Bakgrund: Övervikt och fetma är ett växande hälsoproblem som medför ökad risk för följdsjukdomar och kan bidra till konsekvenser för reproduktiv hälsa. Svårigheter med att bli gravid och upprepade missfall kan leda till att kvinnor blir ofrivilligt barnlösa. Fler kvinnor söker sig till bariatriska enheter med förhoppning om att viktnedgång kan leda till föräldraskap. Tidigare forskning visar att det saknas kunskap och information om graviditet efter bariatrisk operation. Syfte: Att belysa kvinnors erfarenheter av graviditet efter bariatrisk operation. Metod: Kvalitativ undersökning med semistrukturerade intervjuer med tio kvinnor. Data har bearbetats med hjälp av kvalitativ innehållsanalys med induktiv ansats. Resultat: I resultatet framkom tre kategorier och sju subkategorier. Sammanfattningsvis beskrev kvinnorna ett behov av mer information och rekommendationer i samband med graviditet efter bariatrisk operation. Att barnmorskor har kunskap om eller vilja att ta reda på information om graviditet efter bariatrisk operation för att bemöta kvinnorna och frågor är en viktig del för goda relationer. Konklusion: Befintlig kunskap och rekommendationer i samband med graviditet efter bariatrisk operation behöver stärkas. Barnmorskor behöver mer utbildning inom området för att kunna bemöta ett växande behov. Det är önskvärt att ett fungerande samarbete mellan barnmorskor och bariatriska enheter initieras. / Background: Overweight and obesity is a growing health problem that entails an increased risk of sequelae and can contribute to consequences for reproductive health. Having difficulties getting pregnant and repeated miscarriages can lead to women becoming involuntarily childless. More women are applying to bariatric units with the hope of weight reduction leading to parenthood. Previous research has shown that there is a lack of knowledge and information about pregnancy after bariatric surgery. Purpose: To shed light on women's experiences of pregnancy after bariatric surgery. Method: Qualitative survey with semi-structured interviews with 10 women. The data has been processed using qualitative content analysis with an inductive approach. Results: The results revealed three categories and seven subcategories. In summary, the women described a need of more information and recommendations concerning pregnancy after bariatric surgery. If midwives have knowledge of or an ambition to find information about pregnancy after bariatric surgery to meet women’s needs and questions is an important part for good relations. Conclusion: Existing knowledge and recommendations concerning pregnancy after bariatric surgery need to be strengthened. Midwives need more education to be able to meet a growing need. It is desirable that a functioning collaboration between midwives and bariatric units is initiated.

Experiences of Colorado Parents as They Recognized Their Child's Mental Illness

Mental illness is not only the leading cause of disability among adults, but there is also an emerging public health crisis in childhood mental illness. A majority of parents do not recognize symptoms of psychological disorder in their children, and current policies and programs for mental health service delivery are not sufficiently responsive to the early help-seeking dynamics of families. Using a concurrent mixed methods design, this study explored how parents in the Pikes Peak region of Colorado learned to recognize their child's mental illness. Phenomenological interviews, augmented by poetic inquiry and quantitative measurements, were used to discover factors that inhibited or enhanced five mothers' recognitions. These factors were then evaluated using a frequency distribution analysis and a rank-order correlation. The phenomenon of recognition was, for these mothers, a process of waiting to hear that 'normal' had stopped, wherein they miscategorized symptoms as normal behaviors in a passing developmental phase. Prior experience with mental illness appeared to significantly decrease both the length of time and the level of distress necessary for recognition. Ultimately, recognition did not occur until someone in their social network validated their concerns and provided explicit confirmation, which galvanized them to seek treatment. Governance network collaborations can facilitate positive social change by standardizing guidance on how to differentiate symptoms of a disorder from normal childhood development. Public policies and programs such as universal mental health screening, mental health literacy, and more supportive and responsive school policies can foster dialogue for parental recognition in Colorado and throughout the country.

À cause des stratégies de production et de marketing de plus en plus ambitieuses tellesque le Juste-À-Temps et la production adaptée au client, les approches de collaboration logistique verticale qui sont courantes atteignent une limite d'efficacité notamment en transport. La collaboration logistique horizontale (CLH) et plus particulièrement la mutualisation, dont l'efficacité a été prouvée dans la littérature et dans les cas réels, a attiré l'attention des chercheurs ainsi que des praticiens. Cependatn, un des obstacles principaux à la mise en œuvre des CLHs est l'absence d'un mécanisme de collaboration raisonné, en particulier un mécanisme de partage des gains. Nous identifions deux formes d'organisation des CLHs : centralisée limité en pratique à de petites coalitions et décentralisée pouvant comprendre de nombreux participants. Pour des CLHs centralisées, nous proposons un modèle de collaboration qui est un processus de conduite qui intègre les outils d'aide à la décision. Nous développons également un mécanisme de partage par la théorie des jeux. Ce mécanisme est applicable aux différentes catégories des CLHs centralisées, qui peuvent être modélisées par des jeux coopératifs super-additif et non-super-additifs. Afin de proposer un plan de partage crédible aux collaborateurs, ce mécanisme de partage prend en compte la contribution de chacun des collaborateurs, la stabilité de la coalition et leur pouvoir de négociation. Ce cadre est illustré par des exemples numériques issus de cas logistiques. Pour la mise en œuvre des CLHs décentralisées, nous proposons un cadre de travail de logistique collaborative qui est ouvert aux participants potentiels, et avons conçu des protocoles fondés sur le mécanisme d'enchère combinatoire, qui spécifient l'allocation de demande de livraison et la détermination de paiement pour faciliter les collaborations. Cette dernière partie s'appuie sur la théorie dite de Mechanism design. / As the result of the more and more ambitious production and marketing strategies,such as Just-In-Time and increasing customization of products, the current vertical logistics collaboration approaches based on single supply chain seems insufficient to achieve further improvements in transportation efficiency. The horizontal logistics collaboration (HLC), which has been proved an effective approach to efficiency improvement, has attracted both academics and practitioners. One of the main barriers to the implementation of HLCs is the lack of feasible collaboration mechanism, in particular the gain sharing mechanism. We identify two organizational forms of HLCs: the centralized and decentralized ones. For centralized HLCs, we propose a collaboration model that is a collaboration conducting process integrating decision-aiding tools to guide the implementation of the collaboration. We also develop a generally applicable game-theoretic sharing mechanism for different categories of centralized HLCs modeled as super-additive and non-super-additive cooperative games. This sharing mechanism takes into account the collaborators' contribution, the coalition stability, and the bargaining power to propose a credible sharing scheme for collaborators. The approach is illustrated by numerical example taken from logistics cases. For the implementation of the decentralized HLCs, we propose an open collaborative logistics framework, and design the system protocols as the collaboration mechanism that specifies the combinatorial-auction-based request allocation and payment determination to foster the collaborations.

