Cognitive and brain function in adults with Type 1 diabetes mellitus : is there evidence of accelerated ageing?

The physical complications of Type 1 diabetes mellitus (T1DM) have been understood as an accelerated ageing process (Morley, 2008). Do people with T1DM also experience accelerated cognitive and brain ageing? Using findings from research of the normal cognitive and brain ageing process and conceptualized in theories of the functional brain changes in cognitive ageing, a combination of cognitive testing and functional magnetic resonance imaging (fMRI) techniques were used to evaluate evidence of accelerated cognitive and brain ageing in middle-aged adults with T1DM. The first part of this thesis comprises a cognitive study of 94 adults (≥ 45 years of age) with long duration (≥ 10 years) of T1DM. Participants completed cognitive assessment and questionnaires on general mood and feelings about living with diabetes. Findings highlighted the importance of microvascular disease (specifically retinopathy) as an independent predictor of cognitive function. The incidence and predictors of mild cognitive impairment (MCI) were then explored. Results indicate a higher percentage of the group met criteria for MCI than expected based on incidence rates in the general population, providing initial evidence of accelerated cognitive ageing. Psychological factors were explored next. The relationship between the measures of well-being, diabetes health, and cognitive function highlighted the need for attention to patient's psychological well-being in diabetes care. Finally, a subgroup of 30 participants between the ages of 45 and 65 who differed on severity of retinopathy were selected to take part in an fMRI study. Blood oxygen level dependent (BOLD) activity was evaluated while participants were engaged in cognitive tasks and during rest. The findings provided evidence that the pattern of BOLD activation and functional connectivity for those with high severity of retinopathy are similar to patterns found in adults over the age of 65. In line with the theories of cognitive ageing, functional brain changes appear to maintain a level of cognitive function. Evidence of accelerated brain ageing in this primarily middle-aged group, emphasizes the importance of treatments and regimens to prevent or minimize microvascular complications.

自閉症類疾患兒童自閉症狀變化與認知及適應行為關係之縱貫研究 / A Longitudinal Study of Autistic Symptom Severity and its Relation with Cognitive and Adaptive Functions in Children with Autism Spectrum Disorders

