An Interpretive Description of Registered Nurses’ Experiences as Preceptors for Newly Hired Registered Nurses in Acute Pediatric Clinical Settings

Background: In clinical practice, the responsibilities of frontline registered nurses (RNs) include providing patient care as well as precepting students and new staff nurses. The role has been acknowledged as demanding and challenging in acute healthcare settings and the importance of the preceptor role for the ease of the new nurses’ transition to clinical settings has been discussed in the literature. However, less is known about the nurses’ experiences as preceptors in the pediatric acute clinical setting. The purpose of this research was to explore and describe the experiences of RNs as preceptors when they precept newly hired registered nurses in pediatrics settings. Method: An interpretive descriptive design was used to guide the research process. Semi-structured one-on-one interviews, both in-person and virtual, were carried out with five preceptors who are registered nurses in an Ontario hospital. The real-life experiences of these participants are reported using their rich descriptions of events. Data collection and analysis were conducted concurrently and informed by the Braun and Clark (2006) framework. Results: Findings were grouped under the following three themes: the duality role of pediatric nurses; experiences of preceptor pediatrics nurses; and pediatric nurse preceptors’ needs. Implications: This study demonstrates that the role of the registered nurse as a preceptor is diverse and demanding, and that preceptors need to be supported by various stakeholders to ensure that they can carry out their role effectively. Recommendations are made for future practice, education, and research. The goal of this study was to gain an understanding of the registered nurses’ experiences as preceptors for newly hired registered nurses in pediatric healthcare settings. This study showed that preceptors were aware of their dual role, shared experiences of feeling awarded, and discussed challenges they encountered when precepting. Finally, they identified some of their needs that they believed may assist them when being involved in the preceptor-preceptee relationship. It is important to acknowledge the challenges that preceptors encounter when precepting new nurses into the clinical area. Furthermore, it is imperative to prepare nurses for the preceptor role to ensure effective preceptor-preceptee relationships and improve preceptees retention and competence in their role.

Bakgrund De flesta nyutexaminerade sjuksköterskorna upplever en stor utmaning vid övergången från student till sjuksköterska eftersom arbetsuppgifterna och ansvaret som kommer med jobbet kan upplevas som en chock för den nya sjuksköterskan. Sjuksköterskan ansvarar över att upprätthålla en god och säker omvårdnad där delegering och prioritering av arbetsuppgifter ingår. Inom hälso- och sjukvården har arbetsbelastningen ökat mer främst med överbeläggningar och underbemanning av sjuksköterskor vilket har resulterat i en ökad stressnivå hos nyutexaminerade sjuksköterskor. Syfte Syftet är att beskriva hur nyutexaminerade sjuksköterskor upplever arbetsrelaterad stressunder det första arbetsåret. Metod Studien har genomförts som en litteraturöversikt av tolv artiklar från hela världen. Databaser som användes var CINAHL och PubMed. Av dessa tolv artiklar var nio stycken kvalitativa, två stycken kvantitativa och en mixad metod. Resultat Resultatet visade att nyutexaminerade sjuksköterskor hade höga förväntningar på sig vilket ledde till ökad stress. Deras kunskaper och erfarenheter var otillräckliga då patienterna hade komplexa sjukdomar. De försökte hantera ansvaret och förväntningarna som ställdes på dem vilket skapade osäkerhet och rädsla hos nyutexaminerade sjuksköterskor. Slutsats Som nyutexaminerad sjuksköterska är behovet av stöd från kollegor och handledare oerhört viktiga för att de ska känna sig trygga med sitt arbete. För att god kompetens och självförtroende ska utvecklas krävs organisationens stöd för dem. / Background Most newly graduated nurses experience a big challenge with the transition from student to nurse because the assignment and responsibilities that comes with the job have newly graduated nurses in a state of shock. As a nurse, you are responsible for maintaining a good and safe care, which includes delegation and prioritization of tasks. In healthcare, the workload has increased to an increased stress level in newly graduated nurses. Aim The aim of the study was to describe how newly graduated nurses experience occupational stress during the first year of work. Method The study has been carried out as a literature review of twelve articles from around the world. The databases that was used were CINAHL and PubMed. Of these twelve articles, nine had a qualitative method, two had quantitative method and one mixed method. Result The result showed that newly graduated nurses had high expectations of themselves, which led to increased stress. Their knowledge and experience were insufficient when the patients had complex diseases. They tried to manage the responsibilities and expectations placed on them, which created uncertainty and fear in newly graduated nurses. Conclusions As a newly graduated nurse, the need for support from colleagues and supervisors is extremely important for them to feel secure in their work. In order for good competence and self-confidence to develop, the organization ́s is required to give them support.

