Pour combiner les bonnes propriétés mécaniques des matériaux métalliques et obtenir des surfaces bioactives, des revêtements de phosphate de calcium (Ca-P) ont été développés. Le but est d’améliorer la biocompatibilité et la bioactivité du système et créer une barrière contre l’éventuelle libération d’ions toxiques du substrat métallique. Notre démarche a été de développer une nouvelle voie de synthèse. Après un polissage mécanique et décapage chimique du substrat de Titane ou Ti–6Al–4V une couche intermédiaire en titanate de sodium a été obtenue par un prétraitement alcalin sur les substrats. Elle a été ensuite soumise à un traitement thermique afin de créer une couche alvéolaire nanométrique. Celle-ci facilite la croissance une couche de phosphate de calcium par voie autocatalytique d'une manière similaire au procédé de formation de l'os naturel. La caractérisation et l’étude des revêtements des Ca-P obtenus par les trois bains (acides, alcalins et oxydant) sur les substrats métalliques ont été réalisés. Les dépôts ont été étudiés d’un point de vue structural et morphologique. La stabilité des couches de Ca-P a été mesurée dans le fluide corporel (SBF) pour des périodes différentes utilisant des analyses biochimiques. En conclusion, cette méthode est une solution peu coûteuse, fiable, et utilisable à l'échelle industrielle. La couche de Ca-P aussi bien que le titanate de sodium obtenus peuvent permettre d’être imprégnés par des agents actifs comme un ou plusieurs agents antibactériens. / To combine the good mechanical properties of metallic substrate with the bioactivity of some ceramics, to obtain bioactive surface, a calcium phosphate (Ca-P) coating has been developed. The goal is to improve the biocompatibility and bioactivity of metallic implant and create a barrier against the possible release of toxic ions from the metallic substrate. Our approach has been to develop a new synthetic route. After mechanical polishing and chemical etching of the titanium or Ti-6Al-4V substrates, an interlayer of sodium titanate was obtained by an alkaline pretreatment of the substrates. Then, it was subjected to a heat treatment to create a nanometeric layer. The latter facilitates the growth of a layer of Ca-P by means of an electroless manner similar to the forming process of the natural bone. Characterization and study of Ca-P coatings obtained by the three baths (acids, alkalis and oxidizing) on metallic substrates have been made. The deposits were studied from a structural and morphological point of view. The stability of Ca-P layers was measured in simulated body fluid (SBF) for different periods using biochemical analyzes. In conclusion, this method is a cheap and reliable solution that can be used on an industrial scale. The Ca-P as well as the sodium titanate layers can afford to be impregnated with active agents such as one or more antibacterial agents.

