The early non-invasive diagnosis of epithelial tissue alterations in daily clinical routine is still challenging. Since optical coherence tomography (OCT) shows the potential to differentiate between benign and malignant tissue of primal endothelium, OCT could be beneficial for the early diagnosis of malignancies in routine health checks. In this research, a new handheld endoscopic scanning unit was designed and connected to a spectral domain OCT system of our workgroup for the in vivo imaging of the human oral mucosa.

Implication des biofilms dans la rhinosinusite chronique et l’évaluation des traitements avec un modèle in vitro

Introduction : La chronicité de la rhinosinusite, sa résistance aux antibiotiques, et ses exacerbations aiguës laissent croire que les biofilms sont impliqués dans la rhinosinusite chronique. Objectifs : Nous avons évalué la capacité des bactéries Pseudomonas aeruginosa, staphylocoques à coagulase négative et Staphylococcus aureus à former des biofilms par un essai in vitro, et si cette capacité de formation a un lien avec l’évolution de la maladie. Nous avons évalué in vitro l’effet de la moxifloxacine, un antibiotique utilisé dans le traitement de la rhinosinusite chronique sur des biofilms matures de Staphylococcus aureus. Méthodes : Trent et une souches bactériennes ont été isolées de 19 patients atteints de rhinosinusite chronique et qui ont subit au moins une chirurgie endoscopique des sinus. L’évolution de la maladie a été notée comme "bonne" ou "mauvaise" selon l’évaluation du clinicien. La production de biofilm a été évaluée grâce à la coloration au crystal violet. Nous avons évalué la viabilité du biofilm après traitement avec la moxifloxacine. Ces résultats ont été confirmés en microscopie confocale à balayage laser et par la coloration au LIVE/DEAD BacLight. Résultat et Conclusion : Vingt deux des 31 souches ont produit un biofilm. La production d’un biofilm plus importante chez Pseudomonas aeruginosa et Staphylococcus aureus était associée à une mauvaise évolution. Ceci suggère un rôle du biofilm dans la pathogenèse de la rhinosinusite chronique. Le traitement avec la moxifloxacine, à une concentration de 1000X la concentration minimale inhibitrice réduit le nombre des bactéries viables de 2 à 2.5 log. Ces concentrations (100 µg/ml - 200 µg/ml) sont faciles à atteindre dans des solutions topiques. Les résultats de notre étude suggèrent que l’utilisation de concentrations supérieure à la concentration minimale inhibitrice sous forme topique peut ouvrir des voies de recherche sur de nouveaux traitements qui peuvent être bénéfiques pour les patients atteints de forme sévère de rhinosinusite chronique surtout après une chirurgie endoscopique des sinus. / Introduction: The role of biofilms in chronic diseases is increasingly recognized. Chronic rhinosinusitis, with its chronic indolent course, resistance to antibiotics, and acute exacerbations, has an evolution that parallels that of other biofilm-related diseases. Objectives: 1-To develop an in vitro method to assess the biofilm formation capacity. 2- To determine whether biofilm-forming capacity of bacteria demonstrated in chronic rhinosinusitis has an impact on persistence of the disease following endoscopic sinus surgery. 3- To determine the in vitro activity of moxifloxacin against Staphyylococcus aureus in biofilm form. Method: Thirty-one bacterial strains recovered from 19 patients with chronic rhinosinusitis at least one year post-endoscopic sinus surgery. Evolution of disease was assessed by questionnaire and endoscopy as favorable or unfavorable. The bacteria were cultured on a 96-well culture plaque and a semi-quantitative method using crystal violet to quantify biofilm production was used. Confirmation of the effect of the antimicrobial agents on viability was performed with confocal laser microscopy, using a LIVE/DEAD BacLight staining. Results: Twenty-two of 31 samples produced a biofilm thicker or equal to the positive control. Biofilm formation was associated with a poor evolution for Pseudomonas aeruginosa and Staphylococcus aureus, but not for coagulase-negative staphylococci. Biofilm treated with moxifloxacin at 1000X (0.1mg/ml – 0.2 mg/ml) gave a 2 to 2.5 log reduction in number of viable bacteria. Conclusion: We have shown that Crystal violet method is able to detect biofilm formation. There is a correlation between in vitro biofilm production by Pseudomonas aeruginosa and Staphylococcus aureus and unfavorable evolution after endoscopic sinus surgery, suggesting a role for biofilm in chronic rhinosinusitis. Increased concentrations of moxifloxacin, easily attainable in topical solutions have a potential role in the management of biofilm infections.

