Valor da hipertermia no tratamento da hiperplasia benigna da prostata

A hipertermia transuretral da próstata surgiu como um método novo, executado com anestesia local e usando a energia de microondas para produzir temperaturas em tomo de 45°C. Os resultados descritos até o momento apresentam índices de melhora conflitantes. O objetivo do estudo foi verificar o valor da hipertermia transuretral no tratamento de pacientes com fator obstrutivo infravesical devido à HBP. O estudo incluiu 20 pacientes com obstrução infravesical causada pela HBP. Eles foram submetidos à avaliação subjetiva e objetiva, esta incluiu estudo urodinâmico, na seleção dos pacientes e dois meses após cada procedimento. Os pacientes foram randomizados em dois grupos. Este trabalho comparou os efeitos da hipertermia com os do grupo controle. Não houve diferença estatística significativa (p>0,05) nas medidas de obstrução infravesical entre o grupo que recebeu hipertermia e o controle. Nenhum paciente saiu da situação de obstrução infravesical em que se encontrava antes do estudo. A hipertermia, dentro dos parâmetros delineados neste estudo, não foi eficaz no tratamento da HBP, pois não houve mudança significativa nas medidas de obstrução infravesical dos pacientes analisados e os resultados foram semelhantes aos do controle / Abstract: Many attempts have been made to develop an efficacious, low-cost and minimally invasive method for the treatment of benign prostatic hyperplasia (BPH). Transurethral hyperthermia involving the use of microwave energy to produce a temperature of around 45 C has emerged as a new, fast, procedure. anesthesia-free outpatient. The aim of this work was to verify the effectiveness of hyperthermia in the treatment of patients with BPH. This work compared the effects of microwave hyperthermia to sham treatment. The trial included 20 patients with proven symptomatic bladder outflow obstruction (BOa) caused by BPH. For the study, the patients were randomly assigned to two groups, one of which received one hour of microwave hyperthermia treatment while the other served as the control. Each patient underwent a full subjective and objective assessment, including urodynamics, at the time of selection and again two months after every procedure. There was no significant difference (p>0.05) in the objective measures of BOa between the treated and control group. Based on the urodynamics results, all of the patients were still obstructed postoperatively. Within the parameters defined in this trial we conclude that microwave hyperthermia was not efficacious in the treatment of BPH, without improve the BOa, with results similar to control / Mestrado / Cirurgia / Mestre em Cirurgia

Prevalência de hiperatividade detrusora em crianças com hiperatividade vesical

MATERIAL E MÉTODOS: Foram avaliadas 37 crianças, sendo 27 meninas, com idade de 4-15 anos (mediana de 8,0 anos), com quadro de urgência miccional, urofluxometria com curva em “sino”, ausência ultrassonográfica de resíduo pós-miccional e submetidas a estudo urodinâmico completo. Realizada a correlação entre hiperatividade vesical e o diagnóstico de hiperatividade detrusora idiopática na amostra analisada. RESULTADOS: De um total de 37 crianças avaliadas, 26 (70,3%) apresentaram hiperatividade detrusora. Em 24 crianças com incontinência urinária diurna, 18 (75% - p = 0,313) apresentaram hiperatividade detrusora. Gênero feminino (77,8% - p = 0,110), constipação (86,7% - p = 0,073), manobras posturais (84,2% - p = 0,060) e dor hipogástrica (78,3% - p = 0,161) foram variáveis associadas à hiperatividade detrusora, mais próximas da significância estatística. Foi observada associação significativa entre infecção urinária e o número de contrações involuntárias acima de 3 (89,5%; p = 0,028). Foi identificado elevado índice de discrepância entre os resultados da capacidade máxima e média do diário miccional, em relação à capacidade cistométrica máxima, estimada no estudo urodinâmico. CONCLUSÕES: Este estudo demonstrou uma elevada prevalência de hiperatividade detrusora idiopática em crianças com hiperatividade vesical.

