For many years it is common knowledge that is the best food for infants up to 06 months old. Since ancient times, women breastfed their children and, in some occasions, helped feeding other women¿s. Human Milk Banks and their donors are, today, what net-nurses were for more than two thousand years. Objective: Identify perceptions of mothers donors of milk in a human milk bank linked to a midsize Hospital in the hinterlad of Paraiba state. Methodology: The study was conducted by a qualitative approach with 21 milk donors of the Cajazeiras Human Milk Bank. Data collection was carried out by interviews with semi-structured script. The contents of the transcriptions were submitted to analysis and the thematic nuclei found were discussed. Results: the donors were 17 to 41 years old, all of them married or in non-legal consensual marriage and the great majority living only with their companions and children, half of them with paying jobs and the other half as housewives, and the majority of them were women who had more than one children. The following thematic nuclei emerged from the analysis: Altruism or solidarity with the imaginary mother and children, shown by "I donate to help the babies in need"; Social consciousness, avoiding waste, shown by "I had so much milk that it was going to waste"; Empowerment of the breast, or "the babies depend on me to be fed"; Desired pregnancy and history of loss, detected by the recurring stories of losses associated to the idea of a very precious pregnancy, "the doctor said that it was more likely that I would have multiple abortions than giving birth to a living child"; Family support, detecting that family presence can either help ("my mother is my role model and stimulates me a lot") or get in the way ("my mother-in-law was against it, she didn't want me to donate"); and Social support, shown from pre-natal care to the donation itself ("the nurse explained to me that it wouldn¿t lack milk to my baby, that the more you take, the more come"). Conclusion: the perceptions of mothers on milk donation were very positive, surrounded by ideas of solidarity, empathy and pride of their actions and demonstrate also that Health Promotion, from Primary Care at pre-natal care to a good welcome at the Maternity and the Human Milk Bank can be important determinants of the maintenance of breastfeeding and the decision to donate milk. / Introdução: A importância do aleitamento materno é hoje algo indiscutível. Há muitos anos sabe-se que é melhor alimento para menores de 06 meses. Desde a Antiguidade, mulheres amamentavam seus filhos e, em algumas situações, ajudavam a alimentar os de outras mães. Os bancos de leite humano e suas doadoras são, hoje, o que as amas de leite foram por mais de 02 mil anos. Objetivo: Identificar percepções de mães doadoras de leite em um banco de leite humano vinculado a hospital de médio porte no sertão da Paraíba. Método: O estudo foi conduzido por uma abordagem qualitativa com 21 doadoras do Banco de Leite Humano de Cajazeiras. A coleta de dados se deu por entrevistas com roteiro semiestruturado. Os conteúdos das transcrições foram submetidos a análise e foram discutidos os núcleos temáticos encontrados. Resultados: as doadoras tinham idade entre 17 e 41 anos, todas casadas ou em união consensual e a grande maioria residindo apenas com seu companheiro e seu filho ou filhos, metade delas trabalhando fora e a outra metade dona de casa, e a maioria era multípara. Como núcleos temáticos, surgiram das análises dos discursos: Altruísmo ou solidariedade a mãe e criança imaginárias demarcado por "doo para ajudar os bebês que precisam"; Consciência social do não-desperdício constatada pelo "eu tinha tanto leite que iria estragar"; Emponderamento do peito, ou "os bebês dependem de mim para se alimentar"; Gravidez desejada e histórico de perdas, constatado pelas recorrentes histórias de perdas associadas à ideia de uma gravidez muito preciosa, "o médico disse que era mais fácil eu abortar do que ter um bebê"; Suporte familiar, constatando-se que a presença dos familiares tanto pode ajudar muito ("minha mãe é meu exemplo, me estimula muito"), como atrapalhar bastante ("minha sogra era contra, não queria que eu doasse"); e Apoio social, visto no suporte desde o pré-natal até a doação em si ("a enfermeira me explicou que não vai faltar pro meu bebê, que quanto mais tira, mais leite vem"). Conclusões: as percepções das mães sobre a doação de leite foram bastante positivas, cercadas de ideias de solidariedade, empatia e orgulho das ações desempenhadas e demonstram também que as ações de promoção à saúde, desde a atenção básica no pré-natal até um bom acolhimento na maternidade e banco de leite podem ser determinantes importantes na manutenção do aleitamento e na decisão de doar leite.