Introdução: A maioria dos pacientes acometidos pela neoplasia que envolve a confluência biliopancreática é diagnosticada em fase avançada. A Colangiopancreatografia Retrógrada Endoscópica (CPRE) é o método de escolha para a drenagem da via biliar obstruída. Todavia, existe um índice de insucesso em torno de 10%. Nesses casos, técnicas alternativas serão aplicadas, como drenagem percutânea trans-hepática e drenagens cirúrgicas. Objetivo: Avaliar o sucesso técnico, clínico, qualidade de vida e sobrevida da drenagem biliar pela cirurgia convencional e técnica endoscópica ecoguiada em pacientes portadores de neoplasia maligna da confluência biliopancreática. Método: No período de abril de 2010 a setembro de 2013, foram estudados 32 pacientes portadores de neoplasia maligna da confluência biliopancreática. Todos os que foram incluídos nesse estudo apresentaram falha na drenagem biliar por CPRE. Três deles foram excluídos por insucesso técnico (falha na confecção da anastomose hepaticojejunal e da formação da fístula coledocoduodenal ecoguiada). O Grupo I foi formado por 15 pacientes submetidos à Hepaticojejunostomia (HJT) em \"Y\" de Roux e derivação gastrojejunal. O Grupo II foi formado por 14 pacientes submetidos à coledocoduodenostomia ecoguiada (CDT). O sucesso clínico foi avaliado pela queda da bilirrubina sérica total em mais de 50% nos sete primeiros dias após o procedimento. A qualidade de vida foi avaliada pelo questionário SF-36 e a sobrevida pela curva de Kaplan-Meier. Resultados: O sucesso técnico foi de 93,75% (15/16) no Grupo I e de 87,5% (14/16) no Grupo II (p = 0,598). O sucesso clínico ocorreu em 14 (93,33%) pacientes pertencentes ao Grupo I e em 10 (71,43%) do Grupo II. Não houve diferença estatisticamente significativa (p = 0,169). O comportamento médio dos escores de qualidade de vida foi estatisticamente igual entre as técnicas ao longo do seguimento (p > 0,05 Técnica * Momento). Houve alteração média estatisticamente significativa ao longo do seguimento nos escores de capacidade funcional, saúde física, dor, aspectos sociais, aspectos emocionais e saúde mental em ambas as técnicas (p < 0,05). O escore de saúde mental foi, em média, estatisticamente maior nos do Grupo II (CDT) em todos os momentos (p = 0,035). O tempo médio de sobrevida daqueles pertencentes ao Grupo I foi de 82,27 dias e os do Grupo II, de 82,36 dias. Sessenta por cento dos pertencentes ao Grupo I faleceram até 90 dias após o procedimento cirúrgico. Por outro lado, 42,9% dos submetidos à CDT faleceram no mesmo período. Não houve diferença estatisticamente significativa no tempo de sobrevida entre os Grupos (p = 0,389). Aim: To evaluate the technical and clinical success, quality of life and patient survival of biliary drainage by conventional surgery and endosonography-guided technique in patients with malignant neoplasm of the biliopancreatic junction. Methodology: From April 2010 to September 2013, 32 patients with malignant neoplasm of the biliopancreatic junction were studied. All patients included in this study had failed biliary drainage by ERCP. Three patients were excluded due to technical failure (failure in the construction of hepatico-jejuno anastomosis and formation of endosonography-guided choledochoduodenal fistula). Group I comprised of 15 patients who underwent Roux-en-Y hepaticojejunostomy (HJT) and gastrojejunal bypass. Group II consisted of 14 patients who underwent endosonography-guided choledochoduodenostomy (CDT). Clinical success was assessed by the decrease of more than 50% in total serum bilirubin in the first seven days after the procedure. Quality of life was assessed by SF-36 questionnaire and survival by Kaplan-Meier curve. Results: Technical success rate was 93.75% (15/16) in group I and 87.5% (14/16) in group II (p = 0.598). Clinical success occurred in 14 (93.33%) patients in group I and 10 (71.43%) patients in group II. There was no significant statistically difference (p = 0.169). The average quality of life score were statistically equal between the techniques during follow-up (p > 0.05 * Technical Moment). There were statistically significant mean changes during follow-up of functional capacity score, physical health, pain, social functioning, emotional and mental health aspects in both techniques (p < 0.05). The mental health score was, on average, statistically higher in group II (CDT) at all times (p = 0.035). The median survival time of patients in group I was 82.27 days and Group II patients was 82.36 days. Sixty percent of patients in group I died within 90 days after the surgical procedure. On the other hand, 42.9% of the patients who underwent CDT died in the same period. There was no statistically significant difference in survival time between the groups (p = 0.389). Conclusion: Data relating to technical and clinical success, quality of life and survival were similar in both groups and there were no statistically significant differences

