Intérêt de la variabilité du rythme cardiaque comme marqueur de risque / Interest of heart rate variability as a risk marker

La variabilité du rythme cardiaque « Heart Rate Variability (HRV) » consiste en des fluctuations perpétuelles du rythme cardiaque autour de sa fréquence moyenne. L’analyse de l’HRV constitue un biomarqueur du fonctionnement du système nerveux autonome (SNA), par l’intermédiaire de ses effets sur le cœur. De nombreuses études ont montré qu’une diminution de l’HRV est associée à un pronostic péjoratif, notamment dans le post infarctus et dans l’insuffisance cardiaque (IC). Nous nous sommes proposés d’élargir le champ d’investigation de l’HRV à de nouveaux domaines d’application, qu'il s'agisse de pathologies cardiaque ou extra cardiaque. Nous avons ciblé des contextes pathologiques dans lesquels le SNA joue un rôle important : 1-Accidents sur la Voie Publique (AVP) et le risque de développement d’un syndrome de stress post traumatique (PTSD). 2-Effet de la douleur et de l’analgésie péridurale sur l’HRV maternelle au cours de l’accouchement. 3-Lien avec les polymorphismes génétique des récepteurs bêta adrénergiques (β;-AR) chez des patients présentant une dysfonction ventriculaire gauche et implanté d’un défibrillateur automatique implantable (DAI) en prévention secondaire. L'analyse temporelle de l'HRV sur 24 heures est un facteur prédictif de survenue d'un PTSD et aussi de sa sévérité chez les victimes d'AVP. L'indice de variabilité est le meilleur paramètre prédictif de PTSD à 6 mois, avec une aire sous la courbe de 0.92 (IC 95% : 0.785; 1.046). Le seuil de 2.19% confère une sensibilité de 85,7 £ et une spécificité e 81.8 % pour prédire la survenue d'un PTSD. les valeurs prédictives positives et négatives sont de 75 % et 90 % respectivement. L'ANI, reflétant l'influence de la ventilation sur le rythme cardiaque, permet une mesure objective de la douleur chez les parturientes. Enfin, les patients ayant subi les substitutions Arg (pour Gly) en 16 et Gln (pour Glu) en 27 pour les récepteurs β2-AR, présentent une HRV plus faible que les patients ayant la forme sauvage du gène. / The heart rate variability (HRV) has arisen as a promising simple and non invasive biomarker of autonomic nervous system (ANS) function, through its effects on the heart. Multiple studies have shown that the decrease in the HRV is associated usually with poor prognosis. We proposed to further investigate the HRV in other domains of clinical application, including cardiac or extra-cardiac pathologies. In particular, the pathological contexts in which the ANS plays an important role :1-Traffic road accident (RTA) and the risk of development of post traumatic stress disorders (PTSD) in survivors.2-Effect of pain and of epidural analgesia on maternal HRV during childbirth.3-Genetic polymorphisms of beta adrenergic receptors (β-AR) in patients with left ventricular dysfunction and implanted with an implantable cardioverter defibrillator (ICD) for secondary prevention. At first, our study is the first to show that the temporal analysis of 24-h HRV is not only a predictive factor of the occurrence of PTSD but also its severity among victims of RTA AVP. The variability index was the best predictor of PTSD with the area under the receiver-operating curve for discriminating PTSD at 6 month at 0.92 (95% CI:0.785;1.046). Acut-off at 2.19% yielded a sensitivity of 85.7% and a specificity of 81.8% for PTSD. Positive and negative predictive values were respectively 75% and 90%. At next, we showed that the ANI, which reflects the influence of ventilation on heart rate, allows an objective measure of pain in conscious subjects? At last, we found that patients with the substitutions Arg (instead of Gly) at position 16 and Gln (instead of Glu) at position 27 for β2-AR have a lower HRV value than patients with wild type receptor.

