Previous issue date: 2016-03-04 / Introdução: Apesar dos avanços recentes no tratamento de pacientes com doença
de Crohn (DC), os sintomas oclusivos e suboclusivos observados na presença de
estenoses clinicamente significativas permanecem um problema clínico desafiador.
Na DC, a identificação de fatores que se associam à redução do risco de cirurgia é
importante.
Materiais e Métodos: Neste estudo retrospectivo, avaliamos os possíveis fatores
preditivos, incluindo os marcadores inflamatórios associados à redução da
necessidade de intervenção cirúrgica em pacientes com DC que apresentaram o
primeiro episódio de suboclusão intestinal clinicamente resolvido e tratados,
subsequentemente, com azatioprina (AZA) durante três anos.
Resultados: Trinta e seis pacientes com DC suboclusiva foram incluídos, dos quais,
24 não necessitaram de ressecção intestinal. Nenhum dado demográfico ou clínico
associou-se com a resposta à AZA. Apenas a proteína C reativa (PCR) apresentou
correlação com a eficácia da AZA. Para cada aumento de 1 mg na PCR, houve uma
redução do risco de cirurgia em 8% (RR 0,92; IC 0,86-0,98; p=0,008). O grupo
PCR>6 (elevada) apresentou 81% de redução de risco de cirurgia em relação ao
grupo PCR<6 (OR 0,19 IC 0,05-0,64; p=0,008).
Conclusões: Os pacientes que apresentaram PCR elevada tiveram uma menor taxa
de cirurgia a médio e longo prazos durante a terapia com AZA. A PCR pode
identificar pacientes com estenoses predominantemente inflamatórias e responsivas
ao tratamento clínico. / Background: Despite recent advances in the treatment of patients with Crohn's
disease (CD), occlusive and subocclusive symptoms observed in the presence of
clinically significant stenosis remains a challenging clinical problem. In inflammatory
bowel diseases (IBD), the identification of factors associated with reduced risk of
surgery in this context is important.
Materials and methods: In this retrospective study, we evaluated the possible
predictive factors, including inflammatory markers associated with reduced need for
surgical intervention in patients with CD who presented the first episode of clinically
solved subocclusion and treated subsequently with azathioprine (AZA) for three
years.
Results: Thirty-six patients with subocclusive CD were included, of these, 24 has not
required bowel resection. No demographic or clinical data associated with the
response to AZA. Only C reactive protein (CRP) was correlated with the
effectiveness of AZA. For each increase of 1 mg CRP, there was a reduction of
surgery risk in 8% (RR 0.92, CI 0.86-0.98; P = 0.008). The CRP group> 6 (elevated)
had 81% of surgery risk reduction compared to PCR group <6 (OR 0.19 CI 0.05-0.64;
P = 0.008).
Conclusions: Patients with elevated CRP has a lower rate of surgery in the medium
and long term during therapy with AZA. CRP can identify patients with inflammatory
stenosis and responsive to clinical treatment.

Previous issue date: 2009-04-03 / Várias espécies de Senna são bastante conhecidas por suas propriedades terapêuticas, sendo utilizadas na medicina popular para o tratamento de inflamação na garganta e constipação. Neste estudo, foram avaliadas algumas propriedades biológicas e farmacológicas das folhas de Senna macranthera como as atividades anti-inflamatória, laxativa, sobre motilidade intestinal, antimicrobiana e antioxidante. A triagem fitoquímica do seu extrato metanólico e partições permitiu a identificação de compostos fenólicos como flavonoides, taninos e cumarinas na maioria das amostras, com exceção da partição em hexano, que apresentou antronas, triterpenos e esteroides em sua composição. O perfil cromatofráfico obtido por CLAE mostrou a presença da antraquinona emodina em todas as amostras. A partição em hexano reduziu significativamente o edema de pata induzido pela carragenina em ratos, de maneira similar ao diclofenaco de sódio. Atividade laxativa comparável ao padrão bisacodil foi observada para o extrato em metanol e partição em hexano, porém somente o último aumentou a motilidade intestinal. Além disso, o extrato metanólico e partições foram avaliados quanto a sua atividade antioxidante e a partição em acetato