Contudo, ainda não existe um consenso quanto a frequência de alterações do processo de deglutição nessa população. O estudo tem por objetivo estimar a frequência de desordens da deglutição no pré e pós-tratamento de pacientes com câncer de cabeça e pescoço. Uma revisão sistemática foi desenvolvida seguindo o guia para relato de itens de revisão sistemática e meta-análises (PRISMA). Estratégias de busca foram desenvolvidas para as seguintes bases de dados: PubMed, LILACS, Scopus, Web of Science, LIVIVO e SpeechBITE. Adicionalmente, uma busca da literatura cinzenta foi realizada através do Google Scholar, Open Grey e ProQuest. Somente estudos que realizaram avalição diagnóstica da deglutição utilizando exames objetivos como videofluroscopia da deglutição ou videoendoscopia da deglutição foram incluídos na análise. O risco de viés dos estudos incluídos foi analisado com a ferramenta “The Critical Appraisal Checklist for Studies Reporting Prevalence Data from the Joanna Briggs Institute”. A meta-análise de proporção, com efeito fixo ou randômico, foi realizada através do Software estatístico MedCalc versão 14.8.1 (MedCalc Software, Ostend, Belgium). A seleção dos estudos foi realizada em duas fases, por dois revisores, independentemente. Dezesseis estudos passaram pelos critérios de elegibilidade e foram incluídos para análise. Em todos os estudos a deglutição foi avaliada antes e até 12 meses após o tratamento oncológico. Aspiração prévia a tratamento de câncer teve frequência de 11,3% (desvio padrão (SD), 8,7 a 14,3%; amostra total=517), entre 1 a 6 meses após tratamento ocorreu um aumento para 27,1% (SD, 1,0 a 36,0%; amostra total=478) e até 12 meses pós-tratamento teve uma queda para 17,9% (SD, 12,3 a 2,8%; amostra total=153). Penetração de volume acima das pregas vocais e redução de elevação laríngea também foram mais frequentes no período de 1 a 6 meses após tratamento oncológico. Os resultados encontrados nesse estudo indicam que a frequência de desordens de deglutição e suas complicações como a aspiração, em pacientes com câncer de cabeça e pescoço aparenta ser maior no período de até 6 meses após tratamento para o câncer. / The deglutition disorders are common sequelae of the oncologic treatment. However, there is no consensus over the frequency of the alteration in the swallowing mechanisms in this population. The study aims to estimate the frequency of deglutition disorders in patients pre and post-treatment for head and neck cancer. A systematic review was developed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline. Search strategies were developed for the following databases: PubMed, LILACS, Scopus, Web of Science, LIVIVO, and SpeechBITE. Additionally, a search of the grey literature was performed through Google Scholar, Open Grey, and ProQuest. Only studies that performed evaluation of deglutition before and after cancer treatment were included in this systematic review. The studies had to use diagnostic exams for deglutition disorders as Videofluoroscopy swallowing exam, Fiber-optic endoscopic evaluation of swallowing, modified barium swallow or Videofluorographic swallow study. The Critical Appraisal Checklist for Studies Reporting Prevalence Data from the Joanna Briggs Institute was used to assess the risk of bias of the included studies. A proportion of fixed or random effects meta-analysis using the MedCalc Statistical Software version 14.8.1 (MedCalc Software, Ostend, Belgium) were conducted. The selection of the studies was divided in two phases where two reviewers worked independently. Sixteen studies met the eligibility criteria and were included. In all of the studies an assessment of the deglutition was performed previous and up to 12 months after receiving treatment for the cancer. Aspiration previous to the cancer treatment had a frequency of 11.3% (Standard deviation (SD), 8.7 to 14.3%; total sample=517), between 1 to 6 months after treatment, this increased to 27.1% (SD, 19.0 to 36.0%; total sample=478), and up to 12 months after there was a decrease to 17.9% (SD, 12.3 to 24.8%; total sample=153). Penetration above the vocal cords and reduced larynx elevation were also more frequent in the 1 to 6 months’ period after the treatment for the head and neck cancer. The results found in this study indicates that the frequency of deglutition disorders and its complications as aspiration, in patients with head and neck cancer, appears to be higher in the immediate to 6 months’ post-treatment period.

