Clinical application of cross-education to unilateral limb immobilization

Cross-education is a neural adaptation defined as the increase in strength or functional performance of the untrained contralateral limb after unilateral training of the opposite homologous limb. Since cross-education can improve strength in an untrained limb, there is therapeutic potential to apply cross-education to clinical rehabilitation settings; however, a large gap in the literature remains. The first objective of this thesis was to determine if cross-education could improve strength and functional performance (i.e. active range of motion (AROM), self-reported function) of an immobilized limb using a shoulder sling model in both healthy and injured participants. The second objective was to determine if cross-education could improve strength and functional performance (i.e. AROM, self-reported function) of wrist fracture rehabilitation after unilateral training of the non-fractured limb. Study 1 applied cross-education to non-injured participants who wore a shoulder sling and swathe and strength trained the non-immobilized limb. Strength (NORM dynamometer), muscle size (ultrasound), electromyography, and interpolated twitch were measured. Results showed cross-education increased strength and maintained muscle size in the immobilized limb after training the non-immobilized limb. Study 2 applied cross-education using a clinically relevant at-home resistance tubing shoulder strength training program to healthy participants. Results showed significant cross-education effects for untrained shoulder external and internal rotation strength (handheld dynamometer), and increased muscle size (ultrasound) in the trained supraspinatus and anterior deltoid. Study 3 applied cross-education using the clinically relevant strength training program (in Study 2) to post-shoulder surgery rehabilitation and measured strength (handheld dynamometer), muscle size (ultrasound), AROM (goniometer), and self-reported function (Western Ontario Rotator Cuff Questionnaire) (WORC). Results showed the training group had significantly greater supraspinatus muscle thickness at 6 months post-surgery compared to the control group; however, there were no cross-education effects for strength, AROM, or the WORC. Study 4 applied cross-education during rehabilitation from wrist fractures and measured strength (handgrip dynamometer), AROM (goniometer), and self-reported function (Patient Rated Wrist Evaluation) (PRWE). Results showed cross-education improved strength and AROM in the fractured limb 12 weeks post-fracture. In conclusion, there was evidence for cross-education to benefit a healthy immobilized limb and to use a clinically relevant shoulder strength training program to produce cross-education effects. When cross-education was applied to shoulder surgeries there were improvements in muscle size but no effect for strength, AROM or function. However, when applied to wrist fractures, strength and AROM were improved for the injured limb. These findings represent the first well-controlled evidence that cross-education may improve rehabilitation after unilateral injuries.

Is there a sex difference in the bilateral deficit?

The bilateral deficit can be described as when the sum of two unilateral movements exceeds the bilateral output. If the sum does not exceed the bilateral output, a bilateral facilitation is present. Little research has been done in males and females comparison of the bilateral deficit. The purpose of the current study was therefore to compare the bilateral deficit between sexes in a 1 repetition maximum (RM) leg press. Twenty participants, ten males and ten females, were recruited (23.4±2.1 years, 71.2±13.1 kg, 172.2±8.9 cm) and seventeen of them completed the study. The test consisted of two testing sessions (one unilateral with preferred leg, and one bilateral session) with seven days in between. The 1 RM was attained within six attempts, with a three minutes rest between every attempt. The current study showed a bilateral deficit of males (5.6±4.6%) and a bilateral facilitation of females (0.6±5.9%). There was a significant difference between sexes of p=0.031 (p<0.05). Previous studies have examined males and females separately, with different methods and protocols. This study used the same method and protocol in both sexes. Some suggestions of the mechanisms behind the deficit have been proposed and may explain the current findings, but further studies are needed. Future research needs to continue to explore and explain the sex difference in the bilateral strength deficit.

Investigation of vertical mandibular distraction osteogenesis on the masticatory muscles in a ’unilateral hemifacial microsomia like’ defect in the sheep model.