de etila apresentou pronunciada atividade, comparável ao padrão rutina. Significativa atividade antibacteriana sobre Pseudomonas aeruginosa também foi encontrada para o extrato em metanol e partição em hexano. Não foi observada citotoxidade para Artemia salina ou espécies de Leishmania para nenhuma amostra. / Several species of Senna are well known for their therapeutic properties, being used in the folk medicine to treat throat inflammation and constipation. In this study, an attempt was made to evaluate some biological and pharmacological properties of Senna macranthera leaves. In this regard, the anti-inflammatory, the laxative and the intestinal motility effects, and also the antimicrobial and antioxidant activities of its methanolic extract and partitions were determined. The phytochemical screening allowed the identification of phenolic compounds like flavonoids, tannins and coumarins in the majority of the extracts except for the hexane one which presented anthrones, triterpenes and steroids in its composition. The HPLC profile showed the presence of the anthraquinone emodine in all samples. The hexane partition significantly reduced carrageenan-induced paw edema in rats in a manner similar to that of sodium diclofenac. Laxative effect was observed for the methanolic extract and hexane partition comparative to the standard drug bisacodyl, however only the later increased the intestinal motility. Moreover, the samples were evaluated for their antioxidant activity and ethyl acetate partition presented a pronounced activity comparable to the standard rutin. Antibacterial activity against Pseudomonas aeruginosa was also found for metanolic extratct and hexane partition. No cytotoxicity against Artemia salina and Leishmania species were detected for any extracts.

Identification and validation of new markers and potential therapeutic targets for gastrointestinal stromal tumors in murine models and in human pathological materialGromova, Petra 09 June 2011 (has links)
Les tumeurs gastro-intestinales stromales (Gastro-Intestinal Stromal Tumours - GIST en Anglais) sont les sarcomes les plus fréquents du tube digestif. Sur base de leur profil d'expression génique et de similitudes morphologiques, il a été établi que les GIST dérivent des cellules interstitielles de Cajal (Interstitial Cells of Cajal - ICC en Anglais) ou d'un précurseur commun. Le développement et le maintient des ICC sont dépendant de voies de signalisation du récepteur tyrosine kinase KIT. Des mutations oncogéniques de KIT, conduisant indépendamment du ligand à l'activation des voies de signalisation en aval, sont présentes dans environ 85% des GIST. Depuis une dizaine d'années, des molécules de synthèse qui inhibent la phosphorylation -et donc l'activation - de KIT ont été introduites avec succès dans le traitement clinique des GIST. Cependant des résistances, souvent causées par des mutations secondaires, apparaissent fréquemment et environ 50% des patients traités rechutent dans les 2 ans. Le développement de nouvelles stratégies diagnostiques et thérapeutiques pour les GIST demeure donc essentiel. Ces dernières années, de nouveaux marqueurs diagnostiques ou cibles thérapeutiques potentielles ont été rapportés dans la littérature (p.ex. Discovered on GIST-1 (DOG1, anoctamin 1), Protein kinase C theta (PKC theta), Carbonic anhydrase II (CAII)) .Il faut relever que tous ces gènes sont aussi exprimés par les ICC KIT+ du tube digestif normal et que leur présence dans les GIST reflète donc vraissemblablement essentiellement leur parenté avec les ICC.<p>Dans la présente étude, nous nous sommes attachés à identifier de nouveaux marqueurs diagnostiques ou cibles thérapeutiques potentielles exprimés par les GIST mais absent des ICC KIT+ normales.<p>Pour ce faire, nous avons comparé le profile d'expression géniques de l'antre gastrique de souris porteuses de la mutation oncogénique Kit K641E et de souris contrôles (wild type WT en Anglais) par la technique de cDNA microarray. Les différences d'expression génique ont été ensuite confirmées par réactions de PCR quantitative (qPCR) en temps réel et l'immunoréactivité (-ir) pour les candidats les plus prometteurs a été localisée par immunofluorescence (IF) dans la muscularis propria du tube digestif, avec une attention spéciale pour les cellules KIT+.