Omvårdnad vid central venkateter - Sjuksköterskans ansvarsområde - En systematisk litteraturstudieErlandsson, Linda, Nordqvist Nilsson, Cecilia January 2007 (has links)
Syftet med denna litteraturstudie är att sammanställa litteratur gällande olika typer av förband samt omläggningsfrekvens vid omvårdnad av central venkateter. Författarna har gjort en systematisk litteraturstudie och följt Goodmans sju steg i forskningsprocessen. Totalt inkluderades 15 vetenskapliga artiklar efter litteratursökning i databaserna PubMed, CINAHL, Cochrane Library samt via kompletteringar i ELIN. Artiklarna granskades av två oberoende bedömare utefter modifierade granskningsprotokoll och kvalitetsbedömdes därefter. Resultaten av denna sammanställning visar en tendens åt att sterila kompresser än så länge har ett litet försprång gentemot transparenta förband, i synnerhet när insticksstället inte läkt. En omläggningsfrekvens mellan fem och sju dagar verkar ge bäst resultat vid användning av transparenta förband, och varannan till var sjunde vid användning av kompressförband. Fler studier behövs som redovisar signifikanta resultat för att evidensgraden ska kunna höjas. / The aim of this study is to compile literature concerning which dressing type and redressing frequency is the most efficient handling for a central venous catheter (CVC). This study is a systematic literature review according to Goodman. Searches were made in the databases PubMed, CINAHL, Cochrane Library and complements through ELIN. A total of 15 scientific articles of quantitative design and literature reviews were included. The articles were then scrutinized by two independent readers and were then judged by quality. The result shows a tendency that sterile gauze has an advantage in comparison to transparent dressings so far, especially when the exit site has not fully healed. A redressing frequency between five to seven days seems to be the best alternative when using transparent dressings. When using sterile gauze the results have shown more variation; a redressing frequency every other day to every seventh day. More studies are necessary for reaching statisticly significant results.

IMPACT OF HOMOGENIZATION AND UHT PROCESSING ON THE EMULSIFICATION AND PHYSICAL PROPERTIES OF PEA PROTEIN BEVERAGESXiang Cheng (17583861) 10 December 2023 (has links)
<p dir="ltr">Pea protein is one of the most used plant proteins in food products, acting as an alternative to conventional animal protein sources due to its abundant, nutritious, and ease in supply chain characteristics. The objective of this study was to investigate the impact of homogenization and UHT processing parameters on the properties of protein emulsion. Protein emulsions (8% w/w pea protein isolate and 1% w/w sunflower oil) were freshly prepared prior to processing, and the untreated sample was considered as the control (NT). The pilot-scale aseptic processing system (APS) used in this study consisted of two coil-in-shell heaters and two coolers. Samples flowed through each section of the APS system following this order: balance tank, pre-heater, final heater, hold tube, pre-cooler, and final cooler. The homogenizer was located either after the pre-cooler (AC) or the pre-heater (AH) with a controlled temperature of 165F. A third setup was utilized by bypassing the homogenizer in the UHT system. An additional 8-hour continuous run was conducted to mimic a commercial manufacturing operation by recirculating the protein emulsion in the UHT system, and fouling detections were made using a non-intrusive sensor (NICS). 5% w/w soy protein, 1% w/w sunflower oil oil-in-water emulsion was also used for fouling tests. Protein concentration, pH and zeta potential, Cryo-SEM microscopic image, particle size distribution, flocculation index (FI), coalescence index (CI), viscosity and color data were collected and analyzed. The protein concentration had a 23.20 ± 4.00 %, 28.35 ± 5.02 %, 27.98 ± 5.05% and 21.38 ± 5.75% reduction for AC, AH, UHT and NT samples, respectively, when compared with the initial concentration in the formula. AC, AH, UHT and NT samples had pH values of 7.24 ± 0.01, 7.27 ± 0.01, 7.28 ± 0.02, 7.41 ± 0.01, and zeta potential values of -42.91 ± 0.89, -47.30 ± 0.91, -46.91 ± 1.40 and -50.11 ± 1.47 mV. AC sample had a smaller and NT sample had a bigger, respectively, mean weighted size D 4,3 value than AH and UHT