A Detection Method of Ectocervical Cell Nuclei for Pap test Images, Based on Adaptive Thresholds and Local DerivativesOscanoa1, Julio, Mena, Marcelo, Kemper, Guillermo 04 1900 (has links)
Cervical cancer is one of the main causes of death by disease worldwide. In Peru, it holds the first place in frequency and represents 8% of deaths caused by sickness. To detect the disease in the early stages, one of the most used screening tests is the cervix Papanicolaou test. Currently, digital images are increasingly being used to improve Pap test efficiency. This work develops an algorithm based on adaptive thresholds, which will be used in Pap smear assisted quality control software. The first stage of the method is a pre-processing step, in which noise and background removal is done. Next, a block is segmented for each one of the points selected as not background, and a local threshold per block is calculated to search for cell nuclei. If a nucleus is detected, an artifact rejection follows, where only cell nuclei and inflammatory cells are left for the doctors to interpret. The method was validated with a set of 55 images containing 2317 cells. The algorithm successfully recognized 92.3% of the total nuclei in all images collected. / Revisón por pares

Identificação do linfonodo-sentinela em pacientes com carcinoma de colo uterino invasor estádio I-B 1Reis, Ricardo dos January 2005 (has links)
Objetivo: determinar a viabilidade da identificação do linfonodo-sentinela em pacientes com câncer invasor de colo uterino estádio Ib1. Material e Métodos: 16 pacientes consecutivas com câncer de colo uterino agendadas para histerectomia radical com linfadenectomia pélvica bilateral realizaram estudo para detecção de linfonodo-sentinela. Onze pacientes injetaram 1 mCi de tecnécio 99 (99Tc) em quatro pontos do estroma superficial do colo uterino ao redor do tumor, às 12, 3, 6 e 9 h ( 16 horas antes da cirurgia ). No dia da cirurgia, as pacientes foram submetidas ao mapeamento linfático com gamma-probe e azul patente injetado nos mesmos pontos que o 99Tc.Cinco pacientes realizaram a detecção apenas com azul patente. Resultados: foi detectado pelo menos um (1 a 3 por paciente) linfonodo-sentinela em cada uma das 15 pacientes (93,7 %) que realizaram a técnica combinada.Foi detectado pelo menos 1 linfonodo-sentinela em 4 pacientes ( 80% )com azul patente apenas. A maioria dos linfonodos-sentinela foi localizada nas regiões: obturadora (37 %), ilíaca externa (22,2 %) e inter-ilíacas (18,5 %). Seis pacientes (40 %) tiveram linfonodos-sentinela bilaterais. Dos 27 linfonodos-sentinela detectados, 11 (40,7 %) foram detectados pelo corante, 9 (33,3%) pela radioatividade e 7 (26 %) pela radioatividade e corante. O índice de detecção intra-operatória com o gamma-probe foi de 90,9 % ( 11 pacientes ). Destes, 7 linfonodos foram azul e quente (31,8 %), 6 linfonodos foram apenas azuis (27,2 %) e 9 linfonodos foram apenas quentes (40,9 %). A sensibilidade, especificidade e valor preditivo negativo para a detecção do linfonodo-sentinela foram 100%, 85,7 % e 100 % respectivamente. Conclusão: A combinação do radiofármaco 99Tc e azul patente é efetiva na detecção do linfonodosentinela em câncer de colo uterino inicial. / Objective: to establish the feasibility of sentinel lymph node identification in patients with stages Ib1 invasive cervical cancer. Material and Methods: 16 consecutive patients with cervical cancer, were scheduled for radical hysterectomy with bilateral pelvic lymphadenectomy, were submitted to sentinel lymph node detection. Eleven patients was injected 1 mCi of technetium-99 (99Tc) into four points of the superficial cervical stroma around the tumor, at the 12, 3, 6 and 9 o’clock positions (sixteen hours before surgery ). On the day of the surgery, the patients were submitted to gamma-probe-guided lymphatic mapping and patent blue dye was injected into the same points as the 99Tc. Five patients were submitted to detection with patent blue only. Results: at least one sentinel lymph node (one to three per patient) was detected in each of the 15 patients (93.7%) in whom a combination of both methods was used. At least one sentinel lymph node was detected in four patients ( 80 % ) with patent blue only. Most sentinel lymph nodes were identified in the obturator (37%), external iliac (22.2%) and interiliac (18.5%) regions. Six patients (40%) had bilateral sentinel lymph nodes. Of the 27 sentinel lymph nodes detected, 11 (40.7%) were identified by the blue-dye technique, 9 (33.3%) by radiation, and 7 (26%) by radiation and by the blue-dye technique. The intraoperative detection rate was 90.9% with gamma-probe only ( 11 patients ). Of these, 7 lymph nodes were blue-stained and hot (31.8), 6 were just blue-stained (27.2%) and 9 were just hot (40.9%). Sensitivity, specificity and the negative predictive value for sentinel lymph node detection were 100%, 85,7% and 100 % respectively. Conclusion: the combination of 99Tc and patent blue dye is effective in the detection of sentinel lymph nodes in early-stage cervical cancer.

