Foram atendidos no PS do Hospital de Clínicas da Faculdade de Ciências Médicas na Unicamp, de janeiro de 1990 a dezembro de 1995, 23 pacientes com dores abdominais, sendo 20 homens e três mulheres com idade de 44 a 80. Seis pacientes (26,08%) tinham conhecimento da doença e 17 (73,91%) não tinham. Cinco pacientes (22%) foram encaminhados de outros hospitais com diagnóstico equivocado: cólica renal, pancreatite, patologias de coluna vertebral e lombalgia. Foram operados 18 pacientes (78,26%) de urgência e cinco pacientes (21,73%) de emergência. Dos cinco pacientes atendidos na emergência, quatro já tinham feito US, dois com TC e um paciente sem nenhum exame. Na urgência, oito pacientes (34,78%) fIZeramUS e 15 pacientes fizeram Te. Os 23 pacientes foram divididos em dois grupos: os que apresentavam quadro clínico de rotura e que foram atendidos no PS antes de 24h e os que foram atendidos após 24h. Dos 23 pacientes, sobreviveram dez (43,47%) com 56,52% de mortalidade. O infarto agudo do miocárdio foi a patologia que causou maior número de óbitos. A cirurgia para correção do AAAR mostrou que os pacientes atendidos na emergência com evolução clínica de rotura maior que 24h apresentaram melhor sobrevida no pós-operatório que os pacientes com evolução clínica de rotura menor que 24h. Os pacientes atendidos na urgência em comparação com os da emergência apresentaram uma melhor sobrevida no pós-operatório. Assim, conclui-se que quando houver forte suspeita clínica de AAAR, os pacientes devem ser encaminhados imediatamente à sala de operação. O paciente deve ser tratado cirurgicamente antes de transcorridas 24h do primeiro sintoma, considerando que o diagnóstico de AAAR é essencialmente clínico. Os pacientes admitidos no PS, com quadro clínico de abdome agudo devem ser submetidos à laparotomia exploratória, não se esquecendo de que pode ser AAART. Os pacientes com história de AAA serão submetidos à laparotomia para o tratamento da rotura do aneurisma. Os pacientes com mais de 60 anos de idade devem ser submetidos a exame de imagem para avaliar aorta e tratar profilaticamente o aneurisma antes da rotura / Abstract: The present study OOsthe objective of both establishing a nursing in the Emergency room (E.R) for patients suspected, of OOvingruptured abdominal aortic aneurysms (RAAA) and reducing the morbidity and mortality rates. Twenty-three patients with acute abdominal pain were operated on in the emergency room of Clinical Hospital in the Unicamp between January 1990 and December 1995: twenty men and three women with ages varying ftom 44 to 80. Six patients (26,08%) OOdprevious knowledge of their illness and 17 (73,91%) didn't OOveany. Five patients (22%) OOdcome from other hospitaIs with wrong diagnostic: renal pain, pancreatic and vertebral spine pathology. Eighteen patients (78,26%) underwent urgent surgery and five patients (21,73%) were operated on in the Emergency room (E.R). From these, four had already undergone US, two had done CT and one had no exams. In the Urgency room (D.R), eight patients (34,78%) had undergone US and fifteen OOddone CT. From the total of 23 patients, 13 did not survive. The acute myocardial infarction was the pathology, which caused high mortality rates. CONCLUSION: l-Surgery to correct RAAA showed that patients were nursed into the Emergency room with clinical evolution of ruptured afier 24h, showed a better post-operative prognosis than patients of ruptured before 24h; 2-The patients were nursed in the Urgency room comparing with those of Emergency room, showed a better prognosis in the post-operative. So, we conclude tOOtwhen there is strong medical suspicion of RAAA, patients must be immediately taken to the operating room; 3-Patients must be surgically treated within 24h ofthe first syrnptoms, considering tOOthe RAAA diagnosis is essentially clinical; 4-Patients taken into the Emergency room, with acute abdominal pain ofunknown cause, must be submitted to a sound surgery, OOvingin mind tOOthe result can be that of a tamponed RAAA; S-Patients with AAA history will OOveto undergo on operation to correct the rupture of the aneurysm; 6-Patients over 60 must OOvethe aorta evaluated through CT or US and the aneurysm prophylatically treated to prevent rupturing. Orientador: Marcia Ferreira da Rosa Sobreira / Resumo: Alterações hemostáticas são anormalidades suspeitadas em equinos com obstruções intestinais, estando associadas a elevadas taxas de morbidade e mortalidade. Contudo, as técnicas e/ou testes usados rotineiramente para avaliar a hemostasia apresentam grandes limitações, pois avaliam fases ou componentes isolados do processo hemostático. Em contraste, a tromboelastografia é uma técnica atual que avalia a dinâmica do processo de coagulação sanguínea em conjunto, tornando possível a obtenção de informações acuradas. Como estudos recentes relataram que o uso de plasma fresco