Kombination chemischer, gentechnischer und enzymatischer Methoden zur Darstellung schwer synthetisierbarer Proteine

Das fibrillen-bildende beta2-Mikroglobulin (b2M) und das CRF1-Mimetikum mit verzweigter Rückgratstruktur können als „schwierige“ Proteine betrachtet werden, deren Darstellung sich eignet, gegenwärtige Möglichkeiten und Grenzen der Proteinsynthese zu ermitteln. Die Proteine sollen zu spektroskopischen Untersuchungen von Proteinfaltung bzw. Ligand-Rezeptor- Wechselwirkungen eingesetzt werden. Versuche zur Chemosynthese von b2M über drei Segmente führten per NCL zwar zu linearen Produkten mit korrekter Primärstruktur, aber wiederholt wurden zwei, mittels HPLC trennbare Proteine erhalten, deren enzymatische Spaltung zu identischen Fragmenten führte. Eine Isomerisierung (wie z.B. Epimerisierung) als Ursache für die Bildung der zwei Produkte konnte ausgeschlossen werden. Mittels CD- und FTIR-Spektroskopie wurden für beide Produkte beta- Faltblatt-Strukturen ermittelt, die sich sowohl untereinander als auch vom rekombinanten Protein unterschieden. Die „fehlgefalteten“ Syntheseprodukte konnten nicht entfaltet und anschließend in die „korrekte“ Struktur des rekombinanten b2M überführt werden. Es ist denkbar, dass die beobachtete „Fehlfaltung“, deren Ursache nicht geklärt werden konnte, für vom b2M ausgelöste Amyloidosen verantwortlich ist. Das CRF1-Modell, das aus drei zyklischen Peptiden und einem Protein mit Disulfidbrücken besteht, welche auf einem linearen Peptid-Templat verankert sind, wurde durch ein zyklisches Templat zur strukturellen Einschränkung modifiziert. Durch das zyklische Templat ergaben sich keine Syntheseprobleme, aber interessanterweise führte die Zyklisierung des Templats zu einer signifikant höheren Affinität für den Agonisten Urocortin-I im funktionellen Assay. Darüber hinaus wurde gezeigt, dass ein zyklisches Rezeptor-Loop-Peptid mittels EPL im mg-Maßstab erhalten werden kann, was künftig die Synthese isotopenmarkierter Analoga für Struktur-Untersuchungen ermöglicht. / The fibril forming beta2-microglobulin (b2m) and the CRF1 mimic with branched peptide backbone could be considered as “difficult” proteins, whose synthesis is suited for determining present possibilities and limits of protein synthesis. The proteins shall be used for spectroscopic analysis of protein misfolding or ligand-receptor-interaction, respectively. Efforts of the chemosynthesis of b2m over three segments may lead via NCL to linear products with correct primary structure, but two, via HPLC isolatable proteins were repetitively susbstained, whose enzymatic digest lead to identical fragments. An isomerization (such as e. g. epimerization) as reason for the formation of the two products could be excluded. By means of CD and FTIR spectroscopy for both products beta-sheet structure were determined, which differ among themselves as well as from the recombinant protein. The “misfolded” synthetic product could not be unfolded und subsequently converted into the “correct” structure of the recombinant b2m. It is possible that the observed “misfolding”, whose cause could not be clarified, is reasonable for the amyloidosis induced by b2m. The CRF1 model that consists of three cyclic peptides and one protein with disulfid bridges coupled to a linear peptide template, was modified for structural constraints by a cyclic template. In consequence of the cyclic template no synthetic problems aroused, although the cyclisation of the template leads interestingly to a significant higher affinity for the antagonist urocortin-I in the functional assay. Furthermore, it was shown that a cyclic receptor loop peptide could be received via EPL in mg scale, what in future enables the synthesis of isotopically labeled analogs for structure investigations.