It was formulated a two-step self-etching adhesive system composed of dentin primer and bond (cover resin). The primer was prepared with fixed amounts of 2-Hidroxiethyl methacrylate (HEMA), phosphoric acid ester monomer and solvent. The cover resin (bond) was prepared with a homogeneous mixture of Biphenyl-A glicidil dimethacrylate (Bis-GMA), Triethyleneglycol dimethacrylate (TEGDMA), HEMA, photoinitiators as camphoroquinone (CQ) and Ethyl 4-dimethyl amine benzoate (EDMAB); and 1% Butia Capitata‟ seeds oil (butiá). Three adhesives have been tested as commercial references: Clearfil Protect Bond (CPB), Clearfil SE Bond (CSEB) and Adper SE Plus (AP), and two experimental adhesives: Control adhesive without oil (CA), and experimental adhesive containing butiá´s oil (EA). The antimicrobial effect of adhesives was evaluated by the methods of biofilm growth inhibition in a Streptococcus mutans monoculture assay and by the microcosm technique. The physical mechanical performance of these adhesives was evaluated by micro tensile bond strength (μTBS), degree of conversion (DC), and water sorption (WS) and solubility (SL). It also was evaluated the biocompatibility by the genotoxicity and cytotoxicity assays. The data were statistically analyzed by analysis of variance and complementary tests. In the assay of anti-S. mutans biofilm , AP showed the highest effect, followed by EA and CPB. In the microcosm assay, high viability was found in AC for total microorganisms, and minor viability was found in C, EA and CPB for total aciduric. While for total Lactobacillus most viability was found in C and AC, and for S. mutans , all groups had similar values. For the GC, WA, SL e μTBS tests EA was similar to CA, but after six months of aging the μTBS values were reduced for EA. Regarding the cytotoxicity the CPB primer and the AP bond were the most toxic, and regarding genotoxicity all groups have had similar results. According to results obtained, we can conclude that EA have similar immediate antimicrobial effect, physical-mechanical properties and toxicity to commercial adhesives, but after six months the microtensile strength was reduced, but similar to commercial references / O objetivo do presente estudo foi avaliar o efeito antimicrobiano e propriedades físico-mecânicas de um adesivo experimental após incorporação de um óleo essencial proveniente da semente de butiá (Butia capitata). Foi formulado um sistema adesivo autocondicionante experimental de dois passos composto por um primer dentinário e um bond (resina de cobertura). O primer foi preparado a partir de quantidades fixas de 2-Hidroxietil metacrilato (HEMA), monômero ácido fosforado e solvente. A resina de cobertura (bond) foi elaborada a partir da mistura homogênea de Bisfenol-A glicidil dimetacrilato (Bis-GMA), Trietilenoglicol dimetacrilato (TEGDMA), HEMA, iniciadores da polimerização como canforoquinona (CQ) e Etil 4-dimetil amino benzoato (EDMAB); e óleo de Butia capitata ( butiá ) 1%. Foram testados três adesivos como referência comercial: Clearfil Protect Bond (CPB), Clearfil SE Bond (CSEB), Adper SE Plus (AP), e dois adesivos experimentais: Adesivo controle sem óleo (AC) e Adesivo experimental contendo óleo de butia (EA). Foram avaliados o efeito antimicrobiano dos adesivos pelo ensaio de inibição da formação de biofilme em monocultura de S. mutans e pela técnica de microcosmos, assim como o desempenho físico-mecânico destes adesivos através dos ensaios de: resistência de união em dentina (μTBS), grau de conversão (DC) e sorção (WS) e solubilidade (SL). Também foi avaliada a biocompatibilidade por meio de ensaios de citotoxidade e genotoxidade. Os dados foram analisados estatisticamente por análises de variância e testes complementares. Na atividade anti-biofilme com monocultura de S. mutans, AP mostrou maior efeito seguido do EA e do CPB. No modelo de microcosmos, maior viabilidade foi encontrado em AC para microrganismos totais e menor viabilidade foi encontrada no C, EA e CPB para acidúricos totais. Já para lactobacilos totais, maior viabilidade foi encontrada no C e AC, e para S. mutans encontramos valores similares em todos os grupos. Para GC, WA, SL e μTBS, o EA foi similar ao AC, porém após 6 meses de envelhecimento os valores de μTBS diminuíram para o EA. Com relação à citotoxidade, o primer do CPB e o Bond do AP foram o mais tóxicos. Para genotoxidade, todos presentaram resultados similares. De acordo com os resultados obtidos, podemos concluir que: o EA teve efeito antimicrobiano, biocompatibilidade e propriedades físico-mecânicas imediatas similares aos comerciais, mas após 6 meses houve diminuição da resistência de união no entanto foi similar as referencias comerciais.

Acute otitis media in young children:randomized controlled trials of antimicrobial treatment, prevention and quality of life

