Audiologists related to the hearing health of youngsters exposed to urban
noise and loud music, which provokes auditory and extra-auditory effects. It is been more and more common for young people who undergo pure tone
audiometry to present hearing thresholds above OdB HL, eloser to 20 or 25
dB HL, showing the audiometric notch. This is a hearing reduction within the
normal limit, in the frequencies of 3k, 4k or 6 kHz, in presence of a difference
of at least 10 dB from the precedent or following the analyzed frequency. The
audiometric notch may indicate a tendency of development of noise induced
hearing loss with time. Objective: To identify the presence of the audiometric
notch in youngsters from 18 to 25 years old, through pure tone audiometry
performed at a private hospital in São Paulo, from 2005 to 2007, verifying if
gender, frequency, ear and type of notch are sources of variability. Method:
This study is retrospective, quantitative and the procedure consisted in the
investigation of 722 audiograms of normal hearing subjects,being 353
(48.9%) female and 369 (51.1%) males. Results: The hearing notch was
present in 390 (54%) of the 722 studied sample, being more prevalent for
female (223 -57.2%) than male (167 - 42.8%). There were statistically
significant differences between hearing thresholds for the frequencies: 4, 6, 8
kHz in the right ear and in the left ear, for the frequencies of 1 and 4 kHz. The
prevalence of audiometric notch type unilateral - 308 (79%), with lhe bilateral
type present in only 21%. Conclusion: The results showed statistically
significant difference in the presence of audiometric notch between gender,
ears, frequency and type of hearing notch, suggesting that this population
may be at risk for future hearing losses, if adequate measures are not taken
in order to preserve their hearing health / Introdução: Nos últimos anos, tem crescido a preocupação de audiologistas
com a saúde auditiva da população jovem, em decorrência da exposição ao
ruído urbano e à música em intensidade excessiva, levando a efeitos
auditivos e extra-auditivos prejudiciais à saúde. Tem sido cada vez mais
comum os jovens submetidos à audiometria tonal liminar apresentarem limiares que se afastam do 0 dB NA e aproximam-se dos 20 ou 25 dB NA,
caracterizando o entalhe audiométrico. Trata-se de um rebaixamento dentro
da normalidade, nas freqüências de 3k, 4k ou 6 kHz, em presença de uma
diferença de, pelo menos, 10 dB da freqüência anterior ou posterior à
analisada. O entalhe pode indicar uma tendência ao desencadeamento da
perda auditiva induzida por ruído ao longo do tempo. Objetivo: Identificar a
presença do entalhe audiométrico em jovens de 18 a 25 anos, a partir da
análise das audiometrias tonais liminares, realizadas em um hospital
particular do município de São Paulo, no período de 2005 a 2007,
verificando sexo, o lado da orelha, a freqüência e o tipo de entalhe são
fontes de variabilidade. Método: O estudo é do tipo retrospectivo e
quantitativo e o procedimento constou no levantamento dos audiogramas de
722 pacientes normo ouvintes, sendo 353 (48,9%) indivíduos do sexo
feminino e 369 (51,1%) do sexo masculino, em idades variando de 18,01 a
25,99 anos.Resultados: Houve presença de entalhe audiométrico em 390
(54%) dos sujeitos estudados, sendo mais frequente para os do sexo
feminino - 223 (57,2%) do que para o sexo masculino - 167 (42,8%). Houve
diferença estatisticamente significante entre as freqüências na orelha direita,
para 4, 6, 8 kHz e para as freqüências de 1 e 4 kHz, na orelha esquerda.
Houve prevalência de entalhe audiométrico do tipo unilateral - 308 (79%),
estando o tipo bilateral presente em apenas 21%. Conclusão: Os resultados
demonstraram significância estatística na presença do entalhe audiométrico,
para todas as variáveis estudadas, o que sugere que esta população constitui um grupo de risco para futuras perdas auditivas, se medidas não
forem tomadas no sentido de preservar e manter a saúde auditiva dos
jovens em nosso meio

discrimination performance in 7 to 11 year-old children and to describe results
according to gender and to answers of teachers and parents to a questionnaire. Method:
60 children were evaluated: 30 2nd or 3rd grade students and 30 4th or 5th grade. The
distribution of the students according to age was: 26,7% were 7, 21,7% were 8, 15%
were 9, 35% were 10 and 1,6% were 11 years old. They were submitted to visual
inspection of external acoustic meatus, pure tone audiometry, timpanometry (as an
inclusion criteria). The study had three phases: screening questionnaire answered by
parents, learning difficulties questionnaire answered by teachers and sound localization,
motor-acoustic organization and auditory discrimination performance. Results: there
was no statistically significant difference between level of scholarship and tests
performance; teachers answers for the learning difficulties questionnaire and tests
performance. Students who performed poorly in the tests were oral respirators,
presented more ear ache and throat infection episodes, frequently asked repetition of
what was said, loud speakers and had familiar history of hearing loss, according to the
comparison of parents answers to a questionnaire and tests performance. Conclusions:
1. it was not possible to establish a relationship between age and tests performance; 2.