Rizotomia sacral percutanea por radiofrequencia no tratamento da hiperatividade detrusora de origem neurogenica

O tratamento com utilização de drogas anticolinérgicas tem sido a primeira opção no tratamento desses casos. O insucesso do tratamento clínico leva à necessidade de se indicar o tratamento cirúrgico, que inclui a enterocistoplastia, a auto-ampliação vesical e a rizotomia sacra!. O objetivo do estudo foi avaliar a técnica da rizotomia sacral percutânea por meio de radiofreqüência (RF) com utilização de eletrodo 22G e verificar os efeitos na capacidade vesical e na pressão do detrusor no seguimento de 12 meses, em pacientes com lesão medular refratários ao tratamento clínico. Foram avaliados 12 pacientes portadores de lesão medular (7 mulheres e 5 homens) com idades variando de 22 a 58 anos (mediana de 33 anos). Cinco pacientes apresentavam lesão medular em nível cervical, cinco em nível torácico e dois em nível lombar. Dez pacientes apresentavam lesão medular completa e dois, lesão incompleta. O intervalo entre a ocorrência da lesão e a data do procedimento variou de 8 a 96 meses (mediana de 60 meses). Todos os pacientes apresentavam hiperatividade detrusora e foram submetidos ao bloqueio anestésico bilateral da terceira raiz sacral, usando 3ml de bupivacaína a 0,5 %, sem adrenalina, controlado por fluoroscopia. Quatro pacientes foram excluídos do estudo porque não apresentaram aumento superior a 100% da capacidade vesical após esse procedimento. No projeto piloto, três pacientes foram submetidos à rizotomia sacral percutânea por RF utilizando-se eletrodo 18G. Porém, na avaliação 30 dias após o procedimento não foi observado melhora nos parâmetros urodinâmicos. Indicou-se nova rizotomia com eletrodo de menor calibre. Esse grupo de pacientes e mais outros cinco pacientes foram submetidos à rizotomia sacral percutânea por RF utilizando-se eletrodo 22G. Todos os pacientes foram avaliados através de estudo videourodinâmico um mês, seis meses e 12 meses após o procedimento e a capacidade vesical e a pressão do detrusor foram analisadas. Foi observada melhora significativa da capacidade vesical após 12 meses. A média da capacidadevesicalaumentoude 100,2ml (:I: 57,1ml) para 282,9ml (:I:133,4ml),p < 0,05. A pressão do detrusor reduziu-se significativamente no primeiro mês, de 82,4 cm H20 (:I:31,7 cm H20) para 58,5 cm H20 (:I:27,6 cm H20), p < 0,05. No entanto, na avaliação de 6 meses observou-se aumento da pressão detrusora. Aos 12 meses, observou-se retorno da hiperatividade do detrusor em todos os pacientes. Três pacientes com sintomas de disreflexia autonômica demonstraram total alívio dos sintomas; um paciente apresentou resolução do refluxo vésico-ureteral após 6 meses e outro paciente apresentou perda das ereções após o procedimento. Nenhuma complicação séria foi observada durante o seguimento. A rizotomia sacral percutânea por RF, utilizando-se eletrodo 22G, é uma técnica minimamente invasiva, simples e de baixa morbidade. Os resultados após 12 meses de seguimento demonstraram aumento significativo da capacidade vesical, porém, o mesmo não foi observado quanto à redução da pressão do detrusor / Abstract: Neurogenic detrusor overactivity is a result of injuries to the central nervous systetn, such as traumas, degenerative diseases, inflammatory diseases, congenital diseases and stroke. The anti-cholinergic therapy has been the first choice treatment in these cases. Failures in the clinical treatment should indicate the surgical treatment, which includes the enterocystoplasty, the bladder auto-augmentation and a sacral rhizotomy. The aim of the study was to evaluate the percutaneous radioftequency sacral rhizotomy using a 22 gauge electrode and verify the effects on the bladder capacity and the detrusor pressure in a follow-up of 12 months in patients presenting spinal cord injury reftactory to the clínical treatment. Five men and seven women, ages ranging ftom 22 to 58 years- old (median 33), presenting spinal cord injury were evaluated. Five patients showed a lesion to the spinal cord at the cervical region, five presented a thoracic injury and two a lumbar injury. Ten patients had a traumatic complete spinal cord lesion and two an incomplete lesion. The period of time between the occurrence of the lesion and the procedure was ftom 8 to 96 months (median = 60 months). AlI patients had detrusor overactivity and were submitted to a bilateral anesthetic