Protein Dynamics by Solid-State NMR with Ultra-Fast Magic-Angle Spinning : from Microcrystals to Amyloid Fibrils and Membrane Proteins / Dynamique des Protéines par RMN à l’Etat Solide avec Rotation Ultra Rapide à l’Angle Magique : des Microcristaux aux Fibrilles Amyloïdes et Protéines Membranaires

La Résonance Magnétique Nucléaire (RMN) à l’état solide avec rotation à l’angle magique (MAS) est une technique de choix pour l’étude de la structure et de la dynamique de molécules biologiques peu ou non solubles. Un grand nombre d’approches ont été développées pour la reconstitution de structures tridimensionelles à partir de mesures précises de proximités internucléaires, ainsi que pour la détection de mouvements moléculaires avec une résolution atomique sur des échelles de temps couvrant plusieurs ordres de grandeur. Malgré d’impressionnants progrès, les études par RMN MAS sont cependant loin d’être réalisées en routine. Les déterminations structurelles et de dynamique sont souvent démontrées sur des préparations microcristallines modèles, mais sont encore rares pour des systèmes plus complexes tels que les fibrilles amyloïdes non cristallines ou les protéines trans-membranaires insérées dans des bi- couches lipidiques. Mon travail a pour objectif d’étendre les possibilités de la RMN MAS pour l’étude de systèmes biomoléculaires complexes dans différents états d’agrégation. Pour cela, j’ai exploité les possibilités uniques offertes par les hauts champs magnétiques (fréquence de Larmor du 1H 700, 800 et 1000 MHz) combinés avec les sondes MAS de dernières générations capables d’atteindre des vitesses de rotations supérieures à 60 kHz. Ces conditions expérimentales per- mettent d’augmenter la sensibilité de la RMN MAS à l’aide de la détection 1H à haute résolution et d’enrichir la palette de paramètres RMN rapporteurs de la dynamique des protéines. La première partie de cette thèse décrit le développement de nouvelles stratégies pour l’attribution des résonances du squelette de protéines, pour l’élucidation de structures, et pour l’étude de la dynamique du squelette peptidique et des chaînes latérales. Les méthodes présentées réduisent significative- ment les besoins en termes de temps expérimental, de quantités d’échantillon et de marquage isotopique, et permettent d’analyser par RMN des systèmes de plus hauts poids moléculaire. La seconde partie décrit l’application de la RMN MAS avec détection en 1H pour l’évaluation du rôle de la dynamique des protéines dans des processus tels que la formation de fibrilles amyloïdes et le fonctionnement de protéines membranaires. Une première application est l’étude de la tendance de la β-2 microglobuline humaine à former des fibrilles amyloïdes. Une comparaison de la dynamique du squelette peptidique de la protéine sauvage et du mutant D76N dans leur forme cristalline, ainsi que la détermination de propriétés structurales de la forme fibrillaire m’ont permis d’identifier la présence de repliements pathologiques et de formuler des hypothèses sur le mécanisme de formation des fibrilles. Finalement, la dynamique locale et globale de protéines membranaires dans des bicouches lipidiques a été étudiée. En particulier, le mécanisme d’action d’un transporteur d’alkanes, AlkL, de P. putida a été examiné dans un environnement lipidique. La détermination de paramètres pour la dynamique rapide (ps-ns) et lente (μs-ms) du squelette peptidique de la protéine en présence ou en absence de substrat met en évidence des acheminements possibles pour le transfert de molécules vers la membrane et jette les bases pour une meilleure compréhension du processus. / Solid-state NMR with magic angle spinning (MAS) has emerged as a powerful technique for investigating structure and dynamics of insoluble or poorly soluble biomolecules. A number of approaches has been