Epidemiologia da hipertensÃo arterial em uma populaÃÃo urbana de baixa renda / Epidemiology of hypertension in an urban low-income

A hipertensÃo arterial tÃm sido considerada uma importante patologia para a saÃde pÃblica dada sua determinaÃÃo na ocorrÃncia de doenÃas cardiovasculares, notadamente, o infarto do miocÃrdio, a doenÃa vascular cerebral e a insuficiÃncia cardÃaca congestiva. VÃrios fatores de risco relacionados com a hipertensÃo arterial jà foram descritos, entretanto, em nosso Estado ainda à pouco conhecida a importÃncia relativa de cada um deles. Esse estudo teve como objetivo verificar a prevalÃncia e a importÃncia relativa de alguns fatores de risco clÃssicos da hipertensÃo arterial numa populaÃÃo adulta, de baixa renda, do Conjunto habitacional Metropolitano, Caucaia-CE na regiÃo metropolitana de Fortaleza, bem como medir a associaÃÃo entre trabalho domÃstico e hipertensÃo em mulheres. Foi realizado um inquÃrito casa a casa, em 760 domicÃlios de uma Ãrea contÃnua do bairro, quando entÃo foram entrevistados, feito exame antropomÃtrico e aferida a pressÃo arterial, de 1.248 indivÃduos de ambos os sexos, a partir de 30 anos de idade. Foi observada, uma prevalÃncia de hipertensÃo arterial de 23,96% (VI JNC) e 9,85% (OMS). A prevalÃncia de sobrepeso foi de 35,76% e de obesidade foi de 15,18%. As mulheres apresentaram um crescimento significativo da prevalÃncia do sobrepeso e obesidade com o aumento da idade, enquanto os homens nÃo apresentaram. Entre os indivÃduos com sobrepeso e obesidade, a prevalÃncia da hipertensÃo(VI JNC) foi 25,42% e 40,11% respectivamente. NÃo foi observada diferenÃa significativa na prevalÃncia da hipertensÃo entre os sexos. Uma maior renda familiar e maior nÃmero de anos na escola estavam associados significativamente a uma menor prevalÃncia de hipertensÃo arterial. Para medir a associaÃÃo entre trabalho domÃstico e hipertensÃo arterial em mulheres foi realizado um estudo caso controle, com 147 casos e 300 controles, na faixa etÃria de 30 a 55 anos de idade. Casos foram mulheres com PAD igual ou superior a 90 mm Hg e/ou PAS igual ou superior a 140 mm Hg. Controles foram mulheres com PAD inferior a 80 mm Hg e PAS inferior a 120 mmHg. O trabalho domÃstico foi medido em tempo gasto em horas/dia com as atividades de lavar, passar, cozinhar e limpar, alÃm de cuidados com crianÃas, idosos e doentes. Desse total, 99,0% (442) realizavam trabalho domÃstico em casa e 1,0% (5) disseram nÃo fazer qualquer tipo de atividade domÃstica em casa, 9,50% (42) exerciam trabalho domÃstico fora de casa (emprego domÃstico); 12,44% (55) exerciam trabalho nÃo domÃstico em casa (costurar, bordar, vender) e 13,57% (60) exerciam trabalho nÃo domÃstico fora de casa (comerciÃria, operÃria, balconista). Verificou-se associaÃÃo significativa entre realizar trabalho domÃstico fora de casa e hipertensÃo arterial (OR=2,17; IC 95%: 1,04 â 4,53) ajustada para as variÃveis idade, IMC, renda e horas de trabalho domÃstico em casa, que tambÃm apresentaram-se significativamente associadas à hipertensÃo. Os demais tipos de ocupaÃÃo nÃo apresentaram associaÃÃo significativa com a hipertensÃo, inclusive realizar trabalho domÃstico em casa. Esses resultados apontam para a importÃncia da obesidade na populaÃÃo adulta geral, enquanto fator passÃvel de intervenÃÃo, bem como estudos prospectivos especÃficos sobre ocupaÃÃo e hipertensÃo arterial em mulheres. / The High Blood Pressure has been considered to be one of the most important pathologies for the Public Health, due to its importance for the occurrence of the cardiovascular diseases, specially the heart attack, the vascular cerebral disease and the congestive cardiac insufficiency. Many risk factors related to the High Blood Pressure have been described in the literature. However, in our State, relative importance of each one of them is still little known. The aim of the present study was to verify the prevalence and the relative importance of some classic risk factor for the High Blood Pressure in an adult population with low income, as well as to measure the association between domestic work and High Blood Pressure in women. The study area was âThe Conjunto Habitacional Metropolitanoâ, a group of apartment blocks for low income people in the Municipality of Caucaia, Cearà State, Brazil. A survey was carried out, in house by house basis, in 760 households in a continuos area of the âConjunto Habitacional Metropolitanoâ. A total of 1,248 individuals of both sexes and thirty years old or older was enrolled in this survey. During the survey an interview was carried out, a anthopometric exam was conducted and the blood pressure of all individuals was measured. It was observed a prevalence of arterial blood pressure of 23,96% (VI JNC) and 9,85% (WHO). The prevalence of overweight was 35,76% and the obesity was of 15,18%. Women presented an significant increase on the prevalence of overweight and obesity as the age goes on. On the other hand, men did not show this increase. Among the individuals with overweigth and obesity, the prevalence of high blood pressure (VI JNC) was 25,42% in women and 40,11% in men. it was not observed any significant difference of high blood pressure between the sexes. A higher society level and a longer school live were associated with a smaller prevalence of arterial high blood pressure. To measure the association between the work done at home and high blood pressure in women, was executed a case-control study. It involved 147 cases and 300 controls. The average of age was of 30 to 55 years old. The cases were women whose PAD was equal or higher than 90 mm Hg and/or PAS was equal or higher than 140 mm Hg. The controls were women with PAD lower than 80 mm Hg and PAS lower than 120 mm Hg. The domestic work was measured in hours during the day. The activities studied were: wash, cook, clean, and care with children, oldies and people with disease. Form the total, 99,0% (442), did home work and 1% (5) said that they had no work at home, 9,50% (42) worked outside but still doing domestic work, 12,44% (55) do not practice domestic work at home. and 13,57% (60) do not practice domestic work outside (sellers, secretaries). It was verified a significant association between doing domestic work out of the house and High Blood Pressure (OR=2,17; CI 95%: 1,04 â 4,53) adjusted for the following variables: age, IMC, income and hours of domestic work at home. This variables have also shown to be significantly associated with High Blood Pressure. The other kinds of occupation did not show any significant association with High Blood Pressure, even domestic work at home. This findings point out to the importance of the obesity in the overall adult population, while a possible factor of intervention, as well as the need of prospective studies specifically to verify the association between occupation and High Blood Pressure in women.