The intraoperative anatomical limit was compared to nasopalatine and palatine lines. The nasopalatine line predicts more accurately the inferior limit of the transnasal approach.

Développement d’un endomicroscope multiphotonique à deux couleurs pour l’imagerie du métabolisme énergétique cellulaire / Label- free in vivo in situ diagnostic imaging by cellular metabolism quantification with a flexible multiphoton endomicroscope

La microscopie multiphotonique est une modalité d’imagerie de pointe offrant des opportunités d’avancées remarquables en biologie mais aussi dans le domaine médical. Afin d’en exploiter pleinement le formidable potentiel au cœur même de la pratique clinique, le développement de nombreuses sondes miniaturisées à fibre optique pour l’endomicroscopie multiphotonique (EMMP) a eu lieu depuis de nombreuses années et dans de nombreux laboratoires français et étrangers. Il s’est pour l'instant confronté à des limitations majeures comme l’impossibilité de recueillir les signaux d’auto-fluorescence des tissus qui sont intrinsèquement faibles comme ceux venant des co-enzymes métaboliques NADH et FAD. Cette limitation compromet l'utilité de l’EMMP en la restreignant à une imagerie morphologique requérant un marquage exogène des tissus. Ce manuscrit présente une architecture d’EMMP permettant de dépasser cette limitation, capable de proposer une imagerie fonctionnelle du métabolisme cellulaire en temps réel, in vivo, in situ, sans marquage. Le prototype d’EMMP proposé est une amélioration du précédent, où les Grisms en réflexions sont remplacés par des Grisms en transmission, permettant d’élargir la bande spectrale d’utilisation et la transmission du système. Ce prototype voit aussi l’adjonction d’un second laser excitateur afin d’accéder aux fluorescences du NADH et du FAD. Les résultats démontrent capable que nous sommes à même d’imager les fluorescences cellulaires intrinsèques au travers de 5 mètres de fibre optique avec une résolution subcellulaire. Parmi celles-ci nous sommes capables d’exciter et de collecter spécifiquement les fluorescences du NADH et du FAD. Enfin nous détectons assez de photons pour disposer d‘informations quantitatives et donc de proposer une image du rapport d’oxydo-réduction optique en endomicroscopie. / Nonlinear microscopy is a cutting edge imaging modality leading to remarkable step forward in biology but also in the clinical field. To use it at its full potential and at the very heart of clinical practice, there has been several development of fiber-based micro-endoscope. The application for those probes is now limited by few major restrictions, such as the impossibility to collect auto-fluorescence signal from tissues theses being inherently weak such as the fluorescence from NADH or FAD. This limitation reduces the usefulness of the micro-endoscope effectively restraining it to morphological imaging modality requiring staining of the tissue. Our aim is to go beyond this limitation, showing cellular metabolism monitoring, in real time, without any staining. The experimental setup is an upgrade of our precedent one where the reflection- based Grism stretcher is replace with a new generation transmission-based Grism stretcher. Another Laser was also added in order to tune the first laser at 860nm to allow FAD imaging and the second one to 760nm for NADH. The results prove that we assess and image the level of NADH and FAD at subcellular resolution through a five-meter-long fiber. Thus we demonstrate that we are capable of measuring the optical redox ratio in a micro-endoscopic configuration.

