Les filtres de recherche bibliographique optimisés visent à faciliter le repérage de l’information dans les bases de données bibliographiques qui sont presque toujours la source la plus abondante d’évidences scientifiques. Ils contribuent à soutenir la prise de décisions basée sur les évidences. La majorité des filtres disponibles dans la littérature sont des filtres méthodologiques. Mais pour donner tout leur potentiel, ils doivent être combinés à des filtres permettant de repérer les études couvrant un sujet particulier. Dans le champ de la sécurité des patients, il a été démontré qu’un repérage déficient de l’information peut avoir des conséquences tragiques. Des filtres de recherche optimisés couvrant le champ pourraient s’avérer très utiles.
La présente étude a pour but de proposer des filtres de recherche bibliographique optimisés pour le champ de la sécurité des patients, d’évaluer leur validité, et de proposer un guide pour l’élaboration de filtres de recherche.
Nous proposons des filtres optimisés permettant de repérer des articles portant sur la sécurité des patients dans les organisations de santé dans les bases de données Medline, Embase et CINAHL. Ces filtres réalisent de très bonnes performances et sont spécialement construits pour les articles dont le contenu est lié de façon explicite au champ de la sécurité des patients par leurs auteurs. La mesure dans laquelle on peut généraliser leur utilisation à d’autres contextes est liée à la définition des frontières du champ de la sécurité des patients. / Optimized bibliographic search filters are designed to facilitate information retrieval in bibliographic databases, which are almost always the most abundant source of scientific evidence. The purpose of such filters is to support evidence-based decision making. Many of the filters available in the literature are methodological search filters. To reach their full potential they need to be combined with subject filters that identify studies covering a particular topic. In the field of patient safety, it has been demonstrated that deficiencies in the information retrieval process can lead to tragic consequences. Optimized bibliographic search filters covering the field could thus be very useful.
This study is intended to provide subject bibliographic search filters optimized for the field of patient safety, assess their validity, and offer a guide for developing optimized bibliographic search filters.
We propose high-performing bibliographic search filters to retrieve papers dealing with patient safety in health care organizations, that have been explicitly defined as relevant to the patient safety field by their authors, in Medline, Embase and CINAHL. The main generalization issue lies in defining the boundaries of the patient safety field.

The purpose of this study was to determine the nurse educators’ experiences in implementing the evidence-based practice (EBP) in teaching and learning, and to describe the importance and benefits of EBP teaching and learning to the nursing profession, especially for nurse educators and student nurses. A qualitative research design and methods were followed in conducting the study. A non-probability purposive sampling technique was used to access the sample of twelve nurse educators from two nursing campuses under KwaZulu-Natal College of Nursing within Umgungundlovu Health District. Data were collected using semi-structured interviews, the interview guide, and the digital voice recorder. Data were analysed manually, following a content thematic data analysis approach. Two themes emerged as follows:
Challenges experienced by nurse educators with the implementation of EBP in teaching and learning.
Benefits/value of EBP in teaching and learning.
Findings revealed that, although most of the nurse educators are supportive and displayed a positive attitude towards implementing EBP in teaching and learning, the level of knowledge and skills was questionable. This was coupled with a lack of motivation and commitment towards research. Therefore, recommendations for nursing education, nursing practice, and future research were suggested, for successful implementation of EBP in teaching and learning. / Health Studies / M.A. (Health Studies)

Previous issue date: 2010 / A lei brasileira garante o direito à Assistência Farmacêutica, mas ainda há falhas do Estado na efetivação do acesso dos cidadãos aos medicamentos, assim, as ações judiciais de medicamentos têm tido importante papel como via alternativa ao acesso a medicamentos no SUS. Este tipo de ação judicial vem crescendo anualmente e tem como objeto tanto os medicamentos em falta na rede pública como aqueles ainda não incorporados pelo sistema de saúde. O presente trabalho teve como objetivo principal analisar os medicamentos presentes nas demandas judiciais da comarca da capital encaminhadas a Central de Mandados da Secretaria de Saúde e Defesa Civil do Rio de Janeiro, no período de julho de 2007 a junho de 2008, frente às alternativas terapêuticas presentes nas listas de financiamento público e à luz das evidências científicas. Desta forma, foram analisados os medicamentos cadastrados na Central de Mandados da SESDEC/RJ referentes a 281 pacientes