L’intégration croissante des procédés innovants comme les ultrasons, les champs magnétiques, et les champs électriques pulsés a pour but d’améliorer et de stabiliser le déroulement des procédés de fermentation. Le champ électrique pulsé (CEP) est un procédé athermique généralement utilisé pour l’inactivation des pathogènes (Barbosa-Cánovas et al., 2001) ainsi que pour l’extraction des composés intracellulaires d’intérêt (El Zakhem et al., 2006a; Vorobiev & Lebovka 2006). Dans ce travail de thèse, nous proposons d’évaluer l’activité microbienne des cellules soumises à un traitement électrique modéré. Un intérêt particulier est apporté à des aspects fondamentaux comme la croissance et le métabolisme des cellules. Sur le plan technologique, le but fondamentale est de mettre en place et optimiser des protocoles de stimulation de microorganismes pour intensifier les bioprocédés. La fermentation de microorganismes stimulés par CEP a montré des cinétiques plus rapides que les levures non traitées. La stimulation de l’activité fermentaire s’est révélée grâce au suivi de la masse du milieu, les solutés solubles, l’absorbance, les sucres... L’optimisation des protocoles de stimulation a permis de réveler deux comportements logarithmique et saturé. Il a été montré une dépendance importante de l’énergie spécifique sur certains aspects physiologiques notamment la taille et le nombre de colonies. / The continually increasing integration of innovative technologies such as ultrasound, magnetic fields, and pulsed electric fields aims to improve and stabilize the course of fermentation processes. The pulsed electric field (PEF) is an athermal process generally used for pathogen inactivation (Barbosa-Canovas et al., 2001) and for the extraction of intracellular compounds of interest (El Zakhem et al., 2006a; Vorobiev & Lebovka 2006). In this thesis, we propose to evaluate the microbial activity of cells subjected to a moderate electric treatment. Special consideration is given to key aspects such as growth and cell metabolism. Technologically, the fundamental purpose is to implement and optimize microorganisms’ stimulation protocols to intensify their bioprocesses. The positive impact of PEF pre-treatment on yeast cells was shown by their faster fermentation kinetics compared to the control. This was proven by monitoring the weight of the ferment, the soluble solutes, the UV absorbance, and sugar consumption profiles. Two behaviors of electrostimulation, “logarithmic” and “saturated”, were revealed by optimization of the stimulation protocols. Finally, a relationship between the growth rate, the size of the colonies and the applied specific energy is deduced.

Vzdálené účinky svalové elektrostimulace / Distant effects of muscle electrostimulation

This work is trying to identify possible non-stimulatory effects and distant effects of muscle electrostimulation musculus quadriceps femoris based on a literature search. Furthermore evaluates the influence of High Tone Electrical Stimulation (HTEMS) muscles of lower limbs on the activity of the autonomic nervous system, expressed by heart rate variability, a spectral power in the individual frequency bands. Randomized crossover study involved 20 healthy subjects (12 men and 8 women), average age 24.6 ± 5.6 years. Heart rate variability was evaluated before application of HTEMS and immediately after. Measurements were performed with electrostimulator HiToP 191. The results did not show statistically significant increase in spectral power in the individual frequency bands. Heart rate variability was evaluated in two situations: the situation without the application HTEMS and the situation with the aplication of HTEMS. Electrostimulation was done with HITOP 191. The results did not show statistically significant differences in all monitored components. The study also dealt with the reproducibility of measurements of perception and motor threshold for HTEMS.