<p>Plusieurs gènes identifiés appartenaient tant au profil d'expression génique des GIST qu'au profil d'expression des ICC Kit+ de l'intestin grêle murin, validant ainsi la pertinence du modèle murin KitK641E pour l'approche choisie (Chapitre 3). D'autre part, trois gènes identifiés (Neurotensin receptor 1 (Ntsr1), Trophoblast glycoprotein (Tpbg/5T) et Sprouty homolog 4 (Spry4)) étaient quant à eux présents dans la couche hypertrophié de cellules Kit+ de l'antre des souris KitK641E mais absentes des ICC Kit+ chez les souris WT (Chapitres 3, 4, 5). Ces gènes représentant donc de nouveaux candidats potentiels comme marqueurs spécifiques et/ou comme cibles thérapeutiques dans les GIST, nous avons, dans la seconde partie de notre travail, approfondi l'étude de leur expression et de leur régulation en utilisant des modèles cellulaires et tissulaires murins, ainsi que du matériel anatomopathologique de GIST humains.<p>Dans le tube digestif normal, NTSR1 et TPBG/5T4 ir ont été identifiés dans les neurones myentériques mais pas dans les ICCKIT+. Deux "tissue arrays" indépendants, totalisants 97 spécimens humains de GIST, ont révélés la présence de NTSR1-ir dans tous les GIST, en ce compris les cas négatifs pour KIT, tandis que TPBG/5T4-ir était présente dans 36/49 GIST. Un fort immunomarquage pour TPBG/5T4 était statistiquement associée aux tumeurs malignes et de haut risque (Chapitre 5; Annexe 1).<p>L'expression différentielle de membres de la famille des "Sprouty homologues" (Spry) dans l'antre des souris KitK641E a aussi été identifiée. Spry4-ir n'était pas détectable dans les ICC KIT+ des souris WT alors que Spry4-ir était présente dans la couche hyperplasique des cellules Kit+ chez les souris KitK641E. A l'opposé, l'ARN messager de Spry2 présentait un niveau d'expression similaire et Spry2-ir était détectée dans les cellules musculaires lisses - mais pas dans les cellules Kit+ - dans tous les génotypes (Chapitre 3). Pour sa part, l'expression de Spry1 apparaissait réprimée par le mutant oncogénique KitK641E, tant in vivo qu'in vitro, conduisant à la dérégulation de la boucle de rétrocontrôle négatif de la voie Ras/Erk (Chapitre 4). <p>Dans la dernière partie de cette thèse, nous avons étudié l'expression Endoglin (ENG) - aussi connues sous le nom de CD105 – dans le modèle murin de GIST KitK641E, dans les GIST humains et dans le modèle cellulaire murin Ba/F3 in vitro. Biodisponibilité nutritionnelle de systèmes colloïdaux riches en acides gras polyinsaturés : études in vivo et in vitro / Nutritional bioavailability of polyunsaturated fatty acid-rich colloidal systems : in vivo and in vitro studiesCouedelo, Leslie 14 November 2011 (has links)
Les derniers apports nutritionnels conseillés recommandent une consommation plus importante en acides gras polyinsaturés de la série n-3 que celle actuellement constatée dans l’alimentation française. Dans ce contexte, il convenait d’appréhender les facteurs susceptibles de moduler l’absorption et le devenir de leur chef de file, l’ALA, en faisant appel à deux approches, l’une in vivo (rat), l’autre in vitro (Caco-2). L’étude relative au devenir métabolique de l’ALA, selon sa forme physique et chimique de présentation, a été réalisée avec des lipides « modèles » (TAG structurés) ou naturels (huile de lin) riches en ALA, et selon différents systèmes lipidiques (huile en phase continue ou en émulsion de type huile dans eau et de composition en phospholipides variables). Les résultats obtenus in vivo et in vitro à l’égard de l’huile de lin (émulsionnée ou non) montrent que l’émulsification accélère non seulement le passage intestinal de l’ALA mais améliore également sa concentration lymphatique. L’étude cellulaire a par ailleurs démontré que la présence de lysophospholipides dans les micelles mixtes permet d’améliorer la sécrétion de l’ALA dans les lipoprotéines. D’autre part, le devenir métabolique de l’ALA dépend de sa régiolocalisation sur le triglycéride alimentaire. En effet, les