Bakgrund: Många nyutexaminerade sjuksköterskor möter en krävande verklighet med påfrestande arbetsmiljö där de förväntas ta stort ansvar och självständigt arbeta utifrån lagar och etiska principer. Detta leder till känslomässiga utmaningar som kan påverka både välbefinnandet och hälsan. Tidigare studier visar att en yrkesintroduktion med kollegialt stöd, social gemenskap och utrymme för att få vara ny ger nyutexaminerade sjuksköterskor möjlighet till en trygg och positiv start. Syfte: Att undersöka hur nyutexaminerade sjuksköterskor upplever sin första tid som legitimerad sjuksköterska. Metod: En kvalitativ intervjustudie utfördes med tolv nyutexaminerade sjuksköterskor. Data samlades in genom semistrukturerade intervjuer. Materialet sammanställdes och tolkades med en kvalitativ innehållsanalys. Resultat: Analysen resulterade i tre kategorier: En utmanande övergång, Organisatoriskt kaos och En verklighetschock. Konklusion: Nyutexaminerade sjuksköterskor upplever att övergången från handledd student till självständig sjuksköterska är utmanande på flera sätt. Det blir en chock för många att möta en klinisk verksamhet med högre tempo och tyngre arbetsbörda än de tidigare ställts inför. Mer ansvar och utmaningar under utbildningstiden kan hjälpa studenter med mental förberedelse och en positiv inställning samt stöd från kollegor främjar en bra första tid i arbetslivet. / Background: Many newly graduated nurses face a demanding reality with a challenging work environment where they are expected ta take on great responsibility and work independently based on laws and ethical principles. This leads to emotional challenges that can affect both well-being and health. Previous studies show that a professional introduction with collegial support, social community and space to be new gives newly graduated nurses the opportunity for a secure and positive start. Aim: To investigate how newly graduated nurses experience their first time as a registered nurse. Method: A qualitative interview study was conducted with twelve newly graduated nurses. Data was collected through semi-structured interviews. The material was compiled and interpreted using a qualitative content analysis. Findings: The analysis resulted in three categories: A challenging transition, Organizational chaos and A reality shock. Conclusion: Newly graduated nurses experience that the transition from being a supervised student to an independent nurse as challenging in multiple ways. It´s a shock for many to face a clinical reality with a higher pace and heavier workload than they have previously encountered. More responsibility and challenges during the education period can help students with mental preparation and a positive attitude, also support from colleagues promotes a good start into their professional career.

Nyutexaminerade sjuksköterskors strategier för att hantera stress i omvårdnadsarbetet : En litteraturstudie / Strategies of newly graduated nurses to cope with stress in nursing work : A literature review

