O tratamento do hipogonadismo hipogonadotrófico na mulher adulta com hipopituitarismo inclui diversas alternativas terapêuticas de estrógenos e progestágenos, sendo a via oral a de menor custo e a de maior comodidade à paciente. A rota estrogênica oral, entretanto, exerce marcada influência sobre o eixo hormônio de crescimento/fator de crescimento insulinasímile número 1 (GH/IGF-1) nessas mulheres. O tratamento com estrógenos orais, concomitante ao uso de GH em pacientes com hipopituitarismo, antagoniza as ações biológicas do GH e agrava as anormalidades de composição corporal e o metabolismo em geral. Presume-se que o estrógeno oral iniba a secreção/produção de IGF-1 através de um efeito de primeira passagem hepática, causando um aumento da secreção de GH através de inibição do feedback negativo de IGF-1 em mulheres normais. Isso é demonstrado clinicamente por redução da massa magra, aumento da massa gorda, perfil lipídico aterogênico e prejuízo do bem-estar psicológico. Alguns estudos apontam que os progestágenos com ação androgênica revertem o efeito de diminuição dos níveis séricos de IGF-1 induzida pelos estrógenos orais. Progestágenos neutros não apresentam esse efeito, porém, quanto maior a potência androgênica, maior será a reversão do efeito de diminuição de IGF-1. Apresenta altas taxas de comorbidades clínicas e risco de suicídio trazendo prejuízos e custos significativos para o indivíduo com a doença e para a sociedade. Existem evidências que relacionam o TB à alterações no fator de crescimento semelhante à insulina tipo 1 (IGF- 1) e nos sistemas endócrino e imune. O objetivo deste estudo foi avaliar os níveis séricos de IGF-1 em pacientes bipolares comparados com indivíduos controle e sua relação com a inflamação. Foram selecionados 31 pacientes com TB e 33 controles saudáveis. Foram avaliadas as concentrações séricas de IGF-1, hormônio do crescimento (GH), insulina e fator de necrose tumoral α (TNF-α). Como resultado deste estudo, observamos que os níveis séricos de IGF-1 estavam aumentados em pacientes com TB em relação aos controles (p = 0,001). Não foram observadas diferenças estatisticamente significativas entre os grupos nas dosagens de insulina, GH e TNF- α. Este estudo sugere uma associação entre IGF-1 na fisiopatologia do transtorno bipolar. É possível que este aumento periférico esteja relacionado com um aumento da resistência do IGF- 1 no SNC, reduzindo assim a sua ação neuroprotetora. / Bipolar disorder (BD) is a chronic, highly debilitating and its pathophysiology is not well understood. It offers high rates of clinical comorbidities and suicide risk causing losses and significant costs to the individual with the disease and society. There is evidence that relates to changes in TB-like growth factor type 1 insulin (IGF-1) and the endocrine and immune systems. The aim of this study was to evaluate serum levels of IGF-1 in bipolar patients compared with control subjects and their relationship to inflammation. We selected 31 patients with TB and 33 healthy controls. Serum concentrations of IGF-1, growth hormone, were evaluated (GH), insulin and tumor necrosis factor α (TNF-α). As a result of this study, we observed that serum levels of IGF-1 were increased in TB patients compared to controls (p = 0.001). No statistically significant differences were found between groups in insulin dosages, GH, and TNF-α. This study suggests an association between IGF-1 in the pathophysiology of bipolar disorder. It is possible that this increase is associated with peripheral increased IGF-1 resistance in the CNS, thus reducing its neuroprotective action.