Abstract The purpose of this study was to evaluate the effect of antibiotic treatment and surgery on acute otitis media (AOM), and to evaluate quality of life (QoL) among children with AOM and their parents. To evaluate the effectiveness of antibiotics, a total of 82 children with AOM were randomized for antibiotic or placebo treatment for 7 days. The duration of middle ear effusion was measured by daily tympanometry screenings at home over 2 weeks. Duration was also measured at clinical visits, including at entry, after 3 days, after 7 days, and then weekly until both ears were healthy according to pneumatic otoscopy or otomicroscopy, or for a maximum of 2 months. Among the group receiving antibiotics, middle ear effusion disappeared 2.0 weeks earlier than among those receiving placebo (P<0.02). On day 14, 69% of children in the antibiotic group and 38% in the placebo group had normal tympanometry findings (P=0.02). On day 60, 5% of children in the antibiotic group and 24% in the placebo group had persistent middle ear effusion (P=0.01). The effect of surgery was assessed by randomly assigning 300 children with recurrent AOM, aged 10 months to 2 years, into 3 groups: 1. to receive ventilation tubes (VTs), 2. to receive VTs and adenoidectomy and 3. non-surgery. Follow-up of children occurred at clinical visits every 4 months for a 1-year period. If children suffered from upper respiratory symptoms or their parents suspected AOM during this period they were encouraged to receive additional follow-up care. Intervention was considered unsuccessful if a child had 2 AOM episodes in 2 months, 3 episodes in 6 months or persistent effusion lasting for 2 months. Intervention failed in 34% of children in the non-surgery group, 21% in the VT group (P=0.04 compared to non-surgery) and 16% in the group with VT and adenoidectomy (P=0.004 compared to non-surgery). QoL was assessed among 159 children participating in the study on the effect of surgery in children with recurrent AOM. We used disease-specific (Otitis Media-6) and generic instruments (Child Health Questionnaire-50) to measure QoL among children with AOM and their parents, and the effect of surgery on QoL. Children with AOM and their parents had a significantly poorer QoL than healthy children. QoL improved significantly at 1-year follow-up, but it did not reach the level observed in healthy children. Surgery did not have any additional impact on QoL. / Tiivistelmä Työn tavoitteena oli tutkia antibiootin ja kirurgian vaikutusta äkilliseen välikorvatulehdukseen sekä tutkia välikorvatulehduksia sairastavien lasten ja heidän vanhempiensa elämänlaatua. 82 äkillistä välikorvatulehdusta sairastavaa lasta satunnaistettiin saamaan joko antibiootti- tai lumelääkettä. Välikorvaeritteen poistumista seurattiin kotona päivittäisillä tympanometriamittauksilla kahden viikon ajan. Seurantakäynnit olivat yhden, kolmen ja seitsemän päivän kuluttua sekä viikoittain, kunnes korvat oli todettu terveiksi pneumaattisella otoskoopilla tai korvamikroskoopilla tai kahden kuukauden seuranta-aika päättyi. Välikorvaerite poistui kaksi viikkoa aikaisemmin antibiootti- kuin lumelääkkeellä (P<0.02). Tympanometria normalisoitui kahden viikon kuluttua 69 %:lla antibioottiryhmästä ja 38 %:lla lumelääkeryhmästä (P=0.02). 60 päivän kuluttua välikorvaeritettä oli 5 %:lla antibioottiryhmästä ja 24 %:lla lumelääkeryhmästä (P=0.02). Kirurgian vaikuttavuutta toistuviin äkillisiin välikorvatulehduksiin tutkittiin satunnaistamalla 300 10–24 kk:n ikäistä lasta saamaan ilmastointiputket tai sekä ilmastointiputket että kitarisanpoisto tai ei kumpaakaan. Seurantakäynnit olivat neljän kuukauden välein vuoden ajan tai aina kun lapset sairastuivat ylähengitystietulehdukseen tai vanhemmat epäilivät välikorvatulehdusta. Interventio katsottiin epäonnistuneeksi (äkillisiä välikorvatulehduksia 2 / 2 kk, 3 / 6 kk tai jatkuva erite 2 kk) 34 %:lla ilman kirurgiaa hoidetuista lapsista, 21 %:lla ilmastointiputkiryhmän lapsista (P=0.04 verrattuna ilman kirurgiaa hoidettuihin) ja 16 %:lla lapsista, joille tehtiin sekä kitarisan poisto että asetettiin ilmastointiputket (P=0.004 verrattuna ilman kirurgiaa hoidettuihin). Elämänlaadun, äkillisen välikorvatulehduksen sekä siihen liittyvän kirurgian välistä yhteyttä selvitettiin 159 lapsella, jotka osallistuivat kirurgian vaikuttavuutta selvittävään tutkimukseen. Elämänlaatua mitattiin sekä tautikohtaisilla (Otitis Media-6) että yleistä elämänlaatua (Child Health Questionnaire-50) mittaavilla kyselylomakkeilla. Äkillistä välikorvatulehdusta sairastavilla lapsilla ja heidän vanhemmillaan oli merkittävästi huonompi elämänlaatu kuin terveillä. Elämänlaatu parani merkittävästi vuoden seuranta-aikana, mutta ei saavuttanut terveiden tasoa. Kirurgia ei tuonut mitään lisähyötyä elämänlaatuun.