研究目的:欲瞭解自閉症類疾患兒童,橫跨幼兒期、學齡前期至學齡期的自閉症狀發展軌跡,探究當中是否存有不同的發展軌跡,並檢視不同發展軌跡的群體在認知功能與適應行為隨時間的變化型態是否有差異,以及不同群體在早期的社會溝通能力或療育經驗上是否有差異。 方法:本研究有37名自閉症類與24位非自閉症類受試者納入分析,共測量三次:時間點一平均生理年齡為2歲半、時間點二為4歲、時間點三為8歲半。於三個時間點皆以自閉症診斷觀察量表評估自閉症狀嚴重度、以及穆林發展量表或魏氏兒童智力量表第四版評估認知功能,在時間點二與時間點三以文蘭適應行為量表第二版評估適應行為;另外,在時間點一以台灣版兩歲期自閉症篩檢工具評估社會溝通能力,以及在時間點一至時間點二之間,以電話訪問的方式紀錄兒童每週參與療育的時數。 結果:由階層式集群分析結果顯示,社交情感嚴重度校正分數可將自閉症類與非自閉症類兒童分群,並於自閉症類兒童中可再分為維持高嚴重度組與退步組。兩組自閉症類兒童於學齡時期在社交情感症狀皆呈現嚴重度明顯上升的趨勢,在認知功能與適應行為隨時間的變化型態相似,不過維持高嚴重度組在認知功能與適應行為有較低的分數(但未達統計顯著)。此外,兩組自閉症類兒童於時間點一的意圖溝通能力即有差異,不過僅共享式注意力可預測日後社交情感症狀發展軌跡之分群。 總結:由結果可見自閉症狀維持高嚴重度組,於認知功能與適應行為有較明顯缺損,推論自閉症類兒童的自閉症狀嚴重度與認知功能及適應行為並非完全獨立的變項。兩組自閉症類兒童於學齡期症狀嚴重度上升,而此趨勢在退步組兒童更為明顯。雖然本研究並未發現早期療育經驗對於社交情感症狀發展的影響,不過共享式注意力對於日後社交情感症狀有預測力。進一步討論此結果在理論與臨床實務上的應用。 / Purposes: the purpose of the study was to plot longitudinal developmental trajectories of autism symptom severity in the children with autism spectrum disorders (ASDs) from toddler age to school age. And to examine whether these different trajectories are associated with the cognitive function, adaptive behavior, early social communication skill, and early intervention experience. Methods: Thirty-seven children with ASDs and 24 children with developmental delay participated at time 1 (mean chronological age was 2.5 years old), and then followed at time 2 (mean chronological age was 4 years old) and time 3 (mean chronological age was 8.5 years old). The study assessed the autism symptom severity by ADOS and cognitive function by MSEL or WISC-Ⅳ at three time points; adaptive behavior by VABS-II at time 2 and time 3; and early social communication skills by T-STAT at time 1 and also used telephone interviews to record participants’ weekly intervention hours during time 1 to time 2. Results: In hierarchical cluster analysis, social affect calibrated severity score (CSS) could discriminate ASDs and Non-ASDs groups, also could divided ASDs into two groups, called persistent high and worsening groups. The social affect symptom severity increased at school age in both groups. On the other hand, the change of cognitive function and adaptive behavior with time showed similar pattern in the two groups. However, participants in the persistent high group have relatively lower scores of cognitive function and adaptive behavior, although did not rearch statistically significant. Furthermore, two ASDs groups at toddler showed difference in intention communication skills; however, only joint attention could predict social affect trajectory subgrouping. Conclusions: In summery, the study showed that persistent high group has more cognitive and adaptive function impairments than the worsening group in the children with ASDs. It seemed that the autistic symptom are not entirely independent with cognitive function and adaptive behavior. Moreover, the symptom severity increased at school age in both the persistent high and the worsening groups, and this trend was more significant in the worsening group. Additionally, the experience of early intervention showed no significant effects on developmental trajectories of social affect symptom; however, the joint attention was found to be an index to predict the development of social affect symptom in ASDs groups. The theoretical and clinical implications were discussed.

The diploma thesis deals with the impact of dementia syndrome in clients of social field service on workers in direct care. The thesis connects two theoretical levels, which are closely related to the topic. This is primarily the level of the disease itself with dementia syndrome and the circumstances associated with caring for a sick person. The second level deals with the topic of managing a team of workers in direct care, with a focus on field social services. The research in the sequential variant made it possible to emphasize the moments that are specific in the care of clients with dementia syndrome in the home environment. The intention was to name the stress moments that come with working with a given target group in the home environment. At the core of the pressures mentioned by the respondents, the stress resulting from the unpredictability of the situation was found as a common denominator. It is unpredictability, given the client's illness, in the conditions of the home environment, in which the client is used to making his own decisions, despite the worsened cognition. At the next level is the stress resulting from unclear responsibility for the content of the service. Muscle Strength, Acute Resistance Exercise, and the Mechanisms Involved in Facilitating Executive Function and Memory