É definida como uma área maior que 1cm de osso exposto, em região previamente irradiada, e que não apresenta nenhuma evidência de cura durante pelo menos três meses. Diversas modalidades terapêuticas foram propostas, mas o seu tratamento permanece, ainda hoje, desafiador. Dentre estas modalidades está a ozonioterapia, considerada promissora devido ao efeito antioxidante e às propriedades bioestimuladoras do ozônio. Objetivo: Avaliar por meio de microtomografia computadorizada (Micro-CT) e análise histológica os efeitos terapêuticos do ozônio sistêmico, no osso mandibular murino irradiado. Métodos: Vinte ratos Wistar, com idade de 90 dias, foram divididos igualmente em dois grupos: grupo ozonioterapia (OZT) e grupo controle (CNT). Ambos os grupos receberam irradiação através de Braquiterapia de Alta Taxa de Dose (HDR), com dose semanal de 10Gy, durante 5 semanas, totalizando 50Gy de dose de irradiação. O grupo OZT foi submetido à aplicação de gás ozonizado, via insuflação retal, três vezes por semana, durante seis semanas, iniciando uma semana antes do início da radiação ionizante, enquanto o grupo CNT não recebeu qualquer intervenção com ozônio. Todos os animais, de ambos os grupos, foram submetidos ao escaneamento microtomográfico, antes (T0), durante (T1) e após (T2) as intervenções. Os animais foram eutanasiados 15 dias após a finalização dos tratamentos e as peças foram submetidas ao processamento histológico. Resultados: A quantificação dos dados microtomográficos mostrou diferença estatisticamente significante entre o T0 e T2, em ambos os grupos, no parâmetro de Volume ósseo e o Volume da Amostra (BV/TV), evidenciando o desenvolvimento de osteorradionecrose. Nos lados irradiados, a análise histológica demonstrou danos osteogênicos, como desorganização tecidual, fibrose e sinais de necrose. Não foram encontradas diferenças significativas na Micro-CT óssea e nos parâmetros histológicos entre OZT e CNT. Conclusão: Nosso modelo animal induziu a osteorradionecrose, permitindo a análise imaginológica e histológica do dano ósseo causado pela radiação. Entretanto, no protocolo utilizado, a ozonioterapia não foi eficaz na prevenção e no tratamento de osteorradionecrose. / Introduction: Osteoradionecrosis is a potentially serious complication of radiation therapy in the treatment of head and neck cancer. It is defined as an area larger than 1 cm of exposed bone in a previously irradiated region, which shows no evidence of cure for at least three months. Several therapeutic modalities have been proposed, but their treatment remains challenging. Among these modalities there is the ozone therapy, which is considered promising due to the antioxidant effect and the biostimulating properties of ozone. Objective: To evaluate therapeutic effects of the systemic ozone in the irradiated murine mandibular bone, by means of computerized microtomography and histological analysis. Methods: Twenty Wistar rats, aged 90 days, were equally divided into two groups: ozone therapy group (OZT) and control group (CNT). Both groups received irradiation through High Dose Rate Brachytherapy (HDR) with a weekly dose of 10Gy for 5 weeks, totaling 50Gy doses of irradiation. The OZT group was submitted to ozonized gas via rectal insufflation, three times a week for six weeks, starting one week before the beginning of the ionizing radiation while the CNT group received no ozone intervention. All animals of both groups were submitted to microtomographic scanning, before (T0), during (T1) and after (T2) the interventions. The animals were euthanized 15 days after finishing the treatments and their pieces were submitted to histological processing. Results: The quantification of the microtomographic data presented a statistically significant difference between T0 and T2 in both groups, in the parameter of Bone Volume and Volume of the Sample (BV/TV), evidencing the development of osteoradionecrosis. On the irradiated sides, the histological analysis showed osteogenic damage, such as tissue disorganization, fibrosis and signs of necrosis. No significant differences were found in the bone Micro-CT and in the histological parameters between OZT and CNT. Conclusion: Our animal model induced the osteoradionecrosis, allowing imaging and histological analysis of the bone damaged caused by radiation. However, in the protocol used, the ozone therapy was not effective on the prevention and treatment of osteoradionecrosis.

