caso a região noroeste do Estado do Espírito Santo. Para isso são feitas propostas de planejamento em longo prazo por meio do Processo Prospectivo e, em mais curto prazo, por um esquema de certificação sustentável para a atividade, com vistas à sustentabilidade desse polo produtor, considerando sua importância e os problemas existentes. O Processo Prospectivo, feito mediante a utilização da abordagem inicial do Processo Prospectivo, com a varredura (scanning) do ambiente, e aplicado para a região noroeste do estado do Espírito Santo, apresentou-se como uma ferramenta adequada para atingir os objetivos do presente estudo, principalmente por apontar, a partir dos resultados, além da sua continuidade, também a viabilidade de implementar uma certificação de sustentabilidade para o setor de rochas ornamentais, pela concordância observada nos resultados obtidos, na visão dos stakeholders. Para a proposta de certificação sustentável foi feita uma pesquisa por meio de questionário com stakeholders chave da indústria de rochas ornamentais para identificar a percepção desses com relação à iniciativa, que incluiu tópicos gerais e específicos divididos em três temas principais: sustentabilidade da indústria de rochas ornamentais no Brasil; desafios para a sustentabilidade dessa indústria e deficiências e omissões da regulamentação vigente, relativa a esses desafios; e a viabilidade de um processo de certificação sustentável para a indústria de rochas ornamentais no Espírito Santo e a participação dos stakeholders e de seus órgãos nesse processo. Os resultados obtidos com o desenvolvimento do presente estudo permitem concluir que, na visão dos stakeholders da indústria de rochas ornamentais do Brasil, em especial na visão daqueles do estado de Espírito Santo, a formalização das empresas do setor, aliada a um processo de certificação, tem potencial para gerar benefícios significativos para o desenvolvimento local e/ou regional sustentável, o que coincide com outras experiências apresentadas na literatura vigente. Os resultados também mostraram que ainda há muito a ser estudado antes que uma certificação sustentável seja implementada, mas que há sinais encorajadores para a continuidade do trabalho, pois parte significativa dos stakeholders (sociedade, governo e empresários do setor) respondeu favoravelmente a essa iniciativa para o setor, até mesmo em âmbito nacional. Nesse sentido, é reforçada a importância da cooperação entre o setor, a sociedade e governo federal, estadual e municipal em todo o processo, de modo a se criar uma cultura que possibilite a sustentabilidade efetiva desse setor. Finalmente, o trabalho mostra que, apesar dos desafios existentes, as propostas de continuidade do Processo Prospectivo e de viabilização de um processo de certificação sustentável têm potencial para aprimorar a indústria de rochas ornamentais pela adoção de critérios, que visem não somente as questões de caráter econômico, como aquelas que melhorem a performance social e ambiental para a efetiva sustentabilidade dessa indústria. / This work aims to contribute with dimension stones industry by doing proposals and recommendations of guidelines to the sustainability of this industry, based on northwest region of Espirito Santo State as case study. Long term planning by applying the Foresight and of short term a Sustainability Certification Scheme are proposed in order to achieve the sustainability of the producer pole by considering its relevance and the existents problems. Foresight was applied for the northwest region using an initial approach of Foresight with the scanning of the environment, and it is presented as an appropriate tool to achieve the objectives of this study, mainly to point out from the results not only its continuity, but also the feasibility of implementing a sustainability certification scheme for the dimension stones industry, by the agreement observed in the results, in the view of stakeholders. For the sustainability certification scheme a survey was conducted with key stakeholders of the dimension stones industry to identify their perceptions regarding to the initiative, which included general and specific topics in three themes: sustainability of the dimension stones industry in Brazil; challenges to the sustainability of the dimension stones industry, and deficiencies and omissions of current regulatory processes regarding these challenges; and the feasibility of a sustainability certification scheme for the dimension stones industry in Espírito Santo State and the participation of entities/bodies/agencies in this process. The results obtained with the development of this study allow us to conclude that, in the view of stakeholders of the dimension stone industry in Brazil, especially the Espírito Santo State, the formalization of companies in the industry, combined with a certification process has the potential to generate significant benefits for local and/or regional sustainable development, which coincides with other experiences presented in current literature. The results also showed that there is still too much to be studied before a sustainability certification scheme is implemented, but there are encouraging signs for the continuity of the work, as a significant part of stakeholders (society, government and industry entrepreneurs) responded favorably to this initiative the industry, even at the national level. In this sense, it is reinforced the importance of cooperation between industry, society and federal, state and local government throughout the process, in order to create a culture that enables the sustainability of this sector. Finally, the study shows that despite the existing challenges, proposals for continuity of the Foresight and achieving a sustainability certification scheme have the potential to improve the dimension stones industry by adopting criteria that not only consider the economic issues of the industry, but also to improve its social and environmental performance for effective sustainability of this industry.

