The impact of visually demanding near work on neck/shoulder discomfort and trapezius muscle activity : Laboratory studies

Introduction: Musculoskeletal discomfort in the neck and shoulders is common among workers performing visually demanding near work, e.g., on a computer screen, and sustained low-level muscle activity during such work can lead to work-related pain. The relationships between visual demands and muscle activity and discomfort in the neck/shoulder region are at present unclear. Aim: The aims of this thesis were to determine whether neck/shoulder discomfort and trapezius muscle activity increases during visually demanding experimental near work, and to investigate whether eye-lens accommodation is a mediating mechanism behind increased trapezius muscle activity. Methods: The four papers included are based on two experiments with different visually demanding near work tasks (duration 5 and 7 min). Trial lenses of different diopters were used to manipulate the visual demands (i.e., induce more or less accommodation) and thereby create different viewing conditions. Monocular viewing, which does not require active convergence, was used to examine the isolated effect of accommodation. Eye-lens accommodation and trapezius muscle activity were measured continuously during the visual tasks, and in one experiment the participants rated their eye and neck/shoulder discomfort at baseline and after each visual task. Results: Neck/shoulder discomfort and trapezius muscle activity increased during the visually demanding near work and participants experiencing a greater increase in eye discomfort (compared with baseline) also developed more neck/shoulder discomfort with time. There were no significant differences in muscle activity among the viewing conditions, and no effect of isolated accommodation response within the monocular viewing conditions. Conclusion: These findings indicate that accommodation per se is unlikely to mediate trapezius muscle activity. Instead, the increase in trapezius muscle activity observed here may be due to a combination of high visual attention and enhanced requirement for eye-neck (head) stabilisation. Since these results suggest that neck/shoulder discomfort may aggravate with time when the visual demands are high, it is important to provide good visual conditions in connection with visually demanding occupations. / Introduktion: Muskelrelaterade besvär i nack- och skulderområdet är vanligt förekommande, framförallt hos individer som utför synkrävande datorarbete. En orsak till sådana besvär anses vara långvarig lågintensiv aktivitet i dessa muskler. Det är i dagsläget oklart om de belastningar som synsystemet utsätts för vid synkrävande arbete bidrar till ökad muskelaktivitet i nack/skulderregionen. Syfte: Syftet var att undersöka om experimentellt synkrävande närarbete påverkar muskelaktivitet och besvär i nack/skulderregionen, och att undersöka om aktivitet i trapezius muskeln (kappmuskeln) påverkas av ögats ackommodation, d.v.s. när linsens brytkraft förändras för att se skarpt på nära håll. Metod: De fyra delstudierna baseras på två laborativa experiment där försökspersoner (66 respektive 26 stycken) genomförde olika synkrävande uppgifter (fem och sju minuter långa). För att göra synuppgifterna mer eller mindre ansträngande för synsystemet användes linser med olika grad av brytkraft (dioptrier). Dessutom, eftersom monokulärt seende inte kräver aktiv konvergens, genomfördes tre av de fyra synuppgifterna i ena experimentet med ett öga för att undersöka den enskilda effekten av ackommodation. En autorefraktor mätte ögats ackommodation under synuppgifterna och aktiviteten i trapeziusmuskeln registreades kontinuerligt med elektromyografi (EMG). I samband med det ena experimentet skattade deltagarna sina upplevda ögon- och nack/skulderbesvär, både före experimentet (baslinje) och efter varje synuppgift. Resultat: Det synkrävande arbetet ökade både de självskattade nack/skulderbesvären och muskelaktiviteten i trapezius. De personer som upplevde en högre ökning av ögonbesvär (i förhållande till baslinjen), rapporterade också mer nack/skulderbesvär över tid. Det var varken någon signifikant skillnad i grad av muskelaktivitet mellan synuppgifterna, eller något signifikant samband mellan monokulär ackommodation och muskelaktivitet. Slutsats: Resultaten indikerar att ögats ackommodation, i sig, inte påverkar muskelaktiviteten i trapezius. Ökad muskelaktivitet i nack/skulderregionen i anslutning till synkrävande arbete kan istället bero på en kombination av höga krav på visuell uppmärksamhet och ett ökat behov av att stabilisera ögonen (huvudet) i förhållande till objektet i fokus (t.ex. texten på en bildskärm). Eftersom resultaten tyder på att synkrävande närarbete leder till ökade besvär i nack/skuldraregionen över tid, är det viktigt att utforma arbetsplatser och synkrävande arbetsuppgifter (t.ex. vid datorn) på ett sätt som främjar visuell hälsa.

