Neurocognitive implications of diabetes on dementia as measured by an extensive neuropsychological battery.

Diabetes is a disease with a deleterious pathology that currently impacts 4.5 million individuals within the United States. This study examined the ability of a specific neuropsychological battery to identify and classify dementia type, investigated the impact of diabetes on cognition and analyzed the ability of the memory measures of the 7 Minute Screen (7MS) and the Rey-Osterrieth Recall to correctly categorize dementia type when not used in combination with a full battery. The battery in addition to exhaustive patient history, medical chart review and pertinent tests were used in initial diagnosis. Results indicated the battery was sufficient in the identification and classification of dementia type. Within the sample, diabetes did not appear to significantly impact overall battery results whereby only two measures were minimally affected by diabetes. Finally, the memory measures of the 7MS and the Rey-Osterrieth Recall were sufficient to predict membership into the Alzheimer's (AD) and vascular dementia (VD) groups with 86.4% accuracy. The classification percentage dropped to 68.3% with addition of the mild cognitive impairment category. The full battery correctly classified AD and VD dementia 87.5% and appeared to be the most robust.

Neurocognitive outcomes in HIV and childhood trauma

Research to date has provided evidence for neurocognitive decline in individuals infected with HIV/AIDS and in individuals who have experienced early life trauma. However, many gaps remain in our knowledge about the neurocognitive profile of HIV and childhood trauma in South African women. The present study focused on the neurocognitive effects of HIV infection and childhood trauma, both separately and in combination in South African women. The primary aim of the study was to assess neurocognitive functioning in HIV-positive and matched HIVnegative controls, with and without a history of childhood trauma. Moreover, the study sought to assess the synergistic relationship between HIV and childhood trauma in influencing neurocognitive outcomes, a relationship which has not yet been investigated. A neuropsychological battery sensitive to HIV-related impairments was administered to 83 HIV-positive and 47 matched HIV-negative women with histories of childhood trauma. A history of childhood trauma was assessed using the Childhood Trauma Questionnaire short form (CTQ-SF). Forty eight of the 83 HIV-positive women were exposed to childhood trauma. Among the control subjects, a total of twenty women were exposed to childhood trauma. Findings of the present study revealed neurocognitive deficits in memory and executive functions. Results demonstrated significant HIV effects in memory (HVLT-R learning and delay trials), and executive functions (Halstead Category test). Similarly, a trauma effect was evident in delayed recall (HVLT-R delay). Moreover, results revealed a significant interaction effect between HIV status and trauma status on the WAIS-III Symbol Search Task, a task of psychomotor speed. However, HIV-negative controls with a history of childhood trauma scored the highest on this task. Although this finding was unexpected, it may suggest that psychomotor speed may not be a sensitive or discriminating test of childhood trauma in healthy adults. The present study demonstrated evidence for HIV and trauma effects in the ability domains of learning and delayed recall and executive functions. Although the present study did not find evidence for a synergistic relationship between HIV and trauma, it did provide evidence for both HIV and trauma effects on neurocognition, a finding in keeping with previous studies. Future research should be prospective in nature and should better delineate the nature, severity, and temporal relationship of childhood trauma to neurocognitive outcomes, as well as the mediators and moderators of these outcomes. / AFRIKAANSE OPSOMMING: Dit is alombekend dat Suid-Afrikaanse vroue buite verhouding swaar deur MIV/vigs en geslagsgebaseerde geweld getref word. Factor Structure of the Neurocognitive Battery in a Geriatric Sample with Cognitive Impairments

The present study was designed to empirically validate six theoretically derived cognitive domains (verbal memory, visual memory, working memory, attention-concentration, executive functions, and visuospatial abilities) assessed by a comprehensive battery of neuropsychological tests used in the Geriatric Memory Clinic at the University of North Texas Health Science Center in Fort Worth, Texas. The study examined the extent to which various cognitive dimensions are tapped by this battery in a heterogeneous geriatric sample of 114 patients with cognitive impairments. The proposed six-factor model of cognitive functioning has not been supported. Further exploratory factor analysis arrived at a five-factor solution. Factor pattern of the 23 tests supported the following five dimensions: memory, executive control, attention, visuospatial abilities, and cognitive flexibility.

