EXPLORING THE RELATIONSHIP BETWEEN EMOTIONAL BRAIN TRAINING, STRESS, DEPRESSION, FOOD ADDICTION, AND WEIGHT

Obesity is a complex issue; stress, depression, and food addiction, are several psychological conditions that can accompany an obesity diagnosis. Emotional Brain Training (EBT) was evaluated as a new approach to these conditions. Stress, depression, food addiction, and weight were assessed at baseline, after a seven week active intervention, and after a seven week no contact period. The final sample consisted of 26 obese adults. At seven week assessments, EBT participants experienced significant weight loss (p-value = 0.05) and decreased perceived stress (p-value = 0.035). Food addiction also decreased from 50% to 8.3% (or one participant). At fourteen week assessments, EBT participants maintained significant weight loss (p-value = 0.05) and increased perceived stress from the seven week evaluation (p-value = 0.012). The percentage classified as food addicted remained constant at fourteen weeks. This study suggests EBT is an appropriate intervention for weight loss and weight maintenance. In addition, EBT targets a variety of the complex issues surrounding obesity.

Affectivité et alimentation : étude de leurs liens au travers des concepts d’alimentation émotionnelle et d’addiction à l’alimentation / Affectivity and food intake : a study of their links through the concepts of emotional eating and food addiction

L’objectif de ce travail de thèse était d’approfondir la compréhension des liens entre le vécu émotionnel et la prise alimentaire, au travers des deux construits que sont l’alimentation émotionnelle (AE) et l’addiction à l’alimentation (AA). En premier lieu, ce travail a permis de lever un verrou méthodologique relatif à la mesure de l’AE, en réalisant la validation de la version française d’un instrument d’évaluation exhaustif (Emotional Appetite Questionnaire), prenant en compte la variabilité interindividuelle et inter-émotions spécifique à l’AE. La suite de nos travaux, réalisés en population étudiante, ont permis 1) de caractériser différents profils de mangeurs émotionnels; 2) de démontrer que l’AE et l’AA jouent un rôle médiateur entre l’affectivité négative et le surpoids ou l’obésité; 3) de mettre en évidence que le recours à l’AE est un rouage clé dans l’apparition de symptômes cliniques d’AA. La dernière partie de ce travail était consacrée à l’étude de l’impact de l’AE et de l’AA en population clinique, chez des patients souffrant de maladies cardiovasculaires. Les résultats préliminaires de nos travaux mettent en évidence la complexité des liens entre la détresse émotionnelle, les perturbations de la prise alimentaire et la qualité de l’alimentation chez ces patients. Finalement, ce travail de thèse apporte un éclairage sur l’intrication complexe entre les émotions et l’alimentation ainsi qu'une meilleure identification des facteurs pouvant influencer ces liens. De telles connaissances sont cruciales afin d’améliorer les politiques de prévention et favoriser le développement de prises en charge plus ciblées et efficaces. / The objective of this thesis was to deepen the understanding of the links between emotional experience and food intake, by examining the role of two mechanisms: emotional eating (EE) and food addiction (FA). First, this work allowed to overcome a methodological barrier regarding the measurement of EE by validating the French version of the Emotional Appetite Questionnaire. This instrument provides an exhaustive assessment of EE, taking into account the inter-individual and inter-emotional variability specific to this concept. Second, our work, which was carried out in large sample of college students, allowed 1) to characterize different profiles of emotional eaters; 2) to demonstrate that EE and FA mediate the association between negative affectivity and excessive weight or obesity; 3) to demonstrate that EE is a key factor in the severity of clinical symptoms of FA. The last part of this work was devoted to studying the impact of EE and FA in a clinical population of patients with cardiovascular diseases. The preliminary results of this study highlighted the complexity of the links between emotional distress, disordered eating and the quality of food intake in these patients. In conclusion, this thesis sheds light on the complex interplay between emotions and food intake, and provides a better identification of the factors that can influence these links. Such knowledge is crucial in order to improve prevention policies and foster the development of more targeted and effective interventions.

