Um diálogo entre Freud e Lacan fundamentado no caso "o pequeno Hans"

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / This work has dealt with the clinical Little Hans case - statement by Freud in 1909 - and aims to bring the dialogue between this theoretical and Jacques Lacan, with prints and case analysis. Specifically, the study aims to provide for the issue of child psychoanalysis and addressing the problem of phobia while symptomatic output. Considering the clinic with children as contemporary practice of psychoanalysis, there was a theoretical study, whose methodological dynamics was the literature, using mainly from Freud´s texts and Lacan. Therefore, the work is divided into three sections. The first brings the issue of child psychoanalysis, highlighting as it reaches the place of subject of desire and law, making it therefore possible to be analyzed. In addition, support for the next chapter in this first are brought some positions and functions assumed by the child in the family dynamics. The second chapter develops the dialogue between Freud and Lacan about Hans case, presenting the facts following the case, the chronology, the directions of Freud and Lacan considerations. Are treated some issues such as the relationship between Hans and his mother, the question of the paternal function, as well as the complex of Oedipus and castration in Hans. Finally, the third chapter is the phobia as symptomatic output, serving Hans as the means possible so that it is positioned in the world and enter in the symbolic order. Thus, issues are worked that concern the phobia, such as anxiety, anguish, symptomatic formation and its prospects and the specific clinical case, the focus on substitutive function assumed by the phobic symptom on the limping paternal function. We conclude the work by pointing out some directions for the relationship between the current children´s clinic and the symptom. / Este trabalho tem por tema o caso clínico do pequeno Hans comunicado por Freud em 1909 e almeja trazer o diálogo entre esse teórico e Jacques Lacan, com as impressões e análises do caso. Especificamente, o estudo objetiva dispor sobre a questão da criança para psicanálise e abordar a problemática da fobia enquanto saída sintomática. Considerando a clínica com criança como prática contemporânea à psicanálise, realizou-se um estudo teórico, cuja dinâmica metodológica foi a pesquisa bibliográfica, utilizando-se, sobretudo, de textos de Freud e de Lacan. Por conseguinte, o trabalho se encontra estruturado em três capítulos. O primeiro traz a questão da criança para psicanálise, destacando como ela alcança o lugar de sujeito de desejo e de direito, tornando-se, portanto possível de ser analisada. Além disso, por fundamentar o capítulo seguinte, neste primeiro são trazidas algumas posições e funções assumidas pela criança na dinâmica familiar. No segundo capítulo, é desenvolvido o diálogo entre Freud e Lacan acerca do caso Hans, apresentando os fatos que acompanham o caso, a cronologia, os direcionamentos de Freud e as considerações de Lacan. São tratados alguns assuntos como a relação entre Hans e sua mãe, a questão da função paterna, assim como os complexos de Édipo e de castração em Hans. Por fim, o terceiro capítulo traz a fobia como saída sintomática, servindo a Hans como o meio possível para que ele se posicione no mundo e insira-se na ordem simbólica. Assim, são trabalhados temas que tangem à fobia, como a ansiedade, a angústia, a formação sintomática e suas perspectivas e no caso clínico específico, a aposta na função de suplência assumida pelo sintoma fóbico, diante da função paterna claudicante. Concluímos o trabalho apontando alguns direcionamentos para a relação existente entre a clínica infantil atual e o sintoma.

Diagnostik und Therapie von Atemwegsinfekten in der Allgemeinarztpraxis / Diagnosis and treatment of respiratory tract infections in general practice

