The role of community-based organisations in response to the HIV/AIDS in Botswana : the case of Gabane Community Home Based Care OrganisationChibamba, Fortune Michelo 06 1900 (has links)
This study examines the role of Community Based-Organisations (CBOs) in the response to HIV/AIDS as a development challenge drawing examples from the Gabane Community Home-Based Care CBO in Botswana. The study adopted qualitative methods of research and used group discussions, relative unstructured interviews, direct observation and literature review as methods of data collection. The study found out that HIV/AIDS is indeed a development problem and that it can be dealt with using some existing development approaches such as the sustainable livelihoods approaches. The study further identified specific roles that CBOs play in the response to HIV/AIDS. It also revealed the potential that CBOs have in achieving development. In addition, the study identified and outlined challenges that CBOs face in responding to HIV/AIDS. Key recommendations are that CBOs must integrate poverty reduction interventions in their activities. They must also form coalitions and strengthen their capacity to sustain their activities and manage partnerships. / Development Studies

Le concept du Management de la Mobilité agit dans la perspective d’optimiser l’usage de l’offre alternative de mobilité existante et de la développer en créant les conditions favorables d’un changement de culture et de pratiques en matière de déplacements. Notre contribution à travers cette recherche a pour ambition d’éclaircir les bases de débat sur la pertinence des approches du Management de la Mobilité en les examinant de près. Il s’agit de cerner l’efficacité de ces approches, leur fonctionnement, ainsi que leurs apports et leurs limites.Partant de l'hypothèse que les pistes qui mènent à l'objectif de concrétisation d'une mobilité plus durable afin d'atteindre le « facteur 4 » sont multiples, notre démarche repose sur l'analyse de plusieurs terrains de recherche où on tentera de vérifier la pertinence et l'efficacité de l'approche du Management de la Mobilité à travers deux voies possibles de régulation des mobilités quotidiennes.Une première voie que nous l'appelons la « voie corrective » où Il s'agit d'influencer les choix individuels et même plus, des modes de vie entiers par la mise en route des processus de changement à travers des mesures incitatives. Une deuxième voie indissociable de la première, car elle accroit son efficacité et sa pérennité, est plutôt une « voie intégrative ». Pour explorer cette voie, notre recherche nécessite un travail de nature exploratoire pour déterminer les pré-conditions de réalisation du transfert modal afin d’augmenter le potentiel d'usage et la crédibilité des modes alternatifs. Ceci serait possible par l’anticipation des besoins de la mobilité à travers l’intégration du Management de la Mobilité dans le processus de planification urbaine. / The concept of Mobility Management aims to optimize the use of available and alternative mobility modes by providing and creating optimal conditions for travel patterns change. Our contribution through this research is to provide basis for discussion on the relevance of mobility management approaches by analysing them in depth. The research consists in clarifying the effectiveness of these approaches, the way they are implemented, their benefits and limits.Assuming that the ways to lead to more sustainable mobility and achieve the national target of reduction by a factor of four greenhouse gas emissions by 2050 are various, our strategy is based on the investigation of several research fields where we will attempt to validate the adequacy and the effectiveness of mobility management approaches through two assumed ways of mobility regulation. A first way, that we’ve called the « corrective way », consists in influencing individual travel choices by enacting behaviour change processes and implementing incentives measures. A second way which is inseparable from the first, because it increases its long term efficiency, is rather an « integrative way ». It requires for our research an exploratory method to set out pre-conditions for achieving modal shift and increasing potential use and credibility of alternative modes. This could be possible by anticipating travel demand through an integration of mobility management measures into urban planning process.

