The Effects of Massage on Perceived Physical Soreness, Pain and Markers of Inflammation Following High Intensity Unaccustomed Exercise

Massage is often recommended to athletes to facilitate recovery and attenuate DOMS. The purpose of this study was to investigate the effects of massage on perceived muscle soreness and pain, inflammatory and immune markers, ROM, and mood state. Fourteen, recreationally active, women participated in a randomized crossover design study, consisting of 1) 60 min. full body massage following unaccustomed exercise and 2) 60 min. of rest. following unaccustomed exercise. Perceived muscle soreness and pain, active range of motion (ROM), mood state, along with blood concentrations of interleukin-6 (IL-6), C-reactive protein (CRP), creatine kinase (CK), and neutrophil count (NC), was assessed at baseline, 4hrs, and 24hrs following both treatment and control conditions. The aims of this study were 1) to decrease the effects of delayed onset muscle soreness (DOMS), and increase time to recover, and 2) to investigate the effect of massage vs. passive rest on inflammatory and immune markers within the blood. We hypothesized 1) an increase in ROM, a decrease in perceived physical soreness and perceived physical pain, as a result of the massage, compared to control, and 2) a decrease in blood plasma inflammatory markers, CRP, NC, CK, and IL-6, as a result of the massage, compared to control. We found massage following exercise to 1) significantly decreased perceived pain (p=0.001), 2) significantly increased immune iv markers (WBC (p=0.012) and NC (p=0.012)), and 3) significantly decreased ROM (p=0.02), compared to control. Massage had no impact on inflammatory markers (IL-6, CRP, and CK), or mood.

Development of a Mechatronics Instrument Assisted Soft Tissue Mobilization (IASTM) Device to Quantify Force and Orientation Angles

Indiana University-Purdue University Indianapolis (IUPUI) / Instrument assisted soft tissue mobilization (IASTM) is a form of massage using rigid manufactured or cast devices. The delivered force, which is a critical parameter in massage during IASTM, has not been measured or standardized for most clinical practices. In addition to the force, the angle of treatment and frequency play an important role during IASTM. As a result, there is a strong need to characterize the delivered force to a patient, angle of treatment, and stroke frequency. This thesis proposes two novel mechatronic designs for a specific instrument from Graston Technique(Model GT3), which is a frequently used tool to clinically deliver localize pressure to the soft tissue. The first design is based on compression load cells, where 4-load cells are used to measure the force components in three-dimensional space. The second design uses a 3D load cell, which can measure all three force components force simultaneously. Both designs are implemented with IMUduino microcontroller chips which can also measure tool orientation angles and provide computed stroke frequency. Both designs, which were created using Creo CAD platform, were also analyzed thorough strength and integrity using the finite element analysis package ANSYS. Once the static analysis was completed, a dynamic model was created for the first design to simulate IASTM practice using the GT-3 tool. The deformation and stress on skin were measured after applying force with the GT-3 tool. Additionally, the relationship between skin stress and the load cell measurements has been investigated. The second design of the mechatronic IASTM tool was validated for force measurements using an electronic plate scale that provided the baseline force values to compare with the applied force values measured by the tool. The load cell measurements and the scale readings were found to be in agreement within the expected degree of accuracy. The stroke frequency was computed using the force data and determining the peaks during force application. The orientation angles were obtained from the built-in sensors in the microchip.

RYTMISK INSMÖRJNING ENLIGT WEGMAN/HAUSCHKA – en intervjustudie om patienters upplevelser av beröringsterapi i öppenvård