ou congelado estaria apresentado resultados satisfatórios tanto em seres humanos como em animais, o presente estudo teve como objetivo inicial realizar testes hemostáticos convencionais (TP, TTPA, Fibrinogênio) e viscoelásticas (tromboelastografia) do plasma de 20 equinos hígidos, 20 equinos com obstrução simples (compactação de colo) e de 20 com obstrução estrangulativa (vólvulo) de alças intestinais de ocorrência natural. Na interpretação dos resultados, verificou-se que o uso da Análise por Componentes Principais foi efetiva na caracterização dos indíviduos a partir dos resultados dos testes realizados. Ademais, ao transformar a interação entre os componentes do processo hemostático em uma visualização gráfica, a tromboelastografia possibilitou uma interpretação e entendimento efetivos do que está acontecendo na coagulação do indivíduo. However, the techniques and / or tests routinely used to evaluate hemostasis have major limitations, since they evaluate isolated phases or components of the hemostatic process. In contrast, thromboelastography is a current technique that evaluates the dynamics of the blood coagulation process together, making it possible to obtain accurate information. As recent studies have reported that the use of fresh or frozen plasma would present satisfactory results in both humans and animals, the present study aimed to perform conventional hemostatic (PT, APTT, Fibrinogen) and viscoelastic (thromboelastography) plasma analyzes of 20 healthy equines, 20 equines with simple obstruction (colon impactation), and 20 with strangulative obstruction (volvulus) of naturally occurring intestinal loops. Assim, o estudo objetivou identificar, diferenciar, quantificar, avaliar e comparar as proteínas séricas de fase aguda em equinos com cólica de ocorrência natural e naqueles submetidos à obstrução intestinal experimental e correlacioná-las com o leucograma destes animais. Utilizaram-se 27 equinos adultos, 12 animais atendidos junto ao Hospital Veterinário Governador Laudo Natel (FCAV/Unesp) no período de julho de 2010 à julho de 2013 para tratamento de abdômen agudo, sobreviventes ou não, e 15 animais submetidos a obstrução intestinal experimental de intestino delgado ou grosso. Foram colhidas amostras de sangue total com anticoagulante, para realização do leucograma, e sem anticoagulante para realização do eletroforetograma em SDS-PAGE imediatamente antes da laparotomia (M0), após o procedimento cirúrgico (M1) e, diariamente, até o sétimo dia pós-operatório, com exceção do sexto dia (M2, M3, M4, M5, M6 e M7). Para análise dos dados utilizou-se o método estatístico multivariado com análise de fatores. Os equinos com cólica natural apresentaram padrão de resposta inflamatória mais intensa que os equinos submetidos à obstrução experimental. Os equinos submetidos à obstrução experimental apresentaram, ainda, resposta inflamatória ao modelo de obstrução instituído. As proteínas de fase aguda tiveram maior participação na variabilidade da resposta inflamatória que o leucograma e, somente quando avaliou-se a influência do tempo em cada grupo, observou-se correlação entre as proteínas de fase aguda e o leucograma. The study aimed to identify, differentiate, measure, evaluate and compare the acute phase serum proteins in horses with naturally occurring colics and those undergoing experimental intestinal obstruction and correlate them with the leukon count in these animals. From the 27 adult horses studied 12 were treated in the Veterinary Hospital Governor Laudo Natel (FCAV / UNESP) from July 2010 to July 2013 for acute abdomen, survivors or not, and 15 animals were subjected to experimental small or large intestinal obstruction. As compactações são afecções mais comuns de abdome agudo, sendo caracterizadas pela distensão intestinal decorrente da estagnação do bolo fecal no intestino, podendo ocorrer mais comumente no ceco ou cólon. Diversas alterações nos parâmetros hemostáticos de equinos com abdome agudo são descritas, sendo mais comuns estudos em animais com afecções graves. Nesse contexto, o objetivo do presente trabalho foi, mediante análises seriadas, avaliar e comparar o perfil hemostático em conjunto com o hematológico, de equinos que apresentam compactação de cólon maior submetidos ao tratamento medicamentoso com animais sadios, com o intuito de melhor compreender os transtornos hemostáticos decorrentes do quadro. Para isso, foram realizados exames hematológicos (eritrograma e leucograma) e hemostáticos (plaquetograma, fibrinogênio plasmático, TP e TTPA) em 20 equinos adultos, sendo dez com compactação de cólon maior de tratamento medicamentoso (GD) e dez hígidos (GC). Os animais foram avaliados em quatro momentos (T0-T72), a cada 24 horas. Nos resultados, verificou-se que no eritrograma o aumento do RDW no GD foi associado a um possível aumento no consumo de oxigênio durante a reação