Vliv držení těla na pěvecký projev / Impact of a Posture on the Vocal Performance

The aim of the dissertation thesis is to clarify the impact of a posture on the quality of voice performance. Through the method of a natural experiment, the authors carries out a research about the importance of a postural function in connection to building up basic voice skills. We will observe, both from theoretical and practical point of view if and through which means can we influence the incorrect postural habits and how these changes affect voice performance. The methodological handbook is a natural output which takes into account the relation of the postural function, breath and its development with assistance of vocal hearing as a corrector of the quality of voice performance. The outcomes of the research that helped to verify the new methodological approach approved correctness of suggested methods and their general validity. As a result, a knowledgeable teacher can use the new teaching method at all levels of students'musical development. Västlänken i Göteborg är ett tunnelprojekt för järnväg som skall byggas under centrala Göteborg. Tunneln byggs genom både berg och lera, projektet kommer att använda sig av ett flertal tekniker och konstruktionslösningar. Västlänken är ett av de första stora infrastrukturprojekt i Sverige där slitsmurar används som permanenta konstruktioner. På uppdrag av Trafikverket har två olika typer av stödmurskonstruktioner undersökts med avseende på deras klimatpåverkan. Konstruktionslösningarna är en temporär spont med tillhörande tunnelvägg och en slitsmur. Slitsmuren används både som en temporär konstruktion under byggskedet och en del i den permanenta tunnelväggen. Slitsmurar har först nyligen blivit godkända att användas som delar av permanenta konstruktioner av Trafikverket. Av denna anledning finns det inte mycket information om konstruktionens klimatpåverkan. Syftet med rapporten är att undersöka klimatpåverkan från de två olika konstruktionslösningarna. Jämförelsen har gjorts med hjälp av livscykelanalyser för att få den mest övergripande analysen. En livscykelanalys (LCA) är ett verktyg för att synliggöra en produkts totala miljöpåverkan under dess livstid. Detta åstadkoms genom att alla de olika delprocesser som krävs för att skapa produkten inventeras och analyseras. LCA har utförts med datorprogrammet SimaPro och databasen Ecoinvent. I SimaPro har båda konstruktionslösningarna modellerats och deras miljöpåverkan sedan beräknats med ReCiPe 2016. Indata till LCA har samlats in från ritningar och diskussioner med experter på området. Resultatet från livscykelanalysen visar att slitsmurarna i detta projekt har större klimatpåverkan än konstruktionslösningen med spont och en platsgjuten betongmur. För slitsmuren står armering samt betong för den största delen av klimatpåverkan och för sponten är det den stora mängden stål som krävs vid de kraftiga dimensionerna. Då en del av konstruktionerna inom projektet ej var färdigprojekterad när denna rapport författades rekommenderas ytterligare studier på ämnet för att validera resultaten / The West Link Project is as tunnel project for the railroad that will be constructed below central Gothenburg. The project is built through clay and solid rock thus making use of several techniques and structural solutions. The West Link Project (Västlänken) is the first major infrastructure project in Sweden where diaphragm walls are used as a part of the permanent structure. Two different structures have been examined on behalf of the Swedish Transport Administration, with regards to their climatic impact. The two structures examined are a temporary sheet pile with a cast-in-place concrete wall that is used as a part of the tunnel wall, and a diaphragm wall. The diaphragm wall is used as an earth retaining wall during the construction stage and as a part of the permanent tunnel wall. Diaphragm walls have just recently been approved as parts of permanent structures by the Swedish Transport Administration. Due to this there isn’t a lot of information available on their climatic impact. The purpose of this paper is to examine the climatic impact of these two different structures. The comparison has been performed by the use of a lifecycle analysis to get the most comprehensive analysis. A lifecycle analysis (LCA) is a tool that helps to get a perspective on a product’s total environmental impact over the course of its lifetime. This is accomplished by doing an inventory of all the different processes involved in its production. For the LCA the computer program SimaPro, and the database Ecoinvent were used. In SimaPro both of the structure have been modelled and their environmental impact has been calculated with ReCiPe 2016.Input for the LCA have been gathered from drawings and communication with experts. The result of the LCA shows that in this project the diaphragm walls have a higher climatic impact than the sheet pile and concrete wall. With regards to the diaphragm wall the majority of its climatic impact is from the large amounts of reinforcement and concrete used. For the sheet pile the steel used to manufacture sheets of the dimensions used in the project is the largest contributing factor. Due to the fact that some of the structures in the project are still being at the design stage at the time of writing further studies are recommended to validate the results.