Bakgrund: Tidigare forskning har visat på att arbetsrelaterad stress kan bidra till negativa konsekvenser i form av både fysisk och psykisk ohälsa för sjuksköterskor. Att sjuksköterskans välbefinnande påverkas kan i sin tur leda till att omvårdnaden och patientsäkerheten påverkas negativt. Syfte: Syftet med denna litteraturstudie var att identifiera strategier nyutexaminerade sjuksköterskor kan använda sig av för att hantera stress i omvårdnadsarbetet.  Metod: Studien använde en kvalitativ ansats och granskade 11 vetenskapliga artiklar från databaserna CINAHL, PubMed och PsycInfo. Artiklarna kvalitetsgranskades enligt SBU:s mall och analyserades med hjälp av Popenoe med fleras modell. Resultat: Nyutexaminerade sjuksköterskor som upplevde arbetsrelaterad stress använde sig av varierande copingstrategier i sitt arbete. I resultatet framkom fyra teman för stresshantering: Självmedkänsla, Stöd, Kontroll, Återhämtning. Konklusion: Studiens slutsatser pekar på att stöd, självmedkänsla och återhämtning är väsentliga komponenter för att främja välbefinnande hos nyutexaminerade sjuksköterskor och därmed öka patientsäkerheten. För att stödja dessa sjuksköterskor i deras övergång till yrket föreslås implementering av stödsystem och riktlinjer som främjar dessa strategier. Detta kan potentiellt skapa en mer positiv och stödjande arbetsmiljö för sjuksköterskor i början av sina yrkesliv. / Background: Work-related stress, according to research, can contribute to adverse consequences in the form of both physical and mental health issues for nurses. This, in turn, can negatively impact patient care and safety. Aim: The purpose of this literature review was to identify strategies that newly graduated nurses can employ to manage stress in their nursing practice. Method: The study utilized a qualitative approach, reviewing 11 scientific articles from CINAHL, PubMed, and PsycInfo. These articles were assessed for quality using the SBU's criteria and analyzed through the Popenoe et al model. Results: Newly graduated nurses experiencing work-related stress employed various coping strategies in their work. The results revealed four themes for stress management: Self-compassion, Support, Control and Recovery.  Conclusion: The study's conclusions highlight that support, self-compassion, and recovery are essential components to promote well-being and patient safety among newly graduated nurses. To support the new nurses in their transition to the profession, the implementation of support systems and guidelines that promote these strategies is recommended. This could potentially create a more positive and supportive work environment for nurses at the beginning of their careers.

Work-integrated learning in a hospital ward setting : Exploring the interdependency between the professions and the organization