block of the 3rd sacral root using 3 rol of 0.5% bupivacaine, without adrenaline, under fluoroscopic control. Four patients were excluded ftom this study because they did not show an increase of bladder capacity greater than 10QO-aIfoter the procedure. In the pilot project, three patients underwent a percutaneous radioftequency sacral rhizotomy, using a 18 gauge electrode. However, an evaluation made 30 days after the procedure did not show an improvement in the urodynamic parameters. Therefore, a new rhizotomy using a smaller electrode was done. This group of patients and five other patients were submitted to a percutaneous radioftequency sacral rhizotomy using a 22 gauge electrode. AlI patients were evaluated through a videourodynamic study one month, six months and 12 months after the procedure and the bladder capacity and detrusor pressure were analyzed. After 12 nionths, there was a significant improvement in the bladder capacity. The mean bladdercapacityincreasedtrom 100,2rol (::t57,1rol)to 282,9rol (::t133,4rol),p < 0,05. In the first month, the detrusor pressure showed a significant reduction ftom 82,4 cm H20 (:f:31,7 cm H20) to 58,5 cm H20 (::t27,6 cm H20), p < 0,05. However, in the evaluation done after 6 months, an increase in the detrusor pressure was observed. After 12 months, alI patients showed again detrusor overactivity. Three patients with autonomic dysteflexia symptoms showed total relief of the symptoms; one patient presented resolution of the vesicoureteral reflux after six months and another one had an erectile dysfunction after the procedure was carried out. No serious complication was observed. Percutaneous radioiTequency sacral rhizotomy using a 22 gauge electrode is a simple, minimally invasive and low morbidity technique. Results after a follow-up of 12 months demonstrated a significant increase in the bladder capacity, although the reduction in the detrusor pressure was not observed. / Mestrado / Cirurgia / Mestre em Cirurgia

Resposta urodinamica ao cateterismo vesical intermitente e cloreto de oxibutinina em crianças com mielomeningocele

Examinamos urodinamicamente 46 crianças com mielomeningocele que foram classificadas em pressão de perda do detrusor maior de 40 cmH2O (33 pacientes) ou menor de 40 cmH2O (13 pacientes). As variáveis estudadas foram: idade, sexo, pressão intravesical, complacência vesical, contrações vesicais não-inibidas, capacidade vesical, tipo de esfíncter, hidronefrose, refluxo vesicoureteral e cicatriz renal. Os pacientes com pressão de perda do detrusor maior de 40 cmH2O foram tratados com cateterismo vesical intermitente limpo 4 x/dia e cloreto de oxibutinina na dose de 0,2 mg/kg/dose 3 x/dia. Após 30 dias de tratamento, avaliamos a resposta urodinâmica da pressão intravesical, complacência vesical, contrações vesicais não-inibidas e capacidade vesical. A idade dos pacientes com pressão de perda alta é significativamente maior. O sexo não foi significativamente diferente entre os dois grupos. A pressão intravesical e contrações vesicais não-inibidas são significativamente maiores nos pacientes com pressão de perda alta e a complacência vesical é significativamente menor nestes casos. Não houve correlação significativa entre capacidade vesical e pressão de perda. Também não houve correlação significativa entre o tipo de esfíncter e a capacidade vesical, contrações não-inibidas e pressão intravesical. Os pacientes com dissinergia apresentam significativamente menor complacência vesical e aqueles com sinergia apresentam significativamente maior complacência vesical. A dissinergia esteve significativamente associada aos pacientes com pressão de perda alta. Os pacientes com hidronefrose apresentam significativamente maior capacidade vesical e estão correlacionados significativamente com dissinergia e cicatriz renal. Não houve correlação significativa entre hidronefrose e pressão intravesical, complacência vesical, contrações vesicais não-inibidas e pressão de perda.Também não houve correlação significativa entre refluxo vesicoureteral e capacidade vesical, pressão intravesical, complacência vesical, contrações vesicais não-inibidas, tipo de esfíncter e pressão de perda, mas houve associação significativa com hidronefrose e cicatriz renal. Os pacientes com cicatriz renal apresentam significativamente maior capacidade vesical, mas não houve correlação significativa entre cicatriz renal e pressão intravesical, complacência