L’objectif de ce travail de thèse a été d’analyser les stratégies de réduction de la fatigue musculaire en course de trail et potentiellement d’identifier certains paramètres d’influence de cette fatigue. La course de trail est un nouveau sport en essor qui induit une combinaison spécifique de fatigue et dommages musculaires des principaux muscles locomoteurs. Afin de pouvoir conduire des études interventionnelles, une étude descriptive préliminaire a été conduite pour caractériser la fatigue spécifique et les dommages musculaires induits par ce type d’épreuve de trail. Ensuite, la reproductibilité du trail comme modèle de fatigue a été vérifiée afin de pouvoir l’utiliser dans un contexte d’intervention. Enfin, deux études visant à réduire la fatigue induite par le trail ont été conduites. D’une part l’utilisation des vêtements de compression - très à la mode en trail a été analysée comme stratégie d’optimisation de la performance. D’autre part, a aussi été étudié l’effet d’un réchauffement préalable du muscle sur les dommages musculaires : Dans cette optique, une étude contrôlée en laboratoire a été menée, examinant les effets d’un réchauffement passif sur les conséquences fonctionnelles de course en descente chez une population non-entraînée. En résumé, les travaux conduits au sein de cette thèse fournissent une description de la fatigue en trail, et valident l’utilisation du trail comme modèle reproductible de terrain pour investiguer les stratégies de réduction de la fatigue. De plus, ils relativisent l’effet positif des vêtements de compression sur la performance et montrent le lien fonctionnel entre le réchauffement musculaire et la réduction des dommages musculaires induits par un travail excentrique. / The aim of this thesis was to analyse strategies to reduce muscular fatigue in trail running and potentially draw conclusions on the underlying mechanisms. Trail running is a new and upcoming sport that induces a combination of fatigue and muscle damage in the main locomotor muscles. To obtain conclusive evidence on the effect of intervention studies a preliminary descriptive study was undertaken to characterise typical fatigue and damage. Subsequently a model was developed and validated that would allow the investigation of interventions in an applied field setting. A popular current strategy in trail running is the use of compression garments; therefore the effect of these on performance was studied as an intervention. Furthermore, prior heating is anecdotally considered beneficial and recent research has suggested a potential mechanism to link this with reduced muscle damage. Therefore a controlled laboratory study was conducted, examining the effects of passive heating on functional consequences of downhill running in an untrained population. In synopsis, the research conducted for this thesis provides descriptive evidence and a validated terrain model to further investigate fatigue reduction strategies in trail running. Additionally it adds to the current literature in disproving a positive effect of compression garments on performance and demonstrating the functional link between heating and eccentric-induced muscle damage reduction.

São diversas as opções de tratamento da incontinência urinária, como os exercícios dos músculos do assoalho pélvico, tratamento com fármacos, injeção transuretral, e o esfíncter urinário artificial. A Sociedade Internacional de Continência recomenda como tratamento inicial os exercícios dos músculos do assoalho pélvico supervisionado, orientações de estilo de vida adequado, regimes urinários regulares, terapias comportamentais e medicação. A Revisão Sistemática desta Dissertação mostrou a necessidade de mais estudos com melhor qualidade metodológica para evidenciar o uso da eletroestimulação como intervenção eficaz no tratamento da incontinência urinária; O Estudo Transversal Retrospectivo, após a análise de 128 prontuários do Ambulatório de Fisioterapia Pélvica do Hospital Federal dos Servidores do Rio de Janeiro mostrou resultados significativos da Fisioterapia Pélvica para a redução da incontinência urinária e do impacto da incontinência urinária na vida diária destes pacientes. Por fim, o Experimento Controlado Randomizado, duplo cego, mostrou resultados significativos do uso da eletroestimulação associada aos exercícios dos músculos do assoalho pélvico como uma opção de tratamento conservador capaz de potencializar a continência urinária após a prostatectomia radical. / Urinary incontinence in addition to be multifactorial with great therapeutic complexity is a problem of public health order and it deserves more attention because it causes a huge negative impact on the quality of life. The International Continence Society recommended as initial therapy exercises of the pelvic floor muscles supervised, appropriate lifestyle guidance, regular urinary regimes, behavioral therapies and medication. A Systematic Review of this dissertation showed the need for more studies with better methodological quality to highlight the use of electrical stimulation as an effective intervention in the treatment of urinary incontinence; The Cross Retrospective Study, after analysis of 128 medical records of Pelvic Physical Therapy Clinic of the Federal Hospital of Rio de Janeiro's servers showed significant results of pelvic physiotherapy to reduce urinary incontinence and the impact of urinary incontinence on daily life of these patients. Finally, the experiment Controlled randomized, double-blind, showed significant results of the use of electrical stimulation associated with the exercise of the pelvic floor muscles as a conservative treatment option capable of enhancing the urinary continence after radical prostatectomy.

