Do escudo ao escuto : contribuições da supervisão clínico-institucional para a interface psicanálise e saúde mental em CAPS AD

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A presente pesquisa tem por objetivo investigar contribuições da Supervisão Clínico-Institucional para a interface Psicanálise e Saúde Mental em CAPS AD (Centro de Atenção Psicossocial para usuários de Álcool e Drogas) servindo-se, para tal, de entrevistas semi-estruturadas realizadas aos supervisores de CAPS AD da cidade do Recife. Ressaltando o lugar de Supervisão não o único tampouco o principal, mas lugar estratégico nos CAPS AD para articular clínica e instituição a partir da ética da psicanálise, tais depoimentos assumem relevo ao que testemunham de efeitos em sua prática, ao identificar impasses e erigir questões na busca por avançar. Intervir sob a necessidade lógica do lugar de exceção na instituição foi destacada e testemunhada como condição estruturante à construção de um projeto coletivo, sustentando no modelo horizontal dos CAPS AD a instauração de diferenças de lugar na equipe. Tal corte é condição para evitar formação de massa a dois equipe-usuário, para instaurar uma abertura ao não-todo que não se confunda com todo-não (Lebrun, 2009). Poder prescindir do lugar de exceção sob a condição de servir-se deste é operar efeitos de uma transmissão da psicanálise, rumar à assunção do impossível do rapport sexual, castração enquanto motor da clínica e de uma formação continuada. Enquanto limite e desafio, a construção de balizadores clínicos que respeite a diversidade de formações acadêmicas e ainda assim norteie-se pela ética da psicanálise ressaltou uma transferência de trabalho que sustente a construção de um projeto clínico no coletivo, levando em conta a função psíquica da droga para o sujeito, rumando à contramão da démarche segregativa do discurso da ciência e do discurso capitalista. Para além das diferenças de conhecimento, há um saber que habita cada um que se confronta ao Real da clínica. Neste sentido, sair da queixa para a demanda de trabalho por um desejo de analista que sustente transmissão da psicanálise é travessia do escudo (sic) para o escuto, mudança de letra que situa o real de uma mudança de posição de bela alma para avançar rumo a escuta do sujeito do inconsciente, escuta de um corpo de desejo e de gozo. / This work has for objective to investigate the contributions of Supervision Clinic-Institucional for the interface Psychoanalisis and Mental Health in CAPS AD (Psychosocial Care Center for users of Alcohol and Drugs), using, for this point, semi-structured interviews realized with supervisors of CAPS AD from Recife City. Como escutar o corpo que dói?: reflexões sobre acontecimento de corpo e transferência

Do lugar da psicanálise aplicada, com prática no Setor de Psicologia de uma Instituição que oferece atendimento ambulatorial pelo Sistema Único de Saúde - SUS, nos interrogamos sobre como escutar o corpo que dói, atravessado por uma transferência que não está endereçada ao analista num contexto institucional. Nessa clínica, a fixidez discursiva nas queixas corpóreas constitui resistência às tentativas de interpretação ou inserção de enigmas, à entrada do outro no discurso, tornando a relação transferencial uma questão. Distinguimos dois campos em nossa experiência: o trabalho do analista de fazer estremecer o que está fixo e, do lado do paciente, buscar e manter-se no discurso médico-científico, sem deixar brechas. Pela via do campo da linguagem, o que está desintegrado no corpo institucional é capaz de se presentificar no corpo do paciente, sob transferência. Nosso propósito nesta pesquisa é discutir a fibromialgia e sua relação com a ciência, sublinhando o apogeu dos corpos hoje, e contextualizá-la sob o vértice da psicanálise, a fim de problematizar e contextualizar a genealogia da escuta do corpo que dói na teoria psicanalítica. Utilizamos os recursos metodológicos próprios da pesquisa em psicanálise, a interpretação e a transferência, tomando como referência relatos de casos clínicos de sujeitos atendidos no Setor de Psicologia a partir dos registros em prontuários da Instituição. Observamos que o trabalho em equipe pode ser ponto de estofo do que, na visão do profissional de saúde, se reduz muitas vezes ao que é nominável pelas ciências médicas, pois o ganho inconsciente ou gozo do usuário do serviço é, quase sempre, se manter em sua doença.

