Implication des biofilms dans la rhinosinusite chronique et l’évaluation des traitements avec un modèle in vitro

Introduction : La chronicité de la rhinosinusite, sa résistance aux antibiotiques, et ses exacerbations aiguës laissent croire que les biofilms sont impliqués dans la rhinosinusite chronique. Objectifs : Nous avons évalué la capacité des bactéries Pseudomonas aeruginosa, staphylocoques à coagulase négative et Staphylococcus aureus à former des biofilms par un essai in vitro, et si cette capacité de formation a un lien avec l’évolution de la maladie. Nous avons évalué in vitro l’effet de la moxifloxacine, un antibiotique utilisé dans le traitement de la rhinosinusite chronique sur des biofilms matures de Staphylococcus aureus. Méthodes : Trent et une souches bactériennes ont été isolées de 19 patients atteints de rhinosinusite chronique et qui ont subit au moins une chirurgie endoscopique des sinus. L’évolution de la maladie a été notée comme "bonne" ou "mauvaise" selon l’évaluation du clinicien. La production de biofilm a été évaluée grâce à la coloration au crystal violet. Nous avons évalué la viabilité du biofilm après traitement avec la moxifloxacine. Ces résultats ont été confirmés en microscopie confocale à balayage laser et par la coloration au LIVE/DEAD BacLight. Résultat et Conclusion : Vingt deux des 31 souches ont produit un biofilm. La production d’un biofilm plus importante chez Pseudomonas aeruginosa et Staphylococcus aureus était associée à une mauvaise évolution. Ceci suggère un rôle du biofilm dans la pathogenèse de la rhinosinusite chronique. Le traitement avec la moxifloxacine, à une concentration de 1000X la concentration minimale inhibitrice réduit le nombre des bactéries viables de 2 à 2.5 log. Ces concentrations (100 µg/ml - 200 µg/ml) sont faciles à atteindre dans des solutions topiques. Les résultats de notre étude suggèrent que l’utilisation de concentrations supérieure à la concentration minimale inhibitrice sous forme topique peut ouvrir des voies de recherche sur de nouveaux traitements qui peuvent être bénéfiques pour les patients atteints de forme sévère de rhinosinusite chronique surtout après une chirurgie endoscopique des sinus. / Introduction: The role of biofilms in chronic diseases is increasingly recognized. Chronic rhinosinusitis, with its chronic indolent course, resistance to antibiotics, and acute exacerbations, has an evolution that parallels that of other biofilm-related diseases. Objectives: 1-To develop an in vitro method to assess the biofilm formation capacity. 2- To determine whether biofilm-forming capacity of bacteria demonstrated in chronic rhinosinusitis has an impact on persistence of the disease following endoscopic sinus surgery. 3- To determine the in vitro activity of moxifloxacin against Staphyylococcus aureus in biofilm form. Method: Thirty-one bacterial strains recovered from 19 patients with chronic rhinosinusitis at least one year post-endoscopic sinus surgery. Evolution of disease was assessed by questionnaire and endoscopy as favorable or unfavorable. The bacteria were cultured on a 96-well culture plaque and a semi-quantitative method using crystal violet to quantify biofilm production was used. Confirmation of the effect of the antimicrobial agents on viability was performed with confocal laser microscopy, using a LIVE/DEAD BacLight staining. Results: Twenty-two of 31 samples produced a biofilm thicker or equal to the positive control. Biofilm formation was associated with a poor evolution for Pseudomonas aeruginosa and Staphylococcus aureus, but not for coagulase-negative staphylococci. Biofilm treated with moxifloxacin at 1000X (0.1mg/ml – 0.2 mg/ml) gave a 2 to 2.5 log reduction in number of viable bacteria. Conclusion: We have shown that Crystal violet method is able to detect biofilm formation. There is a correlation between in vitro biofilm production by Pseudomonas aeruginosa and Staphylococcus aureus and unfavorable evolution after endoscopic sinus surgery, suggesting a role for biofilm in chronic rhinosinusitis. Increased concentrations of moxifloxacin, easily attainable in topical solutions have a potential role in the management of biofilm infections.