résultats de l’étude faisant appel aux TAG structurés montrent que la position interne n’est que partiellement conservée dans la lymphe, suggérant qu’une hydrolyse des 2-MAG serait opérée par une MG lipase. En conséquence, l’ensemble des résultats obtenus lors de cette étude montre que l’absorption et le transport de l’ALA seraient uniquement modulés selon la forme physique de l’acide gras alors que son devenir et son utilisation métabolique dépendraient de sa régiolocalisation sur le TAG alimentaire. Ces deux facteurs réunis permettraient dés lors de prévenir l’ALA d’une β-oxydation précoce, en vue de favoriser son élongation en dérivés supérieurs dans les tissus cibles. / The last recommended nutrient intakes advise a higher consumption of n-3 polyunsaturated fatty acids series than currently found in French diet. In this context, it was appropriate to apprehend the factors that could modulate the absorption and fate of their leader, ALA, using two approaches, one in vivo (rat), the other one in vitro (Caco-2). The study on the metabolic fate of ALA, according to its physical and chemical submission form, was conducted with "models" lipids (structured TAG) or natural (flaxseed oil) rich in ALA, and according to different lipid systems (oil in continuous phase or in emulsion type oil in water and with variable phospholipid composition). Results obtained in vivo and in vitro for flaxseed oil (emulsion or not) show that emulsification enhances the recovery of ALA at the intestinal level but also improves its lymphatic concentration. The cell study also demonstrated that the presence of lysophospholipids in mixed micelles can improve the secretion of ALA in lipoproteins. On the other hand, the metabolic fate of ALA depends on its location on the glycerol backbone of the dietary triglyceride. The results of the study using structured TAG show that the internal position is partially preserved in lymph, suggesting that a hydrolysis of 2 - MAG by MG lipase could occur. Accordingly, all of the results obtained in this study shows that the absorption and transport of ALA would only be modulated according to the physical form of the fatty acid while its fate and its metabolic use would depend on its location on the dietary TAG. These two factors combined would then allow preventing the early β-oxidation of ALA in order to promote its elongation in higher derivatives in the target tissues.

Previous issue date: 2009-10-09 / An intestinal stoma produces numerous changes in patients' lives, a fact that requires adaptation and can change their quality of life. Therefore, this study aimed at evaluating the quality of life of patients with intestinal stomas registered in the Ostomized Patient Program of Stoma of an outpatient service in a municipality in southern Minas Gerais, Brazil. This is an epidemiological, descriptive, cross-sectional quantitative study that included 13 ostomized patients who were interviewed at home in July and August 2007. Two instruments were used to collect data: (1) characterization of participants and (2) the WHOQOL-Bref, for the "quality of life". For data collection, the interviewees signed an informed consent form, and the protocol was approved by the UNIFENAS Committee of Ethics in Research. The software SPSS version 10 was used for data analysis and the Chi-square and the Cronbach alpha tests were used for the crossing of variables. The results showed that most of the patients were 61 years or older, female, from urban areas, catholic, had elementary school; widow / separated / divorced, had 1 to 3 children and family income of 4 to 6 times the minimum wage. With respect to the physiological need for sleep, 61.5% of the population studied had insomnia, a situation that was aggravated after ostomy. Most of them reported 1 to 2 intestinal evacuations per day, with a pasty consistency. Their greatest difficulty was to adapt themselves to the stoma. When crossing gender and marital status with the difficulty of adapting to the bag and self-care, there was statistical significance. Most respondents did not return to sports, work, and sexual and social activities after ostomy, and