Validering av Alberta Context Tool (ACT) för bruk inom svensk kommunal äldrevård : Ett verktyg för implementering av evidensbaserad vård

The aim of this study was to conduct an instrument test of the Canadian questionnaire Alberta Context Tool (ACT) version Long-Term care for Swedish conditions. ACT is designed in order to measure the context in the care environment and different behaviours related to the changes in clinical practice. In total, 159 Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) within municipality care of the elderly were included in the survey. The test included the instrument's reliability and face validity.The reliability test was implemented through calculation of Cronbach´s Alpha, and showed internal consistency for five of the scales of the ACT-instrument with Cronbach´s Alpha values ranging between 0,728 and 0,873. However, three dimensions got lower values (0,558 - 0,683).The analysis was carried out with content analysis and carried out for LPNs and RNs in separate groups. The majority of LPNs expressed that it was easy to respond to the questions (56%), while nine percent considered it as difficult. Eleven comments were given about questions that were perceived to be unclear, complicated or contained difficult words. In the RN group only 30 percent considered that the questions were easy to respond to. One third of the RNs considered that part of the questions were unclear, and six RNs expressed also which questions they experienced as unclear. In general, the questions in the ACT were perceived as relevant. The instrument's relevance as a tool to measure contextual factors that influence the implementation of evidence based nursing can also be considered to be determined. By modifying the content in the questionnaire in accordance with what appeared in this survey and to implement yet another test, the instrument should be considered to be relevant for use within Swedish municipality care of the elderly. ACT can be used both as a tool in the work on improvement of clinical practice and as a tool for further research about implementation of evidence based nursing.

Bakgrund: Personer med diabetes mellitus typ-2 är ofta väl medvetna om komplikationer som föreligger. De lever med rädsla och ångest inför framtiden. Sjuksköterskans roll att stötta och hjälpa personer med diabetes mellitus typ-2 är viktig så vårdtagaren klarar sin egenvård. För att handleda och uppmuntra till egenvård hos vårdtagaren måste sjuksköterskan utgå ifrån evidens vid omvårdnad av diabetes mellitus typ-2. Syfte: Syftet var att beskriva effekten av fysisk aktivitet hos personer med diabetes mellitus typ-2. Metod: En litteraturöversikt genomfördes för en överblick av kunskapsläget. I förhållande till syftet har kvantitativa artiklar använts i uppsatsen. Sexton artiklar har analyserats med Fribergs trestegs-modell. Resultat: Resultatet visar att interventioner i form av fysisk aktivitet leder till en sänkning av blodtrycket och en förbättrad cirkulation i benen. Även Hba1c, f-P-glukos och insulinresistensen förbättras av fysisk aktivitet. Interventionerna visade sig även ge en minskning av kroppsfett, midjemått och kroppsvikt. Slutsats: Fysisk aktivitet är en grundläggande faktor vid utformandet av omvårdnaden och fysisk aktivitet kan leda till förbättrad hälsa. Att vårdtagaren förstår varför fysisk aktivitet är bra och första steget för ökad egenvårdskapacitet hos personer med diabetes mellitus typ-2. För att hjälpa dessa individer att uppnå ökad förståelse behöver sjuksköterskor specifik kunskap om effekterna av fysisk aktivitet vid diabetes mellitus typ-2. / Background: People with diabetes mellitus type-2 are often aware of the possibility of complications. They live with fear and anxiety concerning the future. The role of the nurse is to support and help people with diabetes mellitus type-2 which enables them to manage their self-care. In order to supervise and encourage self-care to the patient an evidence-based knowledge in diabetes mellitus type-2 is needed by the nurse. Aim: The aim was to describe the effects of physical activity in persons with diabetes mellitus type-2. Method: A literature review was made to get an overview of the state of knowledge. In relation to the aim quantitative articles is used in the essay. Sixteen articles have been analyzed with Friberg´s three stage model. Result: The result shows that interventions in form of physical activity give a lower blood pressure and a better blood circulation in the legs. Even Hba1c, fP-glucose and the insulin resistance gets better after physical activity. The interventions also lower the amount of body fat, the waist measurement decreases and the total body weight decreases. Conclusion: Physical activity is a key factor in planning the nursing care and physical activity can lead to improved health. A patients understanding of why physical activity is beneficial is the first step to increase self-care capacity in people with diabetes mellitus type-2. To help these individuals achieve greater understanding nurses need specific knowledge about the effects of physical activity on diabetes mellitus type-2.

Evidence-based guidelines of using cryotherapy in reducing pain, knee swelling and improving range of motion for patients after total kneereplacement