The competency of municipal police related to road safety This thesis deals with the competency of municipal police focused only on the issue of road safety surveillance. The main purpose of the study is to present basic orientation in particular authorizations to represent public law when enforcing road safety regulations. The study is composed of three main chapters devided in relevant subchapters, each of them dealing with different aspects of the administrative law. The first chapter briefly describes the origins of enacting the municipal police as a municipal body, pointing out the past difficulties, followed by the summary of the law regulation establishing the municipal police and controlling its activities by the municipal self-government. It also defines the municipal police as a part of the administrative law. In the main second chapter the single competencies are described, divided into subchapters by its source according to the Municipal police Act and the Road traffic Act. The permition to control speeding and testing the drunk drivers are most widely described, followed by the issue of towing vehicles. The Emission Trading Scheme for green house gases is a key tool of European climate protection. Including the road transport sector might be a promising strategy to limit its CO2 emissions. This could be realized within a common market (trans-sectoral trading permitted) or separated markets (trans-sectoral trading not permitted). Starting from different assumptions on emission reduction objectives, the impact of both options is analyzed using a quantitative model. Although an emission trading scheme is ecologically effective regardless of the trading model, it turns out that CO2 emissions and emission allowance prices differ strongly between both design options due to sector specific price elasticities of allowance demand. Efficiency and acceptability of pricing policies and transport investments in distorted economies

This thesis contains five papers studying the economic efficiency and political acceptability of road pricing policies and transport investments in distorted economies. Interactions between the transport market and other distorted markets, such as the labor market, can have a large impact on the welfare effect of a road pricing policy or a transport investment. Many road pricing studies therefore try to incorporate effects from other distorted markets in the analysis. Paper I analyzes how the economic efficiency of a road toll in a distorted economy depends on assumptions about the initial tax system. In the road pricing literature, the welfare effect of a road toll is often found to depend on revenue use. Using a simple general equilibrium model paper I shows that the relative efficiency of marginal revenue recycling policies depends more on assumptions regarding inefficiencies in the initial tax system than on the road toll per se. Paper II studies the effect on welfare, equity and labor supply from a road toll in a commuting population with heterogeneous value of time and endogenous labor supply. When explicitly taking into account that commuters have different value of time, the road toll can increase total labor supply even when the revenues are not recycled back to the commuters. The analysis stresses the importance of recognizing traveler heterogeneity when analyzing congestion pricing. Road pricing policies are often characterized by conflicting interests between different stakeholders and different geographical areas. Papers III and IV study the economic efficiency and political acceptability of pricing and investment policies in different institutional and geographical settings. The main contribution of the papers is to explain how political constraints can lead to inefficient tolling strategies. The papers contribute to the existing literature on political acceptability of road pricing by analyzing the conflict and potential trade-off between political acceptability and economic efficiency. A difficulty when assessing the welfare effect of a future transport policy is also that many factors and parameters needed for the analysis are uncertain. Paper V studies the climate benefit of an investment in high speed rail by calculating the magnitude of annual traffic emission reduction required to compensate for the annualized embedded emissions from the construction of the line. The paper finds that to be able to balance the annualized emissions from the construction, traffic volumes of more than 10 million annual one-way trips are usually required, and most of the traffic diverted from other transport modes must come from aviation. / <p>QC 20121010</p>

Ground Target Tracking with Multi-Lane Constraint

Knowledge of the lane that a target is located in is of particular interest in on-road surveillance and target tracking systems. We formulate the problem and propose two approaches for on-road target estimation with lane tracking. The first approach for lane tracking is lane identification based ona Hidden Markov Model (HMM) framework. Two identifiers are developed according to different optimality goals of identification, i.e., the optimality for the whole lane sequence and the optimality of the current lane where the target is given the whole observation sequence. The second approach is on-road target tracking with lane estimation. We propose a 2D road representation which additionally allows to model the lateral motion of the target. For fusion of the radar and image sensor based measurement data we develop three, IMM-based, estimators that use different fusion schemes: centralized, distributed, and sequential. Simulation results show that the proposed two methods have new capabilities and achieve improved estimation accuracy for on-road target tracking.

Contested Road Space: Public Narratives and Bus Rapid Transit in Indore, India

Bus Rapid Transit (BRT) has a demonstrated capacity to improve safety, mobility and accessibility for multiple types of road users. In India, where road safety is an ongoing problem, the national government has supported cities’ efforts to implement BRT as a cost-effective tool to improve urban travel. The case of Indore’s iBus attests to the cultural and institutional barriers that Indian cities may face in gaining public support for BRT. After a contentious implementation process, the High Court ruled to open the dedicated bus lane to private automobiles, resulting in drops in ridership and increases in accidents and travel delay. This study examines the competing, public narratives that framed the issues during and after project construction. Contested notions of the public interest, the cause of traffic problems, and community participation informed the basis for the arguments that culminated in the court ruling.