designed for reconstructing molecular structures from the accurate measurement of internuclear proximities, and for probing motions at atomic resolution over timescales spanning several orders of magnitude. Despite this impressive progress, however, MAS NMR studies are still far from routine. Complete determinations, which are often demonstrated on model microcrystalline preparations, are still rare when it comes to more complex systems such as non-crystalline amyloid fibrils or transmembrane proteins in lipid bilayers. My work aimed at extending the possibilities of MAS NMR for applications on complex biomolecular systems in different aggregation states. For this, I exploited the unique possibilities provided by high magnetic fields (700, 800 and 1000 MHz 1H Larmor frequency) in combination with the newest MAS probes capable of spinning rates exceeding 60 kHz. These experimental conditions al- low to boost the sensitivity of MAS NMR through 1H detection at high resolution and to enrich the palette of probes for protein dynamics. The first part of the thesis reports on my contribution to the development of new strategies for backbone resonance assignment, for structure elucidation, and for investigation of backbone and side-chain dynamics. These methodologies significantly reduce the requirements in terms of experimental time, sample quantities and isotopic labeling, and enlarge the molecular size of systems amenable to NMR analysis. The second part describes the application of 1H detected MAS NMR to evaluate the role of protein dynamics in problems such as amyloid fibril formation and membrane protein function. I first addressed the amyloid fibril formation propensity of human beta-2 microglobulin, the light chain of the major histocompatibility complex I. I performed comparative studies of backbone dynamics of the wild type protein as well as a D76N mutant in crystals, and determined some of the structural features of the fibrillar form. This allowed to identify the presence of pathological folding intermediates and to formulate hypotheses on the mechanism of fibrils formation. Finally, I studied the local and global dynamics of membrane proteins in lipid bilayers. In particular, I investigated the mechanism of action of the alkane trans- porter AlkL from P. putida in lipid bilayers. The measurement of parameters for fast (ps-ns) and slow (μs-ms) backbone dynamics of the protein in presence or in absence of a substrate highlights possible routes for molecular uptake and lays the basis for a more detailed mechanistic understanding of the process.

Impacto do reprocessamento de dialisadores de alto fluxo e alta eficiência sobre o transporte de solutos em sessões de hemodiafiltração online curta diária / Reprocessing high-flux, high-efficiency polysulfone dialyzers on solutes removal in short daily online hemodiafiltration

Introdução: Não há estudos que avaliem o impacto da reutilização do dialisador na remoção de solutos em sessões de hemodiafiltração online curta diária (HDF-OL-D). Objetivo: Avaliar o impacto do reuso do dialisador na dose do tratamento, na cinética de beta2-microglobulina e na extração de solutos em sessões de HDF-OL-D e comparar com sessões de hemodiálise curta diária de alto fluxo (HD-D). Métodos: Foram incluídos 14 pacientes do programa de HD-D. Foram coletadas amostras de sangue pré, no meio e pós-diálise e amostras do dialisato no 1º, 7º e 13º usos do dialisador em sessões de HD-D e de HDF-OL-D. Resultados: A massa total extraída (MTDQ) e a depuração (KDQ) diretamente quantificada dos solutos pequenos (ureia, fósforo, creatinina e ácido úrico), bem como a dose de diálise ofertada, foram semelhantes quando o 1º, 7º e 13º usos do dialisador em sessões de HD-D foram comparados, respectivamente, ao 1º, 7º e 13º usos em sessões de HDF-OL-D. A MTDQ e a KDQ dos solutos pequenos e a dose de diálise foram semelhantes entre os usos do dialisador tanto em sessões de HD-D e quanto em sessões de HDF-OL-D. A MTDQ, a KDQ e o Kt/V diretamente quantificado (Kt/VDQ) de beta2-microglobulina foram maiores em sessões de HDF-OL-D do que nos respectivos usos do dialisador em sessões de HD-D. Não houve diferença quanto à cinética de beta2-microglobulina, avaliada pela MTDQ, KDQ ou Kt/VDQ, entre o 1º, 7º e 13º usos do dialisador em sessões de HD-D nem em sessões de HDF-OL-D. Na HDF-OL-D, a perda intradialítica de albumina foi significativamente diferente entre os usos do dialisador (p < 0,001), estando reduzida no 7º e 13º usos, quando comparados ao 1º uso do dialisador. Não houve correlação entre o volume de enchimento do dialisador e a MTDQ e a KDQ de moléculas pequenas em nenhum dos métodos dialíticos estudados. Em sessões de HDF-OL-D, foi observada uma correlação fraca entre o volume de enchimento do dialisador e a KDQ de beta2-microglobulina, não percebida com a MTDQ ou o Kt/VDQ de beta2-microglobulina. Conclusão: O reprocessamento de dialisadores de alto fluxo e alta eficiência não resulta em comprometimento da dose do tratamento, da cinética de beta2-microglobulina nem da extração de solutos em sessões de HDF-OL-D. A extração de beta2-microglobulina foi maior em sessões de HDF-OL-D do que em sessões de HD-D, sem diferenças significativas na remoção dos demais solutos. O reprocessamento do filtro, em sessões de HDF-OL-D, resultou numa redução significativa da perda intradialítica de albumina. Não parece haver influência do volume de enchimento do filtro na extração de solutos em nenhum dos métodos de diálise estudados / Introduction: There no studies evaluating the impact of dialyzer reutilization on solute removal in daily online hemodiafiltration (D-OL-HDF) sessions. Objectives: Our aim was to evaluate the impact of dialyzer reuse on solute extraction, beta2-microglobulin kinetics and dialysis dose in D-OL-HDF and compare to those in high-flux short daily hemodialysis (D-HD). Methods: 14 patients undergoing a D-HD program were included. Pre, middle and post-dialysis blood samples and effluent dialysate were collected in the 1st, 7th and 13th dialyzer uses in D-HD sessions and in D-OL-HDF sessions. Results: Directly quantified small solute (urea, phosphorus, creatinine and uric acid) total mass removal (TMDQ) and clearance (KDQ), as well as dialysis dosis, were similar when the 1st, 7th and 13th dialyzer D-HD uses were compared to the 1st, 7th and 13th D-OL-HDF uses. TMDQ and KDQ of small solutes and dialysis dose were similar among dialyzer uses in D-HD sessions and also in D-OL-HDF sessions. beta2-microglobulin TMDQ, KDQ and directly quantified Kt/V (Kt/VDQ) were statistically higher in D-OL-HDF dialyzer uses than in the respective D-HD uses. There was no difference in beta2-microglobulin kinetics, evaluated by TMDQ, KDQ or Kt/VDQ, among 1st, 7th and 13th uses in D-OL-HDF sessions or in D-HD sessions. In D-OL-HDF, albumin loss was significantly different among studied dialyzer uses (p < 0.001), being reduced in the 7th and 13th dialyzer uses, when compared to the first use. There was no correlation between dialyzer priming volume and small molecules TMDQ, KDQ in neither analyzed dialytic method. In D-OL-HDF sessions, it was observed a week correlation between dialyzer priming volume and beta2-microglobulin KDQ, not observed with beta2-microglobulin MTDQ or Kt/VDQ. Conclusion: High flux, high efficiency dialyzer reprocessing did not did result in a reduction of the offered dialysis dose, beta2-microglobulin kinetics or solute extraction in D-OL-HDF. beta2-microglobulin removal was greater in D-OL-HDF than in D-HD sessions, without difference in other solutes extraction. There was a significant reduction in intradialytic albumin loss with dialyzer reprocessing in D-OL-HDF sessions. Dialyzer priming volume does not appear to influence solute removal in neither analyzed dialytic method

