Análise tomográfica e histomorfométrica comparativa entre enxerto ósseo autógeno e enxerto ósseo xenogênico em cirurgia de levantamento de seio maxilar em coelhos

O objetivo deste trabalho foi avaliar a neoformacão óssea induzida por um xenoenxerto bovino composto comparativamente ao ósseo autógeno, em cirurgias de levantamento de seio maxilar em coelhos. Nesse estudo utilizouse o seio maxilar de 18 coelhos machos, sendo que no seio maxilar esquerdo foi colocado 200mg de osso autógeno córtico-medular triturado e no seio maxilar direito, o biomaterial, de origem bovina, composto por matriz orgânica cortical e inorgânica medular, \'pool\' de BMPs e colágeno (Gen- Tech®). Realizou-se análise tomográfica por tomógrafo computadorizado (TC) para avaliar a densidade óptica comparativamente entre os grupos e períodos experimentais, a histológica descritiva para verificar a resposta tecidual frente aos materiais implantados e a histomorfométrica para obter os dados de densidade de volume de tecido ósseo neoformado, tecido conjuntivo e/ou granulação, material residual e tecido mielóide. Os resultados obtidos pela TC demonstraram densidade óptica cerca de 2 vezes maior no grupo teste. Na análise histológica observou-se infiltrado inflamatório apenas no período de 2 semanas do grupo teste e exclusivamente ao redor do componente orgânico do biomaterial. Quanto a neoformação óssea, não houve diferença estatisticamente significativa entre os dois grupos experimentais ou ao longo dos períodos, no entanto a área de tecido mielóide foi cerca de 2 vezes maior no grupo controle. Considerando-se os limites do modelo experimental e da técnica empregada podemos concluir que o biomaterial Gen-Tech® é potencialmente indicado como substituto ósseo em cirurgias de levantamento de seio maxilar, uma vez que promove uma neoformação óssea similar à do osso autógeno. / The purpose of this study was to evaluate new bone formation induced by a bovine xenograft compared to autogenous bone in maxillary sinus augmentation in rabbits. The maxillary sinus of 18 male rabbits were used, whereas the left sinus was filled with 200mg of a mixture of cortical and medullar autogenous bone and the right sinus was filled with a mix of organic and inorganic bovine matrix, plus \'pool\' of BMPs and collagen (Gen-Tech®). Computer tomography (CT) analysis was realized to evaluate and compare the optical density among the groups and experimental periods; descriptive histologic analysis was used to verify the tissue response to the materials implanted. In addition, histomorphometric analysis was realized to evaluate volume density of the newly formed bone, connective tissue and/or granulation tissue, residual material and myeloid tissue. The results obtained in the CT demonstrated an optical density approximately 2 times greater in the test groups. Histological analysis revealed an inflammatory infiltrate at 2 weeks in the test group and exclusively around the organic portion of the material. Regarding new bone formation, no statistical differences were observed among the two experimental groups or throughout the experimental periods. However the area of myeloid tissue was 2 times greater in the control group. Considering the limits of the experimental model and technique used in this study, it is possible to conclude that Gen-Tech® is a biomaterial with potential applications as a bone substitute in maxillary sinus augmentation once it is able to promote the formation of new bone similarly to autogenous bone.

