Förekomst av depressionssymtom och antidepressiv behandling bland hemodialyspatienter : en empirisk studie

Syftet med föreliggande empiriska studie var att undersöka och beskriva i vilken omfattning individer behandlade med hemodialys rapporterar symtom på depression. Författarna ville även redogöra för den utsträckning antidepressiva läkemedel ordinerats samt om det existerade några könsskillnader i depressionssymtom och i ordinerad antidepressiv behandling. Studien baserades på ett bekvämlighetsurval från 11 geografiskt spridda dialysenheter i Sverige. Urvalet bestod av 222 patienter som mötte inklusionskriterierna (behärska svenska språket tillräckligt bra för att förstå frågorna i enkäten, genomgått hemodialys behandling i minst 6 månader samt vara 18 år fyllda) varav 141 patienter fullföljde undersökningen. Dessa 141 deltagare fick fylla i den förkortade versionen av CES-D formuläret, ett självskattnings instrument där den egna mentala hälsan skattas. Resultatet visade att drygt en tredjedel av hemodialyspatienterna hade depressionssymtom och endast 12 patienter av 50 med depressionssymtom var ordinerade antidepressiv behandling. Emellertid visade inte föreliggande studie några signifikanta könsskillnader gällande depressionssymtom eller i ordinerad farmakologisk behandling. Författarna drar slutsatsen att depression är vanligt bland hemodialyspatienter. Därför är det viktigt att utbilda både vårdgivare och närstående för att de ska kunna se tecken på depressionssymtom i ett tidigt skede, eftersom rätt behandling kan öka dessa patienters livskvalitet och samtidigt vara kostnadsbesparande för sjukvården / The aim of the present empirical study was to investigate and describe in which extent individuals treated with hemodialysis reported symptoms of depression. The authors also wanted to describe in which extent antidepressant medicine was prescribed. Also if there existed any differences between males and females in depressive symptoms and if there was any divergence in treatment with anti depressive medicals. The study was based on a convenience sample from 11 geographical spread dialys units in Sweden. The sample consisted of 222 patients whom fulfilled the inclusion criterias (they had to have a good knowledge of the Swedish language so they could understand the questionnaire, at least six months of dialysis treatment experience and be at least 18 years old) of whom 141 carried out the study. These 141 participants filled out the short version of the CES-D form, a self report form where the mental health was estimated. The result showed that one third of the hemodialysis patients had symptoms of depression and only 12 patients of 50 had treatment with antidepressant medicine. However the study did not show any significant differences between male and female regarding depressive state and treatment with anti depressive medicine. The authors conclude that depressive symptoms are common among hemodialysis patients. That is why it is important to educate nurses and relatives so they can learn how to see signs of depressive symptoms as early as possible. Right treatment could then increase hemodialysis patients quality of life and also save money for the hospitals.

SYNTHESIS, CHARACTERIZATION AND BLOOD COMPATIBILITY OF CONDUCTIVE CELLULOSE COMPOSITE MEMBRANES

Cladophora cellulose polypyrrole composites are recognized as potential biomaterials with future applications in hemodialysis. In this project conductive Cladophora cellulose-polypyrrole (clad-ppy) composites were prepared using two different oxidizing agents: iron (III) chloride and phosphomolybdic acid (PMo). Cyclic voltammetry, conductivity and Specific surface area measurements were done to characterize the synthesized composites. Furthermore in vitro blood compatibility studies were performed. Whole blood was incubated with clad-ppy membranes and then blood was analyzed for platelet number reduction and complement activation products (C3a and sC5b-9). Clad-ppy with Iron (III) chloride membranes were found to be superior in terms of conductivity and surface area while Clad-ppy with PMo membranes were found to provoke less blood activation. The results indicated that each oxidizing agent gave distinct properties to the composite material.

