Community Risk Factors and Health Inspection Violations in Mississippi Delta Census Tracts

A concerning public health issue in America is about food deserts urban and rural communities that lack grocery retailers that offer affordable, nutritious, and diverse foods. Empirical evidence has shown significant associations between neighborhood disadvantage/disorder risk factors of high poverty and high percentages of ethnic minority residents with presence/ absence of healthy food retailers in food deserts. The purpose of this quantitative correlational study, framed by the disparities in food safety conceptual model, was to examine if county-level poverty, number of African American residents, number of elderly (i.e., age 65 or older) residents, vehicle ownership, and crime rates were significantly associated with presence/absence of healthy food retail environments in a stratified random sample of 160 Mississippi Delta Region counties. Variables were measured using SPSS 25.0 data set from federal sources. Data were analyzed using binary logistic regression. Findings indicated that the percentage of households below poverty level was significantly associated with absence of healthy food retailers, (Wald X2 = 7.62, p = .006). Logistic regression findings further showed that the county percentage of households with at least one vehicle was significantly associated with the presence of healthy food retailers, (Wald X2= 8.75, p = .003). As a result of this study, residents of the Mississippi Delta Region (MDR) may begin to petition their local, county, and state governments to enhance access to healthy foods, and in turn, such government institutions may develop programs and initiatives that help to make healthy foods affordable.

Objetivos: Estudar a influência que a renda das famílias e os preços dos alimentos exercem sobre a aquisição de alimentos mais saudáveis (frutas e hortaliças, F&H) e menos saudáveis (bebidas adoçadas, BA). Metodologia: Utilizaram-se dados sobre aquisição de alimentos coletados pela Pesquisa de Orçamentos Familiares (POF), realizada entre julho de 2002 e junho de 2003 pelo IBGE em uma amostra probabilística de 48.470 domicílios do país. A influência da renda familiar e do preço de frutas e hortaliças e de bebidas adoçadas sobre seu consumo foi estudada empregando-se técnicas de análise de regressão múltipa para estimação de coeficientes de elasticidade, controlando-se variáveis sócio-demográficas e preço dos demais alimentos. Resultados: Com a diminuição do preço de F&H haveria aumento da participação desses alimentos no total de aquisições: 10 por cento de redução nos preços de F&H aumentaria em 7,9 por cento sua participação no total calórico. Com o aumento da renda familiar também haveria aumento na participação calórica de F&H: 10 por cento de aumento na renda aumentaria em 2,7 por cento a participação de F&H no total calórico. O efeito dos aumentos de renda tendeu a ser menor nos estratos de maior renda. Haveria significativa redução na aquisição de bebidas adoçadas frente a aumentos no seu preço: para cada 10 por cento de aumento nos preços de bebidas adoçadas haveria uma redução de 8,4 por cento no consumo desses produtos. Aumentos na renda familiar também influenciariam o consumo de bebidas adoçadas, mas com efeito oposto e de magnitude inferior à metade do observado com o aumento de preços: para cada 10 por cento de aumento na renda familiar haveria um aumento de 4,1 por cento no consumo de bebidas adoçadas. Effect of Dietary Antioxidants on Oxidative Stress, Inflammation and Metabolic Factors : Studies in Subjects with Overweight and with Type 2 Diabetes

Observational studies have indicated that fruit and vegetables, and dietary antioxidants may play an important role in reducing the risk of chronic diseases, potentially by affecting pathogenic mechanisms such as oxidative stress and inflammation. Clinical trials investigating the effects of supplementation with single or a few antioxidants in high doses have, however, shown inconsistent results and thus have not been able to support the observational findings. It was therefore hypothesised that a supplement, containing a combination of antioxidants mainly extracted from fruit and vegetables, and supplied at moderate doses, might act more beneficially than single antioxidants given at pharmacological doses. The effects of such a supplement were investigated in two interventional studies described in this thesis. The effects on antioxidant status, metabolic control, oxidative stress and inflammation were investigated in overweight men and in patients with type 2 diabetes, subjects that could be expected to have elevated levels of oxidative stress and inflammatory activity. The results of the studies did not support the hypothesis that supplementation with antioxidants from fruit and vegetables may have beneficial effects by counteracting oxidative stress and inflammation, despite markedly increased plasma antioxidant concentrations. However, interesting associations were observed in diabetes patients at baseline between intake of antioxidant rich food as well as levels of antioxidants in plasma, and markers of oxidative stress and inflammation. These associations are compatible with the hypothesis that a high intake of fruit and vegetables and dietary antioxidants decrease oxidative stress levels, have anti-inflammatory effects and a beneficial influence on glycaemic control. The results also indicated that glycaemic control may affect the level of oxidative stress. The absence of beneficial effects from antioxidants might to some extent be explained by the initial levels of oxidative stress and inflammation and by the antioxidative status in the subjects included in the studies. Since the levels generally were comparable with those observed in healthy subjects, this might have decreased the ability to observe any beneficial effects of supplementation with additional antioxidants. Continued investigations are needed to characterise the individuals who potentially might benefit from antioxidant supplementation. In view of apparent positive effects from a high intake of fruit and vegetables found in observational studies and until more knowledge is available from interventional trials about possible benefits and potential risks of antioxidant supplementation it still seems reasonable to recommend a diet rich in fruit and vegetables.