vesical, contrações vesicais não-inibidas, tipo de esfíncter e pressão de perda. A média da pressão intravesical inicial era 60,5 cmH2O e diminuiu significativamente para 50,2 cmH2O após o tratamento. A complacência vesical média era 2,8 ml/cmH2O inicialmente e aumentou significativamente para 5,2 ml/cmH2O. A média das contrações vesicais não-inibidas inicial era 37 cmH2O e diminuiu significativamente para 11,4 cmH2O. A capacidade vesical média inicial era 133,9 ml e aumentou significativamente para 215,3 ml. Houve significativa melhora nos parâmetros urodinâmicos com o tratamento que, contudo, não conseguiu atingir valores considerados normais. Isto pode ser explicado pelas alterações viscoelásticas que já tenham ocorrido nestas bexigas e pela provável presença de outros neurotransmissores envolvidos na neurofisiologia patológica. Este trabalho deixa em aberto a possibilidade de que doses mais elevadas de cloreto de oxibutinina usadas por períodos mais prolongados possam ter efeitos benéficos adicionais aos aqui demonstrados. / We performed urodynamic examination in 46 children who were separated in two categories: a) detrusor leak point pressure higher than 40 cmH2O (33 patients) and b) lower than 40 cmH2O (13 patients). The variables studied were: age, sex, bladder pressure, bladder compliance, uninhibited bladder contractions, bladder capacity, type of sphincter, hydronephosis, vesicoureteral reflux and presence of renal scars. The patients with detrusor leak point pressure higher than 40 cmH2O were treated with clean intermittent bladder catheterization four times a day and oxybutynin chloride 0,2 mg/kg/dose three times a day. After 30 days of treatment, we re-evaluated the following urodynamic parameters: bladder pressure, bladder compliance, uninhibited bladder contraction and bladder capacity. Patients with high leak point pressure are significantly older. Sex distribution was not significantly different between the two groups. The bladder pressure and uninhibited bladder contractions are significantly higher in the high leak point pressure patients and the bladder compliance is significantly lower in these cases. There was no significantly correlation between bladder capacity and leak point pressure. Neither there was a significant correlation between the type of sphincter and bladder capacity, uninhibited bladder contractions and bladder pressure. Patients with dyssynergia have significantly lower bladder compliance and those with synergia have significantly higher bladder compliance. There is a significant relationship between dyssynergia and high leak point pressure patients. The patients with hydronephrosis have significantly higher bladder capacity and are significantly correlated with dyssynergia and renal scarring. There was no statistically significant correlation between hydronephrosis and bladder pressure, bladder compliance, uninhibited bladder contractions and leak point pressure. Neither there was a significant correlation between vesicoureteral reflux and bladder capacity, bladder pressure, bladder compliance, uninhibited bladder contractions, type of sphincter and leak point pressure, but a significant relationship between hydronephosis and renal scar was found. The patients with renal scar have significantly higher bladder capacity, but there was no significant correlation between renal scar and bladder pressure, bladder compliance, uninhibited bladder contractions, type of sphincter and leak point pressure. The initial mean bladder pressure was 60.5 cmH2O and decreased significantly to 50.2 cmH2O after the treatment. The mean bladder compliance was 2.8 ml/cmH2O initialy and increased significantly to 5.2 ml/cmH2O. The initial mean uninhibited bladder contractions were 37 cmH2O and decreased significantly to 11.4 cmH2O. The initial mean bladder capacity was 133.9 ml and increased significantly to 215.3 ml. A significant improvement in the urodynamic parameters was found with the proposed treatment. However, this improvement does not reach the state of normality, which can be explained by the viscoelastic modifications that had already ocurred in these bladders and by the appearance of others neurotransmitters most likely involved in the neuropathophysiology of this condition. The present work leaves open the possibility that higher doses of oxybutynin chloride for more prolonged periods may lead to additional beneficial effects.