Síntese, caracterização e ação antimicrobiana de óxidos bimetálicos e de revestimentos depositados à plasma sobre resina acrílica /

Orientador: Ana Lucia Machado / Resumo: Devido ao aumento da tolerância dos microrganismos aos antimicrobianos atualmente disponíveis, existe a necessidade de desenvolvimento de métodos alternativos para o controle das infecções. Na primeira etapa dessa pesquisa, óxidos bimetálicos foram sintetizados por meio dos métodos hidrotermal assistido por micro-ondas e co-precipitação, variando-se as condições de síntese, como temperatura, pH, utilização ou não de surfactantes e tipos de solventes. Os diferentes microcristais de tungstato de prata (α-Ag2WO4) e molibdato de prata (β-Ag2MoO4) obtidos foram caracterizados por difração de Raios X (DRX), microscopia eletrônica de varredura por emissão de efeito de campo (MEV-EC), microscopia eletrônica de transmissão (MET), espectroscopia de energia dispersiva de raios-X (EDX), espectroscopia Raman e mensurações de fotoluminescência (PL) e UV-Vis. A atividade antimicrobiana desses compostos contra células planctônicas de Staphylococcus aureus resistente à meticilina - SARM, Escherichia coli (E. coli) e Candida albicans (C. albicans) foi avaliada por meio da determinação das concentrações inibitórias (CIM) e bactericida/fungicida mínimas (CBM/CFM). In the first part of this study, bimetallic oxides were synthesized by hydrothermal microwave-assisted and coprecipitation methods, varying synthesis conditions, as presence or absence of surfactants and types of solvents used. The different microcrystals of α-Ag2WO4 and β-Ag2MoO4 obtained were characterized by X-ray diffraction (XRD), field-emission scanning electron microscopy (FE-SEM), transmission electron microscopy (TEM), energy diffraction spectroscopy (EDS), Raman spectroscopy and photoluminescence measurements (PL) and UV-Vis. Their antimicrobial activity was evaluated against planktonic cells of methicillin - resistant Staphylococcus aureus - MRSA, Escherichia coli (E. coli) and Candida albicans (C. albicans) by the determination of inhibitory concentrations (MIC) and bactericidal/fungicidal concentrations (MBC/MFC). Efficacy of propolis against fusobacterium nucleatum biofilm

Indiana University-Purdue University Indianapolis (IUPUI) / The primary goal of root canal treatment is to eliminate microbes from the root canal system, which is the cause of pulpal and periapical infections. Research shows that after a single visit of chemomechanical debridement microbes continue to remain within the canal system. An interappointment medication step has been advocated to maximize potential elimination of microbes within the root canal system. Previous studies have shown propolis to be antibacterial against common endodontic microbes. Studies have shown trends in different microbes being present in primary verus secondary endodontic infections. The majority of literature has focused on the efficacy of propolis against Enterococcus faecalis, a microbe commonly implicated in secondary endodontic 95 infections. The aim of this study was to demonstrate the efficacy of propolis against Fusobacterium nucleatum, a microbe commonly found in primary endodontic infections. This study aims to demonstrate the efficacy of propolis against a bacterium of primary endodontic infections (F. nucleatum) as well as against microbial biofilm to further support its potential use as a novel intracanal medicament. Dilutions of propolis were added to cultures of F. nucleatum in microtiter plates in a range from 390 μg/ml to 50,000 μg/ml. The minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), and the minimum biofilm inhibitory concentration (MBIC) were determined. The MIC was determined of the total solution (biofilm+planktonic), planktonic, and biofilm (MBIC) after a 48-hour incubation period. The MBIC was determined by fixing biofilm to the wells and using crystal violet staining with spectrophotometry. The MBC was examined by plating solution from each concentration test well and reading the plates after 48 hours of incubation. The results show that the MIC of the total (biofilm+planktonic) appears to occur at a concentration of 6250 μg/ml. The MBIC appears to occur at the concentration of 1562.5 μg/ml. The planktonic results exhibit no significant difference in test and control wells. There was no MBC at any of the test concentrations. The propolis appears to inhibit bacterial growth and biofilm formation but does not appear to be bactericidal at any of the tested concentrations. The results of this study indicate that propolis has an MIC and MBIC when tested in vitro against F. nucleatum, although it does not show an MBC. There appears to be potentially significant interaction of propolis with biofilm as displayed by the lower concentration needed to exibit inhibitory effects on biofilm formation. This information 96 may contribute to the ability to develop a proper concentration of propolis to use in vivo when treating endodontic infections.