Einleitung: Atemwegsinfekte gehören zu den häufigsten Krankheitsbildern in der Allgemeinarztpraxis. Ziel der Erhebung war es, Daten zur Diagnostik und Therapie von Atemwegsinfekten in der hausärztlichen Praxis zu erheben. Methoden: Während einer jeweils eintägigen Hospitation bei 30 Fachärzten für Allgemeinmedizin wurde deren Vorgehensweise bei allen Patienten mit einem akuten Atemwegsinfekt dokumentiert, die im Zeitraum der Beobachtung den Arzt konsultierten. Es erfolgte eine Unterscheidung zwischen Erst- und Folgekontakten. Ergebnisse: Diagnostisch wurden am häufigsten die Auskultation der Lunge und die Inspektion des Mund-Rachen-Raumes durchgeführt. 98,4% der Patienten mit Erstkonsultationen und 62,5% der Patienten mit Folgekonsultationen erhielten eine medikamentöse Verordnung. Im Durchschnitt erhielten erstkonsultierende Patienten 2,1 (+-1,0), Patienten im Folgekontakt 1,3 (+-1,1) Medikamente. Am häufigsten wurden Medikamente aus der Gruppe der Husten- und Erkältungspräparate verordnet (87,1% der Erstkontakte und 52,9% der Folgekontakte). 43,5% der Erstkontakte und 29,9% der Folgekontakte erhielten ein Antibiotikum (37,5% Makrolide, 21,5% Penicilline, 20,8% Doxycyclin als Monosubstanz oder in Kombination mit Expektorantien). Schlussfolgerung: Nahezu alle Patienten erhielten ein Rezept über mindestens ein Medikament. Die erhobenen Daten lassen vermuten, dass sich bei einer höheren Gewichtung der so genannten "Hausmittel" ein deutliches Einsparungspotential böte. Angesichts des hohen Anteils der Antibiotikaverordnungen sollte die entsprechende Indikationsstellung kritisch überdacht werden.

Michael Hauser Ph.D. ABSTRACT: The thesis focuses on a philosophy of Louis Althusser in his "teoreticist" period, i.e. 1960-1967. The work is divided to four essentials sections: epistemology, ontology, psychoanalyse and ideology. We put accent on epistemological problematic, which is in this period of a development of Althusser's philosophy unequivocally dominant. In introduction of this text is explained historical and political context, in which Althusser realized his "theoretical intervention". The thesis treat on Althusser's conviction, that inadequate theory leads to deformed political practice and that Marxist theory exists till now in his "applied form", notably in The Capital. According to Althusser this Marxist theory wasn't adequately theoretically formed. Our work characterises Althusser's tentative to create and theoretically formulate this theory. This Althusser's tentative is connected with an effort to draw a "line of demarcation" between the Marxism and the pre-Marxist idealist notions, which are foreign to authentic Marxism. Text in Afrikaans / In this thesis the term post-colonialism in the Afrikaans drama is investigated, focussing on the post-sixties. The term post-colonialism is difficult to define. Not only are theories of post-colonialism in a state of continuous flux and shifting emphasis, but as a result of different colonial dominations, separate identities have been constructed in South-Africa; so that defining the terms colonial, post colonial and post-colonial proves to be even more problematic. The purpose of this study is to determine to what extent the Afrikaans drama fits into these discourses. The basic point of departure is the fact that post-colonialism played a considerable role in the development of the Afrikaans drama, at the same time providing a more varied scope. The research covers several aspects of post-colonialism in Afrikaans drama; each dealt with in a separate chapter. A multitude of perspectives are featured within the broader discourse in order to obtain multiple norms and standards in a phase of self-criticism. The focus falls mainly on themes and not on performance aspects. New perspectives on issues such as canon texts, silence, hero-worship, the portrayal of woman, patriarchy, and neo-colonialism are presented (chapter 1). In chapter 2 focus falls on the period before 1960, and notably the question of nationalism (associated with apartheid) and the portrayal of the Afrikaner. The literary canon, forms of violence and the position of the super-Afrikaner are viewed in a new light during the re-writing of post-colonial history and the resulting paradigm shifts after 1960. Renewed emphasis is placed on discourse concerning land (chapter 3). Contrasting concepts regarding race, class, language, gender and religion are reconsidered in order to contribute towards the heterogeneous nature of post-colonialism (chapter 4). The function of theatre is to re-evaluate in the context of a post-1994 democratic system. Texts now focus especially on empowerment, re-discovery and re-ordering of history, reconciliation, inter-cultural contact and a post-apartheid syndrome (chapter 5). Molecular characterization of norovirus stains circulating in rural communities of Limpopo Province of South Africa