Associação do polimorfismo genético do receptor 2A da serotonina (5-HT2A) com incontinência urinária em mulheres idosas

Since serotonin is involved in the modulation of bladder sphincter control, such association is biologically plausible. Since the pharmacological treatment of urinary incontinence (UI) includes the use of serotonin reuptake inhibitors, complementary studies of this polymorphism are of scientific and clinical importance. Objectives: The aims of this study in older women living in a senior living community were: (1) to describe and compare the allelic and genotypic frequencies of the 102C polymorphism of the serotonin 2A receptor (5-HT2A) in incontinent and continent individuals; (2) to describe and compare the urodynamic parameters in incontinents carrying different genotypes/alleles; (3) to describe and compare the frequency of the urodynamic types of urinary incontinence (stress, urge and mixed) in incontinent individuals carrying different genotypes/alleles; and (4) to describe and compare the impact of UI on the quality of life in incontinent individuals carrying different genotypes/alleles. Methodology: A case-control study was conducted including 68 incontinent older women (submitted to urodynamic evaluation) and 162 continent older women (self-reporting). The criteria for exclusion were: use of diuretics, diabetes, non-Caucasian individuals, and severe mobility and neurological disturbances. The polymorphism was analyzed by the PCR-RFLP technique. The Ethics Committee approved the study, and all participants signed an informed consent form. Results: Incontinent older women showed a significantly greater frequency of the TT genotype (TT= 34. 8%, CC= 19. 3%, TC= 45. 9%) compared to continent older women (TT=17%, CC=23. 3%, TC=59. 7%) and the general sample of the population (TT=21. 5%, CC=16. 6%, TC=61. 9%) (p=0. 001). The genetic frequencies of the three samples were in Hardy-Weinberg equilibrium. Urodynamic analysis showed a significant association between the TT genotype and greater detrusor pressure at maximal cystometric capacity, lower maximal cystometric capacity and reduced bladder compliance. The TT genotype also showed a higher prevalence of urge UI (44%) than did the other genotypes (p=0. 01). Analyses of indicators of quality of life and UI demonstrated a greater negative impact in carriers of the C allele (CC+TC).Conclusion: Together, the results suggest that the T102C polymorphism of the 5-HT2A receptor gene has a physiological effect on the lower urinary tract of humans. This hypothesis needs to be confirmed by complementary studies. The effect of an imbalance in the quantity of this receptor, which occurs in the TT genotype, could be associated with a greater chance or urge UI in older women. / Introdução: estudos prévios mostraram possível associação entre o polimorfismo T102C do receptor 2A da serotonina (5-HT2A) e incontinência urinária em idosos. Como a serotonina está envolvida na modulação do controle vesical-esfincteriano, tal associação é biologicamente plausível. Uma vez que a terapêutica farmacológica da incontinência urinária (IU) inclui o uso de inibidores da recaptação da serotonina, estudos complementares deste polimorfismo são de relevância clínico-científica. Objetivos: em mulheres idosas que vivem na comunidade: (1) descrever e comparar as freqüências alélicas e genotípicas do polimorfismo 102C do receptor 2A da serotonina (5-HT2A) em idosas incontinentes e continentes; (2) descrever e comparar os parâmetros urodinâmicos nas idosas incontinentes portadoras de diferentes genótipos/alelos; (3) descrever e comparar a freqüência dos tipos de incontinência urinária urodinâmica (de esforço, de incontinência de urgência e mista) nas idosas incontinentes portadoras de diferentes genótipos/alelos; (4) descrever e comparar o impacto da IU na qualidade de vida nas idosas incontinentes portadoras de diferentes genótipos/alelos. Metodologia: um estudo caso-controle foi conduzido, sendo 68 mulheres idosas incontinentes (submetidas à avaliação urodinâmica) e 162 mulheres continentes (auto-relato). Os critérios de exclusão foram: uso de diuréticos, diabetes, indivíduos de outras etnias que não a caucasiana, distúrbios de mobilidade e neurológicos graves. O polimorfismo foi analisado por técnica de biologia molecular PCR-RFLP. O Comitê de Ética aprovou a pesquisa e todos os participantes assinaram Termo de Consentimento Livre e Esclarecido. Resultados: idosas incontinentes apresentaram freqüência do genótipo TT significativamente maior (TT= 34,8%, CC= 19,3%, TC= 45,9%) do que nas mulheres continentes (TT=17%, CC=23,3%, TC=59,7%) e na amostra geral da população (TT=21,5%, CC=16,6%, TC=61,9%) (p=0,001). As freqüências genéticas das três amostras estavam em equilíbrio de Hardy-Weinberg. Análise urodinâmica mostrou associação significativa entre o genótipo TT e maior pressão detrusora na capacidade cistométrica máxima, menor capacidade cistométrica máxima e complacência vesical reduzida. O genótipo TT também apresentou maior prevalência de IU de urgência (44%) do que os demais genótipos (p=0,01). O conjunto destes resultados sugeriu que o genótipo TT está associado a alterações de fatores fisiológicos, predisponentes a incontinência urinária. Entretanto, análises de indicadores da qualidade de vida e a IU mostraram maior impacto negativo em portadoras do alelo C (CC+TC). Esta condição pode estar associada à sugestão de predisposição de portadoras do alelo C a distúrbios emocionais. Conclusão: o conjunto dos resultados sugere que o polimorfismo T102C do gene do receptor 5-HT2A tenha um possível efeito fisiológico no trato urinário inferior (TUI) de seres humanos. Esta hipótese precisa ser confirmada por estudos complementares. O efeito do desbalanço na quantidade deste receptor, que ocorre no genótipo TT estaria associado à maior chance de IU de urgência na mulher idosa.