learning difficulties questions for the teachers did not allow detecting students who
needed auditory processing evaluation; 3. the questionnaire answered by parents
allowed identifying students at risk for poor performance in the applied auditory
abilities tests / Objetivo: O objetivo deste trabalho foi investigar o desempenho de crianças entre 7 e
11 anos para os testes de localização da fonte sonora, organização acústico motor e
discriminação auditiva, descrevendo os resultados do desempenho das crianças nos
diferentes testes em função do sexo e das respostas dos pais ao questionário. Método:
Foram avaliadas 60 crianças, sendo 30 matriculadas na 2 ªou 3 ª e 30 na 4 ª ou 5 ª série,
distribuídas segundo a faixa etária em : 7 anos (26,7%), 8 anos (21,7%), 9 anos (15%),
10 anos (35%) e 11 anos (1.6%). As crianças foram submetidas a inspeção visual do
meato acústico externo, audiometria tonal liminar, timpanometria (como critério de
inclusão). A coleta foi realizada em três fases: questionário de triagem auditiva com os
pais, questionário com os professores sobre a presença de dificuldade de aprendizagem
e desempenho das crianças nos testes: Teste de Localização da Fonte Sonora, de Teste
de Organização Acústico-Motora e Teste de Discriminação Auditiva com as crianças.
Resultados: a análise da relação entre nível de escolaridade e desempenho nos testes de
não mostrou diferença estatisticamente significante. Para a questão desempenho das
crianças nos diferentes testes segundo a resposta do professor para a questão ter ou não
queixa de dificuldade de aprendizagem, os resultados não foram estatisticamente
significantes. A análise descritiva em valores de porcentagem do desempenho das
crianças nos testes aplicados em relação com às respostas dos pais ao questionário
aplicado e a análise por agrupamento, mostraram que as crianças que não apresentaram
desempenho adequado eram as que os pais informaram que: eram respiradores bucais,
tiveram muitos episódios de dor de ouvido e infecção de garganta , falam muito Hãm e
o quê; fala muito alto e devagar e que tem antecedentes familiares de perda auditiva.
Conclusões: os dados obtidos nessa pesquisa permitem concluir: 1. que não foi possível
estabelecer relação entre faixa etária e desempenho nos testes; 2. que a pergunta sobre
dificuldade de aprendizagem feita ao professor não permite detectar crianças que
necessitariam de avaliação diagnóstica de transtorno de processamento auditivo; 3. que
o questionário aplicado aos pais permitiu identificar os riscos para bom desempenho nos
testes de habilidades auditivas aplicados

Previous issue date: 2007-08-28 / The early diagnosis of auditory deficiencies is one of the decisive factors for the better development of the language functions of a deficient child. It is during childhood that the neurological maturation occurs, thus favoring the development of the basic perceptive abilities, as well as of language itself. Given the importance the early auditory deficiency diagnosis, programmes of universal neonatal hearing screening have been proposed world-wide. With the aim of implementing a neonatal hearing screening programme that has an efficient protocol, this study describes the findings of a hearing screening programme conducted with 140 babies born at the Hospital Evangélico de Belo Horizonte in the period between April and June 2005, utilizing two forms of scheduling: hearing screening conducted 15 days after hospital release and hearing screening conducted immediately after hospital release. The methodology utilized in the study includes: anamnesis of the prenatal, neonatal and postnatal conducted through interviews with the mothers, hearing screening using Transient Evoked Otoacoustic Emissions, Distortion Product Otoacoustic Emissions and research of the Eye-Blink Reflex (EBR). Of the 228 (100%) babies born in this period at the hospital, of which 138 (65,7%) were born in April and 72 (34,3%) in June, 210 (92,1%) have been recruited to have the hearing screening test administered. In the April group hearing screening was scheduled for 15 days after hospital release and in the June group hearing screening was scheduled immediately after hospital release. Of the 210 (100%) babies recruited, 140 (66,6%) attended triage, of which 70 (50,7%) belonged to the 15-day group and 70 (97,2%) to the Release group. Of the 70 (100%) evaluated babies in the 15-day group, 6 (8,5%) have presented indicators of risk of auditory deficiency; and of the 70 (100%) evaluated babies in the Release group, 6 (8,5%) have also presented indicators of risk auditory deficiency. Of all the babies evaluated, 140 (100%) presented EBR. In the research of both the transient evoked and distortion product otoacoustic emissions, 2 babies (2,9%) of the 15-day group have failed the evaluation, and in the Release group 22 (31,4%) have also failed. The findings of this study have demonstrated that the failure rate of the 15-day group in the otoacoustic emission test is lower when compared to the rate of the Release Group, however the consent to the test went higher in this group, given that the ease of having the test performed at the moment of release does not require the mother to return to the hospital / O diagnóstico precoce da deficiência auditiva é um dos fatores decisivos para o melhor desenvolvimento