Bakgrund: Rytmisk insmörjning enligt Wegman/Hauschka (RI) är en beröringsterapi inom antroposofisk vård. RI utförs av sjuksköterskor inom landstingsfinansierad integrativ vård. Patienter får RI i primärvård, öppenvård och slutenvård. Det centrala i den antroposofiska synen på kropp, hälsa och lidande överrensstämmer i stor utsträckning med den vårdvetenskapliga livsvärldsteorin. Syfte: Att belysa hur patienter som fått eller får RI i öppenvård upplevt att behandlingen har påverkat dem. Metod: Kvalitativ innehållsanalys enligt Lundman och Hällgren Graneheim (2012) baserad på semistrukturerade intervjuer med fem patienter.   Resultat: Patienter som fått RI i öppenvård upplevde att deras hälsa förbättrades kroppsligt, själsligt och andligt. Kroppsligt lidande som smärta, nedsatt lungfunktion, tarmbesvär och sömnsvårigheter förbättrades. Själsligt och existentiellt lidande minskade i form av förbättrade kognitiva funktioner och ökad möjlighet att känna avslappning, avgränsning, mening, hopp, bekräftelse, trygghet samt livskraft. Slutsats: I denna studie på patienter i öppenvård liksom i tidigare studier på patienter i slutenvård upplevs RI kunna stärka hälsan kroppsligt, själsligt och existentiellt. RI är en vårdaktivitet där patienten bemöts utifrån ett helhetsperspektiv och vårdvetenskaplig teori omsätts i praktik. Större, jämförande studier med till exempel kontrollgrupp utan behandling eller med läkemedelsbehandling eller samtalsterapi liksom studier över tid är nödvändigt innan RI kan integreras i större utsträckning inom dagens hälso- och sjukvård. / Background: Rhythmical Einreibungen according to Wegman/Hauschka (RE) is a soft massage therapy for use in medicine and nursing extended through anthroposophy. RE is practiced by nurses working with integrative health care in the public financed health system, both with inpatients and outpatients (e.g. in primary care). The ontological definitions of caring science grounded in lifeworld theory of the human body, health and suffering is to a high extent corresponding with anthroposophic medicine. Aim: To explore how outpatients receiving RE have experienced that the therapy have affected them. Method: A quality content analysis, according to Lundman and Hällgren Graneheim (2012), based on semi structured interviews with five patients. Results: A decrease in physical suffering such as pain, reduced lung capacity, IBS and sleeping disorders was experienced. Psychological and spiritual improvement was experienced in cognitive functions, sense of relaxation, meaning, hope, confirmation, security and vitality. Improved health was experienced despite severe illness. Conclusion: RE is a therapy where caring science seems to be put in practice. This study on outpatients receiving RE indicates that it may improve health physically, psychologically and spiritually. The results match with earlier studies on inpatients receiving RE. Further studies, e.g. in comparison with psychotherapy or medical treatments, would be of great interest.