There are more than 500,000 new cases of uterine cervical cancer (CCU) worldwide, which is the second most common cancer among women, and the second leading cause of death among women aged between 14 and 44 years. There is a known relationship between the CCU and the presence of human papillomavirus (HPV). In Brazil, one in four sexually active women are infected with HPV (in the Northeast, one in three), resulting in 19 cases of uterine cancer per 100,000 inhabitants. Laboratory evidence shows that, at present, preventive molecular test HPV-linked cancer through the detection and typing of viral DNA, may be associated with reduced mortality between men and women infected with HPV. It is known that there is a discrepancy between the histological and cytomorphological detection of HPV lesions in routine tests, so the molecular test is very important in this scenario. Thus, this study aimed to determine the HPV prevalence and the socio-demographic profile of infected women in State of Alagoas, hence propose a low cost kit for the more prevalent high risk HPV detection. Cervical samples from women living in the state of Alagoas - Brazil were assessed; these patients were submitted to a questionnaire and signed the free and informed consent form, through which we could collect information relating to the main co-factors commonly associated with the development of cervical lesions. Genomic DNA extraction was performed according to Illustra kit methodology (Asmersham Biosciences), while the DNA concentration in each sample was estimated using spectrophotometry. Viral evidence was obtained by PCR amplification of a 450bp viral DNA region (L1 gene) using the degenerate primers MY09 and MY11 We used the co-amplification of human β- globin gene (268 bp) as positive control, through GH20 and PC04 primers, and human DNA without HPV as negative control. The expected size PCR products were sequenced automatically. To identify the virus type, the sequences were aligned and compared with others using the Blastn tool in NCBI (National Center for Biotechnology Information). From the 757 samples analyzed, 123 (16.3%) were positive for HPV. There was a prevalence of high-risk HPV types (58,5%), with 12,3% HPV 16 positive samples. Individually, the most frequent type was the HPV 6 (21.5%), considered low risk. Regarding the co-factors, it was observed significant difference in the age of the patients; number of partners; number of children; and being pregnant at the time of data collection. The proposed kit will enable detection of the main oncogenic types present in the state by multiplex PCR analysis. The primers used responded well to the established analysis. Further steps are being taken towards the implementation of a commercial test kit. / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Mundialmente ocorrem mais de 500 mil novos casos de câncer cervical uterino (CCU) por ano, sendo a terceira neoplasia mais comum entre mulheres, e a segunda causa de morte na faixa etária entre 14 e 44 anos. Há uma conhecida associação entre CCU e a presença do papilomavírus humano (HPV). No Brasil, uma em cada quatro mulheres sexualmente ativas está infectada pelo HPV (no Nordeste, uma em três), resultando em 19 casos de câncer uterino por 100 mil habitantes. Evidências laboratoriais revelam que a detecção e tipagem do DNA viral para prevenção do câncer causado por HPV, pode estar associado à redução da mortalidade entre pacientes infectados. Sabe-se que há discrepância entre os resultados citomorfológicos e histológicos na detecção de lesões por HPV nos exames rotineiros, por isso o teste molecular assume grande importância. O presente trabalho teve como objetivos definir a prevalência do HPV no Estado de Alagoas e o perfil sociodemográfico das mulheres infectadas, com o intuito de propor um “kit” de baixo custo para detecção dos HPV de alto risco mais prevalentes. Amostras cervicais de mulheres residentes no Estado foram analisadas; as pacientes foram submetidas a um questionário, para a coleta de informações referentes aos principais cofatores comumente associados ao desenvolvimento de lesões cervicais. A extração do DNA genômico foi realizada conforme metodologia do kit Illustra (Asmersham Biosciences), enquanto a concentração de DNA em cada amostra foi estimada através de espectrofotometria. A comprovação da presença viral foi feita por PCR, através da amplificação de uma região de 450pb do DNA viral (gene L1), com iniciadores degenerados MY09 e MY11. Como controle positivo utilizou-se a co-amplificação do gene humano β-globina (268 pb) com os iniciadores GH20 e PC04, além de um controle negativo, DNA humano sem HPV. Os produtos de PCR foram sequenciados automaticamente. Para identificar o tipo viral, as sequências foram alinhadas e comparadas a outras utilizando-se a ferramenta Blastn no NCBI (National Center for Biotechnology Information). Foi possível estabelecer que das 757 amostras analisadas 123 (16,3%) foram positivas para HPV. Houve prevalência dos tipos de HPV de alto risco (58,5%), sendo o HPV16 observado em 12,3% das amostras positivas. O HPV de maior frequência foi o HPV 6 (21,5%), considerado de baixo risco. Sobre os cofatores, pôde-se observar diferença significativa em relação à idade das pacientes; número de parceiros; número de filhos; e ser gestante no momento da coleta de dados. O kit de detecção proposto possibilitará a determinação dos principais tipos oncogênicos presentes no Estado por PCR multiplex. Os iniciadores utilizados responderam bem às análises estabelecidas. Novas etapas se fazem necessárias para implementação do “kit”.

