An exemplar-based approach to search-assisted computer-aided diagnosis of pigmented skin lesions

Zhou, Zhen Hao (Howard) 15 November 2010 (has links)
Over the years, exemplar-based methods have yielded significant improvements over their model-based counterparts in image synthesis applications. Notably, texture synthesis algorithms using an exemplar-based approach have shown success where traditional stochastic methods failed. As an illustrative example, I will present an exemplar-based approach that yields substantial benefits for user-guided terrain synthesis using Digital Elevation Models (DEMs). This success is realized through exploitation of structural properties of natural terrain. In addition to their proliferation in the image synthesis domain, as annotated image datasets become increasingly available, exemplar-based methods are also gaining in popularity for image analysis applications. This thesis addresses the intersection between exemplar-based analysis and the problem of content-based image retrieval (CBIR). A basic problem in CBIR is the process by which the search criteria are refined by the user through the manipulation of returned exemplars. Exemplar-based analysis is particularly well-suited to query refinement due to its interpretability and the ease with which it can be incorporated into an interactive system. I investigate this connection in the domain of Computer-Assisted Diagnosis (CAD) of dermatological images. I demonstrate that exemplar-based approaches in CBIR can be effective for diagnosing pigmented skin lesions (PSLs). I will present an exemplar-based algorithm for segmenting PSLs in dermatoscopic images. In addition, I will present a generalized representation of dermoscopic features for detection and matching. This representation not only leads to an exemplar-based PSL diagnosis scheme, but it also enables us to realize interactive region-of-interest retrieval, which includes a relevance feedback mechanism to facilitate more flexible query-by-example analysis. Finally, I will assess the benefit of this CBIR-CAD approach through both quantitative evaluations and user studies.

Cervical intraepithelial lesions and integration of human papillomavirus / Intraepiteliniai gimdos kaklelio pokyčiai ir žmogaus papilomos viruso integracija

Šepetienė, Agnė 07 March 2011 (has links)
This dissertation observes the association between human papillomavirus (HPV) integration into the host cell genome and cervical intraepithelial lesions. The aim of this study is to determine how the grade of HPV16 DNA integration into the host cell genome (HPV E2 gene deletion) is related to cervical intraepithelial lesions. 253 women were screened for HPV infection and the majority was diagnosed with HPV type 16. The most frequently determined was HPV grade II integration. No statistically significant difference was defined while analyzing the relations between the status of HPV E2 gene integration and the grade of cervical intraepithelial lesions. The fact that integration of HPV type 16 was determined in low grade cervical squamous intraepithelial lesions or in cases with no intraepithelial lesions shows that virus integration occurs at the early stage of carcinogenesis. It is noteworthy that during the follow-up (secondary visit after 6 month) 50% of HPV positive women were identified with mRNR expression which probably establishes the presence of persistent active HPV infection. The research provided is highly significant for improvement of cervical cancer screening programms. Adjusting Pap smear, HPV DNA detection and determination of other HPV biomarkers (such as E2 gene deletion mRNR), it becomes possible to separate out HPV positive women with no clinical signs, although, belonging to the of high risk group for cervical carcinoma developing.

Optical coherence tomography : technology enhancements and novel applications

Silva, K.K.M. Buddhika Dilusha January 2004 (has links)
In the last fifteen years, a great deal of effort has been put forth, worldwide, for investigating and enhancing various aspects of optical coherence tomography (OCT). This thesis begins with a description of the technique of OCT, and an analysis of its underlying theory. The design and construction of an OCT system is described, with particular emphasis on a novel delay scanning method, and novel signal processing. Application of OCT to non-destructive characterisation of seeds, examination of skin lesions, measurement of fluid flow, and refractive index determination, are then demonstrated. Two technological enhancements to OCT are presented in this thesis. The first, an extended-range Fourier domain optical delay line (FDODL), extends the scan range of the traditional FDODL by a factor of almost 9, by scanning the galvanometer mirror around the region of zero tilt-angle. Polarisation optics are used to prevent light coupling back into the interferometer after only a single pass through the FDODL. A non-coplanar version of the FDODL is also presented, which overcomes the losses associated with the polarisation-based design, but trades off scan range to do so. Both versions of FDODL demonstrated excellent linearity and scan uniformity. The second technology presented here, bifocal optical coherence refractometry (BOCR), affords OCT the ability to measure refractive indices within turbid media. It achieves this by generating two confocal gates within the sample. From knowledge of the system parameters, and measurements of the confocal gate separation, the refractive index within the medium is evaluated to within ±0.01. Refractive index mapping is then demonstrated in a number of turbid samples. Three other applications of OCT are also demonstrated in this thesis. The first is the use of OCT to measure full thickness in lupin seeds. Although OCT could not penetrate the entire thickness of the hull, it is demonstrated that the variation in thickness of the two layers observed with OCT, explained 81% of the variation in thickness of the entire hull measured under a SEM. OCT was then applied, for what is believed to be the first time, in a large scale seed screening program. The second application is a preliminary investigation of the suitability of OCT to aid in the diagnosis of skin lesions. Although our system did not possess sufficient positioning accuracy to enable a direct one-to-one comparison between OCT and histology, a number of correspondences between OCT and histology images were demonstrated. The final application of OCT demonstrated here is a novel phase-locked-loop based demodulation scheme, to perform Doppler OCT. This demodulation scheme demonstrated a dynamic range of 98dB, a velocity range of ±20mm/s, and velocity resolution of 0.5mm/s. Using this system, laminar flow was demonstrated in milk flowing through a capillary tube.