demandantes. Foi observado neste estudo uma diversidade de situações dos indivíduos demandantes e dos medicamentos solicitados. Alguns indivíduos eram demandantes de primeira vez, mas foi também identificada a existência de pacientes que já se encontravam recebendo medicamentos pela Secretaria Estadual de Saúde e que recorreram à via judicial para obtenção do mesmo ou de medicamento adicional. No que tange aos medicamentos solicitados, também houve uma miríade de categorias ao se considerarem as evidências disponíveis e as informações existentes. A maior parte dos medicamentos demandados não era financiada pelo sistema de saúde e entre estes, destaca-se: 1) medicamentos solicitados para indicações registradas no órgão sanitário, com evidências científicas e que apresentavam alternativas terapêuticas financiadas pelo sistema; 2)medicamentos cujas evidências em longo prazo ainda não se encontram bem estabelecidas; 3) medicamentos para indicações não aprovadas pela Anvisa; 4) medicamentos sem registro na Anvisa e; 5) medicamentos cujas evidências inexistem ou não recomendam seu uso. Uma vez que as listas de financiamento da Assistência Farmacêutica e os Protocolos Clínicos e Diretrizes Terapêuticas foram construídos com base em evidências, deve-se priorizar a utilização dos medicamentos constantes nestes. Havendo alternativa terapêutica nas listas oficiais para a indicação proposta na prescrição médica àquela demandada, a decisão deve considerareste fato. / The Brazilian law guarantees the right of Pharmaceutical Care, but is falling short of the
state in realization of citizens' access to medicines, so, the medicine lawsuits have been
important as an alternative route to access to medicines in the SUS. This type of lawsuit is
increasing annually and aims medicines missing from the public health system and those not yet
incorporated into this system. This study aimed to examine the medicines in the lawsuits of the District Judicial sent to Central Warrants of the Secretary of Health and Civil Defense of Rio de Janeiro, from July 2007 to June 2008, compared to alternative therapies on the lists of public health system and examine the existence of scientific evidence. Thus, was analyzed the medicines registered in the Central Warrants of SESDEC / RJ referring to 281 patients. Was observed in this study a variety of situations of individuals and the plaintiffs requested medication. Some individuals were the first-time, but also was identified the existence of patients who were already receiving medications for the State Health Department and have appealed to the courts to obtain the same or additional medicine. With respect to medicines requested, there was also a myriad of categories when considering the available evidence and existing information. Most of the medications examined was not financed by the public health system and between them stands out: 1) medicines for indication approved by Anvisa with scientific evidence and having alternative therapies financed by the system, 2) medicines for which evidence on long term are not yet well established, 3) medications for indications not approved by ANVISA, 4) medicines not registered in Anvisa and 5) medicines for which the evidence does not exist or recommend its use. Once the lists of funding for Pharmaceutical Care and Clinical Protocols and Therapeutic Guidelines have been constructed based on evidence, we must prioritize the use of these medications listed. If there is an alternative therapy in the official lists for the proposed indication in the prescription to that defendant, the decision should consider this fact.

Controlled by Knowledge : A Study of two Clinical pathways in Mental HealthcareJerndahl Fineide, Mona January 2012 (has links)
Standardisation of professional work is a major policy concern to ensure quality and efficiency of services and a number of hospitals are now focusing on the use of clinical pathways as an important tool to standardise their work. This study sheds light on the processes set in motion when notions of standardisation meet local practice. In order to gain insight into what clinical pathways mean for professional work in mental health care, the focus of the study was to explore the contexts in which standardisation by “rule production” takes place. Two empirical cases from Norwegian mental health care show how dedicated professionals are in charge of carrying out the standardisation work, strongly influenced by a steering framework of defined governmental policies where employee involvement and responsibility ensured loyalty to the idea. Along with a “package” of ideas, new bodies and techniques, clinical pathways contribute to the institutionalisation of prima facie knowledge in demonstrating that evidence basing is linked to steering and control of employees. Thus, professional autonomy is threatened in an insidious way: through the institutionalisation of evidence-based knowledge as ‘prima facie’ knowledge in combination with professionals who standardise and control their own work. The thesis therefore concludes that the control of professional work has now become a complex and sophisticated process where professional work is “controlled by knowledge”.