da linguagem da criança com deficiência. É na infância que ocorre a maturação neurológica, favorecendo o desenvolvimento das habilidades perceptivas básicas, bem como a linguagem. Tendo em vista a importância do diagnóstico precoce da deficiência auditiva, programas de triagem auditiva neonatal universal têm sido propostos mundialmente. Com o objetivo de implantar um programa de triagem auditiva neonatal com um protocolo eficiente, este estudo descreve os achados de um programa de triagem auditiva, realizado em 140 bebês nascidos no Hospital Evangélico de Belo Horizonte no período de Abril e Junho de 2005, em duas formas de agendamento: triagem auditiva realizada 15 dias após a alta hospitalar e triagem auditiva realizada imediatamente após a alta hospitalar. A metodologia utilizada no estudo incluiu: anamnese dos períodos pré-natal, neonatal e pós- natal, realizada por meio de entrevistas com as mães; triagem auditiva realizada com Emissões Otoacústicas Transientes, Emissões Otoacústicas Produto de Distorção e pesquisa do Reflexo Cócleo Palpebral (RCP). Dos 228 (100%) bebês que nasceram neste período no hospital, foram recrutados para realizar a triagem auditiva 210 (92,1%), sendo 138 (65,7%) nascidos em abril e 72 (34,3%) nascidos em junho. No grupo abril a triagem auditiva foi marcada para 15 dias após a alta hospitalar e no grupo junho a triagem auditiva foi marcada imediatamente após a alta hospitalar. Dos 210 (100%) bebês recrutados compareceram para triagem 140 (66,6%), sendo 70 bebês (50,7%) do grupo 15 dias e 70 bebês (97,2%) do grupo alta. Dos 70 (100%) bebês avaliados no grupo 15 dias, 6 (8,5%) apresentaram indicadores de risco para deficiência auditiva; e dos 70 (100%) bebês avaliados no grupo alta, 6 (8,5%) apresentaram também indicadores de risco para deficiência auditiva. De todos os bebês avaliados, 140 (100%) apresentaram RCP. Na pesquisa das emissões otoacústicas, tanto transiente quanto produto de distorção, no grupo 15 dias 2 bebês (2,9%) falharam e no grupo alta 22 (31,4%) também falharam. Os achados deste estudo demonstraram que no grupo 15 dias o índice de falha no exame de emissões otoacústicas é menor quando comparado ao grupo alta; porém, a adesão ao exame foi maior neste grupo, pois a facilidade em realizar o exame no momento da alta não exige que a mãe retorne ao hospital

Previous issue date: 2007-10-09 / INTRODUCTION: Many situations may difficult the obtaining of thresholds for each ear, separately, in pure tone audiometry, demanding the use of masking. The plateau method, developed by Hood (1960) has being the most utilized for more than four decades. Nevertheless, Turner (2004) has suggested a masking protocol with a different method, called optimized, which could replace the previous one with efficacy in specific cases. PURPOSE: To verify if there is a difference among air and bone conduction hearing thresholds, using the two clinical masking methods: plateau and optimized. METHOD: Forty individuals aged from 15 to 65 years old, with either unilateral or bilateral hearing losses, considering unilateral, bilateral, symmetrical and just bone, proposed by Turner s classification, have participated of this study. They underwent air and bone conduction pure tone audiometry for both ears, without and with the use of the two masking methods. RESULTS: There was a 6.1% difference between the results for the air conduction threshold retest and a 15.1 % for the bone conduction threshold retest, taking into consideration the symmetrical model and a 12.8% for the bone conduction retest, considering the just bone model. There was no statistically significant difference between the air and bone conduction hearing thresholds with the two masking methods, considering unilateral and bilateral models. CONCLUSION: The plateau method can be utilized for all models and the optimized one is the most efficient for unilateral and bilateral models, having no indication for the symmetrical model. Therefore, both masking methods have presented advantages and disadvantages, implying that the audiologist must have knowledge of them in order to choose correctly the masking method to be used / INTRODUÇÃO: Na realização da audiometria tonal, determinadas situações dificultam a obtenção dos limiares para cada orelha separadamente, havendo a necessidade de utilização do mascaramento. O Método Platô, desenvolvido por Hood (1960), vem sendo o mais utilizado há mais de quatro décadas. Contudo, em 2004, Turner sugeriu um protocolo de mascaramento no qual um diferente método, denominado Método Otimizado, poderia substituí-lo de forma eficaz em casos específicos. OBJETIVO: Verificar se há diferença entre os limiares auditivos obtidos, por via aérea e via óssea, utilizando-se dois métodos de mascaramento clínico: o Método Platô e o Otimizado. MÉTODO: Participaram deste estudo 40 indivíduos, com idades entre 15 e 65 anos, que apresentavam perda auditiva unilateral ou bilateral, considerando os modelos unilateral, bilateral, simétrico e somente-ósseo, propostos pela classificação de Turner (2004). Foram realizados os procedimentos de audiometria tonal liminar por via aérea e óssea, para ambas as orelhas, sem e com a utilização dos dois métodos de mascaramento. RESULTADOS: Não houve diferença estatisticamente significante entre os limiares auditivos obtidos com os dois métodos de mascaramento por via aérea e via óssea considerando-se os modelos, unilateral e bilateral. No entanto, houve diferença de 6,1% dos resultados para o re-teste dos limiares por via aérea e de 15,1% para o re-teste por via óssea, considerando o modelo simétrico e de 12,8% para o re-teste de via óssea, considerando o modelo somente-ósseo. CONCLUSÃO: O Método Platô pode ser utilizado para todos os modelos e o Otimizado é mais eficaz para os modelos unilateral e bilateral, não sendo indicado para o modelo simétrico. Diante disso, ambos os métodos de mascaramento apresentaram vantagens e desvantagens, sugerindo que o audiologista tenha conhecimento destas para que proceda a seleção do método de forma consciente