Screening methanolic extracts of Sutherlandia spp as anti-tumor agents and their effects on anti-apoptotic genesRakoma, Mamphago Annah 03 1900 (has links)
Cervical cancer is the most common malignancy after breast cancer in women worldwide. It accounts for 83% of all new cases and 85% cancer death in developing countries. In South Africa, cervical cancer is the common cancer in women with an annual crude incidence rate of 30.2 per 100,000 women and the highest rate were found to be in women between the ages of 66-69 years. Approximately 6800 women in S.A face new case of cervical cancer while accounting for 3700 cancer death annually. Because of unequal access to the health facilities, socio-economic differences, HPV and HIV infection, the rate of cervical cancer in black women is higher (42.1%) compared to the low rate in white women. Because of the name “cancer bush’ given to Sutherlandia Frutescence(S.F) plant by the traditional healers as well as Xhosas, Zulu, Sotho and cape Dutch for its anti-cancer activity, the plant was in this study to confirm its cytotoxic effect on the cervical cancer cell lines.
Aim of the study: to evaluate the methanolic extracts of Sutherlandia Frutescens on cervical cancer cell lines. Materials and Methods: The MTT assay was performed to evaluate SiHa cell lines treated with methanolic extract of S.F (50μg/ml, 100μg/ml, 150μg/ml and 200μg/ml). The three compounds (Canavanine, GABA and Pinitol) were also evaluated for its anti-tumour activity. The cell growth was then showed in real time using Xcilligence. Flow cytometry was employed to determine the mode of action. Caspase 3/7 assay was performed to confirm if cell death was via caspase-dependent or independent and ATP was done to assess the ATP level in S.F treated cells.
Results: MTT shows a significant decline in cells treated with 50μg/ml, 100μg/ml and 200μg/ml of the extract and 50μg/ml was concluded to be the concentration at which 50% of the cells die. The ATP results are inconsistent with MTT result; the ATP level increased in S.F treated cells. Cell index which represents the quantitative measure of cell growth in real time decline upon treatment with 50μg/ml. Flow cytometry showed cells are dying by apoptosis and the cell cycle arrest is mostly in the S phase. The cell death was caspase-dependent as it shows an increased luminescence which is proportional to the number of caspase. The concentrations of the compounds used, Canavanine (1000μM, 1500μM and 2500μM), GABA (100μM, 300μM and 500μM) and Pinitol (30μM, 90μM and 120μM) induce cell death and cell death shows to decrease after the maximum concentration. Conclusion: Sutherlandia Frutescence has proven with number of research that it induces cell death via apoptosis. After evaluating its cytotoxicity, the plant shows to be a promising anti-tumor agent that needs to be clinically proven. / Life Sciences / M. Sc. (Life Science)

O presente estudo, caso-controle, compara os polimorfismos dos genes XPC, ERCC1 e XRCC7 em 77 mulheres com câncer cervical (70 casos de carcinoma espinocelular e 7 casos de adenocarcinoma do colo do útero) e 73 mulheres saudáveis atendidas no Hospital do Câncer Alfredo Abrão, entre Junho de 2007 e Maio de 2009. No Laboratório de Ginecologia Molecular da EPM-UNIFESP, os polimorfismos desses genes foram detectados pela técnica de reação em cadeia da polimerase seguida da análise do polimorfismo do fragmento de restrição (PCR-RFLP). As distribuições genotípicas dos polimorfismos no grupo de casos e controle estavam em Equilíbrio de Hardy-Weinberg (p>0,05). Pelo teste exato de Fisher as distribuições genotípicas dos polimorfismos (XRCC7 (G-C): GG (11,7%), GC (41,6%) e CC (46,8%) no grupo de casos e GG (20,5%), GC (41,1%) e CC (38,4%) no grupo controle (p=0,31); ERCC1 (C-T): CC (39,0%), CT (51,9%) e TT (9,1%) no grupo de casos e CC (53,4%), CT (38,4%) e TT (8,2%) no grupo controle (p=0,20); XPC (A-C): AA (50,6%), AC (41,6%) e AA (7,8%) no grupo de casos e AA (45,2%), AC (37,0%) e CC (17,8%) no grupo controle, p=0,19) não apresentaram diferença estatística significante. pública no Brasil. Algumas recomendações para o controle desta doença no país são
baseados em opiniões de especialistas e, no caso de pacientes com colpocitologias
sugestivas de lesão intra-epitelial escamosa de alto grau (HSIL) e colposcopia insatisfatória
sem lesão visível, consistem em repetir a colpocitologia após três meses. No Setor de
Patologia Cervical do Instituto Fernandes Figueira/ FIOCRUZ, estas pacientes são
encaminhadas diretamente para conização, na intenção de confirmar o diagnóstico e, caso
presente HSIL, já se constituir no tratamento destas lesões.