L’activité tonique diaphragmatique chez les enfants ventilés aux soins intensifs pédiatriques

L’ajustement de la pression expiratoire positive (PEP) durant la ventilation est difficile faute de cibles objectives. Cet enjeu est important en pédiatrie, car les nourrissons sont à risque de dérecrutement alvéolaire du fait de la faible rigidité de leur cage thoracique. Pour prévenir cela, ils maintiennent activement leurs volumes pulmonaires en conservant une activité tonique diaphragmatique (Edi tonique) durant l’expiration. La détection d’une Edi tonique élevée est donc un outil potentiel pour titrer la PEP. L’étude présentée dans ce mémoire a permis de proposer une première définition de l’Edi tonique élevée aux soins intensifs pédiatriques, soit > 3,2 mcV chez les 0-1 an et > 1,9 mcV chez les 1-18ans. Cette définition permettra d’aider à identifier au chevet les patients effectuant des efforts de recrutement et ainsi de pouvoir réévaluer le niveau optimal de PEP. Nous avons également montré que des épisodes d’Edi tonique élevée surviennent chez 62% des patients en ventilation non-invasive et 31% des patients intubés. Ces épisodes sont associés avec la bronchiolite, la tachypnée, et, chez les patients en ventilation non-invasive, à une hypoxémie plus sévère. La majorité des patients ayant présenté des épisodes n’avaient pas de contreindications à l’augmentation de la PEP. Ceci porte à croire qu’une stratégie d’ajustement neural continu de la PEP pourrait être une avenue intéressante. Nous complétons donc présentement l’étude NeuroPAP2 visant à évaluer la faisabilité, la tolérance et l’impact du mode NeuroPAP, un mode de ventilation permettant un ajustement continu de la PEP en fonction de l’Edi. / The optimal adjustment of ventilatory settings at the bedside is a challenge and the lack of clear targets makes setting the PEEP (positive end expiratory pressure) especially difficult. This is an important issue in pediatrics, as infants are at increased risk for alveolar derecruitment because of high chest wall compliance. To prevent this, they tend to actively maintain end expiratory lung volumes using tonic diaphragmatic activity (tonic Edi) throughout expiration. In older children, tonic Edi can be reactivated in disease states. Tonic Edi monitoring is thus a potential tool to guide PEEP titration. This thesis addresses important knowledge gaps about tonic Edi in ventilated children. We propose age-specific definitions of elevated tonic Edi in PICU patients: > 3.2 mcV in 0-1 y.o. and > 1.9 mcV in 1-18 y.o. These definitions will aid bedside identification of patient lung recruitment efforts and can prompt reassessment of optimal PEEP. We have also established that episodes of high tonic Edi are frequent in ventilated children (31% of intubated patients and 62% of patients on non-invasive ventilation). Those episodes are associated with bronchiolitis, tachypnea, and, in NIV patients, more severe hypoxemia. The majority of patients with those episodes have no contraindications to increased PEEP. These results make continuous neural adjustment of PEEP an interesting avenue. We are thus now completing this research program with a study to evaluate the feasibility, tolerance and impact of NeuroPAP, a new ventilatory mode allowing continuous neural adjustment of PEEP based on tonic Edi.

System Identification of a Bridge-Type Building Structure

The Bridge House is a steel building structure located in Poly Canyon, a rural area inside the campus of California Polytechnic State University, San Luis Obispo. The Bridge House is a one story steel structure supported on 4 concrete piers with a lateral force resisting system (LFRS) composed of ordinary moment frames in the N-S direction and braced frames in the E-W direction and vertically supported by a pair of trusses. The dynamic response of the Bridge House was investigated by means of system identification through ambient and forced vibration testing. Interesting findings such as diaphragm flexibility, foundation flexibility and frequency shifts due to thermal effects were all found throughout the mode shape mapping process. Nine apparent mode shapes were experimentally identified, N-S and E-W translational, rotational and 6 vertical modes. A computational model was also created and refined through correlation with the modal parameters obtained through FVTs. When compared to the experimental results, the computational model estimated the experimentally determined building period within 8% and 10% for both N-S and E-W translational modes and within 10% for 4 of the vertical modes.