Ao mesmo tempo também trazem riscos à saúde de seus usuários, sendo descritos problemas psicológicos, problemas musculoesqueléticos e problemas visuais; estes últimos apresentam uma alta prevalência (50%-70%) e os sintomas visuais e oculares, que trazem incômodos para realização de atividades com o computador, reduzem a produtividade dos trabalhadores e diminuem a qualidade de vida dos usuários de computadores. Estes efeitos à saúde visual têm sido pouco estudados pela comunidade científica. Objetivos: Levantar a frequência de casos da Síndrome da Visão do Computador em professores universitários e identificar os fatores de risco associados à presença da CVS. Metodologia: Trata-se de estudo de tipo transversal observacional e participam professores de uma universidade pública do Estado de São Paulo; foram aplicados dois questionários, um versando sobre aspectos do trabalho e do uso de computadores e outro, sobre sintomas oculares e visuais. Além disso, foram realizados exames visuais. A coleta de dados foi feita entre os meses de Fevereiro a Dezembro de 2013 no campus da universidade. Critérios de Inclusão: Professores da universidade. Critérios de Exclusão: Professores que não usem o computador. Resultados: Em uma amostra de 53 professores a Síndrome da Visão do Computador foi encontrada em 30 professores, 19 deles classificados com a forma leve da Síndrome, 10 com a moderada e 1 com a severa; os sintomas que mais incomodaram os professores foram fadiga (30 participantes), ressecamento ocular (20 participantes), irritação ocular (17 participantes). Foram encontrados problemas binoculares em 27 dos 53 professores e diminuição na produção lacrimal em 12/53. Conclusões: Os professores universitários são uma população com uma alta prevalência da Síndrome da Visão do computador, na amostra apresentou-se a síndrome em mais da metade dos participantes 30/23(56%), concordando assim com as prevalências da Síndrome encontradas nos estudos internacionais. Três variáveis foram associadas significativamente ao aparecimento da CVS nos professores universitários, a idade que minimiza as chances de ter CVS (OR: 0,91 IC: 0,83/0,97 P-value: 0,015), as horas de uso diárias no computador que aumentam as chances de apresentar CVS (OR: 1,26 IC: 1,05/1,57 P-value: 0,021), e os problemas vergenciais como insuficiência ou excesso de convergência ou divergência, os quais estão associados positivamente com o aparecimento da síndrome (OR: 3,24 IC: 1,06/10,49 P-value: 0,042). Computers also brought risks to the health of its users, psychological problems, musculoskeletal problems and visual problems, although the latter with a high prevalence (50% -70%) and with visual and ocular symptoms that diminish the quality of life of users, bother working with the computer and reduce worker productivity, has been little studied by the scientific community. Objectives: The study aims to estimate the cases frequency of the Computer Vision Syndrome in teachers and to identify risk factors associated with the presence of CVS. Methodology: This is a cross-sectional-observational study with teachers of a public university which two questionnaires and a visual test is performed to find and evaluate the possible symptoms of Computer Vision Syndrome of as well as their associated factors. The survey is conducted between the months of February to December 2013 at the University campus. Inclusion Criteria: Teachers and researchers at university. Exclusion Criteria: Teachers and researches that do not use the computer. Results: In a sample of 53 teachers of Computer Vision Syndrome was found in 30 teachers, 19 were classified as mild form of the syndrome, 10 with moderate and 1 with severe, the symptoms that bother the teachers were fatigue (30 participants), ocular dryness (20 participants), eye irritation (17 participants). Binocular problems were found in 27 of the 53 teachers and decrease in tear production in 12/53. Conclusions: The teachers are a population with a high prevalence of the Computer Vision Syndrome, the sample presented the syndrome in more than half of the participants 30/23 (56%), thus agreeing with the prevalence of the syndrome found in international studies. Three variables were significantly associated with 5 % with the onset