Objetivo: Medir a prevalência de HSIL e câncer em pacientes com a primeira
colpocitologia sugestiva de HSIL e colposcopia insatisfatória sem lesão visível, além de
identificar uma faixa etária ou grupos em que esta prevalência seja maior, no intuito de
contribuir para a discussão sobre uma conduta clínica mais efetiva e que diminua a
probabilidade de ocorrência de perdas antes do diagnóstico e tratamento adequados nesta
situação.
Material e método: Estudo transversal realizado através de pesquisa em banco de dados de
pacientes recebidas no Setor de Patologia Cervical no período de dezembro de 1989 a abril
de 2007 referidas pela rede básica de saúde para colposcopia, obtendo o diagnóstico final
através dos laudos histopatológicos das peças de conização.
Resultados: Foram incluídas 65 pacientes na situação descrita e encontrado percentual de
33,8% (IC95% 23,1- 45,9%) de HSIL e 4,6% (IC95% 1,1- 12,0%) de câncer confirmados
histologicamente. Os demais casos apresentaram lesão intra-epitelial escamosa de baixo
grau (LSIL - 26,1%, IC95% 16,5- 37,8%), displasia glandular (1,5%, IC95% 0,07- 7,35%)e ausência de doença (33,8%, IC95% 23,1- 45,9%). Não foi encontrada diferença
estatisticamente significativa na razão de prevalência de HSIL e câncer em grupos de
mulheres segundo estratos de idade abaixo e acima de 45 anos e entre mulheres
soropositivas e soronegativas para HIV.
Conclusão: A prevalência de HSIL ou câncer encontrada não parece suficiente para
defender a conduta de encaminhá-las de imediato para conização a fim de investigar o canal
cervical. Porém, são necessários ensaios clínicos randomizados para que possamos saber
qual a conduta mais adequada na situação em estudo. / Introduction: Cervix cancer is a serious public health problem in Brazil. Some of the
recommendations for controlling the disease in the country are based on the opinion of
specialists. In patients with unsatisfactory colposcopy with no visible lesion and cervical
cytology suggesting high-grade squammous intraepithelial lesion (HSIL), the
recommendation is to repeat cervical cytology after three months. In the Cervical
Pathology unit at Instituto Fernandes Figueira/ FIOCRUZ, these patients are directly
subjected to conization in order to confirm diagnosis and to excise the lesions found, if
HSIL is verified.
Objectives: To assess prevalence of HSIL and cancer in patients with unsatisfactory
colposcopy with no visible lesion and first cervical cytology suggestive of HSIL, and to
identify an age group or other groups with higher prevalence in order to contribute to the
discussion about a more effective clinical approach that will also prevent patients from
abandoning follow-up before full diagnosis and treatment.
Method: Cross-sectional study with data from the Cervical Pathology unit database on
patients treated between December 1989 and April 2007 who were referred to the unit for
colposcopy by the primary healthcare network. Final diagnosis was obtained through
histopathological examination of conization specimens.
Results: Of the 65 patients studied, 33,8% (CI95% 23,1- 45,9%) had HSIL, and 4,6%
(CI95% 1,1- 12,0%) had cancer confirmed by histological examination. The other patients
were diagnosed as having low-grade squammous intraepithelial lesion (LSIL - 26,1%,
CI95% 16,5- 37,8%), glandular dysplasia (1,5%, CI95% 0,07- 7,35%), and absence of
disease (33,8%, CI95% 23,1- 45,9%). This study did not find statistically significant difference of the prevalence ratio of HSIL and cancer between age groups of more than 45
and less than 45 years of age, and between women tested positive or negative for HIV.
