Assim, o estudo objetivou identificar, diferenciar, quantificar, avaliar e comparar as proteínas séricas de fase aguda em equinos com cólica de ocorrência natural e naqueles submetidos à obstrução intestinal experimental e correlacioná-las com o leucograma destes animais. Utilizaram-se 27 equinos adultos, 12 animais atendidos junto ao Hospital Veterinário Governador Laudo Natel (FCAV/Unesp) no período de julho de 2010 à julho de 2013 para tratamento de abdômen agudo, sobreviventes ou não, e 15 animais submetidos a obstrução intestinal experimental de intestino delgado ou grosso. Foram colhidas amostras de sangue total com anticoagulante, para realização do leucograma, e sem anticoagulante para realização do eletroforetograma em SDS-PAGE imediatamente antes da laparotomia (M0), após o procedimento cirúrgico (M1) e, diariamente, até o sétimo dia pós-operatório, com exceção do sexto dia (M2, M3, M4, M5, M6 e M7). Para análise dos dados utilizou-se o método estatístico multivariado com análise de fatores. Os equinos com cólica natural apresentaram padrão de resposta inflamatória mais intensa que os equinos submetidos à obstrução experimental. Os equinos submetidos à obstrução experimental apresentaram, ainda, resposta inflamatória ao modelo de obstrução instituído. As proteínas de fase aguda tiveram maior participação na variabilidade da resposta inflamatória que o leucograma e, somente quando avaliou-se a influência do tempo em cada grupo, observou-se correlação entre as proteínas de fase aguda e o leucograma. The study aimed to identify, differentiate, measure, evaluate and compare the acute phase serum proteins in horses with naturally occurring colics and those undergoing experimental intestinal obstruction and correlate them with the leukon count in these animals. From the 27 adult horses studied 12 were treated in the Veterinary Hospital Governor Laudo Natel (FCAV / UNESP) from July 2010 to July 2013 for acute abdomen, survivors or not, and 15 animals were subjected to experimental small or large intestinal obstruction. As compactações são afecções mais comuns de abdome agudo, sendo caracterizadas pela distensão intestinal decorrente da estagnação do bolo fecal no intestino, podendo ocorrer mais comumente no ceco ou cólon. Diversas alterações nos parâmetros hemostáticos de equinos com abdome agudo são descritas, sendo mais comuns estudos em animais com afecções graves. Nesse contexto, o objetivo do presente trabalho foi, mediante análises seriadas, avaliar e comparar o perfil hemostático em conjunto com o hematológico, de equinos que apresentam compactação de cólon maior submetidos ao tratamento medicamentoso com animais sadios, com o intuito de melhor compreender os transtornos hemostáticos decorrentes do quadro. Para isso, foram realizados exames hematológicos (eritrograma e leucograma) e hemostáticos (plaquetograma, fibrinogênio plasmático, TP e TTPA) em 20 equinos adultos, sendo dez com compactação de cólon maior de tratamento medicamentoso (GD) e dez hígidos (GC). Os animais foram avaliados em quatro momentos (T0-T72), a cada 24 horas. Nos resultados, verificou-se que no eritrograma o aumento do RDW no GD foi associado a um possível aumento no consumo de oxigênio durante a reação inflamatória. O leucograma apresentou alterações compatíveis com a situação de estresse e dor apresentados pelos animais do GD na admissão e com a possível reação inflamatória desencadeada pelas lesões na microvasculatura. A diminuição gradativa apresentada pelo fibrinogênio foi associada a uma possível resolução da obstrução e diminuição da inflamação ou ao consumo da proteína durante o processo hemostático. The impactions are one of the most common forms of acute abdomen, being characterized by intestinal distention due to the stagnation of fecal mass in the intestine, and can most commonly occur in the cecum or colon. Several changes in hemostatic parameters of horses with acute abdomen are described, more common animal studies with serious diseases. In this context, the objective of this work was by serial analysis, evaluate and compare the hemostatic profile in conjunction with hematologic horses presenting large colon impactation submitted to medication treatment with healthy animals, in order to better understand the hemostatic disorders arising from the framework. For this, blood tests were performed (erythrocyte and leukocyte count) and hemostatic (platelet parameters, plasma fibrinogen, PT and APTT) in 20 adult horses, ten with medical treatment of large colon impactation (GD) and ten healthy (GC). Eight crossbreed mature horses were used with body weight (BW) average of 364kg, geldings, adapted to diet composed by grass hay and concentrate, in a 60:40 proportion. A complete randomized design was used with horses allocated in three treatments. Treatment I: (Control) (n=2) horses were slaughtered without overload carbohydrate; Treatment II (n=3) and III (n=3), horses subjected to overload carbohydrate, with gastric infusion of 17.6 g starch/kg BW, and slaughtered after 24 and 36 hours, respectively. Horses were subjected to clinical, hematological and fecal evaluations before the overload and 2, 4, 8, 12, 16, 20, 24, 28, 32 and 36 hours after overload. Four hours after overload horses became depressed and keeping until the end of evaluation, and one horse presented lameness 36 