inflamatória. O leucograma apresentou alterações compatíveis com a situação de estresse e dor apresentados pelos animais do GD na admissão e com a possível reação inflamatória desencadeada pelas lesões na microvasculatura. A diminuição gradativa apresentada pelo fibrinogênio foi associada a uma possível resolução da obstrução e diminuição da inflamação ou ao consumo da proteína durante o processo hemostático. The impactions are one of the most common forms of acute abdomen, being characterized by intestinal distention due to the stagnation of fecal mass in the intestine, and can most commonly occur in the cecum or colon. Several changes in hemostatic parameters of horses with acute abdomen are described, more common animal studies with serious diseases. In this context, the objective of this work was by serial analysis, evaluate and compare the hemostatic profile in conjunction with hematologic horses presenting large colon impactation submitted to medication treatment with healthy animals, in order to better understand the hemostatic disorders arising from the framework. For this, blood tests were performed (erythrocyte and leukocyte count) and hemostatic (platelet parameters, plasma fibrinogen, PT and APTT) in 20 adult horses, ten with medical treatment of large colon impactation (GD) and ten healthy (GC). A obstrução do cólon menor é causada principalmente pela compactação de massas ou por enterólitos, acarreta em distensão intraluminal e, consequentemente obstrução dos vasos sanguíneos que irrigam a parede do órgão, desencadeando processo isquêmico. Por conseguinte, ocorrem no organismo alterações metabólicas e enzimáticas, como ativação leucocitária e liberação de enzimas proteolíticas, em direção ao local da injúria. Foram avaliadas as alterações clínicas, hematológicas, celularidade do líquido peritoneal, além da expressão de metaloproteinase 2 e 9 (MMP-2 e MMP-9), em oito equinos, hígidos, adultos, submetidos experimentalmente à obstrução simples de cólon menor, durante 4 horas, por meio de inserção de balão de látex, sob pressão de 80mmHg. Os parâmetros clínicos, hematológicos e o fluido peritoneal foram avaliados antes da obstrução intestinal (M0), imediatamente após a obstrução intestinal (M4) e em intervalos de 12 horas até completarem 72 horas após desobstrução do cólon menor (M72). As atividades gelatinolíticas de MMP-2 e MMP-9, no plasma e líquido peritoneal foram obtidas em M0, M4 e M72, posteriormente submetidas à análise zimográfica. Através do exame físico constatou-se aumento da frequência cardíaca e temperatura retal em M12 e da frequência respiratória em M24. A porção eritrocitária do hemograma não apresentou diferenças significativas, mas evidenciou diminuição discreta de hemácias, hemoglobina, hematócrito e plaquetas em M4. Já o leucograma indicou aumento de leucócitos em M12 e M24 e diminuição na resposta leucocitária a partir de M36 (p≤0,05). Em relação à celularidade do líquido peritoneal verificou-se aumento estatístico dos valores de hemácias, hemoglobina... / Abstract: The syndrome of acute abdomen is one of the diseases that most stands out in the surgical clinical care to the equine species. The obstruction of the lower colon is mainly caused by mass or compression for enterólitos, carries on intraluminal distension and, consequently clogging of blood vessels supplying the organ, triggering ischemic process. Therefore, occur in the body and metabolic changes of enzymes, such as leukocyte activation and proteolytic enzymes release, toward the site of the injury. Were evaluated clinical, haematological changes, peritoneal fluid cellularity, beyond the expression of metalloproteinase 2 and 9 (MMP- 2 and MMP-9), in eight horses, healthy adults, subjected experimentally to simple obstruction of colon minor, during 4 hours, through the insertion of latex balloon, under pressure of 80mmHg. Clinical, hematological parameters and the peritoneal fluid were evaluated before the intestinal obstruction (M0), immediately after the bowel obstruction (M4) and at intervals of 12 hours until they reach 72 hours after clearing the lower colon (M72). Gelatinolíticas activities of MMP-2 and MMP-9, plasma and peritoneal fluid were obtained in M0, M4 and M72, subsequently submitted to zimográfica analysis. Through the physical exam found an increase in heart rate and rectal temperature in M12 and the respiratory rate in M24. The portion of the erythrocyte hemogram showed no significant differences, but showed discrete red blood cells, hemoglobin decrease, hematocrit and platelets in M4. Already the WBC indicated increase of leukocytes in M12 and M24 and decreased leukocyte response from M36 (p ≤ 0.05). Regarding the cellularity of peritoneal fluid was found statistical increase of the values of red blood cells, hemoglobin and hematocrit in M4, leukocytes, mononuclear and polymorphonuclear in M12.
