Évaluation constructiviste de l’application du Guide des meilleures pratiques de soins aux endeuillés auprès des familles ayant vécu une perte périnatale

Cette étude avait pour but d’évaluer, à partir d’un processus de co-construction avec les personnes concernées, dans un contexte de 1ère ligne, la mise en application d’interventions infirmières inspirées du Guide des meilleures pratiques de soins pour les endeuillés (GMPSE) auprès d’un couple ayant vécu une perte périnatale au cours des six dernier mois. Un devis de recherche d’étude de cas basé sur la démarche d’évaluation de la quatrième génération de Guba et Lincoln (1989) a été utilisé. Une infirmière expérimentée auprès des familles endeuillées, s’est inspirée de la guidance du GMPSE pour intervenir auprès d’un couple lors de cinq rencontres thérapeutiques, dont quatre ont été précédées d’une entrevue avec les personnes concernées . Ces entrevues ont permis à ces personnes d’identifier ensemble les interventions les plus utiles et les moins utiles. Le verbatim des rencontres et entrevues ont été enregistrées et transcrites à des fins d’analyses qualitatives, Les résultats de ces analyses font ressortir la pertinence des interventions inspirées du GMPSE et l’apport spécifique de la pratique infirmière auprès de la population visée. Il appert que la sensibilisation des décideurs et des cliniciens aux enjeux des personnes endeuilles soit nécessaire pour favoriser l’implantation du Guide dans les milieux de soins. Enfin, une meilleure appropriation du GMPSE est recommandée autant dans le cadre de la formation, que de la recherche et de la pratique en sciences infirmières. / The purpose of this study was to evaluate from a constructivist perspective, partnering with the stakeholders , the usefulness of nursing interventions inspired from The Best Practice Guidelines in Bereavement Care (BPGBC) in a first line context, during the follow up a couple whom had experienced a prenatal lost in the last six months. A case study design was used along with The Fourth Evaluation’s methodological approach designed by Guba and Lincoln (1989). An experienced nurse in the care of bereaved families, used the BPGBC to guide her interventions in five therapeutic meetings with the couple, followed by four interviews with the stakeholders. The interventions considered the most helpful and the less useful were identified. The gathering of the data and their analysis followed an interactive and ongoing process. The results support the pertinence of the nursing interventions inspired from the BPGBC, as well as the specific contribution of the nursing profession in the bereavement work. Implementing the BPGBC becomes a realistic goal when the managers and the clinical professionals are made aware of the issues encompassed by the grieving population. The nursing profession could incorporate the BPGBC in its training curriculum, in the research field and in the care offered to the bereaved population in different care settings.