Exploring the effect of male child sexual abuse on a sample of men in ZimbabweMarufu, Tapiwa Dadirai 03 1900 (has links)
Several studies suggest that sexual assaults are experienced differently by males than by
females. In dominant discourses in which sexual perpetration is associated with males and
sexual victimisation with females, males are expected to practice and exhibit hegemonic
masculinity. This gendered perception of sexual assaults means that female perpetration of
male child sexual abuse often goes unrecognised and that male perpetration is particularly
problematic for male victims. Using a feminist critical paradigm and critical discourse analysis
as a research design, this study explored how a sample of male victims in Zimbabwe
experienced sexual abuse as children and how they were affected by it. The study, further,
explored how the male victims of childhood sexual abuse sought to reconcile the experience
of victimisation with their identity as males. Nine men participated in the study. The sample
was, due to the sensitive nature of the topic, purposively selected using letters describing the
nature of the study and inviting participation. The letters were distributed through diverse
channels, including a newspaper with national coverage. Most participants reported
experiences of female perpetrated abuse. The study found that, in keeping with dominant
discourses of hegemonic masculinity, the participants struggled to construct themselves as
victims. Many of the participants were considerably and negatively affected by having been
sexually abused. Participants utilised a variety of methods to come to terms with the fact that

Using Evidence Based Practice: The Relationship Between Work Environment, Nursing Leadership and Nurses at the BedsidePryse, Yvette M. 30 January 2013 (has links)
Indiana University-Purdue University Indianapolis (IUPUI) / Evidence based practice (EBP) is essential to the practice of nursing for purposes of promoting optimal patient outcomes. Research suggests that the implementation of EBP by staff nurses is problematic and influenced by beliefs, nursing leadership and the work environment.
The purpose of this descriptive study was to examine variables that describe the relationship among beliefs about EBP, the work environment and nursing leadership on the EBP implementation activities of the staff nurse. The variables of interest were 1) individual staff nurse characteristics, 2) beliefs about EBP, 3) the EBP work environment and 4) nursing leadership for EBP.
A descriptive, quantitative method was used. A sample of 422 Registered Nurses from two urban hospitals (one Magnet and one non-Magnet) completed an online 58 item survey that included questions related to individual belief’s about EBP, the EBP work environment and nursing leadership for EBP as well as EBP implementation activities.
Education, tenure and Magnet status were not significantly related to EBP implementation activities in either the univariate or multivariate analysis. EBP beliefs had a significantly positive relationship with EBP implementation activities in both the univariate and multivariate analyses. Work environment and nursing leadership support for EBP had significant positive relationships with self-reported implementation activities in only the univariate analysis. The most surprising finding was that there were no differences between Magnet and non-Magnet work environments for EBP implementation scores, yet the Magnet hospitals reported higher means on the EBP Beliefs Scale than the non-Magnet hospital.
The results of this have implications for identifying and testing strategies to influence EBP implementation activities through development of nursing leadership skills for EBP and creating a more EBP friendly work environment.

L’évaluation économique en santé consiste en l’analyse comparative d’alternatives de services en regard à la fois de leurs coûts et de leurs conséquences. Elle est un outil d’aide à la décision. La grande majorité des décisions concernant l’allocation des ressources sont prises en clinique; particulièrement au niveau des soins primaires. Puisque chaque décision est associée à un coût d’opportunité, la non-prise en compte des considérations économiques dans les pratiques des médecins de famille peut avoir un impact important sur l’efficience du système de santé. Il existe peu de connaissances quant à l’influence des évaluations économiques sur la pratique clinique. L’objet de la thèse est de comprendre le rôle de l’évaluation économique dans la pratique des médecins de famille. Ses contributions font l’objet de quatre articles originaux (philosophique, théorique, méthodologique et empirique).
L’article philosophique suggère l’importance des questions de complexité et de réflexivité en évaluation économique. La complexité est la perspective philosophique, (approche générale épistémologique) qui sous-tend la thèse. Cette vision du monde met l’attention sur l’explication et la compréhension et sur les relations et les interactions (causalité interactive). Cet accent sur le contexte et le processus de production des données souligne l’importance de la réflexivité dans le processus de recherche.
L’article théorique développe une conception nouvelle et différente du problème de recherche. L’originalité de la thèse réside également dans son approche qui s’appuie sur la perspective de la théorie sociologique de Pierre Bourdieu; une approche théorique cohérente avec la complexité. Opposé aux modèles individualistes de l’action rationnelle, Bourdieu préconise une approche sociologique qui s’inscrit dans la recherche d’une compréhension plus complète et plus complexe des phénomènes sociaux en mettant en lumière les influences souvent implicites qui viennent chaque jour exercer des pressions sur les individus et leurs pratiques.
