It was verified that dentigerous cyst are more frequently present in the earfy three decades of life, with the majority of cases occuring in the second decade (40,740/0 ), and also male (57,41%) and white patients (68,52%) were most affected. In relation to anatomic site, the dentigerous cyst was more prevalent at anterior maxila and posterior mandible, showing, usually, a slow growth pattem. The majority of lesions were asymptomatic and the radiographic observed was frequently na unilocular radiolucency. In regard to the histomorphological analysis, it was noticed that the lesions showed commonly a thin epithelium, with a capsule of fibrous connective tissue, richly vascularized and collagenized with an intense mononuclear inflammatory infiltrate. Finally, clinicopathological was performed and it was find out that cysts that showed a thick epithelium, with a high degree of vascularization and collagenization, intense inflammatory infiltrate in the cystic capsule, were located in the pre-molares region, in patients under 12 years old and the majority showing painfull sintomatology, properly, compatible with inflammatory dentigerous cyst. The findings of the present study indicate that, probably, there is a variant of the dentigerous cyst, and therefore, we suggest the denomination inflammatory follicular cyst for this entity / Este estudo teve como objetivo realizar uma analise clinicopatologica de casos diagnosticados como cisto dentigeno na Disciplina de Patologia Oral do Departamento de Odontologia da UFRN,enfatizando uma possivel correla??o entre os achados histomorfologicos e os dados epidemiologicos constantes nas fichas de solicita??o de exame histopatologico,no intuito de caracterizar uma possivel variante da les?o,denominada cisto dentigero de origem inflamatoria.Foi verificado que o cisto dentigero apresentou-se com maior frequencia nas tres primeiras decadas de vida,com maior numero de casos observados na segunda decada(40,74%);tendo sido o genero masculino o mais afetado,com 57,41% dos casos,e os pacientes leucodermas os mais acometidos com 68,52% da amostra total.A regiao anatomica mais atingida foi a anterior de maxila e a posterior de mandibula, sendo o padrao de crescimento lento o mais comumente observado.A maioia das lesoes apresentou-se assintomatica,e o aspecto radiografico frequentemente identificado foi o radiolucido inilocular.Quanto a analise histomorfologica,observou-se que as lesoes

The epithelial proliferation, which has been identified as one of the key processes in the growth of odontogenic cystic lesions, is influenced by growth factors such as EGFR (epidermal growth receptor factor) and podoplanin (PDPN), many of which may have its production stimulated mainly during inflammatory processes. The objective of this research was to evaluate and compare the immunohistochemical expression of EGFR and PDPN in 30 cases of RCs and 30 cases of DCs, semiquantitatively, in light microscopy, associating it with the degree of inflammation, cellular localization of immunostaining and with the immunostained epithelial layers. Data were statistically analyzed by Chi-square test and Fisher exact test, considering a significance level of 5 %. The results showed high immunoreactivity of both proteins in the lesions studied, only statistically significant difference was observed in immunostaining of PDPN (p=0.033), which proved higher in RCs. The other analyzed parameters showed no relevant significant differences. We conclude that, as EGFR and PDPN showed high immunoreactivity in cystic lesions analyzed, these proteins participate the pathogenesis of these lesions through the epithelial stimulation process, despite having different etiologies. Furthermore, it can infer that the higher immunostaining of PDNP in RCs that DCs showed no distinction indicator between the two lesions, regarding their etiologies, once this protein also showed a considerable expression in DCs, independent of the intensity of the inflammatory infiltrate / Os cistos radiculares (CRs) e dent?geros (CDs), apesar de possu?rem etiologias diferentes, formam uma cavidade patol?gica revestida por epit?lio, a qual cresce em fun??o do ac?mulo de l?quido em seu interior, ? medida que o osso ao redor ? reabsorvido e o epit?lio vai sendo induzido a proliferar. O objetivo desta pesquisa foi avaliar e comparar a express?o imunoistoqu?mica do EGFR e da PDPN em 30 casos de CRs e 30 casos de CDs, de forma semiquantitativa, em microscopia de luz, associando-a com o grau de inflama??o, localiza??o celular da imunocolora??o e com as camadas epiteliais imunomarcadas. Os dados foram avaliados estatisticamente por meio de testes do Qui-quadrado e Exato de Fisher, considerando-se um n?vel de signific?ncia de 5%. Os resultados mostraram que houve elevada imunorreatividade das duas prote?nas nas les?es estudadas, sendo observada apenas diferen?a estat?stica significativa na imunoexpress?o da PDPN (p=0,033), que se mostrou mais elevada nos CRs. Os demais par?metros analisados n?o demonstraram diferen?as significativas relevantes. Conclui-se que, como o EGFR e a PDPN apresentaram elevada imunoexpress?o nas les?es c?sticas analisadas, essas prote?nas participam da patog?nese dessas les?es atrav?s da estimula??o epitelial, apesar de apresentarem etiologias diferentes. Estudo comparativo entre radiografia panorâmica e tomografia computadorizada por feixe cônico no diagnóstico de ameloblastoma, tumor odontogênico queratocístico e cisto dentígero / Comparative study between panoramic radiography and cone beam-computed tomography in the diagnosis of ameloblastoma, odontogenic keratocyst tumor, and dentigerous cyst