Avaliação do efeito do laser de baixa intensidade na regeneração de defeitos ósseos de tamanho crítico preenchidos com osso autógeno ou xenógeno associados à membrana colágena / Evaluation of the effect of low-level laser therapy on the regeneration of critical size defects filled with autogenous bone or xenogenous bone associated to collagenous membrane

O objetivo deste estudo foi avaliar o efeito do laser de baixa intensidade (LBI), na regeneração de defeitos de tamanho críticos preenchidos com osso autógeno ou Bio-Oss® associados à membrana colágena. O estudo foi conduzido em 120 defeitos críticos (5 mm) em calvárias de ratos machos (Rattus norvegicus, albinus, Wistar), pesando entre 250 a 300g, divididos em 12 grupos experimentais (n=10): 1) Grupo C (Controle coágulo sanguíneo); 2) Grupo M (membrana colágena- BioGide®); 3) L (laser de baixa intensidade, GaAlAs, 808 nm, 100 mW, 6J, 210 J/cm2); 4) Grupo OA (osso autógeno); 5) Grupo OA/L (osso autógeno + LBI) 6) Grupo OA/M (osso autógeno + membrana colágena); 7) Grupo L/M (LBI + membrana colágena); 8) Grupo OA/L/M (osso autógeno + LBI + membrana colágena); 9) Grupo BO (Bio-Oss®); 10) Grupo BO/M ( Bio-Oss® + membrana colágena) 11) Grupo BO/L (Bio-Oss®+ LBI); 12) Grupo BO/L/M (Bio-Oss® + LBI + membrana colágena). Aos 30 dias pós-operatórios os animais foram sacrificados e após o processamento tecidual, foram realizadas análises histológica, histométrica e estatística. Os dados foram submetidos ao teste paramétrico ANOVA, seguido pelo teste de Tukey (p<0,05). As variáveis analisadas foram: área de osso neoformado (AON) e área de partículas residuais (APR). Todos os grupos apresentaram maiores quantidades de AON quando comparados ao grupo C (9,96% ± 4,49%). Quando o LBI foi associado aos biomateriais apresentou diferença estatística signicativa somente no grupo BO/L (48,57% ± 28,22%, p < 0,05). Menor APR foi encontrada nos grupos irradiados pelo LBI, com diferença estatística significativa para o grupo BO/L (16,74% ± 15,25%, p < 0,05). O LBI possui efeito positivo na regeneração óssea de DTC, e embora na análise estatística tenha apresentado diferença significante somente quando aplicado sobre o Bio-Oss®, na análise histomorfométrica foi possível observar formação óssea em extensões variadas nos demais grupos. / The purpose of the study was to evaluate the effect of low-level laser therapy (LLLT) on guided bone regeneration of critical size defects filled with autogenous bone or Bio- Oss® associated with the collagen membrane. The study was conducted in 120 critical size defects (5mm) in male rats (Rattus norvegicus, albinus, Wistar),weighing between 250 and 300g, divided into 12 experimental groups (n = 10): 1) Group C (control only blood clot); 2) Group M (collagen membrane- BioGide®); 3) Group LLLT (low-level laser therapy, GaAlAs, 808 nm, 100 mW, 6J, 210 J/cm2); 4) Group AB (autogenous bone); 5) Group AB/LLLT (autogenous bone + LLLT); 6) Group AB/M (autogenous bone + collagen membrane); 7); Group LLLT/M (LLLT + collagen membrane) 8) Group AB/LLLT/M (autogenous bone + LLLT + collagen membrane); 9) Group BO (Bio-Oss®); 10) Group BO/M (Bio-Oss® + collagen membrane) 11) Group BO/LLLT (Bio-Oss® + LLLT), 12) Group BO/LLLT/M Bio- Oss® + LLLT + collagen membrane). The animals were killed 30 days postoperatively. After tissue processing, bone regeneration was evaluated by histomorphometric and statistical analysis was performed (Tukey test, p<0,05). The variables analyzed were the area of newly formed bone (NFB) and residual particle area (RPA). All groups presented greater amounts of AON when compared to group C (9,96% ± 4,49%). When LBI was associated with biomaterials, it showed a significant statistical difference only in the BO/L group (48,57% ± 28,22%, p<0,05). Smaller APR was found in the groups irradiated by LBI, with a significant statistical difference for BO/L group (16,74% ± 15,25%, p<0,05). LBI has a positive effect on bone regeneration of DTC, and although in the statistical analysis it presented significant difference only when applied on Bio-Oss®, in the histomorphometric analysis it was possible to observe bone formation in varied extensions in the other groups.

Estudo de polimorfismos de MTOR e PPP3CA em receptores de transplante renal e sua relação com a resposta a imunossupressores / Study of MTOR and PPP3CA polymorphisms in renal transplant recipients and its relationship with the response to immunosuppressive agents.