Conclusion: The prevalence of cancer and HSIL found in this study does not seem to be
enough to justify direct referral for conization to investigate the cervical canal.
Nevertheless, randomized clinical trials are necessary to determine an approach that
would be more adequate in this situation.

A mortalidade por esse câncer é evitável, uma vez que existem tecnologias para o diagnóstico e tratamento de lesões precursoras, permitindo a cura de 100% dos casos na fase inicial. A Organização Mundial de Saúde (OMS) recomenda aos países membros a elaboração de programas de detecção precoce e tratamento de lesões precursoras e do câncer cervical. Esta pesquisa tem como objetivo geral descrever a situação do rastreio de lesões precursoras do câncer de colo uterino nas unidades públicas de saúde no Departamento de Dakar, no Senegal. Os objetivos específicos foram descrever o rastreio de lesões precursoras do câncer do colo uterino, traçar o perfil das mulheres que realizaram o exame preventivo no ano de 2007 e estudar a frequência de lesões precursoras e de carcinoma invasor. Para a descrição das ações de prevenção secundária, foram feitas visitações às unidades públicas e aplicação de um questionário com aspectos relacionados à estrutura e processos dessa assistência. Para traçar o perfil das mulheres e estudar a frequência das lesões, foram analisados 2.005 laudos citopatológicos. No Departamento de Dakar, ações de controle do câncer cervical são desenvolvidas de forma isolada por algumas unidades hospitalares da rede pública, por iniciativa de profissionais especialistas e/ou organizações não governamentais, e se pode dizer que a construção de uma política na área do câncer cérvico-uterino no Senegal é bem incipiente. Ainda são necessários estudos neste país para um conhecimento mais amplo sobre a epidemiologia das patologias do colo uterino e dos fatores sociodemográficos, culturais e econômicos associados. / The cervical cancer is a problem of public health throughout the world and it is the
principal cause of deaths among women in developing countries. The mortality from this
type of cancer is preventable, once we can rely on technologies for the diagnosis and
treatment of precursor lesions, allowing for cure in 100% of cases in an early stage. The
World Health Organization has recommended its country members to elaborate programs
for early detection and treatment of precursor lesions and cervical cancer. The general aim
of this research is to describe the way screening the precursor lesions of cervical cancer is
done in the public health units in the Dakar Department in Senegal. The specific aim is to
describe the screening of precursor lesions of cervical cancer; draw a profile of the women
who underwent the cervical smear in the year 2007; and study the frequency of precursor
lesions and invasive carcinoma. In order to describe the procedures of secondary
prevention, several visits were made to public health units and a questionnaire containing
aspects related to the structure and procedures of this health care was administered. In
order to study the women´s profile and the frequency of the lesions, 2005 citopathological
reports have been analysed. In the Dakar Department, procedures for cervical cancer
control have been developed in an isolated way by some hospital units of public health by
initiative of specialized professionals and/or non-governmental organizations and we can
say that building up a policy in the area of cervical cancer in Senegal is quite incipient.
Further studies are still necessary for a broader knowledge of the epidemiology of cervical
pathologies and its related economic, cultural and sociodemographic factors in Senegal.

pública no Brasil. Algumas recomendações para o controle desta doença no país são
baseados em opiniões de especialistas e, no caso de pacientes com colpocitologias
sugestivas de lesão intra-epitelial escamosa de alto grau (HSIL) e colposcopia insatisfatória
sem lesão visível, consistem em repetir a colpocitologia após três meses. No Setor de
Patologia Cervical do Instituto Fernandes Figueira/ FIOCRUZ, estas pacientes são
encaminhadas diretamente para conização, na intenção de confirmar o diagnóstico e, caso
presente HSIL, já se constituir no tratamento destas lesões.
Objetivo: Medir a prevalência de HSIL e câncer em pacientes com a primeira
colpocitologia sugestiva de HSIL e colposcopia insatisfatória sem lesão visível, além de
identificar uma faixa etária ou grupos em que esta prevalência seja maior, no intuito de
contribuir para a discussão sobre uma conduta clínica mais efetiva e que diminua a
probabilidade de ocorrência de perdas antes do diagnóstico e tratamento adequados nesta
situação.