Entre as lesões odontogênicas não mineralizadas, o ameloblastoma, o tumor odontogênico queratocístico e o cisto dentígero podem apresentar-se indistinguível na imagem radiográfica. A dificuldade na interpretação da imagem e elaboração do diagnóstico destas lesões é tema abordado em vários estudos que utilizaram exames de imagem avançados, como ressonância magnética e tomografia computadorizada, para definir como podem colaborar na diferenciação, na avaliação da extensão e no planejamento cirúrgico das mesmas. Com a recente introdução da Tomografia Computadorizada de Feixe Cônico (TCFC) como recurso auxiliar de diagnóstico, o presente trabalho foi elaborado tendo em vista descobrir como o diagnóstico pode ou não ser influenciado por essa nova tecnologia por meio da comparação das hipóteses de diagnóstico obtidas em imagens das radiografias panorâmicas e em TCFC, em casos de ameloblastoma, tumor odontogênico queratocístico e cisto dentígero; e também definir as características observadas nas imagens de TCFC que podem auxiliar no diagnóstico diferencial. Foram selecionados 5 casos de cada lesão. As radiografias panorâmicas e os exames de TCFC foram analisados por 15 cirurgiões-dentistas - 4 especialistas em Radiologia, 4 Mestres e 7 Doutores em Estomatologia e Radiologia - para a formulação das hipóteses de diagnóstico. Na primeira etapa foram analisadas as radiografias panorâmicas e na segunda, após um intervalo de pelo menos 30 dias, os exames de TCFC. Dois examinadores, capacitados e calibrados, fizeram a análise e mensuração das características qualitativas e quantitativas das lesões observadas nos exames de TCFC, utilizando o programa i-CAT® Vision. Os resultados demonstraram que não houve diferença estatisticamente significante na média total dos acertos de diagnóstico entre radiografia panorâmica e TCFC, porém houve diferença significante na média de acerto no diagnóstico de ameloblastoma com o uso de TCFC (77,33% ± 21,20%) em comparação à radiografia panorâmica (61,33% ± 23,26%). Os mestres (85,00% ± 19,15%) e doutores (65,75% ± 9,76%) obtiveram uma média de acerto maior no diagnóstico de tumor odontogênico queratocístico com o uso da radiografia panorâmica em relação aos especialistas (35,00% ± 19,15%), com diferença significante. A análise das características quantitativas observadas nas imagens de TCFC revelou que o ameloblastoma mostrou-se maior em tamanho e em expansão comparado ao tumor odontogênico queratocístico e ao cisto dentígero, com diferença estatisticamente significante. De todas as características qualitativas avaliadas, apenas o número de lóculos da lesão (multilocular ou unilocular) mostrou diferença significante, o ameloblastoma apresentou uma maior incidência do aspecto multiloculado comparado ao tumor odontogênico queratocístico e ao cisto dentígero. Não houve diferenças significantes entre as características quantitativas e qualitativas do tumor odontogênico queratocístico e do cisto dentígero. / Among the non-mineralized odontogenic lesions, ameloblastomas, odontogenic keratocystic tumors and dentigerous cysts may present as radiographically indistinguishable. The difficulty in radiographic diagnosis of these lesions is the subject addressed in several studies using advanced imaging, such as magnetic resonance imaging and computed tomography, to define how these technologies can help in differentiating among these lesions, in assessing the extent and surgical planning. Cone-beam computed tomography (CBCT) is a recent diagnostic aid resource and this study was prepared in order to determine how the diagnosis may or may not be influenced by this new technology, comparing the diagnostic hypotheses obtained using images of panoramic radiographs and CBCT in cases of ameloblastoma, odontogenic keratocystic tumor, and dentigerous cyst - and to define the features observed in CBCT images that can help the differential diagnosis. Five cases were selected for each lesion. Panoramic radiographs and CBCT scans were analyzed by 15 dentists - 4 specialists in Radiology, 4 masters and 7 PhD in Stomatology and Radiology - for the formulation of the diagnostic hypotheses. In the first stage the panoramic radiographs were analyzed, and in the second, after an interval of at least 30 days, the CBCT images. Two observers, trained and calibrated, performed the analyses and measurement of qualitative and quantitative features of the lesions evaluated in the CBCT images using the i-CAT® Vision program. The results showed that there was no statistically significant difference in correct diagnostic average between panoramic radiography and CBCT, but there was a significant difference in correct diagnostic average in the diagnosis of ameloblastoma using CBCT (77.33% ± 21.20%) compared to panoramic radiography (61.33% ± 23.26%). Masters (85.00% ± 19.15%) and PhD-level observers (65.75% ± 9.76%) had greater correct diagnostic average in the diagnosis of odontogenic keratocystic tumor using panoramic radiograph compared to specialists (35.00% ± 19.15%), with a significant difference. The analysis of quantitative features observed in the CBCT images revealed that the ameloblastomas were greater in size and expansion compared to the odontogenic keratocystic tumors and the dentigerous cysts, with statistically significant difference. Of all the qualitative features evaluated, only the number of loci of the lesion (multilocular or unilocular) showed significant differences; ameloblastomas showed a higher incidence of multiloculated aspects compared to odontogenic keratocystic tumors and dentigerous cysts. There were no significant differences between quantitative and qualitative features of odontogenic keratocystic tumors and dentigerous cysts.