A opção pelas gated communities fez desaparecer dos espaços de convivência comunitária considerados a primeira vítima colateral de uma cidade que perde a árdua luta enfrentada para resistir ao avanço do isolamento espacial dos moradores grande parte dos atrativos da vida citadina. Diante da importância do ambiente público na formação da criança e do adolescente, a questão envolvendo a tutela da convivência comunitária está inegavelmente baseada na necessidade da efetivação de políticas públicas voltadas para a revitalização e readequação dos espaços urbanos, a fim de resguardar a qualidade de vida daqueles que merecem especial proteção do Estado. Sob este prisma, portanto, que esta obra será desenvolvida. Pretende-se demonstrar que, em relação à criança e ao adolescente, hodiernamente entendidos como pessoas humanas que merecem especial atenção da família, sociedade e Estado, a tutela da convivência comunitária representa um Direito Fundamental, em paralelo ao direito à convivência familiar, em que ambos estão igualmente previstos na norma constitucional, através do artigo 227 da Constituição da República Federativa do Brasil de 1988, e infraconstitucional, por meio dos artigos 4 e 19 do Estatuto da Criança e do Adolescente. / Gated communities option by the end of community interaction spaces considered the first victim sibling of a city that loses the arduous struggle faced to withstand the advancement of spatial isolation of residents most of the attractions of the city life. Dance Intervention for Adolescent Girls with Internalizing Problems : Effects and Experiences

Globally, psychological health problems are currently among the most serious public health challenges. Adolescent girls suffer from internalizing problems, such as somatic symptoms and mental health problems, at higher rates than in decades. By age 15, over 50 % of all girls experience multiple health complaints more than once a week and one in five girls reports fair or poor health. The overall aim of this study was to investigate the effects of and experiences with an after-school dance intervention for adolescent girls with internalizing problems. The intervention comprised dance that focused on resources twice weekly for 8 months. Specifically, this thesis aimed to: I) investigate the effects on self-rated health (SRH), adherence and over-all experience; II) evaluate the effects on somatic symptoms, emotional distress and use of medication; III) explore the experiences of those participating in the intervention; and IV) assess the cost-effectiveness. A total of 112 girls aged 13 to 18 years were included in a randomized controlled trial. The dance intervention group comprised 59 girls, and the control group 53. In paper I, the dance group showed increased SRH scores compared to the control group (p = .02). Girls in the intervention group showed high adherence and a positive overall experience. In paper II, the dance group exhibited a decrease in somatic symptoms (p = .021), emotional distress (p = .023) and use of medication (p = .020) compared to the control group. In paper III, a strategic sample of 24 girls was interviewed. Qualitative content analysis was performed, and five generic categories emerged. Two were “An Oasis from Stress” and “Supportive Togetherness”, which was shown to represent the fundamental basis and setting of the intervention. The main category, participants’ central experience, was understood as “Finding embodied self-trust that opens new doors”. Paper IV revealed that, due to decreased number of visits to the school nurse and an increase in health related quality of life; the intervention was considered to be cost-effective (combined with the usual school health services). In summary, the results of this thesis show that this dance intervention for adolescent girls with internalizing problems generated positive health effects and proved to be cost-effective. For this target group, a non-judgmental environment and supportive togetherness proved to be of importance for participation. The results of this study may provide practical information for school health care staff and caregivers in designing future interventions.

Investigating depression and quality of life in adults diagnosed with HIV or AIDS

Magister Psychologiae - MPsych / HIV and AIDS are disease conditions that have led to high mortality rates in Southern Africa since the late 1970s. The socio-economic system has led to the unequal spread of resources’ and vulnerability and exposure to HIV is more prevalent in poorer communities. The added burden of life stresses cause for many to be isolated and stigmatised and are often not equipped with the necessary support and coping skills to deal with the magnitude of these circumstances. There is a high prevalence of mental disorders and especially depression amongst individuals infected with either HIV or AIDS. Research shows that stressful life events can impact HIV course progression and impacts the QoL of those infected with HIV or AIDS. Given the social and psychological context of HIV and AIDS, the aim of the study was to examine the relationship between depression and QoL in a sample of adults diagnosed with HIV or AIDS. This quantitative, cross-sectional study used the Becks Depression Inventory II (BDI II) and the Quality of Life Enjoyment and Satisfaction Questionnaire (Q-LES-Q), to measure the variables concerned. This battery of questionnaires was administered to a purposive sample of adult individuals diagnosed with HIV or AIDS residing in a previously disadvantaged area in the Cape Metropole region. Using SPSS, data was analysed and descriptive and inferential statistics were conducted. The study found that there were more women than men with HIV or AIDS that were found to be depressed (mild, moderate and severe depression). Furthermore, the depressive state increased when the progression of the disease increased. There were generally no significant differences in the QoL experienced within various areas of life and overall life satisfaction experienced. However, the QoL experienced in work was lower. There was a significant relationship between the depressed state and QoL and life satisfaction experienced in household duties and tasks. The contribution of this study includes informing the larger research project, with regards to future treatment regimes. It will update statistics on the prevalence of depression and QoL of adults diagnosed with HIV or AIDS in the area. This study is framed within a biopsychosocial model and is theoretically underpinned by Beck’s theory of depression. The influence of non-linear frequency compression on music perception for adults with a moderate to severe hearing loss

