The operationalisation of cluster foster care schemes : a social developmental perspective

The presence of poverty, unemployment and children been infected or affected by HIV/Aids contributed to a high level of children in need of care and protection (Patel, 2005:165) and forced communities to came up with a ways of addressing this increasing need of alternative care. Cluster foster care schemes were one of the initiatives that were started spontaneously by community members to address need for more alternative care options (Colby-Newton, 2006:18). This form of alternative care was also included with the proclamation of the Children’s Act, 38 of 2005 and the Children’s Amendment Act, Act 41 of 2007 (Matthias, 2010:172-176). Although cluster foster care schemes are acknowledge and practice as a form of alternative care for children in need of care and protection the operationalising thereof remain a grey area. In an unpublished study by Taback and Associates, it was suggested that further research should be conducted regarding elements that need to be included in the operationalising of cluster foster care schemes (Taback and Associates, 2010:4). The above form the motivation for this study and also provide the basis for the formulation of the research question namely: “What are the operational elements necessary for the operationalising of cluster foster care schemes?” Following from the research problem and research question, the goal for this research was to explore and describe the present functioning of cluster foster care schemes in order to identify operational elements to be documented in a guideline for service providers and social workers. The researcher made use of qualitative research approach to explore and describe the participants’ perception on the research problem. This research endeavour firstly falls in the ambit of the phenomenology research design, which aimed to describe the conscious experience of the everyday life of the participants, which were in this study the managers of cluster foster care schemes as well as foster parents that provide foster care as part of a cluster foster care scheme. In addition to this the researcher also opted to include the exploratory, descriptive and contextual research design, which guided the exploration, description and contextualising of the views of the participants on elements needed to operationalise cluster foster care schemes. In order to achieve the above the researcher the researcher decided to make use of an intervention research model by Rothman and Thomas (1994:3-51), namely the “Intervention Develop and Design” framework (IDD framework). In this study the researcher only used phases one to phase four and certain steps of the IDD framework to develop the operational guidelines for the operationalising of cluster foster care schemes. In concluding the report of the study, the researcher provided summaries of and conclusions related to the background rational; research questions; goals and task objective of the study; the research methodology implemented; the empirical findings and literature consulted; and the developed operational guidelines. He concluded the document with recommendation related to the research process and methodology employed; research findings; and recommendation for further research on the research topic. / Social Work / D. The research presents a qualitative study about the complex intertwining of stories of the individuals involved. In one hand, the caregivers or educators, women who undertake their own struggles in life while dealing with a primitive work environment, where they must embrace children who are both neglected by their own families and by Public Policies. Their stories intersect in the struggles: to care for and to protect these children, while acknowledging them as subjects of civil rights in a context seen by the institution as violent and of abandonment, bring back the caregiver‘s own struggles and create a context of empathy. Based on Winnicott, we analyze what are the main fundamentals to offer those children and how to define and optimize the caregivers‘ role. In addition, by analyzing both the past and current realities of the professionals (Guirado 1987; 2002), we discuss about the intertwining forces inside this intersubjective relationship. In other words, we aim to investigate the meaning of affection and the ways the involved parts demonstrate it versus the guidelines the institution would need to provide. By examining the narrative of seven caregivers along with observation of their routine in the shelters, as well as combining information from the shelter‘s official written notes, we found a strong correlation between the lives of educators and children, with the institution being the biggest divergence since it will affect the perception of otherness. That means the educator empathize and feel compelled to provide care, however, is simply not able to provide sufficient refuge. It is clear that all involved need asylum, including the caregivers since they all strongly idealize the institution of the family. Therefore, this study intends to clarify the challenges faced by the shelters when dealing with providing refuge, since the caregivers‘ background and feelings will play an important role on how they care for the children. We suggest new approaches, from research and public policies standpoints in order to create ultimately new alternatives to improve care to sheltered children. / Esta pesquisa discute a trama afetiva que se constitui no encontro entre educadoras e crianças no Serviço de Acolhimento a partir das histórias de vida desses atores. Um estudo qualitativo sobre o entrelaçamento das histórias de mulheres que enfrentam duramente seu dia-a-dia, com situação de trabalho precarizado no acolhimento de bebês e crianças vistos com descaso pelas Políticas Públicas, cujas famílias apresentam muita dificuldade com as responsabilidades da vida cotidiana. A função de proteger, cuidar, reconhecer como sujeitos de direitos estes pequenos, filhos de histórias que são vistas apenas pelo abandono e violência, mobiliza as marcas infantis e experiências vividas destas educadoras, permeando o encontro entre eles. A partir de Winnicott, pensamos o que seria importante oferecer a estas crianças e como potencializar e delimitar a função destas profissionais. Num percurso pela história da infância abrigada e da realidade de todos ali, produto e produtora de seus integrantes (Guirado, 1987), refletimos sobre os vetores que se cruzam nesta relação intersubjetiva. Isto é, queremos verificar as representações e afetos das experiências de vida dos envolvidos associadas ao que aquela instituição demanda afetivamente. Através de entrevistas narrativas de sete educadoras, observação sistemática da dinâmica da Casa de Acolhimento, assim como da leitura de seu Livro de Ocorrência, averiguamos que entre as histórias de vida de educadoras e acolhidos a institucionalização acaba sendo o diferencial maior, permitindo identificações, mas dificultando o reconhecimento da alteridade. Movimento que permite aproximação e dedicação, mas pode dificultar o acolhimento. Fica claro que todos ali querem acolhimento, especialmente pela forte idealização de família, que falta a todos. Assim, esta tese pretende, com a contribuição da psicanálise, esclarecer melhor os desafios do acolhimento nos abrigos, trazendo à tona o envolvimento e as implicações afetivas das cuidadoras, sugerindo caminhos teóricos, de pesquisa e de políticas públicas que possam aprimorar alternativas para enfrentar estes desafios.