Repenser la désinhibition alimentaire dans l’obésité, sous l’angle de l’hypothèse de l’addiction à l’alimentation / Rethinking food disinhibition in obesity from the perspective of the food addiction hypothesis

L’addiction à l’alimentation (AA) est un concept ancien, mais encore sujet de controverses qui toucherait 18 à 24 % des obèses. Dans cette thèse, nous nous sommes intéressés aux intrications entre régulations du comportement alimentaire, addiction et AA, afin de mieux comprendre les mécanismes liés à la désinhibition de la prise alimentaire. Notre travail a conduit à plusieurs résultats : 1) Une revue de la littérature a permis de montrer l’existence d’interaction étroite entre les niveaux de régulation du comportement alimentaire et comment AA pourrait en être un exemple de dérégulation. 2) Une étude de cohorte a permis de démontrer l’existence de caractéristiques cliniques communes entre addictions et AA et celle d’un probable transfert d’addiction de la nicotine vers l’alimentation. 3) Sur le plan expérimental, nous avons montré qu’il existe des perturbations des ondes P300 et N200, dans l’obésité et la désinhibition alimentaire, proches de celles observées dans les addictions. 4) Enfin, nos résultats suggèrent le rôle potentiel de la ghréline comme marqueur de risque de trouble du comportement alimentaire. Ce travail ouvre des perspectives sur le plan expérimental, avec la suggestion de groupes contrôles plus pertinents ; cliniques, avec la création d’un potentiel test de dépistage rapide ; thérapeutiques, avec la mise en place d’un essai thérapeutique par tDCS / Food addiction (FA) is an old concept, but still subject to controversy. It affects 18 to 24% of obese people. In this thesis, we are interested in overlaps between food behavior regulation, addiction and FA, in order to better understand the mechanisms linked to food intake disinhibition. Our work leads to several results: 1) A review of the literature has shown the existence of close interaction between eating behavior regulation levels and how FA could be an example of their disturbance. 2) A cohort study demonstrated the existence of common clinical features between addiction and FA and a probable addiction transfer from nicotine to food. 3) From an experimental point of view, we have shown that there are disturbances of P300 and N200 ERP, in obesity and food disinhibition, close to those observed in addictions. 4) Finally, our results suggest the potential role of ghrelin as a marker for eating disorders increased risk. This work opens experimental perspectives, with the suggestion of more relevant control groups; clinical perspectives, with the creation of a screening tool; therapeutics perspectives, with the establishment of a therapeutic trial by tDCS

Reward processing in obesity, substance addiction and non-substance addiction

Similarities and differences between obesity and addiction are a prominent topic of ongoing research. We conducted an activation likelihood estimation meta-analysis on 87 studies in order to map the functional magnetic resonance imaging (fMRI) response to reward in participants with obesity, substance addiction and non-substance (or behavioural) addiction, and to identify commonalities and differences between them. Our study confirms the existence of alterations during reward processing in obesity, non-substance addiction and substance addiction. Specifically, participants with obesity or with addictions differed from controls in several brain regions including prefrontal areas, subcortical structures and sensory areas. Additionally, participants with obesity and substance addictions exhibited similar blood-oxygen-level-dependent fMRI hyperactivity in the amygdala and striatum when processing either general rewarding stimuli or the problematic stimuli (food and drug-related stimuli, respectively). We propose that these similarities may be associated with an enhanced focus on reward – especially with regard to food or drug-related stimuli – in obesity and substance addiction. Ultimately, this enhancement of reward processes may facilitate the presence of compulsive-like behaviour in some individuals or under some specific circumstances. We hope that increasing knowledge about the neurobehavioural correlates of obesity and addictions will lead to practical strategies that target the high prevalence of these central public health challenges.