Diagnostika diafragmového výboje ve vodných roztocích a jeho aplikace pro povrchovou úpravu nanomateriálů / Diagnostics of Diaphragm Discharge in Water Solutions and its Application for the Nanomaterials Surface Treatment

The exact mechanism of the discharge in liquids ignition is not sufficiently known up to now. Although during the last years was achieved the great progress and overloading which some of them are written in this theoretical part of thesis. This thesis is divided into two experimental parts. When the first part deals with diagnostics of diaphragm discharge in electrolyte solutions and the second part is focused on its use for uncoiling (higher homogenization) of carbon nanotubes in solutions. In experiment 1, three different sized (4 l, 100 ml, 50 ml) diaphragm discharge configurations were used to diagnose diaphragm discharge in electrolyte solutions. Diagnostics is done through current and voltage waveforms with the addition of synchronized ICCD camera images that have been connected to a four-channel oscilloscope. The V-A characteristic can be described by three events occurring in the electrolyte solution with a gradual increase in voltage. Slowly increasing of the voltage in the solution leads first to electrolysis. The next phase is the formation of microbubbles or bubbles, which is characteristic of the curve by a slight decrease in the increase of the current passing between electrodes. The sudden increase in the current flow is characteristic of the last phase, namely the discharge phase. The distance of the electrodes from the diaphragm does not significantly affect the V-A characteristic. The higher diameter of the pin hole, therefore, has a higher voltage, but this does not affect the origin of bubble generation or breakdown. The higher thickness of diaphragm, the higher voltage is needed to the beginning of the bubbles generation, and consequently the discharge breakdown. Comparison of the voltage of the start generation of the bubbles and breakdown for PET diaphragms and diaphragms from the ceramic there was no mark able difference. One of the most important parameters is the conductivity of the electrolyte solution. The lower voltage is needed for the start generation of the bubbles at the higher solution conductivity, and also the discharge generation is observed at a lower breakdown voltage. The second experimental part is focused on the study of the diaphragm discharge effect on carbon nanotubes. A specially designed U-shaped reactor is used to modify carbon nanoparticles. Tap water and aqueous solutions of organic compounds are used as the electrolytic solutions. The discharge is generated by a non-pulsed DC high source with a voltage in the range of 0-2.8 kV supplied to platinum electrodes located in the electrolyte solution. The experimental results have shown that the diaphragm discharge has positive effects on the disintegration of clusters and agglomerates of carbon nanotubes. The primary effect on disintegration is probably the shock waves generated by the discharge. It turned out that it depends on the electrode configuration, where the treatment in anode space has far greater effects than the treatment in cathode half of the reactor. Effects of carbon nanotubes disintegration in solution are long-lasting and the treatment effect is not loosed after several months. There were detected no significant changes in the structure of plasma-treated nanotubes by Infra-red spectroscopy.

L'activité tonique diaphragmatique chez les enfants avec et sans support respiratoire