Previous issue date: 2007-10-09 / Introduction: The Newborn Hearing Screening Program have been responsible for
anticipating the sensorineural and conductive hearing loss diagnoses for the last few
years. This study refers to the experience of a hearing screening starting before the
hospital discharge in the Neonatal Intensive Care Unit (NICU) and also to the
orientation to the Well-baby Nursery screening after the discharge. Aims: Describe a
Newborn Hearing Screening Program in a maternity hospital of the privative network in
Salvador, from 2000 to 2006, considering the historical context, the process of
implementation, the changes through the years, as well as the hearing findings and the
relation with the quality criteria proposed by the Joint Committee on Infant Hearing
2000.Method and Material: This is a retrospective, cross-sectional study. 4997
newborns and infants were screened through the use of Echocheck, the otoacoustic
emissions equipment and through the observation of the cochlear-palpebral reflex. The
results were divided in four groups: AI = NICU and screening in the pre-discharge; AII
= NICU and screening in the post-discharge; BI = Well-baby Nursery and screening in
the pre-discharge; BII = Well-baby Nursery and screening in the post-discharge.
Results: A description of the evolution and implementation process of the hearing
screening from 2000 to 2006 was done; in relation to the audiological findings, the
hearing screening results described were the ones from 2001 to 2006. There was an
assessment of 38,9% of the newborns and infants in 2001, and 79,5% in 2006.
According to the pre-discharge service analyses, the group AI/BI had from 8,1% to
11,2% of fails; in the post-discharge service analyses, the group AII/BII maintained the
fail index in 2,6%. As final results of the AI/BI group, from the total of 1739 screenings,
there were 21 confirmed cases of hearing alterations, being 20 conductive(6,1:1000) and
4 sensorineural (1,2:1000). Conclusion: The data indicates that the service before the
discharge to the NICU and a gradual extension to the Well-baby Nursery is workable,
ever since the protocol is continuously adjustable. Although the AI/BI group was the
third part of the sample it represented a higher prevalence of hearing alterations. These
show the importance of the hearing screening before the hospital discharge, so a higher
number of congenital hearing problems can be assessed / Introdução: Nos últimos anos, os programas de Triagem Auditiva Neonatal (TAN) têm
sido responsáveis por antecipar o diagnóstico das deficiências auditivas sensorioneurais
e condutivas, quando ambas estão presentes ao nascimento. O presente estudo relata
uma experiência de triagem auditiva iniciada antes da alta hospitalar na UTI Neonatal e
de orientação para triagem no Berçário Comum após a alta. Com o passar dos anos, o
programa estendeu-se para atendimento também no Berçário Comum. Objetivos:
Descrever um programa de Triagem Auditiva Neonatal em uma maternidade da rede
privada de Salvador, no período de 2000 a 2006, no que se refere ao seu contexto
histórico, ao processo de implantação e às modificações ocorridas ao longo dos anos;
também são discutidos os achados audiológicos e a relação com os critérios de
qualidade propostos pelo Joint Committee on Infant Hearing, de 2000. Material e
método: trata-se de um estudo retrospectivo, de caráter transversal. Foram triados 4997
neonatos e lactentes com emissões otoacústicas, equipamento Echocheck, e observação
do reflexo cócleo-palpebral. Os sujeitos foram divididos em quatro grupos: AI= UTI
Neonatal e triagem na pré-alta; AII= UTI Neonatal e triagem na pós-alta; BI= Berçário
Comum e triagem na pré-alta; BII= Berçário Comum e triagem na pós-alta. Resultados:
Foi descrito o processo de evolução e implantação da triagem auditiva no período de
2000 a 2006. Quanto aos achados audiológicos, foram descritos os resultados da triagem
auditiva de 2001 a 2006. Houve cobertura em 38,9% dos neonatos e lactentes em 2001,
chegando a 79,5% em 2006. Na análise dos atendimentos na pré-alta, o grupo AI/BI
apresentou índices de falha de 8,1% a 11,2%; na análise de atendimentos na pós-alta, no
grupo AII/BII, os índices de falha mantiveram-se em 2,6%. Como resultados finais, no
grupo AI/BI, do total de 1739 triagens, houve 21 casos confirmados de alterações
auditivas, sendo 17 condutivas (9,8:1.000) e quatro sensorioneurais (2,3 :1000). No
grupo AII/BII, do total de 3256 triagens, houve 24 casos confirmados de alterações
auditivas, sendo 20 condutivas (6,1:1000) e quatro sensorioneurais (1,2 :1000).