Dialogue, collaboration et transmission du savoir entre intellectuels juifs et chrétiens : la France des XIIIe-XIVe siècles

Cette thèse s’intéresse aux collaborations et au partage du savoir entre intellectuels juifs et chrétiens, en France, durant les XIIIe et XIVe siècles et propose une analyse comparative des différents échanges, dans trois domaines distincts : théologie, philosophie et astronomie. En prenant en compte les sources latines et hébraïques qui témoignent de cette transmission du savoir, nous proposons une étude approfondie divisée en deux parties. La première s’intéresse à l’évolution de l’enseignement dans les communautés juives et dans la société chrétienne. La seconde analyse le contexte de rédaction des Extractiones de Talmut, la transmission du savoir entre Maïmonide et Thomas d’Aquin, la collaboration étroite entre Jacob ben Makhir et Armengaud Blaise, ainsi que l’échange intellectuel significatif entre Gersonide et ses confrères chrétiens. Notre objectif est de répondre aux questions suivantes : les savants chrétiens et juifs recevaient-ils l’information selon leur propre valeur intellectuelle, ne tenant pas compte de leur provenance ? Et existait-il une influence directe de l’un et de l’autre ? Cette étude tente ainsi de montrer les différents motifs de ces échanges à travers un champ contextuel constitué par un événement intellectuel précis. Nous verrons ainsi que ces relations vacillent entre méfiance et admiration. / This thesis sheds lights on collaborations and transfer of knowledge between Jewish and Christian scholars in France during the 13th and 14th centuries. We propose a comparative analysis of different exchanges, in three distinct areas: theological, philosophical and astronomical. Taking into account the Latin and Hebrew sources that testify this transmission of knowledge, we propose an in-depth study, dividing in two sections. The first part narrates the evolution of education in the Jewish communities and in Christian society. The second part analyses the context of the Extractiones de Talmut, the transmission of knowledge between Maimonides and Thomas Aquinas, the close collaboration between Jacob ben Makhir and Armengaud Blaise, as well as the significant intellectual exchange between Gersonides and his fellow Christians. Our objective is to answer the following questions: did Christian and Jewish scholars receive information according to their own intellectual value, ignoring their source? And was there a direct influence from one scholar towards the other? Thus, this study demonstrates the different motives of these exchanges through a contextual field constituted by a specific intellectual event. We will perceive that these relations vacillate between mistrust and admiration.

Responsible leadership in Collaborative governance for systemic sustainable development : A study of leaders' reflections on their internal competencies

Globally and nationally in Sweden, we see an increased awareness of the interconnectedness of sustainability challenges and an understanding that no single actor or sector possesses the resources, knowledge or creativity needed to develop and implement holistic solutions to bring about disruptive transformations of the currently unsustainable system. This has led to an increased demand for collaborative governance and values-based responsible leadership. Despite this growing interest, we identify a gap in research regarding what leadership approach and competencies are needed to harness the potential of collaboration. The purpose of this study is to understand individual leaders’ perspectives on responsible leadership in collaborative governance aimed at systemic sustainable development. By gathering qualitative data through semi-structured interviews with 9 leaders active in collaborative governance in Sweden we contribute with research on internal competencies in leadership. We analyze the individual leaders’ reflections in relation to three concepts from responsible leadership: ethics and values, self-awareness and stakeholder relations. Through this study we find that responsible leadership provides a suitable holistic approach to address the complex challenges of collaborative governance aimed at systemic sustainable development. In addition, we find that the three internal competencies in focus for this study are deeply interconnected and create synergies when applied in collaborative governance. Lastly, we identify that leaders’ self-awareness reflects an interconnectedness between the leaders’ perception of their own inner sustainability and the sustainability of the outer world consisting of stakeholder relations and the societal system they strive to change through their leadership. Through our findings we identify a need for further qualitative research applying an integrated approach to responsible leadership studies through which the synergies and interconnections of competencies can be better understood.