Avaliação dos biomarcadores urinários no controle do tratamento de estenose de junção ureteropélvica em adultos / The role of urinary biomarkers in the assessment of ureteropelvic junction obstruction in adults

INTRODUÇÃO E OBJETIVO: A estenose de junção ureteropélvica (EJUP) é importante causa de obstrução do trato urinário e pode levar a deterioração progressiva da função renal. Há espaço para o aprimoramento de novos métodos diagnósticos capazes de discriminar hidronefrose e uropatia obstrutiva. Acredita-se que os biomarcadores urinários podem fornecer indícios de lesão renal precoce na obstrução urinária. Neste contexto, KIM-1 pode elevar-se na urina por lesão tubular proximal, NGAL por lesão no túbulo proximal, distal ou alça de Henle, CA19-9 por produção excessiva no túbulo obstruído e ?2-microglobulina (beta2M) por injúria ao glomérulo ou ao túbulo proximal. O objetivo do presente estudo foi avaliar as propriedades diagnósticas dos biomarcadores urinários citados em adultos com EJUP, sendo o primeiro estudo na literatura a avaliar tais moléculas nesta população. MÉTODOS: Foram estudados de modo prospectivo pacientes consecutivos acima de 18 anos com diagnóstico de EJUP submetidos a pieloplastia videolaparoscópica de dezembro de 2013 a fevereiro de 2015. Foram excluídos do estudo pacientes com EJUP bilateral, rim contralateral patológico, EJUP em rim único, antecedentes de tratamento cirúrgico para estenose de JUP ou taxa de filtração glomerular inferior a 60 ml/min/1,73m2. Cada paciente forneceu quatro amostras de urina para medição de biomarcadores, uma no pré-operatório e outras com 1, 3 e 6 meses de seguimento pós-operatório. O grupo controle foi constituído por voluntários saudáveis sem hidronefrose à ultrassonografia. RESULTADOS: Foram incluídos 47 pacientes com idade média de 38,6 ± 12,7 anos (intervalo 19 a 64 anos), sendo 17 (36,2%) do sexo masculino e 30 (62,8%) do sexo feminino. O grupo controle foi composto por 40 indivíduos semelhantes ao grupo com EJUP no que concerne idade (p = 0,95) e sexo (p = 0,82). KIM-1 foi o marcador com melhores propriedades diagnósticas, apresentando área sob a curva (AUC) de 0,79 (95% CI 0,70 a 0,89). O NGAL, por sua vez, teve AUC de 0,71 (95% CI 0,61 a 0,83), CA19- 9 teve AUC de 0,70 (95% CI 0,60 a 0,81) e (beta2M) apresentou AUC de 0,61 (95% CI 0,50 a 0,73), sendo o único biomarcador com propriedades inadequadas neste cenário. O KIM-1 foi o marcador mais sensível com o ponto de corte 170,4 pg/mg de creatinina (sensibilidade 91,4%, especificidade 59,1%) e o CA 19-9 o mais específico para o ponto de corte de 51,3 U/mg de creatinina (sensibilidade 48,9%, especificidade 88,0%), enquanto o NGAL foi o que apresentou maior queda após desobstrução, com 90,0% dos pacientes apresentando clareamento superior a 50%. CONCLUSÕES: A avaliação dos biomarcadores urinários é útil no diagnóstico de obstrução em adultos com EJUP submetidos a pieloplastia videolaparoscópica. O KIM-1 foi o marcador mais sensível e o CA 19-9 o mais específico, enquanto o NGAL foi o que apresentou maior que com a desobstrução. Houve queda das concentrações dos marcadores após pieloplastia no período estudado. O papel exato dos biomarcadores urinários no cenário de obstrução em adultos deve ser mais amplamente investigado / INTRODUCTION AND OBJECTIVE: Ureteropelvic junction obstruction (UPJO) is an important cause of urinary tract obstruction and can lead to progressive deterioration of renal function. Thus the development of novel non-invasive methods capable of discriminating obstruction and hydronephrosis may be useful. Elevation of urinary biomarkers may provide early evidence of kidney damage in urinary obstruction. In