The main purpose of this diploma thesis was to evaluate the functioning of the project "School milk and School fruit and vegetables" in the selected region, to find out its possible shortcomings and to propose further direction of this program. For the purposes of this diploma thesis the district of Tábor was selected. As part of the practical part of this diploma thesis, the primary research in this district first carried out through an online questionnaire survey. Subsequently, a survey of parents' opinion was conducted in the selected elementary school through all nine years of schooling. At the end there are mentioned problem areas of the project and suggested concrete recommendations. There is also verified the possibility of using organic food and regional foods in the project, and determined the attitude of schools and parents to the "Dainty" Decree.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / This thesis study aims to verify what conditions the adoption of governance structures in the formation and management of supplies chains in the networks of fruit and vegetables retail in the state of Rio Grande do Sul. For this, the main question for this research is: What are the motivations that take to the structures of governance adopted in the supply chains of retail organizations for FLV? The theorical framework used to help in the comprehension of the complex relationships established in the competitive modern environment is based, above, in the New Institucional Economy, in the Theory of Transaction Costs in the approach of Management in Chain of Supplies and in the Competitive Strategies of network organizations. As a level of analysis the chain of supplies of the retail companies is adopted, from the perspective of internal chain until the interorganizational arrangements. The methodological option performed for this investigation is the Case Study, considering that three regional networks of retail and six suppliers were analysed, covering the area of 5 mesoregions of the state. The choice of the cut on what refers to the section of fruits and vegetables is related to the specificities specially challenging on the distribution of the department of perishable foods and the growing search for health through the ingestion of more healthy , functional and with good nutritional aspects foods. This conjuncture has reflected on the raising of demand of these products and in the reorganization of all productive chain, reaching, specially, the initial link considered in this study: the farmers. As results it is verified that the retail networks when performing the management of their supplying chains use hibrid structures of governance with differente levels of coordination, considering the attributes of the transactions. But, also, conditioned by the characteristics imposed by the generic strategy adopted in each network as a way to answer their consumers. From the sight ocf impact on structuring the chains of supplies for the farmers it is seen that as the demand of the consumers gets higher, proportionally the need of coordinating the chain is increased, forcing the development of specific investments, increasing the mutual dependence. This takes the retail to adopt a more collaborative behavior in the chain. / Este trabalho procura verificar o que condiciona a adoção das estruturas de governança na formação e gestão das cadeias de suprimentos das redes de varejo de frutas, legumes e verduras no estado do Rio Grande do Sul. Para tanto, a principal questão que norteia a pesquisa é: Quais as motivações que levam às estruturas de governança adotadas nas cadeias de suprimentos de organizações varejistas para FLV? O arcabouço teórico utilizado para auxiliar na compreensão dos complexos relacionamentos estabelecidos no ambiente competitivo moderno está baseado, sobretudo, na Nova Economia Institucional, na Teoria dos Custos de Transação, na abordagem da Gestão da Cadeia de Suprimentos e nas Estratégias Competitivas das empresas organizadas em redes. Como nível de análise adota-se a cadeia de suprimentos das empresas varejistas, desde a perspectiva da cadeia interna até os arranjos interorganizacionais. A opção metodológica feita para esta investigação é o Estudo de Caso, sendo que foram analisadas três redes regionais de varejo e seis fornecedores, abrangendo cinco mesorregiões do Estado. A escolha do recorte no que diz respeito à seção de frutas, legumes e verduras está relacionada com as particularidades especialmente desafiadoras na distribuição do departamento de perecíveis e à crescente busca por saúde através da ingestão de alimentos mais sadios, funcionais e com bons aspectos nutricionais. Essa conjuntura tem refletido no aumento da demanda desses produtos e na reorganização de toda cadeia produtiva, atingindo, especialmente, o elo inicial considerado nesse estudo: os agricultores. Como resultados verifica-se que as redes de varejo ao realizarem a gestão das suas cadeias de suprimentos utilizam estruturas de governança híbridas com diferentes níveis de coordenação, considerando os atributos das transações. Mas, também, condicionadas pelas características impostas pela estratégia genérica adotada em cada rede como forma de atender seus consumidores. Do ponto de vista do impacto da estruturação das cadeias de suprimentos para os agricultores percebe-se que conforme aumenta a exigência dos consumidores proporcionalmente aumenta a necessidade de coordenação da cadeia, forçando o desenvolvimento de investimentos específicos, aumentando a dependência mútua. Isso leva o varejo a adotar uma postura mais colaborativa ao longo da cadeia.