Introduction : Les nourrissons, vu la grande compliance de leur cage thoracique, doivent maintenir activement leur volume pulmonaire de fin d’expiration (VPFE). Ceci se fait par interruption précoce de l’expiration, et par le freinage expiratoire au niveau laryngé et par la persistance de la contraction des muscles inspiratoires. Chez les nourrissons ventilés mécaniquement, notre équipe a montré que le diaphragme est activé jusqu’à la fin de l’expiration (activité tonique). Il n’est pas clair si cette activité tonique diaphragmatique compense pour l’absence de freinage laryngé liée à l’intubation endotrachéale. Objectif : Notre objectif est de déterminer si l’activité tonique diaphragmatique persiste après l’extubation chez les nourrissons et si elle peut être observée chez les enfants plus âgés. Méthode : Ceci est une étude observationnelle longitudinale prospective de patients âgés de 1 semaine à 18 ans admis aux soins intensifs pédiatriques (SIP), ventilés mécaniquement pour >24 heures et avec consentement parental. L’activité électrique du diaphragme (AEdi) a été enregistrée à l’aide d’une sonde nasogastrique spécifique à 4 moments durant le séjour aux SIP : en phase aigüe, pré et post-extubation et au congé. L’AEdi a été analysée de façon semi-automatique. L’AEdi tonique a été définie comme l’AEdi durant le dernier quartile de l’expiration. Résultats : 55 patients avec un âge médian de 10 mois (écart interquartile: 1-48) ont été étudiés. Chez les nourrissons (<1an, n=28), l’AEdi tonique en pourcentage de l’activité inspiratoire était de 48% (30-56) en phase aigüe, 38% (25-44) pré-extubation, 28% (17-42) post-extubation et 33% (22-43) au congé des SIP (p<0.05, ANOVA, avec différence significative entre enregistrements 1 et 3-4). Aucun changement significatif n’a été observé pré et post-extubation. L’AEdi tonique chez les patients plus âgés (>1an, n=27) était négligeable en phases de respiration normale (0.6mcv). Par contre, une AEdi tonique significative (>1mcv et >10%) a été observée à au moins un moment durant le séjour de 10 (37%) patients. La bronchiolite est le seul facteur indépendant associé à l’activité tonique diaphragmatique. Conclusion : Chez les nourrissons, l’AEdi tonique persiste après l’extubation et elle peut être réactivée dans certaines situations pathologiques chez les enfants plus âgés. Elle semble être un indicateur de l’effort du patient pour maintenir son VPFE. D’autres études devraient être menées afin de déterminer si la surveillance de l’AEdi tonique pourrait faciliter la détection de situations de ventilation inappropriée. / Background: Infants have to actively maintain their end expiratory lung volume (EELV) due to their high rib cage compliance. Mechanisms such as high respiratory rate, short expiratory time and laryngeal braking are implicated. In mechanically ventilated infants, the diaphragm stays activated until the end of expiration (tonic activity), contributing to EELV maintenance. It is unclear whether tonic activity compensates for the lack of laryngeal braking due to intubation or if it is normally present. Objective: To determine if tonic diaphragm activity remains after extubation in infants, and if it can be observed in older children. Methods: Prospective observational study of pediatric patients 1 week to 18 years-old ventilated for >24 hr with parental consent. Diaphragm electrical activity (EAdi) was recorded using a specific nasogastric catheter during four periods: (i) the acute phase, (ii) pre-extubation, (iii) postextubation, and (iv) at PICU discharge. EADi was analyzed in a semi-automatic manner. Tonic EAdi was defined as the EAdi in the last quartile of expiration. Results: Fifty-five patients, median age 10 months (Interquartile range: 1–48) were studied. In infants (<1 year, n=28), tonic EAdi as a percentage of inspiratory activity was 48% (30-56) in acute phase, 38% (25-44) pre-extubation, 28% (17-42) post-extubation et 33% (22-43) at PICU discharge (p<0.05, ANOVA, with statistically significant difference between recordings 1 and 3-4). No significant change was observed between pre- and post-extubation periods. In older patients (n=27), tonic activity was negligible as a whole (0.6mcv). However, significant tonic EAdi (>1mcv and >10%) was observed in 10 patients (37%). Bronchiolitis was the only independent factor associated with tonic EAdi. Conclusions: In infants, tonic EAdi remains involved in ventilatory control after extubation and restoration of laryngeal braking. Tonic EAdi can be reactivated in older patients. The interest of tonic EAdi as a tool to titrate mechanical ventilation warrants further evaluation.