L’article méthodologique présente le protocole d’une étude qualitative de cas multiples avec niveaux d’analyse imbriqués : les médecins de famille (niveau micro-individuel) et le champ de la médecine familiale (niveau macro-structurel). Huit études de cas furent réalisées avec le médecin de famille comme unité principale d’analyse. Pour le niveau micro, la collecte des informations fut réalisée à l’aide d’entrevues de type histoire de vie, de documents et d’observation. Pour le niveau macro, la collecte des informations fut réalisée à l’aide de documents, et d’entrevues de type semi-structuré auprès de huit informateurs clés, de neuf organisations médicales. L’induction analytique fut utilisée.
L’article empirique présente l’ensemble des résultats empiriques de la thèse. Les résultats montrent une intégration croissante de concepts en économie dans le discours officiel des organisations de médecine familiale. Cependant, au niveau de la pratique, l'économisation de ce discours ne semble pas être une représentation fidèle de la réalité puisque la très grande majorité des participants n'incarnent pas ce discours. Les contributions incluent une compréhension approfondie des processus sociaux qui influencent les schèmes de perception, de pensée, d’appréciation et d’action des médecins de famille quant au rôle de l’évaluation économique dans la pratique clinique et la volonté des médecins de famille à contribuer à une allocation efficiente, équitable et légitime des ressources. / Health economic evaluations are analytic techniques to assess the relative costs and consequences of health services. Their role is to inform the decision-making process. A vast amount of resource allocation decisions are undertaken at the clinical-encounter level; especially in primary care. Since every decision has an opportunity cost, ignoring economic information in family physicians’ practices may have a broad impact on health care efficiency. There is little evidence on the influence of economic evaluation on clinical practice. The objective of the thesis is to understand the role of economic evaluation in family physicians’ practices. Its contributions are presented in four original articles (philosophical, theoretical, methodological, and empirical).
The philosophical article suggests that complexity and reflexivity are two important issues for economic evaluation. Complexity thinking is the philosophical perspective (overarching epistemological approach) underpinning the thesis. This way of thinking focuses attention on explanation and understanding and gives particular emphasis to relations and interactions (interactive causality). This increased emphasis on the context and process of data production highlights the importance of reflexivity in the research process.
The theoretical article develops a new and different conceptualization of the research problem. The originality of the thesis also lay in the research problem being approached from the perspective of Pierre Bourdieu's sociological theory. Bourdieu’s approach embraces complexity. Moving away from individualist, rational models of action, it can contribute to a more complete and complex understanding of social phenomena by revealing the structuring effects of social fields on the individual’s dispositions and practices.
The methodological article presents the protocol of a qualitative embedded multiple-case study research. There were two embedded units of analysis: the family physicians (micro-individual level) and the field of family medicine (macro-structural level). Eight case studies were performed with the family physician as the unit of analysis. The sources of data collection for the micro-level were eight life history interviews with family physicians, documents and observational evidence. The sources of data collection for the macro-level were documents, and eight open-ended focused interviews with key informants, from nine medical organizations. The analytic induction approach to data analysis was used.
The empirical article presents all the empirical findings of the thesis. The findings show an increasing integration of economics concepts into the official discourse of family medicine organizations. However, at the level of practice, the economization of this discourse does not seem to be true depictions of reality as the very great majority of the study participants do not embody this discourse. The contributions include a deep understanding of the social processes that influence family physicians’ schemes of perception, thought, appreciation and action with respect to the role of economic evaluation in their practices, and the family physicians’ willingness to contribute to efficient, fair and legitimate resource allocation.