this scenario, urinary concentrations of KIM-1 may be elevated following proximal tubular injury, while NGAL may increase as result of injury to proximal or distal tubule as well as to loop of Henle, CA19-9 after overproduction in the obstructed tubule and ?2 microglobulin (beta2M) after injury to the glomerulus or the proximal tubule. The aim of this study was to evaluate the diagnostic properties of these urinary biomarkers in adults with UPJO. METHODS: We prospectively studied consecutive patients older than 18 years diagnosed with UPJO undergoing laparoscopic pyeloplasty from December 2013 to February 2015 in our institution. Exclusion criteria included patients with bilateral UPJO, unilateral UPJO with contralateral pathologic kidney, solitary kidney, history of previous surgical treatment for UPJO or glomerular filtration rate below 60 ml/min/1,73m2. Each patient provided four voided urine samples for biomarker measurement, one at preoperative consultation and the others at 1, 3 and 6 months of postoperative follow-up. Healthy individuals with no hydronephrosis on ultrasound evaluation constituted our control group. RESULTS: We included 47 patients with a mean age of 38.6 ± 12.7 years (range 19-64 years), from which 17 (36.2%) were males and 30 (62.8%) were females. The control group consisted of 40 subjects with no statistical difference to the study group regarding age (p = 0.95) and gender (p = 0.82). KIM-1 had an area under the curve (AUC) of 0.79 (95% CI 0.70 to 0.89) and was the biomarker with the best diagnostic properties. CA19-9 had an AUC of 0.70 (95% CI 0.60 to 0.81), NGAL had an AUC of 0.71 (95% CI 0.61 to 0.83) and beta2M had an AUC of 0.61 (95% CI 0.50 to 0.73). KIM-1 was the most sensitive marker with a cutoff of 170.4 pg/mg creatinine (sensitivity 91.4%, specificity 59.1%) whereas CA 19-9 as the most specific one, displaying a cutoff of 51.3 U/mg creatinine (sensitivity 48.9%, specificity 88.0%). NGAL showed the greatest decrease in urinary concentrations after pyeloplasty, in which 90.0% of patients had a clearance greater than 50% in comparison to preoperative values. CONCLUSIONS: The evaluation of urinary biomarkers is useful in the assessment of UPJO in adults undergoing laparoscopic pyeloplasty. Urinary concentrations of CA 19-9, NGAL and KIM-1 were elevated in patients with UPJO and significantly decreased after pyeloplasty. The exact role of those biomarkers in the setting of obstruction in adults should be further evaluated

O KIM-1 foi o marcador mais sensível com o ponto de corte 170,4 pg/mg de creatinina (sensibilidade 91,4%, especificidade 59,1%) e o CA 19-9 o mais específico para o ponto de corte de 51,3 U/mg de creatinina (sensibilidade 48,9%, especificidade 88,0%), enquanto o NGAL foi o que apresentou maior queda após desobstrução, com 90,0% dos pacientes apresentando clareamento superior a 50%. CONCLUSÕES: A avaliação dos biomarcadores urinários é útil no diagnóstico de obstrução em adultos com EJUP submetidos a pieloplastia videolaparoscópica. O KIM-1 foi o marcador mais sensível e o CA 19-9 o mais específico, enquanto o NGAL foi o que apresentou maior que com a desobstrução. Houve queda das concentrações dos marcadores após pieloplastia no período estudado. O papel exato dos biomarcadores urinários no cenário de obstrução em adultos deve ser mais amplamente investigado / INTRODUCTION AND OBJECTIVE: Ureteropelvic junction obstruction (UPJO) is an important cause of urinary tract obstruction and can lead to progressive deterioration of renal function. Thus the development of novel non-invasive methods capable of discriminating obstruction and hydronephrosis may be useful. Elevation of urinary biomarkers may provide early evidence of kidney damage in urinary obstruction. In this scenario, urinary concentrations of KIM-1 may be