Psychosocial predictors of posthumous organ donation intention: a comparison among Chinese, Japanese and American adults. / CUHK electronic theses & dissertations collection

By path analysis, it was found that only self-efficacy and subjective norm, but not general attitudes, significantly predicted individuals' BID across all samples. Moreover, in contrast to the hypothesis, results showed that after-death anxiety predicted self-efficacy but not attitudes. Low after-death anxiety, accurate knowledge regarding organ donation, and high subjective norm promoted self-efficacy. Altruism was the main determinant of attitudes but its indirect impact on BID was weak. The applicability of the model on all four samples was acceptably high, and the configural invariance of the model was generally supported across three ethnic groups. / Previous studies have shown that the general public exhibits favorable attitudes toward posthumous organ donation, but a low donor card signing rate. Hence, positive attitudes may not be a good predictor of the behavioral commitment to donate organs. This dissertation was a cross-cultural study on the actual behaviors of signing or taking away the donor card (i.e. b&barbelow;ehavioral i&barbelow;ntentions to d&barbelow;onate organs after death (BID). It aimed at proposing an integrative, content-specific but culture-general model for posthumous organ donation. The model was composed of two levels of influences on BID, namely, the proximal level (including self-efficacy toward signing the donor card, general attitudes toward posthumous organ donation, and subjective norm for posthumous organ donation) as well as the distal level (including knowledge regarding posthumous organ donation, altruism, and after-death anxiety). The model was first tested and refined among 517 Chinese college students (Study 1), and was then further validated with 290 Chinese community adults (Study 2) in Hong Kong. The applicability of the model was also examined with a Western control sample of 217 Caucasian American college students (Study 3) and a non-Chinese Asian sample of 670 Japanese college students (Study 4). Psychosocial characteristics of three college samples were then compared, and ethnic differences on predictive values of psychosocial factors on BID were studied (Study 5). / The ANOVA results challenged the old assumption of underlying similarities in psychosocial characteristics across Asian ethnic groups. As expected, Americans were the most likely to show BID, followed by Chinese, while Japanese had the lowest tendency to show BID. The thesis deals with the issue of blood donation and it has a total of fifteen parts. The first chapter describes theoretical knowledges of motivational factors, altruism and economic view on it. It is followed by informations about the history of blood donation and treatments with blood, anatomy of blood and blood groups. The section of blood donation provides general informations about conditions of donation, resulting benefits, risks for donor and recipient and commercial collection of plasma. The thesis focuses especially on the blood market in the Czech Republic in terms of legislation, supply of blood, its evaluation and ultimately safety of blood. At the end there are published results of the questionnaire survey. Its aim was to find out how the public is looking at blood donation, its promotion, safety and reasons to donate or not to donate blood.

In the past decade, several studies focused in quantifying, analysing and describing the aspects of live kidney donation. Due to its complexity, it has conflicts in psychic, social, spiritual and existential aspects. The present study aims to describe the fantastic world of donors with the choice of donating a kidney and understand the possible choices of those who have performed the transplant. Meaning to describe the life of each donor, this is a phenomenologic qualitative research with an interview with six kidney donor adults living in São Paulo. The interviews were performed by questions followed by a trigger question considering the participant s answer and following its phenomenologic flow. For data analysis and synthesis, the following steps were put into practice: description, reduction and phenomenon interpretation. The lack of a psychological work on the donors was noticed before, during and after donation. Finally, this essay reveals being a kidney donor has new challenges and confrontations to save the life of a family member, in addition to the subjective aspects in the choices and decisions of being a donor / Com o grande avanço da biotecnologia foi possível observar um crescente número de transplantes renais com doadores vivos no Brasil. Na última década, muitos estudos têm se preocupado em quantificar, analisar e descrever os aspectos do fenômeno da doação renal intervivos. Por sua complexidade, envolve conflitos nas suas dimensões psíquicas, sociais, espirituais e existenciais. Este estudo teve como objetivo descrever o que aconteceu no mundo fenomenal de pessoas doadoras com a escolha de doar um rim e apreender as possibilidades de escolhas dessas pessoas que realizaram o transplante. Com a intenção de descrever a vivência interna de cada doador, foi realizada uma pesquisa qualitativa de abordagem fenomenológica entrevistando seis participantes adultos que realizaram a doação renal na cidade de São Paulo. Foram realizadas entrevistas através da construção de perguntas seguidas à pergunta disparadora, a partir do discurso do doador participante, seguindo o seu fluxo de sentido fenomenológico. Para a análise e síntese dos dados, seguiu-se as seguintes etapas: descrição, redução e interpretação fenomenológica. Apurou-se a insuficiência do trabalho psicológico realizado com os doadores, que deve abranger as fases pré, durante e pós doação. Por fim, o presente estudo revelou que a experiência de ser um doador renal apresenta novos desafios e enfrentamentos em prol do desejo de salvar a vida de um familiar enfermo e ainda reflete aspectos subjetivos que permeiam nas escolhas e decisões de ser doador renal