The Effect of Fall Prevention Exercise Programmes on Fall Induced Injuries in Community-Dwelling Older Adults / La prévention des chutes et des blessures dues aux chutes par l’exercice physique chez les personnes âgéesEl-Khoury, Fabienne 15 May 2015 (has links)
IntroductionLes chutes et les blessures dues aux chutes représentent un véritable enjeu de santé publique. Les programmes d’exercices physiques axés sur l’équilibre permettent de réduire de 30 à 40% le taux de chutes chez les personnes âgées vivant à leur domicile. Cependant, leur efficacité sur la prévention des traumatismes dus aux chutes n’a pas été établie.Ce travail comporte 2 parties :- Une revue systématique de la littérature et méta-analyse des résultats d’essais contrôlés randomisés (ECR) qui évaluent l’efficacité de l’exercice sur différents types de chutes traumatiques chez les personnes âgées en milieu communautaire. - L’analyse des données de l’ECR multicentrique ‘Ossébo’, qui évalue l’efficacité d’un programme d’exercice physique de prévention de chutes traumatiques chez des femmes âgées.MéthodesRevue systématiqueDes recherches bibliographiques ont été effectuées pour repérer les ECR de prévention des chutes par l’exercice physique, réalisés chez des personnes âgées vivant à leur domicile, et présentant des données sur les chutes traumatiques.Ensuite, on a regroupé les définitions des chutes traumatiques trouvées dans les études sélectionnées en 4 catégories :A/ avec conséquence.B/ avec recours à des soins médicaux. C/ ayant entraîné un traumatisme grave.D/ avec fracture.On a réalisé une méta-analyse (MA) pour chaque catégorie, donc on a calculé un effet global (effet poolé) de l’exercice correspondant au ratio des taux d’incidence dans les 2 groupes par un modèle à effet aléatoire.L’essai OsséboLes participantes à l’essai sont des femmes âgées de 75 à 85 ans, vivant à leur domicile, et ayant des capacités physiques diminuées. Au total, 706 femmes, dans 20 centres en France, ont été randomisées en 2 groupes : le groupe intervention (GI), et le groupe contrôle (GC).L’intervention comprend des ateliers hebdomadaires d’exercice en petits groupes pendant 2 ans, et des exercices au domicile.La survenue de chutes a été enregistrée à l’aide des cartes-calendriers. Les circonstances et les conséquences de la chute étaient demandées en cas de signalement d’une chute, afin de classer la chute (sans conséquence, traumatisme modéré, traumatisme grave).Des bilans ont été effectués à 1 an et 2 ans après l’inclusion, selon le même protocole que le bilan initial, qui comprenait notamment des tests fonctionnels simples.Le critère principal est le taux d’incidence des ‘chutes traumatiques’ (modérée et graves). Des modèles à fragilité (modèles de survie avec un effet aléatoire) ont été utilisés pour modéliser ce taux dans les 2 groupes.L’évolution au cours du temps des capacités physiques, et d’autres facteurs ont été comparés grâce à un modèle marginal avec un effet aléatoire au niveau du centre.RésultatsRevue systématique17 essais totalisant 4305 participants ont été sélectionnés. Toutes les interventions évaluées comprenaient des exercices de l’équilibre. Les résultats de la MA montrent que l’exercice est associé à une réduction du taux de chutes traumatiques dans chacune des catégories considérées, avec un effet poolé de 0.63 (IC95% : 0.51-0.77) pour la catégorie A(10 essais). Le RaR poolé était de 0.70 (0.54-0.92) pour la catégorie B (8 essais), de 0.57 (0.36-0.90) pour la catégorie C (7 essais), et de 0.39 (IC 95% : 0.22-0.66) pour la catégorie C (6 essais). L’essai OsséboOn a recensé 397 chutes traumatiques dans le GC, et 305 dans le GI, correspondant à une réduction significative de 19% du taux de chutes traumatiques (‘hazard ratios’ HR= 0.81 IC95% : (0.67 - 0.99). A 2 ans, les femmes du GI ont des performances significativement meilleures que les femmes du GC sur l’ensemble des tests physiquesDiscussionLes programmes d’exercice destinés à prévenir les chutes sont également efficaces pour réduire les traumatismes dus à la chute, y compris les plus graves. Aussi, il est possible de mettre en place à large échelle un programme efficace d’exercice de prévention des chutes traumatiques de longue durée chez des personnes âgées / Context: Exercise programmes can prevent falls in older community-dwellers. However, evidence that these programmes can also prevent injurious falls was poor.Objectives : Systematic review of evidence of the effect of exercise interventions on injurious fall prevention from randomised controlled trials (RCT).Evaluate the effectiveness of ‘Ossébo’, a multi-centre RCT assessing the effectiveness of a 2-year injurious fall prevention balance training programme.Methods:Systematic reviewThe definitions of injurious falls from included studies were classified into more homogeneous categories. This allowed the estimation of a pooled rate ratio for each injurious falls category based on random effects models. Ossébo trial706 women aged 75-85 years ; home-living with diminished functional capacities were included. The 2 groups were compared for rates of injurious falls with a frailty model. Other outcomes included physical functional capacities, and quality of life indicators. Results:Systematic review17 trials involving 4305 participants were included. Four categories were identified: all injurious falls, falls resulting in medical care, severe injurious falls, and falls resulting in fractures. Exercise had a significant preventive effect in all categories.OsséboThere were 305 injurious falls in the intervention group and 397 in the control group, for a HR of 0.81 (0.67 to 0.99). At 2 years, women in the intervention group had significantly better performances on all physical tests and a better perception of their overall physical function. Conclusion:Fall prevention exercise programmes are effective in preventing injurious falls, and are feasible for long-term, wide-spread dissemination

Opposé aux modèles individualistes de l’action rationnelle, Bourdieu préconise une approche sociologique qui s’inscrit dans la recherche d’une compréhension plus complète et plus complexe des phénomènes sociaux en mettant en lumière les influences souvent implicites qui viennent chaque jour exercer des pressions sur les individus et leurs pratiques.
