Examining home-based care of chronically/terminally ill persons by family care givers and their interaction with professional health care providers

According to the Ministry of Health (1996:26), the most common chronic/terminal illnesses were cardiovascular disorders, diabetes, cancer, mental disorders, HIV/AIDS, tuberculosis and asthma. Long term treatment and care and the growing incidence of these conditions necessitated the introduction of home-based care (HBC). Consequently, family care givers play a major role in the provision of care to chronically/terminally ill patients and professional health care providers adopt a supervisory role. This study examined the quality of home care services provided in Botswana. The availability and accessibility of home-based care services and resources have a direct bearing on the quality of home-based care delivery system. The researcher used systems theory was used as the conceptual framework for this study. The study aimed to • determine the accessibility and availability of home-based care services in Molepolole East • investigate what the perspectives and experiences of family care givers, patients and professional health care providers of Botswana home-based care are • determine the roles of professionals health care providers, patients, and family care givers and their relationships in the context of home- based care • identify the needs of chronically/terminally ill patents and family care givers • determine the type of support given to family care givers and patients by professional health care providers and make recommendations for the improvement of home-based care • develop a model to prepare family care givers The research design combined quantitative and qualitative research methods. A sample of convenience was used to obtain information from patients' family care givers and professional health care providers. Interviews and questionnaires were used. A proposed care giving preparedness model is presented to meet needs of the family care givers. The study found that family care givers needs are neither known nor catered for by the professional health care provider. The family care givers were not adequately prepared before adopting the care-giving role. Pain management in people living with HIV in home based care

The purpose of this study was to determine how pain in PLHIV on community home based care programme was managed using the Roy adaptation model (RAM) as the theoretical framework. This study used a descriptive mixed method research design which allowed for the research topic to be investigated and described in terms of both narrative qualitative accounts and descriptive statistics. A sequential exploratory approach was adopted in which qualitative data collection was conducted in the initial phase followed by quantitative data collection. Ten qualitative interviews, guided by the components of the RAM were conducted in the initial phase and a structured interview schedule (questionnaire) partially based on the result of the qualitative phase and (which was also structured in accordance with the RAM) was administered to 120 respondents who met the inclusion criteria of being on CHBC. Convenience sampling was used to select study participants and respondents in both phases of the study. Peripheral neuropathy was the most common pain syndrome experienced by patients. More females suffered numbness/tingling sensation of toes, followed by chest pains than their male counterparts. Pain experienced was moderate to severe which was not treated effectively. The study findings could assist health care providers, policy makers and researchers to invest into improving pain management for PLHIV, strengthen education of patients and their primary caregivers on the causes of pain and modalities to employ for pain relief and implement palliative care guidelines for PLHIV. / Health Studies / D. The aim of this research was to support new mothers in groups, so that they can function optimally as educators. During the research mothers with babies up to three months of age were used by means of a support group. This research project was done after it became evident from a literature study that new mothers suffered with unique problems during this adaption phase. In the empirical survey the researcher gave attention to the new mothers' specific needs and problems during this trimester. The researcher aimed to explore the experiences of the new mother as educator through weekly group sessions. From the results it was clear that there was a need of support from other mothers that is going through the same experiences. In this way new mothers could receive emotional support and obtain knowledge regarding relevant issues experienced during this unique life phase. / Educational Studies / M. Ed. A comparative analysis of quality of life and stigma experienced by people living with HIV receiving antiretroviral therapy in a home-based care project in Malema an Ribaue districts, Mozambique

Stigma is broadly considered as an important obstacle in HIV prevention and treatment. In Mozambique, more PLWHIV have access to treatment than ever. ART increased their QoL and created a new context for stigma production. This cross-sectional study conducted in Northern Mozambique explored a possible association between stigma and QoL. The Berger Stigma Scale and WHOQOL-BREF were used to measure levels of HIV-related stigma and perceived QoL in two groups of PLWHIV receiving ARV, with participation in a HBC programme as the independent variable. The study uncovered a weak to moderate negative correlation between stigma and QoL scores. Statistically significant differences were discovered between the groups’ scores on the Berger Stigma Scale and a higher perceived QoL for PLWHIV in the HBC group on three WHOQOL-BREF subscales and on the overall score. The feasibility of intensive family preservation services in South Africa