Exibem cor que varia de verde a cinza escuro, com textura de fina a média, e foliação que varia de pouco a bem marcada. Todos estes corpos rochosos estão dispostos sob a forma de maciços. Os materiais estudados foram denominados de Verde Netuno, Verde Santa Cruz e Verde Choró, respectivamente para os granulitos das regiões de Itapipoca, Granja e Chorozinho. Foram pesquisados quanto aos aspectos tecnológicos e alteração/alterabilidade quando em uso. O objetivo principal deste trabalho foi estudar as alterabilidades das rochas selecionadas e observar os seus comportamentos uma vez expostas à ação dos agentes de alteração em laboratório, em condições correlacionáveis às do ambiente em que poderão ser aplicadas. A resposta aos ensaios tecnológicos mostra que eles apresentam boa qualidade para uso ornamental e de revestimento. Os estudos de alterabilidade realizados em rochas granulíticas demonstraram que, mesmo rochas aparentemente pouco susceptíveis a alteração, podem apresentar problemas de degradação quando submetidas a ambientes poluídos quimicamente agressivos. Sobre estes aspectos, observou-se que as características petrográficas e os índices físicos (porosidade e absorção) são ferramentas muito importantes para a avaliação da durabilidade das rochas como materiais de revestimento. No contexto geral estas rochas mostraram-se bastante resistentes para uso como materiais de revestimentos. / The materials employed in this research correspond to granulitic rocks, more specifically enderbitic gneisses. Those rocks exhibit of green to dark gray color, fine to medium texture and gentle to well-marked foliation. All of them constitute massif bodies. The denominations Green Netuno, Green Santa Cruz and Green Choró were given to the granulites of Itapipoca, Granja and Chorozinho, respectively. The research was carried out taking in account the technological aspects and weathering/weatherability when in use. The essential aim of this work is to study the alterability of selected rocks and to observe their behavior once exposed to alteration agents in a laboratory environment comparable to actual environments where these rocks are used. The results of the technological tests showed that these rocks present good quality for both ornamental and of coating use. The alterability studies conducted in granulitic rocks demonstrated that, even rocks with an apparent low susceptibility to alteration, may in fact problems when submitted to chemically aggressive polluted environments. On these aspects, it was possible to notice that both the petrographic characteristics and the physical parameters (porosity and absorption) are very important tools for the evaluation of the durability of the rocks as coating materials. Therefore, in a general context the rocks studied are quite resistant regarding both to the alteration capacity and to the technological characteristics.

Estudo experimental do polimento de diferentes \"granitos\" e as relações com a mineralogia / Experimental study of different \"granites\" polishing and its relation with mineralogy

O aspecto técnico das rochas de revestimento é o primeiro condicionante para o seu uso nas edificações, seguido das suas características tecnológicas, da tipologia do jazimento e dos possíveis defeitos decorrentes das etapas de extração e beneficiamento. O presente trabalho aborda os mecanismos que influenciam no processo de polimento, lembrando que a indústria ainda se baseia no empirismo, com generalizações não condizentes que aumentam o custo de produção das placas. Foram escolhidos três tipos de rochas \"graníticas\": Sienogranito (Vermelho Brasília), Migmatito (Jacarandá Rosado) e Monzogranito (Cinza Andorinha), por serem de alta comercialização e consideradas no processamento industrial como \"dura\", \"média\" e \"mole\", respectivamente. Foram ensaiadas no simulador de polimento de rochas (SPR), utilizando o abrasivo magnesiano e antes de iniciar o processo foi determinada a rugosidade superficial de cada amostra. Variando-se a carga aplicada na politriz, a velocidade, o tempo e monitorando a perda de massa associada ao brilho, foi possível estabelecer as condições ideais para os tipos de rochas estudadas. / Dimension stones aesthetics are the main factor considered in their use for buildings, followed by technological characteristics, deposit\'s typology and possible extraction and beneficiacion effects. This paper approaches the influential mechanisms on polishing process, considering that industry still bases itself on empiricisms, with incoherent generalizations that increase production slabs cost. Three types of granite rocks were evaluated: Sienogranite (Vermelho Brasília), Migmatite (Jacarandá Rosado) and Monzogranite (Cinza Andorinha), due to their highly commercialization. The rocks are classified at the industrial process as hard, medium and soft (respectively). They were tested with a polishing rock simulator through the use of magnesium abrasives and had the surface roughness pre-determined for each sample. By altering the polishing machine applied load, as well as speed and time, and having the brightness associated weight loss monitored, it was possible to stabilish ideal conditions for the studied rock types.