Experiences of social workers regarding foster care backlogs at Botlokwa in Capricorn District of Limpopo Province

Thesis (M. A. (Social Work)) -- University of Limpopo, 2020 / Foster care is the best method of alternative care for children that need care and protection. It offers children an atmosphere and environment that recognise their economic, social, educational, health and cultural needs without discrimination. The fundamental objective of foster care is to make sure children are protected and grow up in a safe and healthy environment. Notwithstanding the positive impact that comes with it, it has been confirmed worldwide that foster care suffers from serious deficiencies to an extent that the rights of children are compromised. The Department of Social Development is no exception to foster care backlogs, which result in foster care placement applications accumulating and families’ quality of life being affected and compromised. Social workers are inundated with high caseloads on a daily basis. The study employed qualitative and collective case study design. The non-probability, convenient sampling technique was followed to select twelve statutory social workers at Botlokwa Department of Social Development satellites. Out of these social workers, eight were female and four were male. Data was collected by means of literature review and semi-structured in-depth interviews guided by an interview schedule with open-ended questions. The data was analysed thematically with the help of the Nvivo programme. The study unearthed factors such as shortage of social workers, social auxiliary workers and supervisors, system deficiencies within the children’s court fraternity, insufficient tools of trade, multiple work responsibilities and lack of cooperation from clients as contributing to foster care backlogs. The findings also revealed that in order to cope with these backlogs, some social workers engage in unethical conducts such as comforting clients with lies and not attending to foster care files. Instead, they write process notes and use their own resources to execute their roles and help one another to deal with the backlogs. Therefore, it is recommended that specialisation in foster care, foster care file audit, hiring more social workers, effective working relationship between foster care stakeholders, adopting a computerised reminder system for files due for review, convenient court dates and amendment of form 30 and the advertisement of unknown fathers can be efficient measures to mitigate foster care not only at Botlokwa, but also within the Department of Social Development nationwide. The researcher therefore concludes that foster care backlogs within social work offices is real, and that to curb it requires collaborative efforts x by role players, particularly the Department of Social Development and the Department of Justice and Constitutional Development.

Exploring challenges faced by social workers on the management of foster care backlog at the Department of Social Development, Limpopo Province, South Africa

(Development Planning and Management)) -- University of Limpopo, 2022 / The foster care backlog has been a challenge in the National Department of Social Development for some years which resulted in the lapsing of foster care grants and thousands of children left without legal and statutory protection. Due to this, civil society took the Department of Social Development to court as this challenge impacted negatively on the family functioning of foster families as they lost access to social grants. The Department of Social Development was ordered by the South Gauteng High Court to come up with an extensive legal solution by the end of December 2014 but, to date, the foster care backlog is still piling up and foster families‟ standard of life is threatened. The social workers who are mandated by the Children‟s Act to conduct the statutory process and place children in foster care are inundated by the high foster care backlog. The aim of the research study was to explore the challenges social workers face in relation to managing foster care backlog cases at the Department of Social Development in the Polokwane Municipality area, Limpopo Province. The research study was based on a qualitative research method whereby a small sample of 21 foster care social workers from three service points (Aganang Cluster, Polokwane East and Polokwane West) were interviewed. The researcher used face to face semi-structured interviews to collect primary data from the participants. The findings from the study affirmed that there were indeed challenges in managing the foster care backlog. Lack of support and training, together with inequitable distribution of cases, are the most dominant challenges identified by the participants. In order for the provincial Department of Social Development to reduce the high caseload and for the clients to continue receiving the best service, the research study recommends that the vacant post be filled as soon as possible. Furthermore, training on legislation dealing with children including the Children‟s Act is also recommended for social workers. The researcher is of the opinion that the findings and the recommendations of the study may add value in managing foster care backlog in the Polokwane Municipality area and Limpopo Province as a whole, if they can be considered and applied.