Isolamento social precoce, acesso crônico à dieta rica em sacarose e a programação do sistema dopaminérgico: susceptibilidade a psicoestimulantes e a alimento palatável na vida adulta

A infância e a adolescência são períodos sensíveis de maturação neuronal, caracterizados por alta plasticidade de circuitos encefálicos em desenvolvimento, como é o caso do sistema mesolímbico dopaminérgico. Experiências estressantes neste período, como o isolamento social (IS), podem produzir neuroadaptações nesses circuitos e aumentar a vulnerabilidade ao consumo de drogas e de alimentos palatáveis ao longo da vida. Tendo em vista que extensa literatura analisa longos períodos de isolamento social, que não são modelos adequados para o estresse por isolamento que ocorre em sociedades humanas, o objetivo do presente estudo foi investigar os efeitos de uma exposição curta ao isolamento social durante o período pré-púbere sobre o sistema mesolímbico dopaminérgico e a susceptibilidade para o abuso de drogas e para o consumo compulsivo de alimento palatável em ratas Wistar fêmeas, na idade adulta. Também foi objetivo avaliar o papel da exposição crônica a uma dieta rica em sacarose (DRS) sobre estas variáveis. Como resultados, foi observado que o IS aumentou a resposta locomotora a um desafio com anfetamina, bem como aumentou, no estriado dorsal, o imunoconteúdo do transportador de dopamina, da enzima tirosina hidroxilase e diminuiu os níveis do receptor D2 de dopamina (D2R); além disso, os animais submetidos ao IS na pré-puberdade apresentaram aumento nos parâmetros relacionados ao estresse oxidativo após o desafio. De modo interessante, a exposição a DRS preveniu os efeitos do IS sobre a resposta locomotora, mas não afetou os parâmetros dopaminérgicos. O IS também diminuiu o imunoconteúdo basal de D2R no núcleo accumbens (NAc) e estimulou o consumo do tipo-compulsivo de alimento doce (Froot Loops®). A DRS não interferiu nestes parâmetros. Observamos também que o IS não alterou os níveis basais de corticosterona plasmática, enquanto que a DRS diminuiu tais níveis. Os registros das correntes excitatórias pós-sinápticas (CEPS) espontâneas indicaram, como resultado preliminar, que a DRS reduziu o tempo de subida das CEPS, indicando uma resposta glutamatérgica fugaz. Os achados deste estudo demonstram pela primeira vez que um período curto de IS em uma fase crítica do desenvolvimento é capaz de programar o sistema mesolímbico dopaminérgico de forma a aumentar a susceptibilidade tanto ao uso de drogas quanto ao consumo do tipo-aditivo de alimento doce. Esses efeitos podem ser em parte explicados pela redução dos níveis de D2R basal no NAc e pela maior estimulação do sistema dopaminérgico no estriado frente a um desafio com anfetamina. Os achados desta tese sugerem que experiências estressantes, como o isolamento social, durante um período crítico do desenvolvimento é capaz de programar o sistema de recompensa encefálico de forma permanente e aumentar a susceptibilidade a comportamentos aditivos na vida adulta. Identificar fatores preditores de propensão a esse tipo de comportamento é importante para prevenir o desenvolvimento de dependência de drogas e/ou de distúrbios alimentares, além de possibilitar a identificação de alvos terapêuticos e o desenvolvimento de estratégias de tratamento para estes distúrbios. / Childhood and adolescence are sensitive periods of neuronal maturation, characterized by high plasticity of developing brain circuits, such as the mesolimbic dopaminergic system. Stressful experiences in these periods, such as social isolation (SI), can produce changes in these circuits and increase vulnerability to drug addiction and eating disorders throughout life. Considering that most of literature analyze long periods of social isolation, that are not good models for social stress in human societies, the objective of this study was to investigate the effects of a short post-weaning social isolation on mesolimbic dopaminergic system and the susceptibility to drug and food addiction in female Wistar rats in adulthood. Moreover, we also aimed to evaluate the role of a chronic high sugar diet (HSD) on these variables. It was observed that IS increased the locomotor response to a challenge with amphetamine (AMPH), as well as increased the immunocontent of dopamine transporter, the enzyme tyrosine hydroxylase, decreased the D2 dopamine receptor (D2R) and increased the parameters related to oxidative stress in dorsal striatum after the challenge. Interestingly, exposure to DRS prevented the effects of SI on locomotor response, but did not affect dopaminergic parameters. IS also decreased the basal immunocontent of D2R in the nucleus accumbens (NAc), and stimulated binge eating of high sweet food (Froot Loops®). HSD did not interfere with these parameters. We also observed that SI did not alter plasma corticosterone baseline levels after IS, whereas HSD induced a decrease in these levels. Excitatory postsynaptic currents (EPSC) indicated, as a preliminary result, that the exposure to a HSD reduced the rise time, indicating a more fleeting glutamatergic response. The findings of this study demonstrate for the first time that a short period of SI at a critical period of development is able to programme the mesolimbic dopaminergic system in order to increase susceptibility to both drug and food addiction. These results are possibly due, at least in part, to low basal levels of D2R in NAc and the higher stimulation of the dopaminergic system in striatum after a challenge with AMPH. The findings of this thesis suggest that stressful experiences such as social isolation during a critical period of development are able to permanently program the brain reward system and increase the susceptibility to additive behaviors in adult life. Identifying predisposing factors to this type of behavior is extremely important to prevent the development of drug addiction and/or eating disorders, to identify therapeutic targets and to enable the development of treatment strategies for these disorders.