showed increased feelings of loneliness, contempt, tearfulness and dependence. Their family was the main help. The most evident complications were bleeding and peristomal dermatitis. With regard to the rights of the ostomized, 84.6% reported having been informed by the physician and / or outpatient clinic and the main way of acquiring the collection equipment is through the SUS, free of charge. The measurements of the quality of life of this population were: mean score of overall QOL, 3.61; physical domain, 3.40; psychological domain, 3.43; social relationships domain, 3.46;and the environment domain, 3, 49. The higher average scores rated the quality of life as being above the "neither bad nor good." The WHOQOL-bref was effective to evaluate the QOL of patients with intestinal stomas, with internal consistency above 0.72. Finally, it is important that the ostomized patients be informed about their disease, and receive emotional support and rehabilitation care in order to be able to lead an active, productive and self-sufficient life. / Um estoma intestinal gera inúmeras alterações na vida dos pacientes, fato que requer adaptação e pode alterar sua qualidade de vida. Sendo assim, o presente estudo objetivou avaliar a qualidade de vida dos portadores de estomias intestinais cadastrados no Programa de Estomizados de um serviço ambulatorial em um município do sul de Minas Gerais. Trata-se de um estudo epidemiológico, descritivo, transversal, quantitativo. Participaram do estudo 13 estomizados, que foram entrevistados em suas residências, nos meses de julho e agosto de 2007. Para a coleta dos dados utilizaram-se dois instrumentos: um, de caracterização dos participantes, e outro, referente à qualidade de vida , sendo usado o WHOQOL-Bref. A coleta de dados seguiu os preceitos éticos, onde os entrevistados assinaram um Termo de Consentimento Livre e Esclarecido e tiveram anuência do Comitê de Ética em Pesquisa da UNIFENAS. Para análise dos dados utilizou-se o software SPSS versão 10 e, no cruzamento das variáveis, aplicou-se o teste do qui-quadrado, além do Coeficiente Alfa de Cronbach. Os resultados demonstraram que a maioria dos pacientes estudados tinha 61 anos ou mais, era do sexo feminino; procedente da zona urbana; da religião católica; tinha o primeiro grau incompleto; estava viúvo/desquitado/divorciado; tinha 1 a 3 filhos e renda familiar de 4 a 6 salários mínimos. Com relação à necessidade fisiológica de sono, 61,5% da população estudada demonstrou apresentar insônia, situação que foi agravada após a confecção do estoma. Quanto à freqüência e características das eliminações intestinais, a maioria referiu de 1 a 2 eliminações diárias, com consistência pastosa. A maior dificuldade de adaptação referida pelos estomizados foi com relação ao estoma. Ao cruzar as variáveis sexo e estado civil com a dificuldade de adaptação à bolsa e ao autocuidado, houve significância estatística. A maioria dos entrevistados não retornou às atividades esportivas, de trabalho, sexuais e sociais após a realização da estomia, onde se observou um aumento de sentimentos, como solidão, desprezo, choro fácil e dependência. A principal ajuda referida pela população estudada foi da família. As complicações mais evidenciadas foram sangramento e dermatite periestomal. Quanto aos direitos dos estomizados, 84,6% referiram ter sido informados pelo médico e/ou serviço ambulatorial e a principal forma de aquisição dos equipamentos coletores é a gratuita através do SUS. Quanto à qualidade de vida desta população, o escore médio da QV geral foi de 3,61; do domínio físico, 3,40; domínio psicológico, 3,43; domínio relações sociais, 3,46; e domínio meio ambiente, 3,49, evidenciando elevados escores médios, que classificam a qualidade de vida como sendo acima do nem ruim, nem boa . O WHOQOL-bref mostrou-se eficaz para avaliar a QV de pacientes com estomias intestinais, apresentando consistência interna superior a 0,72. Por fim, é importante que o estomizado seja informado acerca de sua doença, recebendo aporte emocional que lhe garanta suporte e reabilitação, sendo capaz de levar uma vida ativa, produtiva e autossuficiente.