Material e método: Estudo transversal realizado através de pesquisa em banco de dados de
pacientes recebidas no Setor de Patologia Cervical no período de dezembro de 1989 a abril
de 2007 referidas pela rede básica de saúde para colposcopia, obtendo o diagnóstico final
através dos laudos histopatológicos das peças de conização.
Resultados: Foram incluídas 65 pacientes na situação descrita e encontrado percentual de
33,8% (IC95% 23,1- 45,9%) de HSIL e 4,6% (IC95% 1,1- 12,0%) de câncer confirmados
histologicamente. Os demais casos apresentaram lesão intra-epitelial escamosa de baixo
grau (LSIL - 26,1%, IC95% 16,5- 37,8%), displasia glandular (1,5%, IC95% 0,07- 7,35%)

e ausência de doença (33,8%, IC95% 23,1- 45,9%). Não foi encontrada diferença
estatisticamente significativa na razão de prevalência de HSIL e câncer em grupos de
mulheres segundo estratos de idade abaixo e acima de 45 anos e entre mulheres
soropositivas e soronegativas para HIV.
Conclusão: A prevalência de HSIL ou câncer encontrada não parece suficiente para
defender a conduta de encaminhá-las de imediato para conização a fim de investigar o canal
cervical. Porém, são necessários ensaios clínicos randomizados para que possamos saber
qual a conduta mais adequada na situação em estudo. / Introduction: Cervix cancer is a serious public health problem in Brazil. Some of the
recommendations for controlling the disease in the country are based on the opinion of
specialists. In patients with unsatisfactory colposcopy with no visible lesion and cervical
cytology suggesting high-grade squammous intraepithelial lesion (HSIL), the
recommendation is to repeat cervical cytology after three months. In the Cervical
Pathology unit at Instituto Fernandes Figueira/ FIOCRUZ, these patients are directly
subjected to conization in order to confirm diagnosis and to excise the lesions found, if
HSIL is verified.
Objectives: To assess prevalence of HSIL and cancer in patients with unsatisfactory
colposcopy with no visible lesion and first cervical cytology suggestive of HSIL, and to
identify an age group or other groups with higher prevalence in order to contribute to the
discussion about a more effective clinical approach that will also prevent patients from
abandoning follow-up before full diagnosis and treatment.
Method: Cross-sectional study with data from the Cervical Pathology unit database on
patients treated between December 1989 and April 2007 who were referred to the unit for
colposcopy by the primary healthcare network. Final diagnosis was obtained through
histopathological examination of conization specimens.
Results: Of the 65 patients studied, 33,8% (CI95% 23,1- 45,9%) had HSIL, and 4,6%
(CI95% 1,1- 12,0%) had cancer confirmed by histological examination. The other patients
were diagnosed as having low-grade squammous intraepithelial lesion (LSIL - 26,1%,
CI95% 16,5- 37,8%), glandular dysplasia (1,5%, CI95% 0,07- 7,35%), and absence of
disease (33,8%, CI95% 23,1- 45,9%). This study did not find statistically significant

difference of the prevalence ratio of HSIL and cancer between age groups of more than 45
and less than 45 years of age, and between women tested positive or negative for HIV.
Conclusion: The prevalence of cancer and HSIL found in this study does not seem to be
enough to justify direct referral for conization to investigate the cervical canal.
Nevertheless, randomized clinical trials are necessary to determine an approach that
would be more adequate in this situation.

frequente entre as mulheres no inicio da vida sexual, cuja persistência pode
determinar câncer do colo do útero. O câncer do colo do útero pode ser
prevenido secundariamente com triagem cervical e primariamente com vacinas
para HPV, No Brasil, a vacina quadrivalente contra HPV passou a fazer parte
do Programa Nacional de Imunização a partir de 2014, tendo como publico alvo
meninas de 11 a 13 anos. O conhecimento dos enfermeiros sobre HPV, câncer
cervical e vacinas contra HPV é importante para o sucesso do programa de
vacinação. OBJETIVO: Avaliar o conhecimento e as atitudes de enfermeiros
sobre câncer do colo do útero, infecção pelo HPV e vacinas contra HPV.