Le mythe des conventions constitutives et translatives de droits réels / The myth of the agreements establishing and transfering real rights

Dans les Droits antérieurs à la codification de 1804, les conventions ne suffisaient pas, en principe, à constituer et à transférer les droits réels : pour atteindre ce résultat, un mode acquisitif, comme la tradition de la chose, était nécessaire. Quand ce n’était exceptionnellement pas le cas, le droit réel, qui était constitué ou transféré par la seule convention, était néanmoins constitué et transféré erga omnes. Or, à partir du code civil et des réformes législatives postérieures, il n’en va plus ainsi : le principe adopté est celui de la constitution et de la translation des droits réels sola conventione.Mais l’étude des textes législatifs et réglementaires ainsi que des opinions doctrinales du XIXe siècle révèle que le changement initié en 1804 est bien plus profond : si la convention suffit à constituer et à transférer les droits réels, ces droits ne sont constitués et transférés qu’à l’égard des parties.Ainsi, en Droit français actuel, la vente d’un bateau en transfère la propriété à l’égard du vendeur, mais non à l’égard des tiers et, en particulier, des créanciers chirographaires du vendeur ou de l’État. Tant que l’inscription de l’acte de vente n’est pas accomplie, ces tiers peuvent ignorer le transfert de la propriété causé par la vente. Dès lors, certaines questions se posent inévitablement : la vente a-t-elle réellement transféré la propriété ? Une propriété à l’égard du seul vendeur est-elle encore réellement une propriété ? De même, en Droit français actuel, une convention peut constituer une hypothèque, mais ce droit n’est pas opposable aux tiers tant que cet acte n’a pas été inscrit. Ainsi, les créanciers chirographaires du constituant peuvent ignorer le droit du créancier hypothécaire. Mais une hypothèque qui n’est pas un droit de préférence n’est-elle pas un concept absurde ? Cette thèse explique comment le législateur et la doctrine du XIXe siècle, en réduisant le mode acquisitif au titre acquisitif et en distinguant entre effet inter partes et effet extra partes, ont construit un mythe : celui des conventions constitutives et translatives de droits réels. Contrairement aux formules généralement retenues par la loi et les manuels, en Droit français actuel, les droits réels sont constitués et transférés par la convention et la tradition ou l’inscription. / The Law prior to the 1804 codification is ruled by the principle that agreements are not sufficient to establish and transfer real rights: in order to reach this result, an additional modus of acquisition, like the delivery of the thing, is required. When it is not the case, the real right, established or transferred by mere agreement, is nevertheless established and transferred erga omnes. However, since the civil code and the successive legislative reforms, the situation is no longer the same: the principle adopted is the establishment and the transference of real rights sola conventione. Nonetheless, the study of laws and regulations as well as the nineteenth century scholars’ opinions reveals that the change initiated in 1804 is much deeper: if the agreement is sufficient to establish and transfer real rights, these rights are established and transferred to the parties only. For example, in current French Law, the sale of a boat transfers its property regarding to the seller, but not towards to third parties and, in particular, not regarding to the seller’s unsecured creditors or to the State. As long as the registration of the deed of sale is not completed, these third parties may ignore the transfer of property resulting from the sale. A lot of questions can be raised: has the sale really settled a transfer of property? Is a property regarding only to the seller still really a property? Similarly, in current French Law, an agreement may establish a mortgage, but this right is not enforceable against third parties as long as this act has not been registered. Thus, the unsecured creditors of the settlor may ignore the right of the mortgagee. But does it make sense to consider a mortgage without a preference right?This thesis describes how the legislator and nineteenth century scholars, in reducing the acquisitive modus into the acquisitive titulus and in distinguishing between inter partes effect and extra partes effect, have built a myth : the one of the agreements establishing and transferring real rights. Unlike formulas generally used by law and textbooks, in current French Law, real rights are established and transferred by agreement and delivery or registration.