elevated following proximal tubular injury, while NGAL may increase as result of injury to proximal or distal tubule as well as to loop of Henle, CA19-9 after overproduction in the obstructed tubule and ?2 microglobulin (beta2M) after injury to the glomerulus or the proximal tubule. The aim of this study was to evaluate the diagnostic properties of these urinary biomarkers in adults with UPJO. METHODS: We prospectively studied consecutive patients older than 18 years diagnosed with UPJO undergoing laparoscopic pyeloplasty from December 2013 to February 2015 in our institution. Exclusion criteria included patients with bilateral UPJO, unilateral UPJO with contralateral pathologic kidney, solitary kidney, history of previous surgical treatment for UPJO or glomerular filtration rate below 60 ml/min/1,73m2. Each patient provided four voided urine samples for biomarker measurement, one at preoperative consultation and the others at 1, 3 and 6 months of postoperative follow-up. Healthy individuals with no hydronephrosis on ultrasound evaluation constituted our control group. RESULTS: We included 47 patients with a mean age of 38.6 ± 12.7 years (range 19-64 years), from which 17 (36.2%) were males and 30 (62.8%) were females. The control group consisted of 40 subjects with no statistical difference to the study group regarding age (p = 0.95) and gender (p = 0.82). KIM-1 had an area under the curve (AUC) of 0.79 (95% CI 0.70 to 0.89) and was the biomarker with the best diagnostic properties. CA19-9 had an AUC of 0.70 (95% CI 0.60 to 0.81), NGAL had an AUC of 0.71 (95% CI 0.61 to 0.83) and beta2M had an AUC of 0.61 (95% CI 0.50 to 0.73). KIM-1 was the most sensitive marker with a cutoff of 170.4 pg/mg creatinine (sensitivity 91.4%, specificity 59.1%) whereas CA 19-9 as the most specific one, displaying a cutoff of 51.3 U/mg creatinine (sensitivity 48.9%, specificity 88.0%). NGAL showed the greatest decrease in urinary concentrations after pyeloplasty, in which 90.0% of patients had a clearance greater than 50% in comparison to preoperative values. CONCLUSIONS: The evaluation of urinary biomarkers is useful in the assessment of UPJO in adults undergoing laparoscopic pyeloplasty. Urinary concentrations of CA 19-9, NGAL and KIM-1 were elevated in patients with UPJO and significantly decreased after pyeloplasty. The exact role of those biomarkers in the setting of obstruction in adults should be further evaluated

Impacto do reprocessamento de dialisadores de alto fluxo e alta eficiência sobre o transporte de solutos em sessões de hemodiafiltração online curta diária / Reprocessing high-flux, high-efficiency polysulfone dialyzers on solutes removal in short daily online hemodiafiltration

Introdução: Não há estudos que avaliem o impacto da reutilização do dialisador na remoção de solutos em sessões de hemodiafiltração online curta diária (HDF-OL-D). Objetivo: Avaliar o impacto do reuso do dialisador na dose do tratamento, na cinética de beta2-microglobulina e na extração de solutos em sessões de HDF-OL-D e comparar com sessões de hemodiálise curta diária de alto fluxo (HD-D). Métodos: Foram incluídos 14 pacientes do programa de HD-D. Foram coletadas amostras de sangue pré, no meio e pós-diálise e amostras do dialisato no 1º, 7º e 13º usos do dialisador em sessões de HD-D e de HDF-OL-D. Resultados: A massa total extraída (MTDQ) e a depuração (KDQ) diretamente quantificada dos solutos pequenos (ureia, fósforo, creatinina e ácido úrico), bem como a dose de diálise ofertada, foram semelhantes quando o 1º, 7º e 13º usos do dialisador em sessões de HD-D foram comparados, respectivamente, ao 1º, 7º e 13º usos em sessões de HDF-OL-D. A MTDQ e a KDQ dos solutos pequenos e a dose de diálise foram semelhantes entre os usos do dialisador tanto em sessões de HD-D e quanto em sessões de HDF-OL-D. A