Examining mother’s related socioeconomic and demographic determinants of infant and child mortality in the Eastern Cape, South Africa

Magister Philosophiae - MPhil / Infants and under 5 mortality have been universally researched and it reduction by twothird by 2015 has been announced as one of the millennium development goal of the United Nation. Continuous monitoring in form of studies on mothers related factors that determines infant, child and under 5 mortality appears to be a step towards achieving this goal. Therefore using the Eastern Cape Province as a case study, this study utilises the secondary data of the 1998 South Africa Demographic and Health Survey (SADHS) dataset for children to examine some specific mothers related socioeconomic and demographic determinants of infant and child mortality. As at the time this study is being conducted the SADHS 2003 survey datasets are not available. However, reports that summarize the survey and some preliminary results are available. In this study, infant and child mortality have been grouped into 0-12 months and 13-60 months age at death,which is taken as independent variable. Profiles of the distributions of the dependent and the independent variables are described with the aid of cross tabulation. Demographic factors examined include the age of the mother at the time of delivery, order of birth and birth interval, while the socioeconomic factors examined include working status of the mother, mother‟s work place (at home work at home or away) mothers‟ education level , and the place of residence. We found that demographic variables such as age of mothers at first birth, birth order, birth interval, and socioeconomic factors such as mothers‟ education level, and the place of birth, have a significant effect on the probability of child‟s survival. Therefore, the postponement of the female age at birth with an appropriate child spacing, nonetheless, the improvement mother‟s education will enhance the reduction of infant and child mortality. Steps that may be taken towards improving the health status of infant and child, including ways by which infant and child mortality may be reduced are recommended.

A medida foi idealizada originalmente para que as partes pudessem obrigar o cumprimento do dispositivo da decisão proferida pelos Tribunais Superiores nos processos que participaram, ou para garantir o respeito à declaração proferida no controle abstrato de constitucionalidade de determinada norma. Com a vigência do Código de Processo Civil de 2015, desacatar a autoridade da decisão não se resume a desrespeitar os comandos judiciais dos processos subjetivos e objetivos, mas também as razões de decidir, os fundamentos determinantes das decisões elencadas no art. 988 do CPC/2015. A partir da análise da evolução histórico- normativa do instituto, de sua natureza jurídica, de seus fundamentos e finalidades, será demonstrado que é impossível utilizá-lo de forma uniforme, considerando que aos poucos tem se distanciado de suas funções tradicionais, passando a ser instrumento para a resolução de problemas distintos que também exigem soluções distintas: o desrespeito ao que foi decidido ao caso concreto, o desrespeito às decisões em controle de constitucionalidade e a falta de isonomia no julgamento de casos idênticos. Instigado em trazer soluções práticas e utilidade à reclamação, pretende-se especialmente responder como ela poderá se adequar para garantir a observância dos precedentes. Den här rapporten beskriver ett projekt som gick ut på att undersöka några av de metoder och tekniker som en dokumentärfilmare kan använda sig av i sitt arbete. Metoden som användes gick ut på att dels läsa litteratur på ämnet men huvudsakligen att producera en egen dokumentärfilm för att på så vis ta reda på hur olika tekniker fungerade och vilka för och nackdelar som fanns med dem.Väldigt mycket information kom fram genom arbetet, så mycket så att en hel del inte kunde tas med i rapporten på grund av tidsbrist. En subjektiv bedömning gjordes i slutändan för att bestämma vad som var intressant nog för rapportens mål.Rapporten beskriver först bakgrunden till problemet och går sedan vidare med att kronologiskt beskriva arbetsmetoden och de olika situationer som uppstod under arbetets gång. Därefter beskrivs några av de tekniker som användes på ett objektivt vis och slutligen kommer en subjektiv tolkning av resultatet.

Le cholestérol comme agent cryoprotecteur pour la congélation des semences animales