An ethnographic study of caregiving at a daycare centre for divelopmentally challenged children

In this qualitative study the researcher explores the experiences of various caregivers at a day care centre for developmentally challenged children. This study also attempts to draw a distinction between the experiences of formal and informal caregivers’ experiences. The study is underpinned by two assumptions: 1) that in order to obtain a rich understanding of a phenomenon, the views and voices of a variety of individuals need to be considered and 2) that cultural values and practices play a significant role in the way individuals experience a certain phenomenon. The experiences of both formal and informal caregivers are documented using thematic analysis. An analysis of the participants’ global themes revealed that “education/learning/skills development” and “communication barriers” were common themes among both participant groups. Each participant group also expressed unique experiences such as “being a parent”, “becoming a foster parent” and “being useless as a professional-in-training”. It is a relational object that pulsates in lands that are very strongly intertwined between the Single Social Assistance System and the Justice System. To carry out this study presupposes a complex movement that shows a saturated web of historical, cultural, political, economic and social mediations. From this perspective, we chose the qualitative research, carried out with the support of the oral source, as a way of getting as close as possible to the reality and historical processes experienced and counted by the subjects participating in the research. The narrative of three women, Cristina, Nega and Margareth, whose stories intersect not only through the institutionalization of their children, but also through a daily life that is crossed by inequalities, violence and abandonment. In order to help in this process, the narratives of workers working in the Justice System and the Single Social Assistance System also came on the scene: two social workers and an lawyer working in CREAS, an social worker and an psychologist from the Municipal Reception Service, an social worker of the Reception Service for children up to 06 years old, and four tutorial advisers. The orality allows us to understand how subjects give meaning to reality, the possibility of "giving life" to what is set in procedural records, medical records, reports and PIAs, to know the lives and stories behind the persons held As negligent, incapable or dependent. Thus, allowing access to other contours and living beyond the inability to care for their children. It was tried to raise questions that contribute so that families with institutionalized children have access to a social protection citizen and, the destitution of the familiar power, can be more and more an exception / Objetivou-se com a pesquisa conhecer as vivências de acesso aos serviços/políticas públicas de famílias com filhos em situação de acolhimento institucional e as repercussões para o retorno ao convívio com seus filhos. Trata-se de um objeto relacional que pulsa em terrenos que se entrelaçam muito fortemente entre o Sistema Único de Assistência Social e o Sistema de Justiça. Realizar esse estudo pressupõe um movimento complexo, que evidencia uma trama saturada de mediações históricas, culturais, políticas, econômicas e sociais. Partindo dessa perspectiva, escolhemos a pesquisa qualitativa, realizada com o suporte da fonte oral, como forma de nos aproximarmos o mais perto possível da realidade e dos processos históricos vivenciados e contados pelos sujeitos participantes da pesquisa. O fio condutor que teceu a sua construção foram as narrativas de três mulheres, Cristina, Nega e Margareth, cujas histórias se intercruzam não apenas pela institucionalização dos seus filhos, mas também por um cotidiano atravessado por desigualdades, violências e abandonos. Para auxiliar nessa tessitura também entraram em cena as narrativas de trabalhadores que atuam no Sistema de Justiça e no Sistema Único de Assistência Social, sendo: dois assistentes sociais e uma advogada atuando no CREAS, uma assistente social e uma psicóloga do Serviço de Acolhimento Municipal, uma assistente social do Serviço de Acolhimento para crianças de até 06 anos e, quatro conselheiros tutelares. A oralidade nos permite compreender a forma como os sujeitos dão significado a realidade, a possibilidade de “dar vida” ao que está posto nos autos processuais, nos prontuários, nos relatórios e nos PIAs, de conhecer as vidas e histórias por traz das pessoas tidas como negligentes, incapazes ou dependentes. Possibilitando, assim, o acesso a outros contornos e viveres para além da incapacidade de cuidarem de seus filhos. Buscou-se levantar questionamentos que contribuam para que famílias com filhos institucionalizados tenham acesso a uma proteção social cidadã e, a destituição do poder familiar, possa ser cada vez mais uma exceção