A partir do redirecionamento do modelo assistencial no Brasil emerge a necessidade de discussão da reinvenção de práticas alinhadas com o cenário de transformações. As práticas agora devem se desenvolver no sentido de superar o isolamento das grades, dos muros e das chaves, do apagamento do sujeito, da supressão de sintomas e das personalidades dos sujeitos em sofrimento psíquico. Diante disso, é que este estudo tem como objetivos avaliar boas práticas em saúde mental na atenção psicossocial no contexto de um município da região Sul do Brasil. Para tanto, organizou-se como uma pesquisa avaliativa construtivista, com enfoque responsivo, a medida que desenvolve um consenso critico entre os grupos de interesse. Está baseada na Avaliação de Quarta Geração, proposta por Guba e Lincoln (2011), que emerge como opção metodológica e se articula com o referencial teórico das \"Boas Práticas em Saúde Mental Comunitária\", proposto por Thornicroft e Tansella (2010). Este é apresentado de modo a identificar boas práticas na saúde mental, com base na articulação de 3 eixos indispensáveis e indissociáveis: a ética, como princípio fundamental que deverá orientar o planejamento, a assistência e a avaliação dos serviços. A evidência deve embasar as intervenções e serviços. E a experiência como uma evidência do que vem sendo produzido nas experiências locais e regionais de organização da rede de serviços. Como resultado do processo as práticas de Acolhimento, Projeto Terapêutico Singular e Transferência de Cuidados emergiram para discussão. O Acolhimento torna-se concreto nas ações da Porta Aberta, e significa escuta qualificada no momento em que a necessidade emerge, além de representar a oferta de atenção de qualidade baseada no processo relacional que fortalece o estabelecimento de vínculos e a construção de projetos de vida. O Projeto Terapêutico Singular apresenta-se como uma boa prática, a medida que é capaz de dar resposta singularizada e individualizada às necessidades do sujeito, de modo a oportunizar como resultado uma pessoa capaz de andar na vida. É definido de forma compartilhada entre equipe, usuário e família, segundo os objetivos terapêuticos para cada sujeito, e utiliza o técnico de referência, as mini equipes, as oficinas terapêuticas, os grupos terapêuticos, as assembleias, enfim, ofertas e propostas que permitem ao sujeito retomar o envolvimento com os espaços da cidade, no caminho da produção de vida. Com relação à Transferência de Cuidados, esta possibilita ao sujeito que vive o processo não só circular em uma rede de serviços, mas, acima de tudo, buscar, com suporte dela, sua emancipação. Para tanto, investe em práticas que estimulem a autonomia dos sujeitos, por meio de instrumentos como o Acompanhamento Terapêutico, Grupo de Apoio à Alta, espaços de decisões e discussões, estímulo à busca e resolução de problemas, atividades de reconhecimento dos espaços da cidade, transporte, cultura e lazer. Além disso, investe nas ações com a Atenção Básica, a partir do Apoio Matricial com processos de formação continuada com as equipes, e mapeamento da situação da saúde mental nos territórios. Ao final deste estudo, é possível concluir que o modelo de atenção psicossocial demonstrou potencialidade de produzir boas práticas na atenção à saúde mental, e que estas tem transformado a vida dos sujeitos em sofrimento psíquico, bem como tem auxiliado no processo de reconquista de espaços sociais / The redirection of the care model in Brazil raises the need to discuss the reinvention of practices in tune with the scenery of changes. The practices must now be developed in order to overcome the isolation of bars, walls and keys, the deletion of the subject, the suppression of symptoms and the personalities of individuals in psychological distress. Given this, the present study aims to evaluate best practices in mental health in psychosocial care in the context of a city in Southern Brazil. To do so, it is organized as a constructivist evaluation research, with a responsive approach, as it develops a critical consensus among stakeholdres. It is based on the Fourth Generation Evaluation, proposed by Guba and Lincoln (2011), which emerges as a methodological option and articulates with the theoretical framework of \"Best Practice in Community Mental Health\" proposed by Thornicroft and Tansella (2010). This one is presented in order to identify best practices in mental health based on the articulation of three indispensable and inseparable pillars: ethics, as a fundamental principle that should guide the planning, assistance and assessment of services; evidence, which must base the interventions and services; and experience, as an evidence of what is being produced in the local and regional experience of organizing network services. As a result of the process, Welcoming practices, Singular Therapeutic Project and Transfer of Care emerged for discussion. The Welcoming becomes concrete in the actions of the Porta Aberta project, and it means qualified hearing at the moment the need arises, besides representing the offer of quality care based on the relationship process that strengthens the bonds and the construction of life projects. The Singular Therapeutic Project is presented as best practice, as it is able to give individualized responses to the needs of the subject in order to create, as a result, a person who is able to walk through life. It is set in a shared manner among staff, user and family, according to the therapeutic goals for each subject, and it uses the reference technician (ou professional), the mini teams, therapeutic workshops, therapeutic groups, assemblies, in other words, any offers and proposals which allow the subject to resume the engagement with the spaces of the city, on the way to production of life. Regarding the Transfer of Care, it enables the person who lives the process not only to have access to a network of services but, above all, to seek their emancipation through its support. To do so, it invests in practices that encourage the subjects\' autonomy, using instruments such as the Therapeutic Monitoring, Patient Discharge Support Group, spaces for decisions and discussions, motivation for searching and solving problems, recognition activities of city areas , transport, culture and leisure. In addition, it invests in actions with the Basic Health Care, based on the matrix support with processes of continuing education with the teams and mapping of the mental health situation in the territories. At the end of this study, the conclusion was that the psychosocial care model showed capability of producing best practices in mental health, and these have changed the lives of individuals in psychological distress, and have helped in the process of conquering social spaces again