Adherence to the Mediterranean Diet and Environmental Impact of the Diet on Primary School Children Living in Parma (Italy)

The key role of diet in both human health and environmental sustainability is well known. However, there is a lack of studies investigating the environmental impact of children’s dietary behavior. The aim of this observational study was to investigate the dietary environmental impact in a sample of primary school children living in Parma (Italy, n = 172, 8–10 years), in relation to their adherence to the Mediterranean diet (MD). Children completed a 3-day food record in both winter and spring. Dietary records were processed to obtain: (i) adherence to the MD and (ii) mean daily carbon and ecological footprints. Adherence to the MD was similar in winter and spring, with almost half of the participants showing a medium MD score. Carbon and ecological footprints were higher during winter, and the main dietary contributors were red and processed meat for both indexes. A small positive correlation was observed between adherence to the MD and total carbon and ecological footprints. This study provided the first analysis of the relationship between adherence to the MD and environmental impact of primary school children. Further research is needed to better investigate the environmental impact of primary school children’s diet and the possible relationship between the MD and environmental sustainability.

Bestimmungsgründe von Veränderungen in der Wertschöpfungskette für Obst und Gemüse

Die Wertschöpfungskette für frisches Obst und Gemüse in Deutschland hat sich in den letzten Jahrzehnten stark gewandelt. Die Konzentration im Einzelhandel, andere Konsumgewohnheiten und verbesserte Logistik haben Beschaffung und Angebot stark verändert. Daneben sind neue Stakeholder aufgetreten, die erfolgreich die Umsetzung ihrer Anliegen einforderten, was zu höheren Anforderungen an Produktsicherheit und Qualität führte. In der Wissenschaft beschäftigte man sich bislang nur wenig mit der Wertschöpfungskette für frisches Obst und Gemüse in Deutschland. Einige Autoren erforschten die Mechanismen, wie britische Supermärkte Gemüse aus Ostafrika importierten. Das Modell der Value Chain Analysis und die Transaktionskostentheorie bilden den theoretischen Rahmen der vorliegenden Arbeit. Erster Schritt war die Identifizierung der relevanten Akteure und ihrer Position in der Kette. Es zeigte sich, daß es keine einheitliche Value Chain gibt, sondern daß die Beschaffung der einzelnen Produkte unter-schiedlich organisiert ist, sowohl von Unternehmen zu Unternehmen, aber auch innerhalb einer Firma für unterschiedliche Produkte. In einem zweiten Schritt wurden sorgfältig ausgewählte Experten aus Produktion, Zwischen- und Einzelhandel zu den Veränderungen in der Value Chain und deren aktueller Ausgestaltung interviewt. Besonderer Wert wurde dabei auf die Ver-träge und Beziehungen zwischen den einzelnen Handelspartnern gelegt, d.h. wie diese zustande kommen, wie sie aufgebaut sind und wie die Kontrolle der Vereinbarungen funktioniert. In einem letzten Schritt wurden die Faktoren herausgearbeitet, die für das aktuelle Aussehen der Value Chain entscheidend sind. Weitere Ergebnisse waren, wie das Governance-System in der Realität funktioniert, wie konsequent Vorgaben und Standards umgesetzt werden und wie die Verbindungen zwischen den einzelnen Partnern gestaltet sind. Dabei kristallisierten sich drei unterschiedliche Einkaufstypen im LEH heraus. / The value chain for fresh fruit and vegetables in Germany has changed considerably through-out the last decades. The convergence in retail, transformed consumer habits and improved logistics have had great impact on procurement and supply. In addition, new stakeholders have emerged, who successfully demand the implementation of their requirements, resulting in a higher demand for product safety and product quality. So far, science has addressed the value chain model on fresh fruit and vegetables in Germany only briefly. In fact, there are only few studies available on the mechanisms of UK supermar-ket imports of vegetables from East Africa. The model of the Value Chain Analysis and the transaction cost theory should hence form the theoretical framework of this study. The first step was the identification of relevant players and their position within the chain. It was found that there is no single value chain present, but that the procurement of the individ-ual products is organized, changing, both from company to company, but also within a com-pany for different products. In a second step, a group of carefully selected experts from production, intermediary trade and retail were asked to comment on the changes and the current structure of the value chain. Special emphasis was placed on the contracts and relationships between the individual trading partners, i.e. how these agreements are made, what do they include and how their compliance is guaranteed. In a last step, the crucial factors were identified that are critical for the current appearance of the Value Chain. Other results included the determination of the true governance system, findings about how consistent the guidelines and standards are implemented and how the connections between the individual partners are designed. As a result, three different procurement types can be de-termined in food retailing.