L’objectif de cette thèse d’articles était de préciser la place respective des mesures objectives (ex., durée et sévérité de la maladie, traitements reçus) et subjectives (ex., troubles psychiatriques, personnalité) en tant que déterminants potentiels de dépression, de qualité de vie (QV) et d’addiction à l’alimentation chez des patient(e)s souffrant de cancer du sein ou d’obésité sévère. En nous basant sur des modèles biopsychosociaux issus du modèle de Wilson et Cleary, nous avons démontré que : 1) La dépression était plus fortement associée aux caractéristiques psychopathologiques de l’individu (personnalité, dépression avant traitement) qu’à la sévérité de la maladie ou aux types de traitements reçus ; 2) Les facteurs associés à la QV dépendent de la dimension de QV considérée (association entre mesures objectives et QV physique ; association entre mesures subjectives et toutes les dimensions de QV) ; 3) Les patients obèses souffrant d’addiction à l’alimentation ont une vulnérabilité psychopathologique spécifique. Nos travaux suggèrent qu’en cas de maladie chronique, les mesures subjectives sont des déterminants potentiels importants de la santé / This article thesis aimed to assess risk factors for three different health outcome measures (depression, health-related quality of life and food addiction) in two populations (breast cancer patients and morbidly obese patients), by assessing the relative impact of biological and physiological variables (i.e., disease duration and severity, types of treatment used), and individual and psychological variables (i.e., psychiatric disorders, personality). By proposing and putting to the test several integrative biopsychosocial models based on Wilson and Cleary’s theoretical model, we demonstrated that : 1) Depression after breast cancer treatment is more strongly associated with variables related to the premorbid individual and psychological characteristics (i.e., personality and depression before treatment) than to biological and physiological variables (i.e., disease severity, types of treatment used) ; 2) Risk factors for lower quality of life are different depending on the quality of life dimension considered (e.g., physical, psychological or sexual) : biological and physiological variables are associated with physical quality of life ; individual and psychological factors are associated with all quality of life dimensions ; 3) Obese patients with food addiction exhibit some specific psychopathological risk factors. Our work suggest to systematically assess individual and psychological variables in patients with chronic diseases because these variables are important potential predictors for different health outcomes

