Mental Status, Intellectual, and Mood States Associated with Environmental Illness Patients

The purpose of the present study was to begin development of a psychological profile for environmentally ill patients. Existing psychiatric labels are unable to encompass these patients. Test scores were drawn from a pool of 89 patients whose environmental exposures were verified by the presence of toxins in the blood serum. A Mental Status Exam, a Wechsler Adult Intelligence Scale-Revised screen, and the Profile of Mood States were administered. Results indicate a primary pattern which is significantly different from test norms consisting of fatigue, reduced mental functioning, and a lack of psychotic or personality disorder indicators. The reported symptoms of environmentally ill patients were objectively verified by current psychological test instruments. The need for a new diagnostic category for people who have been poisoned by environmental toxins is discussed.

Enhancing Spiritual Awareness Among Undergraduate Students: Improving Physiological Reactivity to and Recovery from Everyday Stressors

Chronic stress contributes to a global burden of disease that include mental illness, cardiovascular disease and early mortality. One pathway linking stress responses to health outcomes involves cardiovascular response to psychological stress. Specifically, vagal response as indexed by heart rate variability (HRV) can be used to examine autonomic processes, dysfunction of which can predict cardiovascular morbidity and mortality. The competitive academic climate on undergraduate campuses and insufficient time for recreation, rest and study, combined with inadequate coping skills can equate to consistent stressors that lead to subsequent stress and psychopathology among college students. Research demonstrates that reduced HRV can be observed among students during exam time, whereas increased HRV is observed in times of rest, suggesting that academic stress contributes to real-time physiological changes that, when prolonged, can be pathogenic in nature. Interventions aimed at influencing these processes via relaxation or other mind-body approaches have shown that enhancing meta-cognitive skills and other coping strategies have proven helpful in both improving perceived stress and psychological distress as well as improving HRV when compared with controls. This study investigates potential positive physiological effects of an eight-week Spiritual-Mind-Body (SMB) intervention, Awakened Awareness for Adolescents (AA-A), for undergraduate students. Specifically, we explore changes in HRV during resting, stress and recovery phases before and after the eight-week intervention. Additionally, we examine the association between change in self-report on measures of personal spirituality and psychological variables (pre-post AA-A), and changes across a host of HRV indices. Results include improvements in HRV recovery from stress and that a process of spiritual recovery is associated with these changes. SMB interventions that aim to improve spiritual and psychological functioning may promote psychophysiological resilience from stress.