Assessment of corporate governance practices in Jordan : an empirical investigation

Corporate Governance (CG) nowadays is on the agenda of most developed and developing countries, including Jordan, and is receiving considerable attention in the business world as well as in the area of academic research, which is an indication of its importance for business development and society as a whole. The knowledge base about CG in developing countries appears to be limited, but it is growing in size and importance. This study therefore aims to investigate current CG practices and barriers to the development of good CG practices in firms. In order to accomplish the research objectives, a mixed research methodology was adopted. The findings of the study contribute to knowledge by providing empirical data to test and extend the theory of CG. The results suggest that most big and old firms are applying best practice of good CG. Regarding factors inhibiting the practice of effective CG, the results indicate that weakness of the legal environment for firms and lack of knowledge of BODs about CG principles are the most important factors. The empirical results find that constitution, compliance and conscience will affect firm’s performance positively. Separation between the position of CEO and Chairman, the existence of independent NEDs, the use of board subcommittees and a strong disclosure regime also help firms to improve performance. On the basis of the empirical results, the study recommends that the government needs to reform the relevant legislation. These suggestions may strengthen the internal governance of firms, thereby increasing performance and maximise shareholders’ wealth.

A Study of Teachers' Challenges with the Inclusion of Middle and High School Students with Autism

Middle and high school general education teachers in the school district in this bounded case study were facing challenges with meeting the needs of students who have autism in the current inclusion program. The purpose of this study was to understand teachers' challenges with components of the inclusion program and serving students with autism in the general education classroom. The conceptual framework was Villa and Thousand's 5 system-level best practices for successful inclusive education. A purposeful sampling procedure was used to select 4 general education teachers who were teaching autistic students in an inclusive setting; this sample included 2 middle school level and 2 high school level teachers from 2 schools in the small rural district. The data collected through classroom observations and semi structured interviews were coded based on Villa and Thousand's best practices of leadership, redefined roles, collaboration, adult support, and promotion as each related to inclusion of autistic students. Results were used to identify challenges teachers were facing that prevented the 5 system-level best practices from being implemented. Key challenges were collaboration between general and special education teachers and lack of professional development for all teachers on inclusion. Findings were used to provide recommendations for how to address challenges in middle and high school inclusion programs and for conducting future studies in different settings. The results of this study could be used by school leaders and other stakeholders to make informed decisions about system level implementation of inclusion program components and for enhancing the learning of students who have autism in the inclusive setting.

Principals' leadership for learning : formative assessment strategies in every classroom