Like Sámis do : A postcolonial and intersectional analysis of the contemporary film representations and self-representations of the Sámi people

The film representation of the Sámi people has evolved during the last century from the ethnographic portrayals that reproduce a romantic stereotype of the good savages, to feature and documentary films that discuss the Sámi identity and its colonial history. In recent years a new generation of Sámi and Swedish documentary directors have focused their work on analysing the impact that multiple structures of power actually have in the production of the Sámi identity and culture. In this research I explore the intersections of such structures in the documentary road movies Sámi Daughter Yoik (2007) by the Sámi-Swedish director Liselotte Wajstedt, and The Only Image of My Father (2004) by the Swedish director Kine Boman. The main purpose of the research is to examine the discussions of identity that these films propose and to analyse the strategies with which the directors question the simplistic representation of the Sámi people. Based on the postcolonial and intersectional perspectives, the text offers a critique of the discourses of authenticity that confine the Sámi identity into the frame of ethnicity. The study gives special attention to the different layers that the directors' identities involve and their role in the construction of alternative representations of the Sámi people. A relevant finding is that the directors have succeeded in representing the Sámi people as complex and heterogeneous, helped by their choices on genre, authorship and their own approach to identity as a performative, multidimensional and dynamic process.

Planläggning av cirkulationsplatser har blivit ett vanligt inslag i en mängd olika städer världen över för att lösa farliga situationer som kan uppstå i traditionella fyrvägskorsningar. Detta eftersom bilisternas hastighet hålls låg i en cirkulationsplats jämfört med en vanlig fyrvägskorsning vilket i sin tur leder till mindre allvarliga olyckor. Är de oskyddade trafikanternas säkerhetsfördelar vid en cirkulationsplats lika tydliga? Syftet med studien är att undersöka utformningen av trafikmiljöer, med fokus på fotgängares och cyklisters säkerhet i anslutning till cirkulationsplatser. Jag har sett på vilka säkerhetshöjande aspekter som bör tas i beaktande för oskyddade trafikanter. Frågeställningarna i arbetet är - Vilka för- och nackdelar för oskyddade trafikanter finns det med planläggning och konstruktion av cirkulationsplatser? - Vilka säkerhetsaspekter tas i beaktande vid cirkulationsplatser? Forskningen baserar sig på en kvalitativ metod. Datainsamlingsmetoder som använts är observation, intervju och kvalitativ textanalys. I undersökningen deltog tre planerare från Gävle, Eskilstuna och Västerås. I undersökningen lyfts säkerhetsaspekter, samt för- och nackdelar fram som de undersökta kommunerna utgår ifrån i planläggningen av cirkulationsplatser och övergångsställen. I resultatet kan konstateras att det med snabba och relativt enkla metoder och åtgärder är möjligt att förbättra fotgängares och cyklisters säkerhet på passager vid cirkulationsplatser. / Planning of roundabouts have become a common feature in various cities around the world to resolve dangerous situations that may occur in traditional four-way intersections. This is because the motorists’ speed is lower in a roundabout compared to a standard four-way intersection, which leads to less severe accidents. Are vulnerable road users safety benefits at a roundabout as clear? The purpose of the study is to examine the design of traffic environments, with a focus on pedestrian and cyclists’ safety in connection to roundabouts. I have seen on the safety aspects that should be taken into consideration for pedestrians and cyclists. The research questions in my study are: - What are the advantages and disadvantages of vulnerable road users with planning and construction of roundabouts? - Which safety aspects are taken into consideration in roundabouts? The research is based on a qualitative approach. Data collection methods used are observation, interview and qualitative text analysis. The study included three planners from Gävle, Eskilstuna and Västerås. The highlighted safety aspects, advantages and disadvantages presented by the surveyed municipalities proceed from with planning of roundabouts and pedestrian crossings. In the result it can be stated that with fast and relatively simple methods and steps are possible to improve pedestrian and cyclist safety on crossings at roundabouts.