Familial history of hypertension : hemodynamic responses to exercise in children

Hypertension afflicts one in four American adults and is a major risk factor for cardiovascular disease. Studies have shown that a family history of hypertension is an important predictor of future hypertension. Two hemodynamic factors control blood pressure (BP); cardiac output (CO) and total peripheral resistance (TPR). Although children of hypertensive parents may exhibit normal levels of these hemodynamic variables at rest, the response of these variables during exercise stress may differ. Therefore, the present study was designed to investigate whether children with a positive family history of hypertension exhibit an exaggerated BP response due to either an increased CO or an attenuated decrease in TPR during dynamic submaximal exercise compared to children of normotensive parents. Eleven children 12.2 ± 1.8 yr (M ± SE) of normotensive parents and 11 children 12.0 ± 2.4 yr of at least one hypertensive parent completed an orientation session, graded maximal cycle ergometer test, and a submaximal exercise bout consisting of 6 minutes of steady state cycling at 50 and 80% of maximal heart rate reserve. Blood pressure, CO and TPR were measured during the last 3 minutes of each submaximal exercise stage. An independent t-test was used to determine differences in the resting measures. The changes in TPR, BP and CO from rest through 80% intensity stage were examined using a twoway (group x intensity) ANOVA. The groups were evenly matched for age, weight, height, and body fatness. The children with a positive family history of hypertension had significantly higher resting systolic BP, diastolic BP (DBP), and mean arterial pressure (MAP) (p<0.05) compared to those children with a negative family history. Although there were no significant interactions among any of the variables studied, there was a tendency for TPR to be higher (p>0.05) at rest and throughout exercise in the positive history group. MAP and DBP were significantly higher in the positive family history group at rest and remained higher throughout exercise. In conclusion children of hypertensive parents exhibit a modest but significantly higher MAP and DBP at rest and during submaximal exercise. This subtle difference in BP control reflects an early trend toward increased TPR. / School of Physical Education