Os imunossupressores tacrolimo (Tac) e sirolimo (Srl) são amplamente utilizados no transplante renal. Estes medicamentos apresentam estreita faixa terapêutica e estão associados a uma vasta gama de efeitos colaterais. Polimorfismos de nucleotídeo único (SNP) parecem ter um impacto significativo sobre a farmacocinética dos imunossupressores. Com o objetivo de avaliar a associação de SNP nos genes PPP3CA e MTOR com a resposta farmacológica dos imunossupressores tacrolimo e sirolimo foram selecionados 156 indivíduos indicados para transplante renal entre os pacientes atendidos no Hospital do Rim e Hipertensão da UNIFESP. Esses indivíduos foram tratados com esquema imunossupressor baseado em tacrolimo ou convertido para sirolimo. Amostras de sangue foram coletadas antes do transplante para extração de DNA. As determinações das concentrações sanguíneas de Tac foram determinadas por chemiluminescent microparticle immunoassay (CMIA) e as concentrações sanguíneas de Srl foram obtidas pela técnica de HPLC (High- Performance Liquid Chromatography). Os polimorfismos do MTOR (c.1437T>C, T c.2997C>T e c.4731G>A) e PPP3CA (c.249G>A) foram identificados por PCR em tempo real. O polimorfismo PPP3CA c.246G>A não foi associado à dose diária de tacrolimo ou sirolimo. Já a concentração sanguínea de tacrolimo foi menor nos portadores do alelo A no terceiro dia e terceiro mês de estudo. Os polimorfismos do MTOR foram relacionados à concentração sanguínea corrigida pela dose de tacrolimo. Os portadores dos alelos raros G, T e C dos polimorfismos c.4731G>A, c.14337T>C e c.2997C>T, respectivamente, apresentaram valores de Co/Do de tacrolimo menores em relação aos não portadores destes alelos. As diferenças significativas ocorreram principalmente nos primeiros três meses de estudo. A concentração sanguínea de tacrolimo foi no geral menor nos portadores dos alelos raros, sendo significativamente menor no décimo quarto dia. Doses maiores de tacrolimo foram associadas aos alelos T do c.14337T>C e C do c.2997C>T. No sexto mês de estudo, os portadores dos alelos raros receberam doses de sirolimo significativamente maiores do que os não portadores. O alelo T do polimorfismo c.1437T>C foi associado a menores valores de Co/Do de sirolimo. Os SNPs c.2997C>T e c.1437T>C do MTOR encontram-se em desequilíbrio de ligação (D\'=0,981; r2=0,690). Nos três primeiros meses de estudo, os portadores do haplótipo TC receberam doses menores de tacrolimo e apresentaram a melhor relação Co/Do. Foi possível observar que após a randomização, o haplótipo TC continuou associado a menores doses de tacrolimo e de sirolimo e manteve a tendência de melhores índices de Co/Do de ambos os fármacos. Os polimorfismos c.1437T>C e c.4731G>A foram associados a parâmetros de função renal no grupo TAC. O alelo G do SNP c.4731G>A relacionou-se a valores menores de ureia no pré-Tx, menor redução de ureia e creatinina entre o pré-Tx e o sexto mês de estudo. O alelo T do SNP c.1437T>C também foi relacionado a menores valores de ureia no pré-Tx e menor redução de creatinina. No grupo TAC, o alelo raro do SNP PPP3CA c.249G>A foi relacionado a menores valores de triglicérides no pré-Tx e no grupo SRL uma menor variação de LDL-colesterol. Os portadores do alelo C do SNP c.2997C>T apresentaram menor aumento de colesterol total e LDL colesterol entre o pré-Tx e o sexto mês de estudo, maiores valores de HDL colesterol no pré-Tx e menores valores de triglicérides no sexto mês de estudo. Os portadores do alelo T do SNP c.1437T>C apresentaram menor aumento de colesterol total, LDL colesterol, VLDL colesterol e triglicérides. No sexto mês de estudo apresentaram menores valores de triglicérides em relação aos não portadores deste alelo. Os portadores do alelo G do SNP c.4731G>A tiveram variação menor de colesterol total, VLDL colesterol e triglicérides. Não foi encontrada relação dos polimorfimos estudados e a rejeição aguda comprovada por biópsia ou com a nefropatia crônica do enxerto. Esses resultados são sugestivos de que os polimorfismos do MTOR e PPP3CA estão associados com a