Ο στόχος αυτής της εργασίας είναι να αποκτήσουμε μία βασική γνώση όλων των διαφορετικών σχεδιαστικών παραμέτρων που πρέπει να εξεταστούν για να είναι εφικτή η κατασκευή και ο έλεγχος ενός πολυαρθρωτού εργαλείου. Επιπλέον, όλες οι αναλυτικές μέθοδοι ελέγχου που βασίζονται στις ιδιαιτερότητες των SMA παρουσιάζονται λεπτομερώς, ώστε να παραχθεί μία ικανοποιητική λύση βασιζόμενη στις μεταβολές κατάστασης των κραμάτων και του συγκεκριμένου βραχίονα. Με άλλα λόγια, μία πλήρης γνώση του πώς σχεδιάζουμε, κατασκευάζουμε, προσομοιώνουμε, ελέγχουμε και απεικονίζουμε ένα λειτουργικό μικροσκοπικό πολυαρθρωτό βραχίονα, με τένοντες βασισμένους σε SMA για ελάχιστα επεμβατική χειρουργική είναι ο στόχος της παρούσας εργασίας. / The purpose of this work is to acquire a fundamental knowledge of all the different design parameters, which must be evaluated in order to be able to fabricate and control a multi-DOF manipulator. Moreover, all the analytical control techniques based on the particularities of the shape memory alloys will be shown in details, in order to provide an efficient solution based on the variations of the alloys and the specific manipulator. In other words, the knowhow of building, evaluating, controlling and displaying a functional tiny multi- DOF SMA-based manipulator for minimally invasive surgery is the purpose of this work.

Η παρούσα διπλωματική εργασία πραγματεύεται την ανάπτυξη ενός αποδοτικού συστήματος ελέγχου που θα επιτυγχάνει τον ακριβή έλεγχο της θέσης ενός λαπαροσκοπικού ρομποτικού εργαλείου που χρησιμοποιεί ‘έξυπνα μορφομνήμονα κράματα’ ως τένοντες-επενεργητές. Το γεγονός ότι το εργαλείο που χρησιμοποιείται είναι με πλεονάζοντες βαθμούς ελευθερίας, καθιστά ιδιαίτερα δύσκολο πρόβλημα την επίλυση της αντίστροφης κινηματικής. Στόχος της παρούσας εργασίας είναι, μέσω τεχνικών υπολογιστικής όρασης, να επιλυθεί προσεγγιστικά η αντίστροφη κινηματική ανάλυση της θέσης των αρθρώσεων του ρομποτικού εργαλείου. Ο προτεινόμενος νόμος ελέγχου χρησιμοποιεί την αριθμητική επίλυση των γωνιών των αρθρώσεων από την αντίστροφη κινηματική για την επιτυχή παρακολούθηση της επιθυμητής τροχιάς του ρομποτικού εργαλείου. / This thesis deals with the development of a robust control system responsible for the precise position control of an innovative, SMA-based tendon-driven endoscopic robotic surgical tool. Given the hyper-redundant features of this robotic tool, the solution to the inverse kinematics problem is quite complicated. Henceforth, the main objective of this thesis is the numerical solution of the joints' positions using image processing techniques. The proposed control law utilizes this information for trajectory tracking purposes of the tool's end-effector.