<p dir="ltr">Past research has extensively explored the benefits of acute aerobic exercise (AE) on memory and executive functions. Additionally, the cross-sectional relationship between muscle strength – a direct outcome of RE – and cognition is unknown, despite the simultaneous onset of muscle and cognitive decline in one’s thirties. However, the effects of acute resistance exercise (RE) on cognition remain understudied, despite the growing popularity of RE and evidence that RE may have distinct effects on cognition.. Therefore, the present study aimed to broaden our understanding of the connection between muscle strength and hippocampal-dependent memory and to investigate the influence of RE on memory and executive function.</p><p dir="ltr">A sample of 125 healthy young adults (18-50 years old) completed this study. On the first day of testing, subjects completed a cognitive battery testing aspects of hippocampal dependent memory, spatial abilities, and working memory, a maximal muscle strength testing session including handgrip strength and one-rep-max testing, and maximal aerobic capacity testing. Subjects completed a bioelectrical impedance assessment (BIA) body scan to measure body composition on Day 2. Day 3 consisted of a randomized controlled trial (RCT), where subjects completed either 42 minute moderate intensity RE (n = 62) or a seated rest (n = 61). Cognitive testing including a memory recognition task, an inhibitory control task, and a working memory task were performed both before and after the intervention. Subjects also completed lactate, blood pressure, and blood draw (only a subset of subjects (n = 59)) before and after intervention.</p><p dir="ltr">The results first revealed that after controlling for known covariates, those with greater handgrip strength performed better on mental rotation tasks (t = 2.14, p = 0.04, Δr2= 0.04), while those with higher upper-body relative strength did better on recognition (t = 2.78, p = 0.01, Δr2 = 0.06) and pattern separation (t = 2.03, p = 0.04, Δr2= 0.04) tasks. Further, while there was no acute effect of RE on memory performance, response times during measures of inhibitory control (t = 4.15, p < 0.01, d = 0.40) and working memory decreased after exercise (t = 7.01, p < 0.01, d = 0.46), along with decreases in P3 latency during the inhibitory control task (t =-5.99, p < 0.01, d = 0.58). Additionally, blood lactate (t =-17.18, p < 0.01, d = 2.06), serum brain derived neurotropic factor (BDNF) (t = -4.17, p < 0.01, d = 0.66), and systolic blood pressure (t = -10.58, p < 0.01, d = 0.99) all increased following RE, while diastolic blood pressure (t = 4.90, p < 0.01,d = 0.50) decreased. Notably, the change in systolic blood pressure (t = -2.83, p = 0.01, Δr2 = 0.06) was associated with improvements in behavioral measures of inhibitory control, changes in lactate (t = -2.26, p = 0.03, Δr2 = 0.04) and systolic blood pressure (t = -3.30, p < 0.01, Δr2 = 0.08) were also related to improved behavioral changes in working memory, and changes in lactate (t = -3.31, p < 0.01, Δr2= 0.08) and BDNF (t = -2.12, p = 0.04, Δr2= 0.08) related to faster P3 latency during inhibitory control. Importantly, these associations between physiological and cognitive changes were consistent across both exercise and rest groups, suggesting that physiological changes were linked to improved cognitive performance regardless of group assignment.</p><p dir="ltr">In conclusion, this study highlights the positive relationships between cross-sectional muscle strength and aspects of memory and spatial abilities, with distinct contributions from handgrip and upper body strength. Furthermore, acute RE was shown to enhance executive functions, particularly in terms of processing speed during inhibitory control (response time and P3 latency) and working memory (response time). This study suggests that RE can be a valid way to garner exercise-induced benefits on executive functions potentially through its influence on lactate, BDNF, and blood pressure, however, since these effects were evident regardless of intervention, more work is needed to determine if RE-induced changes have the same mechanisms. Overall, these findings underscore the potential benefits of muscle strength and RE on enhancing executive function in young and middle-aged adults.</p>

<p>This study is focused on the design, production, and operation of a controlled environmental olfaction chamber to evaluate human physiological and emotional response to volatile chemical emissions (VCPs) from scented household products in addition to careful characterization of the volatile organic compounds (VOCs) present in these product emissions. Utilizing proton transfer reaction time-of-flight mass spectrometry, the chamber can collect VCP emissions and identify VOCs present to complete an accurate chemical profile of household and common product emissions not previously known. This instrument is one of the first of its kind and will serve as a key element in understanding the relationship between human physical and cognitive health and the built environment.</p>