Distraction osteogenesis is a recently developed option for surgical correction of the craniofacial discrepancy but there are few studies that look at the long-term effect of the relapse phenomenon. This study was conducted to look at the bone and muscle response at different consolidation and remodelling periods. Thirty ten-week-old Merino lambs were subjected to this current study. The surgically created defect was performed on the experimental side (right) by superficial masseter myectomy and a condylectomy. The lambs showed a midline shift to the affected side three months later. Marker screws were placed on both sides of the mandible to examine the adjustment of the vertical and horizontal dimension of the mandible. In this present study, the left side was used as the control. The affected mandible was surgically corrected using a vertical ramus distraction osteogenesis protocol. The latency period was 7 days where the device was inactive. Distraction was then performed at 1 mm per day until the distracted gap was 10 mm on a radiograph. Initially, the sheep were divided into 8 groups but later the groups were refined into 6 groups for statistical analysis; Group 1: Immediately post distraction Group 2: Consolidation 2 months and remodelling 1 month Group 3: Consolidation 3 months Group 4: Consolidation 3 months and remodelling 1 month Group 5: Consolidation 3 months and remodelling 2 months Group 6: Consolidation 4 months A ‘hemifacial microsomia like’ defect was successfully created and it was then corrected using vertical ramus distraction. Cephalometric examination showed that the vertical height was significantly increased after the distraction was completed. Examination of the bone and muscle was performed to look at the adjustment of bone structure in relation to relapse. The bone investigation was conducted using radiological analysis, histological analysis and direct measurement of the vertical screws at pre-distraction and at sacrifice as well as histomorphometric analysis. The radiological examination was conducted using cephalometric analysis of the distance between marker screws on both experimental and control sides. Bone histology was investigated on the middle and posterior position of the experimental side. The histomorphometric analysis was conducted using Quantiment analysis software. The muscle responses and adaptation were investigated by measuring: the weight of the masseter and medial pterygoid muscles; length between 6 different points (4 landmarks); cross section and thickness of masseter muscles by ultrasound and by histopathological examination of both masseter and medial pterygoid muscles on the experimental and the control sides. The distracted bone showed a completion of maturation after a three month consolidation period. Bone formation was shown to continue after longer consolidation periods. A two-month consolidation period was insufficient time for the bone to consolidate and stable, as there was evidenced of relapse during this period. There was no increased in muscle mass after distraction. The distraction altered the length of anterior and posterior planes and the size of cross sectional area and thickness of origin and middle level of masseter muscles. There were no changes in the length of the middle and oblique planes and the cross sectional area of the insertion of the masseter muscle. The distracted muscle adapted well after a longer consolidation and remodelling period but was sensitive to any surgical procedure such as device removal. The adjustment was observed to continue within the first month after surgical removal of device. The weight, length of planes, cross section and thickness was temporarily reduced on the experimental side but continued to improve and stabled during the second month after removal of the device. The first month after device removal also showed that histopathological activity was increased after both 2 and 3 month consolidation periods. Importantly, muscle histopathology was back to almost normal activity after the second month of device removal, following a 3 month consolidation period. This study showed that it was possible to create a ‘hemifacial microsomia like’ defect in very young lambs. The defect was then successfully corrected by a vertical distraction osteogenesis procedure. It was also shown that the sufficient time for the device to be fixed in place (consolidation period) was 3 months or longer as the bone and muscle was stable after that period of time. The Effect of Unilateral Load Carriage on the Muscle Activities of the Trunk and Lower Limbs of Young Healthy Males during Gait

The aim of the study was to examine the muscle activities of fifteen male participants (23.44 ±2.63 years) during unilateral hockey bag load carrying of different weights (10%, 20%, and 30% bodyweight) and sizes (small and large). Walking without a hockey bag was the control condition. The results showed that increased peak and integrated EMG occurred with an increased load weight in the semitendinosus, gastrocnemius, rectus abdominis, and vastus medialis. The left rectus femoris and left semitendinosus were both significantly greater than the right corresponding muscle. Carrying the large hockey bag produced greater peak EMG in the right rectus abdominis and the right rectus femoris, whereas the right vastus medialis showed a larger peak EMG in the small hockey bag. It was concluded that the posterior-lateral carrying style of hockey bag load carriage explained the results being similar to both backpack and side pack load carriage studies.

As computer generated special effects play an increasingly integral role in the development of films and other media, simulating granular material continues to be a challenging and resource intensive process. Solutions tend to be pieced together in order to address the complex and different behaviors of granular flow. As such, these solutions tend to be brittle, overly specific, and unnatural. With the introduction of a holistic 3-phase granular simulation, we can now create a reliable and adaptable granular simulation.Our solution improves upon this hybrid solution by addressing the issue of particle flow and correcting interpenetration amongst particles while maintaining the efficiency of the overall simulation. We achieve this by projecting particles onto a 2D manifold and implementing density correction using a quadratic solver. Particle updates are projected back into 3D to spread the particles apart on each frame.Keywords:

Bakgrund: Unilateral benträning är ett relativt outforskat område inom träningsvetenskapen. Traditionellt genomförs fysträning för intermittenta idrotter bilateralt, trots att idrottens rörelser sker unilateralt. Det finns många studier som behandlar samband mellan agility och effektutveckling, men få som har undersökt skillnaden mellan unilateral och bilateral effektutveckling sett till agility. Syfte: Syftet med denna studie var att undersöka om unilateral effektutveckling har en starkare korrelation med agility än vad bilateral effektutveckling och agility har hos kvinnliga fotbollsspelare i division 2. Metod: Kvinnliga fotbollsspelare (n=13, ålder 23 ± 4 år) utförde vertikalhopp med tilläggsbelastning på båda benen, vänster ben och höger ben på 20 kg, 30 kg, 40 kg samt för båda benen på 50 kg och T-test och Pro agility test. Effektutveckling i förhållande till kroppsvikten på båda benen (PBbw), vänster ben (PVbw) och höger ben (PHbw) analyserades och korrelerades sedan mot T-test (Ttest) och Pro agility test (Proagility). Resultat: Signifikanta korrelationer hittades mellan agility och både vänster unilateral (PVbw-Ttest, r = -0,578) och bilateral (PBbw-Ttest, r = -0,741; PBbw-Proagility, r = -0,555) effektutveckling, där korrelationen bilateralt var starkast. Ingen signifikant korrelation hittades mellan höger ben och agility. Konklusion: Bilateral effektutveckling hade en starkare korrelation med agility än vad unilateral effektutveckling och agility hade hos kvinnliga fotbollsspelare i division 2. / Background: Unilateral training is considered to be a new field of interest within the science of sports. Traditionally most of the strength and conditioning training is performed bilaterally even though the majority of motions in sports are carried out unilaterally. Previous studies have examined the correlations between agility and power output but few of them investigated the difference between unilateral and bilateral power output and agility. Objective: The aim of this study was to examine whether the correlation between unilateral power output and agility is stronger than the correlation between bilateral power output and agility in female division 2 soccer players. Method: Female soccer players (n=13, age 23 ± 4 years) performed loaded counter movement jumps with 20 kg, 30 kg and 40 kg for both legs, left leg and right leg and with 50 kg for both legs and T-test and Pro agility test. Power output, in relation to body weight for both legs (PBbw), left leg (PVbw) and right leg (PHbw) was analyzed and correlated against T-test (Ttest) and Pro agility test (Proagility). Results: Significant correlations were found between agility and both left unilateral (PVbw-Ttest, r = -0,578) and bilateral (PBbw-Ttest, r = -0,741; PBbw-Proagility, r = -0,555) power output, where the bilateral correlation was the strongest. No significant correlation was found between the right leg and agility. Conclusion: Bilateral power output had a stronger correlation with agility than unilateral power output and agility in female division 2 soccer players.

A consistência e coerência foram analisados por meio de α-Cronbach, confiabilidade interobservador por meio do Kappa e a correlação entre as escalas de CBS, BIT mRS, NIHSS e Barthel por meio do coeficiente de Pearson. Resultados: Foram avaliados 22 pacientes sendo encontrado consistência e coerência de  = 0,913, obtendo excelente consistência. Em análise de reprodutibilidade interobservador através do Coeficiente de Kappa, a escala para os 10 itens, mostrou uma confiabilidade razoável e alta entre os avaliadores. A CBS apresentou correlação negativa em comparação com á BIT, sendo que a taxa de concordância interobservador não diferiu significativamente entre as escalas. O resultado da correlação de Pearson realizada entre CBS e BIT foi de 0.03 para o avaliador 1 e 0.03 para o avaliador 2. Além da baixa correlação citada, não obtivemos boa correlação entre os demais escores das escalas de NIHSS, Barthel e Rankin, analisados através do coeficiente de Pearson. Conclusão: A CBS é adequada e validada para estudar pacientes com NEU após AVC em uma população brasileira, multicultural e de língua portuguesa. / Introduction: spatial unilateral neglect (SUN) is a prevalent clinical manifestation in stroke patients, however its diagnosis is difficult, also mensuration and repercussion in patients’ life lacks evaluation methods. Catherine Bergego’s scale (CBS) has the objective of evaluate the impact in quotidian life. There is no validation fo r this scale in Brazilian population. Objective: evaluate confiability, comprehension and acceptation of the Portuguese version of CBS for patients with clinical diagnosis of SUN after stroke. Methods: CBS was translated, culturaly adapted and applied by two independent investigators. It’s a transversal study that evaluated 22 patients with stroke diagnosis confirmed by computed tomography, and a previous score in modified Rankin (mRs) scale of 0. Besides CBS there was application of the National Institute of Health Stroke Scale (NIHSS) at patients’ admission. Together with the CBS the Behaviour Innatention Test (BIT) was applied for NEU diagnosis, and Bartel scale for independency in daily life. Consistency and consistency were analyzed using α-Cronbach, inter-observer reliability using Kappa and the correlation between the CBS, BIT mRS, NIHSS and Barthel scales using the Pearson coefficient. Results: Twenty-two patients were evaluated and was observed consistency and coherence of  = 0.913. In the intra-observer reproducibility analysis using the Kappa Coefficient, the scale for the 10 items showed a reasonable and high reliability among the evaluators. CBS had a negative correlation compared to BIT, and the inter-observer agreement rate did not differ significantly between scales. The result of the Pearson correlation between CBS and BIT was 0.03 for the evaluator 1 and 0.03 for the evaluator 2. In addition to the low correlation, we did not obtain a good correlation between the NIHSS, Barthel and Rankin, analyzed using the Pearson coefficient. Conclusion: CBS is adequate and validated to study patients with SUN after stroke in a Brazilian, multicultural and of Portuguese language.