important and frequent functional impairment. In the evolvement of operative techniques of the muscoskeletal system, the cemented total hip replacement is pointed out since it improves the patients quality of life. Physiotherapy is an important resource in the patients care after Total Hip Replacement (THR) to aid their functional return. Objective: to analyze the physiotherapy intervention in the improvement of pain, gait, mobility and quality of life in the postoperative period of patients who have undergone THR, moreover, comparing the physiotherapic ambulatory rehabilitation with orientations and self-care guide.Casuistics and Method: This is a randomized clinical trial and double blinded study, in which patients with osteoarthritis submitted to THR in the period of May to October 2005 were evaluated. They were objectively evaluated either for pain, gait or mobility, according to D´Aubigné and Postel´s criteria in the preoperative, with 1 month and 3 months of postoperative, respectively. Life quality was evaluated by means of the questionnaire SF-36. The patients were randomly divide into two groups: the study group and the control group. The patients of the study group were submitted to a protocol of physiotherapic treatment during twelve weeks, with two-week sessions. The control group did not receive the guide of instructions related to postoperative cares. Mood´s median test was used to evaluate the results for the non-parametric values to compare the results between the groups. Wilcoxon´s test was used to compare the evolution of the groups. A level of significance of p=0.05, with a confidence interval (CI) of 95% was used. Results: Twenty patients, 10 from the study
group and 10, the control group, finished the study. In the first evaluation, there was a significance just regarding the general state of health. This showed a smaller median in the study group (p=0.025), In the second, at the 30th PO
period, there was statistical significance in relation to the study group regarding both the gait (p=0.007) and the mobility (p=0.001). In the third, there was a statistical significance after three months of postoperative, and at the end of the physiotherapeutic intervention in the study group: in the functional capacity (p<0.0005), in the physical aspects (p=0,002), in the pain (p=0,019), in the vitality (p=0,025), in the emotional aspects (p<0.0005), in the gait (p=0.001) and in the mobility (p=0.010). Regarding the evolution, after three months of postoperative period, a greater median was observed in the study group according to the physical capacity (p=0.006), physical aspects (p=0,002), general state of health (p<0,0005), vitality (p=0,025) and emotional aspects (p=0,001). Conclusion: Early physiotherapy procedures can improve the rehabilitation process of patient´s submitted to THR. / A perda de mobilidade do quadril, espontânea ou cirúrgica, é um comprometimento funcional importante e freqüente. Na evolução das técnicas operatórias do aparelho locomotor, destacam-se as relacionadas com a substituição da articulação do quadril por uma prótese total de quadril cimentada que melhora a qualidade de vida dos pacientes. A fisioterapia é um importante recurso nos cuidados de pacientes após a Artroplastia Total de Quadril (ATQ) para auxiliar o retorno funcional. Objetivos: analisar a intervenção da fisioterapia na melhora da dor, marcha, mobilidade e qualidade de vida no pós-operatório de pacientes que realizaram ATQ cimentada, além de comparar a reabilitação fisioterapêutica ambulatorial com um guia de orientações e auto-cuidados. Casuística e Método: Trata-se de um ensaio clínico aleatorizado, mono-cego, no qual foram avaliados os pacientes portadores de osteoartrose submetidos a ATQ no período de maio a outubro de 2005. Os pacientes foram avaliados objetivamente quanto à dor, marcha e mobilidade, de acordo com os critérios estabelecidos por DAubigné e Postel no pré-operatório, com 1 mês e 3 meses de pós-operatório, respectivamente.A qualidade de vida foi avaliada por meio do questionário SF-36. Os pacientes foram divididos, de forma aleatória em dois grupos - o grupo de estudo e o grupo controle. Os pacientes do grupo de estudo foram submetidos a um protocolo de tratamento fisioterapêutico durante doze semanas, com duas sessões semanais. O grupo controle não recebeu intervenção fisioterapêutica padronizada precocemente, tendo recebido apenas o guia de instruções quanto aos cuidados pós-operatórios. Os resultados foram avaliados pelo teste de Mediana de Mood para valores não paramétricos a fim de comparar os resultados entre os grupos.
Nota de Resumo A evolução dos grupos foi comparada utilizando-se o teste estatístico de Wilcoxon. Adotou-se um nível de significância de p= 0,05, com intervalo de confiança (IC) de 95%. Resultados: 20 pacientes terminaram o estudo, 10 pertenciam ao grupo de estudo e 10 ao grupo controle. Na avaliação inicial, houve significância apenas com relação ao estado geral de saúde, que demonstrou uma menor mediana no grupo de estudo (p=0.025).Na segunda avaliação, realizada no 30 PO, houve significância a favor do grupo de estudo com relação à marcha (p=0,007) e com relação à mobilidade (p=0,001). Na terceira avaliação, realizada após três meses de pós-operatório e ao término da intervenção fisioterápica no grupo de estudo, houve significância na capacidade funcional (p<0,0005), nos aspectos físicos (p=0,002), na dor (p=0,019), na vitalidade (p=0,025), nos aspectos emocionais (p<0,0005), na marcha (p=0,001) e na mobilidade (p=0,010). Quanto à evolução, após três meses de pós operatório, percebeu-se uma maior mediana no grupo de estudo quanto à capacidade física (p=0,006), aspectos físicos (p=0,002), estado geral de saúde (p<0,0005), vitalidade (p=0,025) e aspectos emocionais (p=0,001). Conclusão: A fisioterapia precoce auxilia no processo de recuperação dos pacientes submetidos a ATQ.