Previous issue date: 2013 / Associação Fundo de Incentivo à Psicofarmacologia (AFIP) / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) / A doenca de Parkinson (DP) e uma patologia neurodegenerativa, caracterizada pelo comprometimento motor, devido a degeneracao dos neuronios dopaminergicos na via nigro-estriatal. A forma de tratamento mais utilizada e por meio da administracao de levodopa (L-Dopa). Porem, o uso continuo desta substancia, a longo prazo, pode acarretar alteracoes bioquimicas, cognitivas e comportamentais. Uma possivel forma de amenizar esses problemas seria pela pratica de exercicio fisico. Desta forma, o objetivo do presente estudo foi analisar os efeitos do exercicio fisico sobre os aspectos bioquimicos, cognitivos e comportamentais de individuos com a DP. Participaram deste estudo 39 pessoas, sendo 11 do grupo controle (GC) u neurologicamente saudaveis que nao praticou exercicio fisico; 13 do grupo controle com DP (GCP) u nao participou do programa de exercicio fisico; e 15 do grupo treinado com DP (GTP) u participou de um programa de exercicio fisico multimodal, durante dois meses, tres vezes por semana, uma hora por sessao. Todos foram avaliados, pre e pos-intervencao, quanto aos niveis de atividade fisica, progesterona, estradiol, homocisteina, desidrogenase lactica, fator de crescimento semelhante a insulina (IGF-1) e glutationa peroxidase. Foram avaliadas tambem as funcoes cognitivas (testes de memoria, funcoes executivas e atencao). As variaveis comportamentais foram: ansiedade, sintomas depressivos e estresse. Apos as coletas dos dados foram comparados os resultados entre os grupos por meio do teste General Linear Model (GLM). Os resultados apontaram interacao significativa entre os grupos e momentos apenas em homocisteina (F=4,20; p=0,02) e IGF-1 (F=6,97; p<0,01). Nao foram encontradas interacoes significativas entre grupos e momentos nas demais variaveis bioquimicas (p>0,05), bem como nas avaliacoes cognitivas e comportamentais (p>0,05). Rio de Janeiro, RJ, Brasil / A expressão de arginase-1 (Arg1) está associada com a patogênese da leishmaniose visceral (LV), embora os mecanismos que norteiam a sua regulação na doença não sejam adequadamente conhecidos. Nesta tese foram explorados os mecanismos de regulação da Arg-1 no modelo experimental hamster, a fim de aplicar o conhecimento para o tratamento da doença. Encontrou-se que células infectadas in vitro com Leishmania (Leishmania) donovani expressaram Arg-1 e inhibiram a produção do óxido nítrico, o que contribuiu para o aumento da carga parasitária. A regulação da Arg1 dependeu do fator de transcrição STAT-6, que foi ativado de forma independente da IL-4. Para explorar outros mecanismos envolvidos na ativação de STAT-6 foi utilizada uma biblioteca quimica de 80 inibidores dos receptores tirosine quinases (RTK) e uma membrana microarranjo revestidas com anticorpos contra proteínas phosphoriladas dos RTK. Desta forma, foi possível identificar a ativação do receptor do fator de crescimento de fibroblasto-1 (FGFR-1) e o receptor do fator de crescimento semelhante à insulina tipo I (IGF-IR) nos macrofagos. Além disso, realizou-se uma caracterização mais detalhada da cascata de fosporilação canônica dessos receptores. Experimentos in vitro mostraram que macrófagos infectados com L. donovani aumentaram a transcrição de Arg1 quando foram tratados com os ligantes dos RTK, o FGF-2 e o fator de crescimento semelhante à insulina tipo 1 (IGF-I), sugerindo que estes RTK tinham um papel na regulação da Arg1 na LV
Quando inibidores químicos do FGFR-1 e IGF-IR foram testados em macrófagos infectados, a Arg1 e o STAT-6 foram inibidos sugerindo uma ligação entre essas vias. De fato, a estimulação com FGF-2 e IGF-I aumentou a fosforilação e a ativação transcripcional do STAT-6, e se observou um efeito aditivo quando a citocina IL-4 foi adicionada, demonstrando a interação positiva destas vias. A inibição do STAT-6 mediante interferência de RNA impediu ou diminuiu significativamente a Arg-1 induzida nos macrófagos infectados e estimulados com FGF-2 e por IGF-I. respectivamente, indicando um papel chave do STAT-6 na Arg-1 induzida pelos RTK. A relevância destes achados experimentais para os seres humanos foi avaliada em cultivos de sangue total de doadores sadios estimulados in vitro e no plasma de crianças durante a doença ativa, um mês, seis meses e até um ano após tratamento. Foi observado um aumento de Arg-1 após o estimulo in vitro com IL-4 e FGF-2 ou IGF-I, consistente com o achado no modelo hamster. Nos pacientes, os níveis plasmáticos de Arg-1 não mudaram com a evolução da doença após e o tratamento e o FGF-2 só apresentou um aumento associado com a resolução um ano após tratamento. Em conclusão, os sinais comuns ativados através do FGF-1R e IGF-IR convergem com a IL-4 para ativar STAT-6 e a transcrição de Arg-1 na LV experimental, mas determinar o papel destes fatores de crescimento na LV humana requer estudos adicionais. The aim of this study was to evaluate serum levels of IGF-1 in bipolar patients compared with control subjects and their relationship to inflammation. We selected 31 patients with TB and 33 healthy controls. Serum concentrations of IGF-1, growth hormone, were evaluated (GH), insulin and tumor necrosis factor α (TNF-α). As a result of this study, we observed that serum levels of IGF-1 were increased in TB patients compared to controls (p = 0.001). No statistically significant differences were found between groups in insulin dosages, GH, and TNF-α. This study suggests an association between IGF-1 in the pathophysiology of bipolar disorder. It is possible that this increase is associated with peripheral increased IGF-1 resistance in the CNS, thus reducing its neuroprotective action.