Avaliar o efeito dos aspectos nutricionais na sobrevida de pacientes com Doença do Neurônio Motor (DNM) e apresentar as variáveis preditivas para a indicação de terapia nutricional enteral, por gastrostomia endoscópica percutânea (GEP). Material e Métodos. Foi um estudo longitudinal tipo coorte retrospectiva, de 2000 a 2008, e a casuística constituída por 128 pacientes com DNM. Variáveis clínicas, nutricionais e respiratórias foram analisadas. As análises foram conduzidas adotando-se a sobrevida como variável dependente. A sobrevida foi avaliada pela Curva de Kaplain - Meier. As variáveis que apresentaram nível de significância de 20% (p< 0,20) foram selecionadas para o modelo de regressão proporcional de Cox. Resultados. Cento e onze pacientes realizaram a gastrostomia, sendo 59 com a forma apendicular (ELA) e 52 com a forma bulbar (PBP). A desnutrição estava presente em 32% da população antes da GEP, com maior frequência nos pacientes com ELA. O tempo de sobrevida após a GEP foi de 11 meses para os pacientes com PBP e 16 meses para ELA (p< 0,05). As variáveis associadas à sobrevida foram: precocidade na indicação da GEP; redução de CVF %, idade e IMC antes da GEP (hazard ratio de 0, 254 e p = 0, 007) para os pacientes com ELA e exclusão da alimentação por via oral e traqueostomia (hazard ratio de 0, 345 e p= 0, 014) para os pacientes com PBP. Ao final do modelo as variáveis mais associadas com a sobrevida foram precocidade na indicação de GEP, exclusão da alimentação por via oral, para os pacientes com PBP e estado nutricional antes da GEP para os pacientes com ELA. Conclusões. A inserção precoce de gastrostomia endoscópica percutânea, a partir do momento diagnóstico, foi fator protetor para a sobrevida dos pacientes. A desnutrição foi fator prognóstico ruim, especialmente para os pacientes com ELA. Vigilância nutricional durante a evolução da doença pode melhorar os resultados quando o objetivo é aumentar a sobrevida de pacientes com DNM/ELA. / Aims. To evaluate the effect of nutrition on survival of patients with Motor Neurone Disease (MND) and present the predictor variables for indications of nutritional therapy, percutaneous endoscopic gastrostomy (PEG). Methods. It was a retrospective longitudinal cohort study, from 2000 to 2008, and the sample consisted of 128 patients with MND. The variables investigated were clinical, nutritional and respiratory were analysed. Analyses were conducted by adopting the survival as the dependent variable. The survival curve was evaluated by Kaplain - Meier. The variables that had a significance level of 20% (p <0.20) were selected for the proportional regression model of Cox. Results. One hundred and eleven patients underwent gastrostomy, and 59 limb onset (ALS) and 52 with bulbar onset (PBP). Malnutrition was present in 32% of the population before PEG, most frequently in patients with limb onset. The survival time after PEG was 10.5 months for patients with PBP and 16 months for ALS (p <0.05). Variables associated with survival were: early indication in the PEG, for ALS and PBP; reduction of FVC% and BMI before PEG (hazard ratio of 0, 254, p = 0, 007) for patients with limb onset and exclusion of oral feeding and tracheostomy (hazard ratio of 0, 345, p = 0, 014) for patients with bulbar onset. Conclusions. Early insertion of percutaneous endoscopic gastrostomy, from the time diagnosis was a protective factor for patient survival. Malnutrition was a bad prognostic factor, especially for patients with limb onset. A perfuração gástrica por endoscopia é a consequência de alguns de seus procedimentos, e atualmente, com o advento das cirurgias endoscópicas translumenais por orifícios naturais, um meio de manipulação dos órgãos abdominais. Esse é o motivo pelo qual os endoscopistas estão procurando um reparo endoscópico seguro. O objetivo foi avaliar a factibilidade e os resultados do fechamento da abertura gástrica similar àquelas realizadas nos procedimentos cirúrgicos endoscópicos translumenais por orifícios naturais utilizando-se o T-Tag associado à câmara plástica protetora. Sob anestesia geral, dez porcos Landrace foram submetidos a uma perfuração gástrica calibrada em 18 mm de diâmetro. A abertura foi fechada pelo novo método