Children are being removed unnecessarily from their families and placed into substitute care because service programmes lack resources to keep families intact. There are overseas programmes, which provide home services to families, with children at risk of out of home placement. Although there has been an IFPS project there is no established intensive short-term programme in South Africa that deals with families at the brink of dissolution.The objective of the research is to investigate whether an Intensive Family Preservation Programme is feasible in South Africa. The researcher received training in the United States and met with leaders in the field, to discuss the development and implementation of the programme in South Africa. Surveys obtained the attitudes and opinions of the local social workers and commissioners of Child Welfare in Durban towards the programme. The results from opinion surveys indicate that an Intensive Family Preservation Programme is feasible in South Africa. / Social Science / M.A.(Social Work)

Para ampliar esse conhecimento, realizou-se uma revisão integrativa da literatura nacional, por meio das bases de dados LILACS e SCIELO, relativa ao período de 2000 a 2009, sobre o trabalho domiciliar, as condições em que ele acontece e suas repercussões na saúde dos trabalhadores, com o propósito de contribuir para o desenvolvimento de ações de vigilância em saúde para esta categoria de trabalhadores. A amostra utilizada possui 11 trabalhos, de diversas regiões do país. O estudo evidenciou que tanto em metrópoles como em cidades de médio porte, ou mesmo em pequenas cidades da Zona Rural, os trabalhadores em situação de trabalho precário, incluídos aí os trabalhadores domiciliares, tiveram sua saúde afetada pelas condições em que executavam suas atividades. Observou-se que o trabalho domiciliar é fruto da exclusão social e nele encontramos diversos fatores de riscos para a saúde dos trabalhadores. De modo agravante esses fatores de riscos à saúde se estendem além dos próprios trabalhadores, atingindo seus familiares, particularmente idosos e crianças, considerados mais vulneráveis aos efeitos deletérios para a saúde, decorrentes da exposição a que estão sujeitos. Constatou-se a subtração de direitos fundamentais à dignidade da pessoa humana, a negação da equidade, da justiça e da cidadania. É importante ressaltar também que o trabalho domiciliar não deve ser visto como um fenômeno isolado; o mercado de trabalho absorveu essa modalidade de trabalho, promovendo a naturalização da sua precariedade, conformando-o como importante alternativa ao trabalho convencional. Os resultados sugerem que a relação entre trabalho e doença é um processo complexo e dinâmico e que o Sistema Único de Saúde, através das ações de vigilância em saúde dos trabalhadores, pode contribuir para melhorar ou minimizar estasituação. Embora o mundo do trabalho seja muito heterogêneo, há que se conhecer, discutir, refletir e buscar soluções viáveis nos diversos órgãos do governo, no sentido de resgatar o direito ao trabalho decente, à saúde, à proteção social, enfim, à dignidade dos trabalhadores que estão à margem da sociedade e invisíveis aos olhos do Estado. / In the context of globalization, the home-based work is incorporated into the Brazilian productive restructuring process and needs to be unveiled. Aiming to increase knowledge on the issue of home-based work, was held an integrative literature review of studies published in SciELO and LILACS databases, in Portuguese, in the years 2000 to 2009. The goal was to organize, to systematize and to analyze the scientific production on the home-based work, the conditions in which it happens and the impact on the workers’ health, with the objective of contributing for the development of a politics for the surveillance of home-based workers’ health. The sample consisted of eleven articles, from several areas of the country. The findings evidenced that, in a similar way despite of the size of the city, the workers in precarious labor conditions, including the home-based workers, had their health affected by the conditions in which they made their job. It was also observed that the home-based work, where we can find different risk factors for workers’ health, is a consequence of social exclusion. In an aggravating way, these risk factors extend to the workers’ families, especially elders and children, more vulnerable to the harmful effects. It was possible to verify the subtraction of fundamental human rights, the negation of equity, justice and citizenship. It’s also important to emphasize that home-based work should not be regarded as an isolated phenomenon; the labor market absorbed this form of work, promoting the naturalization of its precariousness, conforming it as an important alternative to conventional labor. The results suggest that the relation between work and health is a dynamic and complex process and the Unified Health System (SUS) may contribute, through surveillance actions for the home-based workers, to improve the scenery. Although the work’s world is very heterogeneous, we must know, discuss, reflect and search for feasible solutions in the various government agencies, in order to redeem the rights to a decent work, to health, to social protection, finally to dignity, to the workers that are on the fringes of society and invisible to the State’s eyes.

The purpose of this dissertation is to offer a greater understanding of the potential of genograms through my clinical work from a Bowen Family Systems lens. I account for how I processed and effectively blended metaphorical components, by examining six cases from my two-year journal entries, of bringing genograms to life in sessions. I also explain how I created a useful tool, the Metaphoric Generative Genogram, that can benefit other clinicians working with children and families in the foster care community.