Personer som drabbas av kronisk njursjukdom och startar behandling med hemodialys får en förändrad livssituation. Kärlaccess är en förutsättning för behandling och är personens framtida livlina. Sjuksköterskans uppgift är att ge stöd och stärka personens förmåga att uppleva hälsa trots sjukdom. Möjlighet att nå delaktighet och ökad egenvård är olika för varje enskild person och sjuksköterskan behöver bred kunskap för att möta detta behov. Syftet med studien, som utfördes som en litteraturöversikt, var att utifrån patientperspektivet belysa erfarenheter av hur kärlaccess kan påverka hälsan hos personer i hemodialys. Resultatet visade att tillit till sjuksköterskan och till sig själv, kunskap om sig själv och sin sjukdom, viljan att inte känna sig annorlunda och stöd från personer i sin omgivning påverkade hälsan. Tillit till sjuksköterskan behövs för att, trots rädsla vid placering av nålar, kunna vara delaktig i behandling. Stöd och kunskap har betydelse för personens väg mot egenvård. Mer forskning är önskvärd inom området för att belysa upplevelser hos personer med dialys och kärlaccessens betydelse för självständig behandling. / The life situation changes for individuals suffering from Chronic Kidney Disease who starts treatment with Hemodialysis. A vascular access is a prerequisite and is the individual’s future lifeline. The nurse’s task is to support and reinforce the individual’s ability to experience health despite disease. The possibility to achieve involvement and increased self-care varies for each individual and the nurse requires extensive knowledge to meet this need. The study was carried out as a literature overview. The purpose was, from a patient perspective, to highlight experience of how vascular access can influence the health of people with hemodialysis.The result showed that trust in the nurse and themselves, knowledge about themselves and their illness, willingness not to feel different and support from the people around them influenced the health. Trust in the nurse is needed to facilitate involvement in the treatment, despite fear when inserting the needles. Support and knowledge is of value in order for the individual towards self-care. More research on this subject is desirable to highlight the experiences of individuals in dialysis and the significance of the vascular access in order to achieve independent treatment.

Knowledge Construction of Hemodialysis Toward Health Broadcasting Program Audiences - A Case Study on Kaohsiung Police Radio Station's "Medical Network" Program

Due to high frequency and occurrence of chronic kidney diseases in Taiwan, as well as the low public awareness, this research aims to explore the knowledge construction process of Hemodialysis in health broadcasting programs from a health communication point of view. This research intended to answer the following questions: 1) What are health radio program audience types? 2) How does the knowledge on hemodialysis differ among audiences? 3) What is the knowledge construction process among audience in regards to hemodialysis? Data were collected by ten episodes of the Kaohsiung Police Ration Station¡¦s ¡§Medical Network¡¨ program for a six month period (January ~ June 2011). This research has utilized content analysis method on the audience type, quantitative description on questions identified by the audience and qualitative methods to summarize and interpret the audience¡¦s knowledge construction process on hemodialysis. The results showed that the main audiences for health broadcast programs are mostly male, age 31 to 50 years, holding profession as drivers, service personnel and potential patients. Among them, the potential patients and their family members most often times ask diagnostic questions, falling into the compelled group in seek of knowledge. Those who have not been diagnosed with the disease often times bring up knowledge confirmation questions, belonging to the proactive knowledge chaser group. These two groups also demonstrated different hemodialysis knowledge construction processes. While the radio program host plays the role of knowledge enhancer to the diagnostic-need group (potential patients), the role transfers to a knowledge transformation model for the knowledge confirmation group (non-patients). Participating physicians follow the treatment process of ¡V examination, diagnosis and treatment to deliver information. The research process shall provide broadcasters or other media professionals a best practice on how the audience absorbs information - to study the distribution and motives of the audience and to deliver the knowledge of health and illnesses.

Constructing Service Pathways Model of Hemodialysis Center

Introduction The number of the people who needs dialysis escalates rapidly year each. The rising cost of dialysis is relative. Because Bureau National Health Insurance endures the co-payment of hemodialysis, patients have freedom of choice. Two critical factors that influence patient¡¦s decision in medical care are access and quality. Facing pressure from competition, many hemodialysis service providers believe that high quality service will retain patients to return as well as maintain a healthy physician-patient relationship. Therefore, they apply many quality management tools, which also include service pathways. There are few investigations that discuss service pathways. Thus, this research constructs a model to analyze the effect of service pathways in hemodialysis service providers. Method From Nov. 1, 2003 to Dec. 31, 2004, the researcher interviewed hemodialysis nurses to develop flow chart, customer encounter, and check list for hemodialysis. We also collected the check list records of the hemodialysis patients to examine the effect. Conclusion First, the construction of total service pathways comprised three critical interlocking phases: flow chart, customer encounter, and check list. After that we shall evaluate and correct them continuously. Secondly, the researcher found that most patients are routine members, and come from long-term care center and respiratory care ward (RCW). Thirdly, the indicators influencing hemodialysis results and error incidents were steady. Finally, in the value-added services dimensions, the indicators of nurses¡¦ performance on greeting and asking patients if they want to have a meal were significant, but the indicators of telephone and inpatient interview were unfavorable.