LESÕES DO SISTEMA URINÁRIO EM CÃES / LESIONS OF THE URINARY SYSTEM IN DOGS

Inkelmann, Maria Andréia 16 March 2012 (has links)
Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / This thesis was composed of three scientific papers. The first paper was the more comprehensive. All lesions of the urinary system of dogs necropsied between 1999 and 2010 were analyzed. In this time frame, 3,189 dogs were necropsied at the Laboratório de Patologia Veterinária of the Universidade Federal de Santa Maria (LPV-UFSM) and 1,063 had lesions in the urinary system. In most of the dogs (79.1%), lesions were single and in about 21% they were multiple, totalizing 1,373 lesions. Kidneys were 2.8 times more affected than the lower urinary tract (LUT). One third of the lesions in the urinary system were cause of spontaneous death or reason for euthanasia (SD/EUTH) of the affected dogs. The other lesions were considered incidental findings. The main renal lesions diagnosed were: tubulointerstitial nephritis, infarct, granulomatous nephritis, glomerulonephritis, metastatic/multicentric neoplasms, pyelonephritis/pyelitis, and hydronephrosis. The main LUT lesions were: cystitis, presence of viral inclusions bodies, urolithiasis, urinary bladder dilatation, urinary bladder rupture, and metastatic/multicentric neoplasms. Epidemiological aspects such as gender, breed, and age of affected dogs had significant variations according to the type of lesion diagnosed. Uremia was observed in a significant number of cases of SD/EUTH and was mostly due to renal lesions. The second paper was focused on urolithiasis. From 1990 to 2010, 4,872 dogs were necropsied at LPV-UFSM. Out of these, 1.5% had uroliths along the urinary tract. The epidemiological profile of the affected dogs showed predominance of males; adults; and pure breeds. Clinical signs suggestive of urolithiasis were informed in 30.3% of the dogs and consisted mainly of hematuria, anuria, dysuria and urinary incontinency. The uroliths were found in one or more anatomical sites, and the main affected ones, in descending order, were urinary bladder, kidney, and urethra. Secondary lesions to urolithiasis were observed in about 40% of the cases. The most prevalent were cystitis, urethral obstruction, hydroureter, hydronephrosis, urinary bladder rupture, and pyelonephritis. In 25% of the affected dogs, SD/EUTH occurred due to the secondary lesions of urolithiasis. Extra-renal lesions of uremia were observed in 9 out of the 76 cases. The third article was focused on urinary system neoplasms of dogs. The main purposes of this study were to establish the prevalence and types of primary and metastatic/multicentric neoplasms of the urinary system in dogs submitted to necropsy in about 21 years at the LPV-UFSM. Neoplasms of the urinary system were present in 113 (2.4%) dogs. Twenty seven were primary neoplasms and 86 were metastatic or part of a multicentric tumor. The majority of the primary neoplasms were of epithelial origin. Thirteen dogs had primary renal neoplasms (prevalence of 0.27% over all dogs necropsied). Cystadenocarcinoma/cystadenoma and renal cell carcinoma were the most prevalent primary renal neoplasms and transitional cell carcinoma was the most prevalent urinary bladder neoplasm. Metastatic or multicentric tumors affecting the urinary system were the most prevalent (76.1%); mesenchymal tumors were more common than epithelial tumors; most of them were localized in the kidney. Metastases of mammary tumors and multicentric lymphoma were the most prevalent histologic types. O segundo artigo enfocou a urolitíase.