Conclusão: os dados indicaram que o atendimento antes da alta para UTI Neonatal e
sua extensão gradual para o Berçário Comum são viáveis, desde que o protocolo seja
continuamente ajustado. Embora representasse a terça parte da amostra, o grupo AI/BI
apresentou uma prevalência maior de alterações auditivas, evidenciando a importância
da TAN antes da alta hospitalar para o diagnóstico precoce de alterações auditivas
congênitas

The literature on this subject evaluated those programs as effective. Therefore, the literature does not point the origin of this effectiveness. Based on the information collected from these literature, a private hospital located in São Paulo created the Hearing Aid User Support Group GAUAA, which consists of four monthly meetings. Purpose: To find out the effectivness of a group hearing rehabilitation program for the elderly through shifting from quantitative data to qualitative data.
Method: Thirty subjects, hearing aid users with monoaural adaptation, showing moderate to severe hearing loss with ages between seventy and ninety-two years old have participated in this research. This study provided two situations: on situation I, the International Outcome Inventory for Hearing Aids questionnaire (IOI-HÁ) was applied before and after the program. The objetive was to evaluate the user degree of satisfaction. The collected data were submeted to the Wilcoxon test method analysis and analysed through the Spearman correlation method. The situation II was divided in two phases: on phase I, the subjects statements were collected and analysed after the program. At phase II, the subjects statements were analysed during the GAUAA meetings. These data were analysed by the french line of the speech analysis proposed by Orlandi(1996).
Results: The data found at situation I were statistically significant, but they could not indicate what would be the reason for the positive effectiveness responses from the clinical perpective. Thus, the situation II through the qualitative analysis, made possible to observe the subjects singularities, which suggested that the effectiveness of a hearing rehabilitation program is due to the identification among the participants and by transference, sustained by the coordinator´s position. The GAUAA effectiveness derives from the ludical speech dominance, which its effects promote the effectiveness of the hearing aid use / Tema: possíveis benefícios de um grupo de apoio voltado a idosos com presbiacusia e usuários de auxiliar auditivo.
Objetivo: por meio da articulação entre dados quantitativos e dados qualitativos, localizar a origem da eficácia de um programa de reabilitação auditiva em grupo de idosos. No geral, embora a literatura sobre o tema aborde esse aspecto, não há ainda estudos que propõem parâmetros de atuação e estratégias que levem ao uso sistemático do auxiliar auditivo por parte dessa população.
Método: participaram deste estudo 30 sujeitos com perda auditiva de grau moderado a severo, na faixa etária de 70 a 92 anos, usuários de auxiliar auditivo com adaptação monoaural e participantes do Grupo de Apoio ao Usuário de Auxiliar Auditivo GAUAA, criado em um hospital privado de São Paulo, sendo o trabalho desenvolvido em quatro encontros mensais. Foram analisadas duas situações: a Situação I, em que foi aplicado o questionário QI-AASI (Questionário Internacional-Aparelho De Amplificação Sonora Individual), antes e depois do programa, para avaliar o grau de satisfação do usuário. Os achados foram submetidos ao método de análise dos Testes dos Postos Sinalizados de Wilcoxon. A situação II foi dividida em duas etapas: na 1ª etapa procedeu-se à coleta e análise de depoimentos dos usuários após o programa, e na 2ª etapa, à análise dos dizeres dos participantes durante as reuniões do GAUAA. Esses dados foram analisados à luz da metodologia de Análise de Discurso de linha francesa, articulada à tipologia do discurso proposta por Orlandi(1996). Resultados: Os achados da situação I mostraram-se estatisticamente significantes, mas, clinicamente, não indicaram o que provocava as respostas positivas sobre a participação no GAUAA. Já a situação II, em que foi utilizada a metodologia qualitativa, possibilitou observar a singularidade dos sujeitos, sugerindo que a eficácia de um programa de reabilitação auditiva se dá pela identificação entre os participantes e pela transferência sustentada pela posição do coordenador. Concluiu-se que a eficácia do GAUAA deriva da dominância do discurso lúdico, cujos efeitos promovem a efetividade do uso do auxiliar auditivo

that occurs in patients with hearing loss. Currently, there are several
techniques available to select, adapt, check and validate this hearing device.