Therefore, an alternative is collaboration. In the context of supply chain management, collaboration among members of the same chain refers to the idea of organizations engaged in working together. On the other hand, the collaborative approach should not be adopted between the organization and all the agents with whom interacts, must choose well, for different types of collaborative relationships, according to their actual needs and to save own resources. Thus, some authors classify the types of relationships in the supply chain according to level of collaboration among the agents, and are commonly found in the literature three stages of relationship, namely: cooperation, coordination and collaboration. To identify each of these stages has a distinct set of enhancer elements and actions, these characteristics that have been raised in this study based on systematic literature review. Additionally, it was identified examples of motivating factors for companies to engage in conduct collaborative. It is worth noting that the chain studied here is of the fruits and vegetables. Described this context, the main objective of this dissertation is to characterize the different stages of the collaborative relationship between supplier of fruits and vegetables and supermarket retail chains. The method used in this study corresponds a multi case study, which examined the relationships between a supplier of fruits and vegetables and five retail supermarkets, his customers. As a result, it was identified different stages of collaborative relationship between the supplier in question and each of the retailers surveyed, for the cases studied exhibited distinct sets of characteristics in terms of elements and actions. Regarding the motivation for these companies feel encouraged to develop and maintain collaborative relationships, a set of factors was found to be the same for all relationships investigated and examples of incentives are: to ensure product availability; reduce the impact of out-of-stocks (unavailability of product) in the retail market; reduce physical losses of product; increase sales and; customer loyalty when buying fruits and vegetables in store supermarket. Therefore, through this study, it was obtained drivers capable of guiding suppliers and supermarket stores in search of collaborative management for the chain of fruits and vegetables, highlighting the opportunities associated with improving the availability of the product at shelf, thereby ensuring a better customer service. / Nas relações de mercado, existem várias maneiras para que os agentes negociem entre si, desde negociações no mercado spot, puramente transacional, até relacionamentos caracterizados pela construção de parcerias ou alianças. Por conseguinte, uma das alternativas é a colaboração. No contexto de gestão da cadeia de suprimento, a colaboração entre membros de uma mesma cadeia remete à ideia de organizações engajadas em trabalhos conjuntos. Por outro lado, a postura colaborativa não deve ser adotada entre a organização e todos os agentes com os quais ela interage, devendo optar, assim, por diferentes tipos de relacionamentos colaborativos, de acordo com as suas necessidades reais e de modo a poupar recursos próprios. Dessa forma, alguns autores classificam os tipos de relações na cadeia de suprimento de acordo com nível de colaboração entre os agentes, sendo que são comumente encontrados na literatura três estágios de relacionamento, a saber: cooperação, coordenação e colaboração. Para identificar cada um desses estágios, tem-se um conjunto distinto de elementos facilitadores e ações, características estas que foram levantadas nesse trabalho com base na revisão sistemática da literatura. Adicionalmente, foram identificados exemplos de fatores motivadores para que as empresas se engajem na conduta colaborativa. Cabe destacar que cadeia aqui estudada é a de hortifrúti. Descrito esse contexto, o objetivo principal da presente dissertação é caracterizar os diferentes estágios do relacionamento colaborativo entre fornecedor de frutas, legumes e verduras e redes do varejo supermercadista. O método empregado nessa pesquisa corresponde ao estudo de casos múltiplos, em que foram analisados os relacionamentos entre um fornecedor de hortifrúti e cinco redes do varejo supermercadista, clientes dele. Como resultados, foram identificados diferentes estágios de relacionamento colaborativo entre o fornecedor em questão e cada uma das redes pesquisadas, pois os casos estudados apresentaram conjuntos distintos de características, em termos de elementos e ações. No que se refere à motivação para que essas empresas se sintam encorajadas a desenvolver e manter relações nos moldes de colaboração, o conjunto de fatores encontrado foi o mesmo para os relacionamentos pesquisados, sendo os seguintes exemplos de incentivos: garantir a disponibilidade do produto; reduzir o impacto da ruptura de gôndola (indisponibilidade de produto) no varejo supermercadista; reduzir as perdas físicas de produto; aumentar o volume de vendas e; fidelizar os clientes quanto à compra de hortifrútis na loja supermercadista. Por conseguinte, através dessa pesquisa, obtiveram-se direcionadores capazes de orientar fornecedores e lojas supermercadistas na busca da gestão colaborativa para a cadeia de hortifrúti, destacando as oportunidades associadas à melhoria da disponibilidade do produto no ponto de venda, garantindo, portanto, um melhor atendimento ao consumidor.