Avaliação neuropsicológica de pacientes expostos ao vapor de mercúrio e de pacientes diabéticos do tipo 2 / Neuropsychological assessments of patients exposed to mercury vapor and of type 2 diabetic patients

Exposição a substâncias tóxicas e distúrbios metabólicos são fatores que afetam as funções neuropsicológicas. Foram realizados 2 estudos descritivos com os objetivos de verificar a possibilidade de disfunções neuropsicológicas em indivíduos com histórico de exposição ocupacional ao vapor de mercúrio e em pacientes diabéticos do tipo 2 sem diagnóstico de retinopatia, em ambos os casos, comparados com controles. A bateria neuropsicológica incluiu testes de atenção (Dígitos), controle inibitório (teste de stroop), memória verbal (teste de Buschke) e visual (Reprodução Visual), destreza manual (Grooved Pegboard), fluência verbal (FAS), habilidade viso-motora (Cubos), funções executivas (teste de Wisconsin), conhecimento semântico (Vocabulário) e sintomas de depressão (Inventário Beck de Depressão) e ansiedade (Inventário de Ansiedade Traço-Estado). Adotou-se p<0,05 como nível de significância. As avaliações neuropsicológicas de 26 ex-trabalhadores de fábricas de lâmpadas de mercúrio foram comparadas com 20 controles. Os ex-trabalhadores têm diagnóstico de mercurialismo crônico ocupacional, foram expostos ao vapor de mercúrio no trabalho durante 10,2 ± 3,8 anos e estão afastados da função há 6 ± 4,7 anos. A concentração de mercúrio urinário medida um ano após o afastamento da exposição teve média de 1,8 ± 0,9 mg/gCr. Comparados com os controles, os ex-trabalhadores demonstraram pior desempenho no teste de Stroop partes 1 (p=0,004) e 2 (0,010), no teste de Buschke para recuperação de longo prazo (p=0,028), armazenamento de longo prazo (p=0,045), recuperação consistente longo prazo (p=0,006), e recuperação tardia (p=0,008), fluência verbal (p=0,010) e no Grooved Pegboard tanto para a mão dominante (p=0,019) quanto a não dominante (p=0,008). Os escores de depressão, de estado e traço de ansiedade foram significantemente superiores aos do grupo controle (p<0,001). Os indivíduos com histórico de exposição ao vapor de mercúrio demonstraram redução da velocidade de processamento de informações e déficits de recuperação espontânea de informações verbais, fluência verbal, função motora, além de sintomas de depressão e ansiedade. Os escores altos de depressão e ansiedade são esperados em indivíduos com mercurialismo crônico, mas também podem se associar a problemas psicosociais relacionados ao desemprego. O outro estudo comparou o desempenho neuropsicológico de 19 pacientes diabéticos sem diagnóstico de retinopatia (avaliada através de exame de fundo de olho) e 20 sujeitos controles não diabéticos. Os pacientes têm diabetes há 8,2 ± 8,1 anos, média de hemoglobina glicada (HbA1C) de 7 ± 1,3%, não fazem uso de insulina como tratamento, não manifestam queixas de neuropatia e apresentam valores de microalbuminúria dentro dos limites de normalidade. Nove (47%) são hipertensos. Não foram encontradas diferenças significantes (p<0,05) entre pacientes diabéticos e sujeitos controles em quaisquer medidas realizadas. Os resultados sugerem que o diabetes tipo 2 não se associa diretamente aos déficits neuropsicológicos. Há a possibilidade de que as alterações cognitivas e de humor observadas em pacientes diabéticos do tipo 2 em estudos prévios estejam relacionadas à presença de complicações como retinopatia, hipoglicemia ou neuropatia com sinais clínicos. / Neuropsychological function is known to be affected by exposure to toxic substances and metabolic disorders. Two descriptive studies were performed. The aim was to examine possible neuropsychological dysfunction in individuals with history of occupational exposure to mercury vapor, and in type 2 diabetic patients without retinopathy, in comparison with controls. The neuropsychological assessment included measures of attention (WMS Digit Span), inhibitory control (Stroop Interference Test), verbal memory (Buschke Selective Reminding Test), visual memory (WMS Visual Reproduction), manual dexterity (Grooved Pegboard), verbal fluency (FAS), visuomotor ability (WAIS Block Design), executive function (Wisconsin Card Sorting Test), verbal knowledge (WAIS Vocabulary), and depression (Beck Depression Inventory) and anxiety (State-Trait Anxiety Inventory) symptoms. P<0.05 was taken as significance level. Neuropsychological assessments of 26 fluorescent lamp ex-workers with chronic mercurialism diagnosis who had been exposed to mercury vapor for 10.2 ± 3.8 years were compared with 20 controls. The time since the cessation of exposure was 6 ± 4.7 years and the mean urinary mercury concentration was 1.8 ± 0.9 mg/gCr. Compared with the controls, the ex-workers performed worse on the Stroop Test part 1 (p=0.004) and 2 (0.010), SRT long term recall (p=0.028), long term storage (p=0.045), consistent long term recall (p=0.006), and delayed recall (p=0.008), FAS (p=0.010) and Grooved Pegboard dominant hand (p=0.019) and nondominant hand (p=0.008). Their depression, anxiety state and trait scores were higher than control’s (p<0.001). Individuals with history of exposure to mercury vapor presented slowed processing speed and verbal memory spontaneous recall, verbal fluency and motor function impairment along with symptoms of depression and anxiety. The high scores on depression and anxiety are expected in chronic mercurialism and may be also associated with psychosocial problems related to unemployment. The other study compared the neuropsychological performances of 19 diabetic patients without retinopathy diagnosis (assessed by eye fundus examination) with 20 nondiabetic control subjects. The patients had diabetes for 8.2 ± 8.1 years and their recent glycaemic control levels (HbA1C) mean was 7 ± 1.3%. Other diabetic group’s characteristics included non-insulin treatment, no neuropathy complaints, normal microalbumin urine tests, and 9 (47%) hypertensive patients. No significant (p<0.05) differences were found between the diabetic and control groups on any measure. The results suggest that type 2 diabetes is not directly related to significantly neuropsychological alterations. It is possible that cognitive decrements and mood alterations among type 2 diabetic patients described in previous studies are associated with the presence of complications such as retinopathy, hypoglicaemia, or even neuropathy with clinical signs.