Para tais componentes, o dimensionamento de juntas aparafusadas torna-se complexo, visto que a disposição dos parafusos e a geometria do mancal conduzem ao surgimento de esforços excêntricos à face de fratura, elevando a criticidade do projeto. Em vista disso, e do reduzido número de publicações sobre o assunto, o presente trabalho propôs uma “abordagem híbrida” para a análise de juntas aparafusadas em bielas anguladas, utilizando resultados obtidos a partir de um modelo de elementos finitos simplificado e as ferramentas contidas no roteiro VDI 2230 parte 1. Além disso, foi avaliada a influência do fenômeno de abertura unilateral sobre a vida em fadiga dos fixadores utilizados em bielas anguladas, analisando os resultados obtidos de modo a desenvolver um critério de avaliação para a fadiga desses componentes. A despeito das restrições, a abordagem numérico-analítica desenvolvida demostrou-se atraente, tanto pelo baixo custo computacional associado, quanto por sua precisão, principalmente na determinação da força tensora residual mínima para a prevenção de abertura, mostrando-se adequado para processos de otimização multiparamétricos, selecionando designs promissores passíveis de posterior avaliação através de modelos refinados. Já as análises de influência do fenômeno abertura não apresentaram alterações sensíveis nos níveis de força adicional dos fixadores, fato esse relacionado com a posição dos pontos de abertura em relação ao parafuso e ao mancal. Apesar disso, as simulações realizadas demonstraram que as condições de carregamento avaliadas não influenciaram no diagrama de Haigh local dos nós críticos de cada fixador, o que sugere o uso de resultados extraídos de ensaios de fadiga como variáveis de entrada para simulações em juntas de bielas, incluindo assim as influências benéficas do processo de manufatura. / Connecting rods screws maintain the connection between cap and shank supporting the tensile, compressive and transversal loads applied on the big end. In this case, the design of bolted joints become complex since the screw’s position and bearing geometry lead to eccentric forces acting on the joint split line. Due to that and the reduced number of publications on the subject, this dissertation aims to develop a "hybrid approach" for the analysis of bolted joints in angled connecting rods, using results from a simplified finite element model as input for the VDI 2230 part 1 guideline. In addition to that, this study evaluates the influence of unilateral opening phenomenon on fasteners fatigue life, analyzing the results in order to develop a criterion for fatigue analysis of connecting rod bolts. Despite the restrictions, the “hybrid approach” proved to be attractive due to the low computational cost and accuracy, especially for the calculation of minimal residual tensile force to prevent opening, being suitable for multiparametric optimization during project early phases. The opening phenomenon showed no significant changes in fastener’s additional forces, fact related to the position of joint opening points. Nevertheless, the simulations showed that the loading conditions did not influence on the local Haigh diagram of critical nodes, which suggests the use of fasters fatigue tests as input variables for fatigue simulations, thus including the beneficial influences of the manufacturing process.

(Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Introduction: spatial unilateral neglect (SUN) is a prevalent clinical manifestation in stroke patients, however its diagnosis is difficult, also mensuration and repercussion in patients’ life lacks evaluation methods. Catherine Bergego’s scale (CBS) has the objective of evaluate the impact in quotidian life. There is no validation fo r this scale in Brazilian population. Objective: evaluate confiability, comprehension and acceptation of the Portuguese version of CBS for patients with clinical diagnosis of SUN after stroke. Methods: CBS was translated, culturaly adapted and applied by two independent investigators. It’s a transversal study that evaluated 22 patients with stroke diagnosis confirmed by computed tomography, and a previous score in modified Rankin (mRs) scale of 0. Besides CBS there was application of the National Institute of Health Stroke Scale (NIHSS) at patients’ admission. Together with the CBS the Behaviour Innatention Test (BIT) was applied for NEU diagnosis, and Bartel scale for independency in daily life. Consistency and consistency were analyzed using α-Cronbach, inter-observer reliability using Kappa and the correlation between the CBS, BIT mRS, NIHSS and Barthel scales using the Pearson coefficient. Results: Twenty-two patients were evaluated and was observed consistency and coherence of  = 0.913.