An assessment of the knowledge, attitudes and practices of caregivers of HIV positive children on treatment in Pretoria, South Africa : a case study of out-patients in Kalafong Hospital, Pretoria

Thesis (MPhil)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: This study examines the level of HIV and AIDS knowledge, attitudes, as well as practices amongst caregivers of HIV infected children in Pretoria, using Kalafong hospital as a case study. The study uses a questionnaire as survey instrument. A total of 30 respondents participated in the study, which took place in October of 2013. The respondents were selected via simple random sampling and the results were analyzed. The results showed a remarkably high level of HIV and AIDS knowledge amongst the respondents, from which it was evident, that: A significant number of caregivers were aware of and able to take care of existing medical conditions arising from HIV in children. Most of the home-based care of children living with HIV was carried out by women in a very disproportionate ratio to men. In this regard, the study offers a range of suggestions and recommendations as well as existing best practices, such as the UNAIDS booklet on caregiving within the context of HIV and AIDS. The study was undertaken with the realization that generalizations cannot be made through extrapolation to the larger society because of limitations, such as the sample size of this study. / AFRIKAANSE OPSOMMING:Hierdie studie ondersoek die vlak van MIV/VIGS kennis, houdings teenoor dieselfde, sowel as praktyke onder versorgers van MIV-besmette kinders in Pretoria, met behulp van Kalafonghospitaal as 'n gevallestudie. Die studie maak gebruik van 'n vraelys as opname instrument. 'n Totaal van 30 respondente het deelgeneem aan die studie, wat in Oktober 2013 plaasgevind het. Die respondente is gekies deur 'n eenvoudige ewekansige steekproefneming en die resultate is ontleed. Die resultate toon 'n merkwaardig hoë vlak van MIV en VIGS kennis onder die respondente. Hierdie kennis, houdings en praktyke opgedoen was voldoende om daarop te let: • 'n beduidende aantal van die versorgers is bewus van en in staat om bestaande toestande in MIV-sorg in kinders te versorg. • Die meeste van die tuisversorging van kinders wat met MIV leef is uitgevoer deur vroue in 'n baie oneweredige verhouding met mans. In hierdie verband bied die studie 'n verskeidenheid van voorstelle en aanbevelings sowel as die bestaande beste praktyke soos die UNAIDS boekie oor versorging binne die konteks van MIV en VIGS. Die studie is gedoen onder die besef dat veralgemenings nie gemaak kan word deur ekstrapolasie na die groter samelewing nie, as gevolg van beperkings soos die monster grootte van hierdie studie

O exemplo das internações por dependência crônica de ventilação pulmonar mecânica é apresentado no presente estudo,a partir da observação inicial de que constituem-se situações que determinam longa permanência hospitalar em leitos de alta complexidade, como nosde terapia intensiva pediátrica, sendo também alvo de solicitações através de sentenças judiciais para cobertura de assistência domiciliar ainda precariamente estabelecida no âmbito do sistema de saúde brasileiro. Há mais de três décadas que têm sido realizados estudos em outros países, na tentativa de ampliar-se o conhecimento sobre este problema de saúde, sendo analisados diferentes aspectos relacionados ao mesmo, como alternativas de locais seguros para cuidados em saúde oferecidos a estes pacientes crônicos e as diferenças de resultados médicos e sociais entre as alternativas possíveis. Com o objetivo de conhecer e analisar a ocupação de leitos de terapiaintensiva pediátrica por pacientes com dependência crônica de ventilação pulmonar mecânica, foi realizado um estudo seccional do tipo levantamento censitário de casos, de janeiro a março de 2011, em 29 hospitais públicos localizados na Cidade do Rio de Janeiro. O estudo permitiu identificar 31 pacientes de 0 a 18 anos, internados e em ventilação mecânica invasiva há 3 meses ou mais, em 10 dos 29 hospitais consultados. Nas unidades de terapia intensiva havia 14 internações justificadas por esse tipo de dependência tecnológica, caracterizando uma ocupação de 21% do total de leitos disponíveis de terapia intensiva, no período de estudo. O conhecimento desse quantitativo permite aprofundamento da discussão sobre aspectos éticos, técnicos e normativos que devem ser considerados no desenvolvimento de propostas à assistência de pacientes crônicos, visando a melhor relação entre investimentos e resultados, para os pacientes, seus familiares e a sociedade. / Children and adolescents with chronic health conditions hospitalizations are increasingly evolved in different countries and presented as a health situation that must be considered in planning and organizing action of health services in Brazil. The hospital chronic dependency on mechanical ventilation is an example presented in this study, from the initial observation situation which determining long stay high complexity hospital beds, as in the pediatric intensive care, is also the subject of requests through sentences legal coverage for home care, still precariously established within Brazilian health system. Studies in others countries to expand knowledge about this health problem had been for over three last decades, analyzing different aspects related as alternative safe places for health care offered tothese chronic patients and differences between medical and social results from possible alternatives. With the aim to understand and analyze the occupation ofpediatric intensive care beds with ventilator-dependent patients, a survey was realized from January to March, 2011, in 29 public hospitals located in the city of Rio de Janeiro. The study identified 31 patients from zero to 18 years,hospitalized and on invasive mechanical ventilation for 3 months or more, in 10 of the 29 investigated hospitals. In 14 stays in intensive care units werejustified for this type of technological dependency, characterizing a 21% rateoccupancy of total intensive care beds available, along the research period. Knowledge that quantitative conduct to in-depth discussion on ethical, technical and legal standards should be considered when developing proposals aimed assistance for chronic patients, seeking the best value between investments and outcomes for patients, their family and society.