dose e concentração sanguínea dos imunossupressores tacrolimo e sirolimo, assim como um perfil lipídico menos aterogênico. / The immunosuppressant tacrolimus (Tac) and Sirolimus (Srl) are widely used in renal transplantation. These drugs have a narrow therapeutic range and are associated with a wide range of side effects. Single nucleotide polymorphisms (SNPs) have a significant impact on the pharmacokinetics of immunosuppressants. In order to evaluate the association of SNPs in genes MTOR and PPP3CA with the pharmacological response of immunosuppressive drugs tacrolimus and sirolimus were selected 156 individuals referred for kidney transplantation among patients treated in the Hospital do Rim e Hipertensão, UNIFESP. These individuals were treated with tacrolimus-based immunosuppressive regimen or converted to sirolimus. Blood samples were collected before transplantation for DNA extraction. Determinations of blood concentrations of Tac were determined by Chemiluminescent microparticle immunoassay (CMIA) and blood concentrations Srl were obtained by the technique of HPLC (High-Performance Liquid Chromatography). Polymorphisms of MTOR (c.1437T>C, T c.2997C>T and c.4731G>A) and PPP3CA (c.249G>A) were identified by real-time PCR. The Polymorphism PPP3CA c.246G>A was not associated with the daily dose of tacrolimus or sirolimus. The blood concentration of tacrolimus was lower in carriers of the allele on the third day and third month of study. The Polymorphisms of MTOR were related to blood concentration corrected by the dose of tacrolimus. The carriers of rare alleles G, T and C polymorphisms c.4731G> A, c.14337T> C and c.2997C> T, respectively, had values of Co/Do tacrolimus lower than the non-carriers of these alleles. Significant differences occurred mainly during the first three months of study. The blood concentration of tacrolimus was generally lower in carriers of the rare alleles being significantly lower on the fourteenth day. Higher doses of tacrolimus were associated with alleles c.14337T T>C and C c.2997C>T. In the sixth month of study, the carriers of rare alleles received doses of sirolimus significantly higher than non-carriers. SNPs c.2997C>T and c.1437T>C MTOR are in linkage disequilibrium (D \'= 0.981; r2 = 0.690). In the first three months of study, carriers of the TC haplotype received lower doses of tacrolimus and presented the best value for Co/Do. It was observed that after randomization, the TC haplotype remained associated with lower doses of tacrolimus and sirolimus and continued the trend of higher rates of Co/Do of both drugs. Polymorphisms c.1437T>C and c.4731G>A were associated with renal function parameters in the TAC group. The G allele of SNP c.4731G> A was related to lower levels of urea in the pre-Tx, a smaller reduction of urea and creatinine between the pre-Tx and sixth months of study. The T allele of SNP c.1437T>C was also related to lower levels of urea in the pre-Tx and a smaller reduction of creatinine. In the TAC group, the rare allele of SNP PPP3CA c.249G>A was related to lower levels of triglycerides in the pre-Tx and the SRL group a smaller variation of LDL-cholesterol. The C allele of the SNP c.2997C>T showed a lower increase in total cholesterol and LDL cholesterol between pre-Tx and sixth months of study, higher HDL cholesterol in pre-Tx and lower levels of triglycerides in the sixth month of study. The T allele of SNP c.1437T>C showed a lower increase in total cholesterol, LDL cholesterol, VLDL cholesterol and triglycerides. In the sixth month of the study, they had lower triglyceride levels compared to non-carriers of this allele. The G allele of SNP c.4731G>A change had lower total cholesterol, VLDL cholesterol and triglycerides. There was no relationship between the studied polymorphisms and biopsy-proven acute rejection or chronic allograft nephropathy. These results suggest that MTOR and PPP3CA polymorphisms are associated with dose and blood concentration of immunosuppressants tacrolimus and sirolimus, as well as a less atherogenic lipid profile.