Tradition, Change and Variation : Past and Present Trends in Public Old-age CareTrydegård, Gun-Britt January 2000 (has links)
<p>The general aim of this dissertation is to describe and analyse how public old-age care in Sweden has developed and changed during the last century. The study applies a provider perspective on how care has been planned and professionally carried out. A broader social policy perspective, studying old-age care at central/national as well as local/municipal level, is also developed. A special focus is directed at the large local variation in care and services for the elderly. The empirical base is comprised of official documents and other public sources, survey data from interviews with elderly recipients of public old-age care, and official statistics on publicly financed and controlled old-age care and services.</p><p>Study I addresses the development of old-age care in Sweden during the twentieth century by studying an important occupation in this field – the supervisors and their professional roles, tasks and working conditions. Throughout, the roles of supervisors have followed the prevailing official policy on the proper way to provide care for elderly people in Sweden; from poor relief at the beginning of the 1900s, via a generous level of services in the 1960s and 1970s, to today’s restricted and economy-controlled mode of operation.</p><p>Study II describes and compares two main forms of public old-age care in Sweden today, home help services and institutional care. The care-load found in home-based care was comparable to and sometimes even larger than in service-homes and other institutions, indicating that large care needs among elderly people in Sweden today can be met in their homes as well as in institutional settings.</p><p>Studies III and IV analyse the local variation in public old-age care in Sweden. During the last decades there has been an overall decline in home help services. The coverage of home help for elderly people shows large differences between municipalities throughout this period, and the relative variation has increased. The local disparity seems to depend more on historical factors, e.g., previous coverage rates, than on the present municipal situation in levels of need or local economy and politics.</p><p>In an introductory part the four papers are linked together by an outline of the demographic situation and the social policy model for old-age care in Sweden. Trends that have been apparent over time, e.g. professionalisation and market orientation, are traced and discussed. Conflicts between prevailing ideologies are analysed, in regards to for instance home-based and institution-based care, social and medical culture, and local and central levels of decision-making. ’Welfare municipality’, ‘path dependency’, and ‘decentralisation’ are suggested as a conceptual framework for describing the large and increasing local variations in old-age care. Finally, implications of the four studies with regard to old-age care policy and further research are discussed.</p>

Urban connections with rural areas in home-based business : implications for sustainable rural development in SaskatchewanOfosuhene, Maxwell 19 August 2005
The past two decades have witnessed significant growth in home-based work (HBW), particularly home-based business (HBB) activity and self-employment. These phenomena are attributed to factors such as flexible organization of production. While some empirical accounts on HBW and HBB activity in Canada do exist, they have mostly been conducted at the national or urban level rather than in rural areas. This thesis, therefore, places greater emphasis on rural HBBs in Saskatchewan where out-migration of people is threatening the viability and sustainability of rural and small communities. It is argued that rural sustainability largely depends on economic viability. The study area for the research includes the City of Saskatoon and the countryside surrounding this city. <p>The overall goal of this research is to contribute to the discussion of rural sustainability by considering HBBs as a potential strategy to achieve sustainability in rural areas and small communities. Therefore, the primary objectives of the dissertation are to examine the nature and degree of relationships of home business activity between rural, rural-urban fringe and urban areas, and the implications on links for sustainability of rural households and communities; to examine the relationship of HBB activity to the concept of rural entrepreneurship and business development; and to assess the contributions of rural and small town HBBs to the sustainability of households and communities in Saskatchewan. <p>A combination of the concept of sustainable community development, the von Thunen model and the competitive strategy model (i.e., cost-leadership, differentiation, focus) provide the theoretical framework of the thesis. Through snowball sampling and mail questionnaire surveys, primary data on HBBs were obtained from Saskatoon and its surrounding regions in Saskatchewan for micro-level analysis. Both qualitative and quantitative techniques were employed to analyze the data. <p>Major findings of the thesis include the linkages that HBBs create among places and the impact of links on community sustainability; and the apparent spatial variations in HBBs, motivations, competitive strategies, and benefits of home businesses from the urban core to the limits of the rural hinterland. It was also found that home businesses contribute positively to the sustainability of households and communities in rural Saskatchewan. Specifically, they generate significant revenue and employment opportunities for people, while supporting local economies through their networks, purchasing and selling of goods and services locally as well as keeping managers and members of their households in rural areas and small communities for considerable number of years. Indeed home-based occupation is a vital component of the mechanisms for rural sustainability. Also, this thesis proposed a rural-urban HBB model for future social science research. <p>Major conceptual underpinnings of the research include rural-urban relations; regional and community economic development, sustainable community development; rural entrepreneurship, home-based work, home business, self-employment, competitive strategy, and the von Thunen Isolated State model.