The aim of this thesis is to present a complex information about the etiology, diagnosis and treatment of gastroesophageal reflux disease (GERD) and to further elucidate the function of the diaphragm as the external esophageal sphincter and as a part of the antireflux barrier in patients with GERD. The study included 87 patients with verified GERD. Patients were subjected to a set of postural and respiratory maneuvers - caudal position of chest, abdominal breathing and measuring of the maximal respiratory pressures, all performed on the basis of tolerance to position and maneuver. The high-resolution manometry (HRM) was chosen to record the changes in pressure in the lower esophageal sphincter and for the measurement of respiratory pressures was used the spirometric system MasterScope. It was found that postural and respiratory maneuvers, that activate the diaphragm, increase the pressure in the EGJ. The most significant increase occurred during the maximal inspiratory maneuver, which increased LES pressure up to 261.2 %. The results also show that patients with GERD have reduced the strength of respiratory muscles, especially inspirational muscles. Reaching only 66.5 % of predicted PImax. The aim of this study is definition reference values including reference equations for parameters PImax, PEmax and P0.1 of the normal Czech population aged 14-15 years. 102 subjects were examined for this purpose (55 boys and 47 girls). Except the reference values for boys and girls, this work includes correlations of mouth pressures with anthropometric data and also with other spirometric parameters. In case of boys were found a significant correlation of PEmax parameter with weight, BMI and PEF. In group of boys we also found a correlation of PImax with BMI and PImax with IC. Parameter P0.1 correlates only with VT. In group of girls we found a significant correlations of PImax, PEmax and P0.1 with PEF and MEF25.

MEMS TUNABLE SI-BASED EVANESCENT-MODE CAVITY FILTERS: DESIGN, OPTIMIZATION AND IMPLEMENTATION

<div>The allocated frequency bands for the incoming fifth generation (5G) wireless communication technologies spread broadly from sub 6 GHz to K and potentially W bands. The evolution of the future generations toward higher frequency bands will continue and presents significant challenges in terms of excessive system complexity, production and maintenance costs. Reconfigurable radio architecture with frequency-tunable components is one of the most feasible and cost-effective solutions to meet such challenges. Among these technologies, evanescent-mode (EVA) cavity tunable resonator have demonstrated many of the needed features such as wide tunability, low loss and high linearity. Such a technology typically employs a movable membrane that controls the resonant frequency of a post-loaded cavity. </div><div><br></div><div>The first part of this work focuses on advancing such technology into the mm-wave frequency bands and beyond. Manufacturing tolerance and tuner performance are the two main limiting factors addressed here. This work develops a cost-effective micro-fabrication and package assembly flow which addresses the manufacturing related limitations. On the other hand, introducing micro-corrugated diaphragms and gold-vanadium co-sputtered thin film deposition technology, significantly reduces (4 times) the tuning voltage and enhances tuning stability (7 times). We demonstrate a tunable two-pole band-pass filter (BPF) prototype as the first EVA cavity tunable filter operating in the K-Ka band. </div><div><br></div><div>The second part of this work extensively discusses an optimal RF design flow based on the developed manufacturing technology. It considers all technology constrains and allows the actualization of a high Q transfer function with minimum bandwidth variation within an octave tuning range. Moreover, a new fully passive input/output feeding mechanism that facilitates impedance matching over the entire tuning range is presented. The devised RF methodology is validated through the design and testing of a two-resonator BPF. Measurements demonstrate a tuning range between 20-40 GHz, relative bandwidth of 1.9%-4.7%, and impedance matching over the entire tuning range which is upto 2 times better than previously reported state-of-the-art MEMS tunable filters of this type.</div><div><br></div><div>The third part of this work further advances the technology by proposing the first MEMS-based low-power bi-directional EVA tuning approach that employs both the main bias circuitry as well as a new corrective biasing technique that counteracts viscoelastic memory effects. The two key enabling technologies are extensively discussed: a) a new metal-oxide-metal (MOM) sealed cavity that maintains high quality without requiring complicated metal bonding; and b) a new electrostatic bi-directional MEMS tuner that implements the needed frequency tuning without lowering the resonator quality factor. </div><div><br></div><div>Furthermore, we explore important design and fabrication trade-offs regarding sensitivity to non-ideal effects (residual stress, fabrication imperfections). Measurement of the new prototype bi-directional design, prove that this technology readily corrects residual post-bias displacement of 0.1 um that shifts the frequency by over 1 GHz with less than 2.5 V. It takes over 100 seconds to recover this error in the uni-directional case. This correction does not adversely affect the filter performance.</div>