In this sense, it is reinforced the importance of cooperation between industry, society and federal, state and local government throughout the process, in order to create a culture that enables the sustainability of this sector. Finally, the study shows that despite the existing challenges, proposals for continuity of the Foresight and achieving a sustainability certification scheme have the potential to improve the dimension stones industry by adopting criteria that not only consider the economic issues of the industry, but also to improve its social and environmental performance for effective sustainability of this industry.

Fixação de placas de rochas ornamentais: estudo da aderência com argamassa colante / Setting of dimension stones: study of the tack with adhesive mortar

Nos revestimentos de paredes a fixação de placas de rochas pode ser feita com inserts metálicos ou por aderência com argamassas. Nos assentamentos com argamassas os valores de aderência, por norma, devem ser superiores a 1 MPa e a altura máxima do revestimento não pode ultrapassar 3 m. No presente trabalho foram feitos ensaios com ladrilhos de três tipos de \"granitos\" para comparar a aderência da argamassa existente no mercado, específica para estas rochas, com uma argamassa colante para porcelanatos, de mesmo custo de produção, desenvolvida por pesquisadores do Departamento de Arquitetura da Escola de Engenharia de São Carlos da Universidade de São Paulo. As rochas escolhidas foram \"Vermelho Brasília\" (sienogranito), \"Verde Labrador\" (charnoquito) e \"Preto Indiano\" (migmatito) apresentam características petrográficas e serrabilidades diferentes, o que implica em valores distintos de rugosidade das chapas obtidas pelo desdobramento dos blocos em teares. A aderência destas rochas com as com argamassas foi determinada, tanto na face rugosa como na face polida por meio do ensaio de arrancamento por tração, normatizado para cerâmica. Os resultados mostraram para estas rochas que a aderência das argamassas está relacionada à rugosidade e à mineralogia/textura. A aderência obtida para a argamassa colante desenvolvida para porcelanato foi aproximadamente 2 vezes superior a encontrada para argamassa comercial, mostrando sua excelente qualidade para o assentamento de placas de \"granitos\". / In coverings of walls the setting of dimension stones can be made with metallic inserts or by tack with mortar. In the nestings with mortar the values of tack, for norm, must be superior the 1 MPa and the maximum height of covering cannot exceed 3 m. In the present work, assays with floor tiles of three types of \"granites\" had been made to compare the tack of the existing mortar in the market, specific for these granites, with a adhesive mortar for porcelain, of same cost of production, developed for researchers of the Department of Architecture of the School of Engineering of São Carlos of the University of São Paulo. The chosen stones had been \"Vermelho Brasília\" (sienogranite), \"Verde Labrador\" (charnockite) and \"Preto Indiano\" (migmatite) they present petrographics analysis and saw different, what implies in distinct values of the rugous of plates gotten for the unfolding of the blocks in sewing presses, that use grain of steel as abrasive element. The tack of plates of these stones with mortar was determined in such a way in the rugose face as in the polishing face by way of the assay of pulling up for traction, prescriptive for ceramics. The results had shown that the tack of mortar is related to the rugous and mineralogy/texture for these stones. The tack gotten for the developed adhesive mortar for porcelain was approximately two times superior the found one for commercial mortar, showing its excellent quality for the nesting of \"granite\" plates.