The procedures most frequently used to evaluate the benefits of hearing
AIDS are the functional gain, the insertion gain and the self-report outcome
measure. Objetivo: verificar as diferenças de acordo com o posicionamento
das caixas acústicas na pesquisa do ganho funcional e avaliar a satisfação
do usuário de AAS. Objective: to verify the differences according to the
placement of speakers in the research of functional gain. Method: Twenty
patients with mild or moderate degree of sensor neural hearing loss using
bilateral AAS for a period ranging from 10 to 53 months were evaluated. The
procedures included the application of the IOI-HA (International Outcome
Inventory for Hearing-Aids) questionnaire, the investigation of functional gain
in the frequencies of 0.5 to 4 kHz at three different positions between the
patient and the speaker (zero degrees azimuth, 45 º right and 45º left) and
insertion gain with input signal 50 dBSPL. Results: The measurements of
functional gain in three different positions for the frequency range of 0.5 to 4
kHz showed no statistically significant differences. A comparison of functional
gain in zero degree azimuth and insertion gain indicated statistically
significant differences only at frequencies of 3 to 4 kHz. Most of the results
were below the target established by the NAL-NL1 gain rule. However, 60%
of them presented IOI-HA scores equal or above 25 points. Conclusion:
concerning to functional gain, no significant differences among the three
positions were detected. Although the insertion gain presented results below
expectations, data from the IOI-HA suggests that amplification benefits the
patients / Introdução: O uso do Aparelho de Amplificação Sonora (AAS) pode
minimizar a privação sensorial que ocorre em sujeitos com perdas auditivas.
Atualmente, existem diversas técnicas disponíveis para o processo de
seleção, adaptação, verificação e validação deste dispositivo. Os
procedimentos comumente utilizados para avaliar o benefício do AAS são o
ganho funcional, o ganho de inserção e os questionários de auto-avaliação.
Objetivo: verificar as diferenças de acordo com o posicionamento das
caixas acústicas na pesquisa do ganho funcional e avaliar a satisfação do
usuário de AAS. Método: Foram avaliados 20 sujeitos com perdas auditivas
sensório-neurais de grau leve ou moderado que utilizavam AAS bilateral por
um período que variou de 10 a 53 meses. Os procedimentos incluíram a
aplicação o questionário IOI-HA (International Outcome Inventory for
Hearing-Aids), a pesquisa do ganho funcional nas freqüências de 0.5 a 4
kHz em três diferentes posições entre o sujeito e a caixa acústica (zero grau
azimute, 45º à direita e 45º à esquerda) e ganho de inserção com sinal de
entrada de 50 dBNPS. Resultados: Os resultados das medidas do ganho
funcional nas três diferentes posições para as freqüências de 0.5 a 4 kHz
não indicaram diferenças estatisticamente significantes. A comparação entre
o ganho funcional na posição zero grau azimute e o ganho de inserção
indicou diferenças estatisticamente significantes apenas nas freqüências de
3 e 4 kHz. A maioria dos resultados do ganho de inserção ficou abaixo dos
valores recomendados pela regra de ganho NAL-NL1. Apesar dos resultados
do ganho funcional e de inserção, a maioria (60%) apresentou escore do IOIHA
maior ou igual a 25 pontos. Conclusão: não foram encontradas
diferenças nas medidas do ganho funcional entre as três posições. Apesar
do ganho de inserção apresentar resultados abaixo do esperado, os dados
do IOI-HA indicaram que os pacientes se sentem beneficiados com a
amplificação

establishment of PNASA (2004), which guaranteed full assistance for the hearing impaired.
Thus, patients undergo a number of procedures during the process of selection and fitting of
hearing aids through a service hearing healthcare professional. After nearly six years of
implementation of PNASA, a growing number of patients seeking feedback on services, and
often this is related return is simple procedures that can be readily achieved for the benefit of
patients. This question envisioned the organization of an Emergency Care agenda as a strategy
to meet this demand. Goal: Establish the demographic and audiological, identify and analyze
the reasons of return and the respective managements as well as linking this analysis with the
variables of time of use of HA and the time interval between the last day of the hearing
evaluation and the Emergency Care. Method: The study included 440 subjects who met the
selection criteria of the survey, conducted in DERDIC during the period of four months. These
patients, who asked for return to service, were served through the emergency department, and
the data collected through a specific form developed by the researchers. The outcome analysis
was performed considering: audiological and demographic data of the subjects, the reason for
return and pipelines generated. Results: With regard to demographic characteristics and
audiologic of 440 subjects, we observed that 58.6% were female, 38.4% were elderly (over 60),
40.4% had schooling in primary school, 47 % had moderate hearing loss (in the better ear),
85% had sensorineural hearing loss bilaterally, 88.2% were users of BTE type hearing aid,
86.1% were using ears, 70.7% were use of hearing aids for the first time, 38.1% were using the
hearing aid for a period of up to 12 months from the date of receipt of the devices and 65.5%
reported use of hearing aids for a period equal to or greater than 8 hours / daily. The
combinations occurring more reason to return and conduct, observed in 386 subjects were,
respectively: O and O with 80% Ac and Ac with 58%, M and M 47.5%; Ass Ass and a 46% and
Aj and Aj with 36.3%. Ducts Aj, M and O have been occurring for most subjects with shorter
duration of use of hearing aids and shorter interval of time between the last day of the hearing
evaluation and emergency department concurrently. Conclusion: The characteristics of the
population that calls back to the Hearing Health Service DERDIC / PUCSP, reasons for
returning and professional conduct, reflected significant implications for organization of
processes involved in follow up of hearing aid users. A protocol for orientation in early use of
hearing aids, the scheduling of follow up visits based on the date of last hearing evaluation and
other changes in routine can result in better efficacy of the consultation, therefore greater
benefit to the user of hearing aids, as well as improved quality of service provided / Introdução: A problemática da deficiência auditiva mobilizou profissionais da saúde,
culminando na instituição da PNASA (2004), que garantiu atendimento integral aos deficientes
auditivos. Assim, os pacientes são submetidos há uma série de procedimentos durante o
processo de seleção e adaptação do AASI por meio de um serviço de atenção à saúde auditiva
credenciado. Após quase seis anos de implantação da PNASA, cresce o número de pacientes
que solicitam retorno aos serviços, e muitas vezes este retorno está relacionado há
procedimentos simples que podem ser prontamente realizados em benefício destes pacientes.