Objetivos específicos: estimar a aceitabilidade da vacina contra HPV entre
enfermeiros e identificar barreiras/obstáculos e facilitadores para a aceitação
da vacina contra HPV por enfermeiros. MATERIAIS E MÉTODOS: Estudo de
corte transversal, realizado com enfermeiros cadastrados no Conselho
Regional de Enfermagem da Bahia. Foi enviado um questionário postado no
SurveyMonkey®, via mala direta, sob a forma de link, para os enfermeiros com
cadastro no conselho. O projeto foi aprovado previamente pelo comitê de ética
em pesquisa da FIOCRUZ. RESULTADOS: Participaram do inquérito 1.283
enfermeiros, na sua maioria mulheres, pardas, casadas, Especialistas, atuando
na área assistencial e na rede pública. Foram categorizados por tempo de
graduação, sexo e local de atuação. Apesar de a maioria conhecer o HPV
(99%) e saber da associação com o câncer cervical (98%), muitos
desconhecem que o HPV também causa verrugas (10%) e acreditam que a
camisinha protege completamente contra o HPV (59%). Os enfermeiros não
conhecem as recomendações do Ministério da Saúde sobre a triagem do
câncer cervical, acreditam que esse exame é muito sensível para detecção de
lesões pelo HPV (95%). Desconhecem que o HPV pode causar câncer em
outros sítios (47%) e em homens (40%). Estes achados não diferem
significativamente por sexo. Os enfermeiros atuando na rede pública parecem
conhecer menos sobre HPV, câncer e vacinas contra HPV. A quase totalidade
(99,5%) aceita a vacina contra HPV. Itens facilitadores da aceitabilidade
incluem: acreditar em vacinas como forma de prevenção, segurança da vacina
e a prevenção do câncer de colo uterino. As barreiras identificadas foram:
receio de eventos adversos, pouca idade da filha e falta de recomendação
médica. CONCLUSÕES: Os enfermeiros conhecem moderadamente sobre
HPV, câncer do colo do útero e vacinas contra HPV porém existem importantes
lacunas nesse conhecimento. Os graduados há menos de cinco anos parecem
conhecer menos, embora devessem estar mais atualizados. Os enfermeiros da
rede pública conhecem menos que os da rede privada. / INTRODUCTION: The viral infection with Human Papillomavirus (HPV) is the
most common among women after sexual debut, the persistence of which can
determine cervical cancer. Cervical cancer can be secondly prevented by
cervical screening and primarily by HPV vaccines. In Brazil, the HPV
quadrivalent vaccine has become part of the National Immunization Program
since 2014, targeting girls aged 11 to13 years old. The knowledge of nurses
about HPV, cervical cancer and HPV vaccines is important for success of
vaccination programs. OBJECTIVE: To evaluate knowledge and attitudes of
nurses about cervical cancer, HPV infection and HPV vaccines. Specific
objectives: to estimate the acceptability of HPV vaccine among nurses and
identify barriers/obstacles and facilitators to accept the HPV vaccine by nurses.
MATERIAL AND METHODS: A cross-sectional study carried out with nurses
registered in the Regional Council of Nursing in Bahia. A questionnaire posted
at the SurveyMonkey®, was sent via direct mail, in the form of link, to all nurses
with registered in the board. The project was approved by the Ethics Research
Committee of FIOCRUZ. RESULTS: Overall, 1,283 nurses took part in this
survey, most of them women: mixed race, married, specialists, and working in
patients’ assistance and on the public sector. They were stratified by time of
graduation, gender and workplace. Even though most of them were aware of
HPV (99%) and knew its relation to cervical cancer (98%), many were unaware
that HPV causes warts (10%) and also believe that the use of condom protects
against HPV (59%) successfully. The nurses did not know about the Ministry of
Health recommendations regarding screening for cervical cancer, and believed
that such test has high sensitivity for the detection of HPV lesions (95%). They
were unaware of HPV as a cause of cancer in other sites (47%) and in men
(40%). These findings did not differ significantly by gender. The nurses working
in the public sector seemed to know less about HPV, cancer and HPV vaccines
than those working in the private sector. Almost all accepted the HPV vaccine
(99,5%). Facilitators items of acceptability included: to believe in vaccines as
prevention, the vaccine safety and the prevention of cervical cancer. The
barriers identified were: fear of adverse events, believing the daughter was too
young and a lack of medical recommendation. CONCLUSION: Nurses
moderately know about HPV, cervical cancer and HPV vaccines but there are
important gaps in this knowledge. Those graduating less than five years ago
seemed to know less about HPV. Also, nurses in the public sector less about
HPV than those in the private sector.

