The influence of nursing organisations on the development of the nursing profession in South Africa : 1914-2014

The purpose of the study is to explore past and current professional nursing organisations by means of historical inquiry and to explain the factors that influenced the development of such organisations, as well as the contribution that these organisations made to the professional development of South African nursing in the period between 1914 and 2014. The researcher conducted a literature review and collected data from archival primary and secondary sources. A priori codes provided structure and historical context, yet allowed flexibility. Philosophically critical realism guided the research and enabled the researcher to explain and critique the social world in which South African nursing organisations historically functioned and exerted their professional influence. The findings revealed that in the past one hundred years political, economic and cultural factors present in the social world influenced the nature of South Africa’s professional nursing organisations. Determined to create a female professional image, status and educational exclusivity, South African nursing leaders of the 20th century opted to establish the South African Trained Nurses’ Association (SATNA), a professional nursing association. Influential associations such as SATNA and the South African Nursing Association (SANA) guided the profession to develop a nursing culture based on philosophical and ethical principles of practice. The result was a recognised, respected and trained nursing corps. Over time, however, a social class system, religion, political ideology and nurses’ economic needs reshaped South Africa’s nursing associations and consequently the profession. By the end of the 20th century, South African nursing leaders accepted that nurses needed their socio-economic welfare to be prioritised and therefore the Democratic Nursing Organisation of South Africa (DENOSA), a professional organisation with a trade unionist stance, was established. The result was a trained, politicised, fragmented nursing corps struggling to find its collective professional voice. The greatest legacy bestowed on South African nursing by its first influential organisations is the professional associations evident today. Over time, the South African Nursing Association’s discussion groups that had been established in the 1950s to discuss nursing-related topics evolved into the specialist groups and associations that were present in 2014. / Health Studies / D. The legalising of abortion in many countries has given women the choice or right to decide to terminate pregnancy. The Choice on Termination of Pregnancy Act (Act No 92 of 1996) was promulgated in 1997. This legislation promotes reproductive rights including to choose between having an unwanted pregnancy terminated early, safely and legally. The legislation affects both the women who choose to terminate pregnancy and the staff involved in the termination of pregnancy (TOP) procedures. This exploratory, descriptive and contextual qualitative study was designed to gain insight into the experiences of registered nurses in the procedure for termination of pregnancy and to explore recommendations based on these experiences. The researcher adopted a phenomenological approach. Participants were drawn from registered nurses providing TOP services at Soshanguve Community Health Centre near Pretoria. Registered nurses have the right to refuse to participate in TOP, those that do provide the service are exposed to emotional and psychological trauma. / Health Studies / M.A.

Kwantitatiewe beskrywende studie na die houding van verpleeglundiges teenoor terminasies van swangerskap binne king se sisteem teorie