Chirurgie endoscopique des sinus pour le traitement de la rhinosinusite chronique : évaluation des résultats opératoires et définition du succès

La rhinosinusite chronique (RSC) est une maladie avec une haute prévalence au Canada et des impacts économiques et individuels importants. Une des options thérapeutiques dans l’algorithme complexe de traitement est la chirurgie endoscopique des sinus (CES), qui est préconisée chez les patients qui ont une réponse insatisfaisante à la thérapie médicale. Le présent mémoire porte sur les critères minimaux nécessaires à l’atteinte du succès en matière de CES. Dans un premier temps, une consultation d’experts en RSC Canadiens provenant de 12 institutions nous a permis d’offrir des définitions du succès acceptable ainsi que du succès optimal du point de vue des prestataires de santé. Dans un deuxième temps, une étude auprès de 22 patients souffrant de RSC nous a permis d’identifier les thèmes importants qui doivent guider le processus décisionnel collaboratif. La première étude a permis de conclure qu’une définition du succès post-opératoire en CES se doit de comporter 2 composantes : un aspect objectif (endoscopie nasale) et un aspect subjectif (test d’issues mesurées par le patient ou questionnaire spécifique). Selon les experts, pour parler de succès optimal il faut une résolution complète des symptômes ainsi qu’un résultat endoscopique parfait. La deuxième étude a permis de démontrer que les patients accordent beaucoup plus d’importance à la résolution du symptôme cardinal qui les a amenés à consulter. Cette dernière étude a également permis une exploration plus large des objectifs et attentes, ainsi que de l’expérience patient en ce qui a trait à la RSC et à la CES. En conclusion, l’évaluation des aspects subjectifs devrait être l’aspect le plus important que les cliniciens évaluent pour parler de succès. Une évaluation de la cavité nasale et des sinus devrait complémenter cette évaluation environ 3 mois après la chirurgie. Ce mémoire inclus des algorithmes pour aider les cliniciens dans l’évaluation du succès opératoire après une CES. / Chronic rhinosinusitis (CRS) is a prevalent, complex disease with important economic and individual impacts. Functional endoscopic sinus surgery (FESS) is widely used treatment for CRS, which is considered in patients with an unsatisfactory response to maximal medical therapy. This thesis examines the different ways clinicians may obtain feedback with regards to post-operative success and aims to offer definitions of acceptable success and optimal success. First, a consultation of Canadian experts in CRS from 12 institutions permitted us to construct definitions of acceptable and optimal success from healthcare providers’ viewpoint. Second, a study in collaboration with 22 patients suffering from CRS allowed us to identify key themes which will facilitate the inclusion of primary stakeholders in shared decision-making. The first study allowed us to conclude that a definition of postoperative success must be based on 2 components: an objective aspect (nasal endoscopy) and a subjective aspect (patient-reported outcome measure or specific questionnaire). According to experts, optimal success requires a complete resolution of symptoms as well as a perfect endoscopic result. With the second study, we demonstrated that patients tend to focus on the resolution of their cardinal symptom. This last study also allowed us to widely explore patients’ objectives and expectations, as well as their experience with CRS and FESS. In conclusion, subjective aspects should be the most important determinants of success after FESS. In addition, an evaluation of the nasal cavity and sinuses should complement the subjective evaluation approximately 3 months after surgery. This thesis includes algorithms to aid clinicians in evaluating the outcome of FESS for patients with CRS.