PhD (Microbiology) / Department of Microbiology / Globally, one in ten child deaths before the age of 5 years is due to diarrheal disease, causing almost 800,000 mortalities worldwide, which mostly occur in Sub-Saharan Africa and South Asia. Eighty-eight percent (88%) of diarrheal deaths worldwide are attributable to unsafe water, inadequate sanitation and poor hygiene. Unsanitary environments and poor hygiene practices allow diarrhea causing pathogens including viruses, bacteria and parasites to spread more easily. Norovirus (NoV) are now considered the most common cause of outbreaks of nonbacterial gastroenteritis. However, the factors which control the genetic diversity, the sources of sporadic NoV infections, the transmission and persistence of infection are poorly understood. Limited data are available for NoVs strains in South Africa, especially in rural and peri-urban areas. Despite the excessive burden of diarrhea disease in developing countries, NoVs outbreaks have been to date mostly reported in developed countries. Given that the contribution of the various pathogens to diarrhea may differ substantially between regions depending on local meteorological, geographic, and socio-economic conditions, there is a need to investigate intensively the role of viral agents associated with diarrhea in different settings in Africa continent. How would poor living conditions in rural setting impact the prevalence and genetic characteristics of Norovirus strains circulating Limpopo province is the research question of this study. ix To determine the prevalence and genetic diversity of NoVs strains circulating in the rural communities in the Limpopo Province, South Africa and investigate the genetic relationship between NoVs strains, a cross-sectional study was performed on human stools collected from rural communities. We used qualitative variables of poor living environmental conditions including type of water used at the household of child’s parent or guardian, use of toilet seat, presence of livestock at the household and parent employment status to assess possible environmental risk factors of NoV infection within the study area. Prior to this prospective study, we conducted a systematic review of the PubMed and EMBASE databases for published articles of Human NoVs in Africa between 1990 and 2013 in order to assess the contribution of Human NoVs to diarrhoeal diseases in Africa. This review provides a picture of Human NoVs studies in Africa and reveals that unreported sporadic gastroenteritis cases of Human NoVs are common in Africa. Most are community-associated infections reported from urban settings. Possible environmental transmission routes have been documented. Combined environmental and clinical studies are required for targeted actions to control transmission of Human NoVs in Africa. Between July 2014 and April 2015, outpatient children under 5 years of age from rural communities of Vhembe district, South Africa, were enrolled for the study. A total of 303 stool specimens were collected from those with diarrhea (n=253) and without (n=50) diarrhea. NoVs were identified using real-time one-step RT-PCR. Nucleotide sequencing methods were performed to genotype the strains. Phylogenetic analyses x were performed to compare identified NoVs genotypes to the worldwide circulating strains. One hundred and four (41.1%) NoVs were detected. NoV detection rates in symptomatic and asymptomatic children (OR = 1.24; 95% CI 0.66 – 2.33) were not significantly different. Comparison of the median CT values for NoV in symptomatic and asymptomatic children revealed significant statistical difference of estimated GII viral load from both groups, with a much higher viral burden in symptomatic children to our knowledge this is the first study reporting on the differences in estimated viral load of GII and GI NoV positive cases and controls. The study findings may have implications for the diagnosis of NoV disease and future vaccine development, which may only need to consider GII as the genogroup associated with diarrhea in the South African population. Sequence analyses demonstrated multiple NoV genotypes identified in rural communities of Vhembe district. The most prevalent NoV genotypes were GII.4 Sydney 2012 variants (n=7) among the capsid genotypes, GII.Pe (n=9) among the polymerase genotypes and GII.Pe/GII.4 Sydney 2012 (n=8) putative recombinants among the RdRp/Capsid genotypes. Two unassigned GII.4 variants and an unusual RdRp genotype GII.P15 were found. With note, the rare GII.P15 identified in this study, has a common ancestor with GII.P15 strain from Japan previously reported as GII / untypeable recombinant strain implicated in a gastroenteritis outbreak. To our knowledge this is the first report of this unusual genotype in the African continent. Though not proven predictive of diarrhea disease in this study, the high detection rate of NoV reflects the substantial exposure of children from rural communities to enteric xi pathogens possibly. However in this study no risk factor has been found between NoV positive and qualitative environmental variables of poor living conditions in rural setting. The results also suggest that the difference between asymptomatic and symptomatic children with NoV may be at the level of the viral load of NoV genogroups involved. The findings highlighted NoV genetic diversity and revealed continuous pandemic spread and predominance of GII.Pe/GII.4 Sydney 2012, indicative of increased NoV activity. An unusual RdRp genotype GII.P15 and two unassigned GII.4 variants were also identified from rural settings of the Vhembe district/South Africa. NoV surveillance / NRF

From the responses it was clearly evident that the fears of the unknown, the stigma attached to the victim's close associates and family, was a problem, and the route course of incredible naive and false rumours. A chapter of the research focused on literature to establish what was the major aspects of the problem expressed by the media. Guidelines were provided whereby therapy could address and allay these fears to assist the family to better understand the illness and it's effects. This chapter has in mind to provide the Psychologist with guidelines to provide therapy to both the victim and the family in this regard.