The aim of this study was to describe the nurses‘ perception of termination of pregnancy and her perception of her conduct towards clients that request termination of pregnancy guided by Kings‘ Systems Approach and Goal Attainment Theory. A non-experimental quantitative descriptive correlational design was used. A non-probability purposive sampling method was used consisting of institutions in the Western Cape, available to participate in this study, which deliver termination of pregnancy services, gov ernment health care institutions (21) and Marie Stopes clinics (3). Non-probability convenience sampling was used consisting of registered nurses who are working in termination of pregnancy units in these facilities (N=82). Data were collected through the administration of a self-designed questionnaire. The main findings of the study indicate that respondents (personal system) appear to feel positive about the work they are doing. Formal improvement and development are however not utilised optimally. Regarding the interpersonal system, respondents also show a positive orientation to both the woman opting for termination of pregnancy and the foetus. The potential towards transaction and ultimately goal attainment reside within this system. The respondents are also positive about legislation, employers and policies (social system) relating to termination of pregnancy. This orientation creates a milieu that is conducive to transactions, interactions and goal attainment.The main recommendation is that three systems as proposed by King be continuously integrated within the termination of pregnancy setup through, amongst other measures: values clarification, management support, in service education, the inclusion of termination of pregnancy in undergraduate programmes, focus groups and the like. / Die doel van hierdie studie was om die verpleegkundige se persepsie van terminasie van swangerskap en haar persepsie van haar houding en gedrag teenoor kliёnte wat terminasie van swangerskap versoek, aan die hand van King se Sisteembenadering en Doelwitbereikingsteorie te ondersoek en te beskryf. `n Nie-eksperimentele kwantitatiewe beskrywende korrelasie navorsingsontwerp was gebruik. `n Nie-waarskynlike doelbewuste-steekproefneming van die staatsgesondheidsorg-instansies wat terminasie van swangerskapdienste aanbied, is geneem en wat beskikbaar vir hierdie studie was (21) asook die Marie Stopesdienste (3), in die Wes-Kaap. `n Nie-waarskynlike gerieflikheidsteekproefneming is van verpleegkundiges geneem wat swangerskappe in hierdie afdelings termineer of moet termineer (N=82). Objetivo: Identificar as questões antiéticas, aéticas e iatrogênicas dos Profissionais da Enfermagem através da análise qualitativa dos processos ético-profissionais abertos junto ao COREN SP no período de 1º de janeiro de 2001 a 31 de dezembro de 2004. Quantificar o número de processos ético-profissionais abertos; identificar o perfil dos profissionais denunciados; relacionar as infrações éticas com os artigos elencados no Código de Ética dos Profissionais de Enfermagem; analisar os processos ético-profissionais, sistematizando os resultados de modo a obter os principais tipos de denúncia, seus locais de ocorrência e a identificação dos tipos de denunciantes; discutir a iatrogenia como fator predominante da infração ética. Método: Trata-se de uma pesquisa exploratória, documental e de corte qualitativo baseada na coleta de dados contidos nos autos dos processos ético-profissionais, no âmbito do COREN SP, no período de 1º de janeiro de 2001 a 31 de dezembro de 2004, por meio da utilização de um questionário, o questionário se encontra no Anexo 1. Do material coletado foram excluídos os processos ético-profissionais cujos dados não permitiram o preenchimento completo do questionário. Os dados obtidos foram submetidos à análise estatística. Resultados e Conclusões: Foram analisados 189 (cento e oitenta e nove) processos ético-profissionais no período de 1º de janeiro de 2001 até 31 de dezembro de 2004, período de 4 (quatro) anos. A coleta desses dados foi feita no Conselho Regional de Enfermagem de São Paulo - COREN SP, nos anos de 2010 e 2011. A maioria das ocorrências chega ao CORENSP por meio de denuncia feita pelas CEEs, com relação à distribuição do número de descritores agrupados em tipos de infrações éticas envolvidos na denúncia, dos Processos Ético- Profissionais analisados houve uma predominância da iatrogenia 38,83% dentre os descritores dos processos analisados. As denúncias são feitas em sua grande maioria dentro de hospitais públicos, as denúncias entre a cidade de São Paulo e cidades do interior do estado de São Paulo de acordo com a pessoa jurídica (CEE) como parte denunciante indicam um maior número de denúncias feitas pelas Comissões de Ética de Enfermagem - CEEs do interior do estado de São Paulo. As denúncias de pessoas físicas ocorreram no interior em 63,64% do estado de São Paulo e 36,36% na capital, indicando, portanto um maior número de denúncias feito por pessoa física no interior. Houve uma maior caracterização da parte denunciada como pessoa física na capital do que no interior do estado de São Paulo. As pessoas que denunciam 52% são pacientes, seguidos por acompanhantes 32%. A maioria das pessoas físicas como parte denunciada foram os auxiliares de enfermagem em 55,56%, enquanto enfermeiros foram denunciados 33,9%. Com relação à distribuição dos artigos infringidos do Código de Ética dos Profissionais de Enfermagem, segundo o número e a porcentagem em ordem decrescente o artigo mais infringido, artigo 16, aconteceu em 11,13%. Outras importantes variáveis foram estudadas e serão analisadas em trabalhos futuros / Objective: Identify the unethical, non-ethical and iatrogenic issues of the Nursing staff through qualitative analysis of the ethical legal proceedings taken at COREN SP during the period of January 1st 2001 to December 31st 2004. Quantify the number of ethical processes taken; identify the profile of the professionals accused; relate the ethical infractions with articles listed in the Code of Ethics of Professional Nursing; analyze the ethical and professional proceedings, systematizing the results in order to obtain the main types of complaint, their places of occurrence and identify the types of informers; discuss iatrogeny as a predominant factor of ethical offence. Method: This was an exploratory, documentary and qualitative research based on the collection of data in the record of proceedings within the ethical processes under COREN SP during the period January 1st 2001 to December 31st 2004, by use of a questionnaire, which is in Annex 1. Ethical processes whose data did not allow the complete filling out of the questionnaire were excluded. The data were subjected to statistical analysis. Results and Conclusions: One hundred and eighty-nine (189) ethical proceedings from the period of January 1st 2001 to December 31st 2004 were analyzed, a period of four (4) years. The collection was taken at the Regional Council of Nursing São Paulo - COREN SP, within the years 2010 and 2011. Most occurrences reach CORENSP through complaints made by the Nursing Ethics Committees (NEC). With respect to the distribution of the number of descriptors grouped into types of ethical violations involved in the complaint, there was a prevalence of 38.83% of iatrogenic descriptors in the analyzed processes. Complaints are mostly made within public hospitals. The complaints from the city of São Paulo and other cities in the state, in accordance with the legal person (NEC) indicate a greater number of complaints made by Ethics Committees in Nursing - in the state of São Paulo, not in the capital. Reports from individuals occurred in 63.64% within the state of São Paulo, while in the capital it were 36.36%, thus indicating a greater number of complaints made by individuals within the state of São Paulo. There was a greater characterization of the individual as reported in the capital than in the state of São Paulo. People who complain are 52% patients, followed by 32% companions. Most individuals reported were nursing assistants: 55.56%, while nurses were reported by 33.9%. The most violated article in the Code of Ethics of Professional Nursing was Article 16, which means 11.13%. Other important variables were studied and will be analyzed in a future research