Doação de órgãos e visão da família sobre atuação dos profissonais neste processo: revisão sistemática da literatura brasileira / Family\'s opinion on professionals\' attitude during process of organs donation: a systematic review from Brazilian literature

A história dos transplantes vem rompendo barreiras e enfrentando desafios embora de forma incipiente, por diversas décadas, conquistando impressionantes resultados, demonstrando a evolução científica e tecnológica aplicada ao setor. Assim, observa-se que, ao longo dos tempos, o número de doações tem aumentado. Porém, não o suficiente para reduzir a lista de espera, cabendo então a reflexão acerca das causas que dificultam esse processo. Diversos são os motivos pelos quais afetam a doação de órgãos, ressaltando-se a recusa dos familiares e a não notificação de possíveis doadores. Assim, vale ressaltar a importância do preparo profissional no que diz respeito à abordagem familiar, momento decisivo para que se possa dar continuidade ao processo de captação de órgãos. Diante do exposto, o estudo tem por objetivo explorar, recolher, organizar, sintetizar e compartilhar visão dos familiares em relação a atuação dos profissionais no processo de doação de órgãos. A metodologia retrata uma revisão sistemática da literatura brasileira sobre o tema central baseando na busca de artigos que demonstrassem resultados de pesquisas realizados no País, entre 2001 e 2011, no portal PubMed e nas bases de dados SCOPUS, CINAHL, EMBASE, Web of Science, Science Direct, LILACS, BDENF. Tal busca guiou-se pela pergunta: Qual a visão dos familiares, que passaram pela abordagem para doação de órgãos, referente à atuação dos profissionais que atuam neste processo? Como resultados dos 265 trabalhos encontrados, foram considerados pertinentes, à temática em apreço, 06 artigos, os quais foram submetidos à leitura criteriosa da metodologia utilizada, dos sujeitos investigados, dos resultados obtidos e das conclusões. As sínteses foram construídas a partir da análise temática dos resultados. Surgiram 03 categorias. Sendo elas: 1) Falta de confiança na atuação profissional; 2) Atuação profissional sem compreensão e acolhimento no momento familiar; 3) Falta de informação pelo profissional. Desta última, emergiram duas subcategorias: 1) Falta de informação pelo profissional à família referente à possível Morte Encefálica; 2) Falta de informação pelo profissional à família referente aos trâmites pós-doação. Consideramos, portanto, que a complexidade de ações que são necessárias no processo de doação de órgãos, foi revelada como burocrático, demorado, desgastante e cansativo, resultando em sofrimento e submetendo tanto a família quanto os profissionais a situações estressantes. Disto depreendemos haver necessidade de maiores investimentos na formação dos profissionais que atuam neste processo, melhorando seu suporte emocional e sua atuação, a qual é considerada incipiente pelos familiares. / Transplants\' history are breaking barriers and facing challenges for several decades achieving impressive results. It demonstrates scientific and technological developments applied to this sector. Although an increasing on organs donations have been noted, it is not enough to reduce waiting list. Therefore there is a need for reflection on the causes, which makes this process. There are several reasons that affect organ donation, and shall be emphasized: relatives\' refusal and failure in notifying potential donors. It is also worth highlighting the importance of professional competence in respect of family approach, a decisive moment that gives continuity to the process of organ retrieval. In this light, the study aims to explore, collect, organize, synthesize and share the opinion of relatives about the role of professionals during process of organ donation (POD). As methodology a systematic review of Brazilian literature was conducted between 2001 and 2011. Data was collected from (i) PubMed, (ii) SCOPUS, (iii) CINAHL, (iv) EMBASE, (v) Web of Science, (vi) Science Direct, (vii) LILACS, and (viii) BDENF. Such data collection was performed based on one question: What is the opinion of relatives, who went through organ donation approach, referring to professionals\' actions working in this process? Six out of 265 papers were considered relevant, and then a careful reading executed to assess methodology, subjects investigated, results and conclusions. Analyses have enable to define three categories: (a) lack of confidence in the professional, (b) professional attitude without understanding of relatives emotional state, (c) professional apathy in provide information to family. The last category was subdivided in (i) lack of info about a possible brain death, and (ii) subsequent procedures about POD. These study revealed a complexity in POD particularly on bureaucracy, delay, family\'s fatigue and wear. Thus, we may conclude to be necessary investing efforts on professional training involved in the POD to protect family\'s psychological state.