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / A identificaÃÃo da atividade proliferativa em cistos e tumores odontogÃnicos tem sido apontada como um importante ponto na avaliaÃÃo do comportamento biolÃgico entre diferentes lesÃes. O AntÃgeno Nuclear de CÃlulas Proliferantes (PCNA) e o Ki-67 sÃo os marcadores moleculares mais utilizados na detecÃÃo de cÃlulas proliferantes. MutaÃÃes no gene supressor tumoral p53 podem produzir proteÃnas mais estÃveis, porÃm com alteraÃÃes funcionais de fundamental importÃncia relacionadas à induÃÃo da apoptose ou à inibiÃÃo mitÃtica. A maior estabilidade da p53 mutada possibilita sua detecÃÃo imunohistoquÃmica e esta tÃcnica tem sido utilizada para avaliaÃÃo do comportamento biolÃgico de lesÃes neoplÃsicas e cÃsticas. O Cisto DentÃgero (CD), Queratocisto OdontogÃnico (QO) e o Ameloblastoma UnicÃstico (AU) sÃo lesÃes benignas originÃrias de distÃrbios na formaÃÃo do dente. Apesar das semelhanÃas clÃnicas e radiogrÃficas, estas lesÃes possuem um distinto comportamento biolÃgico, sendo o QO e o AU as lesÃes mais agressivas e que podem recidivar quando submetidos a tratamento conservador. O presente trabalho teve por objetivo estudar, por imunohistoquÃmica, a expressÃo das proteÃnas PCNA, Ki-67 e p53 mutada em quinze QOs, dez CDs e cinco AUs. A mÃdia da percentagem de cÃlulas PCNA+ e Ki-67+ documentadas no forro epitelial dos Queratocistos (46,4 e 13,7%) foi significativamente maior que a dos Cistos DentÃgeros (26,2 e 7,9%). Os AUs apresentaram mÃdia de marcaÃÃo para o PCNA (35,9%) estatisticamente semelhante a observada nos QOs (P=0.136), mas, com relaÃÃo ao Ki-67 (24,4%), os AUs tiveram maior Ãndice de marcaÃÃo. Apesar de ambos marcadores serem utilizados na detecÃÃo de proliferaÃÃo celular foi observada uma maior mÃdia de cÃlulas marcadas para o PCNA em todas amostras. Trinta e trÃs por cento dos casos de Queratocistos apresentaram positividade para a p53. Apenas um caso de Cisto dentÃgero foi positivo. NÃo houve detecÃÃo em nenhum caso de Ameloblastoma UnicÃstico. Apesar da detecÃÃo da proteÃna p53 por imunohistoquÃmica ser considerada a variante mutada, o carÃter clÃnico benigno destas lesÃes sugere uma maior estabilidade desta proteÃna na sua funÃÃo de reparo / The identification of the proliferative activity in cysts and tumors has been pointed as an important aspect in the avaliation of the biologica behavior among different lesions. The proliferating cells nuclear antigen (PCNA) and the Ki-67 are the most used molecular markers in detection of proliferating cells. Mutations in p53 tumor suppressor gene can produce more stable protein, however with loss of function. The greater stability mutation p53 allows its immunohistochemical detection and this technique has been used in avaliation of biological behavior of neoplasic and cystic lesions. The dentigerous cysts (CD), odontogenic keratocyst (QO) and the unicystic ameloblastoma (AU) are benign lesions that have its origin in disturbs in the tooth formation. Despite the clinical and radiografic similarity, these lesions have a distinct biological behavior. Betwen then, QO and AU are the most aggressive lesions and can recur when submitted to conservative treatment. The present work has the objective of studing, through immunohistochemical, the expression of PCNA, Ki-67 and p53 in 15 QOs, 10 CDs and 5 AUs. The average of the percentage of PCNA+ and Ki 67+ cells countained in QOs (46,4 and 13, 7%) was significanting higher than the CD (26,2 and 7,9%). The AUs presented the labeling index for the PCNA (35,9%) statistically similar to that observed in QOs (P=0136), however, compared to Ki-67 (24,4%), the AUs had a bigger labeling index. Although both markers have been used in detection of cell proliferation, a larger average of marked cells for the PCNA it was observed in all the samples. Thirty three percent of the keratocyst cases presented a positivety for the p53. Only one case of dentigerous cysts was positive. It wasnât any case of unicystic ameloblastoma. Although the detection of the protein p53 for immunohistochemical be considered a mutated variation, the benign clinical behavior of these lesions suggest a larger stability of this protein in itâs repair function