samples, which could also be seen in Cryo-SEM images where only AC images contained more visually observable smaller particles. FI and CI for AC, AH and UHT indicated the formation of flocs but no irreversible aggregations were found. Shear-thinning AC, AH, UHT and NT samples had viscosity decreases from 4.00 to 3.56, 3.88 to 3.75, 4.02 to 3.79 and 10.42 to 9.56 mPa*s in 1 1/s to 100 1/s shear rate range. NT sample had a very noticeable color difference from the other three treated samples. Overall, AC samples had similar or better emulsion stability in all aspects than AH and UHT samples, suggesting that AC processing could potentially be used in the protein beverage industry for manufacturing products with improved shelf stability. Severe foulants buildups were neither observed nor detected by a non-intrusive continuous sensor (NICS) in the UHT system within 8 hours of process for both pea protein and soy protein emulsion, indicating that this UHT-homogenization processing can potentially be adapted to current industrial practices for higher-quality protein beverages.</p>

Inledning: God vårdhygien är ett centralt begrepp inom all hälso- och sjukvård för att förhindra spridning av sjukdomsframkallande mikroorganismer, vilka kan ge upphov till vårdrelaterade infektioner. Tandvården ska därför följa Hälso- och sjukvårdslagen (1982:763) och Socialstyrelsens föreskrifter om basala hygienrutiner. I samband med Covid-19 pandemin aktualiserades vikten av vårdhygien ytterligare för att förhindra smittspridning av Covid-19 inom tandvården. Munhålan utgör en stor potentiell risk för överföring av SARS-CoV-2, men även andra mikroorganismer, särskilt vid aerosolgenererande procedurer. Ett tidigare mastersarbete vid Odontologiska fakulteten, Malmö Universitet, påvisade brister i följsamheten med basala hygienrutiner bland tandläkarstudenter. En uppföljning har inte genomförts, vilket uppmärksammades i samband med pandemin och frågeställningen om Covid-19 syn på betydelsen av aseptik och vårdhygien väcktes. Syfte: Undersöka tandläkarstudenters syn på vårdhygien och självbedömd följsamhet med hygienriktlinjer under Covid-19. Material och metod: En anonym webbenkät skickades till samtliga tandläkarstudenter på kurs 4–10 vid Odontologiska fakulteten, Malmö Universitet. Resultat: Den totala svarsfrekvensen var 54%. Resultat visade att samtliga studenter ansåg sig ha tillräckliga kunskaper om hygienrutinerna, men trots detta bröt 20% medvetet mot hygienrutinerna och 4% uppgav att de aldrig följde hygienrutinerna. Slutsats: Den självbedömda kunskapen om hygienriktlinjer är hög, trots det är inte följsamheten med hygienriktlinjerna optimal, vilket innebär att det finns risk för spridning av vårdrelaterade infektioner. / Introduction: Good hygiene is imperative to prevent the spread of health care associated infections. Dental care must therefore comply with the Health and Medical Services Act (1982: 763) and the National Board of Health and Welfare's regulations on basic hygiene guidelines. In conjunction with the Covid-19 pandemic, the importance of hygiene measures was further emphasized to prevent the spread of Covid-19 in dentistry. The oral cavity poses a high potential risk for transmission of SARS-CoV-2, in addition to other microorganisms, especially in aerosol-generating procedures. A previous master's thesis at the Faculty of Odontology, Malmö University, showed a deficiency in compliance with hygiene guidelines among dental students. The question if Covid-19 view of the importance of aseptic techniques and hygiene measures in dentistry was therefore raised. Aim: Investigate dental students' views of healthcare hygiene and self-assessed aseptic measures during Covid-19. Materials and method: An anonymous web survey was sent to all dental students in courses 4–10 at the Faculty of Odontology, Malmö University. Results: The overall response rate was 54%. Results showed that all students considered themselves to have sufficient knowledge of the hygiene routines, but despite this, 20% of students deliberately broke the hygiene routines and 4% stated that they never followed the hygiene routines. Conclusion: The self-assessed awareness of hygiene guidelines and aseptic techniques is high among the students, still the compliance with hygiene protocols is not optimal, meaning there is a risk of spread of healthcare associated infections.