Controle do câncer do colo do útero : avaliação do programa de rastreamento em um serviço orientado pelos princípios da atenção primária à saúde / Cervical cancer control: screening program evaluation in a primary care oriented health serviceWilhems, Daniela Montano January 2012 (has links)
O câncer do colo do útero (CCU) é passível de prevenção e tratamento, quando detectado precocemente e seguido do efetivo tratamento. Ainda assim, é o terceiro tipo mais comum entre as mulheres no mundo e mantém-se como a quarta causa de morte por câncer em mulheres no Brasil. Em países desenvolvidos observou-se diminuição importante da incidência e mortalidade por CCU nas últimas décadas, principalmente após a implantação de programas organizados de rastreamento. Estes programas pressupõem uma rede de atenção coordenada de serviços, capazes de oferecer desde o rastreamento populacional com foco e cobertura no grupo mais suscetível até a garantia de tratamento de qualidade quando necessário. A oferta de exames citológicos do colo do útero tem sido uma importante estratégia para o controle deste câncer na população feminina no Brasil. Entretanto supõe-se que um percentual significativo de mulheres, com exames alterados no rastreamento, que são encaminhadas para avaliação colposcópica para confirmação diagnóstica não chega a fazê-la, e o sistema de saúde também não é eficiente para controlar adequadamente esse evento. Nesta direção, estudo observacional, tipo coorte histórica, foi realizado com usuárias de um serviço de Atenção Primária à Saúde (APS) em Porto Alegre, RS, Brasil. O objetivo principal foi avaliar o programa de rastreamento desenvolvido neste serviço, principalmente em relação ao seguimento dos casos positivos identificados no rastreamento. Todas as mulheres que apresentavam lesões precursoras de câncer do colo do útero, no exame citológico (CP), entre 2001-2007, foram elegíveis para o estudo e acompanhadas até agosto de 2011. Avaliou-se a condição prévia de rastreamento, o diagnóstico/tratamento e o seguimento. Para a análise da qualidade do cuidado foram considerados o alcance dos atributos da APS (acesso, longitudinalidade e coordenação dos cuidados) e a efetividade do tratamento e do acompanhamento subsequente. A ausência de CP anterior estava associada à detecção de alteração de maior grau. A maioria das mulheres (94%) foi encaminhada para avaliação ginecológica, sendo que 97% delas compareceram a consulta. O tempo mediano entre as datas do CP alterado e a consulta no serviço de referência foi de 25 dias. Houve a confirmação do diagnóstico em 80% das mulheres que fizeram histologia. Após tratamento, 89% apresentaram CP normal, sendo que 66% têm seguimento subsequente realizado na APS. Estimamos que a vigilância em saúde com auxílio da busca ativa contribuiu para melhorar o acesso e a adesão aos cuidados. Este serviço trabalha para controle do CCU há mais de 20 anos, e as taxas de mortalidade por este câncer são menores do que as encontradas em Porto Alegre. O maior alcance da longitudinalidade e coordenação dos cuidados, características de um serviço orientado pela APS, pode ter relação com estes resultados. / Cervical cancer is preventable and treatable depending on its early detection followed by effective treatment. Even so, it is the third type of cancer more common among women in the world. In Brazil is the fourth cause of death due to cancer among women. In developed countries, an important decrease in incidence and mortality of cervical cancer has been observed in the last decades, mainly after the implementation of organized screening programs. Such programs are responsible for a network of services capable to offer population screening focusing on more susceptible groups, and including the warranty of high quality treatment when necessary. The offer of cytological exams of the uterine cervix has been an important strategy to control this cancer among Brazilian women. However, it is estimated that a significant proportion of women with abnormal screening exams who are referred to colposcopy to diagnostic confirmation do not perform such exam, and the health system is not efficient enough to control this event. In this perspective, a historical cohort study was performed, including women who attended a Primary Care Service in Porto Alegre city, southern Brazil. The main objective was to evaluate the screening program developed in this service, especially regarding the follow-up of positive cases identified during screening. All women who presented cervical cancer precursor lesions in the cytological exam between 2001 and 2007, were eligible for the study and they were followed-up until august 2011. The women condition prior to screening, diagnostic/treatment and follow-up was evaluated. To analyze the quality of care were considered the scope of the Primary Care attributes (access, longitudinality and coordination of care) and effectiveness of treatment and subsequent follow-up. The absence of a previous cytological exam was associated to the detection of a high grade abnormality. The majority of women (94%) were sent to gynecological evaluation and 97% attended the appointment. The median time between the date of the abnormal cytological exam and the appointment was 25 days. There was diagnostic confirmation through biopsy in 80% of the studied women. After treatment, 89% presented normal cytological exam, being 66% performed in the Primary Care Service. It’s estimated that a health surveillance with active search contributed to improve the access and adhesion to the follow-up. This service works to control the cervical cancer for more than 20 years, and the mortality rates of this cancer are lower than those reported for the whole city of Porto Alegre. The greatest extent of longitudinality and coordination of care, characteristics of a service-driven APS, may be related to these results.