Assim sendo, foi objetivo geral da pesquisa analisar as crenças em saúde de pessoas negras hipertensas relacionadas às barreiras e benefícios das medidas de prevenção e controle da doença. Os objetivos específicos foram: 1) Estimar o percentual de crenças em saúde sobre barreiras e benefícios quanto às medidas de prevenção e controle da hipertensão arterial para essas pessoas; 2) Conhecer os fatores sócio-demográficos associados às crenças em saúde sobre benefícios das medidas de prevenção e controle da hipertensão arterial para elas. Trata-se de estudo descritivo-exploratório, de natureza quantitativa, que adotou como referencial teórico o Modelo de Crenças em Saúde, realizado em um Centro de Saúde de Salvador, com 106 adultos, autodeclarados negros ou pardos e com diagnóstico médico de hipertensão arterial. Para a entrevista, utilizou-se a “Escala de Crenças em Saúde” extraída da tese de Dela Coleta (1995), sobre 13 comportamentos relacionados às medidas de prevenção e controle da doença. Para análise das crenças em saúde, utilizaram-se índices percentuais, freqüência de casos e escores; para verificar a associação dos fatores sócio-demográficos a essas crenças, utilizou-se a razão de prevalência. Os testes estatísticos foram verificados no nível de 5% de significância. A análise global mostrou unanimidade na percepção de benefícios para a medida da pressão arterial e predomínio da categoria “crenças sobre benefícios” para 11 comportamentos de saúde. Homens e mulheres perceberam diferentemente benefícios para os comportamentos de saúde, mas essa diferença não foi estatisticamente significante. Pessoas com idade < 45 anos comparadas a 54 anos perceberam maior crenças sobre benefícios apenas para fazer relaxamento (RP=1,26), ter horas de diversão e lazer (RP=1,29) e praticar exercícios físicos (RP=1,04). Indivíduos com 1º grau em relação àqueles com 3º grau perceberam menor benefícios em 10 comportamentos de saúde, sendo a diferença estatisticamente significante para fazer relaxamento e consultar o médico uma vez por ano. As pessoas sem companheiros em relação às com companheiros perceberam menor crenças sobre benefícios em 11 comportamentos de saúde havendo uma diferença estatisticamente significante para consultar o médico uma vez por ano. Aqueles que recebiam menos de um salário mínimo em relação a pessoas com renda > 3 salários mínimos perceberam menor benefícios para 9 dos doze comportamentos de saúde, exceto para comer menos doce (RP=1,0) e evitar o consumo de álcool (RP=1,06). O estudo contribui para a identificação de grupos de risco e indicadores de adesão ao tratamento, bem como para reflexão sobre novas formas de atenção à saúde.

Association of Standardized Estimated Glomerular Filtration Rate With the Prevalence of Hypertension Among Adults in the United States

National Kidney Disease Education Program has initiated a serum creatinine standardization program. Glomerular filtration rate (GFR) can be re-estimated from standardized serum creatinine measurements. How the standardized estimated GFR (eGFR) influences hypertension prevalence has not been evaluated. In this study, cross-sectional data from 21 205 participants aged 18 years in the National Health and Nutrition Examination Survey 1999-2006 were analyzed. The differences between standardized and non-standardized eGFRs in the prevalence of hypertension and low eGFR were evaluated. Multiple logistic regression models were conducted to determine the association of standardized eGFR with hypertension prevalence. The prevalence of low eGFR estimated from standardized eGFR was higher than that from non-standardized eGFR (all P0.01), except for the 2005-2006 survey. The prevalence of hypertension under standardized eGFR was not significantly different from that under non-standardized eGFR in both groups of participants with eGFR60 and eGFR60 ml min 1 per 1.73 m 2. Adjusted for age, education, gender, race/ethnicity, smoking, serum cholesterol and diabetes mellitus, the participants with standardized eGFR60 ml min 1 per 1.73 m 2 had 56.1% more chance to be hypertensive patients than those with normal eGFR (P0.0001). The difference in the relationship to hypertension prevalence between standardized and non-standardized eGFR was not found significant.

