KVINNORS UPPLEVELSER I SAMBAND MED GYNEKOLOGISK OPERATION : en litteraturstudie

<p>Kvinnor reflekterar oftast inte över sitt könsorgans betydelse förrän de konfronteras med sjukdom eller komplikation kring det. Kvinnor som genomgår gynekologisk operation kan lida av känslor som otillräcklighet och skam p.g.a. okunskap och för-domar. Arbetets syfte var att beskriva kvinnliga patienters upplevelser i samband med gynekologisk operation. En systematisk litteraturstudie gjordes, där 15 veten-skapliga artiklar analyserades med hjälp av innehållsanalys. I resultatet redovisas sju teman som berör operationsbeslutet, information, oro, smärta, sömn, tillfrisk-nandet samt vårdtiden. Resultatet visade att tiden i samband med gynekologisk ope-ration upplevdes på många olika sätt. Tiden innan operationen präglades av kvin-nors behov och önskan om muntlig och skriftlig information. Det framkom även att upplevelsen av oro påverkade patientens smärtnivå negativt. Efter operationen upp-levde vissa en lättnad medan andra hade sömnsvårigheter och ett problematiskt till-frisknande. Uppfattningen av vårdtiden påverkades av alla dessa faktorer. Slutsatser av studien visade att bristande information ledde till preoperativ oro, vilket i sin tur kunde leda till ökad postoperativ smärta. Dessa faktorer medförde ett försämrat väl-befinnande som kunde resultera i negativa upplevelser av vårdtiden och i förläng-ningen även en negativ inställning till vården som helhet.</p>

<p>Syftet med vårt arbete var att kritiskt granska hur pedagogerna på en förskola i Västra Sverige förhåller sig till den förändrade utemiljön på förskolan, något vi valt att benämna den ”moderna” utemiljön. Vi ville också undersöka vad de anser orsakade förändringen och dess utformning. Vi genomförde en kvalitativ intervjustudie samt läste in oss på litteratur och den tidigare forskning som finns kring utemiljön i förskola och skola. Vi kunde av analysen av vårt resultat se att samtliga pedagoger var positiva till förändringen, trots att de kunde finna ett fåtal nackdelar med den. Intervjuerna visar också att de har mycket kunskaper kring utemiljön, men möjligen inte erfarenheten för att våga använda den fullt ut. Vi kunde också finna att Naturskolan, som bidrog med pengar också hade en stor påverkan i utformningen av utemiljön.</p>

<p>Arbetet med detta examensarbete har varit att undersöka om FMT (Funktionsinriktad Musikterapi) kan vara ett komplement till den traditionella specialpedagogiska undervisningen i grundskolan. FMT kan kort beskrivas som ett arbete med motorisk utveckling. Motoriken är av betydelse för individens utveckling och förmåga till inlärning. Skolverket har kommit fram till att barn i behov av stöd inte alltid får rätt sorts stöd och försöker genom olika förändringar att komma tillrätta med detta.</p><p> </p><p>Examensarbetet innehåller resultat av praktikarbeten med två barn som tidigare av olika anledningar givit klasslärarna ”huvudbry”. Barnen som även under perioden fått traditionell specialpedagogisk undervisning, har utvecklats mycket positivt. Klasslärarna är eniga om att mitt arbete med FMT har varit en bidragande orsak till barnens utveckling.</p><p>Arbetet med detta examensarbete har också inriktats på att tydliggöra vad FMT är.</p>

Pharmacological Studies of Four Neuropeptide Y-family Receptor Subtypes

<p>The neuropeptide Y (NPY) family of structurally related peptides includes NPY, peptide YY (PYY) and pancreatic polypeptide (PP). They bind to G-protein coupled receptors named Y receptors, and include in mammals Y1, Y2, Y4, Y5, Y6 and in non-mammalian vertebrates also Y7, Yb and Yc. Subtypes Y1 and Y5 stimulate appetite, while Y2 and Y4 have the opposite effect in mammals. The studies described here concern human Y1 and Y4, chicken Y6 and Y7, and zebrafish Y2. </p><p>Site-directed mutagenesis of human Y1 identified sites important for binding of NPY and PYY as well as Y1 antagonists. The results clarify contradictory findings previously reported by others and identify new sites of interaction. A three-dimensional structural model of the Y1 receptor based upon the high-resolution structure of bovine rhodopsin was generated that increases our understanding of ligand-receptor interactions and hopefully will facilitate the design of novel subtype-selective agonists and antagonists. </p><p>Two naturally occurring variants of human Y4 with substitutions R240C and V276M have been found in a sample of obese children. Functional studies in vitro showed that the cellular response to PP was greatly decreased for R240C and may provide a causative link to juvenile obesity. </p><p>The genes for chicken Y6 and Y7 were found to be located ~1 megabase apart on chromosome 13 syntenic to the human Y6 pseudogene. Y6 mRNA has widespread expression whereas Y7 mRNA was found only in adrenal gland. Truncated fragments of PYY had lower affinity to chicken Y7 and zebrafish Y2 than to Y2 from mammals and chicken. The results suggest that Y2 in mammals acquired the ability to bind truncated PYY, i. e. PYY<sub>3-36</sub>, fairly recently, which has implications for its role in appetite inhibition.</p><p>These differences between receptor subtypes in sequences and pharmacological properties will be useful to further elucidate the structure and activation of Y receptors by site-directed mutagenesis. </p>

Advancing Cultural Studies in Sweden : An Infrastructural Initiative

Societal changes make culture increasingly central but also problematise it. New per-spectives are needed to meet these challenges. The international field of cultural studies is a promising effort to answer these challenges and vitalise cultural research. Sweden may make a significant and indeed unique contribution to this effort, but im-portant steps remain to be taken with this purpose. One such step would be to install a new national-international research institute on a higher level, in order to connect disciplines, universities and regions, and push innovative developments forward. Against such a background, this report leads up to an outline of a proposed new Advanced Cultural Studies Institute of Sweden (ACSIS). This is yet only a proposal, writ-ten at a time when ACSIS yet only exists as an imaginary utopia – though living with an extraordinary vitality in the minds of a wide intellectual network of committed scholars. Funding is presently being sought for, but it is not yet decided in what exact manner the ideas presented here will eventually be made real. The formulation of tasks, organisation and budget is thus yet a hypothetical model. Still, this bold adventure has reached a long way since its first inception. The ACSIS has long been an attractive dream for me and for many of my colleagues among cultural researchers. It is a very great pleasure to see the plans crystallised thus far, as the journey towards an ACSIS has reached its last and decisive phase. The report results from a committee work funded by the Bank of Sweden Tercen-tenary Foundation (Riksbankens jubileumsfond), and the Swedish Council for Re-search in the Humanities and Social Sciences (Humanistisk-samhällsvetenskapliga forskningsrådet). I had the great pleasure to work together with Svante Beckman, Ulf Hannerz, Lisbeth Larsson, Britta Lundgren, Orvar Löfgren, Ove Sernhede and Ulf Lindberg, and was reliably assisted by Åsa Bäckström. The group started working in January 2000, with a series of working meetings. Each member of the group has also had intense discussions of the basic ideas with other Swedish and international scholars, in meetings and by personal communication. Many therefore deserve warm thanks for making this report possible. The material and mental support by the two research funding bodies was essential, as was the generous and always stimulating collaboration in the committee.