apresentado, composto de fios cirúrgicos conectados em uma âncora metálica (T-Tag) posicionados pela parede gástrica através de uma agulha. Uma câmara protetora plástica foi adaptada à extremidade distal do endoscópio para proteger os órgãos abdominais adjacentes da punção da agulha fora do estômago. Seis dispositivos T-Tag foram posicionados na maioria dos casos e os fios atados com um apertador de nó metálico endoscópico formando três pontos de sutura. O teste de vazamento foi realizado com uma pinça endoscópica e distensão da câmara gástrica com ar. Os animais receberam líquidos no mesmo dia do procedimento. Uma dose de antibiótico diária por dois dias foi administrada. Nenhuma complicação foi detectada no período pós-operatório. Um mês depois, a endoscopia revelou a presença de cicatriz em todos os animais, e a maioria apresentava materiais da sutura aderidos à superfície mucosa da região. A região do antro gástrico apresentava poucas aderências identificadas na laparotomia realizada no mesmo momento. O reparo endoscópico utilizando o T-Tag e a câmara protetora plástica é factível, fácil de ser realizada e seguro. São necessários estudos adicionais para mostrar o real valor desse tipo de procedimento / The endoscopic gastric perforation is a consequence of some endoscopic procedures and now a way to manage abdominal organs. This is the reason why endoscopists are studying a safe endoscopic repair. The objective was to evaluate feasibility and results of the gastric opening closure similar to those performed in natural orifice translumenal endoscopic surgery procedures using T-Tag associated with the plastic protection chamber. Ten Landrace pigs underwent a gastric perforation of 1.8 cm in diameter under general anesthesia. The opening was repaired with stitch assembled in a T-Tag anchor placed through the gastric wall with a needle. A plastic transparent chamber, adapted to the endoscope tip protected the abdominal organs from the needle puncture outside the stomach. Six T-Tags were placed in most cases and the stitches were tied with a metallic tie-knot, forming three sutures. The leakage test was performed with a forceps and by air distention. The animals received liquids in the same operative day. One daily shot antibiotic during two days was used. No complication was detected in the postoperative course. One month later the endoscopy revealed a scar in all animals, and the majority of these with suture material. The antral anterior gastric wall was clear with few adhesions in the laparotomy performed in the same time. The endoscopic repair using T-Tag and a protector chamber is feasible, easy to perform and safe. Further studies are needed to show the real value of this kind of procedure

Impacto da microcirurgia endoscópica transanal sobre a função anorretal: avaliação clínica, funcional e da qualidade de vida / Impact of transanal endoscopic microsurgery on anorectal function: a prospective clinical, functional, and quality of life investigation before and after surgery

Introdução: Descrita em 1983 e de sólida aplicação clínica, o impacto da microcirurgia endoscópica transanal (TEM) sobre a função anorretal permanece pouco conhecido. Os objetivos do presente estudo foram avaliar o impacto da TEM na função anorretal conforme avaliações clínicas (Wexner score) e funcional (manometria anorretal) antes e após a cirurgia. Método: Prospectivamente, 23 pacientes consecutivos com lesões retais foram operados com o uso do equipamento TEO® (Karl Storz, Tuttlingen, Alemanha). Para todos os pacientes, o valor do escore de Wexner foi obtido antes e após a cirurgia (7, 30 e 90 dias), e a eletromanometria anorretal foi realizada antes da cirurgia e também no pós-operatório (30 e 90 dias). Resultados: Quatorze pacientes eram homens. A idade média foi 53,7 (24-81) anos. A distância média da lesão à linha pectínea foi de 7 (2-15) cm. A histopatologia revelou adenoma em 14 (61%), tumor neuroendócrino em 5 (21,7%), carcinoma invasivo em 3 (13%) e pólipo hiperplásico em 1 (4,3%) caso. A duração média do seguimento pós-operatório foi de 5 (3-7) meses. O escore de Wexner foi significativamente menor aos 30 dias em comparação com 7 dias (Wilcoxon, p = 0,03). A capacidade retal foi significativamente