L’article méthodologique présente le protocole d’une étude qualitative de cas multiples avec niveaux d’analyse imbriqués : les médecins de famille (niveau micro-individuel) et le champ de la médecine familiale (niveau macro-structurel). Huit études de cas furent réalisées avec le médecin de famille comme unité principale d’analyse. Pour le niveau micro, la collecte des informations fut réalisée à l’aide d’entrevues de type histoire de vie, de documents et d’observation. Pour le niveau macro, la collecte des informations fut réalisée à l’aide de documents, et d’entrevues de type semi-structuré auprès de huit informateurs clés, de neuf organisations médicales. L’induction analytique fut utilisée.
L’article empirique présente l’ensemble des résultats empiriques de la thèse. Les résultats montrent une intégration croissante de concepts en économie dans le discours officiel des organisations de médecine familiale. Cependant, au niveau de la pratique, l'économisation de ce discours ne semble pas être une représentation fidèle de la réalité puisque la très grande majorité des participants n'incarnent pas ce discours. Les contributions incluent une compréhension approfondie des processus sociaux qui influencent les schèmes de perception, de pensée, d’appréciation et d’action des médecins de famille quant au rôle de l’évaluation économique dans la pratique clinique et la volonté des médecins de famille à contribuer à une allocation efficiente, équitable et légitime des ressources. / Health economic evaluations are analytic techniques to assess the relative costs and consequences of health services. Their role is to inform the decision-making process. A vast amount of resource allocation decisions are undertaken at the clinical-encounter level; especially in primary care. Since every decision has an opportunity cost, ignoring economic information in family physicians’ practices may have a broad impact on health care efficiency. There is little evidence on the influence of economic evaluation on clinical practice. The objective of the thesis is to understand the role of economic evaluation in family physicians’ practices. Its contributions are presented in four original articles (philosophical, theoretical, methodological, and empirical).
The philosophical article suggests that complexity and reflexivity are two important issues for economic evaluation. Complexity thinking is the philosophical perspective (overarching epistemological approach) underpinning the thesis. This way of thinking focuses attention on explanation and understanding and gives particular emphasis to relations and interactions (interactive causality). This increased emphasis on the context and process of data production highlights the importance of reflexivity in the research process.
The theoretical article develops a new and different conceptualization of the research problem. The originality of the thesis also lay in the research problem being approached from the perspective of Pierre Bourdieu's sociological theory. Bourdieu’s approach embraces complexity. Moving away from individualist, rational models of action, it can contribute to a more complete and complex understanding of social phenomena by revealing the structuring effects of social fields on the individual’s dispositions and practices.
The methodological article presents the protocol of a qualitative embedded multiple-case study research. There were two embedded units of analysis: the family physicians (micro-individual level) and the field of family medicine (macro-structural level). Eight case studies were performed with the family physician as the unit of analysis. The sources of data collection for the micro-level were eight life history interviews with family physicians, documents and observational evidence. The sources of data collection for the macro-level were documents, and eight open-ended focused interviews with key informants, from nine medical organizations. The analytic induction approach to data analysis was used.
The empirical article presents all the empirical findings of the thesis. The findings show an increasing integration of economics concepts into the official discourse of family medicine organizations. However, at the level of practice, the economization of this discourse does not seem to be true depictions of reality as the very great majority of the study participants do not embody this discourse. The contributions include a deep understanding of the social processes that influence family physicians’ schemes of perception, thought, appreciation and action with respect to the role of economic evaluation in their practices, and the family physicians’ willingness to contribute to efficient, fair and legitimate resource allocation.