MTDQ, a KDQ e o Kt/V diretamente quantificado (Kt/VDQ) de beta2-microglobulina foram maiores em sessões de HDF-OL-D do que nos respectivos usos do dialisador em sessões de HD-D. Não houve diferença quanto à cinética de beta2-microglobulina, avaliada pela MTDQ, KDQ ou Kt/VDQ, entre o 1º, 7º e 13º usos do dialisador em sessões de HD-D nem em sessões de HDF-OL-D. Na HDF-OL-D, a perda intradialítica de albumina foi significativamente diferente entre os usos do dialisador (p < 0,001), estando reduzida no 7º e 13º usos, quando comparados ao 1º uso do dialisador. Não houve correlação entre o volume de enchimento do dialisador e a MTDQ e a KDQ de moléculas pequenas em nenhum dos métodos dialíticos estudados. Em sessões de HDF-OL-D, foi observada uma correlação fraca entre o volume de enchimento do dialisador e a KDQ de beta2-microglobulina, não percebida com a MTDQ ou o Kt/VDQ de beta2-microglobulina. Conclusão: O reprocessamento de dialisadores de alto fluxo e alta eficiência não resulta em comprometimento da dose do tratamento, da cinética de beta2-microglobulina nem da extração de solutos em sessões de HDF-OL-D. A extração de beta2-microglobulina foi maior em sessões de HDF-OL-D do que em sessões de HD-D, sem diferenças significativas na remoção dos demais solutos. O reprocessamento do filtro, em sessões de HDF-OL-D, resultou numa redução significativa da perda intradialítica de albumina. Não parece haver influência do volume de enchimento do filtro na extração de solutos em nenhum dos métodos de diálise estudados / Introduction: There no studies evaluating the impact of dialyzer reutilization on solute removal in daily online hemodiafiltration (D-OL-HDF) sessions. Objectives: Our aim was to evaluate the impact of dialyzer reuse on solute extraction, beta2-microglobulin kinetics and dialysis dose in D-OL-HDF and compare to those in high-flux short daily hemodialysis (D-HD). Methods: 14 patients undergoing a D-HD program were included. Pre, middle and post-dialysis blood samples and effluent dialysate were collected in the 1st, 7th and 13th dialyzer uses in D-HD sessions and in D-OL-HDF sessions. Results: Directly quantified small solute (urea, phosphorus, creatinine and uric acid) total mass removal (TMDQ) and clearance (KDQ), as well as dialysis dosis, were similar when the 1st, 7th and 13th dialyzer D-HD uses were compared to the 1st, 7th and 13th D-OL-HDF uses. TMDQ and KDQ of small solutes and dialysis dose were similar among dialyzer uses in D-HD sessions and also in D-OL-HDF sessions. beta2-microglobulin TMDQ, KDQ and directly quantified Kt/V (Kt/VDQ) were statistically higher in D-OL-HDF dialyzer uses than in the respective D-HD uses. There was no difference in beta2-microglobulin kinetics, evaluated by TMDQ, KDQ or Kt/VDQ, among 1st, 7th and 13th uses in D-OL-HDF sessions or in D-HD sessions. In D-OL-HDF, albumin loss was significantly different among studied dialyzer uses (p < 0.001), being reduced in the 7th and 13th dialyzer uses, when compared to the first use. There was no correlation between dialyzer priming volume and small molecules TMDQ, KDQ in neither analyzed dialytic method. In D-OL-HDF sessions, it was observed a week correlation between dialyzer priming volume and beta2-microglobulin KDQ, not observed with beta2-microglobulin MTDQ or Kt/VDQ. Conclusion: High flux, high efficiency dialyzer reprocessing did not did result in a reduction of the offered dialysis dose, beta2-microglobulin kinetics or solute extraction in D-OL-HDF. beta2-microglobulin removal was greater in D-OL-HDF than in D-HD sessions, without difference in other solutes extraction. There was a significant reduction in intradialytic albumin loss with dialyzer reprocessing in D-OL-HDF sessions. Dialyzer priming volume does not appear to influence solute removal in neither analyzed dialytic method