Effect of low concentrations of antibiotic intracanal medicaments on crown discoloration and push-out bond strength

Indiana University-Purdue University Indianapolis (IUPUI) / Introduction: Some intracanal medicaments used in regenerative endodontics may compromise the bond strength of root cements and lead to tooth discoloration. Objectives: To evaluate the effects of 1) low concentrations of TAP and DAP (1 mg/mL) on push-out bond strength of various root cements, and 2) low concentrations of TAP and DAP (1 mg/mL and 10 mg/mL) on crown discoloration. Materials and Methods: Single rooted human teeth (n = 144) were horizontally decoronated and instrumented according to standardized protocol. The samples were randomized into six experimental groups (Ca(OH)2, 1000 mg/mL TAP and DAP, 1 mg/mL TAP and DAP, and no medicament control group. After four weeks, the medicaments were removed and each group was divided into three subgroups to receive MTA cement, Biodentine cement, or Endosequence Bioceramic putty cement for two weeks. Then, two root cylinders were obtained from each root and push-out bond strength testing was performed. For the crown discoloration experiment, 160 crowns were obtained from intact human molars and randomized into experimental groups as described earlier with the addition of two groups (10 mg/mL TAP and DAP). The pulp chambers in half of the samples from each group were coated with an adhesive bonding agent before receiving the assigned intracanal medicament. Color changes (ΔE) were detected by spectrophotometer at 1 day, 1 week, and 4 weeks after application, as well as after thermocycling. Results: In the push-out bond strength experiment, 1 mg/mL DAP generally demonstrated significantly higher bond strength of root cements compared with the other treatment groups. For the crown discoloration experiment, when an adhesive bonding agent was used prior to (10 mg/mL or 1000 mg/mL) TAP, the crowns had significantly less discoloration than those without adhesive. DAP 10 mg/mL had the least significant color change at all time points regardless of whether adhesive was used. Conclusion: 1) 1 mg/mL DAP and Ca(OH)2 did not have significant negative effect on the bond strength of calcium-silicate-based cement to radicular dentin. 2) 1 mg/mL and 10 mg/mL of DAP and Ca(OH)2 had significantly less effect on the color change of the human tooth crown than all intracanal medicaments used in this study. / 2020-08-13

The Effects of Nano-Hydroxyapatite in a Double Antibiotic Paste-Loaded Methycellulose Carrier on Dental Pulp Stem Cells