This study had a cross-sectional, descriptive and quantitative approach. For data collection, interviews were conducted utilizing the Brazilian Economic Classification Form (ABEP), supplemented; the International Physical Activity Questionnaire (IPAQ long version) was used to evaluate the physical activity of adults, and a questionnaire about knowledge of Hypertension was also utilized for adults. For children and adolescents, the Physical Activity Level Assessment and Sedentary Behavior of 10 to 13 year-old schoolchildren (NAF) were used. An anthropometric screening and blood pressure measurement were performed in all individuals. The data were analyzed utilizing the Statistical Package of Social Sciences (SPSS, 23.0) adopting a level of significance of 5% (p <0.05). The sample consisted of 86 individuals belonging to the quilombola community, residents in rural zone and urban areas. The HAS presented a prevalence of 33.7%. The community is composed mostly of black, female, young, with a monthly family income of a minimum wage, belonging to social classes D-E. Individuals were living in both rural zones and urban areas. The risk factors for arterial hypertension were: age (60 to 88 years, p <0.001), race / color (black), social class (SD), high weight (p = 0.009), BMI on weight or obesity p = 0.002) and place of residence (urban area and rural zone). It was concluded that hypertension had high prevalence and its risk factors were present in the sample. / Este estudo objetivou investigar a prevalência de Hipertensão Arterial e fatores de risco em crianças, adolescentes, adultos e idosos na comunidade quilombola Ilha de São Vicente no Estado do Tocantins. Trata-se de um estudo transversal, descritivo (analítico). Para coleta de dados realizou-se entrevistas por meio do formulário de classificação econômica do Brasil (ABEP) complementado, formulário acerca do conhecimento sobre a Hispertensão (adultos), para avaliar a atividade física de adultos o International Physical Activity Questionnaire (IPAQ versão longa) para crianças e adolescentes utilizou-se a Avaliação do Nível de Atividade Física e Comportamento Sedentário de escolares de 10 a 13 anos de idade (NAF). Foi realizado triagem antropométrica e medição de pressão arterial. Os dados foram analisados com pacote estatístico Statistical Package of Social Sciences (SPSS, 23,0) adotando um nível de significância de 5% (p < 0,05). A amostra foi composta por 86 indivíduos pertencentes a comunidade quilombola, residentes da zona rural e outros zona urbana. A HAS apresentou prevalência de 33,7%. A comunidade é composta em sua maioria por indivíduos negros, do sexo feminino, jovens, renda familiar mensal de um salário mínimo, pertencentes as classes sociais D-E, possuindo indivíduos que residiam na zona rural e outros na zona urbana. Apresentaram-se como fatores de risco: idade (60 a 88 anos; p <0,001), raça/cor (preta), classe social (D-E), peso elevado (p=0,009), IMC sobre peso ou obesidade (p=0,002) e local de moradia (zona urbana e zona rural). Conclui-se que a HAS apresentou elevada prevalência e seus fatores de risco se mostraram presentes nos indivíduos.

Considering that hypertension affects individuals in their most productive age while facing work and living risk factors, it is important to investigate its occurrence and predisposing factors in different occupational segments. The objective of this study was to identify the prevalence of hypertension among workers attended to in a medical service of a public university, their hypertension levels, the risk factors present, and their knowledge of the factors that influence the arterial pressure. The epidemiologic study was conducted in the Health Department of the Federal University of Rio Grande do Norte with 102 workers that sought care in the medical clinic during the months of March to May 2009. Data were collected by means of a questionnaire and measurements of systolic and diastolic arterial pressure (SAP and DAP) that were classified in stages according to the Brazilian Society of Hypertension and the degree of risk for cardiovascular events according to the criteria of the Brazilian Society for Cardiology. Data were analyzed using descriptive statistics. The workers were, on average, 54 years of age; the majority (67%) was male and had primary or middle educational level; they worked mainly in supplemental units and deanship offices conducting different functions such as security guards, administrative assistants, health auxiliaries and constructions workers; 48 (47%) of the workers identified themselves as hypertensive for 8 years on average, with the majority executing hard labor and administrative functions. Among the workers with hypertension, the number of the pressure levels classified as pre-hypertensive, stage I and II were: (12% in the SAP and 20% in the DAP); (16% in the SAP and 9% in the DAP); and (15% in the SAP and 5% in the DAP), respectively. The workers that did not identify themselves as hypertensive presented classifications with greater frequencies were: normal (16% in the SAP and 30% in the DAP); and pre-hypertensive (21% in the SAP and 16% in the DAP). The risk factors identified in more than 50% of the workers were: tobacco smoking, alcohol consumption and indices of being overweight, although physical activities are also present. Of the 48 workers diagnosed as hypertensive, those that had 5 risk factors present and limitrophic pressure levels (12%), in stage I hypertension (16%) and stage II hypertension (15?%) were categorized as being in high risk for vascular events. The number of workers that indicated they had knowledge of the factors that influence their hypertension was less than 39% for each factor. It is concluded that there is a high prevalence of systemic arterial hypertension in the university workers, even amongst those already under treatment. They constitute a population at risk considering their age group, their work functions, and their inadequate life habits. Health care of these hypertensive workers that seek attention in the Health Department is an important aspect of the internal workers health policy in the institution. Objetivou-se, neste estudo, identificar a preval?ncia da HAS em servidores atendidos em um servi?o de sa?de de uma universidade p?blica, as varia??es de n?veis press?ricos, os fatores de risco presentes e o conhecimento que esses servidores possuem sobre os fatores que influenciam a eleva??o da press?o arterial. A pesquisa epidemiol?gica foi realizada no Departamento de Atendimento ao Servidor (DAS) da Universidade Federal do Rio Grande do Norte (UFRN), com 102 servidores atendidos na clinica m?dica no per?odo de mar?o a maio de 2009. Os dados foram coletados por meio de um question?rio e pela aferi??o dos n?veis press?ricos arteriais sist?licos e diast?licos (PAS e PAD). As aferi??es foram classificadas em est?gios de hipertens?o, segundo os par?metros da Sociedade Brasileira de Hipertens?o (SBH), e quanto ao risco cardiovascular que indicavam, com base nos crit?rios da Sociedade Brasileira de Cardiologia (SBC). Os dados foram analisados utilizando t?cnicas de estat?stica descritiva. Os servidores tinham, em m?dia, 54 anos de idade; a maioria era do sexo masculino (67%) e com escolaridade de n?vel fundamental ou m?dio (76%); trabalhavam principalmente nas unidades suplementares e nas pr?-reitorias; exerciam diversas fun??es, com predomin?ncia de vigil?ncia, assist?ncia administrativa, como auxiliares nos setores de sa?de e em obras estruturais. Identificou-se que 47% (48) dos servidores se autorreferiram como portadores de hipertens?o por oito anos em m?dia, a maioria exercendo fun??es bra?ais e administrativas. Entre os trabalhadores com hipertens?o autorreferida, observaram-se percentuais de trabalhadores com n?veis press?ricos nas classifica??es de pr?-hipertenso (12% no PAS e 20% no PAD), em est?gio I de hipertens?o (16% no PAS e 9% no PAS) e no est?gio II da doen?a (15% no PAS e 5% no PAD). As aferi??es dos n?o portadores foram classificada em: normal (16% no PAS e 30% no PAD) e pr?-hipertenso (21% no PAS e 16% no PAD). Os fatores de risco identificados em mais de 50% dos sujeitos foram: tabagismo, etilismo e sobrepeso, embora houvesse ind?cios de alguns h?bitos positivos para combater o sedentarismo. Dos 48 servidores com diagnostico de hipertens?o que tiveram 5 cinco fatores de risco presentes e n?veis press?ricos lim?trofes (12%), em est?gio I e II foram categorizados como de alto risco vascular, respectivamente, 16% e 15%. Nos servidores que indicaram ter conhecimento dos fatores que influenciam a eleva??o de sua press?o arterial, o percentual informado foi menor do que 39% para cada fator de risco citado. Guidelines for the prevention of the burden of hypertension among adults in Hawassa City administration of Ethiopia

