Proteinuria has been considered an important sign to identify the hypertensive disorders encountered during pregnancy and it is related to the maternal and perinatal outcomes. Objective: the aim of this study was to evaluate the urinary protein patterns and their molecular weight in hypertensive pregnant women, through urinary electrophoresis. Subjects and methods: Sodium dodecyl sulphatepolyacrylamide gel electrophoresis (SDS-PAGE) was performed on the urine of143 hypertensive pregnant women that were divided into the following groups: chronic hypertension (CH), pre-eclampsia/eclampsia (PE), pre-eclampsia superimposed upon chronic hypertension (CH+PE), and gestational hypertension (GH). No. of bitstreams: 1 Dissertacao Raphaela Maioni Xavier.pdf: 942765 bytes, checksum: 101261116404b23fe8473b4c0bb5ae74 (MD5) Previous issue date: 2011-03-25 / The etiopathogenesis of gestational hypertension has been widely studied, but it is not very well established in medical literature. Due to the low uteroplacental perfusion, light to moderate hypertension in the third trimester of pregnancy may be an adaptive response of the maternal organism in order to keep adequate blood flow in the fetal compartment. This paper aims to identify the socioeconomic factors of the parturients with gestational hypertension associated with the maternal and fetal conditions, as well as the gross placental pathological changes. The study was conducted in a federal public maternity hospital in Goiânia, in the state of Goiás, between March 2009 and February 2010. Placental and clinical data of 61 parturients with and without diagnosis (control group) of gestational hypertension were collected. 18 placentas of parturients with chronic hypertension (CH) (29.5%), 11 with gestational hypertension (HGE) (18%), 3 with eclampsia (4.9%), 28 of pre-eclampsia (PE) (45.9%), and 1 diagnosed with HELLP Syndrome (1.63%). The main placental changes found were: hematomas, presence of fibrin, fibrosis, infarction, calcifilaxis and areas of detachment. There was statistically significant difference concerning the occurrence of infarction on the maternal side of women with eclampsia (p = 0.047). The deposit of fibrin on the maternal side of the placentas of women with gestational hypertension was significantly larger than that of the women in the control group (p = 0.008). Calcifilaxis was larger on the maternal side of the placentas of women in the control group when compared to the group with gestational hypertension (p = 0.056). A statistically significant result was obtained when analyzing the presence of fibrin in the groups of PE and control (p = 0.024). There was a significantly larger deposit of fibrin on the fetal side of the placentas of women with HGE than of those in the control group (p = 0.048). Hypertensive disorders of pregnancy are studied as a worldwide problem of public health and rank third among the causes of maternal death in the world and first in Brazil. This is the reason why this study was carried out taking into consideration the need of an increasingly humanized obstetric care focused on the main placental changes and their impact on maternal and fetal vitality in the puerperium.

Estudo dos polimorfismos do gene da adiponectina em mulheres com hipertensão gestacional e pré-eclâmpsia / Study of polymorphisms of the adiponectin gene in women with gestational hypertension and preeclampsia

Machado, Jackeline de Souza Rangel 27 February 2012 (has links)
Introduction: adiponectin is involved in energy homeostasis by regulating glucose and lipid metabolism. Additionally, it presents anti-inflammatory and anti-atherosclerotic functions. Polymorphisms in adiponectin gene (ADIPOQ) can modulate the concentrations of adiponectin. The influence of these polymorphisms on the development of gestational hypertension (GH) and preeclampsia (PE) is unknown. Aim: The aim of this work was to examine the influence of polymorphisms in the gene ADIPOQ on the development of gestational hypertension and preeclampsia. Patients and Methods: we studied 401 pregnant women: 161 healthy pregnant (HP), 113 pregnants with gestational hypertension (GH) and 127 pregnants with preeclampsia (PE). Polymorphisms ADIPOQ -11391G>A (rs17300539), - 11377C>G (rs266729), 45T>G (rs2241766) and 276G>T (rs1501299) were genotyped by allelic discrimination by PCR in real time. Haplotypes were inferred using the PHASE 2.1 program. Results: there were no statistically significant differences in allele and genotype frequencies of the polymorphisms studied. In the analysis of haplotypes, we observed small differences in haplotype frequencies between groups, however, none of these differences was statistically significant (P> 0.05). Conclusion: we found no association between the genotypic and allelic variants of the ADIPOQ gene polymorphisms with the development of gestational hypertension and preeclampsia.

Possomato-Vieira, José Sérgio January 2018 (has links)
(Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Throughout gestation, several physiological adaptations occur to support the growing fetus. In some women, fails in these mechanisms may cause gestational hypertensive disorders. Preeclampsia (PE) is a gestational hypertensive disorder characterized by a de novo increase in arterial blood pressure (> 140x90 mmHg) after the 20th gestational week. It is normally accompanied by proteinuria and affects 5-7% of pregnant women. It is currently accepted that the physiopathology of PE involves two different stages: the first one is related to a bad placentation process, consequent placental ischemia/hypoxia and release of bioactive factors; and the second stage is related to the occurrence of endothelial cell damage. Endothelial dysfunction is characterized by an increased release of vasoconstrictors substances, such as endothelin, and a concurrent reduction in the release of vasodilators substances, such as nitric oxide (NO). Recent studies point to the importance of hydrogen sulfide (H2S), a molecule that shows several similarities with NO, such as its gaseous nature and the enzymatic production in endothelium. In cardiovascular system, H2S exerts numerous functions, such as angiogenesis modulation and vasodilators effects, however only a few studies evaluated H2S role in gestational hypertension. For the accomplishment of the works, pregnant Wistar rats were used. Initially, gestational hypertension was induced with the use of NO synthase (NOS) inhibitor, Nω-nitro-L-arginina-metil...