Introdução: A obstrução infravesical (OIV) de longo prazo secundária a hiperplasia prostática benigna (HPB) pode causar alterações funcionais e morfológicas na bexiga. Um dos principais eventos consiste no aumento da deposição de colágeno e perda de complacência vesical, levando a alteração de armazenamento e esvaziamento urinário. O aumento da deposição de colágeno na matriz extracelular (MEC) da musculatura detrusora é a principal razão para a diminuição da complacência vesical. Na bexiga, assim como em outros órgãos, este fenômeno depende da atividade equilibrada de enzimas proteolíticas, incluindo as metaloproteinases (MMP) e os seus inibidores endógenos (inibidores teciduais de metaloproteinases-TIMPs). Como estes fenômenos são desconhecidos na bexiga obstruída, o objetivo deste estudo foi avaliar a expressão gênica de colágeno, MMPs e seus inibidores na bexiga de pacientes com obstrução infravesical. Material e Métodos: Foi realizada uma análise prospectiva e controlada de 43 pacientes com OIV devido a HPB, que foram submetidos à ressecção transuretral da próstata (RTUP) entre 2011 e 2012. Como grupo controle foram selecionados espécimes de músculo detrusor de 10 pacientes que foram submetidos a prostatectomia radical retropúbica devido adenocarcinoma de próstata. Todos estes pacientes tinham idade menor que 60 anos, tamanho de próstata menor que 30 gramas ao ultra-som e escore internacional de sintomas prostáticos (IPSS) menor que 7. Todos os pacientes foram submetidos a estudo urodinâmico pré e pós operatório (após 6 meses). A biópsia de fragmento de músculo da bexiga foi realizada ao final da RTUP e colocada em solução estabilizadora de RNA para quantificação da expressão de colágenos I e III, metaloproteinases de matriz 1, 2 e 9, e inibidores de MMPs (TIMP1, TIMP2 e RECK) na bexiga de pacientes com HPB. Os genes descritos foram avaliados através da técnica de reação em cadeia da polimerase quantitativa em tempo real (qRT-PCR). Resultados: Todos os pacientes com HPB tinham confirmado OIV, através da análise do estudo urodinâmico (média de pressão detrusora no fluxo máximo de 78,5 cmH2O e fluxo urinário máximo de 7,7 ml / s). O gene MMP1 mostrou-se superexpresso em pacientes com HPB (mediana = 1,87). MMP9, TIMP1 e RECK estavam subexpressos na maioria dos casos, enquanto TIMP2, colágeno I e III foram superexpressos (1,5, 4,4 e 1,9 vezes, respectivamente). No que diz respeito às características clínicas e urodinâmicas encontramos que MMP2 foi mais expresso entre pacientes com um baixo IPSS global (0,005) e sem urgência (p=0,035). Colágeno III foi mais expresso em pacientes com contrações vesicais não inibidas (p = 0,049). Os outros genes não mostraram nenhuma correlação estatística com quaisquer características clínicas ou urodinâmicas. Após 6 meses de RTU, pacientes que possuíam expressão aumentada de duas ou mais MMPs, apresentaram resolução da CNI em 66,6% dos casos, contra 14,0% quando apenas uma ou nenhuma MMP estava aumentada (p=0,038) Conclusões: Encontramos um perfil de superexpressão de MMP1, TIMP2, colágenos I e III, e expressão baixa de MMP9, TIMP1 e RECK nos pacientes com OIV. Considerando o escore de sintomas prostáticos e a urgência miccional, encontramos curiosamente uma maior expressão de MMP2 em pacientes menos sintomáticos e sem urgência miccional. Encontramos uma associação entre a maior expressão de colágeno III com HD. In the bladder, as in other organs, this phenomenon is dependent on the balanced activity of proteolytic enzymes, including matrix metalloproteinases (MMPs) and their endogenous inhibitors, tissue inhibitors of metalloproteinases (TIMPs). The imbalance between MMPs and TIMPs is a key regulator in ECM turnover. Since these mechanisms are unknown in the obstructed bladder, the objective of this study was to evaluate gene expression of collagen, MMPs and their inhibitors in patients with bladder outlet obstruction due to BPH. MMP1 gene showed an important an overexpression in patients with BPH (median = 1.87). A similar phenomenon occurred in a lesser extent to MMP2, to which 13 of 23 subjects had under-expression (mean = 1.2). MMP9, TIMP1 and RECK were under-expressed in the majority of cases, while TIMP2, colagen I and III were over-expressed (1.5, 4.4 and 1.9x respectively) (figure). With regard to clinical and urodynamic characteristics we found that MMP2 was more over-expressed among patients with a low global IPSS (0.005) and without urgency (p=0.035). Colagen III was more over-expressed in patients with non-inhibited bladder contractions (p=0.049), RECK was more over-expressed in patients with a decreased complacence (p=0.049). The other genes showed no statistical correlation with any clinical or urodynamic characteristics. Detrusor overactivity is related with higher collagen III expression, this fact may be due to a lower MMP1 expression. A lower global IPSS and no urgency were related to a higher expression of MMP2, sugesting that this gene may be inhibiting collagen deposition in the bladder.