Bakgrund: Postoperativa sårinfektioner orsakar lidande för den drabbade patienten och kostnader för patienten, vården och samhället. Intraoperativ kontamination av operationsinstrumenten är en av källorna till dessa infektioner. Operationssjuksköterskan är ansvarig för aseptiken och operationsinstrumenten. Syfte: Syftet med denna pilotstudie var att mäta graden av bakteriell kontamination över tid på operationsinstrument som används vid öppen neurokirurgi samt observera utvalda källor till kontamination under samma tid. Metod: Denna studie designades som en kvantitativ icke-experimentell observationsstudie. Datainsamlingen skedde under hösten 2015 och bestod av omhändertagandet av specifika instrument som använts av operatören under operationen efter förutbestämd åtgången knivtid. Bakterieodlingar på dessa instrument skedde sedan på laboratorium enligt en förutbestämd metod och graden av kontamination mättes genom observation av mängden colony forming units (CFU). Resultaten presenteras i form av förändringar av mängden CFU. Resultat: Fem operationer observerades och tio odlingar utfördes på tio instrument. Totalt tio CFU registrerades under samtliga observationer. Ingen signifikant ökning av antal CFU kunde observeras från en till två timmars knivtid (P=0,156). Antal dörröppningar ökade från 3,4 till 9,0 och antal personer på operationssalen ökade från 5,4 till 5,8. Slutsats: Trots det ringa antal observationer som ingick i denna studie tyder resultaten på god aseptik och minimal bakteriell kontamination på operationsinstrumenten under operationens två första timmar. Ytterligare forskning med längre observationer, fler observationer och observationer kring annan typ av kirurgi behövs. / Background: Post-operative surgical site infections (SSI) cause suffering for the afflicted patient and is a great cost for the patient, the health care system and society. Intra-operative contamination of surgical instruments is one of the sources of these infections. The operating room nurse is responsible for preventing infections. Purpose: The purpose of this pilot-study was to measure the degree of bacterial contamination over time on surgical instruments used in open neurosurgery as well as to observe specific sources of contamination during this time. Method: The design was a quantitative nonexperimental observational study. The data collection took place during autumn 2015 and consisted of the sampling of specific instruments used by the surgeon during surgery after preset elapsed operating times. Bacterial culturing on these instruments was then carried out at a laboratory according to a preset routine and the degree of contamination was measured by observing the amount of colony forming units (CFU). The results are presented as changes in CFU. Results: Five operations were observed and ten bacterial culturings were conducted on ten instruments. A total of ten CFU was registered during all observations. No significant increase in the number of CFU could be observed from one to two hours of elapsed surgery (P=0,156). The amount of door openings increased from 3,4 to 9,0 and the number of personnel in the operating room increased from 5,4 to 5,8. Conclusion: Despite the small number of observations included in this study the results indicate good aseptics and minimal bacterial contamination on the surgical instruments during the first two hours of surgery. Further research with longer observations, more observations and observations on other forms of surgery is needed.

Today is the implantation of joint replacements, although a radical, but very functional restoration frequent way ill joint. The implants are placed high demands on their biocompatible and long-term function, which is directly related to wear of joint replacements in the human body. Therefore, analysis of the complex behavior of the implant in the human body is a fundamental problem of biomechanics of implants. This issue is dedicated, this dissertation thesis. Dissertation thesis deals with the solution design and development of experimental device for testing the TEP acetabula and wear identification using optical method. The proposed device allows the joint burden hole TEP physiological manner, but by working with the acetabulum burdened by Power resultant joint axis passes through the hole. The physiological loading of articular wells and evaluating the size of the loss of polyethylene is currently intensively addressed. The results of this study may in the near future, serving as information for assessing the conduct of wear prosthetic hip joint in vivo. The work is devoted to solving the problem of the size required to detect the loss of polyethylene for joint replacement arthroplasty: Design and development of experimental equipment for testing the acetabulum of the hip joint arthroplasty. Identify wear using optical methods. Design and development of experimental equipment was made in the system Inventor Professional 2010 at the level of 3D modeling. Great attention was paid to the design fit into the acetabulum TEP testing machines and gripping the acetabulum into an optical device. Management options were tested with combinations of experimental equipment engines. Suggestions were made and then tests for fixing the acetabulum into an optical device. Has the possibility of depth analysis and identification of wear joined with innovative solutions to evaluate the loss of polyethylene acetabulum arthroplasty. Identification of abrasion (loss of polyethylene) joint arthroplasty holes were carried out experimental measurements using an optical holographic interferometry. The comprehensive analysis of the results of experimental measurement of the loss of polyethylene shows that this method can identify both the size but also an area where there is even to small changes in the surface, which depend on the abrasion. One can also say that using this method we are able to affect the will or displacements that affect the size of the loss of polyethylene in joint arthroplasty hole. Despite the number of known and lessons learned arising from the results of solving the problem of design and development of experimental equipment for testing the acetabulum and hip arthroplasty wear identification using optical methods, this work is that the initial study to identify problematics wear of joint replacements. Complete resolution of this issue is beyond the scope of one doctoral thesis.