Resposta urodinamica ao cateterismo vesical intermitente e cloreto de oxibutinina em crianças com mielomeningocele

Examinamos urodinamicamente 46 crianças com mielomeningocele que foram classificadas em pressão de perda do detrusor maior de 40 cmH2O (33 pacientes) ou menor de 40 cmH2O (13 pacientes). As variáveis estudadas foram: idade, sexo, pressão intravesical, complacência vesical, contrações vesicais não-inibidas, capacidade vesical, tipo de esfíncter, hidronefrose, refluxo vesicoureteral e cicatriz renal. Os pacientes com pressão de perda do detrusor maior de 40 cmH2O foram tratados com cateterismo vesical intermitente limpo 4 x/dia e cloreto de oxibutinina na dose de 0,2 mg/kg/dose 3 x/dia. Após 30 dias de tratamento, avaliamos a resposta urodinâmica da pressão intravesical, complacência vesical, contrações vesicais não-inibidas e capacidade vesical. A idade dos pacientes com pressão de perda alta é significativamente maior. O sexo não foi significativamente diferente entre os dois grupos. A pressão intravesical e contrações vesicais não-inibidas são significativamente maiores nos pacientes com pressão de perda alta e a complacência vesical é significativamente menor nestes casos. Não houve correlação significativa entre capacidade vesical e pressão de perda. Também não houve correlação significativa entre o tipo de esfíncter e a capacidade vesical, contrações não-inibidas e pressão intravesical. Os pacientes com dissinergia apresentam significativamente menor complacência vesical e aqueles com sinergia apresentam significativamente maior complacência vesical. A dissinergia esteve significativamente associada aos pacientes com pressão de perda alta. Os pacientes com hidronefrose apresentam significativamente maior capacidade vesical e estão correlacionados significativamente com dissinergia e cicatriz renal. Não houve correlação significativa entre hidronefrose e pressão intravesical, complacência vesical, contrações vesicais não-inibidas e pressão de perda.Também não houve correlação significativa entre refluxo vesicoureteral e capacidade vesical, pressão intravesical, complacência vesical, contrações vesicais não-inibidas, tipo de esfíncter e pressão de perda, mas houve associação significativa com hidronefrose e cicatriz renal. Os pacientes com cicatriz renal apresentam significativamente maior capacidade vesical, mas não houve correlação significativa entre cicatriz renal e pressão intravesical, complacência vesical, contrações vesicais não-inibidas, tipo de esfíncter e pressão de perda. A média da pressão intravesical inicial era 60,5 cmH2O e diminuiu significativamente para 50,2 cmH2O após o tratamento. A complacência vesical média era 2,8 ml/cmH2O inicialmente e aumentou significativamente para 5,2 ml/cmH2O. A média das contrações vesicais não-inibidas inicial era 37 cmH2O e diminuiu significativamente para 11,4 cmH2O. A capacidade vesical média inicial era 133,9 ml e aumentou significativamente para 215,3 ml. Houve significativa melhora nos parâmetros urodinâmicos com o tratamento que, contudo, não conseguiu atingir valores considerados normais. Isto pode ser explicado pelas alterações viscoelásticas que já tenham ocorrido nestas bexigas e pela provável presença de outros neurotransmissores envolvidos na neurofisiologia patológica. Este trabalho deixa em aberto a possibilidade de que doses mais elevadas de cloreto de oxibutinina usadas por períodos mais prolongados possam ter efeitos benéficos adicionais aos aqui demonstrados. / We performed urodynamic examination in 46 children who were separated in two categories: a) detrusor leak point pressure higher than 40 cmH2O (33 patients) and b) lower than 40 cmH2O (13 patients). The variables studied were: age, sex, bladder pressure, bladder compliance, uninhibited bladder contractions, bladder capacity, type of sphincter, hydronephosis, vesicoureteral reflux and presence of renal scars. The patients with detrusor leak point pressure higher than 40 cmH2O were treated with clean intermittent bladder catheterization four times a day and oxybutynin chloride 0,2 mg/kg/dose three times a day. After 30 days of treatment, we re-evaluated the following urodynamic parameters: bladder pressure, bladder compliance, uninhibited bladder contraction and bladder