Les jardins communautaires et collectifs de Montréal : une exploration de leur place dans la réduction des iniquités de santé et de saine alimentation

Prendre part à un jardin communautaire ou collectif (JCC) est associé entre autres à une consommation accrue de fruits et légumes (FL) dans la littérature. Avec la popularité croissante des JCC dans les pays développés, l’impact des JCC sur les iniquités de santé demeure méconnu. Cette étude analyse 1) la répartition des JCC à Montréal en fonction de certaines caractéristiques sociodémographiques des voisinages et 2) l’association entre la proximité résidentielle d’un JCC et la consommation quotidienne de FL chez les adultes. Le nombre moyen de JCC dans les voisinages de Montréal a été comparé selon la densité populationnelle, la proportion de ménages locataires, la proportion d’immigrants et les proportions de personnes à faible revenu ou sans diplôme d’études secondaires. Une régression logistique multivariée a été effectuée pour évaluer l’association entre la proximité résidentielle d’un JCC (présente si jardin à ≤ 500 m de la résidence) et la consommation quotidienne de FL (suffisante étant ≥5 fois/jour). Les analyses ont été répétées avec une proximité à 300 m et à 1 000 m. Les analyses comparatives ont révélé un nombre significativement plus élevé de JCC dans les voisinages plus denses, avec plus de locataires et avec plus de personnes à faible revenu ou sans diplôme d'études secondaires. La proximité résidentielle à un JCC s’avère non associée à la fréquence de consommation de FL chez les adultes. La répartition actuelle des JCC à Montréal favorise un accès accru dans les quartiers plus défavorisés et présente un potentiel pour réduire les iniquités de santé. Dans notre étude, vivre près d’un jardin ne serait pas associé à une consommation plus importante de FL. Des études supplémentaires sont nécessaires pour évaluer d'autres dimensions de l'accès et de l’impact sur les iniquités de santé. / Taking part in community/collective gardening has been linked to greater fruit and vegetable (FV) consumption. However, little is known regarding their impact on social health inequalities. This study aims to examine 1) the distribution of community/collective gardens in Montreal and 2) the association between residential proximity to a community garden and daily FV consumption among adults. First, the mean number of gardens within a neighbourhood was compared as a function of neighbourhood-level socio-demographic indicators. Second, a multivariable logistic regression was performed to evaluate the relationship between FV consumption and the presence of a community/collective garden. FV consumption of individuals was classified as sufficient (≥ 5 times/day) or insufficient (< 5). Presence of a community/collective garden was dichotomized as being present (≤ 500 m from residence) or absent (> 500 m). Sensitivity analyses were conducted to assess the impact of modifying the cut-point using 300 m and 1000 m. Comparative analyses revealed significantly more gardens in neighbourhoods with higher population densities, higher tenancy rates, and higher proportions of people living under the low-income threshold or without a high-school diploma. There was no trend according to immigrant proportions. Our regression results showed no association between residential proximity and the frequency of FV consumption among adults. The current distribution of community/collective gardens in Montreal favours increased access in more disadvantaged neighbourhoods and shows potential to reduce health inequities. Living close to a garden doesn’t seem to be associated with the FV consumption pattern. Additional studies are needed to assess other important dimensions of access and the impact on health inequities.