Esta questão idealizou a organização de uma agenda de Pronto Atendimento como estratégia
para atender esta demanda. Objetivo: Estabelecer as características demográficas e
audiológicas, identificar e analisar os motivos do retorno e as respectivas condutas, assim
como relacionar a referida análise com as variáveis: tempo de uso do AASI e intervalo de
tempo decorrido entre a última avaliação audiológica e o dia do Pronto Atendimento. Método:
Participaram do estudo 440 sujeitos que atenderam aos critérios de seleção da pesquisa,
realizada na DERDIC durante o período de quatro meses. Estes pacientes, que solicitaram
retorno ao serviço, foram atendidos por meio do Pronto Atendimento, e os dados coletados por
meio de ficha específica elaborada pelos pesquisadores. A análise de resultados foi realizada,
considerando-se: dados demográficos e audiológicos dos sujeitos, o motivo do retorno e as
condutas geradas. Resultados: Com relação às características demográficas e audiológicas
dos 440 sujeitos, verificamos que: 58,6% eram do gênero feminino, 38,4% eram idosos (acima
de 60 anos), 40,4% possuíam nível de escolaridade no ensino fundamental incompleto, 47%
tinham grau de perda auditiva moderada (na melhor orelha), 85% tinham perda auditiva do tipo
neurossensorial bilateral, 88,2% eram usuários de AASI do tipo retroauricular, 86,1% faziam
uso de adaptação binaural, 70,7% faziam uso do AASI pela primeira vez, 38,1% faziam uso do
AASI por um período de até 12 meses contados a partir da data de recebimento dos
dispositivos e 65,5% referiram uso do AASI por um período igual ou maior que 8 horas/diárias.
As combinações mais ocorrentes de motivo do retorno e conduta, observadas em 386 sujeitos,
foram, respectivamente: O e O com 80%; Ac e Ac com 58%; M e M com 47,5%; Ass e Ass com
46% e Aj e Aj com 36,3%. As condutas Aj, M e O foram mais ocorrentes para sujeitos com
menor tempo de uso do AASI e menor intervalo de tempo decorrido entre a última avaliação
audiológica e o dia do Pronto Atendimento concomitantemente. Conclusão: As características
da população que solicita retorno ao Serviço de Atenção a Saúde Auditiva da DERDIC/PUCSP,
os motivos do retorno e as condutas profissionais realizadas, refletiram implicações
significativas para a organização de processos envolvidos no acompanhamento de usuários de
AASI. A criação de um protocolo de acompanhamento para sessões de orientação nos
primeiros meses de uso do AASI, o agendamento do retorno para acompanhamento realizado
pela própria secretaria com base na data da última avaliação audiológica e a criação de datas
de rotina, sem necessidade de marcação para confecção de pré-moldes são sugestões de
estratégias que almejam maior resolubilidade dos retornos e benefício ao usuário de AASI,
assim como a melhora na qualidade do serviço prestado

impact caused by the use of hearing aids in the elderly. Aiming at placing the
reader inside the universe of this research project, a literature review was
made, focusing the aging process, the hearing impairment and its implications
and the sound amplification usage. Method: The procedure used was the
gathering of the thematic verbal history in seven hearing impaired elderly
collaborators, all hearing aids wearers aging from 62 to 82 years old. The
results revealed that some of the collaborators had their hearing impairment
detected by a member of the family s suspicion, due to quarrels and
arguments, which arouse from the hearing disability in their environment.