capacity. Patients with high leak point pressure are significantly older. Sex distribution was not significantly different between the two groups. The bladder pressure and uninhibited bladder contractions are significantly higher in the high leak point pressure patients and the bladder compliance is significantly lower in these cases. There was no significantly correlation between bladder capacity and leak point pressure. Neither there was a significant correlation between the type of sphincter and bladder capacity, uninhibited bladder contractions and bladder pressure. Patients with dyssynergia have significantly lower bladder compliance and those with synergia have significantly higher bladder compliance. There is a significant relationship between dyssynergia and high leak point pressure patients. The patients with hydronephrosis have significantly higher bladder capacity and are significantly correlated with dyssynergia and renal scarring. There was no statistically significant correlation between hydronephrosis and bladder pressure, bladder compliance, uninhibited bladder contractions and leak point pressure. Neither there was a significant correlation between vesicoureteral reflux and bladder capacity, bladder pressure, bladder compliance, uninhibited bladder contractions, type of sphincter and leak point pressure, but a significant relationship between hydronephosis and renal scar was found. The patients with renal scar have significantly higher bladder capacity, but there was no significant correlation between renal scar and bladder pressure, bladder compliance, uninhibited bladder contractions, type of sphincter and leak point pressure. The initial mean bladder pressure was 60.5 cmH2O and decreased significantly to 50.2 cmH2O after the treatment. The mean bladder compliance was 2.8 ml/cmH2O initialy and increased significantly to 5.2 ml/cmH2O. The initial mean uninhibited bladder contractions were 37 cmH2O and decreased significantly to 11.4 cmH2O. The initial mean bladder capacity was 133.9 ml and increased significantly to 215.3 ml. A significant improvement in the urodynamic parameters was found with the proposed treatment. However, this improvement does not reach the state of normality, which can be explained by the viscoelastic modifications that had already ocurred in these bladders and by the appearance of others neurotransmitters most likely involved in the neuropathophysiology of this condition. The present work leaves open the possibility that higher doses of oxybutynin chloride for more prolonged periods may lead to additional beneficial effects.

Os retrovírus são conhecidos desde 1911, mas somente em 1980 foram associados a doenças proliferativas. No caso do HTLV-I , várias doenças a ele estão relacionadas, entre elas a paraparesia tropical espástica também conhecida como mielopatia associada ao HTLV-I (ou TSP/HAM, segundo a Organização Mundial da Saúde) descrita em 1985. Sua principal área endêmica é o Japão. No Brasil, foram descritos os primeiros casos em 1989, e desde 1994 há obrigatoriedade de testagem para o HTLV-I em doadores de sangue. No Rio Grande do Sul, a prevalência é de 0,39% em doadores repetidamente reagentes e de 2,39% em descendentes de japoneses ali radicados. O quadro urológico e urodinâmico dos pacientes infectados raramente tem sido descrito na literatura médica. O objetivo deste trabalho foi avaliar urodinamicamente pacientes infectados pelo HTLV-I com e sem TSP/HAM. Quarenta e oito pacientes foram estudados prospectiva e consecutivamente, dos quais 26 tinham TSP/HAM e outros 22 não. Dos pacientes com mielopatia, 80,76% apresentavam bexiga hiper-reflexa e 34,61% tinham dissinergia detrussor-esfincteriana. Quanto à urofluxometria, os pacientes com TSP/HAM tiveram fluxo miccional anormal em 82,6% dos casos. Nos pacientes sem mielopatia, a bexiga mostrou-se hiper-reflexa em 22,72% dos casos, com nenhuma dissinergia registrada. O fluxo miccional foi normal em 70% dos casos Quatorze pacientes sem TSP/HAM e apenas um com TSP/HAM tiveram normais todos os parâmetros analisados o que representa 31,25% dos 48 pacientes infectados.As alterações urodinâmicas em pacientes com mielopatia foram significativamente maiores que aquelas encontradas em pacientes sem mielopatia.