In order to explore further the capabilities of parallel computing architectures such as grids, clusters, multi-processors and more recently, clouds and multi-cores, an easy-to-use parallel language is an important challenging issue. From the programmer's point of view, OpenMP is very easy to use with its ability to support incremental parallelization, features for dynamically setting the number of threads and scheduling strategies. However, as initially designed for shared memory systems, OpenMP is usually limited on distributed memory systems to intra-nodes' computations. Many attempts have tried to port OpenMP on distributed systems. The most emerged approaches mainly focus on exploiting the capabilities of a special network architecture and therefore cannot provide an open solution. Others are based on an already available software solution such as DMS, MPI or Global Array and, as a consequence, they meet difficulties to become a fully-compliant and high-performance implementation of OpenMP. As yet another attempt to built an OpenMP compliant implementation for distributed memory systems, CAPE − which stands for Checkpointing Aide Parallel Execution − has been developed which with the following idea: when reaching a parallel section, the master thread is dumped and its image is sent to slaves; then, each slave executes a different thread; at the end of the parallel section, slave threads extract and return to the master thread the list of all modifications that has been locally performed; the master includes these modifications and resumes its execution. In order to prove the feasibility of this paradigm, the first version of CAPE was implemented using complete checkpoints. However, preliminary analysis showed that the large amount of data transferred between threads and the extraction of the list of modifications from complete checkpoints lead to weak performance. Furthermore, this version was restricted to parallel problems satisfying the Bernstein's conditions, i.e. it did not solve the requirements of shared data. This thesis aims at presenting the approaches we proposed to improve CAPE' performance and to overcome the restrictions on shared data. First, we developed DICKPT which stands for Discontinuous Incremental Checkpointing, an incremental checkpointing technique that supports the ability to save incremental checkpoints discontinuously during the execution of a process. Based on the DICKPT, the execution speed of the new version of CAPE was significantly increased. For example, the time to compute a large matrix-matrix product on a desktop cluster has become very similar to the execution time of the same optimized MPI program. Moreover, the speedup associated with this new version for various number of threads is quite linear for different problem sizes. In the side of shared data, we proposed UHLRC, which stands for Updated Home-based Lazy Release Consistency, a modified version of the Home-based Lazy Release Consistency (HLRC) memory model, to make it more appropriate to the characteristics of CAPE. Prototypes and algorithms to implement the synchronization and OpenMP data-sharing clauses and directives are also specified. content and structure of introductory training. The research question formulated for this study
is: What are the introductory training needs of nannies in the United Kingdom? The
researcher approached the study qualitatively and empirical data was collected by means of a
focus group discussion with a group of nannies. Empirical data indicates that nannies need
training regarding:
• The professional aspect of nannying;
• the physical, emotional, and intellectual needs of children;
• the various developmental levels of children and the role of the nanny within each
phase;
• working with parents;
• different religions and cultures and their childcare practices.
It was found that nannies need training that is structured in an affordable, brief, specific and
practical way. The study therefore reveals that nannies have specific needs regarding both the

Community home based care for HIV and AIDS patients : a Malawian experiencePindani, Mercy 11 1900 (has links)
The purpose of this study was to describe the experiences that HIV and AIDS patients who are on Community Home Based Care Programme have in the Lilongwe district of Malawi. Finally; concrete propositions were developed for the implementation of quality community and home based care programmes in Malawi.
A qualitative research design using an interpretive phenomenology was used. The study took place in semi-rural and urban areas of Lilongwe district using patients from 3 major organizations dealing with people living with HIV and AIDS. Purposive sampling technique was used to choose the sample and a total of 15 in-depth interviews were conducted. Data was analyzed using ATLAS ti version 5.
Findings revealed that the majority of the participants were concerned that they were living with HIV and AIDS. Most of them expressed anxiety, worries and fears of death. Another majority complained about the burden of opportunistic infections. Almost half of the participants felt guilty bringing misery to their families and complained of stigma and discrimination. However, a minority group of participants felt that to live with HIV and AIDS is not the end of life. Relatives were mentioned as the main care providers to HIV and AIDS patients. However, they were also cited high as a source of stigma.
Conclusions were made that women and girls are at the highest risk as they bear the burden of care. It was therefore recommended that the Government of Malawi and all Non Governmental Organizations should develop women social economic status through promotion of education, provision of loans and provision of Gender Sensitive trainings. Formal training for care of HIV and AIDS patients should be introduced to all primary caregivers to render quality care in the homes and therefore this study has developed and pre-tested an educational program for this group. / Health Studies / D.