Introduction: Knee osteoarthritis is one of the common causes leading to musculoskeletal disability of the elderly around the world. Total knee replacement (TKR) is an effective and common treatment for end stage knee arthritis. Most papers suggest that early rehabilitation could improve postoperative knee function. However, postoperative pain and local swelling are the complications that diminish range of motion (ROM) and inhibit patients’ recovery. A comprehensive review of the literature reveals that cryotherapy is an effective and safe method to overcome these complications. In this paper, a guideline of using cryotherapy in reducing postoperative pain and knee swelling so as to improve the ROM of patients and promote recovery after TKR has been developed. Objectives: The objectives of this translational research are: (1) to look for relevant papers that related to cryotherapy in reducing postoperative pain and knee swelling so as to improve the ROM of patients after TKR; (2) to gather and criticize the data obtained; (3) to develop a guideline of using cryotherapy in reducing pain, knee swelling and improving ROM of patients after TKR based on the evidence from the reviewed literature; (4) to assess the implementation potential of the newly developed guidelines; and (5) to establish the implementation and evaluation plans for the new innovation. Methods: An empirical literature search published from 2001 to 2011 by several searching engines regarding cryotherapy in reducing postoperative pain and knee swelling with the aim to improve the ROM of patients after TKR has been conducted. After that, the qualities of relevant studies were retrieved and criticized by using the appraisal checklist of the SIGN (2004). The derived evidences were then be summarized and synthesized. An evidence-based guideline was established with reference to the evidence from the reviewed literatures and the results of the quality assessment. Recommendations are graded by SIGN (2004). The implementation potential including transferability, feasibility and cost-benefit ratio of the innovation were assessed as well. Lastly, implementation and evaluation plans have been developed to assess and appraise the effectiveness of the new guideline. Results: Finally, seven studies were chosen as final references after methodological quality assessment. Four main types of comparison were made from these seven reviews including: (1) continuous compressive cryotherapy vs. compressive crepe bandage; (2) comparison in different temperature of cryotherapy; (3) outcome measures; and (4) complications in cryotherapy. After the summary, six main categories of recommendations were synthesized: (1) continuous compression cryotherapy; (2) effective temperature; (3) potential complications; (4) regular assessment; (5) duration; and (6) intermitted ice pack regimen. Based on these syntheses and recommendations, a guideline of using cryotherapy for patients after having TKR was developed. Conclusion: With the implementation of the newly developed evidence-based cryotherapy guidelines for patients after having TKR in local clinical settings, the improvement in the ROM of the knee joint will be anticipated as a result of the reduction in postoperative pain and knee swelling. / published_or_final_version / Nursing Studies / Master / Master of Nursing

Bakgrund: Snabb kunskapsutveckling, ökade krav på kvalité och kostnadseffektivitet inom hälso- och sjukvården gör det viktigt att sjuksköterskor har god förmåga att söka och använda evidensbaserad kunskap. Idag finns ett glapp mellan forskning och klinik. Sjuksköterskor upplever flera hinder till att arbeta evidensbaserat. Under hösten 2010 och våren 2011 pågick ett projekt på ett sjukhus i en stad i mellansverige. Projektet hette ”Klinisk bibliotekarie”, syftet var att höja vårdkvalitén och patientsäkerheten genom att det dagliga vårdarbetet ska bli mer evidensbaserat och att sjuksköterskorna ska känna större säkerhet i att ta fram vetenskapligt underlag för sitt arbete. Syfte: Belysa sjuksköterskornas erfarenhet av projektet ”Klinisk bibliotekarie”. Metod: Studien har en kvalitativ design. Data samlades in genom fokusgrupper och observationer. Tjugoen sjuksköterskor deltog i projektet ”Klinisk Bibliotekarie” och av dessa har nitton deltagit i föreliggande studie genom fokusgruppdiskussioner. Observationer har skett vid två tillfällen. Data analyserades med manifest och latent innehållsanalys. Resultat: Sjuksköterskorna anser att interventioner för att öka evidensbaserat arbete ska baseras på frivillighet och intresse. Vidare anser sjuksköterskorna att det ska avsättas adekvat med tid för det arbetet. En positiv effekt av projektet är, enligt sjuksköterskorna, att det etablerats en kontakt med bibliotekarien. Konklusion: För att omvårdnaden ska bli mer evidensbaserad behövs intresserade sjuksköterskor och avsatt tid. En klinisk bibliotekarie kan vara ett stöd för sjuksköterskorna. / Background: Rapid development of knowledge, increased demands on quality and cost efficiency within healthcare makes it important that nurses have the ability to seek and use evidence-based knowledge. Today there is a gap between research and clinic. Nurses are experiencing multiple barriers to work evidence based. In autumn 2010 and spring 2011 has a project been going on at a hospital in a town in central Sweden. The project was called "Clinical Librarian" and aimed to raise the quality of care and patient safety by the daily care work to become more evidence based and that nurses should feel more secure in the development of scientific basis for its work. Objective: To highlight nurses' experience of working with the project "Clinical Librarian". Method: The study has a qualitative design. Data has been collected through focus groups and observations. Twenty-one nurses participated in the project “Clinical Librarian” and of these nineteen participated in this study through focus group discussions. Observations have been made on two occasions. Data has been analyzed with manifest and latent content analysis. Results: The nurses feel that interventions to improve evidence-based nursing should be based on voluntary participation and interest. They also believe that it should be allocated adequate time for such work. A positive impact of the project, according to the nurses, is that it established a contact with the librarian. Conclusion: To become more evidens-based tce care needs interestad nurses and time for the purpose. A clinical librarian can be a support for the nurses.

Este estudo propõe avaliar o nível de evidência de três atividades de enfermagem da intervenção Controle de Infecção.Objetivo:Avaliar o nível de evidência das atividades de enfermagem relacionadas à orientação e ensino sobre a lavagem das mãos, da Intervenção de Enfermagem Controle de Infecção, apresentada na Classificação das Intervenções de Enfermagem (NIC).Método:Estudo de revisão integrativa da literatura. O estudo parte de três perguntas de pesquisa sobre o nível de evidência do ensino da lavagem das mãos para profissionais de saúde, pacientes e visitantes.Foram utilizadas três estratégias de busca nas bases Cochrane, CINAHL e Pubmed em Janeiro/2015 e incluídos artigos publicados em inglês, português ou espanhol no período de 2004 a 2015.Foi utilizada a classificação de nível de evidência do Instituto Joanna Briggs. Resultados:No ensino ao profissional de saúde, foram incluídos 39 artigos e os estudos apresentavam nível de evidência 1 (01; 2%), 2 (11; 28%) e 3 (27; 69%). No ensino ao paciente foram incluídos cinco artigos, que apresentavam nível de evidência 1 (01; 20%), 2 (01; 20%) e 3 (03; 60%).