menor aos 30 dias após a cirurgia e recuperada aos 90 dias após a cirurgia (ANOVA, p = 0,04). Conclusões: Após TEM, um impacto modesto na função anorretal pode ser observado. O comprometimento transitório resulta de perda de capacidade retal e não por comprometimento dos esfíncteres anais cessando completamente 90 dias após a cirurgia. Em última análise, não conseguimos detectar um impacto na qualidade de vida após TEM / Background: The impact of transanal endoscopic microsurgery (TEM) on anorectal function remains poorly available, particularly when considering that the technique involves undertaking full- or partial-thickness excision of the rectal wall. Moreover, in spite of wide adoption of TEM, its impact on quality of life remains unknown since most evidence derives from retrospective studies. Objective: The objectives of the present study were to evaluate the impact of TEM on sphincter function determined by clinical (Wexner score), functional (anorectal manometry), and quality of life (FIQL) evaluations conducted before and after surgery. Design: prospective, observational, single-center, 23 consecutive patients with rectal lesions underwent were operated on using the TEO® equipment (Karl Storz, Tuttlingen, Germany). Wexner and FIQL scores were obtained before and after surgery (7 days, 30 days and 90 days postoperatively). Anorectal manometry was obtained before surgery, and postoperatively after 30 and 90 days. Main Outcome Measures: Wexner and FIQL scores; anorectal manometry results. Results: Fourteen patients were men. Mean age was 53.7 (24-81) yrs. Mean distance from the lesion to the dentate line was 7 (2-15) cm. A full- thickness resection was undertaken in 18 (78.3%) cases. Histopathology revealed adenoma in 14 (61%), neuroendocrine tumor in 5 (21.7%), invasive carcinoma in 3 (13%), and hyperplastic polyp in 1 (4.3%) case. Postoperative rectal wound separation occurred in 2 patients and 1 patient developed atrial fibrillation. The mean duration of postoperative follow-up was 5 (3-7) months. Overall, Wexner score significantly declined between postoperative days 7 and 30 (Wilcoxon, p = 0.03). Rectal compliance exhibited significant decline 30 days after surgery and recovery at 90 days after surgery (ANOVA, p = 0.04). It was not possible to measure any difference in the FIQL results before and after surgery. Limitations: small sample size; limited follow-up. Conclusions: Following TEM, a modest impact on anorectal function could be confirmed. Interestingly, anorectal function impairment after surgery was not due to sphincter dysfunction, but resulted from loss of rectal compliance. Ultimately, we could not detect a significant impact on quality of life after TEM

Comparação entre prótese plástica e metálica na paliação endoscópica da obstrução maligna da via biliar: revisão sistemática e metanálise baseadas em estudos randomizados / Endoscopic stenting for inoperable malignant biliary obstruction: a systematic review and meta-analysis

INTRODUÇÃO: A prótese biliar endoscópica é aceita em todo o mundo como a primeira escolha de tratamento paliativo na obstrução biliar maligna. Atualmente ainda persistem dois tipos de materiais utilizados em sua confecção: plástico e metal. Consequentemente, muitas dúvidas surgem quanto a qual deles é o mais benéfico para o paciente. Esta revisão reúne as informações disponíveis da mais alta qualidade sobre estes dois tipos de prótese, fornecendo informações em relação à disfunção, complicação, taxas de reintervenção, custos, sobrevida e tempo de permeabilidade; e pretende ajudar a lidar com a prática clínica nos dias de hoje. OBJETIVO: Analisar, através de metanálise, os benefícios de dois tipos de próteses na obstrução biliar maligna inoperável. MÉTODOS: Uma revisão sistemática de ensaios clínicos randomizados (RCT) foi conduzida, com a última atualização em março de 2015, utilizando EMBASE, CINAHL (EBSCO), Medline, Lilacs / Centro (BVS), Scopus, o CAPES (Brasil), e literatura cinzenta. As informações dos estudos selecionados foram extraídas tendo em vista seis desfechos: primariamente disfunção, taxas de reintervenção e complicações; e, secundariamente, custos, sobrevivência e tempo de permeabilidade. Os dados sobre as características