of CVS on university teachers , the age that minimizes the chances of having CVS (OR: 0.91 IC: 0.83/0.97 P-value: 0.015), hours daily use of the computer that increase the chances of presenting CVS (OR: 1.26 IC: 1.05/1.57 P-value: 0.021), and vergenciais problems like deficiency or excess of convergence or divergence, which are positively associated with the onset of the syndrome (OR: 3.24 IC: 1.06/10.49 P-value: 0.042). Atualmente, os distúrbios oculares relacionados ao uso do computador são aqueles que vêm sendo mais estudados por sua alta prevalência mundial, e por serem cada vez mais frequentes em determinadas profissões. O objetivo desse estudo foi analisar os sintomas da Síndrome da Visão do Computador (CVS) e os fatores associados à função visual entre os trabalhadores, de ambos os sexos, usuários de computador em função administrativa, do prédio da administração de um hospital público universitário terciário de São Paulo (n=303). Realizada pesquisa quantitativa observacional transversal analítica, entre os anos de 2014 e 2015, em que os trabalhadores foram submetidos a exame clínico oftalmológico e responderam um questionário, com dados: sociodemográficos, ocupacionais, condições de trabalho, fatores psicossociais no trabalho, informações sobre saúde e avaliação da função visual. A avaliação estatística foi realizada a partir da análise descritiva das variáveis, quantitativas e qualitativas. Para o Questionário de Função Visual (VFQ-25) foi realizado teste de Kolmogorov-Smirnov, a fim de caracterizar a população do estudo, e os testes de ANOVA, Mann-Whitney e Kruskal-Wallis, para a análise dos fatores associados. Foi criado um modelo de regressão linear múltipla, utilizando stepwise forward, com as variáveis que apresentaram níveis de significância com p<0,20. Permaneceram no modelo final, as variáveis que apresentaram níveis descritivos p<0,05. Os sintomas mais frequentes foram: cansaço nas vistas no trabalho (47,9 por cento ), peso nos olhos no trabalho (38,3 por cento ) e cansaço nas vistas em casa (36,3 por cento ). Os agravos mais prevalentes foram: a presbiopia (66,3 por cento ) e o astigmatismo (47,9 por cento ). Foi encontrada associação entre a idade (OR 0,218; IC95 por cento -0,276-0,161) e o esforço no trabalho (OR 0,656; IC -0,928-0,383) com a função visual. No esforço no trabalho, os itens que apareceram como principais fontes de estresse foram: interrupções no trabalho (3,7 por cento ), trabalho depois da hora (3,6 por cento ) e aumento da exigência (3,6 por cento ). Nesse estudo, conclui-se pela importância da organização do trabalho, dos fatores psicossociais no trabalho associados à presença de sintomas da CVS e, consequentemente, às alterações da função visual nos trabalhadores usuários de computador. Sugerem-se medidas para melhorar as condições do ambiente de trabalho e a realização de exames oftalmológicos periódicos para essa categoria de trabalhadores. / Currently, computer-use related eye disorders have been the most studied for their high prevalence worldwide, and for being increasingly more usual in some professions. The methods included analytical observational cross-sectional quantitative research, between 2014 and 2015, where workers underwent ophthalmologic examination and answered a questionnaire with the following input: sociodemographic, occupational data, work conditions, psychosocial factors at work, health information and assessment of visual function. A statistical evaluation was made based on the descriptive analysis of quantitative and qualitative variables. The Kolmogorov-Smirnov test was performed for the Visual Function Questionnaire (VFQ-25) in order to characterize the study population, and the ANOVA test, Mann-Whitney and Kruskal-Wallis test for the analysis of associated factors. A multiple linear regression model was created using stepwise forward with variables displaying p <0.20significance levels. The variables with p <0.05 descriptive levels were in the final models. The most frequent symptoms were \"fatigue in the views at work\" (47.9 per cent ), \" weight in the eyes at work\" (38.3 per cent ) and \"fatigue in the views at home\" (36.3 per cent ). The most prevalent eye diseases were presbyopia (66.3 per cent ) and astigmatism (47.9 per cent ).