hours after overload. Any difference (P>0.05) were observed in heart rate, respiratory rate, body temperature and hoof temperature. Increase in packed cell volume and plasma protein concentration were observed 24 hours after overload, varying from 26.7 to 32.0% and 7.1 to 8.1 g/dL, respectively (P<0.05). Differences were observed (P<0.05) in plasmatic lactate concentration in zero, 20 and 28 hours after overload, with values of 0.7, 1.04 and 1.22 mmol/L, respectively. Plasma endotoxin concentration didn't cross 0.1000 EU/mL and may be not present. There were any difference (P>0.05) in fecal and digesta water content, however, fecal pH reduced along 36 hours post-overload (P <0.01), varying from 6.09 to 4.46. Content of large intestine in horses subjected to overload presented whitish-green color, milk aspect, with gas bubbles and acid odor. There weren t difference (P>0.05) in water content of feces and digesta, however, fecal pH reduced along 36 hours post-overload (P <0.01), varying from 6.09 to 4.46. Buffering capacity of ceco-colon digesta and feces were reduced in horses subjected to overload. Right dorsal colon, transverse colon and descendent colon were segments, except stomach, that presented lower pH values, varying from 4.49 to 4.56. Eosinophils infiltration were presented in mucosa and submucosa of all horses, however, only horses submitted to overload presented neutrophils and eosinophils leucocitoestase with neutrophils predominance restricted to large intestine. Tract gastrointestinal circulatory alterations observed were congestion, edema and lymphatic vessels dilatation, more evident in submucosa, with larger inflammatory cells infiltration in horses subjected to overload. Intestinal mucosa 36 hours after overload presented larger degree of imunorreactivity anti-myeloperoxidase, followed by horses evaluated at 24 hours after overload and control horses, varying from 2.7 to 4.0, 1.0 to 3.7 and 1.0 to 2.5, respectively. Overload carbohydrate in horses promoted intensive fermentation in ceco-colon, predisposing clinical disturbances, digesta alterations and mucosa and submucosa lesions at gastrointestinal tract of horses with light to moderate degree,36 hours after overload. / Este trabalho teve como objetivo avaliar as altera??es sist?micas, da mucosa e conte?do do trato gastrointestinal de eq?inos submetidos ? sobrecarga diet?tica com amido. Internal Hernia Masquerading As Necrotizing Enterocolitis

In extremely preterm infants, acute abdominal emergencies are fortunately less common with improving care. Spontaneous intestinal perforation and necrotizing enterocolitis are conditions where emergency surgery is most often needed. Conservative medical management and placement of temporary drain are often used in the initial management. Internal hernia (IH) is an uncommon cause of bowel obstruction in neonates, is difficult to diagnose and unfortunately are found only at autopsy. The presentation in preterm infants, distinction between these conditions, and the need for early diagnosis of IH are discussed.

Fragestellung: Wie nützlich ist das Mehrschicht-Spiral-CT bei der Diagnostik der Darmischämie? Studiendesign: Retrospektive Studie Patientenkollektiv: 112 Patienten, die in einem Zeitraum von drei Jahren bei Verdacht auf Darmischämie in einem Mehrschicht-Spiral-CT untersucht wurden. Ergebnis: Bei den 112 Verdachtsfällen handelte es sich in 26 Fällen um eine Darmischämie. Das CT erkannte davon 24 (92%), ein Fall wurde falsch negativ (4%) befundet und ein Fall (4%) wurde als unklar eingestuft. Von den 86 Fällen ohne Darmischämie wurden 77 (89%) korrekt erkannt, drei (4%) wurden falsch positiv befundet und 6 (7%) Fälle wurden als unklar eingestuft. Die Ergebnisse der bei manchen Patienten zusätzlich durchgeführten Sonografie – und Röntgenuntersuchungen waren erheblich schlechter (Sono: Sensitivität: , Spezifität: ; Röntgen: Sensitivität: , Spezifität: ). Die erhobenen Laborparameter (Leukozyten, Lactat, D-Dimere) waren zur Beurteilung, ob es sich um eine Darmischämie handelt, nicht spezifisch genug. Schlussfolgerung: Das Mehrschicht-Spiral-CT ist mit einer Sensitivität von 92% und einer Spezifität von 97% gut zur Erkennung einer Darmischämie geeignet. Unklare Fälle sind mit hoher Wahrscheinlichkeit nicht operationswürdig. / Objective: To verify the diagnostic importance of helical CT in the diagnosis of mesenteric ischemia. Design: Retrospective study Subjects: 112 patients, who where examined by helical CT during a 3-year-period, with suspected mesenteric ischemia. Main Outcome: 26 cases proved to be mesenteric ischemia. Helical CT was diagnostic in 24 (92%) cases, wrong in one (4%) case and uncertain in one (4%) case. No ischemia was correctly diagnosed in 77/86 (89%) cases, wrongly in three (4%) and 6 (7%) were uncertain. Sonografie and plain film radiographs were taken on some patients, but the results were unsatisfactory. Sonography: Sensitivity: 0%, specificy: 85,7%. Plain film radiographs sensitivity: 0%, specificy: 100%. Results of laboratory test were of little diagnostic value (white blood cell count, lactate, d-dimere). Conclusions: Helical CT is a valuable tool for detecting mesenteric ischemia with a sensitifity of 92% and a specificy of 97%. Suspected mesenteric ischemia which cannot be proved by helical CT are most probable non surgical cases.