Parce que le cholestérol spermatique influence la résistance des spermatozoïdes à la congélation, nous avons émis l’hypothèse que les spermatozoïdes supplémentés en cholestérol exogène subiraient moins de dommages pendant la cryoconservation améliorant leur pouvoir fécondant après décongélation. Etant insoluble, le cholestérol est incorporé dans la méthyl β-cyclodextrin formant un complexe hydrosoluble, cholestérol lié à la cyclodextrine (CLC). Cette étude à montré que le traitement CLC, dans un dilueur de lait écrémé, améliore la cryorésistance de la semence de bouc en augmentant le niveau de cholestérol spermatique, responsable de la résistance des spermatozoïdes au choc au froid et au stress osmotique, sans affecter la capacitation induite in vitro après décongélation. Par insémination artificielle, les semences ayant subient une augmentation de cholestérol exogène ont montré un meilleur taux de fertilité et de prolificité in vivo par rapport à la semence non traitée. Ces observations suggèrent que le traitement CLC pourrait être utilisé pour améliorer la fertilité des troupeaux et mérite des explorations chez d’autres espèces. Evaluer l’efficacité du CLC sur la cryorésistance de la semence de bélier nous a permis de montrer que le traitement CLC est plus efficace dans un dilueur de lait écrémé par rapport à un dilueur de jaune d’œuf commercial. Toutefois, cette amélioration de la qualité spermatique post-dégel n’est pas suffisante pour induire une augmentation de la fertilité in vivo, suggérant la nécessité d’optimiser le protocole de congélation ou une adaptation du protocole d’insémination. Cependant, combiner le traitement CLC au tréhalose (un cryoprotecteur externe) n’a pas montré d’effet synergique sur l’amélioration de la qualité spermatique post-dégel. Finalement, pour amorcer un transfert technologique nous avons montré que le traitement CLC permet de conserver la qualité spermatique de la semence de bouc en présence de plasma séminal, dont le retrait était recommandé en raison de son effet délétère. De plus, le traitement CLC permet le maintain de la qualité de la semence bouc conservée en frais pendant 24 h à 5°C avant congélation facilitant son transport de la ferme vers le laboratoire. Ces résultats démontrent que le cholestérol joue un rôle crucial et essentiel dans la cryoprotection des semences animales. / Because sperm cholesterol content contributes to their resistance to freezing, we hypothesized that exogenous cholesterol incorporated in sperm reduces cryodamage and thus improves the fertilizing capacity of thawed sperm. Being a lipid, cholesterol is insoluble in aqueous media, rendering difficult to deliver to cells in vitro. Methyl β-cyclodextrin is a carrier molecule that couples to cholesterol to form a water-soluble compound, “cholesterol-loaded cyclodextrin” (CLC) that can transfer the cholesterol into cell membranes. The research within this thesis shows that CLC treatment of goat semen enhances fresh sperm resistance to cold shock and osmotic stress due to the increased cholesterol, involving in sperm cryoresistance in a skim milk-based extender without affecting sperm in vitro capacitation after thawing. A pilot field trial in goats demonstrated that artificial insemination with sperm that underwent increased exogenous cholesterol yielded higher fertility and prolificacy rates in vivo compared to untreated semen. These observations suggest that CLC treatment could be used to improve frozen sperm quality and fertility rate of other species. Using ram sperm, our study demonstrated that CLC treatment was more efficient in a skim milk-based extender compared totraditional egg yolk-based extender. However, in vivo fertility of the ram semen that was cryopreserved in the skim milk-based extender with CLC did not differ from semen that was cryopreserved in egg yolk-based extender without CLC. Further research is warranted to combine CLC with other cryoprotection strategies or to modify the insemination protocol to adequately permit capacitation in vivo. However, using CLC treatment with trehalose, a cell impermeable cryoprotectant, did not demonstrate any synergic effect of the cryoprotectants on thawed sperm quality. Finally, in an effort to develop a protocol for semen cryopreservation that can be accessible to the emerging goat industry in Quebec, a pilot test trial demonstrated that goat sperm treated with CLC are more resistant to exposure to seminal plasma than CLC-free sperm in skim milk-based extender. Additionally, CLC treatment markedly improves the post-thaw quality of the sperm after temporary storage for 24 h prior to processing. Together, these results demonstrated that cholesterol has a fundamental and innovative role in animal semen cryoprotection.

Par contre, cette formule ne s’applique qu’aux domaines simplement connexes. Récemment, Darren Crowdy a obtenu une généralisation de cette formule pour les domaines multiplement connexes. Celle-ci envoie des domaines circulaires sur des domaines polygonaux. Ce mémoire vise à faire la démonstration de la formule développée par Crowdy. Pour ce faire, il faudra définir la fonction de Schottky–Klein ainsi que la fonction de Green modifiée. Il faudra aussi introduire les domaines canoniques.

Le traitement de l'information dans l'insomnie

L’hyperactivation corticale chez les individus souffrant d’insomnie a été confirmée à de maintes reprises, mais les impacts de cette hyperactivation ont été moins étudiés. Parmi ces impacts, se retrouve l’hypothèse qu’une hyperactivation corticale puisse se traduire en une altération du traitement de l’information durant le sommeil. Le présent mémoire présente des données concernant le traitement de l’information durant le sommeil chez des individus souffrant d’insomnie, ainsi que des bons dormeurs, tout en liant ces données avec la qualité de leur sommeil. Les résultats suggèrent que le traitement de l’information varie selon la qualité de sommeil et ce tant chez les bons dormeurs que les individus souffrant d’insomnie. Les résultats suggèrent également, qu’à qualité de sommeil comparable, les individus souffrant d’insomnie présentent une altération du traitement de l’information durant le sommeil et ce particulièrement au niveau de la capacité à inhiber le traitement de stimuli non-pertinents. / Cortical hyperarousal has been well documented among insomnia sufferers. The effects of that hyperarousal are however still misunderstood. Since hyperarousal appears to be present during both wake and sleep among insomnia sufferers, it should translate into enhanced information processing at night and might impact the subjective perception insomnia sufferers have of their sleep. The scientific article that is presented in this master’s thesis investigated information processing during sleep among insomnia sufferers and good sleepers while controlling for sleep quality. Results suggest information processing varies regarding sleep quality for both insomnia sufferers and good sleepers. However, the data also reveals that when sleep quality is accounted for, insomnia sufferers still show an alteration of their information processing when compared to good sleepers. This alteration specifically affects their ability to inhibit the processing of irrelevant information, which could make them more vulnerable to environmental and internal stimulation during sleep.