The complaints from the city of São Paulo and other cities in the state, in accordance with the legal person (NEC) indicate a greater number of complaints made by Ethics Committees in Nursing - in the state of São Paulo, not in the capital. Reports from individuals occurred in 63.64% within the state of São Paulo, while in the capital it were 36.36%, thus indicating a greater number of complaints made by individuals within the state of São Paulo. There was a greater characterization of the individual as reported in the capital than in the state of São Paulo. People who complain are 52% patients, followed by 32% companions. Most individuals reported were nursing assistants: 55.56%, while nurses were reported by 33.9%. The most violated article in the Code of Ethics of Professional Nursing was Article 16, which means 11.13%. Other important variables were studied and will be analyzed in a future research

A phenomenological study of the experiences of nurses directly involved with termination of pregnancies in the Limpopo Province

The South African government promulgated the Choice on Termination of pregnancy Act (CTOP Act, 92 of 1996). This was a dramatic declaration of intent unprecedented in the African continent and globally. This act changed the outlook of the practice of termination of pregnancy by ensuring that services play a critical role in the delivery of the service. This study, which is qualitative in nature, explored the experiences of Termination of Pregnancy service providers working in three designated public health institutions in the Limpopo Province. Interviews were conducted with six service providers to look at how they construct their practice of providing termination of pregnancy services. The information was analyzed and interpreted by means of a thematic analysis method. Major themes that emerged from the participants’ experiences centred on their relationship with family, colleagues, management, clients, and the community reflecting a sense of alienation and lack of adequate infrastructural support.