Contribution à l'étude des successions écologiques du phytoplancton dans la lagune de Ghar El Melh / Contribution to the study of ecological successions of the phytoplankton in the lagoon of Ghar El Melh

Il s’agit de l’étude des successions écologiques du phytoplancton dans la lagune de Ghar El Melh au niveau de 5 stations pendant (i) un suivi bimensuel allant du janvier 2011 à janvier 2012 et (ii) un suivi saisonnier à partir du novembre 2012 à mars 2013. L’étude écologique a été consacrée au microphytoplancton en relation avec les facteurs abiotiques (température, salinité et nutriments) et biotiques (microalgues épiphytes, kystes des dinoflagellés, ultraplancton et ciliés). L’analyse hydrologique du milieu a montré une variation principalement saisonnière de la plupart des paramètres avec des valeurs élevées de la température et de la salinité en été et des concentrations exceptionnelles des nutriments en automne et en hiver. Le phytoplancton de la lagune de Ghar El Melh a été principalement dominé par les dinoflagellés et les diatomées avec une manifestation alarmante des espèces nuisibles atteignant plus que 70 % du peuplement. Ces proliférations ont été remarquées durant toute l’année avec une manifestation plus marquante dans les régions internes de la lagune ce qui est principalement tributaire à la température de l’eau, l’azote total, le nitrogène et l’orthophosphate. Une importante contribution des espèces épiphytes au phytoplancton a été signalée notamment celle du dinoflagellé toxique Prorocentrum lima. L’inventaire spécifique du phytoplancton dominé par l’espèce Prorocentrum micans (> 28 %) et d’autres espèces ne produisant pas de kyste a été en concordance avec les faibles valeurs enregistrées pour les kystes de dinoflagellés dans le sédiment où le maximum enregistré a été de l’ordre de 229 kystes g-1. Une part importante de l’ultraplancton a été représentée par le nanophytoplancton et les hétérotrophes procaryotes, dépassant largement les concentrations microphytoplanctoniques, ce qui est un indice de l’eutrophisation du milieu. Des corrélations hautement significatives ont été trouvées entre le microphytoplancton et les organismes microzooplanctoniques évoquant un probable rôle de ces derniers dans le contrôle des proliférations microalguales nuisibles. / We explored the distribution of the microphytoplankton community in relation to both abiotic (temperature, salinity and nutrients) and biotic factors (epiphytes microalgae, dinoflagellate cysts, ultraplankton and ciliates) in Ghar al Melh Lagoon at 5 stations during (i) a biweekly sampling from January 2011 to January 2012 and (ii) a seasonal sampling from november 2012 to march 2013. The results pointed out a seasonal variation of all parameters with high water temperature and salinity in summer and exceptional concentrations of nutrients in autumn and winter. Microphytoplankton was mainly dominated by dinoflagellates and diatoms with sequences of harmful species proliferation attaining more than 70 % of total phytoplankton. These sporadic proliferations occurred throughout the year with exacerbation in the inner parts of the lagoon mainly related to temperature and total nitrogen, nitrogen and orthophosphate. An important contribution of epiphytic species to phytoplankton abundance was recorded with especially the presence of the toxic dinoflagellate Prorocentrum lima. Microphytoplankton was dominated by Prorocentrum micans (> 28 %) and other non-cyst former species consistent with the low dinoflagellate cysts found at sediment surface (maximum = 229 cysts g-1 DS). Urtraplankton was chiefly represented by nanophytoplankton and heterotrophic prokaryotes, with low microphytoplanktonic abundance. This indicates that the lagoon is undergoing eutrophication. Significant correlations were reported between phytoplankton and microzooplankton suggesting a potential role of the former in controlling harmful microalgae proliferations.