Previous issue date: 2015-03-27 / The objective of the experiment evaluating different levels of egg powder residue in the diets of piglets weaned at 28 days of age, assessing performance, hematological parameters, the bacterial count and the quantification of immunoglobulin (IgM). The study was conducted at the Swine Federal Institute of Education Science and Technology of South of Minas Gerais Campus Muzambinho. One design was used in a randomized block design with four treatments and six replications and three animals per experimental unit. The initial weight was used as a criterion for constitution of blocks and treatments consisted of four powdered egg levels (0.0%, 2.0%, 4.0% and 6.0%). The results were evaluated using the linear regression analyzes and polynomial, and the adjustments evaluated for accuracy (R2). With the increase of egg powder residue levels there was an improvement in animal performance. In general, with increasing levels of powdered egg residue there was an increase in final weight of the animals, weight gain and an improvement in feed conversion. Regarding the blood components, there was a reduction on packed cell volume and total protein of seguimentados with raising the level of egg powder residue. The egg powder residue improves the performance of piglets weaned at 28 days of age. As one increases the inclusion of egg residue powder in the diet, there is a higher incidence of beneficial bacteria in tratogastrinteninal piglets, keeping them imonologicamente healthier / Objetivou-se, com a condução do experimento, avaliar diferentes níveis de resíduo de ovo em pó na dieta de leitões desmamados aos 28 dias de idade, avaliando o desempenho, os parâmetros hematológicos, a contagem bacteriana e a quantificação de imunoglobulinas (IgM). O trabalho foi desenvolvido na Suinocultura do Instituto Federal de Educação Ciência e Tecnologia do Sul de Minas Gerais Campus Muzambinho. Estabeleceu-se um delineamento em blocos ao acaso, com quatro tratamentos e seis repetições, sendo três animais por unidade experimental. Usou-se o peso inicial como critério para constituição dos blocos e os tratamentos foram constituídos por quatro níveis de ovo em pó (0,0%; 2,0%; 4,0% e 6,0%). Avaliaram-se os resultados por meio das das análises de regressão linear e polinomial e os ajustes, pela precisão (R2). Com o aumento dos níveis de resíduo de ovo em pó, houve melhora não só no peso final como também no ganho de peso médio diário e na conversão alimentar dos animais. Constatou-se redução do volume globular, dos segmentados e da proteína total com a elevação do nível de resíduo de ovo em pó. Além disso, com o aumento dos níveis do ovo, observou-se maior número de Bifidobacterium e Lactobacillus no cólon proximal dos leitões e menor contagem de Escherichia coli. O resíduo de ovo em pó melhora o desempenho dos leitões desmamados aos 28 dias de idade. À medida que se aumenta a inclusão de resíduo de ovo em pó na dieta, há maior incidência de bactérias benéficas no trato gastrointestinal dos leitões, mantendo-os imunologicamente mais saudáveis.

Previous issue date: 2017-06-13 / Passive smoking has become a major problem stemming from damage to the body from people involuntarily exposed to cigarette smoke. Concern with this group of people has led the scientific community to research alternatives that might be used to prevent the harm caused by secondhand smoke. Thus, the objective of the present study was to evaluate the deleterious effects of chronic passive smoking on the histomorphometry and histopathology of the duodenum of growing rats supplemented or not with functional prebiotic, probiotic and symbiotic foods. A total of 96 rats were distributed in a completely randomized design in a 2 x 4 factorial scheme. Animals were supplemented with functional foods and passively or not exposed to cigarette smoke for 180 days for one hour daily five days a week. At the end of the experimental period, samples of the initial duodenum were collected for histomorphometric evaluation and histopathological analysis. Animals exposed to secondhand smoke showed less development of duodenal villi and crypts, increased inflammatory profiles and degenerative changes. It was observed that supplementation with functional foods provided a mitigating effect of the toxic effects of exposure to passive smoking in all analyzed parameters. It is concluded that the use of prebiotic, probiotic and symbiotic in the diet of rats exposed to passive smoking may be a possible way to attenuate the toxic effects induced by secondhand smoke on the duodenal mucosa, and may serve as prevention of mucosal damage and development of Diseases caused by passive exposure to cigarette smoke. / O tabagismo passivo tornou-se um grande problema decorrente de danos causados ao organismo de pessoas expostas involuntariamente a fumaça de cigarro. A preocupação com esse grupo de pessoas levou a comunidade científica a pesquisar alternativas que possam vir a ser utilizadas para prevenir os danos causados pelo fumo passivo. Dessa forma, o objetivo do presente trabalho foi avaliar os efeitos deletérios do tabagismo passivo crônico sobre a histomorfometria e histopatologia do duodeno de ratos em crescimento suplementados ou não com os alimentos funcionais prebiótico, probiótico e simbiótico. Foram utilizados 96 ratos distribuídos em um delineamento inteiramente casualizado em esquema fatorial 2 x 4. Os animais foram suplementados com os alimentos funcionais e expostos passivamente ou não a fumaça de cigarro durante 180 dias por uma hora diária cinco dias por semana. Ao final do período experimental, foram colhidas amostras da porção inicial do duodeno para avaliação histomorfométrica e análise histopatológica. Os animais expostos ao fumo passivo apresentaram menor desenvolvimento das vilosidades e criptas duodenais, aumento nos perfis inflamatórios e alterações degenerativas. Foi observado que a suplementação com os alimentos funcionais proporcionou um efeito mitigador dos efeitos tóxicos da exposição ao tabagismo passivo em todos os parâmetros analisados. Conclui-se que a utilização de prebiótico, probiótico e simbiótico na dieta de ratos expostos ao tabagismo passivo pode ser uma possível forma de atenuar os efeitos tóxicos induzidos pelo fumo passivo sobre a mucosa duodenal, podendo servir como prevenção de danos a mucosa e desenvolvimento de doenças futuras ocasionadas pela exposição passiva a fumaça de cigarro.