Onicomicose é uma infecção crônica das unhas provocadas por fungos. É a doença mais comum das unhas em todo o mundo e constitui cerca de metade de todas as alterações ungueais em indivíduos imunocompetentes e imunossuprimidos. Nos últimos anos, vários estudos tem demonstrado aumento na prevalência de onicomicose. Esse aumento pode ser atribuído a diversos fatores, incluindo o aumento da expectativa de vida, e propagação do vírus da imunodeficiência humana (HIV). Nesse sentido, o número de pacientes renais crônicos vem aumentando em todo mundo. No entanto, a literatura relata poucos estudos sobre a prevalência e as características das onicomicoses nestes pacientes. Diante disso, nossos objetivos foram determinar a prevalência, etiologia e perfil de suscetibilidade das onicomicoses provenientes de pododáctilos de pacientes dialíticos (DL), receptores de transplante renal (RTR) e pacientes grupo controle. Foram examinados 510 pacientes, sendo 336 provenientes da Unidade de Transplante Renal (149 DL e 187 RTR), e 174 pacientes do Ambulatório de Clínica Geral, sem histórico de doença associada à imunossupressão (Grupo controle). Os isolados foram identificados por testes fenotípicos e moleculares. O perfil de suscetibilidade foi realizado pelo método de microdiluição em caldo frente aos fármacos fluconazol, itraconazol, voriconazol e terbinafina. A prevalência de onicomicose foi de 23,4% nos pacientes DL e 23,0% nos RTR, significativamente maior que no grupo controle (13,2%). Nos pacientes DL, onicomicose foi associada com diabetes, mas não com o tempo de diálise e sexo. Nos pacientes RTR, onicomicose foi mais prevalente em homens, mas não houve associação com duração do transplante, diabetes e protocolo de tratamento imunossupressor. Trichophyton rubrum foi a agente mais prevalente (45,9%), seguido por T. mentagrophytes (24,5%) e Candida parapsilosis (18%). Todos antifúngicos foram eficazes frente aos isolados de dermatófitos, sendo terbinafina o mais eficiente. Os isolados de C. parapsilosis foram todos sensíveis aos quatro antifúngicos de acordo com os atuais \"end-points\". Nossos achados permitiram concluir que pacientes renais (DL e RTR) tem risco aumentado de desenvolver onicomicose. Nossos dados também revelaram que a etiologia e a suscetibilidade das espécies isoladas são comparáveis aos encontrados em outros grupos de pacientes não renais descritos na literatura. / Onychomycosis is a chronic fungal infection of the nail. It is the most common disorder of nails worldwide and constitutes about 50% of all nail changes of the immunosuppressed and healthy patients. In recent years, several reports have shown increased prevalence of onychomycosis. This increase can be attributed to several factors, including increased life expectancy, and to the human immunodeficiency virus (HIV) epidemics. In this regard, the number of chronic renal patients is also increasing worldwide. However, the literature reports few studies on onychomycosis in these patients. Therefore, our objectives were to determine the prevalence, etiology and susceptibility profile of onychomycosis of the toenail of patients undergoing hemodialysis treatment (HD), kidney transplant recipients (KTR) and control patients. Were examined 510 patients, 336 attending the Renal Transplantation Unit (187 KTR e 149 HD) and 174 patients attending an internal medicine outpatient service with diseases other than renal disease (control group). The isolates were identified by phenotypic and molecular tests. Antifungal susceptibility tests were performed using a broth microdilution method against the antifungal drugs voriconazole, itraconazole, fluconazole and terbinafine. The prevalence of onychomycosis in HD patients (23.4%) and KTR (23.0%) was significantly higher than control group (13.2%). In HD patients, onychomycosis was associated with diabetes but not duration of dialysis and gender. In KTR, onychomycosis was more prevalent in males, but not associated with duration of transplantation, diabetes or immunosuppressive protocols. Trichophyton rubrum was the most prevalent species (45.9%) followed by T. mentagrophytes (24.5%) and Candida parapsilosis (18%). While all antifungals were efficient against the dermatophyte isolates, terbinafine was the most effective. All C. parapsilosis isolates were sensitive to the antifungals according to the current end-points. In conclusion, this study shows that HD patients and KTR are at high risk of contracting onychomycosis. Our data also show that the etiology and susceptibility of the species are comparable with those found in other groups of non-renal patients described in the literature.