dos participantes do RCT, critérios de inclusão e exclusão e tipos de próteses também foram extraídos. Os vieses foram avaliados principalmente através da escala de Jadad. Esta metanálise foi registrada no banco de dados PROSPERO pelo número CRD42014015078. A análise do risco absoluto dos resultados foi realizada utilizando o software RevMan 5, calculando as diferenças de risco (RD) de variáveis dicotômicas e média das diferenças (MD) de variáveis contínuas. Os dados sobre a RD e MD para cada desfecho primário foram calculados utilizando o teste de Mantel-Haenszel e a inconsistência foi avaliada com o teste Qui-quadrado (Chi2) e o método de Higgins (I2). A análise de sensibilidade foi realizada com a retirada de estudos discrepantes e a utilização do efeito aleatório. O teste t de Student foi utilizado para a comparação das médias aritméticas ponderadas, em relação aos desfechos secundários. RESULTADOS: Inicialmente foram identificados 3660 estudos; 3539 foram excluídos por título ou resumo, enquanto 121 estudos foram totalmente avaliados e foram excluídos, principalmente por não comparar próteses metálicas (SEMS) e próteses plásticas (PS), levando a treze RCT selecionados e 1133 indivíduos metanálise. A média de idade foi de 69,5 anos, e o câncer mais comum foi de via biliar (proximal) e pancreático (distal). O diâmetro de SEMS mais utilizado foi de 10 mm (30 Fr) e o diâmetro de PS mais utilizado foi de 10 Fr. Na metanálise, SEMS tiveram menor disfunção global em comparação com PS (21,6% versus 46,8% p < 0,00001) e menos reintervenções (21,6% versus 56,6% p < 0,00001), sem diferença nas complicações (13,7% versus 15,9% p = 0,16). Na análise secundária, a taxa média de sobrevida foi maior no grupo SEMS (182 contra 150 dias - p < 0,0001), com um período maior de permeabilidade (250 contra 124 dias - p < 0,0001) e um custo semelhante por paciente, embora menor no grupo SEMS (4.193,98 contra 4.728,65 Euros - p < 0,0985). CONCLUSÃO: SEMS estão associados com menor disfunção, menores taxas de reintervenção, melhor sobrevida e maior tempo de permeabilidade. Complicações e custos não apresentaram diferença / INTRODUCTION: Endoscopic stenting is accepted worldwide as the first choice palliative treatment for malignant biliary obstruction. There are still two types of materials currently being used, which are plastic and metal. Therefore, many doubts are raised as to which one is the most beneficial to the patient. This review gathers the highest quality information available about these two types of stent, giving information in regards to dysfunction, complication, reintervention rates, costs, survival, and patency time; and intends to help handle clinical practice nowadays. OBJECTIVE: To analyze through meta-analyses the benefits of two types of stents in the inoperable malignant biliary obstruction. METHODS: A systematic review of randomized clinical trials (RCT) was conducted, with the last update on March 2015, using EMBASE, CINAHL (EBSCO), MEDLINE, LILACS/CENTRAL (BVS), SCOPUS, CAPES (Brazil), and gray literature. Information of the selected studies was extracted in sight of six outcomes: primarily regarding dysfunction, reintervention and complication rates; and secondarily costs, survival, and patency time. The data about characteristics of trial participants, inclusion and exclusion criteria and types of stents were also extracted. The biases were mainly assessed through the Jadad scale. This meta-analysis was registered in the PROSPERO database by the number CRD42014015078. The analysis of the absolute risk of the outcomes was performed using the software RevMan 5, by computing risk differences (RD) of dichotomous variables and mean differences (MD) of continuous variables. Data on RD and MD for each primary outcome were calculated using the Mantel-Haenszel test and inconsistency was qualified and reported in Chisquared (Chi2) and the Higgins method (I2). Sensitivity analysis was performed withdrawing discrepant studies and using random effect. Student\'s t-test was used for the comparison of weighted arithmetic means regarding secondary outcomes. RESULTS: Initial searching identified 3660 studies; 3539 were excluded through title or abstract, while 121 studies were fully