O trabalho objetivou avaliar o efeito de probióticos e antibióticos utilizados como promotores do crescimento sobre o desempenho de frangos de corte e a capacidade dos agentes de alterar o ecossistema intestinal de aves criadas em baterias e sobre piso. Adicionalmente foi estudado o efeito dos probióticos sobre a presença de oocistos na cama das aves. Os antibióticos tiveram sua eficácia, como promotores de crescimento, comprovada para aves criadas sobre piso, mas não em bateria. Foram observadas alterações na estrutura da comunidade de Bacteria no trato intestinal de frangos criados em baterias e sobre piso e suplementados com antibióticos. Não houve evidência de efeito favorável dos probióticos sobre o desempenho e incidência de oocisto na cama das aves. Os probióticos não tiveram a capacidade de colonizar o epitélio intestinal de frangos de corte. Foram observadas discretas modificações na estrutura da comunidade de Bacteria de frangos criados em bateria e sobre piso e suplementados com dietas contendo probióticos. A estrutura da comunidade de Bacteria do intestino delgado de frangos foi modificada em função do ambiente no qual frangos, suplementados com probióticos e antibióticos, foram criados. Frangos isentos de qualquer tipo de promotor de crescimento apresentaram 15 unidades taxonômicas operacionais distintas na microbiota intestinal aderida ao epitélio, predominantemente Lactobacillus e Pseudomonas. / This study was conducted to evaluated the effects of growth promoter probiotic and antibiotics on the perfomance and organ morphometry of broiler chickens raised in floor pens and in batteries and the ability of the additive to promote changes in the intestinal ecosystem. Additionally, the effect of probiotics on the presence of coccidia oocysts in the litter was evaluated. The efficacy of antibiotics as growth promoters was observed when the chickens were raised in floor pens but not in batteries. Antibiotic supplementation caused changes in the structure of Bacteria community of the intestinal tract of chicken raised in floor pens or in batteries. The probiotic additives tested did not result in improvement in performance in both environmental or in oocyst incidence in the litter. Also, the probiotic did not have the ability to colonize the intestinal epithelium of the birds. Discrete changes in the structure of Bacteria community were observed when probiotics were supplemented to chickens raised in floor pens or in batteries. Bacteria community structure in the small intestine of chicken was modified as a function of the environmental in which the birds were raised. Chicken fed diets devoid of growth promoters had 15 distinct phylogenetic groups in the microbiota adhered to the intestinal epithelium.