Raumens adaptacija įgyta po pirmo krūvio, vadinama pakartoto krūvio efektu (PKE). PKE aiškiai reiškiasi po ekscentrinių pratimų ir yra siejamas su raumens pažeida. Raumens pažeidos dydis priklauso nuo pradinio intensyvumo, kartojimų kiekio bei raumens ištempimo amplitudės. Nėra aišku, kaip skiriasi pakartotas krūvio efektas, atliekant didėjantį ir vienodą fizinį krūvį. Tyrimo hipotezė: antro krūvio metu padidinus krūvio intensyvumą ir raumens ištempimo amplitudę sumažės pakartotas krūvio efektas. Tyrimo tikslas – nustatyti ir palyginti PKE po vienodo ir didėjančio fizinio krūvio. Uždaviniai: 1. Palyginti PKE po šuolių nuo skirtingo aukščio pakylos; 2. Palyginti PKE po šuolių su skirtinga raumens ištempimo amplitude. Praktinė darbo reikšmė: aiškesnis pakartoto krūvio efekto suvokimas gali padėti pasirinkti teisingą krūvio strategiją saugantis nuo raumenų pažeidos. Metodika: Pirmo tyrimo metu, pirma tiriamųjų grupė, šuolius atliko nuo didėjančio aukščio pakylos, pirmo krūvio metu buvo 30 cm, o antro - 60 cm aukščio pakylos. Kita tiriamųjų grupė šuolius atliko nuo vienodo 60 cm aukščio pakylos. Antro tyrimo metu, pirma tiriamųjų grupė šuolius atliko su didėjančia raumens ištempimo amplitude, pirmo krūvio metu 120º, antro - 90º kampu per kelio sąnarį. Kita grupė pirmo ir antro krūvių metu šuolius atliko su vienoda ištempimo amplitude - 90º per kelio sąnarį. [toliau žr. visą tekstą] / Muscle’s adaptation gained after first workload is called repeated bout effect (RBF). RBF is clearly noticed after eccentric exercise and is associated with muscle damage. Muscle damage’s severity depends on the primary intensity and the extension of muscle’s amplitude. It is not clear how repeated bout effect changes performing enhanced and stable physical workload. Hypothesis. The repeated bout effect will reduce after enhancing the level of workload and the extension of muscle’s amplitude. Aim. To investigate and compare repeated bout effect after stable and enhanced physical workload. Objectives: 1. To compare RWE after jumps from different hight platforms. 2. L’objectif général de ce travail doctoral était d’étudier les effets de la fatigue (physique et mentale) sur le contrôle postural. Tout d’abord, les effets de la fatigue musculaire du quadriceps fémoral induite par des contractions volontaires et des contractions électro-induites ont été comparés. Les résultats ont montré que des contractions volontaires altéraient davantage le contrôle postural que des contractions électro-induites pour une perte de force musculaire équivalente. Ceci s’expliquerait par la présence d’une fatigue centrale après les contractions volontaires, ainsi qu’une sollicitation différente des fibres musculaires entre les deux modalités de contraction. Des mécanismes compensatoires seraient mis en œuvre après ces deux modalités de contractions fatigantes, mais de façon plus prononcée pour les contractions volontaires. Concernant la fatigue mentale, les effets d’une tâche cognitive exigeante et prolongée sur ordinateur (induisant un état de fatigue mentale) et d’un visionnage de documentaire (tâche contrôle) sur le contrôle postural ont été comparés. De prime abord, la fatigue mentale affecterait le contrôle postural en condition de privation visuelle, en réduisant l’efficacité des stratégies d’attention associatives. De façon surprenante, visionner un documentaire conduirait également à une altération du contrôle postural, potentiellement en raison du maintien prolongé d’une posture assise affaissée. La posture assise pourrait ainsi constituer un facteur confondant dans les protocoles expérimentaux comportant une évaluation posturale. Une analyse statistique de classification en cluster hiérarchique a permis de mettre en évidence des sensibilités différentes au sein des individus à l’égard de la tâche mentalement fatigante. Le niveau de fatigue mentale serait associé au niveau de dégradation du contrôle postural sur un support stable avec les yeux ouverts. La réalisation d’une tâche cognitive prolongée exigeant une attention visuelle soutenue face à un écran d’ordinateur provoquerait une fatigue visuelle. Cette fatigue visuelle affecterait différemment les individus. Elle toucherait plus particulièrement les sujets les plus dépendants des informations visuelles, pour lesquels le contrôle postural se dégraderait davantage que les autres individus. / The overall objective of this doctoral work was to study the effects of fatigue (physical and mental) on postural control. First, the effects of femoral quadriceps muscle fatigue induced by voluntary contractions and electro-induced contractions were compared. The results showed that voluntary contractions affected postural control more than electro-induced contractions for an equivalent loss of muscle strength. This could be explained by the presence of central fatigue after voluntary contractions, as well as a different strain on muscle fibres between the two modes of contraction. Compensatory mechanisms would be implemented after these two modes of tiresome contractions, but more pronounced for voluntary contractions. With regard to mental fatigue, the effects of demanding and prolonged cognitive computer work (inducing mental fatigue) and documentary viewing (task control) on postural control were compared. At first glance, mental fatigue would affect postural control in conditions of visual deprivation, reducing the effectiveness of associative attention strategies. Surprisingly, viewing a documentary would also lead to an alteration in postural control, potentially due to the prolonged maintenance of a collapsed sitting posture. Sitting posture could thus be a confounding factor in experimental protocols involving postural evaluation. A statistical analysis of hierarchical cluster classification revealed different sensitivities within individuals towards the mentally demanding task. The level of mental fatigue would be associated with the level of degradation of postural control on a stable support with eyes open. Performing a prolonged cognitive task requiring sustained visual attention to a computer screen would cause visual fatigue. This visual fatigue would affect individuals differently. It would particularly affect the subjects most dependent on visual information, for which postural control would be more degraded than other individuals.