Constructing Bayesian Networks with Sequential Patterns for Hemodialysis

In this thesis, I introduce a multivariate discretization algorithm to discretize the continuous variables of clinical pathways of Hemodialysis and use the clustering algorithm to shift time stamps to reduce the number of nodes of Bayesian networks. The generalized sequential patterns algorithm is used to find the possible patterns, which have far-reaching effect on the next nodes of the Bayesian networks of Hemodialysis. Bayesian network is a graphical model that encodes probabilistic relationships among variables of interest, and easily incorporates with new instances to maintain rules up to date. Bayesian networks are used to represent knowledge of frequent state transitions in medical logs. Bayesian networks and sequential patterns algorithms can only handle discrete or categorical data. Therefore, we have to discretize the continuous variables with suitable technique to generalize the node, and shift the time stamps of nodes to reduce the variations in time. With these generalizations, we improve the problem of over-fitting of the Bayesian networks of Hemodialysis. We expect the discovered patterns can give more information to medical professionals and help them to build the reciprocal cycle of knowledge management of Hemodialysis.

Närvaro av närstående var en faktor som positivt påverkade patienten och utan närståendes stöd krävdes större insatser från sjukvården. Till närstående räknas anhöriga, men även andra som har nära relationer med patienten, som till exempel vänner eller grannar. Syftet med denna litteraturstudie var att undersöka hur litteraturen beskriver situationen för närstående till hemodialyspatienter och vilken typ av stöd de efterfrågade, samt att beskriva inkluderade studiers urvalsgrupper. Det är en beskrivande litteraturstudie där 10 kvalitativa artiklar från databasen CINAHL inkluderades. Studiens resultat utmynnade i två huvudkategorier och åtta underkategorier. Situationen för de närstående präglades av oro, rädslor och osäkerhet kopplade till den sjukes hälsotillstånd och hemodialysbehandlingarna som orsakade stora begränsningar. Livssituationen påverkade många av de närståendes relationer, där den vårdande rollen och den sociala isoleringen hade framträdande platser. Resultatet visade att de närstående hade behov av att samtala om existentiella frågor, främst om döden, vilket gav en uppfattning om allvaret i de närståendes livssituation. Det var tydligt att stödet från sjukvården till de närstående upplevdes som mer betydelsefullt än stödet från övrig omgivning. De närstående hade svårigheter att uttrycka önskemål om stöd och efterfrågade initiativ till samtal från sjukvården. / Chronic kidney disease with life-sustaining hemodialysis treatment caused changes in living conditions for patients and next of kin. The presence of kin was a factor that positively affected the patient and without relatives support, greater efforts was required from health care. To next of kin families are counted, but also others who have close relationships with the patient, such as friends or neighbors. The purpose of this literature review was to examine how the literature describe the situation for relatives to hemodialysis patients and what type of support they requested, and to describe included studies’ sample groups. It´s a descriptive literature review where 10 qualitative articles from database CINAHL was included. The results of the study led to two main categories and eight subcategories. The situation for the next of kin was marked by anxiety, fears and uncertainties linked to the patient’s state of health and hemodialysis treatments that caused severe limitations. The life situation affected many of the next of kin´s relationships, where the caring role and social isolation had prominent places. The result showed that the relatives had a need to talk about existential questions, mainly about death, which gave an idea of the seriousness of the relatives’ life situation. It was clear that the support from health care to the next of kin was perceived as more important than support from others. The relatives had difficulties in expressing the wishes of support and requested initiate calls from health care.