Title: The effect of foam roller on muscle tone after physical load Objectives: The aim of this thesis was to summarize theoretical knowledge about muscle, changes in muscle tension and possibilities of its affect. The aim of the practical part was to find out the effect of the foam roller on muscle tone after exercise. Methods: Practical part of this thesis was processed in biomechanical laboratory at FTVS UK in Prague and included 16 probands. We measured muscle tone of biceps brachii, m. triceps surae and m. quadriceps femoris before exercise, then the probands performed an activity aimed at increasing the muscular tension of the musculature mentioned above, after which we measured the muscle tension again. The foam rolling of each muscle group lasted for 60 seconds and then we measured the muscle tone for the last time. A myotonometer device was used and the data were transmitted by KISTLER MULTICHANNEL CHARGE AMPLIFIER and processed by the DeweSoftX3 (64 bit) programme in SIRIUS DEWEsoft S-BOX2 computer. At the end, we compared measured values with subjective sensations of the probands who filled quick questionnaire. It is an investigative study of relationship mother-baby. The bibliography compiled has suggested that the mother or the individual taking on the mother´s role has a key role in the process of falling asleep and maintaining baby´s sleep. Simultaneously, having a good and restful night sleep was something much valued and attempted by the parents. By applying the technique of change of sleep habits, which has been developed by the author, this study analyses three pairs of mother-baby basing its findings on the Jungian theory. The objective of the study has been achieved since the technique has proved to be efficient when stipulating different intervals of waiting time, according to psychological resources characterized by each phase of the human development. By providing an exclusive time for the mother-baby interaction through giving the baby a body massage, the baby was offered different experiences, both of primary bond and separation / O objetivo deste trabalho é verificar as possibilidades da aplicação de técnica de modificação dos hábitos de sono, em crianças com dificuldade para dormir, em um estudo investigativo da relação mãe-filho. A bibliografia estudada sugeriu que, a mãe ou a pessoa que faz o papel de mãe exerce papel fundamental no processo de adormecer e manter o sono do bebê, ao mesmo tempo em que para os pais, poder ter uma noite de sono ininterrupta era algo valorizado e buscado. Foi realizada uma pesquisa qualitativa com dez docentes, atuantes nos cursos: Técnico em Enfermagem, Técnico em Estética, Técnico em Massoterapia, Técnico em Podologia e Técnico em Segurança do Trabalho, da unidade Jundiaí do Serviço Nacional de Aprendizagem Comercial SENAC (São Paulo), em duas etapas distintas: primeiramente, por meio de entrevistas individuais e, posteriormente, por meio de dois encontros coletivos psicodramáticos, com a presença de seis dos dez participantes, em cada um dos encontros. O quadro teórico está fundamentado nas produções de: Placco e Souza (2006), Imbernón (2011), Tardif (2002), Freire (1983, 2002), Barato (2003, 2004), Shulman (1987) e Moreno (1975, 2008). Os dados nos possibilitam analisar os processos de socialização primária e profissional dos docentes, identificando a constituição da profissionalidade docente como o desenvolvimento do exercício docente, in loco e na relação aluno-docente, de modo processual, gradual e infindável, baseada nas experiências pessoais, escolares e profissionais dos docentes. Outro resultado significativo, na pesquisa, é a relevância da interdependência dos saberes docentes, especialmente os profissionais-experienciais, os pedagógicos, os pessoais e os do profetizar, que se imbricam no desenvolvimento da profissionalidade. Embora não se reconhecendo como profissionais docentes, as falas dos pesquisados nos permitem afirmar que há um desenvolvimento dessa profissionalidade e dos saberes docentes pedagógicos, com o compromisso e a intencionalidade explícitos com a aprendizagem dos alunos e da docência, com o exercício profissional Considering the gap in the literature on vocational education as a field of study, this research aims to investigate the construction of professionality and teaching knowledge in the technical domain of Environment, Health and Safety - fields of Health and Wellness. A qualitative survey was conducted with 10 teachers of the following courses: Nursing Technician, Esthetics Technician, Massage Therapy Technician, Podiatry Technician, and Occupational Safety Technician from the Jundiaí unit of Serviço Nacional de Aprendizagem Comercial (Senac São Paulo), in two distinct stages first, through individual interviews and later, through two group psychodramatic encounters, with six participants per encounter. The theoretical framework is based on the works of: Placco e Souza (2006), Imbernón (2011), Tardif (2002), Freire (1983, 2002), Barato (2003, 2004), Shulman (1987), and Moreno (1975, 2008). The data allowed us to analyze the teachers primary and professional socialization processes and identify the construction of teaching professionality as the development of the teaching practice, both on site and in the student-teacher relation, in a gradual, never-ending process, based on the teachers personal, school, and professional experiences. Another significant result of the research is the relevance of the interdependence of teaching knowledge - especially professional/experiential, educational, personal, and prophetic skills, which interweave in the development of professionality. Although the respondents do not identify themselves as teaching professionals, their reports allow us to state that the educational professionality and teaching knowledge are developed with an explicit commitment and intentionality to the students learning and education, with professional practice and with the enhancement of their educational skills. 中醫醫療行為法律規制之研究-以中醫推拿助理為中心 / A Study on the Legal Regulation of Chinese Medical Practices-Focus on the Chinese Massage Assistants