Others realized their own difficulties in understanding other person s speech
and decided to look for adequate treatment. For the majority of them the
hearing aids have offered an improvement in their quality of life, leading to a
social play enjoyment. The family and friend s respect comes back when they
are wearing their hearing aids and it is precisely this respect that makes them
happier and more sociable persons. Conclusion: The negative impact provided
by the hearing aid usage tends to decrease as the elderly become satisfied
with what they are listening to and with life, which, gradually, becomes to be
what it was before the hearing impairment. The thematic verbal history
procedure can help the gathering of more information so that we can better
understand the hearing impairment psychosocial implications, bringing us the
possibility of identifying and solving problems in these fields, as they are
uttered / O objetivo deste estudo foi analisar, por meio de relatos orais, o impacto
proporcionado pelo uso de aparelho de amplificação sonora em idosos. Com o
intuito de situar o leitor a respeito do universo no qual esta pesquisa se insere, foi
efetuada uma revisão de literatura, com ênfase no processo de envelhecimento, na
deficiência auditiva e suas implicações e no uso da amplificação sonora. Método: Foi
empregado o procedimento da história oral temática, fazendo parte sete
colaboradores com idades entre 62 e 82 anos, portadores de deficiência auditiva
neurossensorial, usuários de aparelhos de amplificação sonora. Os resultados
revelaram que alguns colaboradores desta pesquisa tiveram sua deficiência auditiva
descoberta, inicialmente, por algum membro da família ou amigos, em virtude das
críticas e discussões que as dificuldades auditivas geravam em seu ambiente.
Outros perceberam suas próprias dificuldades para compreender o que as pessoas
falavam de decidiram procurar pelo tratamento adequado. Para a maioria deles, os
AAS proporcionaram melhora na qualidade de vida, levando-os a desfrutar
novamente do convívio social. O respeito da família e dos amigos retorna quando os
idosos estão com os aparelhos de amplificação sonora, e é justamente esse respeito
que faz deles, pessoas mais felizes e sociáveis. Conclusão: O impacto negativo
proporcionado pelo uso dos aparelhos de amplificação sonora tende a diminuir
quando os idosos estão satisfeitos com o que voltam a ouvir e com a vida que, aos
poucos, volta a ser o que era antes da deficiência auditiva. O procedimento da
história oral temática pode auxiliar na obtenção de mais informação, a fim de que
possamos melhor compreender as implicações psicossociais da deficiência auditiva,
trazendo-nos a possibilidade de identificar e solucionar os problemas nessas áreas,
na medida que são verbalizados

For this, we made an option to a qualitative perspective of knowledge production, using as method the analysis of the discursive practices as a way of understanding the production of meaning in daily life (Spink, Medrado, 2004). In this way, after the accomplishment of nine opened interviews, it was possible to establish the following categories: definitions, causes, hearing aids usage, social, affectivity and derivations and to make dialogic maps for each participant. As results we observed that the hearing impairment acquired the meaning of the limitation of the contact with the other, being associated to the aging. To the hearing impaired was attributed the negative meaning of someone that is less capable. To the hearing aids had been attributed positives and negatives meanings, because they acquired the character of instruments that make possible the reestablishment of the contact with the other and that take away the hearing impaired from the situation of incapacity, but do not make it in a total way, because do not solves all the hearing impairment s difficulties. The reactions of the participants and of the others when such difficulties appears had shown the escape of the situations where the communication is demanded, being the hearing impairment a factor of social disaggregation. The emotions expressed by the participants had shown the significant impact of the hearing impairment and of the use of the hearing aids in their lives. So we conclude that the hearing impairment and the use of hearing aids influence at the acceptance of the first one and at the fitting of those equipments, being able to answer for the giving up of its use. So we conclude that the hearing impairment and the hearing aids acquire meaning, essentially, in the relationship with the others, in its improvement or restriction, a time that happens on what allows such relationship, that is, the communication / Esta pesquisa teve como objetivo estudar o sentido da deficiência auditiva e do uso de aparelhos de amplificação sonora (AAS) para idosos. Para tal, optamos por uma perspectiva qualitativa de produção de conhecimento, utilizando como método a análise das práticas discursivas como forma de compreender a produção de sentidos no cotidiano (Spink, Medrado, 2004). Desse modo, após a realização de nove entrevistas abertas, foi possível estabelecer as categorias definições, causas, uso de AAS, social, afetividade e derivações e construir mapas dialógicos para cada participante. Como resultados observamos que a deficiência auditiva adquiriu o sentido da limitação do contato com o outro, sendo associada ao envelhecimento. Ao deficiente auditivo foi atribuído o sentido negativo de ser menos capaz. Aos AAS foram atribuídos sentidos positivos e negativos, uma vez que adquiriram o caráter de instrumentos que possibilitam o re-estabelecimento do contato com o outro e que afastam o deficiente auditivo da situação de incapacidade, mas que não o fazem totalmente, uma vez que não resolvem todas as dificuldades comunicativas decorrentes da deficiência auditiva. As reações dos participantes e dos outros frente a tais dificuldades mostraram a fuga das situações em que a comunicação é exigida, sendo a deficiência auditiva, então, mais um fator de desagregação social. As emoções expressas pelos participantes revelaram o significativo impacto da deficiência auditiva e uso de AAS em suas vidas. Assim, concluímos que os sentidos da deficiência auditiva e uso de AAS influenciam na aceitação da primeira, bem como na adaptação desses equipamentos, podendo responder pela desistência de seu uso. Além disso, notamos que a deficiência auditiva e os AAS adquirem sentido, essencialmente, na relação do sujeito com o outro, em sua facilitação ou restrição, uma vez que incidem sobre aquilo que permite tal relacionamento, ou seja, a comunicação