assessed and were excluded mainly because they did not compare Self Expanding Metal Stents (SEMS) and Plastic Stents (PS), leading to thirteen RCT selected and 1133 subjects meta-analyzed. The mean age was 69.5 years old, that were affected mostly by bile duct (proximal) and pancreatic tumors (distal). The preferred SEMS diameter used was the 10 mm (30 Fr) and the preferred PS diameter used was 10 Fr. In the meta-analysis, SEMS had lower overall stent dysfunction compared to PS (21.6% versus 46.8% p < 0.00001) and fewer reintervention (21.6% versus 56.6% p < 0.00001), with no difference in complications (13.7% versus 15.9% p=0.16). In the secondary analysis, the mean survival rate was higher in the SEMS group (182 versus 150 days - p < 0.0001), with a higher patency period (250 versus 124 days - p < 0.0001) and a similar cost per patient, although lower in the SEMS group (4193.98 versus 4728.65 Euros - p < 0.0985). CONCLUSION: SEMS are associated with lower stent dysfunction, lower reintervention rates, better survival, and higher patency time. Complications and costs do not show difference

Dans cette thèse, nous avons travaillé sur les différents versants de la technique de dissection sous-muqueuse et les problèmes que pose ce geste quasi chirurgical dans des unités d'endoscopie initialement médicales. Tout d'abord, nous avons travaillé sur la formation à la technique en développant un modèle d'apprentissage sur colon de bovin plus adapté à la situation européenne où les lésions colo-rectales sont les plus fréquentes. Ce modèle de rectum de bovin, simple à trouver et à préparer permet une formation dans des conditions plus proches de la paroi colique humaine que celles offertes par l'estomac de cochon. Un travail à plus grande échelle évaluant les bénéfices d'une aide à l'apprentissage par un logiciel interactif dédié mené sur ce modèle avec 37 étudiants français et japonais est en cours d'analyse et sera publié prochainement. Ensuite, nous avons réfléchi à la stratégie de la procédure pour la rendre plus simple en évaluant précocement la technique du tunnel pour la dissection des lésions œsophagiennes. Cette stratégie permet de maintenir une traction sur les bords lésionnels et nous offrent une sorte de triangulation en élargissant physiquement la zone de travail. Cette stratégie est devenue un standard pour les résections œsophagiennes dans de nombreuses équipes. Enfin, nous avons travaillé conjointement avec la société Nestis® au développement d'un outil permettant d'optimiser la procédure de dissection sous-muqueuse en associant les bénéfices des bistouris bi fonction (injectant et coupant avec le même outil}, de la haute pression pulsée et des solutions macromoléculaires visqueuses. Le système Nestis® permet pour la première fois cette association et a démontré son intérêt en termes de sécurité et de performance par rapport à la méthode classique utilisant l'aiguille et un bistouri électrique conventionnel. Avec cet outil bi fonction, il n'est plus nécessaire de changer d'instrument puisque toutes les étapes de la procédure sont désormais réalisées avec un seul et même outil. D'autres projets sont déjà prévus avec ce matériel pour étudier ses bénéfices et sa sécurité en dissection colique humaine qui est réputée comme la plus difficile compte tenu de la finesse de la paroi. Enfin, ce matériel offre la possibilité d'injecter sous pression des principes actifs qui pourrait dans le futur permettre de prévenir la survenue de sténoses œsophagiennes ou diriger la cicatrisation. Nous avons ainsi travailler avec la pharmacie de l'hôpital Edouard Herriot pour stabiliser la solution macromoléculaires de mélange de glycérol pour permettre son utilisation en pratique quotidienne / First of all, we worked on the training for unexperienced operators by developing a bovine colon model more adapted to the European situation where colo-rectal lesions are the most common. This model of rectum from bovine, easy to find and to prepare allows training in conditions most close to the human colonic wall than those offered by the pig stomach. Furthermore, such models allows to teach the initial skills but avoiding the risk of adverse events for the first procedures in humans. A future work evaluating