Introduction: Contemporary hospital settings are a challenging learning and working environment for healthcare professionals. The challenges are largely related to the increasing need for healthcare caused by an ageing population, a lack of personnel resources, and demands for time efficiency. Therefore, it is of interest to study how individuals and teams learn during these working conditions. Nurses are especially exposed when entering working life, why this profession has been of interest to explore further in this thesis. Furthermore, the interprofessional team is significant in providing qualitative care at the hospital wards, which could be affected by the lack of nurses and demands for efficiency. Aim: To explore work integrated learning from an individual nurse and interprofessional team learning perspective in a contemporary hospital ward setting. Methodology: Two qualitative approaches were used in this thesis. Study I was a qualitative content analysis based on individual interviews with 10 nurses within hospital ward care. Study II was an insider action research project (IAR) within a specific hospital ward. Data from the project in Study II were analyzed using the cycle of expansive learning. Results: Study I showed that, during their early working life, newly graduated nurses learned their profession by developing and using different learning strategies. Their initial strategy was to learn how to perform daily tasks as safely and effectively as possible. However, in learning how to provide qualitative care, the nurses realized the importance of participating in bedside care to gain clinical experience. Opportunities to learn from clinical experiences and reflection were essential for developing as a new nurse. Study II explored work integrated learning in an interprofessional ward team. The learning process resulted in the establishment of a digital planning board to illustrate the patients’ recovery process during their stay at the ward. This board opened an opportunity to learn about core values and goals for discharge, but also contributed to boundary awareness between the professionals, which was important for collaboration to improve patient safety in daily work. Discussion: The results reveal that work-integrated learning was manifested in an interdependency between the professionals and the health-care organization. The interdependency was multifaceted as the professionals were dependent on each other to learn as individuals in relation to each other, and to develop their joint work processes. The professionals were also dependent on the organizational prerequisites to learn. This result shows the need for the professionals to develop their own learning strategies to manage daily work, which was associated with the professional’s motivation in leading their own professional development towards qualitative and safe care. Conclusion: The results of this thesis reveal that the professions within a hospital ward setting are largely dependent on each other to create conditions for work integrated learning. The individuals and the team both had strong determination, will, and desire to learn to provide patients with care that was as qualitative and safe as possible. The quality of work-integrated learning was found to be related to the professions’ awareness of boundaries, knowledge, and responsibilities related to each other within the interprofessional team. / Denna studie har utforskat det arbetsintegrerade lärandet hos nyutbildade sjuksköterskor och hos ett interprofessionellt team, i en sjukhusavdelningsmiljö. Resultatet visade att organisatoriska förutsättningar såsom brist på personal, högt vårdflöde samt uppgiftsväxling, tvingar de nyutbildade sjuksköterskornas att anta strategier som gör att det måste förlita sig på andrahandsinformation för bedömning av patienters status. Även om det patientnära arbete uttrycks som väsentligt för att lär sig yrket, tvingas de nyutbildade ofta att prioritera administrativa uppgifter och därmed missas väsentligt information viktig för lärande och för att kunna hantera arbetet säkert och kvalitativt. Organisatoriska förutsättningar riskerar därmed också att ”förlänga” lärandet mot ett expert-kunnande. Resultat visa dock att det finns en styrka i att hitta sina egna vägar att ta sig fram i en svår lärandemiljö, där den professionelles ”jävlar anamma”, engagemang och motivation för att lära, påverkar både riktning och innehåll i lärandet. Resultatet visar att hos både den enskilda sjuksköterskan och hos teamarbetets medlemmar fanns ett driv, vilja och önskan att lära för att kunna ge patienterna en så god och säker vård som möjligt. Det arbetsintegrerade lärandet visade sig också vara relaterat till professionernas medvetenhet om hur deras egna professionella gränser, kunskap och ansvar förhöll sig till övriga medarbetare i teamet. Att skapa ett medvetande om professionella gränser inom teamet innebar att utveckla kunskap om varandras professionella ansvar och bidrag till vårdteamet som helhet samt vilka värderingar och målsättningar som kunde leda teamets arbete framåt. Undersköterskans roll för sjuksköterskans och vårdteamets arbetsintegrerade lärande var central. Att tydliggöra gränser mellan professionerna kräver ett gediget arbete som tar tid, motivation och uthållighet hos både individer och ledning. Ett arbete som kan vara svårt att åstadkomma i vardagsarbetet och därför kräver särskilda insatser men kan ha avgörande beredelse för vårdkvalitén och patientsäkerheten. Sammanfattningsvis kan denna uppsats bidra med fördjupad kunskap om arbetsintegrerat lärande i dagens sjukhusvård. Inom sjukhusvården, där effektivitetskrav dagligen måste balanseras gentemot bristande resurser, kan sjukhusavdelningar lärandemiljö ifrågasättas. Bakgrund: För den nyutexaminerade sjuksköterskan kan den första tiden i yrket upplevas som utmanande och påfrestande då förväntningarna inte alltid möter verkligheten. Stress är ett ofta förekommande fenomen inom sjuksköterskeyrket och kan manifestera sig på en rad olika sätt. Stressen påverkar både människan och utförandet av den holistiska patientvården.Syfte: Syftet var att belysa upplevelsen av stress för nyutexaminerade sjuksköterskor under sitt första verksamma år.Metod: Litteraturstudien genomfördes genom att systematiskt söka i två olika databaser vilket genererade tio vetenskapliga artiklar. Databearbetningen utfördes genom en klart framträdande innehållsanalys.Resultat: I resultatet belyses det hur stress påverkar den första tiden för nyutexaminerade sjuksköterskor och vad stressen kan leda till hos en nyutexaminerad sjuksköterska. Resultatet presenteras i tre teman som svarar på studiens syfte. De teman som belyses är: bristande självkänsla, rädsla för bristande vårdresultat och utmattning. Stress kunde leda till att nyutexaminerade sjuksköterskor upplevde brister i arbetets utförande, kände sig otillräckliga och behövde ta ansvar för dåliga vårdresultat. Stressen kunde även leda till utmattning och risk för att nyutexaminerade sjuksköterskor lämnade professionen.Slutsats: Hos den nyutexaminerade sjuksköterskan kan stress inverka på olika sätt. Stress kan påverka hur nyutexaminerade sjuksköterskor utför sitt arbete, samt i vilka situationer stressen uppkommer och vad den kan leda till. / Background: For the newly graduated nurse the first year can be perceived as challenging and strenuous because the expectations do not always measure up with reality. Stress is a widely discussed phenomenon within the nursing profession and can manifest in a variety of ways. Stress can also affect both the person and the holistic patientcare.Aim: The aim of the study was to illustrate the experience of stress for newly graduated nurses during their first professional year.Method: The literature study was conducted by systematically searching two databases which generated ten scientific articles. The data processing was carried out by a clearly prominent content analysis.Findings: The result illustrates how stress affects newly graduated nurses and what the stress could lead to for a newly graduated nurse. The results are presented in three themes that answer the study ́s purpose. The themes are: lack of self-esteem, fear of poor care results and exhaustion. Stress could lead to deficiencies in the performance of the work, feelings of being insufficient and having to take responsibility for poor care results. Stress could also lead to fatigue and a risk of newly graduated nurses leaving the profession.Conclusion: Stress can affect newly graduated nurses in different ways. Stress can affect how newly graduated nurses perform their work, as well as in what situations the stress arises and what it can lead to.