「無語良師」: 一個數位互動敘事之初探與實踐 / The silent mentor: a preliminary delving and practices of digital interactive narration

「無語良師」是一群已往生者用身體教育醫學院學生的故事。這群慈濟大學醫學院的遺體捐贈者,在生前即自願將遺體作為醫學院學生解剖課程之用,為了能夠完成捐贈,大多數的老師於生命最後都放棄積極性的治療。而學生們不僅要造訪捐者家人,為其立傳,每次課程前也必須向遺體敬禮問訊,並於期末課程結束後,縫合遺體為其穿衣、送靈。其解剖課程兼具醫學與人文,為現代醫學教育中少有之課程模式,這群捐贈者在慈濟大學即稱為「無語良師」或「大體老師」。 2005年筆者應工作之需製作其紀錄片,在記錄過程中,蒐集各方資料並持續進行拍攝,在素材漸豐之際,數位敘事產製內容之產製概念方興未艾,特別是以網站的形式或互動式DVD,發揮其非線性敘事之特色並強化使用者之互動性,此為內容產製者提供一個不同於以往線性傳播之新的溝通平台。 數位內容具有「數值化的再現」(numerical representation)、「模組化」(modularity)、「自動化」(automation)、「變異性╱液態化」(variability / liquidity)與「轉碼化」(transcoding)(Manovich,2001)之特性,並以其「非線性」與「節點」等之敘事模式創造互動與多元對話的可能。本研究將爬梳數位互動敘事之原理,並以「無語良師」紀錄片之素材作為原料,以實驗性質,嘗試將原線性敘事之故事轉化為數位互動內容作品。 / The Silent Mentor” tells the stories of the body donators who contributed their cadavers to teach the medical students the way to respect and cherish human body. The body donators of the Medical School of Tzu Chi University signed the will to give up any progressive treatment so that they may donate the bodies to the students to carry out surgery practices. Before the students practice dissection they are urged to visit donator’s family and write the life stories of donators, so that students may come to know that they are not dissecting a cadaver but an altruistic spirit and a giving soul. Students bow to the silent mentor every time before practicing dissection. At the end of the practices, students will stitch the body of silent mentor inch to inch, to insure that the body has been reinstated. Aftermath, students put clothes on silent mentors and participate their memorial ceremonies to express their deep gratitude and farewell to silent mentors. The program of dissection that combines both medical knowledge and humanity has been the paradigm of modern medical education. The body donators thus, named as “The Silent Mentor”. At the time I am producing a documentary on this extraordinary story for Da Ai TV Station, I extensively gather relative data and conduct interviews on this subject. Before I go for editing, the digital narration increasingly becomes an alternative trend of new media technology. It is especially true that the Internet interactive format and interactive DVD have developed the nonlinear narrative advantage and thus, provide an accessible platform for the viewers to mutually interact with the content providers. Digital content accommodates the characteristics of numerical representation, modularity, automation, variability / liquidity, and transcoding, furthermore, via the nonlinear and plots, it provides the accessibility of multiple and interactive dialogues. My research will aim to delve and develop the principles and techniques of digital interactive narration. Then the documentary of “The Silent Mentor” will be my illustration, from which, I am going to experimentally develop a model describing how would the traditional linear narrative element, which “The Silent Mentor” should be carried, transform into nonlinear digital interactive narrative model.

A study of inscribed reliefs within the context of donative inscriptions at Sanchi

Inscribed relief art at the early Buddhist archaeological site of Sanchi in India exhibits at least one interesting quality not found elsewhere at the site. Sanchi is well known for its narrative reliefs and reliquaries enshrined in stūpas. However, two inscribed images of stūpas found on the southern gateway record the gifts of two prominent individuals. The first is a junior monk whose teacher holds a high position in the local order. The second is the son of the foreman of the artisans of a king. Both inscribed stūpa images represent a departure from a previous donative epigraphical habit. Instead of inscribing their names on image-less architectural pieces, these two particular individuals inscribed their names on representations of stūpas, a symbol with a multiplicity of meanings. In this thesis, I use two perspectives to analyze the visual and verbal texts of these inscribed reliefs. In the end, I suggest that these donations were recorded as part of the visual field intentionally, showing the importance of not only inscribing a name on an auspicious symbol but also the importance of inscribing a name for the purpose of being seen. / text