Avaliação da influência de fatores clínicos e radiográficos como preditivos no índice de sucesso de 144 transplantes dentários autógenos / Evaluation of the influence of clinical and radiographic factors as predictive of the success rate in 144 autogenous dental transplants

O presente estudo avaliou uma casuística constituída por 144 transplantes dentários autógenos realizados em 128 pacientes, submetidos a controles periódicos de 6 meses a 23 anos. O objetivo do trabalho foi avaliar a influência de fatores clínicos e radiográficos no índice de sucesso dos transplantes realizados. Clinicamente avaliou-se a alteração na coloração dentária, vitalidade pulpar, comprometimento periodontal, tipo dentário transplantado e sítio receptor. Radiograficamente foram avaliados a presença ou não de reabsorção radicular, anquilose dento-alveolar, alterações no compartimento pulpar, complementação do desenvolvimento radicular, presença de periapicopatia persistente e relação coroa/raiz. Os dados foram coletados, analisados e submetidos a cálculos estatísticos pertinentes. As reabsorções dentárias, a anquilose dentoalveolar, a proporção coroa/raiz, a presença de periapicopatia persistente, o desenvolvimento de bolsa periodontal profunda e a mobilidade dentária destacaram-se de forma significante estatisticamente P<0,05. O tipo dentário doador, o sítio receptor, as alterações na coloração dentária e o desenvolvimento radicular na época da cirurgia do transplante não alcançaram índices de significância relacionados ao insucesso dos casos. A presença de processos esclerosantes pulpares mostrou-se comum aos transplantes bem sucedidos de dentes com rizogênese incompleta. Estes achados contribuem para o desenvolvimento de um protocolo terapêutico bem fundamentado. / The present study examined a sample consisting of 144 autogenous dental transplants performed in 128 patients, subjected to periodic follow-up from 6 months to 23 years. The objective was to evaluate the influence of clinical and radiographic factors in the success rate of the performed transplants. Clinical evaluation aimed the change in tooth color, pulpal vitality, periodontal involvement, transplanted tooth type and recipient site. Radiographic evaluation aimed the presence or absence of root resorption, ankylosis, pulpal changes, complementation of the root development, presence of persistent periapical lesions and crown-root ratio. The data were collected, analyzed and submitted to the relevant statistical calculations. Tooth resorptions, ankylosis, crown to root proportion, presence of persistent periapical lesions, development of periodontal pocket depth and tooth mobility was highlighted as statistically significant P <0.05. The donor tooth type, the receptor site, changes in tooth color and root development at the time of the transplant surgery did not reach levels of significance related to the failure of the cases. The presence of pulpal sclerosis processes shown to be common to successful transplantation of teeth with incomplete root formation. These findings contribute to the development of a reliable therapeutic protocol.

Trata-se de uma pesquisa que utilizou a revisão integrativa da literatura que objetivou as possíveis intervenções de enfermagem para os familiares de clientes transplantados de medula óssea. Realizou-se a busca de artigos e teses nas bases de dados eletrônicas Lilacs e Medline (período de 01/01/1990 a 05/05/2005), utilizando-se as palavras: “bmt" e “family" e “bone marrow transplantation" e “family" respectivamente. Foram identificadas duas publicações no Lilacs, sendo que apenas uma foi pertinente ao tema proposto e 807 publicações no Medline, das quais apenas 24 abordavam o tema proposto, a amostra foi constituída por 25 publicações. Para análise dos artigos utilizou-se um instrumento para identificação das características dos artigos, níveis de evidências, delineamento da pesquisa, características da crise familiar, fatores que influenciam a habilidade das famílias para enfrentar e se adaptar a uma crise e, por fim, intervenções de enfermagem propostas. Foi utilizado o modelo de crise proposto por Hill e Hansen que classifica a crise em quatro categorias: características do evento, ameaças percebidas, avaliação dos recursos