The purpose of this study was to explore how school leaders address both the technical and professional socialization needs of teachers during the planning and implementation of student formative assessment in every classroom, an initiative that is often implemented as a means of gathering the data needed to support the academic needs of all learners (Black & Wiliam, 1998; Bolman & Deal, 2008; Heath & Heath, 2010; Leithwood & Seashore Louis, 2011; Wiliam, 2010). In order to determine school leadership considerations for addressing the technical and professional socialization needs of teachers during the implementation of student formative assessment in every classroom, two research questions guided the study: 1. What do teachers perceive to be their technical and professional socialization needs experienced during the planning and implementation of student formative assessment and how are these met? 2. What are the school principals’ perceptions of how they address the technical and professional socialization needs of teachers during the planning and implementation of student formative assessment? Using a grounded theory approach, this qualitative study examined the perceptions of both teachers and principals through a multi-site case study design (Miles & Huberman, 1994). This design was chosen in order to gather insight regarding the perceptions and experiences of principals and teachers at three elementary campuses that have implemented student formative assessment in every classroom. The sites and participants for this study were purposefully selected. Data were collected through interviews and focus groups. In order to confirm emerging theoretical explanations, the researcher gathered additional data through a review of relevant documents, such as district and campus improvement plans. The prominent technical needs identified by teachers in this study were the development of a campus-wide common terminology, participation in vertical teaming, and the maintenance of the support role of a campus instructional specialist. Furthermore, teacher participants identified their professional socialization needs as reassurance from the principal with new professional learning, a gradual pace of implementation for the student formative assessment initiatives, meaningful teacher-to-teacher interaction, open and transparent communication with the principal, and opportunities to participate in building cohesive grade-level teams. Principals perceived their technical supports as facilitating vertical teaming, providing a campus instructional specialist, embedding time for collaborative professional development, and setting clear expectations for implementation. Additionally, principals perceived their professional socialization supports for teachers as facilitating the building of cohesive grade-level teams, providing reassurance with new implementation, promoting open and transparent communication, promoting a gradual implementation pace and facilitating meaningful teacher-to-teacher interactions. / text

E-gov innovations on one hand are a driver of public sector improvement and development, in other hand it is a process, which expect high competence, investments and political power. In order to disclose the topic of the paper, the analysis of literary sources, legal documents, projects under implementation was carried out and qualitative questionnaire of experts was done. Having analysed the concepts of e-gov and innovation, factors constructing innovative process in public sector, trends of e-gov innovations in practice and priorities of EU and Lithuania’s strategy and policy, the definition of e-gov innovation was interpreted, the trends for development and the strategies for e-gov innovation promotion were identified.

This dissertation serves as a report on the qualitative exploration of best practices in animal-assisted therapy (AAT) with children. The aim of the study was to explore and describe best practices in the field of AAT with children in the Western Cape. The research made use of the case study design, which offered the researcher the opportunity to interact with a small number of individuals and look for patterns in the research participants’ words and experiences. Four participants, who were trained mental health professionals and had offered AAT to children, were selected to participate in the research. A literature review of animal-assisted therapy and best practice provided a theoretical basis for the research. The history of AAT, the uses and benefits of AAT and the animals incorporated in AAT were explored. The term “best practice” and its application in mental health and AAT were also discussed. The verbatim transcriptions of the audiotaped data were analysed and interpreted using Interpretative Phenomenological Analysis (IPA) and an AAT conceptual framework (brought about by the literature review). Themes were developed from the empirical data and substantiated by the literature review. Five main themes emerged as a result of the analysis. The first theme, Training of the animal-assisted therapist, focused on both the mental health training and the AAT training of the therapist. The second theme, Training of animals incorporated in animal-assisted therapy, revealed the importance of training the animal to be included in AAT. The third theme, Different client populations in animal-assisted therapy, looked at which populations should be included, excluded or included and carefully managed during AAT. The fourth theme, Ethical considerations in animal-assisted therapy, revealed the importance of considering both the child’s and the animal’s welfare. The fifth theme, Regulation of animal-assisted therapy, gave a more in-depth description of the current regulation of AAT in the Western Cape and provided suggestions for the regulation of AAT. An evaluation of UK sheep farmers' attitudes and behaviours towards sustainable roundworm control