Newly Graduated Nurses’ Experiences Of The Intervention Practitioner Training Nurse. : A Qualitative Interview Study.

Aims and objectives: To describe newly graduated nurses’ experiences of the intervention practitioner training nurse.  Background: Newly graduated nurses need support to establish the profession. Several studies have shown that the first years in the profession, is the most challenging time. Method: The intervention practitioner training nurse was initiated to empower, and support newly graduates nurses in their professional role. Data were collected through semi-structured interviews with ten newly graduate nurses having experience of the intervention. The data were analysed using qualitative content analysis. Results: The analysis results in the overarching theme; “Organizational prerequisites” The theme consisted of three categories, "Activator" comprised the subcategories compiliating and attractive workplace. This involved that the practitioner training nurse were the activator creating a clear structure and the wards became more attractive workplaces. “Supportive nursing" comprised the subcategories present assistance, emotional support, and patient safety. This involved that practitioner training nurse constituted an important support function and helped to ensure patient safety. “Professional development” comprised the subcategories nurse's competence, feeling of security, and learning. This created the opportunity for professional development. Conclusion: The newly graduate nurses' experience of the intervention shows that the creation of an organisational structure enabled the practitioner training nurse to be an important support and to contribute to professional development. This was accomplished by strategic decision of the hospital management. Relevance to practice: The results of the current study can be transferred to other similar healthcare organizations and can be a support for managers who plan to initiate interventions to empower and support newly graduated nurses.

Background: Work-related stress can negatively affect the health of newly graduated nurses, whichcan have consequences such as a shortage of nurses. If factors are identified that can lead to work-related stress, it also opens up the possibility of preparing newly graduated nurses for the work-related stress that may occur in their new profession. Aim: The aim was to investigate factors that can lead to work-related stress in newly graduated nurses. Method: A literature review with a qualitative approach that included 10 articles. Results: Factors that can lead to work-related stress in newly graduated nurses were presented with two themes: work environment and transition. Four subthemes were formed to describe the experiences of newly graduated nurses: responsibility, feeling inadequate, self-confidence and competence. Conclusion: Work-related stress can be linked to the work environment and affect patient safety, it canalso lead to mental illness or that nurses leave the profession during the transition period. Work-relatedstress in the first years of the new profession may be linked to a lack of support from colleagues andmanagers. With the help of a satisfactory introduction and mentoring program, where experiencednurses act as support for the newly graduated nurses, work-related stress can be reduced. / Bakgrund: Arbetsrelaterad stress kan påverka nyexaminerade sjuksköterskors hälsa negativt vilket kan få konsekvenser så som sjuksköterskebrist. Med hjälp av en tillfredställande introduktion och mentorprogram, där erfarna sjuksköterskor/handledare agerar stöd till de nyexaminerade sjuksköterskorna, kan känslan av arbetsrelaterad stress hos de nyexaminerade sjuksköterskorna minska.