Efeitos de um programa de exercícios diafragmáticos de curta duração na mecânica respiratória e capacidade funcional de pacientes com DPOC: ensaio clínico controlado e aleatorizado / Effects of a short-term diaphragmatic breathing program on respiratory mechanics and functional capacity of COPD patients: a randomized controlled trial

Introdução: As alterações da mecânica diafragmática e a alta atividade dos músculos da caixa torácica estão associadas com maior sensação de dispnéia em pacientes com DPOC. Tem sido demonstrado que a respiração diafragmática (RD) aumenta a mobilidade abdominal durante o padrão diafragmático voluntário, porém, até o presente momento, nenhum estudo investigou as mudanças na mobilidade abdominal adotadas naturalmente. O objetivo desse estudo foi investigar os efeitos de um programa de treinamento de respiração diafragmática (PTRD) na mobilidade tóraco-abdominal, mobilidade diafragmática e capacidade funcional de pacientes com DPOC. Método: Trinta pacientes com DPOC (VEF1 42 +/- 13% do predito) foram alocados aleatoriamente para o grupo treinamento (GT) ou grupo controle (GC). O GT completou um PTRD supervisionado de 4 semanas (3 sessões semanais individualizadas). A efetividade do treinamento foi avaliada por meio da mensuração da relação da amplitude de movimento da caixa torácica pelo abdômen (CT/ABD; variável primária) e da mobilidade diafragmática (variável secundária). A relação CT/ABD foi quantificada utilizando a pletismografia respiratória por indutância durante RD voluntária e respiração natural (RN) e a mobilidade diafragmática foi determinada por avaliação ultra-sonográfica. O teste de caminhada em 6 minutos (TC6min) e os fatores de saúde relacionados à qualidade de vida (FSRQV) também foram avaliados. Resultados: Apenas os pacientes do GT apresentaram uma melhora na mobilidade diafragmática (18,8%) e uma redução na relação CT/ABD durante RN (26,1%) e RD voluntária (28,3%), sugerindo que a mobilidade abdominal aumentou em ambas as condições. Também foram observadas melhoras no TC6min e nos FSRQV no GT. Não foi observada diferença no GC para nenhuma variável mensurada. Conclusões: O PTRD para pacientes com DPOC induziu um aumento do recrutamento diafragmático durante a respiração natural resultando em melhora da capacidade funcional / Background: Impairment of diaphragm mechanics and enhanced activity of the chest wall muscles are associated with increased dyspnea in COPD patients. Diaphragmatic breathing (DB) has been suggested to improve abdominal motion but only during voluntarily DB, and no controlled studies have investigated the naturally adopted change in abdominal motion. The aim of this study was to investigate the effects of a diaphragmatic breathing training program (DBTP) on thoracoabdominal motion, diaphragmatic mobility and functional capacity in COPD patients. Methods: Thirty subjects (FEV1 42+/-13% predicted) were randomly allocated to either training (TG) or control group (CG). TG completed a 4-week supervised DBTP (3 individualized weekly sessions). Effectiveness was assessed by amplitude of the rib cage to abdominal motion ratio (RC/ABD ratio; primary outcome) and diaphragmatic mobility (secondary outcome). The RC/ABD ratio was measured using respiratory inductive plethysmography during voluntarily DB and natural breathing (NB). Diaphragmatic mobility was measured by ultrasonography. A 6-minute walk test (6MWT) and health-related quality of life (HRQoL) were also evaluated. Results: Only COPD patients from the TG demonstrated an improvement in diaphragmatic mobility (18.8%) and a reduction of the RC/ABD ratio during both NB (26.1%) and voluntarily DB (28.3%), suggesting that the abdominal motion improved in both conditions. An improvement in the 6MWT and in HRQoL was also observed in the TG. No differences were found in the CG for any measured outcome. Conclusions: We concluded that DBTP for COPD patients induced increased diaphragm recruitment during natural breathing resulting in an improvement in functional capacity