The purpose of this randomized split-mouth clinical study was to longitudinally evaluate the effectiveness of composite resin restorations of non-carious cervical lesions performed by direct and semi-direct techniques. A total of 30 volunteers with a need for class V cervical restorations were selected. Each patient received two restorations, one performed by the direct technique and the other by the directindirect technique, totaling 60 restorations. Assessment at baseline, 7 days, 6, 12 and 24 months, we performed using the modified USPHS criteria. Each patient was evaluated at return by two calibrated examiners. Data analysis was performed through descriptive statistics analysis using percentage of success of the restorations according to the criteria studied and scores obtained. For inferential analysis, the Student T test was used to evaluate the differences between extension, depth and time. Chi-Square/Fisher tests were used to compare rates between groups after each period (p <0.05). The results were evaluated by survival and annual failure rates (Kaplan-Meier). Differences were detected regarding to time, in which direct and direct-indirect procedures last 21.8(± 14.50) and 35.3 (± 19.89) minutes, respectively. Of the 60 restorations performed, 1 direct restoration was lost due to retention failure and no patient was missing the return in 7 days. At 6 months, 4 restorations were lost due to lack of retention in the direct technique and 5 restorations were lost for the same reason in the direct-indirect technique. Only 1 patient was absent. At 12 months return, 2 restorations were lost and 2 patients were absent in both techniques. And at 24 months, 2 patients did not attend the return and no restoration failed by the retention criterion by the direct technique. In the direct-indirect technique, 1 patient was absent and 1 restoration was lost due to lack of retention. The cumulative success of the direct technique was 99%, 93.1%, 88.5% and 88.5% in the 7-day, 6th, 12th and 24thmonths periods respectively. In the direct-indirect technique, cumulative success was 100%, 92.8%, 88.4% and 83.7% in the 7-day, 6, 12 and 24-month periods, respectively. It is concluded that, regardless of the technique employed, both are good restorative options and have high cumulative success rates in the studied periods. Working time was longer in the direct-indirect technique than in the direct technique. There was reduction of dental sensitivity in both techniques. There was gingival trauma immediately after the restorative procedures performed by the two techniques, with trauma reduction with the along the evaluated periods.

Sesterhenn, Renata January 2013 (has links)
Due to changes in the husbandry process on an industrial scale, associated with the expansion of the poultry sector in the areas of genetics, nutrition and health, the skin lesions have become increasingly important. At present, the major diseases that affect the skin in broilers are cellulite, fowl pox, the keratoacantoma (KA) or dermal squamous cell carcinoma (DSCC) and nonspecific dermatitis, the latter generally associated with trauma. The classification of skin diseases is inaccurate, since the inspection tend to group these skin lesions in carcasses of broilers in a single category called "dermatosis". The ulcerative skin lesions of broilers are common causes of total or partial condemnation of carcasses in slaughterhouses. The keratoacantoma is one of the most frequent causes of these injuries, and of unknown etiology. On the other hand, ulcerative lesions macroscopically similar to the keratoacantoma and granulomas caused by foreign body, possibly of plant origin, has been observed in some birds slaughtered in slaughterhouse in Rio Grande do Sul (Brazil). Based on these premises, the aim of this study was to histologically evaluate and analyze the epidemiological factors involved in the appearance of ulcerative skin: keratoacantoma and foreign body granulomatous dermatitis (FBGD) in broilers slaughtered in slaughterhouses under federal and state inspection. 363 samples were analyzed with ulcerative lesions, a total of 1,373,989 poultry displayed. Of these, 169 carcasses were diagnosed as keratoacantoma, representing a prevalence of 0.012% and 210 carcasses were diagnosed as granulomatous dermatitis foreign body, representing 0.015%. The prevalence founded in the present work to KA is within the range found by authors in previous works, since the prevalence DSCC was higher than that reported in 2004. There was a higher prevalence of KA in slaughter plants under federal inspection and more cases of DSCC in slaughter plants under state inspection. The most frequent location of ulcers was the back and chest for keratoacantoma for DSCC. Ulcerative lesions disseminated in the housing 100% were diagnosed as keratoacanthoma. Grossly, the lesions can be both unique diagnostic for keratoacanthoma as granulomatous dermatitis, requiring histopathology for definitive diagnosis. With this work, it was concluded that new skin lesions should be analyzed, to increase the sampling of specimens, decreasing the variability among the predisposing skin lesions.