ESRD is regarded as a life-threatening disease, which shortens the life-expectancy of a patient if he does not receive treatment. ESRD can be divided into three stages: the pre-dialysis, dialysis and transplant stages. These stages are signified by differences in medical treatment methods. None of these leads to a cure but all alle.viate symptoms of the uremic syndrome, increase the life-expectancy of the patient and are supposed to improve his quality of life. Depression and anxiety are general symptoms found in end-stage renal patients. The reasons for this are the patient's psychological reaction to renal disease, dialysis and/or transplant. Patients must also endure the side effects of medical treatment. Adjustment in vocational and social functioning is also evident. ESRD not only has a psycho social impact on the patient but also affects the healthy spouse. This is why most married couples together experience the impact of ESRD in their marital and family life. For optimal adjustment to ESRD couples need to master certain adaptational tasks, such as treating ESRD as a shared problem, adopt the roles of patient and caregiver, manage the various needs of closeness and distance between patient and caregiver and change of career roles, as well as effectively communicating with each other and the medical team, and instrumentally and emotionally support eac~ other in order to maintain the marital bonds. It was also found that religion has an important adaptational function for the married couples. A support system of family, friends, other renal patients, the medical team and a psychologist are also important to aid the couples' adjustment. Lastly it should be possible for renal couples to maintain, or even to positively reconstruct their marital relationship, and to experience marital satisfaction in the face of the threat of ESRD. Para tanto procedeu entrevistas com nove sujeitos, Psicólogos, Analistas do Comportamento, também Professores, considerados informantes qualificados, selecionados pelo banco de currículos LATTES, que atendessem aos critérios de formação e produção acadêmica, ligados ao ensino de Psicologia e/ou Educação. Foi solicitado que expressassem considerações acerca das contribuições e obstáculos da relação entre Análise do Comportamento e Educação e também acerca de sua própria atividade de ensino. Os resultados, organizados em categorias, corroboram estudos da literatura ao apontar tanto para a contribuição já demonstrada pela abordagem, quanto pelos obstáculos das incompreensões. Também é presente uma análise dos limites dos próprios analistas do comportamento em prover maior comunicação com a Educação e demonstrar com sua prática educativa a viabilidade do exercício da Análise do Comportamento no campo educacional. Interviews were carried out with nine subjects - Psychologists, Behavior Analysts, and Teachers - all of whom were considered qualified informants, selected through the LATTES curriculum bank, attending to necessary qualification and academic production criteria and related to the teaching of Psychology and/or Education. They were requested to express their views on the contributions and obstacles of the relationship between Behavior Analysis and Education, and on their own teaching practice. The results, organized into categories, corroborate the studies found in literature, pointing to the fact that some contribution is already demonstrated by the teaching approach used, but is somehow impeded by incomprehension. It also presents an analysis of the Behavior Analysts themselves, of their limitations in communicating better with the Educators and in demonstrating with their educating practice the role Behavior Analysis can play in Education. Considerations as to how the teaching approach is addressed are presented, as well as considerations emphasizing the importance of teaching teachers Teaching and Learning Theories, a task in which the mastering of the principles and applications of the Behavior Analysis are of great relevance. / Doutorado / Psicologia Educacional / Doutor em Educação

M.A. (Psychology) / The health context of South Africa is on the one hand unique in comparison to the rest of the world. On the other hand does it also. show characteristics of both Third World and First World disease patterns. There is a substantial component of the South African health sector that is negatively affected. This can possibly be ascribed to previous health policies. South Africa has unique characteristics concerning the chronic degenerative aspects of the First World disease pattern. White South Africans have the same cardiovascular disease patterns as the rest of the world with the exception that the South African disease patterns has a much larger incidence and degree of seriousness that the rest of the world. Research in the area of the chronic degenerative nature of heart disease and vascular disease is of great importance. It becomes necessary to address degenerative disease and also lifestyle diseases not only medically but also in terms of an individual's lifestyle. The management of an individual's lifestyle will not only have preventive consequences in the South African context, but it can also be utilised in the treat~ent of cardiovascular disease. Research undertaken at the Clinic and Centre for Behavioral Medicine at the Rand Afrikaans University found that the management or treatment of the Type A behavior pattern for the prevention of recurrent cardiovascular diseases were particularly effective. It therefore seems that technology developed elsewhere proves to be effective for the South African context. According to Johnston (1992) two types of risk factors contribute to the development of cardiovascular disease. The first constitute of classical risk factors which include aspects of blood pressure and cholesterol. The second risk factor includes psychological aspects and in particular the Type A behavior pattern and its components. Johnson and Broman (1987) indicate that the components of anger and hostility of the Type A behavior pattern constitute the most important behavioral factor of Type A coronary-prone behavior and cardiovascular disease. Research also indicate that the component of hostility presents a significant predictor of cardiovascular disease (Helmers et al., 1993) . The role of aggression and its components in the Type A behavior pattern was investigated in this study. An attempt was made to establish whether there is a simultaneous reduction in aggression, its components and the Type A behavior pattern and whether certain components of aggression were more important that others. A group of 39 heart patients were investigated on the following indexes: psychological, cardiological and biochemical in order to establish heart disease risk factors in a biopsychosocial context. A modified Type A treatment progranme was administered to this group over a period of twelve weeks at a local heart rehabilitation centre. A second group of 19 patients served as a no-treatment waitinglist control group, but simultaneously underwent an aerobic exercise and cardiovascular counselling programne. The results of this study indicated that cynical hostility was probably the major risk factor of all the components of aggression in the Type A behavior pattern. The second most important component of aggression in the Type A behavior pattern is the expression of anger in general. The latter also corresponds with results found in research on this subj ect. Ov-ert or specific expression of anger .nd the control of anger also contribute to the psychosocial causation of Type A behavior pattern in cardiovascular disease. Comparisons of the experimental and control groups after the intervention showed statistically significant differences of anger expression in general, specific anger expression, inhibition of anger, control of anger, and hostility. It was concluded that significant differences for the diverse components of aggression have been found due to the experimental intervention programme.