Efeitos de um programa de exercícios resistidos em idosos com comprometimento da memória / Effects of resistance training program in elderly with memory impairment

O envelhecimento populacional ocorrido nas últimas décadas provocou um aumento da prevalência das doenças crônicas. Neste contexto, o estudo do comprometimento cognitivo entre idosos assume grande importância, sobretudo as condições que podem preceder as manifestações clínicas da Doença de Alzheimer. O objetivo deste estudo foi detectar os efeitos de um programa de exercícios resistidos no desempenho cognitivo e na força muscular de idosos sedentários com comprometimento da memória. Trinta e um idosos sedentários sem demência ou depressão foram distribuídos aleatoriamente em dois grupos: Grupo Atividade Física e Grupo Controle. O programa de exercícios resistidos durou nove meses e consistiu de três séries de seis exercícios por sessão, realizados em aparelhos com sistema de alavancas, por aproximadamente uma hora, duas vezes por semana. Ambos os grupos foram submetidos aos seguintes testes cognitivos a cada três meses: Teste Comportamental de Memória de Rivermead (RBMT), Amplitude de Dígitos Diretos e Indiretos do WAIS, Escala de Queixas de Memória e Teste Cognitivo de Cambridge (CAMCOG). Após nove meses, o grupo que realizou os exercícios resistidos teve um aumento significativo do escore padronizado do RBMT (p=0,021) e da força muscular (p<0,001), sem diferença significativa dos demais parâmetros avaliados. Estes resultados indicam que os exercícios resistidos supervisionados podem melhorar o desempenho da memória em idosos sedentários com prévio comprometimento, além de determinar o aumento da força muscular. / The aging population in last decades increased the prevalence of chronic diseases. In this context, the study of cognitive impairment among elderly people assumes great importance, principally of conditions than can precede the clinics manifestations of Alzheimer disease. The objective was to detect the effects of resistance training program on cognitive performance and muscle strength of sedentary elderly with memory impairment. Thirty-one sedentary patients aged 60 and older, without dementia or depression, were randomly assigned to two groups: Physical Activity Group and Control Group. The resistance training program occurred on nine months, consisted of three series of six exercises by workout, performed with lever-kind weight training machines, nearly one hour, two times for week. Both groups were submitted by cognitive tests each three months: Rivermead Behavioral Memory Test (RBMT), WAIS forward digit span and backward digit span, and subjective Memory Complains Scale, and CAMCOG. Before nine months, the resistance training group had a significant increase in RBMT profile score (p=0.021) and muscle strength (p<0.001). There weren\'t significant differences in others parameters assessed. These findings showed that supervised resistance training can enhance a memory performance in sedentary elderly with previous impairment and improve muscle strength.

Treating emotion perception deficits following traumatic brain injury

While the cognitive disturbances that frequently follow severe TBI are relatively well understood, the ways in which these affect the psychosocial functioning of people with TBI are yet to be determined and have thus received little attention in treatment research. Growing evidence indicates that that a significant proportion of individuals with TBI demonstrate deficits in the perception of affective information from the face, voice, bodily movement and posture. As accurate evaluation of emotion in others is critical for the successful negotiation of social interactions, effective treatments are necessary. Until recently, however, there have been no rehabilitation efforts in this area with TBI groups. The present research aims to redress this absence. The literature on emotion perception deficits in TBI is examined, and a theoretical rationale for intervention is presented. Several lines of research are reviewed which suggest that rehabilitation targeting such deficits is tenable. These include research on emotion perception remediation with other cognitively impaired populations, findings from cognitive neuroscience suggesting the potential for neuronal restoration after brain damage, and the successful applications of remediation techniques, in particular errorless learning and self-instruction, for treating other cognitive deficits in a range of neurological disorders and TBI. Discussion of this research is followed by a description of two randomised controlled trials aimed at improving emotion perception in individuals with TBI. The findings are discussed with reference to useful theoretical models of rehabilitation, learning and emotion perception. Suggestions for future directions of research are outlined together with relevant design issues.