One of the major constraints limiting the efficiency of sheep production is the control of gastrointestinal nematode parasites. Parasite control by strategic use of anthelmintics is threatened by the emergence of nematode populations that are resistant to the drugs available. It is therefore increasingly apparent that steps toward maintaining sustainable productivity in the growing face of anthelmintic resistance (AR) is required by farmers. To facilitate the uptake of sustainable approaches to parasite management, a comprehensive understanding of the various factors that may influence farmers’ decision-making processes is required. In order to establish which factors are influential, and determine their impact on farmers’ roundworm control behaviours, both qualitative and quantitative research methods were used including focus groups as well as attitudinal and behavioural questionnaires. A retrospective analysis was initially conducted of historical surveys designed to identify farm specific characteristics and parasite management practices including anthelmintic usage. The objectives of the analysis were to identify factors associated with uptake of best practice advice including farm characteristics and information sourcing. In addition, the implementation of sustainable roundworm control practices was investigated using two surveys conducted in 2000 and 2010. Logistic regression models were applied for univariable and multivariable analysis of dependent and independent variables. The next step was to conduct a series of focus groups in different geographic regions of Scotland. The main purposes were to explore sheep farmers’ attitudes towards different aspects of roundworm management, as well as to identify potential motivators and barriers to uptake of sustainable parasite control practices. The findings aided in the development of an attitudinal questionnaire used to canvass opinions representative on a national level. The concluding study involved a telephone survey of 400 Scottish sheep farmers, designed to elicit attitudes regarding roundworm control, AR and sustainable roundworm control practices. A quantitative statistical analysis technique (Structural Equation Modelling) was then used to test the relationships between socio-psychological factors and the uptake of sustainable roundworm control practices. The analysis of historical questionnaire data demonstrated evidence of a shift towards the use of practices to reduce the rate of AR development, most notably a decline in the practice of ‘dose and move’ as well as an overall reduction in treatment frequency. Statistical analysis identified significant associations between farm characteristics and specific treatment strategies. For example, larger farms were more likely to adopt a set treatment regimen (P=0.036), compared with smaller farms, which were more likely to treat based on clinical signs of infection (P=0.021). Sourcing of roundworm control information primarily from veterinarians was most associated with treatment timings with no parallels between time points. From the qualitative focus group studies conducted we identified four overarching themes impacting on sheep producers’ attitudes to roundworm control and best practice advice. These themes comprised: a lack of perceived need to change, the complexity of advice, the ease of implementation of recommended practices and the effectiveness of extension approaches. Additionally, the most important and implementable guidelines identified by sheep farmers were: ‘working out a roundworm control strategy with an advisor’ and ‘administering anthelmintics effectively’ with regard to correct drenching practice i.e. appropriate dosing, administration and drug storage procedures. These findings exhibited similarities with veterinarians’ rankings with also, ‘testing for AR’, ‘preserving susceptible parasites’ and ‘reducing dependence on anthelmintics’ receiving the lowest rankings for importance and implementability. The quantitative analysis from the attitudinal/behavioural questionnaire identified eleven factors with significant influences on the adoption of sustainable roundworm control practices. The key influences on overall adoption were farmers’ baseline understanding about roundworm control and self-reported confirmation of anthelmintic resistance in their flock. Additional positive influences included, positive attitudes to veterinary services, enterprise type and perceived risk of AR. Factors that were shown to have the greatest relative effects on individual parasite control practices included; the perceived resource requirements for implementing a quarantine strategy, farmers’ AR suspicions for instigating AR testing and the confirmation of AR for adopting faecal egg count monitoring. The findings have highlighted several factors which can influence sheep farmers’ decisions to reject or adopt recommended roundworm control practices. It is evident that the perceived complexity, lack of need and practicality of the current recommendations necessitates changes to how extension is designed and disseminated to farmers. The findings also suggest that improving farmers’ acceptance and uptake of diagnostic testing and improving underlying knowledge and awareness about nematode control is a significant target to influencing adoption of best practice behaviours. The importance of veterinarians as highly-trusted information resources validates the need for further engagement with veterinarians concerning sustainable parasite control approaches, to facilitate collaboration with farmers. The need for interaction between farmers and their advisors is key to resolving the issues raised to enable the necessary explanation, justification and execution of recommended practices to suit farmers’ needs and farming conditions.