Identificar fatores preditores de propensão a esse tipo de comportamento é importante para prevenir o desenvolvimento de dependência de drogas e/ou de distúrbios alimentares, além de possibilitar a identificação de alvos terapêuticos e o desenvolvimento de estratégias de tratamento para estes distúrbios. / Childhood and adolescence are sensitive periods of neuronal maturation, characterized by high plasticity of developing brain circuits, such as the mesolimbic dopaminergic system. Stressful experiences in these periods, such as social isolation (SI), can produce changes in these circuits and increase vulnerability to drug addiction and eating disorders throughout life. Considering that most of literature analyze long periods of social isolation, that are not good models for social stress in human societies, the objective of this study was to investigate the effects of a short post-weaning social isolation on mesolimbic dopaminergic system and the susceptibility to drug and food addiction in female Wistar rats in adulthood. Moreover, we also aimed to evaluate the role of a chronic high sugar diet (HSD) on these variables. It was observed that IS increased the locomotor response to a challenge with amphetamine (AMPH), as well as increased the immunocontent of dopamine transporter, the enzyme tyrosine hydroxylase, decreased the D2 dopamine receptor (D2R) and increased the parameters related to oxidative stress in dorsal striatum after the challenge. Interestingly, exposure to DRS prevented the effects of SI on locomotor response, but did not affect dopaminergic parameters. IS also decreased the basal immunocontent of D2R in the nucleus accumbens (NAc), and stimulated binge eating of high sweet food (Froot Loops®). HSD did not interfere with these parameters. We also observed that SI did not alter plasma corticosterone baseline levels after IS, whereas HSD induced a decrease in these levels. Excitatory postsynaptic currents (EPSC) indicated, as a preliminary result, that the exposure to a HSD reduced the rise time, indicating a more fleeting glutamatergic response. The findings of this study demonstrate for the first time that a short period of SI at a critical period of development is able to programme the mesolimbic dopaminergic system in order to increase susceptibility to both drug and food addiction. These results are possibly due, at least in part, to low basal levels of D2R in NAc and the higher stimulation of the dopaminergic system in striatum after a challenge with AMPH. The findings of this thesis suggest that stressful experiences such as social isolation during a critical period of development are able to permanently program the brain reward system and increase the susceptibility to additive behaviors in adult life. Identifying predisposing factors to this type of behavior is extremely important to prevent the development of drug addiction and/or eating disorders, to identify therapeutic targets and to enable the development of treatment strategies for these disorders.

Food addiction : a cost-effective treatment proposal within a developing country context

This study explores the possible efficacy of a low carbohydrate and high fat nutritional intervention (LCHF) as a treatment possibility aiming to improve the ability of self-control and regulation in the context of carbohydrate-addiction. The study first outlines why increased simple carbohydrate consumption has been implicated as a risk-factor in numerous chronic conditions, and then explores the possibility that a reduction of such consumption could lower general medical expenditure in the healthcare sector of already overburdened institutions, especially in developing countries like South Africa. Since the neurobiological evidence for food addiction is compelling, this study investigates the impact of a low carbohydrate and high fat eating (LCHF) regimen by measuring the change in the severity of addictive behaviour in relation to a reduced carbohydrate consumption. Results indicate that a LCHF nutritional intervention lessened addictive behaviour after just 30 days, resulting in a statistically significant decrease in addiction symptoms from day 1 to day 30. The weight and BMI values of the participants recorded at the end of the study showed a reduction from those obtained during the pre- treatment stage, and the self-perceived ‘feeling in control’ also improved in all participants after the intervention. The introduction of a LCHF nutritional intervention presents a relatively cost-effective treatment and preventative measure to combat carbohydrate over-consumption and its numerous health complications, and it is therefore hoped that the positive findings of this study will foster further research, using larger samples, into this type of nutritional intervention against addictive eating behaviour. / Psychology / M.A. (Psychology)