PURPOSE: The main aim of this study was to develop guidelines for prevention of hypertension among adults in Hawassa City, Ethiopia. METHOD: The study used mixed methods approach. In Phase 1, a quantitative community-based comparative cross-sectional survey was used to establish the burden of hypertension and its determinant factors in the urban and peri-urban areas. The survey was meant to describe adult’s knowledge and attitudes regarding hypertension risky behaviour and its prevention. Secondly, in the qualitative phase, focus group discussion (FGD) among health care providers of selected sub-cities of the first phase of the study was conducted to explore their perspectives towards efforts exerted for the prevention of hypertension. Thirdly, based on the findings of the two phases of the study, review of literatures, the researcher’s insights and exploring expertise opinion using a Delphi technique, a guideline for hypertension prevention among adults was developed. RESULTS: The overall prevalence of hypertension was 21.2%, higher in the urban setting (24.5%) than the peri-urban (14.7%) (x2=7.81 and p-value 0.003). Fifty-five (55) of the hypertensive participants were newly screened. In the urban setting age, wealth, use of top added salt on plate, mode of transport, family history of hypertension, ever-told have diabetes and body mass index were significantly association with hypertension. In the peri-urban setting age, sex, vegetable eating habit, performing physical fitness activities and family history of hypertension showed significantly association with hypertension. Regarding knowledge of hypertension risky behaviour and prevention, 46.1% (n=282) of the respondents were with “low”, 37.6% (n=230) with “medium” and 16.3% (n=100) with “high” level of knowledge on hypertension risky behaviour and prevention. The majority of respondents demonstrated positive attitude toward perceived susceptibility and perceived severity whereas the minority had poor attitude toward their perceived benefit of applying hypertension prevention. In the qualitative study, four themes and 12 sub-themes emerged, which explored health care providers’ perspectives regarding efforts exerted for the prevention of hypertension In conclusion prevalence of hypertension is high and has become a public health problem but activities done on the prevention are minimal. Guidelines developed from this research, if utilised, will assist in lessening the burden of hypertension in Ethiopia among adults. / Health Studies / D. Litt. et Phil. (Health Studies)