Association légionelles-amibes et traitements biocides pour les circuits de refroidissement / Legionella-Amoeba association and chemical treatments for cooling systems

Legionella est une bactérie pathogène, ubiquitaire des environnements aqueux. L'utilisation des eaux de surface, par les centrales nucléaires, provoque un réchauffement de ces eaux, qui associé aux conditions de fonctionnement peut entraîner sa prolifération. Afin de limiter le développement de cette bactérie, en deçà des seuils réglementaires, EDF met en œuvre un traitement à la monochloramine dans ses circuits. Notre étude a eu pour but d’améliorer les connaissances sur les associations entre les légionelles et leur principal hôte : les amibes, et sur les facteurs susceptibles d’influencer leur sensibilité au traitement biocide. Nous avons, tout d’abord, mis en évidence la permissivité de 12 souches d’amibes à 2 souches de L. pneumophila. La température est apparue comme le paramètre le plus influant, la prolifération de Legionella semble favorisée à 40°C. Puis, nous avons cherché à développer un protocole pour séparer Legionella selon son état d’association. Ce protocole n’est pas abouti à l’heure actuelle. Finalement, nous avons comparé la sensibilité, de Legionella, dans différents états physiologiques et étudié l’impact de la qualité de l’eau et des MES. Les cellules post-amibes et dans une moindre mesure, les cellules en phases stationnaires sont les moins sensibles. L’effet de la matrice a été mis en évidence pour l’eau de Vienne, par contre la sensibilité n’a pas été modifiée suite à l’élimination ou l’enrichissement en MES. Cette étude constitue une base de connaissances nouvelles pour orienter les choix de gestion du traitement anti-Legionella à la monochloramine et d’exploitation des circuits. / Legionella is a pathogenic gram negative bacterium, ubiquitously found in waterborne environment. The use of surface water, in open recirculating cooling circuits of nuclear power plants, causes a warming of these waters, which may cause proliferation of Legionella. To limit the development of this bacterium, under the legal thresholds, EDF implements a monochloramine treatment. The aim of our study was to improve the knowledge on the association between Legionella and their main host : amoebae, and on the factors that may influence their susceptibility to biocide treatment. We first showed the permissiveness of 12 strains of amoebae to 2 strains of L. pneumophila. Temperature is appeared as the most influential parameter, the proliferation of Legionella seems favored at 40°C. Then, we searched to develop a protocol to quantify Legionella according to its state of association. This protocol is not operational, yet. Finally, we compared the sensitivity of Legionella, under different physiological states and studied the impact of water quality and suspended matter. Post-amoeba cells and at a lesser extent, stationary cells, are the less sensitive. The effect of the matrix was demonstrated for the Vienne river water, on the other side the elimination or enrichment in suspended matter has not modified the sensitivity. This study provides a basis for a better knowledge to manage monochloramine treatment against Legionella, and for operating cooling systems.

Heteromorph ammonites and dinoflagellate cysts from the Upper Cretaceous Northumberland Formation on Hornby Island, British Columbia, Canada are examined. The collection and preparation of new material has enabled the recognition of eleven species of which only three have been reported from the locality. Of these taxa represented from three heteromorph ammonite families in the study area, five are new occurrences and three are new to science. This expansion of the Hornby Island ammonite fauna is presented alongside a pioneering taxonomic survey of dinoflagellate cysts from the same rocks. Together, these macro- and microfossils reinforce a late Campanian age for the Northumberland Formation with the upper extent of the section approaching the Campanian-Maastrichtian boundary (CMB) interval. The palaeoecology and evolutionary relationships of these heteromorph ammonoids are considered with new insights into their ontogenetic development and neritic palaeoenvironmental circumstances. The dinoflagellate cysts and associated terrestrial palynomorphs have also allowed for enhanced palaeoenvironmental reconstruction and depositional setting inference. The scope of the studied material, and the presence of key index taxa, enables refined biostratigraphy and a stronger basis for correlation of the Hornby Island succession with neighboring coeval biotic provinces. / Graduate / 2018-08-10

Dinoflagellate cysts across the Cretaceous–Paleogene (K/Pg) boundary in the North Pacific; biostratigraphy, diversity, and paleoenvironmental reconstructions