Chimini, Jessica Sabbatine January 2018 (has links)
Among these adaptations, we found increase in cardiac output and blood volume, which are compensated by the drastic reduction of systemic vascular resistance, promoting the maintenance of maternal blood pressure in gestations. Vascular endothelium plays an important role in this physiological process by secreting vasoactive factors such as nitric oxide, a vasodilator that is increased in healthy pregnancies. In some women, fails in these mechanisms may cause gestational hypertensive disorders. Preeclampsia is a gestational hypertensive disorder characterized by elevated blood pressure (≥ 140 x 90 mmHg) after the 20th gestational week. It is often accompanied by proteinuria and affects 5-8% of pregnant women. Currently, although the pathophysiology of preeclampsia is unknown, studies suggest that its occurrence involves poor migration of cytotrophoblasts culminating in poor placentation and consequent ischemia/placental hypoxia, responsible for the release of bioactive factors promoting damage to endothelial cells. Endothelial dysfunction results in the reduction of the bioavailability of nitric oxide leading to the occurrence of the main characteristics of preeclampsia, mainly maternal hypertension and fetal growth restriction. Recent studies point to the importance of maintaining levels of nitric oxide, which is associated with health... (Complete abstract click electronic access below) / Mestre

Nascimento, Regina Aparecida do. January 2018 (has links)
(Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Nitric oxide (NO) is a gas produced mainly by endothelial cells, and during the gestational period contributes to avoid an exacerbated increase in systemic vascular resistance (SVR), since during this period, there is a rise in blood volume and heart rate. When there are endothelial dysfunctions, the bioavailability of NO is reduced, causing vasoconstriction and this, in turn, consequently increases the activity of MMPs. This decrease in NO and increase in MMPs culminates in gestational hypertensive disorders. According to previous studies in non-gestational hypertension models, doxycycline, an antibiotic derived from tetracyclines, lowered systolic blood pressure due to its ability to inhibit MMP activity. However, there is a shortage of works that have developed studies on the effects of doxycycline in animal models of gestational hypertension. For this reason, we investigated the effects of doxycycline on the inhibition of MMPs in gestational hypertension, induced by Nω-nitro-L-arginine-methyl ester (L-NAME) in rats. We performed a zymography to evaluate the activity of MMP-2 and -9 in the placenta, uterus and thoracic aorta. We also evaluated systolic blood pressure, fetalplacental development and NO metabolites. Plasma antioxidant capacity, plasma levels of soluble fms-like tyrosine kinase-1 (sFlt-1) and placental growth factor (PLGF) were also evaluated. Our data shows that the treatment with L-NAME increased blood pressure and decreased the offspring of these animals. ... (Complete abstract click electronic access below) / Doutor

January 2018 (has links)
These disorders are the major causes of both maternal and fetal morbidity and mortality. Studies have demonstrated a reduction in the bioavailability of nitric oxide (NO) in gestational hypertensive diseases, making it one of the conditions responsible for aggravate the endothelial dysfunction during the course of this disease. In this sense, studies demonstrate that oral administration of sodium nitrite (NaNO2) and administration of sildenafil citrate may reverse the reduction of NO bioavailability and potentiate the NO-cGMP signaling pathway, respectively. Wistar rats were used to perform the study, in which, after confirmation of pregnancy, they received L-NAME i.p for induction of hypertension and treatments with sodium nitrite and/or sildenafil citrate orally between days 14-21 of pregnancy. Systolic blood pressure was measured by the tail plethysmography method. Maternal-fetal parameters such as: fetal and placental weight, number of viable and reabsorbed fetuses were performed after the death of the rats on the 21st gestational day. Plasma of the rats were stored for plasma levels of nitrite + nitrate, cGMP, myeloperoxidase (MPO), lipid peroxidation (TBARS), TEAC, MTT sFlt-1 and VEGF. In addition the plasma of the rats were incubated with human umbilical vein endothelial cells (HUVECS) to evaluate endothelial NO production. In both studies, we found reduction of systolic blood... / Gestational hypertensive disorders are complications that affect around 5-10% of pregnancies. In both studies, we found reduction of systolic blood pressure with nitrite and sildenafil, both alone and in combination. Both nitrite and sildenafil were able to reduce reabsorption, possibly by increasing fetal viability. No increase in plasma NO was found in all groups receiving nitrite, while curiously NO synthesis in HUVECS incubated with plasma from rats was increased in both drugs. Still in relation to cell culture, we found increased cell viability in hypertensive rats treated with sildenafil. Plasma cGMP concentration was increased in normotensive rats receiving sildenafil. Both drugs have an antioxidant effect, but only sildenafil was able to reduce plasma myeloperoxidase activity (MPO). Angiogenic (VEGF) and anti-angiogenic (sFlt-1) factors present in gestational hypertensive diseases were increased in rats receiving L-NAME, but sodium nitrite was able to reduce these factors. Our data suggest that both nitrite and sildenafil, both alone and in combination, have antihypertensive and antioxidant effects. Therefore, our results suggest that activation of the NO-cGMP pathway increased blood flow at the maternal-fetal interface and protected against hypertension and fetal growth restriction induced by L-NAME. / FAPESP 16/18782-3 / FAPESP 12/21305-1