Les magazines que contiennent des sessions dans cette periode discriminée sont trois: Fon Fon, Allons lire et La Cigale. La thèse c’est divisée en deux blocs avec des sessions differentes. Le premier bloc représente la première partie de la thèse dans la quelle on voit l’admission théorique sur le but de recherche. Le deuxième bloc, qui représente la deuxième partie de la thèse, presente l’exploration du but de recherche à partir des magazines Fon Fon, Allons lire et La Cigale, pendant la periode de 1940 à 1950. L’ensemble des articles qui compose les sessions de ces magazines configurent la source documentaire, composée de 213 articles où je décrits et j’analyse les informations , les descriptions et les représentations de la femme mère, caractérisée par les experts, qui leur écrivaient dans ces sessions spécifiques destinées à mères par l’exercice de la maternité. La recherche démontre que les experts que s’ont dédié plus fréquemment dans la publication de textes et de messages à les femmes mères ont été les médecins.Ceux-ci exercent une vraie éducation de la femme pour la fonction de la maternité; ils ont entré dans l’histoire familière , en intervenant et en engendrant des normes por la routine de la éducation du fils, en engendrant une athmosfère de faute, de dilution, de l’autonomie parentale,à travers de l’imposition de la puissance de la connaissance spécialisée, en disqualifiant les traditions familières et des endroits, en disqualifiant les parents, les parents et en cherchant prendre les espaces des laïques et des orientations des personnes plus vieilles et plus proximes de la famille. (Résumé complet accès életronique cidessous) / O presente trabalho se propôs a compreender, analisar e interpretar os textos em forma de artigos, produzidos em seções destinadas às mães em revistas voltadas ao grande público entre os anos de 1940 a 1950. As revistas que mantêm seções nesse período discriminado são: Fon Fon, Vamos Ler e A Cigarra. A tese está dividida em dois blocos de diferentes seções. O primeiro bloco retrata a primeira parte da tese, onde se evidencia o aporte teórico sobre o objeto de pesquisa; o segundo bloco representa a segunda parte da tese, que retrata a exploração do objeto de pesquisa a partir das revistas Fon Fon, Vamos Ler e A Cigarra, compreendendo o período de 1940 a 1950. O conjunto dos artigos que compõe as seções dessas revistas, configuram a fonte documental, composta de 213 artigos em que descrevo e analiso as informações, descrições e representações da mulher mãe, caracterizada pelos especialistas, que escreviam a elas, nessas seções específicas destinadas às mães, para o exercício da maternidade. A pesquisa revela que os especialistas que mais frequentemente se dedicaram na editoração de textos e mensagens às mulheres mães foram os médicos. Estes exercem uma verdadeira educação da mulher, visando a função da maternidade; adentram na história familiar, interferindo e fundamentando normas para a rotina da criação do filho, criando uma atmosfera de culpa, diluição da autonomia parental, através da imposição do poder do conhecimento especializado, descredenciando as tradições familiares e locais; desacreditando os pais, os parentes e procurando tomar os espaços dos leigos e das orientações das pessoas mais velhas e próximas da família. The magazines that keep articles in this period itemized are three: Fon Fon, Vamos Ler e a Cigarra. The Work is divided in two blocks of different sessions. The first block pictures the first part of the work highlights the theoretical input about the research from the magazines Fon Fon, Vamos Ler e a Cigarra between 1940 and 1950.