Transplante renal e risco de câncer de cabeça e pescoço : revisão sistemática e meta-análise / Kidney transplantation and head and neck cancer risk: systematic review and meta-analysis

Kidney transplantation and head and neck cancer risk: systematic review and meta-analysis Paulo Ricardo Saquete Martins Filho, Aracaju/SE, Brazil, 2013. Background: Kidney transplantation is considered the treatment of choice for end-stage kidney disease, but a wide-ranging excess risk of post-transplant malignancies has been recognized as a complication of long-term immunosuppression. De novo malignancies are important cause of morbidity and mortality in kidney recipients. We performed a systematic review and meta-analysis to determine the risk of head and neck cancer after kidney transplantation. Methods: A systematic search was performed in PUBMED, EMBASE, SCOPUS, and LILACS databases to identify cohort studies reporting on the risk of head and neck cancer in kidney recipients. The assessment of validity of selected studies was performed using the STROBE statement and the Newcastle-Ottawa Scale (NOS) for Cohort Studies. Only studies with NOS ≥ 6 were included in the meta-analysis. Pooled relative risks (RR) were calculated using the Mantel-Haenszel or DerSimonian-Laird method, depending on statistical heterogeneity. To detect publication bias, Egger‟s test, Duval and Tweedie‟s analysis, and leave-one-out sensitivity analysis were conducted. Results: A total of 9 high-quality cohort studies were included in the meta-analysis. The pooled RR of head and neck cancer after kidney transplantation was 8.2 (95% CI 4.0-16.6, p<0.0001). A significant excess risk of cancer was observed in the lip (RR = 43.6, 95% CI 24.2-78.4, p<0.0001). The pooled RR of oral cavity/pharynx and salivary gland was 3.5 (95% CI 2.5-5.0, p<0.0001) and 5.6 (95% CI 1.3-24.0, p = 0.020), respectively. No evidence of publication bias was observed. Conclusion: There is an increased risk of head and neck cancer after kidney transplantation. The head and neck should be examined routinely during the post-transplant surveillance. / Transplante renal e risco de câncer de cabeça e pescoço: revisão sistemática e meta-análise, Paulo Ricardo Saquete Martins Filho, Aracaju/SE, Brasil, 2013. Introdução: O transplante renal é considerado o tratamento de escolha para a doença renal terminal, porém um aumento do risco de câncer pós-transplante tem sido reconhecido como uma complicação da imunossupressão em longo prazo. Malignidades pós-transplante são uma importante causa de morbidade e mortalidade em recipientes renais. Foi realizada uma revisão sistemática e meta-análise para determinar o risco de câncer de cabeça e pescoço após transplante renal. Método: Uma busca sistemática foi realizada nas bases de dados PUBMED, EMBASE, SCOPUS e LILACS para identificar estudos de coorte que estimaram o risco de câncer de cabeça e pescoço após transplante renal. A avaliação da validade dos estudos selecionados foi realizada através da iniciativa STROBE e da Newcastle-Ottawa Scale (NOS) para estudos de coorte. Somente estudos com NOS ≥ 6 foram incluídos na meta-análise. Os riscos relativos (RR) combinados foram calculados através do método de Mantel-Haenszel ou de DerSimonian-Laird, a depender da presença de heterogeneidade estatística. Para detectar viés de publicação, foram utilizados o teste de Egger, a análise de Duval e Tweedie e análise de sensibilidade leave-one-out . Resultados: Um total de 9 estudos de coorte de alta qualidade foram incluídos na meta-análise. O RR combinado do câncer de cabeça e pescoço após transplante renal foi de 8.2 (IC 95%4.0-16.6, p<0.0001). Um significante aumento do risco de câncer foi observado no lábio (RR = 43.6, IC 95% 24.2-78.4, p<0.0001). O RR combinado para o câncer de cavidade oral/faringe e glândulas salivares foi 3.5 (IC 95% 2.5-5.0, p<0.0001) e 5.6 (IC 95% 1.3-24.0, p = 0.020), respectivamente. Não houve evidência de viés de publicação. Conclusão: Há um aumento no risco de câncer de cabeça e pescoço após transplante renal. A região de cabeça e pescoço deve ser examinada rotineiramente durante a vigilância pós-transplante.