Contexte : La malnutrition aiguë sévère (MAS) est un problème majeur de santé publique dans lemonde et affecte 17 millions d’enfants âgés de moins de 5 ans. La MAS induit une perte de poidsrapide, souvent associée à une dysfonction entérique environnementale (DEE). La DEE se caractérisepar une augmentation de l’inflammation, de la perméabilité intestinale, une diminution de la taille des villosités et de l’absorption des nutriments. La DEE peut ainsi limiter l’efficacité des protocolesde stabilisation et de re-nutrition chez l’enfant dénutri.L’objectif de cette thèse a été de développer un modèle de dénutrition avec entéropathie pour évaluer des laits thérapeutiques enrichis en nutriments ciblant la fonction de barrière intestinale.Résultats : Lors de la phase de développement du modèle, plusieurs approches ont été testées comme la restriction calorique, le régime hypoprotéiné, l’utilisation de lipopolysaccharides et de l’indométacine. Seule la combinaison d’un gavage quotidien d’indométacine pendant une semaine chez des souris dénutries par un régime pauvre en protéines a permis de développer un retard de croissance associé à une entéropathie. Suite à la validation de ce modèle, nous avons évalué la supplémentation du lait thérapeutique F-75 avec de la glutamine, de la leucine, de la gomme arabique et des levures séléniées. La glutamine et la leucine améliorent la fonction de barrière intestinale. Dans nos conditions expérimentales, l’enrichissement du lait thérapeutique avec la glutamine et la leucine combinés a eu des effets limités sur la fonction de barrière. La gomme arabique et les levures séléniées ont des propriétés prébiotiques et probiotiques et exercent des effets bénéfiques sur la barrière intestinale. L’enrichissement du lait thérapeutique avec l’association de ces deux composés permet d’inhiber l’inflammation intestinale et d’augmenter l’abondance de bactéries bénéfiques comme Faecalibacterium prausnitzii.Conclusion : Les travaux réalisés au cours de cette thèse ont permis de développer un nouveau modèle de dénutrition avec entéropathie. Le lait thérapeutique enrichi en gomme arabique et levures séléniées exerce des effets bénéfiques sur la fonction de barrière intestinale dans notre modèle. / Background : Severe acute malnutrition (SAM) is a global health issue affecting 17 million children under the age of 5. SAM induces rapid weight loss and is often associated with environmental enteric dysfunction (EED). EED is characterized by intestinal hyperpermeability and inflammation, villus blunting and nutrient malabsorption. EED might, therefore, limit stabilization and re-nutrition protocol efficacy. Objectives : This thesis aimed to develop an undernutrition model with enteropathy to evaluate the effect of a therapeutic milk enriched with nutrients on gut barrier function. Results : During preclinical model development, several approaches were tested: calorie restriction, low-protein diet, use of lipopolysaccharides and indomethacin. Only daily indomethacin gavage during one week in protein-energy undernourished mice induced growth faltering associated with enteropathy. After preclinical model validation, we evaluated the effect of therapeutic milk supplemented with glutamine, leucine, gum arabic and/or selenium-enriched yeast on gut barrier function. Glutamine and leucine induce beneficial effects on gut barrier function. In ourexperimental conditions, therapeutic milk enriched with a combination of glutamine and leucine had a limited impact on this parameter. Gum arabic and selenium-enriched yeasts have prebiotic and probiotic properties on gut barrier function. Therapeutic milk supplemented with gum arabic and selenium-enriched yeast inhibited intestinal inflammation and enhanced specific bacteria abundance such as Faecalibacterium prausnitzii.Conclusion : The studies conducted during this thesis permitted to develop a new model of undernutrition with enteropathy. Therapeutic milk enriched with arabic gum and selenium-enriched yeast triggered beneficial effects on gut barrier function in our preclinical model.

Background: Diet may play an important role in the management of patients with short bowel syndrome who have colon in continuity. However, macronutrient absorption has not been well characterized, and the most appropriate dietary constituents have not been well defined. Objective: To define carbohydrate absorption characteristics in patients with short bowel syndrome and determine the potential role of pectin as a dietary substrate. Methods: The authors studied the effect of a custom pectin-based supplement in 6 subjects (3 male/3 female) aged 29-67 years with jejunocolonic anastomosis, 4 of whom required long-term parental nutrition. Small intestinal absorption capacity, macronutrient and fluid balance, gastrointestinal transit time, and energy consumption were measured. Results: Data showed that 53% nitrogen, 50% fat, and 32% total energy were malabsorbed. In contrast, the majority (92%) of total carbohydrate was utilized. Fecal short-chain fatty acids (SCFAs) were increased, an indication of increased fermentation. Although only 4% of starch was recovered in stool, it is indicative of considerable starch malabsorption, thus providing the main carbohydrate substrate, for colonic bacterial fermentation. In contrast, non-starch polysaccharide was a relatively minor fermentation substrate with only 49% utilized. Eighty percent of the pectin was fermented. Supplementation was associated with increased total SCFAs, acetate, and propionate excretion. There was a trend observed toward greater fluid absorption (-5.9% ± 25.2%) following pectin supplementation. Nonsignificant increases in gastric emptying time and orocolonic transit time were observed. Conclusion : Despite malabsorption, starch is the primary carbohydrate substrate for colonic bacterial fermentation in patients with short bowel syndrome, although soluble fiber intake also enhances colonic SCFA production.