the benefits of a learning support by a dedicated interactive software on this model with 37 french and Japanese students is now being analyzed and will be reported soon. Then we thought about the strategy of the procedure in order to make it more simple using the tunnel technique to perform ESD for the esophageal lesions. This strategy helps to maintain traction on the edges and offers a sort of triangulation physically expanding the working space. This strategy has become a standard for esophageal resections in many teams and we still work to improve its efficacy. Finally, we worked jointly with Nestis® Company to develop a tool to optimize the submucosal dissection procedure by combining the benefits of the catheters bi function (injecting and cutting with the same tool), but adding high pulsed pressure and capability to inject viscous macromolecular solutions. The Nestis® system allows for the first time this association and demonstrated his interest in terms of security and performance compared with the conventional method using the needle and a conventional electrocautery device. With this bi function tool, it is not necessary to change instrument frequently since all stages of the procedure are now done with a single device. Other projects are already included with this material to explore its benefits and its safety in human colonic dissection that is deemed as the most difficult due to the thinner wall. Finally, this material offers the possibility to inject pressurized active drugs which could be used in the future to prevent the occurrence of esophageal strictures or to direct healing. We also worked with the hospital Edouard Herriot pharmacy to stabilize the solution glycerol mix to allow its use in daily practice in our unit

In vivo imaging in the oral cavity by endoscopic optical coherence tomography

The common way to diagnose hard and soft tissue irregularities in the oral cavity is initially the visual inspection by an experienced dentist followed by further medical examinations, such as radiological imaging and/or histopathological investigation. For the diagnosis of oral hard and soft tissues, the detection of early transformations is mostly hampered by poor visual access, low specificity of the diagnosis techniques, and/or limited feasibility of frequent screenings. Therefore, optical noninvasive diagnosis of oral tissue is promising to improve the accuracy of oral screening. Considering this demand, a rigid handheld endoscopic scanner was developed for optical coherence tomography (OCT). The novelty is the usage of a commercially near-infrared endoscope with fitting optics in combination with an established spectral-domain OCT system of our workgroup. By reaching a high spatial resolution, in vivo images of anterior and especially posterior dental and mucosal tissues were obtained from the oral cavity of two volunteers. The convincing image quality of the endoscopic OCT device is particularly obvious for the imaging of different regions of the human soft palate with highly scattering fibrous layer and capillary network within the lamina propria.

Esophageal varices are expanded veins of the submucosa that develop in patients with portal hypertension. They develop as collaterals between the portal vein and the superior vena cava. Varices are seen when the HPVG rises >12mmHG and can lead to a life-threatening bleeding episode. Endoscopic band ligation (EBL) is the treatment of choice of acute variceal bleeding. It is also performed for primary and secondary prophylaxis of bleeding from esophageal varices. After EBL, patients are at risk of postinterventional bleeding. Therefore, patients are often hospitalized until endoscopy proves all applied ligation bands have dropped off. At present, there is no standardized algorithm for surveillance of patients after EBL. Furthermore, risk factors for bleeding complications after EBL are poorly evaluated. The available studies mostly investigated patient collectives .after endoscopic sclerotherapy. The aim of this study was to investigate bleeding behaviour after EBL, to make recommendations for in- and out-patient surveillance after EBL and to analyze independent risk factors for bleeding complications after EBL.