Tratamento da incontinência urinária de esforço por meio de eletroestimulação funcional dos músculos do assoalho pélvico: revisão sistemática de ensaios clínicos aleatorizados / Treatment of stess urinary incontinence by the pelvic floor muscles: a systematic review of randomized clinical trials

Conselho Nacional de Desenvolvimento Científico e Tecnológico / Este estudo teve como objetivo verificar, através de uma revisão sistemática de ensaios clínicos aleatorizados, os benefícios da estimulação elétrica funcional endovaginal ou dos tratamentos conservadores às pacientes com incontinência urinária de esforço, e demonstrar qual modalidade de tratamento conservador apresenta melhores resultados na terapêutica dessas mulheres: a estimulação elétrica funcional endovaginal, em comparação com os cones vaginais ou a realização de exercícios perineais. Para tanto, foram realizadas buscas nas principais bases de dados científicos, por estudos que atendessem a pergunta da pesquisa, tipo de intervenção e tipo de participantes selecionados. Destes, foram selecionados 7 estudos que foram submetidos à análise dos revisores, que avaliaram os seguintes desfechos: episódios de perda urinária, quantificação das perdas urinárias através do pad-test, força da musculatura perineal, qualidade de vida, volume residual, capacidade cistométrica máxima, melhora dos sintomas, satisfação e cura. Todas as terapias pesquisadas apresentaram melhora dos sintomas da incontinência urinária de esforço; no entanto, segundo os desfechos avaliados, apresentaram diferença no resultado comparativo. Quanto às perdas urinárias, ao pad-test e à força da musculatura perineal, a realização dos exercícios pélvicos obteve os melhores resultados. Já a terapia por estimulação elétrica endovaginal e a terapia com os cones apresentaram resultados semelhantes, não sendo encontrada diferença significativa em nenhum dos desfechos analisados. De acordo com os achados obtidos nesta revisão sistemática, entendemos que o tratamento pela estimulação elétrica traz benefícios às pacientes com incontinência urinária de esforço. Of these, seven studies were selected and analyzed by the reviewers who assessed the following outcomes: frequency of urinary leakage, quantification of urinary leakage through the pad-test, perineal muscle strength, quality of life, residual volume, maximum cystometric capacity, improvement, satisfaction and healing. All therapies surveyed showed improvement of symptoms of stress urinary incontinence; however, according to the outcome parameters, they showed differences in the comparative result. As for urinary leakage, the pad-test and strength of the perineal muscles, the pelvic exercises achieved the best results. The electrical stimulation therapy and transvaginal cone therapy showed similar results, no significant difference was found in any of the outcomes. According to the findings of this systematic review, we believe that treatment by electrical stimulation is beneficial to patients with stress urinary incontinence. The pelvic exercises have proven to be the therapy that more significantly reduces the symptoms caused by this condition