The inexorable deterioration of chronic degenerative diseases brings the elderly to a paradox, that is, on the one hand, the threat of annihilation is painful, causing anguish and suffering; on the other hand, it makes them prone to assigning new meanings to their existence, reconsidering their perspectives in order to find reasons to live. The restlessness in face of the unfathomable limits of life, which takes shape with our growing consciousness of mortality itself, requires that the elderly take charge of what ails them. Thus, this work seeks to grasp the modes through which the elderly suffering from chronic degenerative diseases subjectify aging, falling ill and dying. The participants in this study were five old-age individuals suffering from chronic diseases both in and out of hospital facilities. The tools utilized included narrative interviews carried out individually with each participant and a Field Journal. The narratives collected from the elderly and the notes taken in the field sessions were analyzed from the Foucauldian discourse analysis approach. Drawing on Foucault’s writings, the research findings and the successive implications and problems we proposed allowed us to locate old-age in the realm of ethics, aesthetics and stylistics of existence. From this point of view, the participants’ accounts revealed unique elements of subjectivity, of producing their existence and of positioning themselves in face of life and death. Such production of subjectivity is based on an ethical-aesthetical model. The interviewees sought to re-create and re-invent their own aging while seeking to invent their own selves like works of art, devising and assigning life with a particular style. We look at old-age through the angle of the art of living sustained by self-care which also entails the ineluctable caring others. / A velhice, atravessada por enfermidades graves, expõe o constante entrechoque entre vida e morte. O transcurso inexoravelmente progressivo de uma doença crônico-degenerativa mobiliza o idoso a confrontar-se com um paradoxo: por um lado, a ameaça de aniquilamento provoca dor, angústia e sofrimento, enquanto que, por outro, impele-o à tendência de ressignificar sua existência, redimensionando perspectivas na busca de um sentido para o viver. A inquietude frente ao insondável limite da vida, radicalizada na consciência da própria mortalidade, exige dos sujeitos idosos assumir uma posição frente àquilo que os aflige. Este trabalho objetiva, pois, compreender os modos de subjetivação de idosos portadores de doenças crônico-degenerativas ante o envelhecer, o adoecer e o morrer. Participaram deste estudo cinco idosos em condições crônicas de adoecimento que estavam ou não em regime de internação hospitalar. Os instrumentos utilizados foram entrevista narrativa realizada individualmente com cada participante e um Diário de Campo. As narrativas produzidas pelos idosos e o registro das observações em campo foram analisados utilizando-se a abordagem da analítica discursiva na perspectiva foucaultiana. Dos caminhos trilhados nesta pesquisa com os sucessivos desdobramentos das articulações e problematizações as quais propusemos, permitiu-nos, sob a inspiração dos escritos de Foucault, situar a velhice no campo da ética, da estética e das estilizações da existência. Sob essa ótica, encontramos em seus discursos, circunscrições dos modos singulares de se subjetivarem, de produzir suas existências e de posicionarem-se perante à vida e à morte. A produção desses modos próprios de subjetivar-se está pautada num modelo ético-estético e político. Os idosos entrevistados procuravam re (criar) e re (inventar) suas próprias velhices; buscavam a invenção de si mesmos como uma obra de arte, construindo e conferindo à vida um estilo particular. Lançamos o olhar para a velhice pelo viés das artes de viver sustentadas pelo cuidado de si que passa também pelo imperioso cuidar do outro.