da família e experiência passada em crises. Os resultados mostraram que 96% dos artigos foram obtidos do Medline, sendo que 12 (48%) artigos foram desenvolvidos por enfermeiros e os demais por outros profissionais. Em relação ao tipo de revista, 36% dos artigos foram publicados em periódicos específicos de enfermagem, sendo que 72% provém dos EUA, 16% da Grécia, Canadá, Polônia e Brasil (distribuídos igualmente) e 12% não foi possível identificar a procedência. As evidências clínicas variaram entre os níveis 4(64%), 5 (4%) e 6 (28%), e mostraram a importância da comunicação efetiva e clara entre os membros familiares, equipe e paciente para minimizar a ansiedade e depressão; que os tipos específicos de família, suas características, coesão, poucos conflitos, orientações culturais e intelectuais e ênfase religiosa promovem maior suporte emocional aos mesmos e que os grupos de apoio e redes de apoio minimizam a ansiedade, angústia e a depressão vivenciada pelo paciente e sua família. Quanto às intervenções percebe-se que a maioria está voltada para os aspectos psicológicos e sociais, sendo relevantes a ação do enfermeiro nas propostas de estratégias de enfrentamento para o familiar e o paciente frente às diversas fases do TMO. As intervenções propostas na literatura vão ao encontro das sugeridas pela NIC para os diagnósticos de enfermagem “processos familiares interrompidos e enfrentamento familiar comprometido"; em especial o suporte emocional (64%), suporte familiar (36%), redução da ansiedade (28%), grupo de apoio (32%), melhora do enfrentamento (40%) e assistência quanto aos recursos financeiros (24%). Apreende-se que outras intervenções sugeridas pela NIC poderão ser desenvolvidas pelos enfermeiros. / This study presents an integrative literature review of possible nursing interventions for family members of bone marrow transplant (BMT) patients. We looked for articles and theses in the electronic databases Lilacs and Medline (from 01/01/1990 to 05/05/2005), using the following keywords: “bmt" and “family" and “bone marrow transplantation" and “family" respectively. Two publications were identified in Lilacs, one of which was relevant for the proposed subject, against 807 publications in Medline, 24 of which dealt with the proposed theme. Thus, the sample consisted of 25 publications. In the analysis, we used an instrument to identify the articles’ characteristics, levels of evidence, research design, family crisis characteristics, factors influencing the families’ ability to deal with and adapt to a crisis and, finally, proposed nursing interventions. The crisis model by Hill and Hansen was used, which classifies a crisis into 4 categories: event characteristics, perceived threats, family resources and past crisis experience. The results evidenced that 96% of the articles came from Medline, 12 (48%) of which were developed by nurses and the remainder by other professionals. What the type of journal is concerned, 36% of the articles were published in specific nursing periodicals, 72% of which were from the USA, 16% from Greece, Canada, Poland and Brazil (equal distribution) and, for 12%, the origin could not be identified. Clinical evidence varied between levels 4 (64%), 5 (4%) and 6 (28%) and demonstrated the importance of effective and clear communication between family members, team and patient with a view to minimizing anxiety and depression; that specific family types, their characteristics, cohesion, few conflicts, cultural and intellectual orientations and religious emphasis provide greater emotional support and that support groups and networks minimize the anxiety, anguish and depression patients and relatives experience. Most interventions are aimed at psychological and social aspects. Nursing actions are relevant in strategies proposed for family members and patients to cope with the different phases of BMT. The interventions proposed in literature are in accordance with suggested NIC interventions for the nursing diagnoses “interrupted family processes and compromised family coping"; especially emotional support (64%), family support (36%), anxiety reduction (28%), support group (32%), improved coping (40%) and financial aid (24%). We believe that nurses can develop other NIC interventions.