Slutsats: Nyutexaminerade sjuksköterskor behöver stöd under deras första tid som legitimerad sjuksköterska för att minska den arbetsrelaterade stressen och att fler stannar kvar inom professionen vilket leder till ökad patientsäkerhet. / Background: The nursing profession involves high demands and many newly graduated nurses experience stress before having to possess the required knowledge. Stress can affect patient safety.Aim: The aim was to investigate work-related stress in newly graduated nurses and how it affects patient safety. Method: The method was a literaturereview with systematic data collection and selection, where a total 13 articles were included in the results.Results:The result is based on two main categories: Causes of work-related stress and consequences of work-related stress. The result showed that work-related stress in newly graduated nurses was due to several factors, such as organizational factors, lack of leadership, not possessing the knowledge required and to be new to the team. These factors lead to nurses being forced to prioritize work under pressure, which affects the quality of nursing, jeopardize patient safety and a willingness of several newly graduated nurses to leave the profession. Conclusion: Newly graduated nurses need support during their first time as a registered nurse to reduce work-related stress and for more people to remain in the profession which leads to increased patient safety.

Bakgrund Studier visar att nyexaminerade sjuksköterskor som kommer ut till arbetslivet beskriver det som en ”verklighetschock” då det kan vara stressfullt och de är inte beredda på det. Akuta vårdsituationer är något man ställs inför som sjuksköterska och det förväntas av dem att kunna hantera detta. Studier visar även att det finns svårigheter för de nyexaminerade sjuksköterskorna som kommer ut i arbetslivet. Syfte Syftet var att beskriva nyexaminerade sjuksköterskors upplevelse av att ställas inför akuta vårdsituationer. Metod Examensarbetet har genomförts som en strukturerad litteraturstudie med inslag av den metodologi som används vid systematiska översikter. Databaserna CINAHL och PubMed har använts och elva artiklar hittades. En integrerad analys användes för att ta fram resultatet. Resultat Resultatet landade i tre kategorier: emotionella reaktioner, olika aspekter av kompetens och organisatorisk påverkan. Inom varje kategori formulerades två underkategorier. Underkategorierna är tvivel på sig själv, hantering av svår sjukdom och död, teoretisk kompetens, handlingsberedskap, brist på stöd samt brist på tid. Slutsats Resultatet visar att nyexaminerade sjuksköterskor upplever den första tiden som utmanande. Utmaningar som upplevdes var brist på resurser som exempelvis tid och stöd från övriga kollegor. Otillräcklig erfarenhet var något som hindrade handlingsberedskapen i akuta vårdsituationer. En tänkbar lösning till detta skulle kunna vara ett introduktionsprogram med mentorskap för de nyexaminerade sjuksköterskorna. / Background Studies show that newly graduated nurses who come out to workforce describes it as a“reality shock” because it can be stressful, and the newly graduated nurses is not preparedfor that. Acute care situations are something that a RN face and is expected to manage. Studies also shows that there are difficulties for newly graduated nurses entering the workforce, therefore this bachelor ́s thesis was accomplished. Aim The aim was to describe newly graduated nurses’ experiences of managing acute care situations. Method This bachelor ́s thesis has been accomplished as a structured literature study with elements of the methodology used in systematic reviews. The databases CINAHL and PubMed have been used and eleven articles was found. An integrated analysis was used to find the result. Results The results resulted in three categories: emotional reactions, different aspects of competence and organizational impact. Within each category, two subcategories were formulated. The subcategories are self-doubt, management of serious illness and death, theoretical competence, preparedness of action, lack of support and lack of time. Conclusions The results show that newly graduated nurses experience the first period as challenging. Challenges that were experienced were a lack of resources such as time and support from other colleagues. Insufficient experience was something that hindered readiness for action in acute care situations. A possible solution to this could be an introductory program with mentoring for the newly graduated nurses.