Indiana University-Purdue University Indianapolis (IUPUI) / The effects of hydroxyapatite in a DAP-loaded MC carrier on dental pulp stem cells Introduction: Regenerative endodontic procedures (REP) require disinfection techniques to eliminate bacteria from the infected immature root canal system and promote new growth of the pulp-dentin complex. Double antibiotic paste (DAP), a mixture of ciprofloxacin and metronidazole, has shown efficacy in doing so while minimizing cytotoxicity on dental pulp stem cells (DPSC). Stem cells, scaffolding, and growth factors are necessary in the maturation, proliferation, and differentiation of mesenchymal stem cells into the root canal system. Nano-hydroxyapatite (n-HA) has a history of biocompatibility and, in addition, has shown promising effects as a tissue bioengineering material. Objective: The aim of this in vitro study was to investigate the proliferation and mineralization of DPSC in the presence of 1% DAP and methylcellulose (MC) with varying concentrations of nano-hydroxyapatite. Materials and Methods: DPSC were plated in 24-well plates containing culture media. The next day, semi-permeable 0.1 mm Transwell chambers were inserted into the wells to separate the reservoirs for medicaments. Treatment paste composed of methylcellulose containing 1% DAP with either 0.25%, 0.50%, or 1.0% nano-hydroxyapatite was added along with culture media. Methylcellulose alone and calcium hydroxide (Ultracal) were used as control groups. After 3 days, cells were evaluated for cytotoxic effects using an MTS proliferation assay (n = 10, in triplicate). DPSCs were also cultured with these medicaments for 7 days in osteogenic media and evaluated for alkaline phosphatase (ALP) activity and mineralization activity (n = 13, in triplicate). Comparisons between groups for differences in mineralization, BSA, and ALP activity were performed using analysis of variance (ANOVA), with different variances allowed for each group and a random effect included in the model to account for correlation within each of the three trials. A simulation-based model was used to adjust for multiple comparisons. Results: Addition of n-HA treatment groups increased mineralization significantly greater than calcium hydroxide, with MC alone and MC+DAP+0.5% HA providing the greatest effect. Regarding ALP, all HA concentrations performed significantly greater than MC and DAP concentrations. Proliferation demonstrated similar metabolic activity in all experimental groups with few comparisons significant. Conclusion: The challenge in REPs is to maintain survival, and preferably promote the proliferation and development of DPSCs into the pulp-dentin complex with a consistent treatment outcome. The combination of DAP with hydroxyapatite may allow for both disinfection and improved mineralization and cellular differentiation. This contribution has shown significant ability to increase stem cell differentiation into an osteogenic lineage as well as calcium deposition, indicating end goal results of regenerative procedures.

The impact of hydroxyapatite on alkaline phosphatase activity and mineral deposition of dental pulp stem cells using a double antibiotic paste loaded methylcellulose carrier

Indiana University-Purdue University Indianapolis (IUPUI) / Introduction: Regenerative endodontic procedures (REPs) are a type of endodontic treatment aimed at replacing damaged tooth structures, including dentin and root structures, as well as cells of the pulp-dentin complex. Double antibiotic paste (DAP) has been shown to be efficacious in achieving disinfection of the root canal system while minimizing cytotoxicity to dental pulp stem cells (DPSCs). Hydroxyapatite (HA) is an extracellular, mineralized component of bone that has shown much promise as a scaffold in the field of regenerative medicine. Objective: The objective of this study was to evaluate the effects of HA in a DAP loaded methylcellulose (MC) carrier on the differentiation and mineral deposition of DPSC over time. Materials and Methods: DPSCs were plated in 24-well plates with culture media. The following day, semi-permeable 0.1 m chambers were inserted into the wells to separate the reservoirs and permit delivery of medicaments. 100 L treatment paste composed of MC with 1% DAP and either 0.5% or 1.0% nano-HA was added, followed by additional culture media. After 3 days of treatment, medicaments were removed and DPSCs were cultured for an additional 9 days with replacement of media every 3-4 days. At Day 12, DPSCs were evaluated for alkaline phosphatase (ALP) activity using a biochemical assay and mineral deposition using an Alizarin Red S Ca2+ staining assay (4 wells/group). Comparisons between groups were performed using one-way analysis of variance (ANOVA) with a 5% significance level used for all tests. Results: A trend towards increased ALP and mineral deposition activity was noted among the groups with HA added to DAP with MC. Although these trends were not statistically significant, a trend towards increased ALP and mineral deposition was observed after 3-day medicament exposure. The results were similar to previous findings using 7-day medicament treatments. Conclusion: The addition of HA showed a trend towards improved differentiation and mineral deposition of DPSCs compared to DAP with MC. Although additional studies are required, these results showed suggest that even with a shortened treatment time, increased differentiation and mineral deposition of DPSCs may be possible. This study provides additional support that low concentration DAP in a MC carrier has potential application in regenerative endodontic procedures. The novel addition of HA may provide additional osteogenic potential.