The central objective of this study is to understand phytoplankton community response following the global mass extinction event at the Cretaceous–Paleogene (K/Pg) boundary. The objective is approached through analysis of dinoflagellate cyst assemblages across the boundary interval in the North Pacific. Dinoflagellate cysts are powerful tools for deep time paleoenvironmental reconstructions and this group of microfossils has been vastly underutilized in this region of the world. On this premise, comprehensive marine palynological surveys were undertaken for the Oyster Bay Formation of eastern Vancouver Island, British Columbia, Canada and Shatsky Rise in the northwest Pacific. The Oyster Bay Formation work resulted in the discovery of the first K/Pg boundary succession west of the Rocky Mountains based on biostratigraphic controls and refined taxonomy for the genus Cannosphaeropsis found within these strata. Three new taxa are described: Cannosphaeropsis franciscana subsp. vacuoseptata subsp. nov., Cannosphaeropsis franciscana subsp. vesiculata subsp. nov. and Phelodinium fensomei sp. nov. The cyst assemblages reveal endemic associations and signals of transition between offshore coastal to estuarine settings in keeping with global eustatic trends. Oyster Bay Formation results and interpretations are compared to analyses of core samples from Deep Sea Drilling Project Site 577 at Shatsky Rise. Contrast is seen between a diverse, highly productive coastal to estuarine environment in the Oyster Bay Formation as represented by organic-walled taxa and an oligotrophic bathyal environment at Shatsky Rise during the same interval as represented by a small number of calcareous taxa. These two regions form the basis for comparison between differing assemblages in order to ascertain the extent to which phytoplankton communities were affected by changes in sea-surface and water mass conditions in conjunction with the K/Pg event. The findings reveal measurable impacts of climate and paleoenvironmental change reflected by shifts in assemblage composition and cyst morphology. A lack of extinction among many forms is consistent with studies from around the globe as presented in an exhaustive review of the literature. The dinoflagellates were marginally impacted with the most specialized taxa presenting a record of sea-surface temperature fluctuation, nutrient supply and opportunistic niche exploitation. / Graduate / 2022-08-20

La maladie chronique rénale de la glycogénose de type I, des mécanismes moléculaires aux nouvelles stratégies thérapeutiques / The chronic kidney disease of the glycogen storage disease type I, molecular mecanisms and new therapeutic strategies

La glycogénose de type Ia (GSDIa) est une maladie métabolique rare causée par une déficience en glucose-6-phosphatase (G6Pase), due à des mutations de la sous-unité catalytique (G6PC). Cette enzyme confère au foie, aux reins et à l’intestin la capacité de produire du glucose. Les patients atteints de GSDIa sont donc incapables de produire du glucose et souffrent d’hypoglycémies sévères lors de jeûnes courts. De plus, la déficience en G6Pase provoque une accumulation de glucose-6 phosphate dans le foie et les reins, conduisant à l’accumulation de glycogène et de lipides. A long terme, la plupart des patients souffre d’une maladie chronique rénale (MCR), qui peut évoluer en insuffisance rénale, nécessitant une mise sous dialyse ou une transplantation rénale. Cette MCR se caractérise par une fibrose, ainsi que par le développement de kystes dans les stades tardifs. Au niveau du foie, les patients développent une hépatomégalie et une stéatose hépatique qui peut évoluer vers le développement d’adénomes ou carcinomes hépatocellulaires. Le but de mes travaux de thèse a été d’identifier les mécanismes moléculaires impliqués dans l’établissement de la pathologie rénale et la formation des kystes, à l’aide de modèles murins invalidés pour le gène G6pc spécifiquement dans les reins (souris K.G6pc-/-). Alors que la GSDIa est une maladie caractérisée par l’accumulation hépatique et rénale de glycogène, nous avons d’abord montré que le développement de la fibrose, à l’origine de la perte de la fonction rénale, était induit par l’accumulation de lipides, indépendamment du contenu en glycogène. De plus, l’utilisation d’un agoniste de PPARα, le fénofibrate, en diminuant le contenu lipidique rénal, a ralenti l’installation de la fibrose et l’évolution de la MCR. Le mécanisme moléculaire impliqué est l’activation du système rénine angiotensine par les dérivés lipidiques, qui induit l’expression du facteur profibrotique TGFβ1. De même, le fénofibrate en limitant l’accumulation de lipides hépatiques a prévenu le développement d’atteintes hépatiques caractéristiques de la GSDI. Ainsi, l’activation du catabolisme des lipides par des agonistes de PPARα semble une stratégie thérapeutique intéressante pour réduire la progression des maladies