Objective: To date, the main focus in frequency lowering hearing aid studies has been in relation to speech perception abilities. With improvements in hearing aid technology, there is a growing interest in musical perception as a dimension that could improve hearing aid users’ quality of life. The purpose of this study was two-fold: Firstly, to develop a test of music perception for adult hearing aid users and secondly, to evaluate the influence of non-linear frequency compression (NFC) on music perception with the use of the Music Perception Test (MPT) compiled by the researcher. Research design and research sample: Phase 1 entailed the compilation of the MPT and can be described as design-based. A quasi-experimental research design was selected to establish the structure of the method employed in Phase 2, which involved the fitting of participants (n=40) with NFC hearing aids. Objective data was obtained with the hearing aids with NFC active and inactive. Phase 3 was characterized by a survey design which elicited subjective impressions of the participants’ musical experiences with NFC active and inactive. Results: Results proved that normal hearing adults as well as adults using hearing aids were able to complete all the sub-tests of the MPT. Furthermore, the use of NFC resulted in a statistically significant improvement in hearing aid users’ perception of timbre and melody, but not of pitch. Overall, no statistically significant improvement in their perception of rhythm was observed, although their performance on some rhythm sub-tests improved significantly. The use of NFC also brought about a statistically significant improvement in hearing aid users’ perception of the music qualities of overall fidelity, tinniness and reverberance. Although participants experienced the loudness, fullness, crispness, naturalness and pleasantness of music more positively with NFC, these benefits were not statistically significant. Conclusion: The MPT can be used successfully for assessing music perception in hearing aid users within the South African context and may therefore result in more accountable hearing aid fittings. The use of NFC may increase hearing aid users’ appreciation of music whilst not influencing music perception negatively. Given that a large percentage of hearing aid users express a loss in enjoyment of music, audiologists should not ignore the possible benefits of NFC, especially if one takes into account that previous research indicated speech perception benefits with this technology. / Thesis (DPhil)--University of Pretoria, 2012. / Speech-Language Pathology and Audiology / Unrestricted

Tabletop Role-playing Games (TRPG) and Group Coherence / Rollspel och gruppkoherens : En enkät- och intervjustudie

Tabletop role-playing games (TRPGs), also known as Pen-and-Paper role-playing games(PnP-RPGs), are games that can be described as a mix between board games and improvisationaltheatre. Each player takes on the role of a single character, while the game’s leader(GM or DM) is in charge of both simulating the game world, acting out all remainingcharacters and facilitating the game rules. The games are often played in campaigns,continual stories told over several gameplay sessions. This thesis investigates whetherplayers’ types/characteristics and preferences affect the enjoyment of play, and in particularwhether being coherent with the rest of the role-play group in these respects affects theenjoyment of TRPG play. The research questions were addressed primarily by a surveyamong TRPG players that received 1,982 completed questionnaires. These were thenanalysed by correlation and factor analyses in order to find connections between responses.The thesis work shows that the length and depth of previous experiences have almost nocorrelation to the level of enjoyment. Varjespelare antar rollen av en karaktär, medan spelledaren (GM eller DM) ansvarar för simuleringenav spelvärlden, agerandet av resterande karaktärer samt ansvar för regelhållningen.Denna uppsats undersöker om spelartyper (egenskaper) och deras preferenser påverkarspelglädjen, och i synnerhet huruvida det är viktigt att ha samma uppfattning som den övrigagruppen i dessa frågor (gruppkoherens). Forskningsfrågorna besvarades huvudsakligengenom en enkätundersökning bland TRPG-spelare som gav 1982 komplett ifyllda enkätsvar.Dessa analyserades sedan med korrelations- och faktoranalyser för att hitta kopplingarmellan svaren. Uppsatsen visar att längden och djupet av tidigare spelupplevelser nästan intehar något samband med nivån av spelglädje.