中醫按摩源起於遠古時代,早於殷商甲骨文卽有按摩輔助人員之記載,春秋戰國名醫扁鵲併用弟子按摩搶救瀕死患者,戰國秦漢時期《黃帝內經》並以「按龜之法」作為按摩從業人員考覈取材標準,唐朝太醫署按摩科設立官方按摩博士及助理制度;說明中醫按摩法制早已完備。雖自明朝起「按摩」始有「推拿」之稱,清朝並分為十二流派,但按摩推拿法制仍能綿衍不絕。   然正當亞洲各國紛紛建立中醫推拿業務法制並重視第三醫學之際,我國行政主管機關卻仍在原地踏步、裹足不前。造成「真正從事中醫推拿助理業務者,無正規學歷與證照考覈管理制度;真正合法醫事人員,卻不從事中醫推拿助理業務且無相關法律配套」之窘境。 綜觀我國憲法、大法官解釋及相關法律規範,並未明文限制人民從事中醫推拿助理之業務。故在就業平等原則下,人民當具有自由選擇為職業之工作權、具有平等接受就業服務及不受歧視之就業與執業機會平等。且各種療法(包括民俗療法)皆存在著風險,甚至已關乎人命問題;是以不論各種療法之風險高低,均應有在職訓練及證照考覈制度管制、相關從業人員身份立法,不應有差別待遇或階級區分。依釋字第666號葉百修大法官協同意見書解釋之精神觀之,行政主管機關應有效提出配套措施、在職訓練及證照考覈制度,實不得以中醫推拿助理所執行之業務非正統醫療、有害社會或不具社會價值為理由,禁止其特定業務活動,使人民得從事該活動之自由『排除於職業自由的保障範圍之外』,藉此淘空憲法所保障中醫推拿助理之工作權。 據此,本論文蒐集歸納歷代與當代中醫按摩推拿歷史及法制文獻;比較當前亞洲國家推拿業務法制與英德日中職業證照制度;分析憲法及現行法秩序下,中醫推拿助理職業自由之保障與限制;並從歷年法院相關實務見解、衛生署行政函釋爭議、立法院質詢改革要點,彙整並比較當前司法、行政及立法機關之見解與措施,並對利弊得失作出評析。最後,提出個人自擬「中醫推拿技術士考覈與管理辦法草案」、「中醫推拿師法草案」及短中長期具體建議,作為未來中醫推拿法制與助理考覈管理改革之重要參考方針。 關鍵字:中醫推拿助理、工作權、職業自由、證照考覈制度 / The Chinese massage originated in ancient times. At the Shang dynasty, the oracle- bone scriptures had already recorded a number of massage assistance personnel . At the Eastern Zhou dynasty, a famous country Chinese herb doctor "pian-chiueh" who rescued the dying with massage therapy by disciples . At the Warring States Period and Qin-Han dynasties , " Laws of pressing the turtle " as the massage employee's standard examination had already described by "Huangdi Neijing(Yellow Emperor's Canon of Medicine)". At the Tang dynasty , imperial government established the official massage doctor and assistant systems . So that Chinese massage system had long been a complete system since then . Althrough "massage" started "tueinar" of call since Ming dynasty, and which divided into twelve divisions at Qing dynasty; the systems of Chinese massage could be still transmitted for ever and ever. However, many Asian countries have established legitimate Chinese massage systems and paid much attention to the third medical science in succession ; but administrative organizations in our country still have hesitated whether to move forward or not.Therefore, those embarrassing situations caused a great deal of trouble to us that" those who engaged in Chinese massage assistants with no formal qualifications and licence examination systems; and those who engaged in legitimate medical personnel with no legal standards of Chinese massage". Taking a comprehensive view of our country Constitution , J .Y . interpretations and related laws,which don't explicitly limit any people engaged in traditional Chinese massage assistants. Therefore , under the principle of equality in employment , people have freely choice of jobs for the right to work as a career;whom have equal acceptance of employment service and free from discrimination of jobs. Various therapies(including folk remedies) all exist risks which even have been already close to human life problems. Based on a variety of treatments regardless of the level of risk,one should have job training, evaluation licence examination systems,the relevant legislation as employees;but should not be discrimination or class distinction. According to the coordination view of J .Y. interpretation NO. 666 by the Honorable Justice Pai- Hsiu Yeh , administrative organizations should put effective integration of on-the-job training and licence examination for Chinese massage assistants ; then those shouldn't forbid their particular business activity in which guaranteed by the Constitution by reason of non-medical, harmful or valueless to the society. Accordingly, this thesis summarized collection of ancient and contemporary history of Chinese massage and legal documents; compared the massage systems in a few Asian countries and occupational licence systems in England , Germany , Japan and China ; analyzed under the Constitution and existing legislation in order to comprehend guarantee and restriction of occupational freedoms for Chinese massage assistants . In addition , it made assessment of the advantages and disadvantages about the related views of judicial , executive and legislature's organs till now. Finally, it proposed the personal draft plans " ordinance draft for Chinese massage technician 's assessment and management " , " law draft for the Chinese massagist " and the short , medium , long term recommendations as important reform references for policy in the future.