rénales et hépatique de la GSDI. La deuxième partie de mes résultats suggèrent que le développement de kystes rénaux chez les patients atteints de la GSDI pourrait être causé par une altération du cil primaire, organelle jouant un rôle clé dans le maintien d’une structure et fonction normale des reins. En effet, une augmentation de la longueur du cil primaire a pu être observée dans les reins des souris K.G6pc-/- associée à une dérégulation de différentes protéines impliquées dans sa structure et sa fonction, par rapport aux souris contrôles. Nous avons également mis en évidence une reprogrammation métabolique de type Warburg, caractérisée par une activation accrue de la glycolyse aérobie, une inhibition de l’oxydation mitochondriale du pyruvate et une production de lipides. Ainsi, l’ensemble de ces perturbations va favoriser la prolifération cellulaire et le développement de kystes, et pourrait mener au développement de tumeur rénale comme observée chez une souris K.G6pc-/-. En conclusion nous avons démontré que, dans le cadre de la GSDI, l’accumulation de lipides dans les reins et le foie, secondaire à la déficience en G6Pase, joue un rôle clé dans le développement des complications hépatiques et rénales à long terme. Également, la reprogrammation métabolique rénale de type Warburg, prenant place dans le cadre de la GSDI, associée à un défaut du cil primaire pourrait être à l’origine de la formation des kystes et de tumeurs rénales. Ces études, en permettant une meilleure compréhension de la physiopathologie des complications à long terme de la GSDIa, offrent de nouvelles perspectives concernant les stratégies thérapeutiques à développer pour une meilleure prise en charge des patients atteints de GSDIa / Glycogen storage disease type Ia (GSDIa) is a rare metabolic disease caused by glucose-6-phosphatase (G6Pase) deficiency, due to mutations on the gene encoding G6Pase catalytic subunit (G6PC). This enzyme confers to the liver, kidneys and intestine the ability to produce glucose. Thus, patients with GSDIa are unable to ensure endogenous glucose production and suffer from severe hypoglycemia during fasting in the absence of nutritional control. In addition, G6Pase deficiency causes intracellular accumulation of glucose-6 phosphate in the liver and kidneys, leading to metabolic defects and the accumulation of glycogen and lipids. Over time, most adult patients suffer from chronic kidney disease (CKD), which can progress to kidney failure, requiring dialysis or kidney transplantation. This nephropathy is characterized in particular by tubulo-interstitial fibrosis and glomerulosclerosis, as well as by the development of cysts in the late stages. Moreover, patients develop hepatomegaly and hepatic steatosis that may progress to the development of hepatocellular adenomas or carcinomas. The aim of my thesis was to identify the molecular mechanisms involved in the establishment of renal pathology and cyst formation in GSDIa, by using mouse models where G6pc gene is specifically deleted in the kidneys (K.G6pc-/- mice). While GSDIa is a disease characterized by glycogen accumulation in the liver and kidneys, we first showed that the development of fibrosis, which causes progressive loss of kidney function, was induced by intracellular accumulation of lipids, regardless of glycogen content. The molecular mechanism probably involved is the activation of the renin angiotensin system by lipid derivatives such as diacylglycerol, which induced the expression of the profibrotic factor TGFβ1 and an epithelial-mesenchymal transition. In addition, the use of a PPARα agonist, i.e. fenofibrate, by decreasing renal lipid content, reduced the development of fibrosis and CKD evolution. Similarly, fenofibrate treatment prevented the accumulation of lipids in the liver and the development of liver damages that cause tumor development. Thus, the activation of lipid catabolism by PPARα agonists such as fenofibrate seems to be an interesting therapeutic strategy to reduce the progression of renal and hepatic diseases of GSDIa. The second part of my results suggest that the development of renal cysts in GSDI patients may be caused by an alteration of the primary cilia, a non-motile organelle that plays a key role in maintaining normal kidney structure and function. Indeed, defects in the primary cilia are involved in many polycystic kidney diseases. In summary, an increase in the length of the primary cilia was observed in the kidneys of K.G6pc-/- mice, which could be explained by a deregulation of the expression of different proteins involved in cilia structure and function, compared to control mice. We also demonstrated a metabolic reprogramming leading to a Warburg metabolism, characterized by the increased activation of aerobic glycolysis and the inhibition of mitochondrial pyruvate oxidation and lipid production in K.G6pc-/- mice. Thus, all