Defining the mechanisms by which lenalidomide can modulate the human T cell alloresponse to improve the outcome of allogeneic haematopoietic stem cell transplantation

Immunomodulatory drugs (IMiDs) could enhance both direct anti-tumour and graft-versus-tumour effects after allogeneic haematopoietic stem cell transplantation (AHSCT). However, clinical experience with IMiDs after AHSCT using adult peripheral blood (APB) as a stem-cell source has been limited by graft-versus-host disease. Characterization of the mechanisms by which IMIDs modulate alloresponses of T cells and identification of differential effects on T cells from different cell sources could facilitate more effective use of these drugs in the setting of AHSCT. Using in vitro modelling, multi-parameter flow cytometry and gene expression analysis, I have determined the impact of the widely used IMiD lenalidomide on alloresponses of APB and umbilical cord blood (UCB)-derived T cells. Lenalidomide-treatment potentiates net alloproliferation of APB-derived T cells by selectively enhancing proliferation of CD8+ T cells. These CD8+ T cells have enhanced effector memory differentiation, are enriched for polyfunctional effectors, have enhanced direct-cytotoxicity against heamatopoietic target-cells and have a distinct gene expression profile with altered expression of key immunoregulatory-genes and depletion of cellular ikaros. Importantly, while effects on CD8+ T cells derived from UCB are similar, lenalidomide has contrasting effects on allospecific proliferation of APB and UCB-derived CD4+ T cells. While lenalidomide-treatment has no effect on alloproliferation of APB-derived CD4+ T cells, it reduces alloproliferation of UCB-derived CD4+ T cells. The reduction in UCB-derived CD4+ T cell alloproliferation is accompanied by selective expansion of CD4+CD25+FOXP3+ regulatory T cells (Treg), resulting in an overall reduction in UCB-derived T cell alloproliferation. These findings demonstrate that lenalidomide has a differential impact on alloresponses of T cells from different cell sources; alloresponses of APB-derived T cells are increased via selective expansion of polyfunctional CD8+ effectors, while alloresponses of UCB-derived T cells are limited by expansion of tolerogenic Treg. These findings have important implications for the future use of IMiDs in the setting of AHSCT.

Resposta tecidual ao enxerto xenógeno inorgânico de osso bovino : avaliação histomorfológica e histomorfométrica /

Resumo: Proposição: Avaliar a resposta tecidual ao enxerto xenógeno inorgânico de osso bovino, por meio de análise histomorfológica e histomorfométrica. Material e método: O enxerto foi obtido da costela de boi resfriada após 8 horas de sacrifício do animal e processado da seguinte maneira: imersão por 60 minutos em água oxigenada a temperatura ambiente, com trocas a cada 15 minutos; imersão em água oxigenada aquecida, a uma temperatura de 100ºC, por 30 segundos e resfriamento imediato com água destilada a uma temperatura variando de 6º a 8ºC; enxague em água destilada corrente por 6 horas; imersão em álcool 70º, 90º e 100º, respectivamente, sendo 15 minutos em cada álcool; esterilização em autoclave. Em seguida, 10 coelhos brancos (Nova Zelândia) foram utilizados, originando dois grupos: Grupo I - enxerto inserido no pavilhão auricular e Grupo II - enxerto inserido e fixado em tíbia. Decorridos 180 dias, foi realizado sacrifício dos animais por injeção excessiva de anestésico. Resultados: No Grupo I, o enxerto apresentou-se em íntimo contato com tecido subcutâneo. No Grupo II, o enxerto apresentou-se em justaposição com o leito ósseo receptor. E nos dois grupos havia espaços medulares preenchidos por medula óssea ativa e osso neoformado, além da ausência de osteócitos no osso bovino e presença destas células no osso neoformado, próximo aos espaços medulares. Conclusão: Em razão dos resultados obtidos o enxerto xenógeno inorgânico de osso bovino demonstrou biocompatibilidade, capacidade osteocondutora e discreta perda de volume, preenchendo requisitos importantes do material ideal para reconstrução óssea. / Abstract: Objective: To evaluate tissue response to inorganic bovine bone xenograft by histomorphological and histomorphometrical analyses. Material and methods: The bone grafts was obtained from cooled bovine rib 8 hours after slaughtering and were processed as follows: 60-minute immersion in hydrogen peroxide at room temperature, with changes at 15-minute intervals; 30-second immersion in hydrogen peroxide at 100ºC; immediate cooling with distilled water at temperatures ranging from 6º to 8ºC; rinsing in running distilled water for 6 hours; immersion in alcohol grades of 70º, 90º and 100ºGL (15-minute in each solution); autoclaving. Thereafter, 10 white New Zealand rabbits were distributed into two groups: Group I - graft inserted in the ear pavilion and Group II - graft inserted and fixed in the tibia. After 180 days, the animals were sacrificed by anesthetic overdose. Results: In Group I, the graft was in intimate contact with the subcutaneous tissue, while in Group II the graft was juxtaposed to the recipient site. Both groups presented medullar spaces filled with active bone marrow and newly formed bone. In addition, osteocytes were absent in the bovine bone graft and present in the newly formed bone, close to the medullar spaces.