these disorders would promote cell proliferation and cyst development, and could lead to the development of renal tumor, as recently observed in one K.G6pc-/- mouse (out of 36 studied mice). In conclusion, we have shown that, in GSDI, the accumulation of lipids in the kidneys and liver that occurs secondary to G6Pase deficiency plays a key role in the development of hepatic and renal long-term complications. In addition, the Warburg like metabolic reprogramming taking place in the GSDIa kidneys, associated with a defect in the primary cilia, could be at the origin of cysts formation and renal tumors. These new studies, by providing a better understanding of the pathophysiology of long-term complications of GSDIa, offer new perspectives on therapeutic strategies to be developed for better management of patients

Seroprävalenz von Toxoplasma gondii bei deutschen Schlachtputen aus konventioneller Haltung sowie Untersuchungen zum Einfluss von Salz, pH-Wert und Rohwurstreifungsprozessen auf die Infektiosität von Toxoplasma-gondii-Gewebezysten

Die Toxoplasmose gehört zu den bedeutsamsten Zoonosen. Sie wird durch den weltweit verbreiteten Erreger T. gondii ausgelöst und verläuft bei immunkompetenten Menschen meist subklinisch. Dennoch können Rezidivierungen und schwere Krankheitsverläufe auftreten. Besonders gefährdete Personen sind Immunsupprimierte und seronegative Schwangere. T. gondii kann alle warmblütigen Lebewesen infizieren. In der Folge kommt es zur Bildung von Gewebezysten in Muskulatur und Organen. Daten zum Vorkommen und zur Verbreitung des Erregers bei deutschen Schlachtputen liegen nicht vor und sollten im Rahmen dieser Arbeit gewonnen werden. Das hohe Infektionsrisiko bei oraler Aufnahme von rohem oder nicht vollständig durchgegartem zystenhaltigem Fleisch ist bekannt. Exakte Erkenntnisse zur Tenazität der T.-gondii-Gewebezysten gegenüber dem Einfluss von Salz, pH-Wert und Wurstreifungsprozessen liegen jedoch nicht vor. Da sowohl Schweine- als auch Putenfleisch häufig zur Herstellung von Fleischerzeugnissen wie z. B. Rohwürsten und Rohschinken eingesetzt wird, sollte die Persistenz der Gewebezysten unter dem Einfluss verschiedener Salzgehalte und pH-Werte sowie kombiniert im Rahmen der Wurstreifungsprozesse untersucht werden. Ziel ist eine genauere Beurteilung des Infektionsrisikos beim Verzehr kurz gereifter Rohwürste (Mettwurst) und anderer nicht erhitzter Fleischzeugnisse. Untersuchungen: 1. Es wurden 1913 Blutserumproben von Schlachtputen aus 14 Mastbetrieben in fünf Bundesländern mittels kinetischem ELISA auf das Vorkommen von IgG-Antikörpern gegen T. gondii untersucht. 2. In den In-vitro-Studien wurde der Einfluss von NaCl bzw. Nitritpökelsalz (NPS) in wurstrelevanten Konzentrationen (2,0, 2,5 3,0 %) und des pH-Wertes (5, 6, 7) auf die Gewebezysten in Mäusemuskulatur und -hirn untersucht. Die Überprüfung der Infektiosität der exponierten Gewebezysten erfolgte mittels Maus-Bioassay. 3. Putenrohwurstbrät (2,0 % NaCl oder NPS, mit und ohne Starterkultur) wurde mit zystenhaltiger Mäusemuskulatur beimpft. Während der Wurstreifung wurde die Infektiosität der Zysten im Maus-Bioassay untersucht. Ergebnisse:  Die T.-gondii-Gesamtseroprävalenz in deutschen Schlachtputenbeständen betrug 18,4 %. Dieser Wert ist überraschend hoch. Es handelt sich um die ersten in Deutschland durchgeführten Erhebungen. Jahreszeitliche Schwankungen wurden beobachtet, wobei in den Wintermonaten gemästete Putenherden eine höhere Prävalenz zeigten als die im Sommer gehaltenen Tiere.  Gewebezysten besitzen eine gewisse pH-Wert-Toleranz. Sie blieben bei pH-Werten von 5 bis 7 bis zu 26 Tage infektiös.  Gewebezysten sind gegenüber dem Einfluss von Salz empfindlich. Bei NaCl Konzentrationen von 2,0 % blieben sie bis zu acht Tagen infektiös, bei  2,5 % maximal ein Tag.  Gegenüber NPS sind die Zysten offenbar empfindlicher als gegenüber NaCl; sie blieben bei 2,0 % NPS-Konzentration vier Tage infektiös.  Bei Kombination der Effekte von Salz und pH-Wert verkürzt sich die Infektionsfähigkeit der Zysten in der Rohwurstreifung auf ca. 24 Stunden. Schlussfolgerungen: Die unerwartet hohen Seroprävalenzen weisen daraufhin, dass Putenfleisch ein Infektionsrisiko für Toxoplasmose darstellen kann. Weiterführende Untersuchungen zum Vorkommen des Erregers in Schlachtputen sind notwendig. Tendenziell zeigen die Ergebnisse der Tenazitätsuntersuchungen einen raschen Verlust der Infektiosität des Erregers, so dass in lang gereiften Rohwürsten oder in herkömmlich produzierten kurz gereiften Rohwürsten (NPS > 2,5 %) nicht mit einem Vorkommen von infektiösen Zysten zu rechnen ist. Eine Ausnahme könnten allerdings salzreduzierte (NaCl  2,0 %), ohne NPS hergestellte oder sehr kurz gereifte Rohwürste darstellen, hierbei kann ein Infektionsrisiko nicht vollständig ausgeschlossen werden. Zur Beurteilung von Rohschinken sind weitere Untersuchungen notwendig.
